Cystic low-grade collecting duct renal carcinoma with liver compression—A challenging diagnosis and therapy: A case report

BACKGROUND A collecting duct carcinoma is a very rare, malignant renal epithelial tumor.Distant metastases are present in one third of cases at the time of diagnosis. It is known to have a poor prognosis.CASE SUMMARY A 42-year-old male was sent to our surgery clinic for removal of a 119.2 mm ×108.3 mm encapsulated cystic mass, which was localized in the 8th segment of the right liver lobe. The lesion was first identified on ultrasonography. A computed tomography scan confirmed the presence of a Bosniak type Ⅲ cystic lesion,which affected the liver and convexity of the right kidney. Surgical intervention involved a right nephrectomy, with removal of the cystic mass. The patient was mobilized on the first postoperative day and was discharged after 7 d. The histological and immunohistochemical examination revealed a low-grade collecting duct renal carcinoma, which is a rare variant of papillary carcinoma,with low malignant potential. The patient did not receive chemotherapy and after 21 mo of follow-up, a radiological examination and laboratory analyses showed normal aspects. No relapse or other complications were reported.CONCLUSION To manage renal tumors properly, a correct histopathological diagnosis is crucial,as is early diagnosis and correct surgical treatment. 展开更多

Acute tubular necrosis(ATN)induced by ischemia is the most common insult to donor kidneysdestined for trarsplantation.ATN results from sweling and subsequent damage to cells lining thelkidney tubules.In this study,we demonstrate the capability of optical coherence tomography(OcT)to image the renal microst ructures of living human donor kidneys and potentially providea measure to det ermine the extent of A TN.We also found that Doppler-based OCT(i.e.,DOCT)reveals renal blood flow dynamics that is another major factor which could relate to post-transplant renal finction.All OCT/DoCT oberva tions were performed in a noninvasive,sterileand timely manner on intact human kidneys both prior to(er vivo)and following(in vivo)theirtransplantation.Our results indicate that this imaging model provides transplant surgeons withan objective visualization of the transplant lidneys prior and immediately post transplantation. 展开更多

Acetaminophen is a drug used to treat many conditions as headache, muscle aches, arthritis, backache, toothache, and fever between others, but collateral effects of this drug are not well known yet. Here is tested its effect on proximal tubule epithelium. Acetaminophen (APAP) at doses of 200, 500, 1000 and 1500 mg/Kg i.p. caused cell damage and changes in F-actin distribution in the proximal tubule of male Wistar rats. After 48 hours of treatment, the proximal tubule epithelium showed tumefaction and necrosis. Dose of 200 mg/kg decreased the F-actin and was observed a structure in patches in the basal cytoplasm of epithelial cells of the proximal tubule. This effect was increased depending on the administered dose. Dose of 1000 mg/kg produced the highest histological damage and changes in the actin cytoskeleton. Results of this study suggested that nephrotoxic damage produced by high doses of APAP included breakdown of cytoskeleton in proximal tubule epithelium. 展开更多

目的:分析肾集合管癌的CT和MRI表现,提高对肾集合管癌诊断的准确性。方法:回顾性分析7例经病理证实的肾集合管癌的临床表现、影像学及病理学特征。其中5例行CT平扫及增强扫描,2例行MRI平扫及增强扫描。结果:7例中男4例,女3例。年龄44... 目的:分析肾集合管癌的CT和MRI表现,提高对肾集合管癌诊断的准确性。方法:回顾性分析7例经病理证实的肾集合管癌的临床表现、影像学及病理学特征。其中5例行CT平扫及增强扫描,2例行MRI平扫及增强扫描。结果:7例中男4例,女3例。年龄44~76岁,中位年龄58岁。肿瘤位于左肾5例,右肾2例。肿瘤直径3.6~6.7cm,平均5.2cm。7例均是实性肿块,形态不规则、边界不清。5例肾脏轮廓改变。CT平扫为等密度,T1WI为等或稍高信号,T2WI为低信号,密度或信号多不均匀,内可见片状坏死区,增强后呈轻度不均匀延迟强化。5例有腹膜后淋巴结肿大,其中1例有同侧肾上腺转移,1例有颈部淋巴结转移。3例有肾静脉癌栓形成。结论:肾集合管癌的CT和MRI表现有一定的特征性,在肾内出现以髓质为中心、边界不清、增强后轻度延迟强化肿块,早期即有淋巴结转移时应考虑此病的可能。 展开更多

目的探讨肾连接小管(CNT)过渡为集合管系的空间走行规律。方法灌流固定3只C57/BL/6J小鼠,取肾组织块并树脂812包埋,从肾被膜到肾乳头共得到2000张2.5μm厚的连续切片,显微镜图像数字化并配准,计算机程序追踪38条CNT走行并测量其长度。... 目的探讨肾连接小管(CNT)过渡为集合管系的空间走行规律。方法灌流固定3只C57/BL/6J小鼠,取肾组织块并树脂812包埋,从肾被膜到肾乳头共得到2000张2.5μm厚的连续切片,显微镜图像数字化并配准,计算机程序追踪38条CNT走行并测量其长度。结果来自长髓袢肾单位的CNT在胃皮质中上行,沿途与5~6条短髓袢肾单位CNT相互吻合成弓形管,弓形管或部分来自最浅表皮质的CNT在皮质浅层与集合管汇合。CNT各自走行约150~500μm,弓形管长度为600~900μm。结论肾单位只在皮质浅层经CNT或经弓形管汇入集合管系,集合管在穿过中层和深层皮质时不再有新的肾单位加入,提示肾小球滤过液在集合管下行过程中不再接受来自于其他肾单位的滤液,重吸收功能不受任何肾单位的影响。 展开更多

Background:Ischemic acute kidney injury(AKI)is a common syndrome associated with considerable mortality and healthcare costs.Up to now,the underlying pathogenesis of ischemic AKI remains incompletely understood,and specific strategies for early diagnosis and treatment of ischemic AKI are still lacking.Here,this study aimed to define the transcriptomic landscape of AKI patients through single-cell RNA sequencing(scRNA-seq)analysis in kidneys.Methods:In this study,scRNA-seq technology was applied to kidneys from two ischemic AKI patients,and three human public scRNA-seq datasets were collected as controls.Differentially expressed genes(DEGs)and cell clusters of kidneys were determined.Gene ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis,as well as the ligand-receptor interaction between cells,were performed.We also validated several DEGs expression in kidneys from human ischemic AKI and ischemia/reperfusion(I/R)injury induced AKI mice through immunohistochemistry staining.Results:15 distinct cell clusters were determined in kidney from subjects of ischemic AKI and control.The injured proximal tubules(PT)displayed a proapoptotic and proinflammatory phenotype.PT cells of ischemic AKI had up-regulation of novel pro-apoptotic genes including USP47,RASSF4,EBAG9,IER3,SASH1,SEPTIN7,and NUB1,which have not been reported in ischemic AKI previously.Several hub genes were validated in kidneys from human AKI and renal I/R injury mice,respectively.Furthermore,PT highly expressed DEGs enriched in endoplasmic reticulum stress,autophagy,and retinoic acid-inducible gene I(RIG-I)signaling.DEGs overexpressed in other tubular cells were primarily enriched in nucleotide-binding and oligomerization domain(NOD)-like receptor signaling,estrogen signaling,interleukin(IL)-12 signaling,and IL-17 signaling.Overexpressed genes in kidney-resident immune cells including macrophages,natural killer T(NKT)cells,monocytes,and dendritic cells were associated with leukocyte activation,chemotaxis,cell adhesion,and complement activation.In addition,the ligand-receptor interactions analysis revealed prominent communications between macrophages and monocytes with other cells in the process of ischemic AKI.Conclusion:Together,this study reveals distinct cell-specific transcriptomic atlas of kidney in ischemic AKI patients,altered signaling pathways,and potential cell-cell crosstalk in the development of AKI.These data reveal new insights into the pathogenesis and potential therapeutic strategies in ischemic AKI. 展开更多
