The mouse model of antibiotics-induced diarrhea was established by gastric perfusion with mixed antibiotics(23.33 m L·kg^(-1)·d^(-1)) composed of gentamicin sulfate and cephradine capsules administered for 5 days, and the control group was treated with an equal amount of sterile water. Contents of the jejunum and ileum were then collected and metagenomic DNA was extracted, after which analysis of bacterial lactase genes using operational taxonomic units(OTUs) was carried outafter amplification and sequencing.RESULTS OTUs were 871 and 963 in the model group and control group, respectively, and 690 of these were identical. There were significant differences in Chao1 and ACE indices between the two groups(P < 0.05). Principal component analysis, principal coordination analysis and nonmetric multidimensional scaling analyses showed that OTUs distribution in the control group was relatively intensive, and differences among individuals were small, while in the model group, they were widely dispersed and more diversified. Bacterial lactase genes from the intestinal contents of the control group were related to Proteobacteria, Actinobacteria, Firmicutes and unclassified bacteria. Of these, Proteobacteria was the most abundant phylum. In contrast, the bacterial population was less diverse and abundant in the model group, as the abundance of Bradyrhizobium sp. BTAi1, Agrobacterium sp. H13-3, Acidovorax sp. KKS102, Azoarcus sp. KH32 C and Aeromonas caviae was lower than that in the control group. In addition, of the known species, the control group and model group had their own unique genera, respectively.CONCLUSION Antibiotics reduce the diversity of bacterial lactase genes in the intestinal contents, decrease the abundance of lactase gene, change the lactase gene strains, and transform their structures.展开更多
Background:Tannic acid(TA)is potential to reduce diarrhea in weaning pigs,but knowledge about the influence of TA on intestinal barrier integrity and function is still scarce.This experiment was conducted to investiga...Background:Tannic acid(TA)is potential to reduce diarrhea in weaning pigs,but knowledge about the influence of TA on intestinal barrier integrity and function is still scarce.This experiment was conducted to investigate the effects of dietary TA supplementation on growth performance,diarrhea rate,intestinal barrier integrity and function of weaned pigs.Methods:A total of 108 crossbred(Duroc×Landrace×Yorkshire)piglets,with an initial average body weight of 6.60±0.27 kg,were allotted to 3 groups(6 pigs/pen and 6 replicates/group)in a randomized complete block design according to their gender and body weight.Piglets were fed the basal diet with 0(control,CON),0.2%and 1.0%TA,respectively.The trial lasted for 28 d.Results:Compared with the CON group,dietary 0.2%and 1.0%TA supplementation didn’t affect ADFI,ADG and F:G(P>0.05),but reduced diarrhea rate,diarrhea index and diarrhea score of piglets(P<0.05),reduced diamine oxidase(DAO)activity and D-lactic acid concentration in serum(P<0.01).The higher occludin expression and localization were observed in the duodenum,jejunum and ileum after supplementation with 0.2%or 1.0%TA(P<0.05).Adding 0.2%TA to diet significantly decreased crypt depth,increased villus height/crypt depth ratio in the duodenum(P<0.05),and dietary 1.0%TA tended to decrease crypt depth(P<0.10)and significantly decreased villus height(P<0.05)of the ileum.Moreover,lower malondialdehyde content in the ileum was detected in the pigs fed 1.0%TA(P<0.05).In the duodenum,both 0.2%and 1.0%TA groups had higher occludin(OCLN)mRNA and 0.2%TA group had higher zonula occludens-2(ZO-2)level(P<0.05).Meanwhile,dietary 1.0%TA supplementation tended to up-regulate OCLN mRNA levels in the jejunum(P<0.10)and 0.2%TA supplementation tended to up-regulate zonula occludens-1(ZO-1)mRNA levels in the ileum(P<0.10).Conclusion:In conclusion,dietary supplementation of 0.2%or 1.0%TA could effectively alleviate postweaning diarrhea without altering growth performance in weaned piglets,which might be achieved by improving intestinal barrier integrity and function.展开更多
Background:This study aimed to investigate the hydration properties of different-source fibrous materials by com-paring their water-binding capacity(WBC),water swelling capacity(WSC),viscosity,and in vivo effects of s...Background:This study aimed to investigate the hydration properties of different-source fibrous materials by com-paring their water-binding capacity(WBC),water swelling capacity(WSC),viscosity,and in vivo effects of selected samples on growth performance,nutrient digestibility,diarrhea,and intestinal health in weaned piglets.Methods:A total of 13 commercially available fibrous materials were first compared in chemical composition and in vitro hydration property.Subsequently,40 weaned piglets were randomized to five experimental dietary groups(8 piglets per group):control diet(a basal diet without dietary fiber,CON),basal diet supplemented with 5%microcrys-talline cellulose(MCC),5%wheat bran(WB),5%Moringa oleifera leaf powder(MOLP),or 5%sugar beet pulp(SBP),fol-lowed by analyzing their growth performance and diarrhea rate in a 28-d experiment.After the feeding experiment,anaesthetized piglets were killed,and their intestinal and colon content or plasma samples were analyzed in nutrient digestibility,intestinal morphology,intestinal barrier,short-chain fatty acids(SCFAs),and bacterial population.Results:In vitro studies showed low hydration properties for WB and MCC,while medium hydration properties for MOLP and SBP.In vivo studies indicated that compared with medium hydration property groups,low hydration prop-erty groups showed(1)exacerbated diarrhea,impaired intestinal health,and reduced apparent fecal digestibility of dry matter,gross energy,acid detergent fiber,and neutral detergent fiber;(2)decreased SCFAs concentration and rela-tive levels of Lactobacillus and Bifidobacterium,but increased levels of Escherichia coli and Brachyspira hyodysenteriae in colon contents.Additionally,SBP showed optimal performance in reducing diarrhea and increasing SCFAs produc-tion.Correlation analysis revealed a positive correlation of fiber hydration properties with in vitro SCFAs production,and diarrhea index and nutrient digestibility were negatively and positively correlated with SCFAs levels in the colon contents of weaned piglets,respectively.Conclusions:Different-source dietary fibers varied in their hydration properties and impacts on diarrhea,microbial composition and SCFAs production in weaned piglets.WB and MCC could exacerbate diarrhea and impair nutrient digestibility,probably because their low hydration properties were detrimental to gut microbial homeostasis and fermentation.Our findings provide new ideas for rational use of fiber resources in weaned piglets.展开更多

METHODS: A total of 60 diarrhea patients with deficiency and cold syndrome were randomly divided into observation group and control group, with 30 cases in each group. The control group was treated with conventional western medicine. The observation group was given the treatment of Zhenren Yangzang Decoction combined with modified Shenling Baizhu Powder on the basis of the same treatment for 4 weeks. The clinical symptoms, fecal flora and immunoglobulin content of the 2 groups were observed and the clinical efficacy was evaluated. RESULTS: After treatment, the symptoms scores of fecal diarrhea, aversion to cold, cold limbs, loss of appetite, cold and painful waist and knee, abdominal fullness, abdominal distension and abdominal pain, and intestinal bacilli contents were significantly decreased(P < 0.05). The contents of lactobacillus, bifidus bacilli, IgG, IgM and IgA were significantly increased(P < 0.05), and the improvement of the above indexes in the observation group was significantly better than that in the control group(P < 0.05). The total effective rate after treatment in the observation group was 93.3%, which was significantly higher than 73.3% of the control group(P < 0.05). CONCLUSION: The treatment of diarrhea patients with deficiency and cold syndrome with Zhenren Yangzang Decoction and modified Shenling Baizhu Powder can effectively improve the intestinal micro-ecological environment, improve the immune function of patients and promote the rapid recovery of patients.展开更多

Herein, we report an unusual case of VIP-producing pheochromocytoma marked by persistent shock, fushing, and watery diarrhea and high sensitivity to octreotide. A 53-year-old woman was hospitalized for sudden-onset hypertension with convulsions, which then rapidly evolved to persistent shock, fushing, and watery diarrhea. Abdominal computed tomography indicated a left adrenal mass, accompanied by bleeding;and marked elevations of both plasma catecholamine and VIP concentrations were documented via laboratory testing. Surprisingly, all clinical symptoms responded swiftly to octreotide treatment. Once surgically treated, hormonal levels normalized in this patient, and the clinical symptoms dissipated. Postoperative pathological and immunohistopathological studies confrmed a VIP-secreting pheochromocytoma with strong, diffuse positivity for somatostatin receptor type 2. During a 6-mo follow-up period, she seemed in good health andwas symptom-free.展开更多

Aim: The clinical manifestations, imaging, histopathology, and immunohistochemical characteristics, treatment and prognosis of this case of MEITL were analyzed retrospectively, and combined with literatures learning to provide the experiences and lessons for diagnosis and treatment. Case Presentation: Here, we present a 68-year-old Asian woman with unexplained intestinal perforation and lung imaging changes as the first manifestation, and gradually appearing refractory diarrhea and pulmonary cavities. During the period, due to the unknown diagnosis, all treatments were mainly based on anti-infection and antidiarrhea, but her symptoms had no improvement after therapy, even died of poor basic physical conditions. Colonoscopy biopsy showed atypical lymphocyte infiltration-like growth and the formation of local superficial ulcers. Immunohistochemical tests showed that T-lymphocyte hyperplasia was predominant and led to a diagnosis of MEITL. Conclusion: MEITL is mainly manifested by gastrointestinal symptoms and lacks specific symptoms. The diagnosis of MEITL should be based on comprehensive judgment of clinical manifestations, pathological features and immunohistochemical detection results. Positive biopsy confirmed and timely chemotherapy may be conductive to a better prognosis.展开更多
[Objectives]The aim of this study was to investigate the effects of APS-sEPS(a polysaccharide compound of Astragalus polysaccharides and sulfated Epimedium polysaccharide)on intestinal mucosal immunity and structural ...[Objectives]The aim of this study was to investigate the effects of APS-sEPS(a polysaccharide compound of Astragalus polysaccharides and sulfated Epimedium polysaccharide)on intestinal mucosal immunity and structural morphology.[Methods]Firstly,the diarrhea model was established using the optimal dose of magnesium sulfate in mice.Then,the diarrhea mice were randomly divided into three groups and given either physiological saline(diarrhea model group)or injected with APS-sEPS or APS.The normal mice were selected as a control group.After administration,the duodenum,jejunum and ileum were processed microtome section,and observed for describing the small intestine morphology,villus height and crypt depth.The tissue homogenates of the duodenum,jejunum and ileum were gathered to detect the changes of sIgA,IL-4 and IL-10.[Results]The results indicated that APS-sEPS could effectively relieve diarrhea in mice.In the APS-sEPS group,the villus heights of duodenum,jejunum and ileum were increased and the depth of crypt was reduced.The contents of IL-4,IL-10 and sIgA in jejunum and ileum in APS-sEPS group were significantly higher than that in the control group(P<0.05).[Conclusions]These results indicated that APS-sEPS promoted the recovery of intestinal morphological structure and enhanced the mucosa immunity of the small intestine.展开更多

BACKGROUND Obesity is associated with a significantly increased risk for chronic diarrhea,which has been proposed as Linghu’s obesity-diarrhea syndrome(ODS);however,its molecular mechanisms are largely unknown.AIM To...BACKGROUND Obesity is associated with a significantly increased risk for chronic diarrhea,which has been proposed as Linghu’s obesity-diarrhea syndrome(ODS);however,its molecular mechanisms are largely unknown.AIM To reveal the transcriptomic changes in the jejunum involved in ODS.METHODS In a cohort of 6 ODS patients(JOD group),6 obese people without diarrhea(JO group),and 6 healthy controls(JC group),high-throughput sequencing and bioinformatics analyses were performed to identify jejunal mucosal mRNA expression alterations and dysfunctional biological processes.In another cohort of 16 ODS patients(SOD group),16 obese people without diarrhea(SO group),and 16 healthy controls(SC group),serum diamine oxidase(DAO)and D-lactate(DLA)concentrations were detected to assess changes in intestinal barrier function.RESULTS The gene expression profiles of jejunal mucosa in the JO and JC groups were similar,with only 1 differentially expressed gene(DEG).The gene expression profile of the JOD group was significantly changed,with 411 DEGs compared with the JO group and 211 DEGs compared with the JC group,129 of which overlapped.The enrichment analysis of these DEGs showed that the biological processes such as digestion,absorption,and transport of nutrients(especially lipids)tended to be up-regulated in the JOD group,while the biological processes such as rRNA processing,mitochondrial translation,antimicrobial humoral response,DNA replication,and DNA repair tended to be down-regulated in the JOD group.Eight DEGs(CDT1,NHP2,EXOSC5,EPN3,NME1,REG3A,PLA2G2A,and PRSS2)may play a key regulatory role in the pathological process of ODS,and their expression levels were significantly decreased in ODS patients(P<0.001).In the second cohort,compared with healthy controls,the levels of serum intestinal barrier function markers(DAO and D-LA)were significantly increased in all obese individuals(P<0.01),but were higher in the SOD group than in the SO group(P<0.001).CONCLUSION Compared with healthy controls and obese individuals without diarrhea,patients with Linghu’s ODS had extensive transcriptomic changes in the jejunal mucosa,likely affecting intestinal barrier function and thus contributing to the obesity and chronic diarrhea phenotypes.展开更多

E Method] DUAN-NAI-AN was given to the weaned piglets by oral administration and its effects on diarrhea rate, growth performance, in- testinal microflora, pH values of different parts of the intestine and intestinal mucosal morphology were observed. [ Result] DUAN-NAI-AN could im- prove the growth performance of weaned piglets by increasing the growth rate and phase weight gain, showing as higher livability, average daily gain and daily feed intake as well as lower diarrhea rate with 19.24% decline by contrast to the control. In addition, DUAN-NAI-AN could keep the intestinal mucosal mechanical barrier and normal digestive function by strengthening intestinal resistance and thus enhance the intestinal mucosal im- mune level. It could also decrease the proportion of potential pathogens and their toxicity effects inside the intestine to help the piglets transfer to and then maintain the dynamic balance of intestinal microflora in a very short period, and it also showed a trend of decreasing the pH value of piglet in- testinal contents. Neuroblastoma(NB) is the most common extracranial solid tumor in children. Diarrheal NB is quite rare and is not easy to diagnose in the early stage. Six cases of diarrheal NB in our hospital treated from 1996 to 2006...Neuroblastoma(NB) is the most common extracranial solid tumor in children. Diarrheal NB is quite rare and is not easy to diagnose in the early stage. Six cases of diarrheal NB in our hospital treated from 1996 to 2006 were retrospectively analyzed, including characteristics such as electrolyte imbalance, pathologic features, vasoactive intestinal peptide(VIP) immunohistochemical staining results, treatment, and prognosis. All patients were boys with 3-8 loose or watery stools each day and routine fecal tests were normal. Abdominal tumors were identified by B-ultrasound. Drugs were ineffective. Three patients underwent surgery, and the remaining three patients received surgery and chemotherapy. Diarrhea stopped after treatment in five patients. Two patients died due to intractable hypokalemia. The tumor was located in the adrenal gland in four patients, in the upper retroperitoneum in one patient, and in the presacral area in one patient. Pathologic findings were NB and ganglioneuroblastoma. Five patients were at clinical stage Ⅰ-Ⅱ, and one was at stage Ⅲ. Four patients survived(followed-up for 6 mo to 4 years). Immunohistochemical staining for VIP was positive. Refractory diarrhea is a paraneoplastic syndrome of NB and is rare. Patients aged 1-3 years who present with chronic intractable diarrhea should be followed closely. Intractable diarrhea, hypokalemia, and dysplasia are the initial clinical manifestations. Increased VIP is characteristic of this disease. Potassium supplementation plays a vital role in the treatment procedure, especially preoperatively. The prognosis of diarrheal NB is good following appropriate treatment.展开更多

Water transfer across the intestinal epithelial membrane seems to occur because of aquaporins(AQPs). Accumulating evidence indicates that alterations in AQPs may play an important role in pathogenesis. Here, we focus on changes in AQPs in a mouse model of rotavirus diarrhea. In the present study, 32 of 35 mice developed diarrhea and mild dehydration within 24 hours after infection with rotavirus strain SA11. Intestinal epithelial cells demonstrated cytoplasmic vacuolation, malaligned villi, and atrophy. AQP1 expression was significantly attenuated in the ileum and colon in comparison with controls; likewise, AQP4 and-8 protein expression were significantly decreased in the colon of rotavirus diarrhea-infected mice. In contrast, AQP3 protein expression was significantly increased in the colon of rotavirus-infected mice in comparison with controls. These results indicate that rotavirus diarrhea is associated with the downregulation of AQP1,-4, and-8 expression. Therefore, AQPs play an important role in rotavirus diarrhea.展开更多

Enzyme linked immunosorbent assay was used to detect HRV antigen in feces. the HRV was tested by real-time PCR(Rt-PCR). the sections of the intestinal tissue were stained with hematoxylin and eosin to observe the morphologic variation by microscopy. Immunofluorescence was used to determine the HRV in intestinal tissue. HRV particles in cells of the ileum were observed by electron micrography.RESULTS: When inoculated with HRV, mini-pigs younger than 30 d developed diarrhea in an agedependent manner and shed HRV antigen of the sameinoculum, as demonstrated by Rt- PCR.Histopathological changes were observed in HRV inoculated mini-pigs including small intestinal cell tumefaction and necrosis. HRV that was distributed in the small intestine was restricted to the top part of the villi on the internal wall of the ileum, which was observed by immunofluorescence and transmission electron microscopy. Virus particles were observed in Golgi like follicles in HRV-infected neonatal minipigs. Guizhou mini-pigs were more sensitive to HRV than bamma with respect to RV antigen shedding and clinical diarrhea.CONCLUSION: these results indicate that we have established a mini-pig model of HRV induced diarrhea. Our findings are useful for the understanding of the pathogenic mechanisms of HRV infection.展开更多
To evaluate the effects of dietary protein levels and probiotic supplementation on microbial intestinal fermentation, diarrhea incidence, and performance in weaned piglets, 162 piglets were randomly assigned to three ...To evaluate the effects of dietary protein levels and probiotic supplementation on microbial intestinal fermentation, diarrhea incidence, and performance in weaned piglets, 162 piglets were randomly assigned to three treatments: high-protein diet (20%), with antibiotics (HPa);high-protein diet (20%), without antibiotics (HP);and low-protein diet, without antibiotics but with probiotics (LPpb). Piglets and feed were weighed weekly to calculate the average daily gain (ADG), average daily feed intake (ADFI), and gain: feed ratio (G:F). Four piglets per treatment were killed on day 21 postweaning to collect ileal and colon digesta for measurement of short-chain fatty acids (SCFAs), branched-chain fatty acids (BCFAs), lactic acid (LA), and ammonia (AM). In the digesta collected from the ileum and colon, piglets fed the HPa diet had the lowest concentration of volatile fatty acids. Additionally, the HP diet produced the highest concentration of BCFAs, while the LPpb diet produced moreacetic, propionic, and butyric acids than the HPa and HP diets. Piglets fed the HP diet had higher incidence and severity of diarrhea than piglets fed LPpb and HPa diets, and similar values were observed between these two groups. The second week postweaning was the most critical for diarrhea measurements;during the second week, animals had higher incidence and severity of diarrhea. Piglets fed the HPa and LPpb diets had similar ADGs, while those fed the HP diet had the poorest ADG. Similar results were observed with ADFI and G:F. A low-protein diet supplemented with probiotics changed the fermentation profile, reducing toxic metabolites, promoting gut health, decreasing the incidence and severity of postweaning diarrhea, and improving the performance of piglets.展开更多

Oral rehydration solution (ORS), a passive rehydrating therapy developed approximately 40 years ago, remains the mainstay treatment. Although ORS is effective for hydration, since it does not inhibit enterotoxin-mediated excessive secretion, reduced absorption and compromised barrier function - the primary mechanisms of diarrhea, ORS does not offer a rapid relief of diarrhea symptom. There are a few alternative therapies available, yet the use of these drugs is limited by their expense, lack of availability and/or safety concerns. Novel anti-diarrheal therapeutic approaches, particularly those simple affordable therapies, are needed. This article explores intestinal calcium-sensing receptor (CaSR), a newly uncovered target for therapy of diarrhea. Unlike others, targeting this host antidiarrheal receptor system appears “all-inclusive”: it is anti-secretory, pro-absorptive, anti-motility, and anti-inflammatory. Thus, activating CaSR reverses changes of both secretory and inflammatory diarrheas. Considering its unique property of using simple nutrients such as calcium, polyamines, and certain amino acids/oligopeptides as activators, it is possible that through targeting of CaSR with a combination of specific nutrients, novel oral rehydrating solutions that are inexpensive and practical to use in all countries may be developed.展开更多

文摘AIM To investigate the diversity of bacterial lactase genes in the intestinal contents of mice with antibiotics-induced diarrhea.METHODS Following 2 d of adaptive feeding, 12 specific pathogenfree Kunming mice were randomly divided into the control group and model group. The mouse model of antibiotics-induced diarrhea was established by gastric perfusion with mixed antibiotics(23.33 m L·kg^(-1)·d^(-1)) composed of gentamicin sulfate and cephradine capsules administered for 5 days, and the control group was treated with an equal amount of sterile water. Contents of the jejunum and ileum were then collected and metagenomic DNA was extracted, after which analysis of bacterial lactase genes using operational taxonomic units(OTUs) was carried outafter amplification and sequencing.RESULTS OTUs were 871 and 963 in the model group and control group, respectively, and 690 of these were identical. There were significant differences in Chao1 and ACE indices between the two groups(P < 0.05). Principal component analysis, principal coordination analysis and nonmetric multidimensional scaling analyses showed that OTUs distribution in the control group was relatively intensive, and differences among individuals were small, while in the model group, they were widely dispersed and more diversified. Bacterial lactase genes from the intestinal contents of the control group were related to Proteobacteria, Actinobacteria, Firmicutes and unclassified bacteria. Of these, Proteobacteria was the most abundant phylum. In contrast, the bacterial population was less diverse and abundant in the model group, as the abundance of Bradyrhizobium sp. BTAi1, Agrobacterium sp. H13-3, Acidovorax sp. KKS102, Azoarcus sp. KH32 C and Aeromonas caviae was lower than that in the control group. In addition, of the known species, the control group and model group had their own unique genera, respectively.CONCLUSION Antibiotics reduce the diversity of bacterial lactase genes in the intestinal contents, decrease the abundance of lactase gene, change the lactase gene strains, and transform their structures.

文摘Background:Tannic acid(TA)is potential to reduce diarrhea in weaning pigs,but knowledge about the influence of TA on intestinal barrier integrity and function is still scarce.This experiment was conducted to investigate the effects of dietary TA supplementation on growth performance,diarrhea rate,intestinal barrier integrity and function of weaned pigs.Methods:A total of 108 crossbred(Duroc×Landrace×Yorkshire)piglets,with an initial average body weight of 6.60±0.27 kg,were allotted to 3 groups(6 pigs/pen and 6 replicates/group)in a randomized complete block design according to their gender and body weight.Piglets were fed the basal diet with 0(control,CON),0.2%and 1.0%TA,respectively.The trial lasted for 28 d.Results:Compared with the CON group,dietary 0.2%and 1.0%TA supplementation didn’t affect ADFI,ADG and F:G(P>0.05),but reduced diarrhea rate,diarrhea index and diarrhea score of piglets(P<0.05),reduced diamine oxidase(DAO)activity and D-lactic acid concentration in serum(P<0.01).The higher occludin expression and localization were observed in the duodenum,jejunum and ileum after supplementation with 0.2%or 1.0%TA(P<0.05).Adding 0.2%TA to diet significantly decreased crypt depth,increased villus height/crypt depth ratio in the duodenum(P<0.05),and dietary 1.0%TA tended to decrease crypt depth(P<0.10)and significantly decreased villus height(P<0.05)of the ileum.Moreover,lower malondialdehyde content in the ileum was detected in the pigs fed 1.0%TA(P<0.05).In the duodenum,both 0.2%and 1.0%TA groups had higher occludin(OCLN)mRNA and 0.2%TA group had higher zonula occludens-2(ZO-2)level(P<0.05).Meanwhile,dietary 1.0%TA supplementation tended to up-regulate OCLN mRNA levels in the jejunum(P<0.10)and 0.2%TA supplementation tended to up-regulate zonula occludens-1(ZO-1)mRNA levels in the ileum(P<0.10).Conclusion:In conclusion,dietary supplementation of 0.2%or 1.0%TA could effectively alleviate postweaning diarrhea without altering growth performance in weaned piglets,which might be achieved by improving intestinal barrier integrity and function.

文摘Background:This study aimed to investigate the hydration properties of different-source fibrous materials by com-paring their water-binding capacity(WBC),water swelling capacity(WSC),viscosity,and in vivo effects of selected samples on growth performance,nutrient digestibility,diarrhea,and intestinal health in weaned piglets.Methods:A total of 13 commercially available fibrous materials were first compared in chemical composition and in vitro hydration property.Subsequently,40 weaned piglets were randomized to five experimental dietary groups(8 piglets per group):control diet(a basal diet without dietary fiber,CON),basal diet supplemented with 5%microcrys-talline cellulose(MCC),5%wheat bran(WB),5%Moringa oleifera leaf powder(MOLP),or 5%sugar beet pulp(SBP),fol-lowed by analyzing their growth performance and diarrhea rate in a 28-d experiment.After the feeding experiment,anaesthetized piglets were killed,and their intestinal and colon content or plasma samples were analyzed in nutrient digestibility,intestinal morphology,intestinal barrier,short-chain fatty acids(SCFAs),and bacterial population.Results:In vitro studies showed low hydration properties for WB and MCC,while medium hydration properties for MOLP and SBP.In vivo studies indicated that compared with medium hydration property groups,low hydration prop-erty groups showed(1)exacerbated diarrhea,impaired intestinal health,and reduced apparent fecal digestibility of dry matter,gross energy,acid detergent fiber,and neutral detergent fiber;(2)decreased SCFAs concentration and rela-tive levels of Lactobacillus and Bifidobacterium,but increased levels of Escherichia coli and Brachyspira hyodysenteriae in colon contents.Additionally,SBP showed optimal performance in reducing diarrhea and increasing SCFAs produc-tion.Correlation analysis revealed a positive correlation of fiber hydration properties with in vitro SCFAs production,and diarrhea index and nutrient digestibility were negatively and positively correlated with SCFAs levels in the colon contents of weaned piglets,respectively.Conclusions:Different-source dietary fibers varied in their hydration properties and impacts on diarrhea,microbial composition and SCFAs production in weaned piglets.WB and MCC could exacerbate diarrhea and impair nutrient digestibility,probably because their low hydration properties were detrimental to gut microbial homeostasis and fermentation.Our findings provide new ideas for rational use of fiber resources in weaned piglets.
文摘OBJECTIVE: To observe the effects of Zhenren Yangzang Decoction combined with modified Shenling Baizhu Powder on the immune function and intestinal microecology of diarrhea patients with deficiency and cold syndrome. METHODS: A total of 60 diarrhea patients with deficiency and cold syndrome were randomly divided into observation group and control group, with 30 cases in each group. The control group was treated with conventional western medicine. The observation group was given the treatment of Zhenren Yangzang Decoction combined with modified Shenling Baizhu Powder on the basis of the same treatment for 4 weeks. The clinical symptoms, fecal flora and immunoglobulin content of the 2 groups were observed and the clinical efficacy was evaluated. RESULTS: After treatment, the symptoms scores of fecal diarrhea, aversion to cold, cold limbs, loss of appetite, cold and painful waist and knee, abdominal fullness, abdominal distension and abdominal pain, and intestinal bacilli contents were significantly decreased(P < 0.05). The contents of lactobacillus, bifidus bacilli, IgG, IgM and IgA were significantly increased(P < 0.05), and the improvement of the above indexes in the observation group was significantly better than that in the control group(P < 0.05). The total effective rate after treatment in the observation group was 93.3%, which was significantly higher than 73.3% of the control group(P < 0.05). CONCLUSION: The treatment of diarrhea patients with deficiency and cold syndrome with Zhenren Yangzang Decoction and modified Shenling Baizhu Powder can effectively improve the intestinal micro-ecological environment, improve the immune function of patients and promote the rapid recovery of patients.

文摘Vasoactive intestinal peptide-producing tumors (VIP-oma) usually originate in the pancreas and are chara-cterized by diarrhea, hypokalemia, and achlorhydria (WDHA syndrome). In adults, nonpancreatic VIPoma is very rare. Herein, we report an unusual case of VIP-producing pheochromocytoma marked by persistent shock, fushing, and watery diarrhea and high sensitivity to octreotide. A 53-year-old woman was hospitalized for sudden-onset hypertension with convulsions, which then rapidly evolved to persistent shock, fushing, and watery diarrhea. Abdominal computed tomography indicated a left adrenal mass, accompanied by bleeding;and marked elevations of both plasma catecholamine and VIP concentrations were documented via laboratory testing. Surprisingly, all clinical symptoms responded swiftly to octreotide treatment. Once surgically treated, hormonal levels normalized in this patient, and the clinical symptoms dissipated. Postoperative pathological and immunohistopathological studies confrmed a VIP-secreting pheochromocytoma with strong, diffuse positivity for somatostatin receptor type 2. During a 6-mo follow-up period, she seemed in good health andwas symptom-free.
文摘Background: Monomorphic epithelial intestinal T-cell lymphoma (MEITL), previously known as type II Enteropathy-associated T-cell lymphoma (EATL), is a rare intestinal tumor with strong invasiveness, poor prognosis, atypical clinical manifestations, and is difficult to diagnose. Aim: The clinical manifestations, imaging, histopathology, and immunohistochemical characteristics, treatment and prognosis of this case of MEITL were analyzed retrospectively, and combined with literatures learning to provide the experiences and lessons for diagnosis and treatment. Case Presentation: Here, we present a 68-year-old Asian woman with unexplained intestinal perforation and lung imaging changes as the first manifestation, and gradually appearing refractory diarrhea and pulmonary cavities. During the period, due to the unknown diagnosis, all treatments were mainly based on anti-infection and antidiarrhea, but her symptoms had no improvement after therapy, even died of poor basic physical conditions. Colonoscopy biopsy showed atypical lymphocyte infiltration-like growth and the formation of local superficial ulcers. Immunohistochemical tests showed that T-lymphocyte hyperplasia was predominant and led to a diagnosis of MEITL. Conclusion: MEITL is mainly manifested by gastrointestinal symptoms and lacks specific symptoms. The diagnosis of MEITL should be based on comprehensive judgment of clinical manifestations, pathological features and immunohistochemical detection results. Positive biopsy confirmed and timely chemotherapy may be conductive to a better prognosis.

文摘[Objectives]The aim of this study was to investigate the effects of APS-sEPS(a polysaccharide compound of Astragalus polysaccharides and sulfated Epimedium polysaccharide)on intestinal mucosal immunity and structural morphology.[Methods]Firstly,the diarrhea model was established using the optimal dose of magnesium sulfate in mice.Then,the diarrhea mice were randomly divided into three groups and given either physiological saline(diarrhea model group)or injected with APS-sEPS or APS.The normal mice were selected as a control group.After administration,the duodenum,jejunum and ileum were processed microtome section,and observed for describing the small intestine morphology,villus height and crypt depth.The tissue homogenates of the duodenum,jejunum and ileum were gathered to detect the changes of sIgA,IL-4 and IL-10.[Results]The results indicated that APS-sEPS could effectively relieve diarrhea in mice.In the APS-sEPS group,the villus heights of duodenum,jejunum and ileum were increased and the depth of crypt was reduced.The contents of IL-4,IL-10 and sIgA in jejunum and ileum in APS-sEPS group were significantly higher than that in the control group(P<0.05).[Conclusions]These results indicated that APS-sEPS promoted the recovery of intestinal morphological structure and enhanced the mucosa immunity of the small intestine.

文摘Diarrhea has become the leading cause of illness and death among infants and young children in developing countries.Clinically,patients with diarrhea showed damaged intestinal epithelial villi,usually accompanied by lactase deficiency.In this study,we evaluated the therapeutic effects of lactose-free milk powder on rats and children with diarrhea.Antibiotic-associated diarrhea(AAD)model was established by gavage with antibiotic mixture in SD rats,followed by administration of milk powder containing lactose or not.The results showed that lactose-free milk powder ameliorated A AD-related diarrhea symptoms,and accelerated the recovery from diarrhea.And 16S sequencing results indicated lactose-free milk powder contributed to increase theα-andβ-diversity of intestinal flora,and restore the intestinal microbiota disorder.In conclusion,our data demonstrate that lactose-free milk powder could alleviate diarrhea by restoring gut microbiota and intestinal barrier function.
文摘BACKGROUND Obesity is associated with a significantly increased risk for chronic diarrhea,which has been proposed as Linghu’s obesity-diarrhea syndrome(ODS);however,its molecular mechanisms are largely unknown.AIM To reveal the transcriptomic changes in the jejunum involved in ODS.METHODS In a cohort of 6 ODS patients(JOD group),6 obese people without diarrhea(JO group),and 6 healthy controls(JC group),high-throughput sequencing and bioinformatics analyses were performed to identify jejunal mucosal mRNA expression alterations and dysfunctional biological processes.In another cohort of 16 ODS patients(SOD group),16 obese people without diarrhea(SO group),and 16 healthy controls(SC group),serum diamine oxidase(DAO)and D-lactate(DLA)concentrations were detected to assess changes in intestinal barrier function.RESULTS The gene expression profiles of jejunal mucosa in the JO and JC groups were similar,with only 1 differentially expressed gene(DEG).The gene expression profile of the JOD group was significantly changed,with 411 DEGs compared with the JO group and 211 DEGs compared with the JC group,129 of which overlapped.The enrichment analysis of these DEGs showed that the biological processes such as digestion,absorption,and transport of nutrients(especially lipids)tended to be up-regulated in the JOD group,while the biological processes such as rRNA processing,mitochondrial translation,antimicrobial humoral response,DNA replication,and DNA repair tended to be down-regulated in the JOD group.Eight DEGs(CDT1,NHP2,EXOSC5,EPN3,NME1,REG3A,PLA2G2A,and PRSS2)may play a key regulatory role in the pathological process of ODS,and their expression levels were significantly decreased in ODS patients(P<0.001).In the second cohort,compared with healthy controls,the levels of serum intestinal barrier function markers(DAO and D-LA)were significantly increased in all obese individuals(P<0.01),but were higher in the SOD group than in the SO group(P<0.001).CONCLUSION Compared with healthy controls and obese individuals without diarrhea,patients with Linghu’s ODS had extensive transcriptomic changes in the jejunal mucosa,likely affecting intestinal barrier function and thus contributing to the obesity and chronic diarrhea phenotypes.
文摘BACKGROUND Angioleiomyoma is a rare and benign stromal tumor typically found in subcutaneous tissue.It rarely occurs in the gastrointestinal tract.Among the reported cases,the most common complication was gastrointestinal bleeding.Perforation has only been reported as a complication in the last few decades.CASE SUMMARY This case report detailed the discovery of intestinal angioleiomyoma in a 47-yearold male presenting with abdominal pain that had persisted for 3 d.After suspecting hollow organ perforation,surgical intervention involving intestinal resection and anastomosis was performed.CONCLUSION The report underscores the significance of early surgical intervention in effectively treating angioleiomyoma while emphasizing the pivotal role of timely and appropriate measures for favorable outcomes.
文摘[ Objective] The study aimed to explore the function mechanism of microbial ecological agent DUAN-NAI-AN on controlling weaned piglet diarrhea. E Method] DUAN-NAI-AN was given to the weaned piglets by oral administration and its effects on diarrhea rate, growth performance, in- testinal microflora, pH values of different parts of the intestine and intestinal mucosal morphology were observed. [ Result] DUAN-NAI-AN could im- prove the growth performance of weaned piglets by increasing the growth rate and phase weight gain, showing as higher livability, average daily gain and daily feed intake as well as lower diarrhea rate with 19.24% decline by contrast to the control. In addition, DUAN-NAI-AN could keep the intestinal mucosal mechanical barrier and normal digestive function by strengthening intestinal resistance and thus enhance the intestinal mucosal im- mune level. It could also decrease the proportion of potential pathogens and their toxicity effects inside the intestine to help the piglets transfer to and then maintain the dynamic balance of intestinal microflora in a very short period, and it also showed a trend of decreasing the pH value of piglet in- testinal contents. [ Conclusion] DUAN-NAI-AN is an effective microbial ecological agent for the control of weaned piglet diarrhea.
文摘Neuroblastoma(NB) is the most common extracranial solid tumor in children. Diarrheal NB is quite rare and is not easy to diagnose in the early stage. Six cases of diarrheal NB in our hospital treated from 1996 to 2006 were retrospectively analyzed, including characteristics such as electrolyte imbalance, pathologic features, vasoactive intestinal peptide(VIP) immunohistochemical staining results, treatment, and prognosis. All patients were boys with 3-8 loose or watery stools each day and routine fecal tests were normal. Abdominal tumors were identified by B-ultrasound. Drugs were ineffective. Three patients underwent surgery, and the remaining three patients received surgery and chemotherapy. Diarrhea stopped after treatment in five patients. Two patients died due to intractable hypokalemia. The tumor was located in the adrenal gland in four patients, in the upper retroperitoneum in one patient, and in the presacral area in one patient. Pathologic findings were NB and ganglioneuroblastoma. Five patients were at clinical stage Ⅰ-Ⅱ, and one was at stage Ⅲ. Four patients survived(followed-up for 6 mo to 4 years). Immunohistochemical staining for VIP was positive. Refractory diarrhea is a paraneoplastic syndrome of NB and is rare. Patients aged 1-3 years who present with chronic intractable diarrhea should be followed closely. Intractable diarrhea, hypokalemia, and dysplasia are the initial clinical manifestations. Increased VIP is characteristic of this disease. Potassium supplementation plays a vital role in the treatment procedure, especially preoperatively. The prognosis of diarrheal NB is good following appropriate treatment.

文摘Rotavirus diarrhea is a major worldwide cause of infantile gastroenteritis; however, the mechanism responsible for intestinal fluid loss remains unclear. Water transfer across the intestinal epithelial membrane seems to occur because of aquaporins(AQPs). Accumulating evidence indicates that alterations in AQPs may play an important role in pathogenesis. Here, we focus on changes in AQPs in a mouse model of rotavirus diarrhea. In the present study, 32 of 35 mice developed diarrhea and mild dehydration within 24 hours after infection with rotavirus strain SA11. Intestinal epithelial cells demonstrated cytoplasmic vacuolation, malaligned villi, and atrophy. AQP1 expression was significantly attenuated in the ileum and colon in comparison with controls; likewise, AQP4 and-8 protein expression were significantly decreased in the colon of rotavirus diarrhea-infected mice. In contrast, AQP3 protein expression was significantly increased in the colon of rotavirus-infected mice in comparison with controls. These results indicate that rotavirus diarrhea is associated with the downregulation of AQP1,-4, and-8 expression. Therefore, AQPs play an important role in rotavirus diarrhea.

文摘AIM: to establish a new animal model for the research of human rotavirus(HRV) infection, its pathogenesis and immunity and evaluation of potential vaccines.METHODS: 5-d, 30-d and 60-d-old Chinese mini-pigs, Guizhou and bamma, were inoculated with a single oral dose of attenuated strain Wa, G1, G3 of HRV, and PbS(control), respectively, and fecal samples of pigs from 0 to 7 d post infection(DPI) were collected individually. Enzyme linked immunosorbent assay was used to detect HRV antigen in feces. the HRV was tested by real-time PCR(Rt-PCR). the sections of the intestinal tissue were stained with hematoxylin and eosin to observe the morphologic variation by microscopy. Immunofluorescence was used to determine the HRV in intestinal tissue. HRV particles in cells of the ileum were observed by electron micrography.RESULTS: When inoculated with HRV, mini-pigs younger than 30 d developed diarrhea in an agedependent manner and shed HRV antigen of the sameinoculum, as demonstrated by Rt- PCR.Histopathological changes were observed in HRV inoculated mini-pigs including small intestinal cell tumefaction and necrosis. HRV that was distributed in the small intestine was restricted to the top part of the villi on the internal wall of the ileum, which was observed by immunofluorescence and transmission electron microscopy. Virus particles were observed in Golgi like follicles in HRV-infected neonatal minipigs. Guizhou mini-pigs were more sensitive to HRV than bamma with respect to RV antigen shedding and clinical diarrhea.CONCLUSION: these results indicate that we have established a mini-pig model of HRV induced diarrhea. Our findings are useful for the understanding of the pathogenic mechanisms of HRV infection.
文摘To evaluate the effects of dietary protein levels and probiotic supplementation on microbial intestinal fermentation, diarrhea incidence, and performance in weaned piglets, 162 piglets were randomly assigned to three treatments: high-protein diet (20%), with antibiotics (HPa);high-protein diet (20%), without antibiotics (HP);and low-protein diet, without antibiotics but with probiotics (LPpb). Piglets and feed were weighed weekly to calculate the average daily gain (ADG), average daily feed intake (ADFI), and gain: feed ratio (G:F). Four piglets per treatment were killed on day 21 postweaning to collect ileal and colon digesta for measurement of short-chain fatty acids (SCFAs), branched-chain fatty acids (BCFAs), lactic acid (LA), and ammonia (AM). In the digesta collected from the ileum and colon, piglets fed the HPa diet had the lowest concentration of volatile fatty acids. Additionally, the HP diet produced the highest concentration of BCFAs, while the LPpb diet produced moreacetic, propionic, and butyric acids than the HPa and HP diets. Piglets fed the HP diet had higher incidence and severity of diarrhea than piglets fed LPpb and HPa diets, and similar values were observed between these two groups. The second week postweaning was the most critical for diarrhea measurements;during the second week, animals had higher incidence and severity of diarrhea. Piglets fed the HPa and LPpb diets had similar ADGs, while those fed the HP diet had the poorest ADG. Similar results were observed with ADFI and G:F. A low-protein diet supplemented with probiotics changed the fermentation profile, reducing toxic metabolites, promoting gut health, decreasing the incidence and severity of postweaning diarrhea, and improving the performance of piglets.

文摘Management of acute diarrhea remains a global challenge, particularly in resource-limiting countries. Oral rehydration solution (ORS), a passive rehydrating therapy developed approximately 40 years ago, remains the mainstay treatment. Although ORS is effective for hydration, since it does not inhibit enterotoxin-mediated excessive secretion, reduced absorption and compromised barrier function - the primary mechanisms of diarrhea, ORS does not offer a rapid relief of diarrhea symptom. There are a few alternative therapies available, yet the use of these drugs is limited by their expense, lack of availability and/or safety concerns. Novel anti-diarrheal therapeutic approaches, particularly those simple affordable therapies, are needed. This article explores intestinal calcium-sensing receptor (CaSR), a newly uncovered target for therapy of diarrhea. Unlike others, targeting this host antidiarrheal receptor system appears “all-inclusive”: it is anti-secretory, pro-absorptive, anti-motility, and anti-inflammatory. Thus, activating CaSR reverses changes of both secretory and inflammatory diarrheas. Considering its unique property of using simple nutrients such as calcium, polyamines, and certain amino acids/oligopeptides as activators, it is possible that through targeting of CaSR with a combination of specific nutrients, novel oral rehydrating solutions that are inexpensive and practical to use in all countries may be developed.
文摘Treatment of diarrhea in children by shallow needling and by drugs was studied in 3 separategroups for comparison.For Group Ⅰ,the lateral line Ⅱ of the forehead was taken,i.e.theupper,middle and lower points of the line staring from Toulinqi(UB 15)vertically down-wards to the place 1 cm below the hairline were selected.For Group Ⅱ,shallow needling wasdone on body acupoints,the main points selected being Qihai(Ren 6),Shuifen(Ren 9),bi-lateral Tianshu(St 25),and bilateral Zusanli(St 36);the adjuvant acupoints were Taibai(Sp 3)and Gongsun(Sp 4).The depth of shallow needling was merely puncturing the skin.For Group Ⅲ the drugs used were Liteling,Berberine,Gentamycin,Ampicillin,and SMZCo.The results of treatment in Group Ⅰ and Ⅱ differed insignificantly,while both weresignificantly superior to drugs in Group Ⅲ.Experimental studies also showed that shallowneedling enhanced the humoral and cellular immunity and promoted the intestinal peristalticfunction.
