Background:This study uses a literature review and the Delphi expert consultation method to construct a competency evaluation model for infection control personnel in traditional Chinese medicine(TCM)hospitals.The aim is to strengthen infection control management in TCM hospitals,assess the competency of infection control personnel in their positions,and assist them in identifying the competencies that need improvement.Methods:Based on the literature research method and the Delphi expert consultation method,a competency model for the position was constructed through two rounds of expert consultations,analyzing the relationships between various factors and establishing a hierarchical structure model.Pairwise comparisons were made among the elements at the same level to construct a judgment matrix.Through the analytic hierarchy process,the weight coefficients of the indicators at each level in the competency model were obtained.Results:This study conducted a comprehensive assessment of various capabilities and practices related to hospital infection control.The survey results indicate that participants excelled in multiple areas,with a high overall satisfaction rate.95.28%of participants were able to develop hospital infection monitoring plans based on national infection control policies,demonstrating a good understanding and execution of these policies.94.09%of participants were familiar with high-risk populations and key departments in the hospital.91.73%of participants were able to establish monitoring scopes based on the trends of multidrug-resistant bacteria and conduct information monitoring,reflecting strong response capabilities.92.91%of participants were able to collaborate with relevant departments to conduct bacterial resistance monitoring,showing a good team spirit.94.49%of participants were able to perform targeted monitoring,including surgical site infections,indicating that they have effective monitoring strategies.91.34%of participants were able to collect and organize monitoring data and establish a systematic database,demonstrating good data management skills.90.16%of participants were able to interpret laws and regulations related to hospital infection management,indicating a high level of legal knowledge.89.37%of participants generally possessed good communication skills.92.52%of participants were able to guide medical staff on occupational safety and protective knowledge,showing an emphasis on occupational health.Participants demonstrated a strong desire to learn and innovate,with 87.01%actively participating in continuing education and research activities,reflecting a pursuit of professional development.Conclusion:Based on the results of the two rounds of expert consultations,a competency evaluation model for infection control personnel in TCM hospitals was formed.Through the analytic hierarchy process,the weight coefficients of various indicators at different levels in the model were obtained,and the research results have good scientific validity and reliability. 展开更多

As outposts for surveillance of public health events, hospitals are an important front in the fight against the epidemic. Therefore, it is very crucial to construct infection prevention and control system actively to contain the outbreak. Objective:In 2022,a pneumonia caused by novel coronavirus broke out in Shanghai.A large number of medical personnel were deployed nationwide to support the Fangcang Shelter Hospital of National Exhibition and Convention Center(Shanghai).Because the of the underdeveloped infrastructure and the disorganized layout process,the phenomenon of infection among medical personnel was rampant given the highly contagious nature of the Omicron variant.This paper discusses the effect of 6S management in infection control management of Fangcang Shelter Hospital of National Exhibition and Convention Center(Shanghai).Methods:A series of 6S management practices were carried out based on the evaluation of the control management of Fangcang Shelter Hospital of National Exhibition and Convention Center(Shanghai)during the anti-epidemic period,and the effect of the implementation of 6S management was studied based on the rapport among team members and the overall control system process,the management of wearing and stripping channels,the time of wearing protective articles and the psychological and emotional changes of the team members.Results:After the implementation of 6S management mode,the management of dressing room,epidemic prevention materials,and occupational protection awareness and infection control safety were significantly improved.More than 95.5%of the team members believed that improving the working environment in the shelter increased the confidence of victory in the fight against the epidemic,reduced the preparations before entering and leaving the cabin,improved the work efficiency and ensured occupational safety.It ensures the realization of the goal of“zero infection”and is set as a benchmark of control work of Fangcang Shelter Hospital of National Exhibition and Convention Center(Shanghai).Conclusion:The application of 6S management mode to the shelter infection control management can create and maintain a good working environment in the shelter,promote the implementation effect of various systems and processes of infection control,reduce the potential infection risk in the shelter and improve the efficiency of infection control management. 展开更多

<strong>Objective:</strong> We assess the application effect of the “3 + 1” mode in the COVID-19 epidemic prevention and control at the infection ward of a designated comprehensive hospital for COVID-19 ... <strong>Objective:</strong> We assess the application effect of the “3 + 1” mode in the COVID-19 epidemic prevention and control at the infection ward of a designated comprehensive hospital for COVID-19 treatment.<strong> Method: </strong>Based on the features of the inpatients of the infection ward and their relatives, a “3 + 1” mode for the COVID-19 prevention and control is developed to conform to the demands for epidemic prevention and control and the overall prevention and control scheme of the whole hospital. Here, “3” stands for the epidemic prevention and control system, personnel management, and prevention and control measures;“1” stands for COVID-19 testing.<strong> Result: </strong>From March 1, 2020 to March 31, 2021, a total of 3056 patients were hospitalized in the three infection wards. Among them, 265 patients had a fever, and 113 patients had respiratory symptoms. None of them were infected with COVID-19. The participation rate of the test about the knowledge related to COVID-19 and the knowledge mastery rate were both 100% among the medical staff. None of the inpatients and their companions or the medical staff was diagnosed with COVID-19.<strong> Conclusion: </strong>Thus the “3 + 1” mode proves successful for avoiding nosocomial infection and the spread of the epidemic. 展开更多

Background: Novel coronavirus pneumonia (NCP) and osteoporotic vertebral compression fractures (OVCF) are the high incidences of diseases in the elderly. During the epidemic period, if not treated in time, the complications are high and the mortality is high. If we do not pay attention to infection prevention and control in pre-hospital emergency care, it will lead to the first time infection of medical staff and in-hospital cross infection in emergency outpatient receiving area. The correct consideration of both and the establishment of perfect pre-hospital emergency treatment and infection prevention and control synchronous strategy is an important premise to ensure the stable, orderly and safe medical treatment. Objective: To explore the effect of synchronous implementation of pre-hospital emergency care, nursing and infection pre-vention and control for senile OVCF during the epidemic. In order to improve the efficiency of pre-hospital emergency care and prevent the spread of infection. Method: A total of 92 elderly patients with OVCF who received pre-hospital treatment in 18 hospitals in Zhangjiakou City during the epidemic prevention and control period from January 2020 to November 2022 and met the inclusion criteria were selected as research objects, including 24 males and 68 females, aged 65 - 82 (74.2 ± 2.2) years. All patients were associated with concomitant injuries and underlying diseases. All patients in this group underwent predictive pre-hospital rescue and infection prevention and control procedures. Results: All the 92 elderly patients with OVCF received timely pre-hospital treatment during the epidemic period, and no aggravation occurred of the 92 patients, 35 were in the high risk area, 10 were in the medium risk area, and 47 were in the low risk area. Exclude OVCF for NCP Patients were treated according to the conventional diagnosis and treatment principles. Suspected and confirmed cases are transferred to designated surgical hospitals for treatment. All patients were followed up 3 months, 6 months and 12 months after treatment. There was no death rate, high satisfaction of pre-hospital first aid, high diagnostic accuracy, and good curative effect. None of the rescue personnel had any infection rate, and no hospital infection transmission and nosocomial cross infection occurred. Conclusion: It is the first step to safely treat patients and prevent cross infection to establish a perfect synchronous strategy of pre-hospital first aid and infection prevention and control. 展开更多

Nosocomial infections or healthcare associated infections occur in patients under medical care.These infections occur worldwide both in developed and developing countries.Nosocomial infections accounts for 7% in developed and 10% in developing countries.As these infections occur during hospital stay,they cause prolonged stay,disability,and economic burden.Frequently prevalent infections include central line-associated bloodstream infections,catheter-associated urinary tract infections,surgical site infections and ventilator-associated pneumonia.Nosocomial pathogens include bacteria,viruses and fungal parasites.According to WHO estimates,approximately 15% of all hospitalized patients suffer from these infections.During hospitalization,patient is exposed to pathogens through different sources environment,healthcare staff,and other infected patients.Transmission of these infections should be restricted for prevention.Hospital waste serves as potential source of pathogens and about 20%–25% of hospital waste is termed as hazardous.Nosocomial infections can be controlled by practicing infection control programs,keep check on antimicrobial use and its resistance,adopting antibiotic control policy.Efficient surveillance system can play its part at national and international level.Efforts are required by all stakeholders to prevent and control nosocomial infections. 展开更多

Hospital-acquired infections(HAIs) are serious problems for healthcare systems, especially in developing countries where public health infrastructure and technology for infection preventions remain undeveloped. Here, we characterized how strategy and technology could be mobilized to improve the effectiveness of infection prevention and control in hospitals during the outbreaks of Ebola, Middle East respiratory syndrome(MERS), and severe acute respiratory syndrome(SARS) in Asia and West Africa. Published literature on the hospital-borne outbreaks of SARS, Ebola, and MERS in Asia and West Africa was comprehensively reviewed. The results showed that healthcare systems and hospital management in affected healthcare facilities had poor strategies and inadequate technologies and human resources for the prevention and control of HAIs, which led to increased morbidity, mortality, and unnecessary costs. We recommend that governments worldwide enforce disaster risk management, even when no outbreaks are imminent. Quarantine and ventilation functions should be taken into consideration in architectural design of hospitals and healthcare facilities. We also recommend that health authorities invest in training healthcare workers for disease outbreak response, as their preparedness is essential to reducing disaster risk. 展开更多

Nosocomial infections are also known as hospital-acquired/associated infections. National Healthcare Safety Network along with Centers for Disease Control for surveillance has classified nosocomial infection sites into 13 types with 50 infection sites, which are specific on the basis of biological and clinical criteria. The agents that are usually involved in hospitalacquired infections include Streptococcus spp., Acinetobacter spp., enterococci, Pseudomonas aeruginosa, coagulase-negative staphylococci, Staphylococcus aureus, Bacillus cereus, Legionella and Enterobacteriaceae family members, namely, Proteus mirablis, Klebsiella pneumonia, Escherichia coli, Serratia marcescens. Nosocomial pathogens can be transmitted through person to person, environment or contaminated water and food, infected individuals, contaminated healthcare personnel's skin or contact via shared items and surfaces. Mainly, multi-drug-resistant nosocomial organisms include methicillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, Pseudomonas aeruginosa and Klebsiella pneumonia, whereas Clostridium difficile shows natural resistance. Excessive and improper use of broadspectrum antibiotics, especially in healthcare settings, is elevating nosocomial infections, which not only becomes a big health care problem but also causes great economic and production loss in the community. Nosocomial infections can be controlled by measuring and comparing the infection rates within healthcare settings and sticking to the best healthcare practices. Centers for Disease Control and Prevention provides the methodology for surveillance of nosocomial infections along with investigation of major outbreaks. The absence of infection control policies, guidelines and trained professionals also contributes to the magnitude of the problem. The aim of this study was to assess the Knowledge, attitude and practices towards infection control measures among healthcare workers at Old Mutare Hospital. <strong>Methods:</strong> A cross-sectional survey design was used to acquire information. A purposive sampling method was used to select 22 nurses, 15 nurse Aids and 2 laboratory technicians (Lab Tech). A self-administered questionnaire with four components addressing demographic data, knowledge, attitudes and practices was used to collect data that was completed by a checklist. <strong>Results:</strong> The study findings revealed a poor knowledge of infection control measures among the nurse’s aide. The attitude and practices among participants were impartially good in all variables. The participants had scored over 50% towards their attitude and their practices on infection control. A significant statistical difference was found between the professional status of the participants and the ability to explain how one can get Hospital Acquired Infections (HAIs) and awareness of infection control programs in their hospital (r<sub>s</sub> = 0.640, 0.645) with P < 0.05 (0.01, 0.01) respectively. <strong>Conclusion:</strong> Despite of having a fair attitude and practice towards infection control, the participants had presented poor knowledge;therefore, it had been recommended that in service training and workshop should be planned by the administration to update their knowledge and attain full compliance towards their practice. 展开更多

目的探讨“计划-实施-检查-处理”(plan-docheck-act,PDCA)管理循环方式对医务人员手卫生依从性及院感发生率的影响。方法随机选取茂名市人民医院50名医务人员实施PDCA管理循环。比较实施前(2021年1—12月,对照组)、实施后(2022年1—12... 目的探讨“计划-实施-检查-处理”(plan-docheck-act,PDCA)管理循环方式对医务人员手卫生依从性及院感发生率的影响。方法随机选取茂名市人民医院50名医务人员实施PDCA管理循环。比较实施前(2021年1—12月,对照组)、实施后(2022年1—12月,观察组)医务人员的手卫生依从性、手卫生知识考试成绩、合格率、院感发生率。结果观察组在手卫生依从性方面表现卓越,接触患者前后、周围环境后、无菌操作前、体液暴露后的洗手率分别为88.89%、93.06%、61.67%、83.89%、96.11%,显著高于对照组的76.94%、80.33%、44.44%、65.00%、88.89%(P<0.05)。观察组手卫生知识考试得分和合格率分别为(91.28±12.15)分和96.00%,高于对照组的(82.37±10.23)分和76.00%(P<0.05)。观察组的感染例次率(1.39%)明显低于对照组(6.39%)(P<0.05)。结论PDCA管理循环方式可提高医务人员手卫生依从性,同时可提升手卫生知识水平,降低院感发生率,为提升医疗安全水平提供了有效策略。 展开更多

The adoption by health professionals to the practice of patient isolation is a decision that reduces patients’ risks of occupational exposure to potentially contaminated biological material and prevents nosocomial infections. Verify the compliance of health professionals to the practices of contact precautions in patients colonized by microorganisms in a teaching hospital. This was a prospective observational cohort study in a special-sized hospital located in southeastern Brazil. The observation of health professionals in delivery of health care to patients with contact precaution was performed regarding hand hygiene, use of overcoat, glove, and mask. We carried out 1502 observations involving the following professional categories: nursing technicians and/or nursing assistants (n = 1028;68.4%), nurses (n = 200;13.3%), physical therapists (n = 185;12.3%), and physicians (n = 89;5.9%), totaling 971.8 hours. Regarding the level of compliance of professionals to contact precaution practices with hand hygiene after the procedures, the use of overcoats, gloves, and masks, all were statistically significant (p Klebsiella pneumoniae was the most positive pathogen identified in laboratory tests. This study provided the mapping of the compliance of health professionals to the practices of contact precautions in order to support a safer management of patient care reducing the risks of Healthcare-Associated Infections (HAI). 基于CiteSpace对国内外医院感染护理相关研究的可视化分析

目的 医院感染问题关系到医院的医疗质量与医患安全,本文探讨国内外医院感染护理相关研究的发展现状、研究热点及趋势,为开展今后研究提供参考依据。方法 计算机检索2010年1月1日—2022年12月31日有关医院感染护理的相关文献,中文数据... 目的 医院感染问题关系到医院的医疗质量与医患安全,本文探讨国内外医院感染护理相关研究的发展现状、研究热点及趋势,为开展今后研究提供参考依据。方法 计算机检索2010年1月1日—2022年12月31日有关医院感染护理的相关文献,中文数据库包括中国知网(CNKI)、中国生物医学文献数据库(CBM)、万方数据库、维普数据库,英文检索Web of Science(WOS)数据库,运用CiteSpace软件对年度发文量、作者合作、机构合作、关键词、突现词等特征进行可视化分析。结果 最终纳入2508篇文献,其中中文文献942篇,英文文献1566篇。国内医院感染护理相关文献的发文量总体呈下降趋势,国外研究发文量与国内相反;国内高频关键词主要有危险因素、护理干预、手卫生等,国外高频关键词除ICU外其余一致;可视化分析显示英文文献作者间合作和研究机构间合作较中文文献多,国内核心作者6位,国外核心作者12位;中文的发文机构主要为华中科技大学附属医院,英文为哥伦比亚大学。结论 有效控制医院感染情况、强化感染质量管理、评估识别发现的医院感染风险点、针对性采取风险干预措施并持续评估跟踪、聚焦于护理人员医院感染防控的研究对保障患者安全和提升护理质量具有重要意义,今后研究应针对护理相关高危因素采取基于循证的措施降低医院感染发生率。 展开更多

风险评估联合PDCA循环法在多重耐药菌感染防控中的应用

目的分析风险评估联合计划-执行-检查-处理(plan-do-check-act,PDCA)循环法在多重耐药菌感染(multiple drug-resistant organism,MDRO)防控中的应用效果。方法选取2021年7月—2023年6月菏泽市中医医院重症加强护理病房(intensive care u... 目的分析风险评估联合计划-执行-检查-处理(plan-do-check-act,PDCA)循环法在多重耐药菌感染(multiple drug-resistant organism,MDRO)防控中的应用效果。方法选取2021年7月—2023年6月菏泽市中医医院重症加强护理病房(intensive care unit,ICU)收治的患者360例,按照干预时间将2021年7月—2022年6月的180例患者作为对照组,2022年7月—2023年6月的180例患者作为研究组。对照组采取常规管理措施防控MDRO,研究组在其基础上采用风险评估联合PDCA循环法进行MDRO防控。比较2组MDRO防控措施的执行情况与MDRO的检出率。结果研究组的危急值报告、规范隔离、开具隔离医嘱、诊疗用品专用、诊疗时个人防护、手卫生、终末消毒的执行情况评分均高于对照组,差异有统计学意义(P<0.05)。研究组MDRO检出率为5.56%,低于对照组的12.22%(P<0.05)。结论风险评估与PDCA循环法联合应用可以进一步提升MDRO防控工作的执行情况,降低MDRO发生风险。 展开更多

目的:医院感染在增加患者痛苦和病死率的同时,也造成了巨大的经济损失。采取有效的措施改善医院感染情况,做好医务人员手部清洁、消毒刻不容缓。SMART原则即在目标设置时遵循明确性、可衡量性、可接受性、相关性、时限性原则。本研究在... 目的:医院感染在增加患者痛苦和病死率的同时,也造成了巨大的经济损失。采取有效的措施改善医院感染情况,做好医务人员手部清洁、消毒刻不容缓。SMART原则即在目标设置时遵循明确性、可衡量性、可接受性、相关性、时限性原则。本研究在医院感染预防与控制工作中结合SMART原则,围绕手卫生等医院感染问题采取一系列措施,并对实施效果进行评价。方法:本研究为回顾性研究。2020年1至6月,在中南大学湘雅医院国际医疗部运用SMART原则开展了感染预防与控制干预工作,实施感染预防与控制质量改进计划。以指标数据作为目标衡量标准,建立以医院感染预防与控制知识知晓度、手卫生依从率为过程监测,手术部位感染率、导尿管相关尿路感染率为结果监测的评价指标。将干预后(2020年7月至2021年6月)各项评价指标与干预前(2019年1至12月)进行比较。结果:纳入在国际医疗部工作的51名医务人员,其中医师12名,护士39名;男性6名,女性45名;年龄21~57(30.2±7.1)岁。干预后医务人员医院感染预防与控制知识知晓度、手卫生依从率较干预前均明显提升(均P<0.05),但手术部位感染率、导尿管相关尿路感染率干预前后差异均无统计学意义(均P>0.05)。干预后内、外科病房医务人员医院感染预防与控制知识知晓度、手卫生依从率均较干预前有所提高(均P<0.05),外科病房医务人员医院感染预防与控制知识知晓度较低但改进明显,内科病房医务人员手卫生依从率较高且改进明显;干预后男性和女性医务人员医院感染预防与控制知识知晓度、手卫生依从率均较干预前有所提高(均P<0.05),均为男性较女性提升明显;干预后年龄<30岁、30~40岁的医务人员医院感染预防与控制知识知晓度、手卫生依从率均较干预前有所提高(均P<0.05),年龄30~40岁的医务人员医院感染预防与控制知识知晓度较低但2项指标改进均最明显;干预后医生、护士的医院感染预防与控制知识知晓度均较干预前有所提高(均P<0.05),且医生知识知晓度改进较为明显,护士手卫生依从率较干预前有所提高(P<0.05);干预后初级、中级职称医务人员医院感染预防与控制知识知晓度、手卫生依从率均较干预前有所提高(均P<0.05),均为中级职称医务人员改进较为明显。结论:基于SMART原则,结合多种国内外感染预防与控制相关评价标准,实施质量改进计划,可有效提升医务人员感染预防和控制知识知晓度及手卫生依从率,强化医院感染的预防与控制,保障患者的安全。 展开更多
