Current understanding of autoimmune pancreatitis (AIP) recognizes a histopathological subtype of the disease to fall within the spectrum of IgG4-related disease. Along with clinical, laboratory, and histopathological data, imaging plays an important role in the diagnosis and management of AIP, and more broadly, within the spectrum of IgG4-related disease. In addition to the defined role of imaging in consensus diagnostic protocols, an array of imaging modalities can provide complementary data to address specific clinical concerns. These include contrast-enhanced computed tomography (CT) and magnetic resonance (MR) imaging for pancreatic parenchymal lesion localization and characterization, endoscopic retrograde and magnetic resonance cholangiopancreatography (ERCP and MRCP) to assess for duct involvement, and more recently, positron emission tomography (PET) imaging to assess for extra-pancreatic sites of involvement. While the imaging appearance of AIP varies widely, certain imaging features are more likely to represent AIP than alternate diagnoses, such as pancreatic cancer. While nonspecific, imaging findings which favor a diagnosis of AIP rather than pancreatic cancer include: delayed enhancement of affected pancreas, mild dilatation of the main pancreatic duct over a long segment, the &#x0201c;capsule&#x0201d; and &#x0201c;penetrating duct&#x0201d; signs, and responsiveness to corticosteroid therapy. Systemic, extra-pancreatic sites of involvement are also often seen in AIP and IgG4-related disease, and typically respond to corticosteroid therapy. Imaging by CT, MR, and PET also play a role in the diagnosis and monitoring after treatment of involved sites. 展开更多

Solitary organ autoimmune disorders,formerly known as autoimmune pancreatitis(AIP),autoimmune sialadenitis,and autoimmune sclerosing cholangitis,are now considered organ-specific manifestations of systemic immunoglobulin G4-related disease(IgG4-RD).AIP and IgG4-RD are characterized by elevated serum concentration of IgG4 antibody(Ab),accumulation of IgG4-expressing plasmacytes in the affected organs,and involvement of multiple organs.It is well established that enhanced IgG4 Ab responses are a hallmark of AIP and IgG4-RD for diagnosis and monitoring disease activity.However,a significant fraction of patients with AIP and IgG4-RD who develop chronic fibroinflammatory responses have normal serum concentrations of this IgG subtype.In addition,disease flare-up is sometimes seen even in the presence of normalized serum concentrations of IgG4 Ab after successful induction of remission by prednisolone.Therefore,it is necessary to identify new biomarkers based on the understanding of the pathophysiology of AIP and IgG4-RD.Recently,we found that activation of plasmacytoid dendritic cells producing both interferon-α(IFN-α)and interleukin-33(IL-33)mediate murine AIP and human IgG4-RD.More importantly,we provided evidence that serum concentrations of IFN-αand IL-33 could be useful biomarkers for the diagnosis and monitoring of AIP and IgG4-RD activity after induction of remission in these autoimmune disorders.In this Frontier article,we have summarized and discussed biomarkers of AIP and IgG4-RD,including Igs,autoAbs,and cytokines to provide useful information not only for clinicians but also for researchers. 展开更多

BACKGROUND IgG4-related disease can manifest diversely,including autoimmune pancreatitis and IgG4-related cholangiopathy.We are reporting a very unusual cause of pancreatic cancer triggered in a previously unknown IgG4-related disease.CASE SUMMARY A 75-year-old man was diagnosed with a 43 mm×33 mm pancreatic head tumor after consulting for abdominal pain and jaundice.A pancreaticoduodenectomy was carried out uneventfully,and the histopathology report showed an early stage of acinar-cell pancreatic cancer.The patient reconsulted on the 30th postoperative day with fever,jaundice and asthenia.Magnetic resonance cholangiopancreatography evidenced an extense bile duct stricture.A percutaneous biliary drainage proved to be ineffective,even after exchanging it with larger bore drainage.Reviewing the surgical specimen,features compatible with IgG4-related disease were observed.Consequently,empiric treatment with steroids was initiated achieving excellent results.CONCLUSION IgG4-related disease may cause chronic inflammation of the pancreas and can condition pancreatic malignancies. 展开更多

Autoimmune pancreatitis(AIP)is a form of chronic pancreatitis that is categorized as type 1 or type 2according to the clinical profile.Type 1 AIP,which predominantly presents in a few Asian countries,is a hyper-IgG4-related disease.We report a case of IgG4-related AIP overlapping with Mikulicz’s disease and lymphadenitis,which is rare and seldom reported in literature.A 63-year male from Northeast China was admitted for abdominal distension lasting for one year.He presented symmetric swelling of the parotid and submandibular glands with slight dysfunction of salivary secretion for 6 mo.He had a 2-year history of bilateral submandibular lymphadenopathy without pain.He underwent surgical excision of the right submandibular lymph node one year prior to admission.He denied any history of alcohol,tobacco,or illicit drug use.Serological examination revealed high fasting blood sugar level(8.8 mmol/L)and high level of IgG4(15.2 g/L).Anti-SSA or anti-SSB were negative.Computed tomography of the abdomen showed a diffusely enlarged pancreas with loss of lobulation.Immunohistochemical stain for IgG4 demonstrated diffuse infiltration of IgG4-positive plasma cells in labial salivary gland and lymph node biopsy specimens.The patient received a dose of 30 mg/d of prednisone for three weeks.At this three-week follow-up,the patient reported no discomfort and his swollen salivary glands,neck lymph node and pancreas had returned to normal size.The patient received a maintenance dose of 10mg/d of prednisone for 6 mo,after which his illness had not recurred. 展开更多

Based on histological and immunohistochemical exami- nation of various organs of patients with autoimmune pancreatitis (AIP), a novel clinicopathological entity of IgG4-related sclerosing disease has been proposed. This is a systemic disease that is characterized by extensive IgG4-positive plasma cells and T-lymphocyte infiltration of various organs. Clinical manifestations are apparent in the pancreas, bile duct, gallbladder, salivary gland, retroperitoneum, kidney, lung, and prosrate, in which tissue fibrosis with obliterative phlebitis is pathologically induced. AlP is not simply pancreatitis but, in fact, is a pancreatic disease indicative of IgG4- related sclerosing diseases. This disease includes AlP, sclerosing cholangitis, cholecystitis, sialadenitis, retro-peritoneal fibrosis, tubulointerstitial nephritis, interstitial pneumonia, prostatitis, inflammatory pseudotumor and lymphadenopathy, all IgG4-related. Most IgG4-related sclerosing diseases have been found to be associated with AlP, but also those without pancreatic involvement have been reported. In some cases, only one or two organs are clinically involved, while in others, three or four organs are affected. The disease occurs predominantly in older men and responds well to steroid therapy. Serum IgG4 levels and immunos-taining with anti-IgG4 antibody are useful in making the diagnosis. Since malignant tumors are frequently suspected on initial presentation, IgG4-related sclerosing disease should be considered in the differential diagnosis to avoid unnecessary surgery. 展开更多

The clinical features of AIP which presentas a localized pancreatic mass greatly mimic pancreaticcancer. Cases with AIP mimicking pancreatic cancer arereported numerously. However, cases of IgG4-related AIPmanifesting acute pancreatitis symptoms and extensiveperipancreatic effusion are rarely observed. Meanwhile,acute pancreatitis is a common disease. If misdiagnosed asacute pancreatitis, IgG4-related AIP may progressseriously. Ig G4-related disease is a recently recognized entity linked initially to autoimmune pancreatitis and has been subsequently described in nearly every organ system. Men over the age of 50 represent the most affected demographic group and a comprehensive set of diagnostic criteria has been developed to aid treating clinicians. Though elevated levels of Ig G4 in the serum are suggestive of the disease, definitive diagnosis is made on histopathology. Treatment is tailored to the clinical presentation with corticosteroid therapy known to have proven efficacy. Gastric manifestations of the Ig G4-related disease primarily come in two varieties, notably chronic ulceration or pseudotumor formation. Autoimmune pancreatitis conveys increased risk for Ig G4-related disease of the stomach, which is independent of Helicobacter pylori status. In this case report, we present an acute gastric-pericardial fistula secondary to Ig G4-related disease that required urgent operative management. To our knowledge, this is the first report in the medical literature describing this complication of Ig G4-related disease. 展开更多

BACKGROUND Immunoglobulin G4-related disease(IgG4-RD)is a multisystemic mass forming immune-mediated disease that affects almost every organ and is a diagnostic challenge for every clinician.There is a lack of adequate epidemiological data worldwide,and evidence-based treatment recommendations are not yet established.We report the first case of IgG4-RD from Lithuania and the Baltic Sea region presented with thyroiditis,orbital myositis,orbitopathy,uveitis,scleritis,sialadenitis,autoimmune pancreatitis and prostatitis.CASE SUMMARY A 54-year-old Caucasian male was admitted to our tertiary Centre complaining of severe weight loss,diarrhoea,abdominal pain,salivary gland swelling,sicca symptoms and diplopia.On examination,bilateral palpable masses in the projection of major salivary glands,severe protrusion of the left eyeball and cachexia were noted.The patient was previously diagnosed with autoimmune thyroiditis and endocrine ophthalmopathy.The magnetic resonance imaging(MRI)of the head revealed enlarged extraocular muscles indicating orbital myositis.The biopsy from the salivary gland mass indicated sialadenitis.Abdominal MRI showed signs of autoimmune pancreatitis,and a serological test revealed the elevated serum IgG4 concentration.The patient was then diagnosed with IgG4-RD and successfully treated with prednisolone.There was a significant clinical,serological and radiological improvement after one month of treatment and no signs of relapse within twenty months.However,it took almost 18 years and the efforts of eight different medical specialists to establish the correct diagnosis.CONCLUSION A comprehensive approach to the patient is essential to improving the recognition of rare immune system conditions,such as IgG4-RD. 展开更多

目的研究血清免疫球蛋白(Ig)G4水平在IgG_(4)相关肝胆胰疾病的诊断及疗效评估中的价值。方法通过收集2020年8月~2022年3月来广东省人民医院检验科进行血清IgG_(4)水平检测且明确诊断为肝胆胰疾病的患者267例,将其分为IgG_(4)相关肝胆胰... 目的研究血清免疫球蛋白(Ig)G4水平在IgG_(4)相关肝胆胰疾病的诊断及疗效评估中的价值。方法通过收集2020年8月~2022年3月来广东省人民医院检验科进行血清IgG_(4)水平检测且明确诊断为肝胆胰疾病的患者267例,将其分为IgG_(4)相关肝胆胰疾病组(n=19)和非IgG_(4)相关肝胆胰疾病组(n=248)。另外选取50例体检健康者作为对照组。采用乳胶免疫比浊法检测血清IgG_(4),比较各组血清IgG_(4)水平。应用ROC曲线评价血清IgG_(4)水平鉴别诊断IgG_(4)相关肝胆胰疾病的最佳诊断界值及敏感度和特异度。收集实验室检测资料,分析其他实验室指标与血清IgG_(4)水平之间的相关性。结果IgG_(4)相关肝胆胰疾病组血清IgG_(4)水平[3.86(2.25,6.90)g/L]显著高于非IgG_(4)相关肝胆胰疾病组[0.75(0.47,1.29)g/L]和健康对照组[0.71(0.40,1.03)g/L],差异具有统计学意义(Z=3.641,6.314,均P<0.01)。非IgG_(4)相关肝胆胰疾病中胆管肿瘤疾病组有20%(4/20)的患者IgG_(4)水平高于正常值,在非IgG_(4)相关肝胆胰疾病中阳性率最高。通过IgG_(4)相关肝胆胰疾病组和其他对照组绘制ROC曲线,最佳诊断界值为2.02g/L,AUC为0.875(95%CI:0.953~0.995)。血清IgG_(4)水平仅与IgG呈显著正相关(r=0.943,P<0.05)。IgG_(4)相关肝胆胰疾病组中其他实验室指标阳性率排名前三的是GGT(75.00%),ALP(68.75%)和空腹GLU(61.11%)。IgG_(4)相关肝胆胰疾病患者有效治疗后血清IgG_(4)水平均明显降低(Z=-2.547,P=0.011)。结论血清IgG_(4)水平鉴别诊断IgG_(4)相关肝胆胰疾病最佳诊断界值为2.02g/L。动态观察血清IgG_(4)水平对诊断IgG_(4)相关肝胆胰疾病有重大提示作用。血清IgG_(4)也是判断临床治疗有效的重要指标。 展开更多

AIM: To identify clinical and pathological differences between serum immunoglobulin G4 (IgG4)-positive (SIP) and IgG4-negative (SIN) type 1 autoimmune pancreatitis (AIP) in South Korea. METHODS: AIP was diagnosed by the international consensus diagnostic criteria. The medical records and pathology were retrospectively reviewed and IgG4-positive cells were counted in a high power field (HPF). Type I AIP was defined as a high serum level of IgG4or histological finding. SIN type 1 AIP was defined as a histological evidence of type 1 AIP and a normal serum IgG4 level. The clinical and pathological findings were compared between the two groups. The analysis was performed using Student's t test, Fischer's exact test and Mann-Whitney's U test. A P value of < 0.05 was considered statistically significant. As repeated com- parison was made, P values of less than 5% (P < 0.05) were considered significant. RESULTS: Twenty five patients with definite type 1 AIP (19 histologically and six serologically diagnosed cases) were enrolled. The mean tissue IgG4 concentrations were significantly higher in SIP than SIN group (40 cells per HPF vs 18 cells per HPF, P = 0.02). Among eight SIN patients, the tissue IgG4 concentrations were less than 15 cells per HPF in most of cases, except one. The sensitivity of serum IgG4 was 68% (17 SIP and eight SIN AIP). Other organ involvement was more frequent- ly associated with SIP than SIN AIP (59% vs 26%, P = 0.016). However, the relapse rate and diffuse swelling of the pancreas were not associated with serum IgG4 level. The concentrations of IgG4-positive cells per HPF were higher in SIP than SIN AIP (40 vs 18, P = 0.02). CONCLUSION: The sensitivity of serum IgG4 was 68% in type 1 AIP. High serum IgG4 level was associated with other organ involvement and tissue IgG4 concentration but did not affect the relapse rate in type 1 AIP. 展开更多

Its etiology and pathogenesis are still under investigation, especially during the last decade Another aspect of interest is the attempt to establish specific criteria for the differential diagnosis between autoimmune pancreatitis and pancreatic cancer, entities that are frequently indistinguishable. DATA SOURCES: An extensive search of the PubMed database was performed with emphasis on articles about the differential diagnosis between autoimmune pancreatitis and pancreatic cancer up to the present. RESULTS: The most interesting outcome of recent research is the theory that autoimmune pancreatitis and its various extra-pancreatic manifestations represent a systemic fibro inflammatory process called IgG4-related systemic disease The diagnostic criteria proposed by the Japanese Pancreatic Society, the more expanded HISORt criteria, the new definitions of histological types, and the new guidelines of the International Association of Pancreatology help to establish the diagnosis of the disease types. CONCLUSION: The valuable help of the proposed criteria for the differential diagnosis between autoimmune pancreatitis and pancreatic cancer may lead to avoidance of pointless surgical treatments and increased patient morbidity. 展开更多

Type 1 autoimmune pancreatitis (AIP) or chronic sclerosing sialadenitis (K&#x000fc;ttner&#x02019;s tumour) is an uncommon disorder that has recently been confirmed as an IgG4-related disease. Here, we describe a rare case of a 53-year-old male patient who primarily presented with pancreatic body mass, left neck mass and several lumps in his lower lip mimicking pancreatic cancer (PC) and neck metastasis. The patient underwent pancreatic body mass and labial gland lumps resection as well as an ultrasound-guided biopsy of the left neck mass. He was diagnosed with IgG4-related focal type of AIP (f-AIP) and K&#x000fc;ttner&#x02019;s tumour by immunohistochemistry. The patient responded well to corticosteroid therapy and remains healthy with no signs of recurrence at one year follow-up. The differentiation of f-AIP from PC is very important to avoid unnecessary pancreatic resection. 展开更多

Ordinary chronic pancreatitis is a well-known risk factor for pancreatic cancer,whereas such an association with autoimmune pancreatitis(AIP)is widely debated.Due to the rarity of the latter disorder,there are few specific clinical and epidemiological studies investigating the relation between AIP and pancreatic cancer,which do not seem to support it.However,these studies are affected by several limitations and,therefore,a link between AIP(and,specifically,type 1 AIP)and pancreatic cancer cannot be ruled out definitively on this basis.Moreover,several immunopathological aspects of type 1 AIP and,in general,immunoglobulin G4-related disease can create an immunological context that may impair the tumoral immunosurveillance and promote the pancreatic carcinogenesis and its progression.In detail,Th2 immunological dominance,type 2 macrophage polarization and basophil infiltration observed in type 1 AIP,may play a permissive role in creating a favorable immunological environment for pancreatic carcinogenesis,in addition to the immunosuppressive therapies that can be used in these patients. 展开更多

AIM To examine thyroid function and clinical features of hypothyroidism in autoimmune pancreatitis(AIP) patients.METHODS We examined thyroid function in 77 patients with type 1 AIP(50 males, 27 females; median age 68 years, range 33-85) diagnosed according to the Japanese diagnostic criteria for AIP 2011. We compared clinical and serological findings between patients with and without various categories of hypothyroidism. The change in hypothyroidism after steroid therapy was also examined. RESULTS Eight patients(10%) had hypothyroidism of 6 patients had subclinical hypothyroidism with a normal serum free thyroxine(FT4) and high thyroid stimulating hormone(TSH) level, and 2 patients had central hypothyroidism with low serum free triiodothyronine(FT3), FT4 and TSH levels. A significant goiter of the thyroid was not observed in any patient. There were no significant differences in age; male to female ratio; serum concentrations of IgG and IgG 4-related disease(IgG4-RD); presence of antithyroglobulin antibody, antinuclear antigen or rheumatoid factor; or presence of extrapancreatic lesions between the 6 patients with subclinical hypothyroidism and patients with euthyroidism. After steroid therapy, both subclinical and central hypothyroidism improved with improvement of the AIP.CONCLUSION Hypothyroidism was observed in 8(10%) of 77 AIP patients and was subclinical in 6 patients and central in 2 patients. Further studies are necessary to clarify whether this subclinical hypothyroidism is another manifestation of IgG4-RD. 展开更多

Autoimmune pancreatitis(AIP)is a type of immune-mediated pancreatitis subdivided into two subtypes,type 1 and type 2 AIP.Furthermore,type 1 AIP is considered to be the pancreatic manifestation of the immunoglobulin G4(IgG4)-related disease.Nowadays,AIP is increasingly researched and recognized,although its diagnosis represents a challenge for several reasons:False positive ultrasound-guided cytological samples for a neoplastic process,difficult to interpret levels of IgG4,the absence of biological markers to diagnose type 2 AIP,and the challenging clinical identification of atypical forms.Furthermore,60%and 78%of type 1 and type 2 AIP,respectively,are retrospectively diagnosed on surgical specimens of resected pancreas for suspected cancer.As distinguishing AIP from pancreatic ductal adenocarcinoma can be challenging,obtaining a definitive diagnosis can therefore prove difficult,since endoscopic ultrasound fine-needle aspiration or biopsy of the pancreas are suboptimal.This paper focuses on recent innovations in the management of AIP with regard to the use of artificial intelligence,new serum markers,and new therapeutic approaches,while it also outlines the current management recommendations.A better knowledge of AIP can reduce the recourse to surgery and avoid its overuse,although such an approach requires close collaboration between gastroenterologists,surgeons and radiologists.Better knowledge on AIP and IgG4-related disease remains necessary to diagnose and manage patients. 展开更多

Background:Type 1 autoimmune pancreatitis(AIP)is the pancreatic manifestation of IgG4-related disease.However,this benign disease can result in the peripancreatic vascular involvement(PVI)on occasion,which increases the difficulty of diagnosis and treatment of this clinical entity as well as for differentiating it from pancreatic malignancies.Methods:We retrospectively reviewed the information on demographics,clinical presentation,laboratory,imaging and endoscopic findings of 101 hospitalized patients with type 1 AIP treated in our department.All the patients were divided into non-PVI and PVI groups according to the first hospitalized medical data.Univariate and multivariate analyses were performed to analyse the potential predictive parameter(s)of PVI in AIP patients.Results:Among the 101 type 1 AIP patients,52(51.5%)exhibited PVI,with a male/female ratio 5.5:1.Their average age was 58.37±8.68 years old.Univariate analysis revealed that the location of pancreatitis lesions,including the pancreatic tail(P=0.010),the presence of splenomegaly(P=0.001)and the white blood cell(WBC)number in peripheral blood(P=0.020),were significantly associated with PVI.The location of pancreatitis lesions,including the pancreatic tail(P=0.023),and the presence of splenomegaly(P=0.010)were found to be independent predictors of the development of PVI by a multivariable regression analysis.A total of 18 out of 25 patients in PVI group who underwent corticosteroid treatment and no less than 6 months radiological follow-up showed improvement in vascular lesions,and no case exhibited exacerbation of PVI lesions during follow-up.Of 36 patients in non-PVI group who were followed up for no less than 6 months,only one case exhibited PVI.Conclusions:This retrospective study demonstrated that type 1 AIP was associated with a high proportion of PVI.Pancreatic tail involvement and splenomegaly may predict the PVI in type 1 AIP.PVI lesions are reversible in a subset of patients. 展开更多
