Motivational interviewing in postoperative rehabilitation and chronic disease management: Current findings and future research directions

This editorial highlights a recently published study examining the effectiveness of music therapy combined with motivational interviewing(MI)in addressing an-xiety and depression among young and middle-aged patients following percuta-neous coronary intervention.It further explores existing evidence and potential future research directions for MI in postoperative rehabilitation and chronic disease management.MI aims to facilitate behavioral change and promote healthier lifestyles by fostering a trusting relationship with patients and enhan-cing intrinsic motivation.Research has demonstrated its effectiveness in posto-perative recovery for oncological surgery,stroke,organ transplants,and gastroin-testinal procedures,as well as in managing chronic conditions such as diabetes,obesity,and periodontal disease.The approach is patient-centered,adaptable,cost-effective,and easily replicable,though its limitations include reliance on the therapist’s expertise,variability in individual responses,and insufficient long-term follow-up studies.Future research could focus on developing individualized and precise intervention models,exploring applications in digital health management,and confirming long-term outcomes to provide more compre-hensive support for patient rehabilitation.

Video Feedback Improves Anesthesia Residents'Communication Skill and Performance on Showing Empathy During Preoperative Interviews

Objective To determine the impact of scenario-based lecture and personalized video feedback on anesthesia residents'communication skills during preoperative visits.Methods A total of 24 anesthesia residents were randomly divided into a video group and a control group.Residents in both groups took part in a simulated interview and received a scenario-based lecture on how to communicate with patients during preoperative visits.Afterwards,residents in the video group received personalized video feedback recorded during the simulated interview.One week later all the residents undertook another simulated interview.The communication skills of all the residents were assessed using the Consultation and Relational Empathy measure(CARE)scale by two examiners and one standardized patient(SP),both of whom were blinded to the group allocation.Results CARE scores were comparable between the two groups before training,and significantly improved after training in both groups(all P<0.05).The video group showed significantly greater increase in CARE score after the training than the control group,especially assessed by the SP(t=6.980,P<0.001).There were significant correlations between the examiner-assessed scores and SP-assessed scores(both P=0.001).Conclusion Scenario-based lectures with simulated interviews provide a good method for training communication skills of anesthesia residents,and personalized video feedback can enhance their performance on showing empathy during preoperative interview.

Music therapy combined with motivational interviewing

BACKGROUND Percutaneous coronary intervention(PCI)is one of the main treatment methods for myocardial infarction(MI).Despite its positive effects,patients often experience different degrees of anxiety and depression after the intervention.Therefore,effectively changing the emotional state of patients with MI during PCI remains a focus of clinical research.AIM To assess the effect of music therapy and motivational interviewing in young and middle-aged patients with anxiety and depression after PCI.METHODS We collected data from January 2022 to December 2023 from 86 young and middle-aged patients with MI after PCI.They were divided into observation and control groups according to the random throwing method.The observation group consisted of 43 patients undergoing music therapy and motivational interviewing,and the control group(43 cases)underwent music therapy and conventional communication.The two groups were then compared on mood status[Chinese Brief Mood Status Scale(POMS)],coping methods[Medical Coping Methods Questionnaire(MCMQ)Chinese version],and healthy lifestyle behaviors[Heart Health Self-Efficacy and Self-Management(HH-SESM)scale].RESULTS Two weeks post-intervention,the observation group had lower POMS scores,improved MCMQ scores,and higher HH-SESM scores than the control group(P<0.05).CONCLUSION The combined intervention of music therapy and motivational interviewing for young and middle-aged patients with anxiety and depression after MI can effectively regulate their mood,reduce anxiety and depression symptoms,and stimulate patients to actively face their condition.It also encourages the formation of healthy behavioral habits.

The Impact of Street Interviews on Chinese EFL Undergraduates’Intercultural Communicative Competence

The study adopts Byram’s Intercultural Communicative Competence(ICC)model as the theoretical framework,and structured interviews were conducted to explore the impact of street interviews on the participants’ICC.Analysis results revealed street interviews help students acquire knowledge in intercultural attitudes,intercultural knowledge,skills of discovery and interaction,and critical cultural awareness,aligning with the four components of Byram’s ICC model.Meanwhile,they have positive perceptions of street interviews.Therefore,street interviews have a positive effect on the development of participants’ICC.

基于就医服务需求的医院导向标识系统设计改造研究

目的:设计改造一套基于就医服务需求的医院导向标识系统。方法:以行业标准为依据,按照“统一样式、精准指引、服务患者”原则对现有医院标识系统进行改造。从就医服务需求侧出发,利用就诊路线热力图筛选和识别重点就诊路线,结合实地堪访结果,设计针对性的路线指引标识,并利用半结构式访谈评价改造效果。结果:本研究共改造室外导向标识设施设备点位1117个,室内导向标识设施设备点位38个,增设热点路线指引设施15套。统一了诊区标识的字体字号和颜色,以及科普宣传栏的形象设计。改造前后的问卷访谈结果显示门诊标识系统便捷性评分提升1.46分,准确性评分提升1.87分,可及性评分提升0.86分。结论:从就医服务需求侧入手,开展医院导向标识系统设计改造能够有效发现就医指引和标识设置上的薄弱环节,显著提高患者就医效率和医院就医服务质量。

社会调查访员身份与问卷质量预测

本文从访员身份视角探讨如何构建基于并行数据的质量控制体系和问卷质量预测模型。利用GPS、人脸识别等调查并行数据,本文首先从访问时长、访问负担和访员行为三个维度构建了CULS质量控制体系,并在多元回归框架下探讨了问卷质量的影响因素。问卷质量预测分析结果显示,基于质量控制指标和受访户特征构建的机器学习模型可以较准确地预测学生访员完访问卷质量,但对社区专职工作人员完成问卷质量预测一致率较低。本文进一步探讨了预测模型构建中初始训练集和更新训练集大小对预测一致率的影响,分析结果显示预测一致率随初始训练集和更新训练集样本量的增加而提高,更新训练集可以有效提高调查中期预测效果。本文结论对面访调查的访员遴选、质量控制指标构建、问卷质量预测和质量核查成本控制都具有一定借鉴意义。

我国自提出普通高中特色发展至今已有三十年时间,并提出到2022年,普通高中多样化有特色发展的格局基本形成。然而,这一目标似乎远未达成。为此,尝试基于对专家和校长的深度访谈,揭示普通高中特色发展的现实困境与制约因素。研究发现,目前我国普通高中特色发展面临着理想与现实偏差的特色发展目标达成性困境,表面繁荣与本质偏移的特色发展实施混沌性困境,学生个性支持不足与沦为获取分数的新赛道的特色发展效果低质化困境。普通高中特色发展的“滞缓”主要受制于以下因素:专项政策与顶层规划“缺位”,普通高中特色发展主体素养“缺席”,普通高中特色发展保障体系缺失的“羁绊”,“五唯”导向评价制度“枷锁”,功利主义教育文化氛围“桎梏”。基于此,从明方向、优环境、强动力和增能力四个维度提出了普通高中特色发展的实践路向,即构建“国家统一推进、市级政府统筹实施”的政策体系,建立“全面与个性兼顾、分数与特色兼容”的高中教育质量观,构建“自主招生、招考分离”的招生考试制度,形成办学自主权下放之下“自主增能、他者赋能”的能力提升机制。

以实践为导向构建社区医务人员安宁疗护服务能力评价工具及信效度研究

背景我国安宁疗护服务已进入深度实践时期,许多城市将基层医疗卫生服务机构作为开展安宁疗护的首要场所。社区安宁疗护服务主要为全科团队式服务,但目前尚缺乏切实统一的社区安宁疗护服务人员相关能力考核标准与要求。目的结合社区安宁疗护服务实践,编制社区卫生服务中心医务人员安宁疗护服务能力评价工具,并对其信效度进行研究。方法于2022年6月—2023年3月,通过文献回顾、质性访谈、两轮德尔菲专家咨询进行条目分析,形成安宁疗护服务能力评价工具;于2023-04-01—2023-05-12对上海市浦东新区1281名社区卫生服务中心医务人员开展一般资料、对安宁疗护服务的认知和经历、安宁疗护服务能力评价工具和人文关怀能力量表的问卷调查,检验安宁疗护服务能力评价工具的信度、效度,并采用多元线性回归分析探讨安宁疗护服务能力的影响因素。结果通过文献检索分析和质性访谈形成49个初始条目。德尔菲专家咨询法共邀请上海市15名研究和开展安宁疗护服务的权威专家,其中专家权威系数为0.87,整体指标体系的Kendall和谐系数为0.624(P<0.01),经过两轮德尔菲专家咨询,最终形成具有50个条目的评价工具。问卷调查部分共发放1300份问卷,回收1284份,问卷回收率为98.77%(1284/1300)。安宁疗护服务能力评价工具标准化后的Cronbach'sα系数为0.993,Spearman-Brown折半信度为0.935,总体信度较好。效度检验显示,KMO值为0.987,Bartlett's球形检验值为119119.104,P<0.001,表明该量表效度良好,且适合因子分析。因子分析显示,所有条目的因子载荷系数≥0.4,共同度>0.2,因此保留50个条目。抽取4个公因子,累积方差贡献率为86.517%。相关分析显示,本次构建的安宁疗护服务能力评价工具的总得分和人文关怀量表总分呈显著正相关关系(r=0.819,P<0.001),说明该评价工具具有良好的校标关联效度。多元线性回归分析结果显示,性别、学历、从事社区工作年限、是否经历亲属离世、是否经历安宁疗护培训、安宁疗护服务工作时长、是否愿意从事安宁疗护服务工作是安宁疗护服务能力的影响因素(P<0.05)。结论本研究构建的社区卫生服务中心医务人员安宁疗护服务能力评价工具有较好的科学性和可靠性,信效度较好,可用于评价社区医务人员在安宁疗护服务中的各项能力。

基于O'NET半结构化访谈建构大学生职业适应力故事

职业适应力是大学生转型为职场人士的关键能力,其中职业兴趣是核心要素。然而,当前对大学生职业兴趣的分析主要局限于定量评估,导致学生对自身职业兴趣的理解较为表面,甚至开始怀疑其价值。研究采用O'NET半结构化访谈,结合主题演绎驱动理论,深入分析了大学生在职业适应力故事中的关注、控制、好奇、自信、合作五个维度,旨在探讨半结构化访谈如何定性地将职业兴趣代码与这五个维度的故事关联起来,并揭示其在提升大学生职业适应力方面的作用。研究结果显示,通过半结构化访谈建构的职业适应力故事,成功整合了定量与定性职业兴趣评估,这种方法不仅能帮助大学生更深入地理解自身的霍兰德职业兴趣代码,还能促使他们系统地叙述职业适应力故事,从而有效提升其职业适应力。

倡导全科医学的质性研究

背景 中国的全科医学研究需要从哲学思想和方法学上进一步拓展突破学科发展瓶颈的路径。目的 通过与对标期刊的质性研究发表文章进行比较分析,提出进一步倡导我国全科医学质性研究的策略。方法 对《英国全科医学》杂志发表的研究文章类别、研究主题和研究对象进行简要分析,反思医学界开展质性研究的现状和挑战,提出促进全科医学质性研究的策略。结果 《英国全科医学》杂志的质性研究占研究类型文章的38%(41/107)。全科医学以人为本的宗旨,促使研究者探索患者、居民、医生、护士和相关者的体验、观点、感知、意见,从而筹划和提供具有人性化及响应性和参与性的基本医疗服务。社会科学的方法学专家和对社会学研究方法感兴趣的学术型全科医生,是质性研究在全科医学领域应用的主要推动力量。以实证主义为哲学思想的传统的生物医学模式,是医生开展质性研究时需要逾越的学科篱笆。学术型全科医生要理解本体论和认识论的不同,承认和尝试解释主义的思维方式。与相对容易掌握的质性资料收集技术和编码技术不同的是,在质性研究培训和学习过程中,要特别关注突破思维惰性,以解释主义作为了解世界和全科医学的另一个窗口。结论 质性研究是全科医学学科发展的重要方法,更好质量的质性研究首先取决于丰富的思想,以及为实现思想而操作的工具。
