Objective:To determine the causes of death among human immunodeficiency virus/acquired immunodeficiency syndrome(HIV/AIDS) patients as a step to planning strategies to improve mortality from this condition.Methods:Thi...Objective:To determine the causes of death among human immunodeficiency virus/acquired immunodeficiency syndrome(HIV/AIDS) patients as a step to planning strategies to improve mortality from this condition.Methods:This study retrospectively analyzed the mortality pattern of adult HIV/AIDS patients in the University of Calabar Teaching Hospital from January 2005 to December 2007.The data were obtained from sexually transmitted infection/acquired immunodeficiency syndrome(STI/AIDS) clinic register,admissions and discharge/death registers as well as the patients’ case records and the hospitals monthly mortality reviews.Information obtained included age,sex,diagnosis and cause(s) of death.The causes of death considered were the direct causes of death,since the originating antecedent cause of death is the same in all the patients,in this case,HIV/AIDS.Data was analysed using Epi Info 2002.Results:The total number of mortalities during the study period was 350,100 were HIV positive representing 28.6%of all deaths.While advanced HIV/AIDS disease was the leading cause of death in our study representing 27.0%,tuberculosis was the single leading cause of deaths in HIV/AIDS patients constituting about 24.0%of deaths.This was followed by sepsis and septicaemia(13.0%), meningitis and encephalitis,and anaemia accounting for 11.0%,while respiratory diseases constituted 5.0%of the mortality burden.The highest number of deaths occurred in those aged between 21-50 years(82.0%).Conclusions:The study has shown that HIV/AIDS is a major cause of morbidity and mortality in our hospital.The causes of death reflect the varied spectrum of infection and other forms of organ involvement that affect HIV/AIDS patients.The present dismal situation of adult patients living with HIV/AIDS calls for enhanced strategies to decrease the mortality trend observed in Nigeria and sub-Saharan Africa.展开更多

Methods: Structured self-administered questionnaires were distributed to all public health nurses in the region, and 1535 valid questionnaires were retrieved (valid response rate 78.7%). Results: More than half of the participants (52.8%) reported difficulties with HIV/AIDS services. However, inadequate functional health literacy among patients is a well-known barrier to effective doctor-patient communication, which has an adverse effect on health outcomes. This study aimed at investigating the association of functional health literacy and doctor-patient communication among HIV/AIDS patients in Homa Bay County, Kenya. It was a cross-sectional hospital-based survey conducted among 362 HIV/AIDS patients receiving care at the eight sub-county hospitals of Homa Bay county. Data was collected using a self-administered structured questionnaire. Cronbach’s alpha and confirmatory factor analysis tests were used to ascertain the reliability and validity of study instruments, while Logistic regression logistic analysis was used to measure the association between functional health literacy and doctor-patient communication. 51% of the respondents were females, and the majority (30.9%) of respondents fell in the age group of 45 years and above. The respondents’ functional health literacy levels were inadequate at weighted means scores of 3.23 [SD 1.31]. The study further established that functional health literacy significantly influenced doctor-patient communication [Nagelkerke R square = 0.318]. There is, therefore, a need to document the determinants of functional health literacy to improve it and make doctor-patient interaction an enjoyable and meaningful experience.展开更多

Rural China is the weak link of HIV prevention, and rural areas of Henan province which is one of the most...Nowadays, there is a trend of HIV prevalence transmitting from high-risk group to average-risk group in China. Rural China is the weak link of HIV prevention, and rural areas of Henan province which is one of the most high-risk regions in China have more than 60% of the AIDS patients in the province. Thus, improving the HIV awareness and implementing health education become the top-priority of HIV/AIDS control and prevention. A multistage sampling was designed to draw 1129 people living with HIV/AIDS (PLWHAs) and 1168 non-PLWHAs in 4 prevalence counties of Henan province. A health promoting and social-psychological support model was constructed to improve the health knowledge of participants. Chi-square tests and unconditional logistic regression were performed to determine the intervention effect and influencing factors. All groups had misunderstandings towards the basic medical knowledge and the AIDS transmission mode. Before the intervention, 59.3% of the HIV/AIDS patients and 74.6% of the healthy people had negative attitudes towards the disease. There was statistically significant difference in the improvement of knowledge, attitude and action with regards to HIV prevention before and after intervention (P〈0.05). PLWHAs who were males (OR=l.731) and had higher education level (OR=1.910) were found to have better HIV/AIDS health knowledge, whereas older PLWHAs (OR=0.961) were less likely to have better HIV/AIDS health knowledge. However, the intervention effect was associated with the expertise of doctors and supervisors, the content and methods of education, and participants' education level. It was concluded that health education of HIV/AIDS which positively influences the awareness and attitude of HIV prevention is popular in rural areas, therefore, a systematic and long-term program of HIV control and prevention is urgently needed in rural areas.展开更多

BACKGROUND Many human immunodeficiency virus(HIV)infected patients suffer from depression,but a little focus is given to detecting and treating depression in primary health care.Detection of depression can be improved...BACKGROUND Many human immunodeficiency virus(HIV)infected patients suffer from depression,but a little focus is given to detecting and treating depression in primary health care.Detection of depression can be improved by introducing short,reliable,and valid screening instruments.AIM To determine the psychometric properties of the patient health questionnaire-2(PHQ-2)and patient health questionnaire-9(PHQ-9)for depression screening and diagnosis,and the sensitivity and specificity of the PHQ-2 in HIV infected patients.METHODS A cross-sectional study was conducted on 158 HIV-infected patients aged 18 years and above in Lahore,Pakistan.PHQ-2 was implemented to screen depression.PHQ-9 was implemented to diagnose major depressive disorder as a reference standard.Reliability,Validity tests and receiver operating characteristic curve were computed.RESULTS The Cronbach's alpha of PHQ-2 and PHQ-9 were 0.732 and 0.759,respectively.The study results showed that the score of 2 on PHQ-2 indicates the highest Youden's index of 0.924,with both sensitivity and specificity of 0.96,and the area under the curve for PHQ-2 was 0.98(95%CI:0.953-0.998).CONCLUSION Good psychometric properties for the PHQ-2 and PHQ-9 indicated their significant potential as tools for depression screening and diagnosis in the HIV-infected population.展开更多
