Effects of high-quality neurosurgical nursing care on improving clinical nursing quality

BACKGROUND With continuous advancements in medical technology,neurosurgical nursing is constantly developing and improving to provide higher-quality nursing services.AIM To explore the effects of different types of high-quality nursing care on clinical nursing quality and patient satisfaction in neurosurgical nursing.METHODS Eighty patients who received neurosurgical treatment in the Affiliated Hospital of Southwest Medical University from June to December 2020 were selected as study participants and categorised into study and control groups.The study group comprised 40 patients who received 4 different types of high-quality nursing care,whereas the control group comprised 40 patients who received conventional nursing care.After a specific period,nursing satisfaction levels and adverse event and complication rates were compared between the two groups.RESULTS Satisfaction with high-quality care was higher than that with conventional care,and high-quality health services and regional services showed the highest satisfaction levels,with an average score of 12 on the Glasgow scale.The satisfaction levels of the study and control groups were 75%and 57%,respectively,with a statistically significant difference(t=7.314,P<0.05).During the nursing period,the adverse event and complication rates were the highest in patients with level III pathology grade and those who underwent neurosurgery(40.02%and 85.93%,respectively),and the difference was statistically significant.CONCLUSION In neurosurgical nursing,employing appropriate high-quality nursing methods can effectively reduce adverse event and complication rates in patients,thereby improving the quality of nursing care and increasing clinical nursing value. 展开更多

BACKGROUND The treatment of heart failure not only needs to relieve the clinical symptoms and improve the quality of life for patients but also needs to select scientific and reasonable ways to prevent or delay the progression of the disease,thus reducing the mortality and hospitalization rate.Although the previous regimen can effectively relieve symptoms in the early stage of treatment,long-term use may cause adverse events,such as arrhythmia,and even increase mortality.Therefore,conventional treatment cannot meet the actual health needs of patients,and scientific nursing intervention is very necessary.AIM To investigate the application of self-care based on full-course individualized health education (FCIHE) and its influencing factors in patients with chronic heart failure (CHF).METHODS We enrolled CHF patients who were admitted to our center between September 2015 and June 2016 and divided them into an intervention group (n = 50) and control group (n = 50) using a random number table.Routine nursing care was applied to the control group,and FCIHE was offered to the intervention group.The self-care behavior,6-min walking distance (6MWD),and 36-item short form health survey (SF-36) scores were compared between the two groups.The influencing factors of the self-care were also analyzed.RESULTS The 6MWD was not significantly different between the two groups at admission (P > 0.05);however,at 3 and 6 mo after discharge,6MWD was significantly increased,and it was significantly longer in the intervention group (P < 0.05).The scores for self-care behavior showed no significant difference at admission between the two groups (P > 0.05);however,at 3 and 6 mo after discharge,the total scores for self-care maintenance,management,confidence,and behavior of the intervention group were significantly higher than those of the control group (P < 0.05).There were no significant differences in the SF-36 scores at admission (P > 0.05);however,at 3 mo and 6 mo after discharge,the scores for all eight subscales,including physical functioning,role limitations due to physical problems,bodily pain,general health perceptions,vitality,social functioning,role-limitations due to emotional problems,and mental health,were significantly higher in the intervention group (P < 0.05).As shown by logistic regression analysis,the influencing factors of self-care mainly included age,cardiac function class,and education background (odds ratio > 1;all P < 0.05).CONCLUSION FCIHE improved self-care behavior and cardiac function in CHF patients.Age,cardiac function,and education level affected the implementation of self-care among CHF patients. 展开更多

Background: As the prognosis of lung cancer (LC) patients improves, subcutaneously implanted central venous access device ports (CV-ports) have frequently been used for continuing chemotherapy (CC) or palliative care (PC). In this study, we examined the clinical course of LC patients with subcutaneously implanted CV-ports from the time of receiving chemotherapy to the endpoint of cancer. Materials and Methods: We retrospectively reviewed the clinical data and treatment history of LC patients with subcutaneously implanted CV-ports between June 2008 and November 2013 using clinical records and a pharmacy database. Results: Of the 132 LC patients with subcutaneously implanted CV-ports, 79 (59.8%) had CV-ports for CC (the CC group) and 53 (40.2%) had CV-ports for PC (the PC group). After CV-port implantation, LC patients in the CC group received a median of two regimens with a median of 6 cycles. The median survival time of patients in the CC and PC groups was 457 and 44 days, respectively. In the CC group, the median survival time of small cell and non-small cell LC patients was 342 (95% confidence interval, 235 - 627) and 563 (95% confidence interval, 368 - 728) days, respectively. Nine patients (6.8%) had their CV-ports removed due to complications. Forty (30.3%) of the 132 enrolled patients were referred for at-home PC. The at-home death rate observed among these 40 patients was 30.0% (N = 12). Conclusion: CV-ports may contribute to seamless oncological care. 展开更多

Objective:To study the effect of high-quality nursing combined with breathing exercises on patients with COPD.Methods:Using the random number table method of medical experiments,60 patients with COPD received in our hospital from March 2020 to March 2021 were used as research samples.According to the differences in treatment measures,they were equally divided into control group and intervention group.Symptomatic support treatment and nursing routine,high-quality nursing combined with respiratory function exercise treatment and nursing were given respectively,and the application effects of the two groups were compared and analyzed.Results:The controllable rate of disease between the intervention group and the control group was 93.33%(28/30)and 66.67%(20/30)respectively,which was statistically significant(P<0.05).The comparison between the intervention group and the control group on the pulmonary function indexes of VT,TPTEF/Te,VEF/Te/,Ti/Te was statistically significant(P<0.05).The results of the intervention group on exercise pulse and 6-minute walking distance were significantly higher than those of the control group(P<0.05).Conclusions:The combination of high-quality nursing care and breathing exercises has outstanding disease controllable rate in patients with COPD,especially in improving the lung function of the patients and the level of treatment and care.It can be used as a feasible measure in the subsequent clinical treatment and nursing practice of patients.It is worthy of clinical promotion and implementation. 展开更多

Objective:To explore and analyze the effect of high-quality nursing on patients with epidemic hemorrhagic fever with renal syndrome.Methods:Ninety patients with epidemic hemorrhagic fever with renal syndrome treated in Huyi District People’s Hospital from October 2021 to January 2022 were randomly divided into an experimental group and a control group,with 45 cases in each group.The control group received routine nursing and the experimental group received high-quality nursing.The effect of routine nursing and high-quality nursing were compared and analyzed based on the Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),the degree of satisfaction with nursing care,and the patients’quality of life.Results:Before nursing,there was no significant difference in the patients’psychological state between the two groups(P>0.05);after nursing,there was significant difference in the patients’psychological state between the two groups(P<0.05);the degree of satisfaction with nursing care of the experimental group was significantly higher than that of the control group(P<0.05);comparing the quality of life of the two groups,the quality-of-life score of the control group was significantly lower than that of the experimental group(P<0.05).Conclusion:For patients with epidemic hemorrhagic fever with renal syndrome,high-quality nursing can significantly and effectively improve their psychological state,level of satisfaction with nursing care,and quality of life. 展开更多

我国临床药学专业本科教育药学服务能力培养现状分析

目的探索适合我国药学服务人才培养的本科教育模式。方法分类收集我国10所高等学校(山东大学、上海交通大学、四川大学,安徽医科大学、哈尔滨医科大学、南方医科大学、天津医科大学、南京医科大学,中国药科大学、沈阳药科大学)2018年至2... 目的探索适合我国药学服务人才培养的本科教育模式。方法分类收集我国10所高等学校(山东大学、上海交通大学、四川大学,安徽医科大学、哈尔滨医科大学、南方医科大学、天津医科大学、南京医科大学,中国药科大学、沈阳药科大学)2018年至2021年临床药学专业本科学生的培养方案,包括课程设置、学时和学分方案等,并对药学服务能力培养相关课程体系进行比较与分析。采用问卷调查法,调研四川大学华西药学院2018级至2022级在校临床药学专业本科学生对现行培养方案的认识、评价与需求。结果10所高等学校中,临床药学培养方案总学分和总学时、必修课程学分和学时设置最多的均为南京医科大学(290学分和3886学时,209学分和3562学时)。选修课程设置学分最多的为四川大学(35学分)。各医科大学理论课程设置学时较多,四川大学、中国药科大学、山东大学的实践课程设置学时较多,理论课程、实践课程学时最多的分别为南方医科大学(2910学时)和四川大学(1332学时)。各高等学校(除上海交通大学无法统计外)毕业实习时长均达到42周。中国药科大学和四川大学开设的药学服务类课程相对较多(14门、494学时和14门、424学时),且兼顾了理论和实践教学。共发放问卷149份,回收有效问卷141份,四川大学临床药学专业学生对培养方案和药学服务相关课程总体满意度高(均超过70%),但仍有24.11%的学生认为理论课程过多,实践课程过少。结论调研的10所高等学校临床药学专业本科学生的培养方案存在较大差异,且均缺乏系统的药学服务能力培养课程体系。建议以药学服务胜任力为目标,完善临床药学专业本科实践课程体系建设,增设跨学科、跨专业实践课程,建设药学服务课程群,将药学服务能力培养贯穿本科教育始终,构建适合我国药学服务人才培养的本科教育模式。 展开更多

信息化医养结合式院外干预在淋巴瘤患者全程管理中的应用

目的:探讨信息化医养结合式院外干预在淋巴瘤患者全程管理中的应用效果。方法:选取2022年1月1日~6月30日收治的273例淋巴瘤患者作为对照组,采用常规院外干预;选取2022年7月1日~12月31日收治的250例淋巴瘤患者作为研究组,实施信息化医养... 目的:探讨信息化医养结合式院外干预在淋巴瘤患者全程管理中的应用效果。方法:选取2022年1月1日~6月30日收治的273例淋巴瘤患者作为对照组,采用常规院外干预;选取2022年7月1日~12月31日收治的250例淋巴瘤患者作为研究组,实施信息化医养结合式院外干预。比较两组干预前后癌因性疲乏情况[采用癌因性疲乏量表(CFS)]、心理状况[采用简明心境问卷(POMS)]、应对方式[采用医学应对方式问卷(MCMQ)]、健康行为[采用健康行为量表(HPLP-Ⅱ)]。结果:干预后,两组CFS各维度评分均低于干预前(P<0.05),且研究组低于对照组(P<0.01);干预后,两组POMS中精力-活力维度评分高于干预前(P<0.05),其他维度评分均低于干预前(P<0.05),且研究组优于对照组(P<0.01);干预后,两组MCMQ中屈服、回避维度评分均低于干预前(P<0.05),面对维度评分高于干预前(P<0.05),且研究组优于对照组(P<0.01);干预后,两组HPLP-Ⅱ各维度评分均高于干预前(P<0.05),且研究组高于对照组(P<0.01)。结论:将信息化医养结合式院外干预应用于淋巴瘤患者全程管理中,能有效改善其癌因性疲乏情况及负性情绪,促使其以积极的应对方式对待疾病,提高其健康行为。 展开更多

目的探讨“互联网+”的移动护理模式对癌痛患者的相关应用效果。方法选取2021年1月—2023年12月广西中医药大学附属瑞康医院肿瘤一病区收治的癌痛患者84例作为研究对象,按入院先后顺序分对照组(42例)和干预组(42例),对照组予以3个月常... 目的探讨“互联网+”的移动护理模式对癌痛患者的相关应用效果。方法选取2021年1月—2023年12月广西中医药大学附属瑞康医院肿瘤一病区收治的癌痛患者84例作为研究对象,按入院先后顺序分对照组(42例)和干预组(42例),对照组予以3个月常规宣教护理,干预组在常规健康教育的基础上通过互联网给予全程关怀护理模式干预3个月。对比两组患者服药依从性、爆发痛发生率、癌痛治疗认知度、自我效能感等指标,评估两组临床效果。结果两组患者干预后服药依从性评分、癌痛治疗认知度、自我效能感评分均高于干预前,且干预组均高于对照组,干预组爆发痛发生率低于对照组,差异均有统计学意义(P<0.05)。结论“互联网+”的移动护理模式可有效提高患者的癌痛治疗认知度、服药依从性和自我效能感,使患者疼痛得到缓解,降低了爆发痛的发生率,提高了患者的生存质量。该模式简单便于操作,经济有效,值得在临床上推广。 展开更多

1例67岁男性患者因“阵发性抽搐发作2周余”入院,诊断为抗γ-氨基丁酸B型受体(GABA_(B)R)抗体相关脑炎。入院后给予免疫治疗,治疗过程中出现癫痫持续状态、肺部感染、低蛋白血症、凝血功能异常等。临床药师结合患者病情,在免疫治疗方案... 1例67岁男性患者因“阵发性抽搐发作2周余”入院,诊断为抗γ-氨基丁酸B型受体(GABA_(B)R)抗体相关脑炎。入院后给予免疫治疗,治疗过程中出现癫痫持续状态、肺部感染、低蛋白血症、凝血功能异常等。临床药师结合患者病情,在免疫治疗方案的选择上给予甲泼尼龙联合静脉注射免疫球蛋白,在需要抗感染治疗时给予美罗培南(1 g q8h ivgtt);应用多烯磷脂酰胆碱护肝,给予氨溴索联合布地奈德改善肺功能,应用低分子量肝素钙预防血栓形成,对患者的营养和电解质平衡方面进行监护,给予患者全程化监护,最终患者病情好转出院。 展开更多
