The impacts of organizational culture and neoliberal ideology on the continued existence of incivility and bullying in healthcare institutions:A discussion paper

Countless research studies have demonstrated the detrimental effects of incivility and bullying in healthcare.Despite the abundance of proposed solutions to this issue,many healthcare leaders continue to fail in mitigating the existence of such negative behaviors in the workplace.Personality attributes of perpetrators and victims have received attention,but much less research has examined the organizational and neoliberal causations of incivility and bullying in healthcare.Being the largest occupational group in the health sector,nursing professionals have the greatest influence and are crucial in ending these behaviors.This discussion paper outlines the effects of incivility and bullying in healthcare and provides a critical analysis on how organizational culture and neoliberal ideology influence the pervasiveness and persistence of these negative behaviors.The analysis reveals that organizational cultures that misuse power,disregard equality,and facilitate oppression,foster the existence of incivility and bullying in the workplace.Such cultures permit perpetrators to misuse their authority to control resource allocation,ignorance to social inequalities,and the silence of victims.Furthermore,the neoliberal concept of deregulation,austerity,and individualism further these behaviors.The neoliberal reforms have led to underfunding of anti-bullying programs and policies,use of bullying behaviours as management strategies,and victim-blaming for profit maximization.Financial cutbacks have resulted in denial and acceptance of uncivil and bullying behaviours in healthcare institutions,which endangers the rights of healthcare providers to a safe workplace environment.To curtail these negative behaviors,robust anti-bullying policies and programs must be strictly enforced and sustained in practice.Further exploration on the association of organizational culture and neoliberal principles to incivility and bullying in healthcare is greatly warranted. 展开更多

Psychometric evaluation of Iranian version of nursing faculty’s incivility questionnaire

Objective:Faculty incivility is a growing problem in nursing schools that disrupts the learning process and affects nursing care of patients.Fostering and managing faculty incivility requires knowing this problem using a valid and reliable instrument.The purpose of this research was to evaluate the validity and reliability of the faculty’s perceived incivility questionnaire in the Iranian community.Methods:In this methodological study,the participants involved were 357 nursing students.They were selected between October and December 2019.The faculty’s perceived incivility questionnaire was evaluated in terms of content and construct validity.McDonald’s omega and Cronbach’s alpha coefficients were calculated.Exploratory and confirmatory factor analyses were conducted to evaluate the construct validity of the questionnaire.Results:The content validity index(CVI)was 0.86-0.91 for the items and 0.87 for the whole instrument.In factor analysis,two factors of violent and irresponsible behaviors were extracted.These factors explained>41.769%of the variance.Model fit indices(Parsimonious Comparative Fit Index[PCFI]=0.783,Parsimonious Normed Fit Index[PNFI]=0.756,root mean square error of approximation[RMSEA]=0.051,Goodness-of-Fit Index[GFI]=0.955,Adjusted Goodness-of-Fit Index[AGFI]=0.932,Comparative Fit Index[CFI]=0.955,χ^(2)/df=2.26)indicated the proportion of factors.Internal consistency was 0.87-0.93.Conclusions:Faculty’s perceived incivility questionnaire is a valid and reliable tool with two factors that can be used to evaluate the incivility in Iranian faculty members. 展开更多

Incivility in the population has become of great interest within the past decade, particularly in the wake of the school massacre in Columbine and the recent movie theatre mass murder in Aurora, Colorado. While citizens struggle to make sense of these violent behaviors, higher education officials are perhaps most vested in exploring the causes, displays, and solutions to uncivil behavior among both faculty and students. The effects of incivility, whether classified as minor disruptions or major violence, may affect the student nurse and impede his or her progress and ability to become an empathic nurse, which is a goal of nursing education. Academic incivility may contribute to bullying in the workplace, which has been identified as a cause of attrition and contributes to the national nursing shortage. This article describes the effects of uncivil behavior on nursing faculty and students and the effect this may have on the nursing workforce. 展开更多
