BACKGROUND The incidence of cholecystolithiasis is on the rise.Use of information,motivation,and behavioral skills can play a positive role in promoting changes in individual health behaviors.However,reports on the effects of information-motivationbehavioral(IMB)skills model based high-quality nursing as a perioperative nursing intervention for patients with gallstones are nonexistent.AIM To explore the application of IMB skills model based high-quality nursing in patients with gallstones.METHODS Two hundred and sixteen patients with cholecystolithiasis treated at our hospital from January 2022 to January 2023 were enrolled and divided into a control,highquality,and combined nursing groups,with 72 patients in each group.The control,high-quality,and combination groups received conventional,high-quality,and IMB skills model based perioperative nursing services,respectively.Differences in clinical indicators,stress levels,degree of pain,emotional state,and quality of life were observed,and complications and nursing satisfaction among the three groups were evaluated.RESULTS After nursing,the time to recovery of gastrointestinal function in the high-quality and combined nursing groups was significantly shorter than that of the control group,with the recovery of gastrointestinal function being the fastest in the combined nursing group(P<0.05).After nursing intervention,cortisol and norepinephrine levels in the high-quality and combined nursing groups were closer to normal than those of the control group 24 h after surgery,with the combined nursing group having the closest to normal levels(P<0.05).After 3 and 7 d of intervention,the patients’pain significantly improved,which was more prominent in the highquality and combination groups.Meanwhile,the pain score in the combination group was significantly lower than those of the control and high-quality nursing groups(P<0.05).After nursing intervention,the emotional states of all patients improved,and the scores of patients in the combination group were significantly lower than those of the control and high-quality nursing groups.The quality of life of patients in the high-quality and combined nursing groups significantly improved after nursing intervention compared to that of the control group,with the combined nursing group having the highest quality of life score.After intervention,the incidence of complications in the high-quality and combination groups was significantly lower than that of the control group(P<0.05),but the difference between the combination and high-quality nursing groups was not significant.Nursing satisfaction of patients in the high-quality and combination groups was significantly higher than that of the control group,with the nursing satisfaction being the highest in the combination group(P<0.05).CONCLUSION IMB skills model based nursing can improve surgical stress levels,degrees of pain,emotional state,quality of life,and nursing satisfaction of patients with gallstones and reduce the incidence of complications. 展开更多

Objective:To explore and analyze the work process-based practical training teaching model for basic nursing skills in vocational colleges and its implementation effects.Methods:A total of 82 nursing students from our school were selected for the study,which was conducted from April 2023 to April 2024.Using a random number table method,the students were divided into an observation group and a control group,each with 41 students.The control group received conventional practical training teaching,while the observation group followed the work process-based practical training model for basic nursing skills.The assessment scores and teaching satisfaction of the two groups were compared.Results:The comparison of assessment scores showed that the observation group performed significantly better than the control group(P<0.05).The comparison of teaching satisfaction also indicated that the observation group had significantly higher satisfaction than the control group(P<0.05).Conclusion:The work process-based practical training teaching model for basic nursing skills in vocational colleges can improve students’assessment scores and enhance teaching satisfaction,demonstrating its value for wider application. 展开更多

BACKGROUND Vestibular dysfunction(VH)is a common concomitant symptom of late peri-pheral vestibular lesions,which can be trauma,poisoning,infection,heredity,and neurodegeneration,but about 50%of the causes are unknown.The study uses the information-motivation-behavioral skills(IMB)model for health education,effectively improve the quality of life,increase their self-confidence,reduce anxiety and depression,and effectively improve the psychological state of patients.AIM To explore the effect of health education based on the IMB model on the degree of vertigo,disability,anxiety and depression in patients with unilateral vestibular hypofunction.METHODS The clinical data of 80 patients with unilateral vestibular hypofunction from January 2019 to December 2021 were selected as the retrospective research objects,and they were divided into the control group and the observation group with 40 cases in each group according to different nursing methods.Among them,the control group was given routine nursing health education and guidance,and the observation group was given health education and guidance based on the IMB model.The changes in self-efficacy,anxiety and depression,and quality of life of patients with unilateral VH were compared between the two groups.RESULTS There was no significant difference in General Self-Efficacy Scale(GSES)scale scores between the two groups of patients before nursing(P>0.05),which was comparable;after nursing,the GSES scale scores of the two groups were higher than those before nursing.The nursing group was higher than the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the scores of Hospital Anxiety and Depression Scale(HADS)and anxiety and depression subscales between the two groups before nursing(P>0.05).After nursing,the HADS score,anxiety,and depression subscale scores of the two groups of patients were lower than those before nursing,and the nursing group was lower than the control group,and the difference was statistically significant(P<0.05).After nursing,the Dizziness Handicap Inventory(DHI)scale and DHI-P,DHI-E and DHI-F scores in the two groups were decreased,and the scores in the nursing group were lower than those in the control group,and the difference was statistically significant(P<0.05).CONCLUSION Health education based on the IMB model can effectively improve patients'quality of life,increase self-efficacy of patients with unilateral vestibular hypofunction,enhance patients'confidence,enable patients to resume normal work and life as soon as possible,reduce patients'anxiety and depression,and effectively improve patients'psychological status. 展开更多

Application of a hospital–community–family trinity rehabilitation nursing model combined with motor imagery therapy in patients with cerebral infarction 被引量:7

BACKGROUND Rehabilitation nursing is considered an indispensable part of the cerebral infarction treatment system.The hospital–community–family trinity rehabilitation nursing model can provide continuous nursing services across hospitals,communities,and families for patients.AIM To explore the application of a hospital–community–family rehabilitation nursing model combined with motor imagery therapy in patients with cerebral infarction.METHODS From January 2021 to December 2021,88 patients with cerebral infarction were divided into a study(n=44)and a control(n=44)group using a simple random number table.The control group received routine nursing and motor imagery therapy.The study group was given hospital–community–family trinity rehabilitation nursing based on the control group.Motor function(FMA),balance ability(BBS),activities of daily living(BI),quality of life(SS-QOL),activation status of the contralateral primary sensorimotor cortical area to the affected side,and nursing satisfaction were evaluated before and after intervention in both groups.RESULTS Before intervention,FMA and BBS were similar(P>0.05).After 6 months’intervention,FMA and BBS were significantly higher in the study than in the control group(both P<0.05).Before intervention,BI and SS-QOL scores were not different between the study and control group(P>0.05).However,after 6months’intervention,BI and SS-QOL were higher in the study than in the control group(P<0.05).Before intervention,activation frequency and volume were similar between the study and the control group(P>0.05).After 6 months’intervention,the activation frequency and volume were higher in the study than in the control group(P<0.05).The reliability,empathy,reactivity,assurance,and tangibles scores for quality of nursing service were higher in the study than in the control group(P<0.05).CONCLUSION Combining a hospital–community–family trinity rehabilitation nursing model and motor imagery therapy enhances the motor function and balance ability of patients with cerebral infarction,improving their quality of life. 展开更多

Background: The self-consciousness and practicality of preferentially prescribed essential medicines (EMs) are not high enough in county hospitals. The purposes of this study were to use the information-motivation-behavioral skills (IM B) model to identify the predictors of essential medicines prescribing behavior (EMPB) among doctors and to examine the association between demographic variables, IMB, and EMPB. Methods: A cross-sectional study was carried out to assess predictive relationships among demographic variables and IM B model variables using an anonymous questionnaire administered in nine county hospitals of Anhui province. A structural equation model was constructed for the I MB model to test the instruments using analysis of moment structures 17.0. Results: A total of 732 participants completed the survey. The average age of the participants was 37.7 ± 8.9 years old (range: 22-67 years old). The correct rate of information was 90.64%. The average scores of the motivation and behavioral skills were 45.46 a: 7.34 (hundred mark system: 75.77) and 19.92 ± 3.44 (hundred mark system: 79.68), respectively. Approximately half(50.8%) of respondents reported that the proportion of EM prescription was below 60%. The final revised model indicated a good fit to the data (x^2/df= 4.146, goodness of fit index = 0.948, comparative fit index = 0.938, root mean square error of approximation = 0.066). More work experience (β = 0.153, P 〈 0.001 ) and behavioral skills (β = 0.449, P 〈 0.001 ) predicted more EMPB. Higher income predicted less information (β = -0.197, P 〈 0.001) and motivation (β =0.204, P 〈 0.001 ). Behavioral skills were positively predicted by information (β = 0.135, P 〈 0.001 ) and motivation (β = 0.742, P 〈 0.001 ). Conclusion: The present study predicted some factors of EMPB, and specified the relationships among the model variables. The utilization rate of EM was not high enough. Motivation and behavior skills were crucial factors affecting EMPB. The influence of demographic variables, such as income and work experience, on EMPB should be fully appreciated. Comprehensive intervention measures should be implemented from multiple perspectives. 展开更多

Adherence in CABG Patients: An Empirical Test of a Health Behavior Model

To benefit from a Coronary Artery Bypass Graft Surgery (CABG), patients must adhere to their therapeutic regimen. To test the extent of which the Information-Motivation-Behavioral Skills (IMB) model constructs explain the variability in adherence behaviors in CABG patients, and also to examine the relationship between the IMB model constructs, CABG patients (N = 152) were randomly assigned to either an intervention group, or a standard care control group. They completed pretest measures, and were reassessed later. Although perfect replication of the IMB model’s predicted pathways was not achieved in this study, support for several of these pathways was demonstrated. The estimated model for adherence-demonstrated good fit of the data. Motivation was a significant predictor of adherence behavior in CABG patients. These findings suggest that IMB model-based intervention that improves motivation can enhance adherence. Therefore, CABG patients’ education programs should mainly target motivation in order to affect adherence behavior. 展开更多

TTM与OSCE考核对新入职产科护士实践技能培养的研究

目的探讨行为阶段转变理论模型(the transtheoretical model and stages of change,TTM)与客观结构化临床考核(objective structured clinical examination,OSCE)模式对于新入职产科护士实践技能培训的指导价值。方法选取2020年1月—202... 目的探讨行为阶段转变理论模型(the transtheoretical model and stages of change,TTM)与客观结构化临床考核(objective structured clinical examination,OSCE)模式对于新入职产科护士实践技能培训的指导价值。方法选取2020年1月—2021年3月南京医科大学附属妇产医院接收的新入职产科护士78名作为对照组,选取2021年4月—2022年9月南京医科大学附属妇产医院接收的新入职产科护士78名为作观察组。对照组采取常规的集中培训方法,观察组采取TTM与OSCE考核培训方法。对比2组新入职产科护士的实践技能考核评分情况、经培训后综合能力以及满意度情况。结果观察组培训后的实践技能考核得分高于对照组[(93.33±2.42)分vs.(88.52±2.71)分],差异有统计学意义(P<0.001);观察组产科护士经培训后在护理评估、临床思维、沟通技能、人文关怀、整体评价各个方面综合能力得分高于对照组,差异有统计学意义(P<0.001);观察组产科护士总满意率(94.87%)高于对照组(83.33%),差异有统计学意义(P<0.05)。结论针对新入职产科护士,实施TTM与OSCE考核培训方式可以提高护士实践技能及综合能力,并提高满意度。 展开更多

基于IMB模型的渐进性分期康复护理在腰椎间盘突出症术后的应用

目的研究基于信息-动机-行为技巧(IMB)模型的渐进性分期康复护理在腰椎间盘突出症术后的应用效果。方法选择2021年2月至2023年11月郑州大学第一附属医院收治的156例腰椎间盘突出症患者作为研究对象,按随机数表法分为对照组和研究组各78... 目的研究基于信息-动机-行为技巧(IMB)模型的渐进性分期康复护理在腰椎间盘突出症术后的应用效果。方法选择2021年2月至2023年11月郑州大学第一附属医院收治的156例腰椎间盘突出症患者作为研究对象,按随机数表法分为对照组和研究组各78例,对照组患者予以常规护理,研究组患者予以基于IMB模型的渐进性康复护理,两组患者均护理至出院,并随访3个月。随访3个月后回院复查时,比较两组患者护理干预前后的腰椎功能评分、视觉模拟评分(VAS)评分、焦虑自评量表(SAS)评分和遵医行为评分,同时比较两组患者术后切口感染、神经根水肿和下肢静脉血栓的发生情况。结果干预前,两组患者的腰椎功能评分比较差异无统计学意义(P>0.05);随访3个月后,两组患者的日本骨科学会(JOA)评分均高于干预前,Oswestry功能障碍指数(ODI)评分均低于干预前,且研究组患者的JOA评分为(25.19±1.01)分,明显高于对照组的(21.74±2.02)分,ODI评分为(6.98±1.15)分,明显低于对照组的(9.27±1.71)分,差异均有统计学意义(P<0.05);干预前,两组患者的VAS评分、SAS评分和遵医行为评分比较差异均无统计学意义(P>0.05);随访3个月后,两组患者的VAS评分、SAS评分均低于干预前,遵医行为评分均高于干预前,且研究组患者VAS评分和SAS评分分别为(1.42±0.45)分、(40.33±5.19)分,明显低于对照组的(2.01±0.53)分、(45.37±4.81)分,遵医行为评分为(66.33±9.54)分,明显高于对照组的(57.07±8.12)分,差异均有统计学意义(P<0.05);研究组患者的并发症总发生率为2.56%,明显低于对照组的11.54%,差异有统计学意义(P<0.05)。结论基于IMB模型的渐进性分期康复护理可提高LDH术后患者的康复依从性,促进患者腰椎功能恢复,改善患者心理状态和减轻术后疼痛,且术后并发症发生率低。 展开更多

基于信息—动机—行为技巧模型的链式管理对促进早产儿母乳喂养的效果评价

目的评价基于信息-动机-行为技巧模型(information motivation behavioral skills model,IMB)的链式管理对促进早产儿母乳喂养的干预效果。方法选取2022年1月—6月入住湖南省某三级甲等医院新生儿重症监护室符合纳入标准的88例早产儿母... 目的评价基于信息-动机-行为技巧模型(information motivation behavioral skills model,IMB)的链式管理对促进早产儿母乳喂养的干预效果。方法选取2022年1月—6月入住湖南省某三级甲等医院新生儿重症监护室符合纳入标准的88例早产儿母亲为研究对象,2022年1月—3月的44例为对照组,2022年4月—6月的44例为试验组,对照组接受常规母乳喂养支持护理,试验组接受基于IMB的链式管理。比较两组母亲母乳喂养知识、婴儿喂养态度、母乳喂养自我效能得分和6个月内早产儿纯母乳喂养率、再入院率等的差异。结果干预后,试验组母乳喂养知识得分高于对照组(t=-4.747,P<0.001)、婴儿喂养态度得分高于对照组(t=-17.321,P<0.001)、母乳喂养自我效能得分高于对照组(t=-2.352,P=0.021);试验组早产儿矫正月龄1个月内纯母乳喂养率(95.2%)高于对照组(73.8%),差异有统计学意义(χ^(2)=7.372,P=0.007),3个月内纯母乳喂养率(88.1%)高于对照组(69.0%),差异有统计学意义(χ^(2)=4.525,P=0.033),6个月内纯母乳喂养率(66.7%)高于对照组(40.5%),差异有统计学意义(χ^(2)=5.791,P=0.016);试验组早产儿6个月内再入院率(2.3%)低于对照组(16.7%)(χ^(2)=4.974,P=0.030)。结论IMB结合链式管理可提高早产儿矫正月龄6个月内纯母乳喂养率,降低再入院率。 展开更多

目的 探究信息-动机-行为技巧(IMB)模式下集束化干预方案在脊柱骨折合并脊髓损伤患者中的应用效果。方法 选取2020年5月至2023年5月江苏省淮安市第一人民医院118例脊柱骨折合并脊髓损伤患者作为研究对象,按信封抽签法分为常规组和IMB组,... 目的 探究信息-动机-行为技巧(IMB)模式下集束化干预方案在脊柱骨折合并脊髓损伤患者中的应用效果。方法 选取2020年5月至2023年5月江苏省淮安市第一人民医院118例脊柱骨折合并脊髓损伤患者作为研究对象,按信封抽签法分为常规组和IMB组,各59例。常规组实施常规护理干预,IMB组在常规护理的基础上实施IMB模式下集束化干预。比较两组干预前和干预1个月后的干预效果、生活质量、心理弹性、创伤后成长及并发症情况。结果 干预前,两组干预效果、生活质量、心理弹性、创伤后成长比较,差异无统计学意义(P>0.05);干预后,两组触觉评分、运动评分、椎体相对高度高于干预前,且IMB组高于常规组,差异有统计学意义(P<0.05);两组Cobb’s角均小于干预前,且IMB组小于常规组,差异有统计学意义(P<0.05);两组生活质量评分高于干预前,且IMB组高于常规组,差异有统计学意义(P<0.05);两组心理弹性量表各项评分均高于干预前,且IMB组高于常规组,差异有统计学意义(P<0.05);两组创伤后成长量表各项评分均高于干预前,且IMB组高于常规组,差异有统计学意义(P<0.05);IMB组并发症总发生率低于常规组,差异有统计学意义(P<0.05)。结论 IMB模式下集束化干预方案在脊柱骨折合并脊髓损伤患者中的应用效果较为显著。 展开更多

目的:探讨基于信息-动机-行为技巧(IMB)模式的术后早期康复护理对胫骨平台骨折术后康复效果的影响。方法:选取173例单侧胫骨平台骨折患者为研究对象,依据术后康复护理方案不同分为对照组(n=82)和观察组(n=91)。对照组患者予以常规康复护... 目的:探讨基于信息-动机-行为技巧(IMB)模式的术后早期康复护理对胫骨平台骨折术后康复效果的影响。方法:选取173例单侧胫骨平台骨折患者为研究对象,依据术后康复护理方案不同分为对照组(n=82)和观察组(n=91)。对照组患者予以常规康复护理;观察组患者予以基于IMB模式的早期康复护理,干预时间均为3个月。比较两组患者干预前后康复锻炼知信行水平[康复锻炼知信行水平量表评分];干预前、干预1个月及3个月后膝关节功能情况[Lysholm膝关节量表评分];住院时间、骨折愈合时间、完全负重时间及干预期间并发症发生情况。结果:干预后,两组患者康复锻炼知信行水平量表总分及各维度评分均升高(P<0.05),且观察组高于对照组(P<0.05);Lysholm膝关节量表评分均随护理时间推移而升高(P<0.05),且观察组各时间点均高于对照组(P<0.05)。干预3个月后,观察组患者膝关节优良率高于对照组(P<0.05)。观察组患者住院时间、骨折愈合时间及完全负重时间短于对照组(P<0.05);术后并发症总发生率低于对照组(P<0.05)。结论:基于IMB模式的术后早期康复护理对胫骨平台骨折患者治疗效果优异,可有效提高患者康复锻炼知信行水平,减轻患者术后疼痛,促进膝关节功能恢复,且可降低术后并发症发生率。 展开更多

目的 构建以提升叙事护理能力为主导的临床护士人文关怀培训模式,并探讨其在临床培训中的应用效果。方法 2022年12月随机选取无锡市某三甲医院6个病区,进行护士整群抽样,根据纳排标准,最终纳入60名临床护士作为研究对象,采用叙事护理基... 目的 构建以提升叙事护理能力为主导的临床护士人文关怀培训模式,并探讨其在临床培训中的应用效果。方法 2022年12月随机选取无锡市某三甲医院6个病区,进行护士整群抽样,根据纳排标准,最终纳入60名临床护士作为研究对象,采用叙事护理基础理论课程学习、叙事故事共情、应用临床实践、参与情境体验、书写反思病历、参与人文素材开发创作相结合的沉浸体验式培训方式进行为期6个月的叙事护理培训。比较培训前后护士叙事护理知识—态度—行为得分、医学叙事能力、共情能力、人文关怀能力和患者对护士满意度,同时调查培训后护士对培训方案的满意度。结果 培训后护士的叙事护理知信行各维度水平、医学叙事能力、共情能力、人文关怀能力及患者对护士满意度均明显提高(P<0.05)。护士对总体培训方案满意度为100%。结论 以提升叙事护理能力为主导的人文关怀培训模式能有效提高临床护士的叙事知信行水平、叙事护理能力、共情能力、人文关怀能力和患者满意度。该培训模式具有实践意义与推广价值,可为临床护士人文关怀培训提供借鉴。 展开更多
