METHODS A randomized control study of celiac artery perfusion plus transcatheter hepatic arterial chemoembolization (TACE) (observation group) and intravenous chemotherapy (control group) for 99 cases with hepatic me-tastasis from rectal cancer was performed. The perfusion was repeated once at 4 weeks after the first treatment of 52 cases in the observation group, and it was subsequently repeated at an interval of 2 or 3 months. Using intrave-nous administration, the perfusion was repeated once every 3 weeks with 47 cases in the control group. RESULTS Three months after treatment, the patients in the observation group who showed a relief or elimination of a former superior abdominal pain amounted to 70.6%, and those with a diminution of their intrahepatic mass reached 55.8%. In the control group, the patients with a relief or disappear-ance of hepatalgia reached 20%, and those with a diminution of their intrahe-patic mass reached 10.6%. The 1, 2 and 3-year survival rates were 80.8%, 46.2% and 25.0% in the cases of the observation group and 61.7%, 19.1% and 4.3% in the control group, respectively. CONCLUSION For the patients who failed to receive a surgical opera-tion on their hepatic metastasis from rectal cancer, celiac artery perfusion plus TACE is a more effective regimen for improvement of the clinical symp-toms and extension of the survival time, compared to intravenous chemo-therapy, and is a better choice for palliative therapy.展开更多
BACKGROUND Hepatocellular carcinoma(HCC)is one of the most common malignant tumors in the world,which is seriously threatening the lives of patients.Due to the rapid development of the disease,patients were in the mid...BACKGROUND Hepatocellular carcinoma(HCC)is one of the most common malignant tumors in the world,which is seriously threatening the lives of patients.Due to the rapid development of the disease,patients were in the middle and advanced stages at the time of diagnosis and missed the best time for treatment.With the development of minimally invasive medicine,interventional therapy for advanced HCC has achieved promising results.Transarterial chemoembolization(TACE)and transarterial radioembolization(TARE)are currently recognized as effective treatments.This study aimed to investigate the clinical value and safety of TACE alone and combined with TACE in the treatment of progression in patients with advanced HCC and to find a breakthrough for the early diagnosis and treatment of patients with advanced HCC.AIM To investigate the efficacy and safety of hepatic TACE and TARE in advanced descending hepatectomy.METHODS In this study,218 patients with advanced HCC who were treated in the Zhejiang Provincial People’s Hospital from May 2016 to May 2021 were collected.Of the patients,119 served as the control group and received hepatic TACE,99 served as the observation group and were treated with hepatic TACE combined with TARE.The patients in two groups were compared in terms of lesion inactivation,tumor nodule size,lipiodol deposition,serum alpha-fetoprotein(AFP)level in different periods,postoperative complications,1-year survival rate,and clinical symptoms such as liver pain,fatigue,and abdominal distension,and adverse reactions such as nausea and vomiting.RESULTS The observation group and the control group had good efficacy in treatment efficiency,reduction of tumor nodules,reduction of postoperative AFP value,reduction of postoperative complications,and relief of clinical symptoms.In addition,compared with the control group,the treatment efficiency,reduction of tumor nodules,reduction of AFP value,reduction of postoperative complications,and relief of clinical symptoms in the observation group were better than those in the TACE group alone.Patients in the TACE+TARE group had a higher 1-year survival rate after surgery,lipiodol deposition was significantly increased and the extent of tumor necrosis was expanded.The overall incidence of adverse reactions in the TACE+TARE group was lower than that in the TACE group,and the difference had statistical significance(P<0.05).CONCLUSION Compared with TACE alone,TACE combined with TARE is more effective in the treatment of patients with advanced HCC.It also improves postoperative survival rate,reduces adverse effects,and has a better safety profile.展开更多
目的探讨脾部分栓塞术联合肝动脉化疗栓塞术在原发性肝癌伴脾功能亢进中的临床疗效及应用价值。方法80例原发性肝癌伴脾功能亢进患者,采用简单化随机法将所有患者分为对照组和实验组,每组40例。对照组患者实施肝动脉化疗栓塞术,实验组...目的探讨脾部分栓塞术联合肝动脉化疗栓塞术在原发性肝癌伴脾功能亢进中的临床疗效及应用价值。方法80例原发性肝癌伴脾功能亢进患者,采用简单化随机法将所有患者分为对照组和实验组,每组40例。对照组患者实施肝动脉化疗栓塞术,实验组患者在对照组的基础上联合脾部分栓塞术。比较两组患者临床疗效、不良反应发生情况以及手术前后谷丙转氨酶(ALT)、总胆红素(TBIL)、白细胞计数(WBC)、血小板计数(PLT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)。结果实验组治疗总有效率95.0%高于对照组的77.5%,差异具有统计学意义(P<0.05)。实验组患者术前ALT为(55.57±8.56)U/L、TBIL为(24.25±3.12)μmol/L,术后7 d ALT为(33.27±4.08)U/L、TBIL为(17.58±2.15)μmol/L;对照组患者术前ALT为(55.29±8.37)U/L、TBIL为(24.64±3.15)μmol/L,术后7 d ALT为(49.68±6.65)U/L、TBIL为(22.12±2.86)μmol/L。与术前比较,术后7 d两组患者ALT、TBIL水平均有所改善,且与对照组比较,实验组患者的ALT、TBIL水平改善效果更明显,均恢复至正常水平,差异具有统计学意义(P<0.05)。实验组患者术前WBC为(2.48±0.26)×10^(9)/L、PLT为(64.36±10.15)×10^(9)/L,术后7 d WBC为(6.57±1.18)×10^(9)/L、PLT为(117.38±18.15)×10^(9)/L;对照组患者术前WBC为(2.39±0.18)×10^(9)/L、PLT为(64.51±10.22)×10^(9)/L,术后7dWBC为(3.68±0.53)×10^(9)/L、PLT为(83.16±12.86)×10^(9)/L。与术前比较,术后7 d两组患者的WBC、PLT水平均有所升高,且与对照组比较,实验组患者的WBC、PLT水平升高幅度更明显,均恢复至正常水平,差异具有统计学意义(P<0.05)。实验组患者术前PT为(19.26±2.24)s、APTT为(54.35±8.12)s,术后7 d PT为(10.27±1.88)s、APTT为(31.63±4.15)s;对照组患者术前PT为(19.29±2.26)s、APTT为(54.26±8.10)s,术后7 d PT为(16.56±2.05)s、APTT为(44.32±6.86)s。与术前比较,术后7 d两组患者的PT、APTT均有所缩短,且与对照组比较,实验组患者PT、APTT缩短幅度更明显,均恢复至正常水平,差异具有统计学意义(P<0.05)。两组患者术中、术后均未出现严重不良反应,实验组不良反应发生率7.5%低于对照组的27.5%(P<0.05)。结论肝动脉化疗栓塞术联合脾部分栓塞术能够有效改善原发性肝癌伴脾功能亢进患者的肝功能、血常规,促进患者的凝血功能恢复,且安全性良好,值得临床推广应用。展开更多
To evaluate the effect, side-effect and prospect of hepatic arterial perfusion embolization (HAPE) with Zedoary turmeric oil (ZTO) in treating primary hepatic carcinoma (PHC).Methods: Clinical study was carried out by...To evaluate the effect, side-effect and prospect of hepatic arterial perfusion embolization (HAPE) with Zedoary turmeric oil (ZTO) in treating primary hepatic carcinoma (PHC).Methods: Clinical study was carried out by administration of 1-3 ml ZTO through arterial catheter to induce embolism in 32 patients of PHC, and compared with 32 patients treated by hepatic arterial perfusion embolization with chemical agents (HAPE-C) in the control group. The Chinese herbal medicine was given orally to both groups according to Syndrome Differentiation of TCM. In the experimental study, transplantation hepatic carcinoma model was established in 40 rats. They were randomly divided into the treated group and the control group, 20 in each group, and were perfused with 10 mg/kg ZTO and 0.2-0.3 ml normal saline respectively to observe the effect of treatment.Results: The effect of treatment in the ZTO group was CR in 1 case and PR in 13 cases, the total effective rate being 43.75%, with AFP negative reversed in 7 cases, titer decreased in 7; while in the control group it was PR in 10 cases, the total effective rate being 31.25%, AFP negative reversed in 5, titer decreased in 2, and the difference of therapeutic effect between the two groups was insignificant (P>0.05). The post-perfusion thrombotic syndrome occurrence, with the symptoms of fever, abdominal pain, vomiting, etc. in the two groups was similar, but no bone marrow inhibition occurred in the ZTO group, which was different from the control group (P<0.01, P<0.05). The mean survival time, median survival time, 1-, 2-, 3- and 4-year survival rate in the ZTO group was 13.84 months, 10 months, 37.5%, 18.87%, 9.70% and 6.4% respectively, and in the control group, 8.03 months, 6 months, 15.6%, 6.27%, 0% and 0% respectively, the mean survival time, median survival time and 1-year survival rate in the ZTO group were significantly superior to those in the control group (P<0.05). Experimental study showed that the effect in the treated group was better than that in the control group in tumor growth inhibition with the tumor growth rate as 10.8±4.5%% vs 20.6±12.7%, P<0.05, tumor necrosis degree (P<0.01) and survival time prolonged (14.8±3.4 days vs 11.7±1.9 days, P<0.05).Conclusion: HAPE-ZTO in treating PHC showed the therapeutic effect similar to that of HAPE-C, but superior to the latter in survival time prolongation and bone marrow inhibition.展开更多
