BACKGROUND Hypertrophic cardiomyopathy(HCM)is one of the most prevalent inherited myocardial disorders and is charac-terized by considerable genetic and phenotypic heterogeneity.A subset of patients with HCM progress ...BACKGROUND Hypertrophic cardiomyopathy(HCM)is one of the most prevalent inherited myocardial disorders and is charac-terized by considerable genetic and phenotypic heterogeneity.A subset of patients with HCM progress to a dilated phase of HCM(DPHCM),which is associated with a poor prognosis;however,the underlying pathogenesis remains inadequately understood.CASE SUMMARY In this study,we present a case involving a pedigree with familial DPHCM and conduct a retrospective review of patients with DPHCM with identified gene mutations.Through panel sequencing targeting the coding regions of 312 genes associated with inherited cardiomyopathy,a heterozygous missense mutation(c.746G>A,p.Arg249Glu)in the MYH7 gene was identified in the proband(III-5).Sanger sequencing subsequently confirmed this pathogenic mutation in three additional family members(II-4,III-4,and IV-3).A total of 26 well-documented patients with DPHCM were identified in the literature.Patients with DPHCM are commonly middle-aged and male.The mean age of patients with DPHCM was 53.43±12.79 years.Heart failure,dyspnoea,and atrial fibrillation were the most prevalent symptoms observed,accompanied by an average left ventricular end-diastolic size of 58.62 mm.CONCLUSION Our findings corroborate the pathogenicity of the MYH7(c.746G>A,p.Arg249Glu)mutation for DPHCM and suggest that the Arg249Gln mutation may be responsible for high mortality.展开更多

Objective: To explore the patterns of TCM syndromes and syndrome distribution of knee osteoarthritis through modern literature research. Methods: All journal articles from China Knowledge Network, Wanfang, and China B...Objective: To explore the patterns of TCM syndromes and syndrome distribution of knee osteoarthritis through modern literature research. Methods: All journal articles from China Knowledge Network, Wanfang, and China Biomedical Database from January to August 1, 2019 were searched, and documents that met the criteria for inclusion and exclusion were screened out. Knee joints were extracted on the premise of data standardization. Inflammatory syndrome types, syndromes, regional distribution of literature, and commonly cited literature of syndrome reference standards are used to establish a database using an Excel table for analysis using frequency analysis. Results: A total of 2027 articles were included. The geographical distribution of the included articles was mainly East China (23.53%), Central China (19.93%), and South China (19.24%). The commonly cited reference for the knee osteoarthritis syndrome reference standard is "Chinese medicine" Guiding Principles for Clinical Research of New Drugs (47.09%),"Traditional Chinese Medicine Diagnosis and Efficacy Standards" (40.27%), "Traditional Chinese Medicine Diagnosis and Treatment Plan for 95 Major Diseases in 22 Specialties" (4.58%), "Diagnosis and Treatment Plan for Knee Paralysis", " KOA TCM Experts' Consensus (2015 Edition) "," Traditional Chinese Medicine ", the common types of knee osteoarthritis are liver and kidney deficiency (17.65%), blood stasis (12.40%), wind cold and dampness (11.76%) , Cold dampness obstruction, yang deficiency and cold coagulation, liver and kidney deficiency, tendon and blood stasis, qi stagnation and blood stasis syndrome;common disease site syndromes are liver and kidney, and disease syndrome syndromes are dampness, cold, blood stasis, and yin Deficiency, wind, yang deficiency, heat, qi stagnation, fine deficiency, sputum, qi deficiency, and blood deficiency;the most common form of syndrome combination is the combination of two and three syndromes. Conclusion: TCM syndrome types and the distribution of syndrome elements are in line with the current mainstream understanding of the pathogenesis of knee osteoarthritis. The syndrome differentiation of knee osteoarthritis can summarize the main pathogenesis of the disease. Helps to classify TCM syndromes of knee osteoarthritis.展开更多

The presented research illustrates the applicability and productiveness of the systematic literature review methodology, a non-empirical methodology in the geological sciences, particularly volcanology. The systematic...The presented research illustrates the applicability and productiveness of the systematic literature review methodology, a non-empirical methodology in the geological sciences, particularly volcanology. The systematic literature review methodology is a replicable, rigorous, and transparent methodology for synthesizing existing literature to answer questions on a specific topic. The synthesis allows for knowledge consolidation, such as identifying knowledge gaps. In our illustration of this methodology, we focused on the expanding knowledge about the magma pathway at Mount Cameroon, one of Africa’s active volcanoes. Our synthesis of the relevant international geoscience research literature is based on the framework of knowledge about the magma pathway beneath a typical basaltic volcano. The framework has three primary components: magma supply, storage, and transport to erupting vents. Across these components is a total of twelve secondary components. The result is a previously non-existent and fragmented overall understanding of the magma pathway at Mount Cameroon. The gaps in the understanding (such as in the magma supply rates, timescales of chamber processes, and magma ascent rates) may be addressed in future research. Another key implication of the presented research lies in the proof of concept of the systematic literature review methodology as an applicable qualitative research methodology in the study of volcanoes.展开更多

Objective:To explore the feasibility of remotely obtaining complex information on traditional Chinese medicine(TCM)pulse conditions through voice signals.Methods: We used multi-label pulse conditions as the entry poin...Objective:To explore the feasibility of remotely obtaining complex information on traditional Chinese medicine(TCM)pulse conditions through voice signals.Methods: We used multi-label pulse conditions as the entry point and modeled and analyzed TCM pulse diagnosis by combining voice analysis and machine learning.Audio features were extracted from voice recordings in the TCM pulse condition dataset.The obtained features were combined with information from tongue and facial diagnoses.A multi-label pulse condition voice classification DNN model was built using 10-fold cross-validation,and the modeling methods were validated using publicly available datasets.Results: The analysis showed that the proposed method achieved an accuracy of 92.59%on the public dataset.The accuracies of the three single-label pulse manifestation models in the test set were 94.27%,96.35%,and 95.39%.The absolute accuracy of the multi-label model was 92.74%.Conclusion: Voice data analysis may serve as a remote adjunct to the TCM diagnostic method for pulse condition assessment.展开更多
According to current discipline classification,folk literature is usually categorized as an individual research field,distinguished from classical literature or classical canon,which leads to the fact that in current ...According to current discipline classification,folk literature is usually categorized as an individual research field,distinguished from classical literature or classical canon,which leads to the fact that in current research,the study of folk literature is often separated from the study of classical texts,or even opposed to it.This paper intends to review the relations and relationships between the two and argues that,at least,in ancient Greek world of culture,there is a considerable intersection between folk literature and classical literature.In many cases,a myth appeared both in the two literary fields,and thus these two fields were often intertwined and in conversation.Taking Athenian autochthony as a case study,this paper shows that this myth not only appeared as a folk story in daily occasions such as symposium,architecture,and vase painting,but also entered classic texts such as the tragic works of the three great tragedians and even Plato’s philosophical works,becoming one part of“classical literature”.By examining the application of the myth in folk literature and classical literature,it can be seen that in ancient Greece,folk and classical literature not only communicated and borrowed from each other,but also formed a dialogue between the two,thus constructing a lively and living literary world.展开更多

However, specific therapeutic communication methods have not been systematized. The purpose of this study was to clarify therapeutic communication methods for families/family members from the perspectives of verbal communication and non-verbal communication through a review of existing literature. Methods: We conducted a search using the medical literature databases PubMed and Ichushi-Web using the keywords “therapeutic communication”. Analysis was performed on seven articles from PubMed and 14 articles from Ichushi-Web that described therapeutic communication methods performed by healthcare professionals for families/family members. Through directed content analysis, therapeutic communication methods were subcategorized, and classified into three categories: verbal communication, non-verbal communication, and verbal/non-verbal communication. Results: A total of 23 subcategories were extracted. Verbal communication included 11 subcategories, such as “asking questions using the communicatee’s words as they are”. Non-verbal communication included five subcategories, such as “noticing changes in the content of the communicatee’s story”. And verbal/non-verbal communication featured seven subcategories, such as “making the communicatee aware of one’s own beliefs”. Conclusion: Therapeutic communication methods included basic care/caring in family interviews/meetings, as well as verbal communication and non-verbal communication that act on family/family members’ beliefs. It is believed that changes in family/family members’ beliefs can be used to eliminate, reduce, or improve problematic conditions in the family. .展开更多

BACKGROUND The presence of a right-sided sigmoid colon is a rare anatomical variation usually discovered incidentally during surgical interventions.This case report details an unexpected right-sided sigmoid colon iden...BACKGROUND The presence of a right-sided sigmoid colon is a rare anatomical variation usually discovered incidentally during surgical interventions.This case report details an unexpected right-sided sigmoid colon identified during a laparoscopic appendectomy and examines the pertinent literature to explore its clinical importance.CASE SUMMARY A 71-year-old woman presented with acute appendicitis.A preoperative computed tomography(CT)scan showed peri-appendiceal inflammation but no significant anatomical abnormalities.During laparoscopic surgery,an unexpected finding was encountered:The sigmoid colon was situated on the right side and exhibited an abnormal relationship with the cecum and ascending colon.Postoperative pathological examination confirmed appendicitis with no additional pathological findings.The right-sided sigmoid colon anomaly was verified through intraoperative assessment and later re-evaluation with CT and colonographic imaging.The patient underwent a laparoscopic appendectomy and experienced a smooth postoperative recovery.CONCLUSION This case highlights the necessity of being attentive to anatomical variations during laparoscopic surgery,particularly when managing appendicitis.A review of the literature indicated that the occurrence of a right-sided sigmoid colon is infrequent and may be associated with anomalies in midgut rotation during embryonic development.Awareness of this variation can help prevent surgical complications and inform future clinical practice.展开更多

文摘A Spanish businessman becomes a dedicated advocate of traditional Chinese medicine after witnessing its effects in treating his terminally ill young son.MY relationship with traditional Chinese medicine(TCM)-and with China-began in 1988,when I traveled to Beijing with my two-year-old son Joan.He was suffering from an extremely rare disease called leukoencephalopathy,in which the brain is affected,and we wanted to explore treatment possibilities.

文摘Objective:To explore the feasibility of remotely obtaining complex information on traditional Chinese medicine(TCM)pulse conditions through voice signals.Methods: We used multi-label pulse conditions as the entry point and modeled and analyzed TCM pulse diagnosis by combining voice analysis and machine learning.Audio features were extracted from voice recordings in the TCM pulse condition dataset.The obtained features were combined with information from tongue and facial diagnoses.A multi-label pulse condition voice classification DNN model was built using 10-fold cross-validation,and the modeling methods were validated using publicly available datasets.Results: The analysis showed that the proposed method achieved an accuracy of 92.59%on the public dataset.The accuracies of the three single-label pulse manifestation models in the test set were 94.27%,96.35%,and 95.39%.The absolute accuracy of the multi-label model was 92.74%.Conclusion: Voice data analysis may serve as a remote adjunct to the TCM diagnostic method for pulse condition assessment.

文摘According to current discipline classification,folk literature is usually categorized as an individual research field,distinguished from classical literature or classical canon,which leads to the fact that in current research,the study of folk literature is often separated from the study of classical texts,or even opposed to it.This paper intends to review the relations and relationships between the two and argues that,at least,in ancient Greek world of culture,there is a considerable intersection between folk literature and classical literature.In many cases,a myth appeared both in the two literary fields,and thus these two fields were often intertwined and in conversation.Taking Athenian autochthony as a case study,this paper shows that this myth not only appeared as a folk story in daily occasions such as symposium,architecture,and vase painting,but also entered classic texts such as the tragic works of the three great tragedians and even Plato’s philosophical works,becoming one part of“classical literature”.By examining the application of the myth in folk literature and classical literature,it can be seen that in ancient Greece,folk and classical literature not only communicated and borrowed from each other,but also formed a dialogue between the two,thus constructing a lively and living literary world.
文摘Background and Purpose: Therapeutic communication is a new term in family health care nursing, defined by Hohashi (2019) as a method of family intervention, and characterized by inclusion of not only verbal conversation but also nonverbal interaction. However, specific therapeutic communication methods have not been systematized. The purpose of this study was to clarify therapeutic communication methods for families/family members from the perspectives of verbal communication and non-verbal communication through a review of existing literature. Methods: We conducted a search using the medical literature databases PubMed and Ichushi-Web using the keywords “therapeutic communication”. Analysis was performed on seven articles from PubMed and 14 articles from Ichushi-Web that described therapeutic communication methods performed by healthcare professionals for families/family members. Through directed content analysis, therapeutic communication methods were subcategorized, and classified into three categories: verbal communication, non-verbal communication, and verbal/non-verbal communication. Results: A total of 23 subcategories were extracted. Verbal communication included 11 subcategories, such as “asking questions using the communicatee’s words as they are”. Non-verbal communication included five subcategories, such as “noticing changes in the content of the communicatee’s story”. And verbal/non-verbal communication featured seven subcategories, such as “making the communicatee aware of one’s own beliefs”. Conclusion: Therapeutic communication methods included basic care/caring in family interviews/meetings, as well as verbal communication and non-verbal communication that act on family/family members’ beliefs. It is believed that changes in family/family members’ beliefs can be used to eliminate, reduce, or improve problematic conditions in the family. .

文摘BACKGROUND The presence of a right-sided sigmoid colon is a rare anatomical variation usually discovered incidentally during surgical interventions.This case report details an unexpected right-sided sigmoid colon identified during a laparoscopic appendectomy and examines the pertinent literature to explore its clinical importance.CASE SUMMARY A 71-year-old woman presented with acute appendicitis.A preoperative computed tomography(CT)scan showed peri-appendiceal inflammation but no significant anatomical abnormalities.During laparoscopic surgery,an unexpected finding was encountered:The sigmoid colon was situated on the right side and exhibited an abnormal relationship with the cecum and ascending colon.Postoperative pathological examination confirmed appendicitis with no additional pathological findings.The right-sided sigmoid colon anomaly was verified through intraoperative assessment and later re-evaluation with CT and colonographic imaging.The patient underwent a laparoscopic appendectomy and experienced a smooth postoperative recovery.CONCLUSION This case highlights the necessity of being attentive to anatomical variations during laparoscopic surgery,particularly when managing appendicitis.A review of the literature indicated that the occurrence of a right-sided sigmoid colon is infrequent and may be associated with anomalies in midgut rotation during embryonic development.Awareness of this variation can help prevent surgical complications and inform future clinical practice.
