Analysis on the Cohesive and Coherent Features of John Kennedy's First Inaugural Address

John Kennedy's first inaugural address is one of the widely appreciated speeches worldwide. It is famous not only for calling up the American people to well serve the country, but also for its extraordinary linguistic power to arouse the listeners' emotions, which lies to a great extent in the marvelous employment of the cohesive and coherent devices in the process of its delivery. Cohesion and coherence are two elementary and significant concepts in the theoretical system of discourse analysis. There-fore, they play an important role in the structuring, arrangement, interpretation and analysis of a discourse. In this sense, it is significant to analyze the cohesive and coherent features of John Kennedy's first inaugural address in order to obtain a penetrating comprehension of the speech in many aspects. A detailed analysis on the cohesive and coherent features of the speech has been conducted in this paper. In the aspect of cohesion in the address, the devices employed fall into two categories: structural cohesion and non-structural cohesion. Structural cohesive devices used in the discourse are mainly grammatical cohesion and lexical cohesion like repetition, ellipsis, conjunction, etc. Non-structural methods adopted in the speech are transitivity, mood and modality, thematic progression, parallel structure and so on. In the aspect of coherence, five levels of coherent methods have been employed, namely, lexical level, syntax level, semantic level, phonological level and social semiotic level. The neat intermingling of the cohesive and coherent methods function cooperatively and lead to the smooth going of the text.

Does St.John's Wort cause regression in gastrointestinal system adenocarcinomas?

St.John's Wort(SJW) is an old herb which has long been consumed widely for its anti-inflammatory,antiviral,and anti-depressive properties.Here we present a detailed clinical evaluation of three cases(two colon and one duodenal adenocarcinoma) with remarkable and intensive lymphoplasmocytic host reaction,at the basal part of tumor,intensive fibrosis,giant cells,plasma cell increase in lymph nodes and few giant cells in germinal centers in resection specimens.The observation of similar host reaction in those tumors having otherwise usual appearance was interesting.None of the cases received neoadjuvant chemoradiotherapy or additional treatment before surgery but only SJW.These cases are presented to increase the awareness about such cases.Further research is needed to reveal the possible effect of SJW,which has long been consumed for different treatment purposes,on human tumors.

Paratuberculosis Infection in Camel (<i>Camelus dromidarius</i>): Current and Prospective Overview

Camel (Camelus dromidarius) is an important source of meat and milk and an iconic animal of the Saudi Arabian heritage. The accumulative evidence indicated the spread of paratuberculosis infection in the camel herds. Despite the explicit studies on the details of the disease in camels and methods of its diagnosis, paratuberculosis infection in camels suffers from wide gap of knowledge of the disease pathogenesis, camel immune responses to the infection and factors that enhance camel’s resistance to the infection. The review discusses the current available information of the disease pathobiology and the approaches employed in the diagnosis of paratuberculosis infection in camel. Effective control of the disease in camel prompts for urgent innovation of the current approaches in the diagnosis. Efficient policies and inspection tools are becoming vital to tackle the possible threats of Crohn’s disease to the public health due to the meat and milk consumption.

Anxiety-Inducing Dietary Supplements: A Review of Herbs and Other Supplements with Anxiogenic Properties

Anxiety disorders comprise the most common group of mental disorders to affect the general population in the United States. These disorders are heterogeneous in nature, highly comorbid with one another, and pose a degree of difficulty regarding diagnosis and management. The exact etiology and pathophysiology of anxiety remains to be elucidated;however, it is likely that it is multifactorial, and all potential causes of anxiety must be investigated, including substance-induced anxiety. Among substances that may induce anxiety are dietary supplements. As utilization of these products appears to be high in the US population, it is important to identify which of available supplements may cause anxiety. Objective: To review the scientific literature to identify dietary supplements associated with induction of anxiety and related symptoms. Methods: A search of Natural Medicines Comprehensive Database, MedlinePlus Herbs and Supplements, and Natural Standard was performed to identify dietary supplements with anxiogenic properties. Dietary supplements were included based on product availability, utilization trends, and if there was sufficient evidence that the substance could elicit anxiety via direct pharmacologic effects. Agents were excluded from the review if anxiety was solely due to withdrawal from the substance or only occurred in the setting of intoxication. A search of MEDLINE and PUBMED was performed to identify relevant peer reviewed publications concerning induction of anxiety and/or its associated symptoms by those dietary supplements chosen for review. Conclusions: Dietary supplements can contribute to and cause anxiety and its related symptoms via overstimulation of the Central Nervous System. Healthcare professionals should screen all patients presenting with symptoms consistent with anxiety for recent or current use of all products capable of inducing anxiety, including dietary supplements.

Survival mechanisms of <i>Mycobacterium avium</i>subspecies <i>paratuberculosis</i>within host species and in the environment—A review

Mycobacterium avium subsp. paratuberculosis causes chronic inflammation of the intestine known as Johne’s disease (JD) in domestic and wild ruminants including primates. MAP has also been associated with inflammatory bowel disease (IBD) so called Crohn’s disease (CD) of human beings, which is incurable even after surgery. By virtue of the pasteurization resistant power, high endemicity of the infection in animals continues to be the permanent source of infection to human population. High bio-burden of MAP in wide range of biotic (animal hosts including human beings) and abiotic environment in each and every country where it has been investigated, serves a reminder about the survival abilities of the MAP in diverse range of environmental conditions. Ability of the MAP to evade immune system of the host and the temporal events during infection of the macro-phages, is an area of major concern and research activities as the pattern of distribution are quiet different from those of other pathogenic intracellular organisms. Moreover, the organism can survive over a wide range of environmental conditions such as high and low environmental temperatures, pasteurization, low pH, and high salt concentration etc. This superior survival efficiency from environmental degradation and dormancy within host allows the pathogen to be available for causing disease and pathogenicity in animals and human beings, when conditions are favorable. Perusal of literature reveal that, despite the availability of whole genome sequence of MAP, a very little is known about the replication, persistence and survival mechanisms of this pathogen. Therefore, this review tries to address the survival mechanisms of Mycobacterium avium subspecies paratuberculosis in the different host species and adverse environmental conditions in order to allow designing of more rational diagnostic and control procedures.

Mycobacterium avium subspecies paratuberculosis in the etiology of Crohn's disease, cause or epiphenomenon?

The origin of inflammatory bowel disease is unknown. Attempts have been made to isolate a microorganism that could explain the onset of inflammation, but no pathological agent has ever been identified. Johne's disease is a granulomatous chronic enteritis of cattle and sheep caused by Mycobacterium avium subspecies paratuberculosis(MAP) and shows some analogies with Crohn's disease(CD). Several studies have tried to clarify if MAP has a role in the etiology of CD. The present article provides an overview of the evidence in favor and against the "MAP-hypothesis", analyzing the methods commonly adopted to detect MAP and the role of antimycobacterial therapy in patients with inflammatory bowel disease. Studies were identified through the electronic database, MEDLINE, and were selected based on their relevance to the objective of the review. The presence of MAP was investigated using multiple diagnostic methods for MAP detection and in different tissue samples from patients affected by CD or ulcerative colitis and in healthy controls. On the basis of their studies, several authors support a close relationship between MAP and CD. Although increasing evidence of MAP detection in CD patients is unquestionable, a clear etiological link still needs to be proven.

Pleomorphic hepatocellular carcinoma following consumption of hypericum perforatum in alcoholic cirrhosis

Hepatocellular carcinoma(HCC)often develops in patients with underlying liver disease,yet HCC with syncytial giant cells(SGCs)is extremely rare.Herein,we report a 55-year-old man with a 6-year history of alcoholic cirrhosis who during his regular checkup presented with marked elevation of alpha-fetoprotein.Clinical examination and imaging analyses revealed a tumor-like lesion in segment 4 of the liver,which was removed by limited wedge resection.Histological analysis by hematoxylin and eosin staining indicated pleomorphic and atypical nodules,with some SGCs,embedded within the boundaries of the neoplastic lesion.The adjacent liver parenchyma showed microvesicular steatosis,pericellular fibrosis,and moderate hemosiderin accumulation(grade 2,as determined by Prussian blue iron stain)in hepatocytes and Kupffer cells but no copper accumulation(as determined by orcein stain).Immunohistochemical analysis showed hepatocyte antigen-positive staining for the neoplastic cells and SGCs.The diagnosis was made for cirrhosis-related HCC with SGCs.The previous reports of pleomorphic HCC have featured osteoclast-like(i.e.,mesenchymal type)giant cells,making this case of epithelial type giant cells very rare.The patient’s 6-month history of hypericum perforatum/St John’s wort self-medication may have prompted the cirrhosis or HCC progression or the unusual SGC manifestation.
