Safety and immunogenicity of human papillomavirus-16/18 AS04-adjuvanted vaccine in healthy Chinese females aged 15 to 45 years:a phase Ⅰ trial 被引量:3

Globally,about 70% of cervical cancers are associated with human papillomavirus (HPV)-16 or HPV-18 infection.A meta-analysis of epidemiologic studies in China showed that HPV was present in 98% of cervical cancer samples.The HPV-16/18 AS04-adjuvanted vaccine Cervarix has shown a high level of protection against HPV-16/18 infections and associated cervical lesions.This phase Ⅰ trial (NCT00549900) assessed the safety,tolerability,and immunogenicity of the vaccine in Chinese.Thirty healthy Chinese females,aged 15 to 45 years with a median age of 29.5 years,received three doses of Cervarix in Months 0,1,and 6.Safety was assessed via recording solicited local and systemic symptoms within 7 days and unsolicited symptoms within 30 days after each vaccination.Serious adverse events,new onset of chronic diseases,and other medically significant conditions were recorded throughout this trial.As an exploratory objective,HPV-16/18 antibody titers were determined by enzyme-linked immunosorbent assay in serum samples collected in Months 0 and 7.Pain at the injection site was the most frequently reported local symptom.Two subjects reported medically significant adverse events.Both cases were assessed as unrelated to vaccination by the investigator.In Month 7,100% seroconversion was observed for both anti-HPV-16 and anti-HPV-18 with high geometric mean antibody titers.HPV-16/18 AS04-adjuvanted vaccine,evaluated for the first time in Chinese females,was generally well tolerated and immunogenic,as previously shown in global studies. 展开更多

卵巢癌血清CCL18、HK10水平变化及与肿瘤恶性生物学行为的关系

目的探讨卵巢癌血清趋化因子18(CCL18)、人激肽释放酶10(HK10)水平变化及与肿瘤恶性生物学行为的关系。方法选取2018年1月至2023年4月河北省沧州中西医结合医院收治的100例卵巢癌患者作为研究组,50例良性卵巢肿瘤患者作为对照组,50名健... 目的探讨卵巢癌血清趋化因子18(CCL18)、人激肽释放酶10(HK10)水平变化及与肿瘤恶性生物学行为的关系。方法选取2018年1月至2023年4月河北省沧州中西医结合医院收治的100例卵巢癌患者作为研究组,50例良性卵巢肿瘤患者作为对照组,50名健康志愿者作为健康组。比较三组血清CCL18、HK10水平,研究组不同肿瘤恶性生物学行为患者血清CCL18、HK10水平,研究组不同化疗灵敏度患者化疗前和化疗3、6个周期后血清CCL18、HK10水平变化值,分析化疗前后血清CCL18、HK10水平变化值与化疗灵敏度的关系及对化疗灵敏度的预测价值。结果研究组血清CCL18、HK10水平均高于对照组、健康组(P<0.05)。研究组不同国际妇产科联盟(FIGO)分期、淋巴结转移、肌层浸润程度血清CCL18和HK10水平、不同分化程度血清HK10水平比较,差异具有统计学意义(P<0.05)。研究组化疗不灵敏患者CCL18△1(△1为化疗3个周期后与化疗前变化值的绝对值)、HK10△1均低于化疗灵敏患者(P<0.05)。CCL18△1、HK10△1联合检测的曲线下面积(AUC)显著高于单一指标(P<0.05)。结论卵巢癌血清CCL18、HK10水平升高与肿瘤恶性生物学行为关系密切,且其变化值能够辅助临床预测化疗灵敏度,两者联合检测具有较高的化疗灵敏度预测价值。 展开更多

Objective:To study the effects of 18β-glycyrrhetinic acid (GA) on proliferation inhibition, apop totic induction, and the relationship between GA-induced apoptosis and intracellular Ca2+ concentration in human breast carcinoma (MCF-7) cells. Methods: After MCF-7 cells were treated with GA at the concentrations from 50 μmol/L to 250 μmol/L for 24 h, cell viability of proliferation was assessed by MTT assay. After the cells were treated with 100 μmol/L, 150 μmol/L, and 200 μmol/L GA for 24 h, the rates of cell apoptosis were examined by terminal deoxynucleotide transferase mediated dUTP nick-end-labeling method and flow cytometry with Annexin V/propidium iodide fluorescent stain. After the cells treated with 150 μmol/L GA for 24 h, intracellular Ca2+ concentration was measured by Fure-2 fluorescein load method. Results: After the cells were treated with GA at the concentrations from 100 μmol/L to 250 μmol/L, the rates of proliferative inhibition were increased significantly (P<0.05 and P<0.01) in a dose dependent fashion. IC50 of the proliferation inhibition was 234.33 μmol/L. Treated with 100 μmol/L, 150 μmol/L, and 200 μmol/L, the rates of cell apoptosis were increased significantly (P<0.01). Intracellular Ca2+ concentration after treatment with GA was higher evidently than that of control (P<0.05). Conclusion: 18β-glycyrrhetinic acid has the effects of the proliferation inhibition and the apoptotic induction on MCF-7 cells. The rise of intracellular Ca2+ level may be depended on apoptosis induced by GA in MCF-7 cells. 展开更多

The total length of the synthesized gene is 571 bp; 18 oligonucleotides, DNA fragments were designed and synthesized by the phosphoramidite four-step chemical method. The whole DNA sequence was synthesized by a one-step total gene synthesis method, and then inserted in pUC18 vector. Five positive clones identified by blue-white colony screening were sent to Shanghai Sangon Biological Engineering Technology and Service Co., Ltd. for sequencing. The sequencing result shows that one clone contained the complete correct gene in all the five positive clones. 展开更多

BACKGROUND End-stage liver disease is a global health complication with high prevalence and limited treatment options.Cell-based therapies using mesenchymal stem cells(MSCs)emerged as an alternative approach to support hepatic regeneration.In vitro preconditioning strategies have been employed to strengthen the regenerative and differentiation potential of MSCs towards hepatic lineage.Chemical compounds of the triterpene class;glycyrrhizic acid(GA)and 18β-glycyrrhetinic acid(GT)possess diverse therapeutic properties including hepatoprotection and anti-fibrosis characteristics.They are capable of modulating several signaling pathways that are crucial in hepatic regeneration.Preconditioning with hepato-protective triterpenes may stimulate MSC fate transition towards hepatocytes.AIM To explore the effect of GA and GT on hepatic differentiation of human umbilical cord-MSCs(hUC-MSCs).METHODS hUC-MSCs were isolated and characterized phenotypically by flow cytometry and immunocytochemistry for the expression of MSC-associated surface molecules.Isolated cells were treated with GA,GT,and their combination for 24 h and then analyzed at three time points;day 7,14,and 21.qRT-PCR was performed for the expression of hepatic genes.Expression of hepatic proteins was analyzed by immunocytochemistry at day 21.Periodic acid Schiff staining was performed to determine the functional ability of treated cells.RESULTS The fusiform-shaped morphology of MSCs in the treatment groups in comparison with the untreated control,eventually progressed towards the polygonal morphology of hepatocytes with the passage of time.The temporal transcriptional profile of preconditioned MSCs displayed significant expression of hepatic genes with increasing time of differentiation.Preconditioned cells showed positive expression of hepatocyte-specific proteins.The results were further corroborated by positive periodic acid Schiff staining,indicating the presence of glycogen in their cytoplasm.Moreover,bi-nucleated cells,which is the typical feature of hepatocytes,were also seen in the preconditioned cells.CONCLUSION Preconditioning with glycyrrhizic acid,18β-glycyrrhetinic acid and their combination,successfully differentiates hUC-MSCs into hepatic-like cells.These MSCs may serve as a better therapeutic option for degenerative liver diseases in future. 展开更多

AIMTo noninvasively investigate tumor cellularity measured using diffusion-weighted magnetic resonance imaging (DW-MRI) and glucose metabolism measured by <sup>18</sup>F-labeled fluorodeoxyglucose positron emission tomography/computed tomography (<sup>18</sup>F-FDG-PET/CT) during radiation therapy (RT) for human papillomavirus negative (HPV-) head and neck squamous cell carcinoma (HNSCC).METHODSIn this prospective study, 6 HPV- HNSCC patients underwent a total of 34 multimodality imaging examinations DW-MRI at 1.5 T Philips MRI scanner [(n = 24) pre-, during- (2-3 wk), and post-treatment (Tx), and <sup>18</sup>F-FDG PET/CT pre- and post-Tx (n = 10)]. All patients received RT. Monoexponential modeling of the DW-MRI data yielded the imaging metric apparent diffusion coefficient (ADC) and the mean of standardized uptake value (SUV) was measured from <sup>18</sup>F-FDG PET uptake. All patients had a clinical follow-up as the standard of care and survival status was documented at 1 year.RESULTSThere was a strong negative correlation between the mean of pretreatment ADC (ρ = -0.67, P = 0.01) and the pretreatment <sup>18</sup>F-FDG PET SUV. The percentage (%) change in delta (∆) ADC for primary tumors and neck nodal metastases between pre- and Wk<sub>2-3</sub> Tx were as follows: 75.4% and 61.6%, respectively, for the patient with no evidence of disease, 27.5% and 32.7%, respectively, for those patients who were alive with disease, and 26.9% and 7.31%, respectively, for those who were dead with disease.CONCLUSIONThese results are preliminary in nature and are indicative, and not definitive, trends rendered by the imaging metrics due to the small sample size of HPV- HNSCC patients in a Meixoeiro Hospital of Vigo Experience. 展开更多

In situ hybridization and Western Blot were implemented to verify the transient expression of recombinant hIL 18 in Cos 7 and D5.Results The eukaryotic expression plasmid pVAX1 IL 18 was constructed successfully.hIL 18 was transiently expressed in Cos 7 and D5.Conclusion The eukaryotic expression plasmid pVAX1 IL 18 was constructed. In situ hybridization and Western Blot results proved the successful transient expression of pVAX1 IL 18 in Cos 7 and D5.Therefore,the work has settled the foundation for further biological research on hIL 18,including immunogene therapy through hIL 18. 展开更多

目的探讨普萘洛尔对人脐静脉内皮细胞(human umbilical vein endothelial cell,HUVEC)增殖、凋亡、迁移及成管能力,以及对性别决定区Y框18(sex determining region Y-box 18,SOX18)、基质金属蛋白酶-7(matrix metalloproteinase-7,MMP-7... 目的探讨普萘洛尔对人脐静脉内皮细胞(human umbilical vein endothelial cell,HUVEC)增殖、凋亡、迁移及成管能力,以及对性别决定区Y框18(sex determining region Y-box 18,SOX18)、基质金属蛋白酶-7(matrix metalloproteinase-7,MMP-7)和血管内皮生长因子A(vascular endothelial growth factor A,VEGFA)表达的影响。方法以不同浓度的普萘洛尔处理HUVEC,采用CCK-8法检测细胞活力,选出最佳浓度及时间。实验分为对照组,普萘洛尔不同浓度(50、100、150μmol/L)组,采用流式细胞术、细胞划痕实验和成管实验观察HUVEC凋亡、迁移和管腔形成情况,Western blot和实时荧光定量PCR法检测SOX18、MMP-7、VEGFA蛋白及mRNA表达水平。结果与对照组相比,普萘洛尔不同浓度组细胞凋亡率升高(P<0.05),且随药物浓度增加显著升高(P<0.05);普萘洛尔不同浓度组伤口愈合率降低,成管节点数和成管总长度减少,SOX18、MMP-7、VEGFA蛋白及mRNA表达下调(P<0.05)。结论普萘洛尔可抑制HUVEC的增殖、迁移、成管能力并促进细胞凋亡,降低SOX18、MMP-7和VEGFA表达水平。 展开更多

目的 探究非酒精性脂肪性肝病(NAFLD)病人血清长链非编码RNA人类白细胞抗原复合体18(lncRNA HCG18)与氧化应激指标及肝纤维化程度的相关性。方法 以儋州市人民医院2020年11月至2022年3月收治的92例NAFLD病人为NAFLD组,另选取同期健康体... 目的 探究非酒精性脂肪性肝病(NAFLD)病人血清长链非编码RNA人类白细胞抗原复合体18(lncRNA HCG18)与氧化应激指标及肝纤维化程度的相关性。方法 以儋州市人民医院2020年11月至2022年3月收治的92例NAFLD病人为NAFLD组,另选取同期健康体检志愿者90例为对照组。根据NAFLD肝纤维化(NFS)评分将NAFLD组病人分为排除晚期纤维化组(30例)、中间状态组(46例)、晚期纤维化组(16例)。采用实时荧光定量PCR(qRT-PCR)法检测血清lncRNA HCG18表达水平;酶联免疫吸附测定(ELISA)检测氧化应激指标水平;Pearson相关性分析NAFLD病人血清lncRNA HCG18与氧化应激指标的关系;Spearman相关性分析NAFLD病人血清lncRNA HCG18与NFS评分的关系。采用受试者操作特征(ROC)曲线分析血清lncRNA HCG18对NAFLD的诊断价值。结果 NAFLD组lncRNA HCG18(1.37±0.29比1.01±0.24)、丙二醛[(6.96±2.13)μmol/L比(4.17±1.04)μmol/L]水平明显高于对照组(P<0.001),谷胱甘肽过氧化物酶(GSH-Px)[(72.39±11.26)U/mL比(98.25±13.92)U/mL]、超氧化物歧化酶(SOD)[(68.36±10.65)U/mL比(92.84±12.89)U/mL]水平明显低于对照组(t=9.11、11.19、13.79、13.98,P<0.001)。排除晚期纤维化组、中间状态组、晚期纤维化组lncRNA HCG18(1.20±0.25比1.39±0.30比1.65±0.35)、丙二醛[(5.64±1.88)μmol/L比(7.12±2.16)μmol/L比(8.99±2.51)μmol/L]水平依次显著升高(F=12.36、13.06,P<0.001),GSH-Px[(81.56±13.58)U/mL比(71.74±10.52)U/mL比(57.06±9.05)U/mL]、SOD[(81.45±12.52)U/mL比(66.52±10.25)U/mL比(49.13±8.29)U/mL]水平依次显著降低(F=24.27、48.42,P<0.001)。Pearson相关性分析显示,NAFLD组血清lncRNA HCG18水平与丙二醛呈正相关(r=0.59,P<0.001),与GSH-Px、SOD均呈负相关(r=-0.57、-0.62,P<0.001);Spearman相关性分析显示NAFLD组血清lncRNA HCG18水平与NFS评分呈正相关(r=0.69,P<0.001)。血清lncRNA HCG18诊断NAFLD的ROC曲线下面积(AUC)为0.85。结论 NAFLD病人血清lncRNA HCG18显著升高,与氧化应激指标及肝纤维化程度存在一定的相关性。 展开更多

血清DKK1、sRAGE、hCAP-18对急性淋巴细胞白血病患儿的诊断和预后不良的预测价值

目的 探讨血清Dickkopf相关蛋白1(DKK1)、可溶性晚期糖基化终末产物受体(sRAGE)、人类阳离子抗菌蛋白-18(hCAP-18)对急性淋巴细胞白血病患儿的诊断和预后不良的预测价值。方法 选取2020年4月至2022年4月南京鼓楼医院集团宿迁医院收治的8... 目的 探讨血清Dickkopf相关蛋白1(DKK1)、可溶性晚期糖基化终末产物受体(sRAGE)、人类阳离子抗菌蛋白-18(hCAP-18)对急性淋巴细胞白血病患儿的诊断和预后不良的预测价值。方法 选取2020年4月至2022年4月南京鼓楼医院集团宿迁医院收治的84例急性淋巴细胞白血病患儿作为研究组,包括标危28例、中危36例、高危20例。另选取同期在南京鼓楼医院集团宿迁医院体检的84例健康儿童作为对照组。收集所有研究对象的临床资料。采用酶联免疫吸附试验检测所有研究对象血清DKK1、sRAGE、hCAP-18水平。对研究组患儿进行为期1年的随访,并按随访结果分为预后不良组和预后良好组。绘制受试者工作特征(ROC)曲线分析血清DKK1、sRAGE、hCAP-18在急性淋巴细胞白血病患儿诊断和预后不良的预测价值。采用多因素Logistic回归分析影响急性淋巴细胞白血病发生的因素。结果 与对照组相比,研究组患儿白细胞计数(WBC)、DKK1水平显著升高,sRAGE、hCAP-18水平显著降低,差异均有统计学意义(P<0.05)。与研究组标危患儿相比,中危和高危患儿血清DKK1水平显著升高,sRAGE、hCAP-18水平显著降低,差异均有统计学意义(P<0.05);与中危患儿相比,高危患儿血清DKK1水平显著升高,sRAGE、hCAP-18水平显著降低,差异均有统计学意义(P<0.05)。血清DKK1、sRAGE、hCAP-18联合诊断急性淋巴细胞白血病的曲线下面积(AUC)显著高于血清DKK1、sRAGE、hCAP-18单独诊断的AUC(Z=2.820,P=0.005;Z=5.170,P<0.001;Z=5.272,P<0.001)。多因素Logistic回归分析结果显示,血清WBC、DKK1、sRAGE、hCAP-18是急性淋巴细胞白血病发生的影响因素(P<0.05)。随访1年后,26例患儿纳入预后不良组,58例患儿纳入预后良好组。与预后良好组相比,预后不良组患儿血清DKK1水平显著升高,sRAGE、hCAP-18水平显著降低,差异均有统计学意义(P<0.05)。与预后不良组中危患儿相比,高危患儿DKK1水平显著升高,sRAGE、hCAP-18水平显著降低,差异均有统计学意义(P<0.05)。血清DKK1、sRAGE、hCAP-18联合预测急性淋巴细胞白血病患儿预后不良的AUC明显高于血清DKK1、sRAGE、hCAP-18单独预测的AUC(Z=2.312,P=0.021;Z=3.160,P=0.002;Z=2.926,P=0.003)。结论 急性淋巴细胞白血病患儿血清DKK1高表达,sRAGE、hCAP-18低表达,且三者联合检测对急性淋巴细胞白血病具有一定的诊断和预后预测价值。 展开更多

目的评价人乳头状瘤病毒(human papillomaviruses,HPV)16/18二价疫苗在预防宫颈HPV16/18相关感染和癌前病变的作用,对预防接种人群提出合理化建议。方法通过Web of science搜索引擎搜索MEDLINE、Web of Science核心合集、BIOSIS Preview... 目的评价人乳头状瘤病毒(human papillomaviruses,HPV)16/18二价疫苗在预防宫颈HPV16/18相关感染和癌前病变的作用,对预防接种人群提出合理化建议。方法通过Web of science搜索引擎搜索MEDLINE、Web of Science核心合集、BIOSIS Previews等数据库,通过Endnote X9 Online Search搜索EBSCO数据库,通过Cochrane Library搜索Cochran Central Register of Controlled Trials(CENTRAL)数据库。另外,通过www.clinicaltrials.gov临床试验平台获取相关文献和数据。搜索的英文主题词或关键词包括HPV16/18、bivalent vaccine、cervical neoplasm和clinical trial。采用Review manager 5.3评估HPV16/18二价疫苗接种的近期和远期疗效、可靠性、危险度和统计偏倚,并根据结果做预防接种管控分析。结果总计纳入了2004-2017年19篇英文文献报告的10个临床随机研究结果,40066例受试者进入Meta分析。在HPV阴性的健康女性受试者中,接种HPV16/18二价疫苗者与对照组相比HPV16/18相关宫颈偶发性感染(incident infection)、6个月和12个月的持续性感染(persistent infection,PI)相对风险(relative risk,RR)分别降低68%~78%、82%~94%和84%~97%;HPV16/18相关宫颈不典型鳞状细胞-意义未明及以上(ASC-US+)、宫颈上皮内瘤变1级及以上(CIN1+)和2级及以上(CIN2+)的患病RR分别降低89%~94%、87%~91%和87%~92%;任何HPV相关ASC-US+、CIN1+、CIN2+、CIN3+和原位癌(AIS)的患病RR,分别降低28%~29%、50%、64%~72%、86%~93%和93%。在健康女性受试者中,不管入组时HPV DNA状态如何,与对照组相比,接种二价疫苗至少降低1次HPV16/18相关ASC-US+、CIN1+、CIN2+、CIN3+和AIS患病RR,分别为57%、50%、45%、34%和70%;任何HPV表型相关CIN1+、CIN2+、CIN3+和AIS的患病RR降低,分别为26%~28%、28%~33%、34%~45%和77%。结论二价疫苗对于基线时没有感染HPV的年轻女性宫颈偶发性和持续性感染以及宫颈癌前病变具有良好的保护作用。即使对于HPV暴露的不同风险人群,二价疫苗对于降低宫颈癌前病变和原位癌的患病风险也有一定的作用。 展开更多

目的:研究肉眼拟诊为宫颈糜烂的患者其组织学诊断及人乳头瘤病毒16型(或18型)E6蛋白(HPV16/18E6)的表达及其临床意义。方法:随机抽取年龄为18~65岁的肉眼观为宫颈糜烂的患者100例,分为宫颈轻度糜烂组(30例)、中度糜烂组(41例)、重度糜... 目的:研究肉眼拟诊为宫颈糜烂的患者其组织学诊断及人乳头瘤病毒16型(或18型)E6蛋白(HPV16/18E6)的表达及其临床意义。方法:随机抽取年龄为18~65岁的肉眼观为宫颈糜烂的患者100例,分为宫颈轻度糜烂组(30例)、中度糜烂组(41例)、重度糜烂组(29例),光滑组100例(对照组)。分别行阴道镜下宫颈活检,作出病理组织学诊断,采用免疫组织化学法(SP法)测定宫颈组织中HPV16/18E6表达情况。结果:(1)各组宫颈活检的病理结果分为正常、宫颈炎、宫颈上皮内瘤样病变(CIN)Ⅰ、CINⅡ、CINⅢ及宫颈癌(Ca)。各组间疾病构成差异有统计学意义(P<0.01),且CINⅡ、CINⅢ及Ca随着疾病严重程度亦呈增加趋势。(2)宫颈光滑组、轻度糜烂组、中度糜烂组及重度糜烂组的HPV16/18E6蛋白阳性率分别为21.0%、26.7%、31.7%及51.6%,差异有统计学意义(P<0.01)。其中,光滑组与轻度糜烂组及中度糜烂组、轻度糜烂组与中度糜烂组阳性率差异无统计学意义(均P>0.05);重度糜烂组与其他各组比较差异有统计学意义(P<0.05或P<0.01)。结论:肉眼拟诊为宫颈糜烂的患者其组织学诊断多样化,糜烂程度越高,发生宫颈上皮内瘤变及宫颈癌的可能性越大,HPV16/18E6的表达水平亦有随之升高的趋势。 展开更多
