Liver cancer,one of the most common malignancies worldwide,ranks sixth in incidence and third in mortality.Liver cancer treatment options are diverse,inclu-ding surgical resection,liver transplantation,percutaneous ablation,transarterial chemoembolization,radiotherapy,chemotherapy,targeted therapy,immuno-therapy,and traditional Chinese medicine(TCM).A multidisciplinary team(MDT)is essential to customize treatment plans based on tumor staging,liver function,and performance status(PS),ensuring individualized patient care.Treatment decisions require a MDT to tailor strategies based on tumor staging,liver function,and PS,ensuring personalized care.The approval of new first-line and second-line drugs and the establishment of standard treatments based on immune checkpoint inhibitors have significantly expanded treatment options for advanced liver cancer,improving overall prognosis.However,many patients do not respond effectively to these treatments and ultimately succumb to the disease.Modern oncology treatments,while extending patient survival,often come with severe side effects,resistance,and damage to the body,negatively impacting quality of life.Huang et al's study published at World Journal of Gastroenterology rigorously validates the anticancer properties of Calculus bovis,enhancing our understanding of TCM and contributing to new liver cancer treatment strategies.For over 5000 years,TCM has been used in East Asian countries like China to treat various diseases,including liver conditions.Analysis of real-world clinical data suggests that for patients with advanced-stage tumors lacking effective treatments,integrated TCM therapies could provide significant breakthroughs. 展开更多

In this issue of World Journal of Gastroenterology,Huang et al reported that Calculus bovis(CB),a traditional Chinese herbal medicine,impedes the growth of liver cancers in vivo.Through further in vitro studies,they showed that CB suppressed the M2 polarization of tumor-associated macrophages by suppressing the Wnt signaling pathway,which consequently inhibited the growth of liver cancer.Although the effects of traditional Chinese herbal medicine are often not scientifically proven,Huang et al successfully identified the molecular mechanism involved in the anticancer effect of CB,and it is anticipated that the molecular mechanisms involved in the effects of other traditional Chinese herbal medicines will be scientifically elucidated,as demonstrated in this article. 展开更多

Primary liver cancer seriously threatens human health,affects the quality of life,and has a high mortality rate.Traditional Chinese medicine(TCM)has long been used in the prevention,treatment,and adjuvant treatment of primary liver cancer.Clinical trials were conducted in recent years to evaluate the efficacy and safety of TCM.The mechanism of action of TCM in primary liver cancer treatment also received extensive attention.This study summarizes the common monomial Chinese herbal medicines and their components,formulae,and the proprietary Chinese medicines used to treat primary liver cancer.Common medicines include Chansu(Bufonis venenum),Kushen(Sophorae flavescentis radix),Renshen(Ginseng radix et rhizoma),and Jianghuang(Curcumae longae rhizoma).This study also reviewed the literature on liver cancer treatment using TCM.The results confirmed the safety and efficacy of TCM in liver cancer treatment.The possible mechanisms involved are also listed,including influencing the cell cycle,inducing cancer cell apoptosis and autophagy,inhibiting cancer cell angiogenesis and metastasis,regenerating the microenvironment and DNA methylation,and regulating the innate immune response.For primary liver cancer patients who often have a long disease course,Chinese herbal medicine often plays a role in protecting and conditioning the body.Research aimed at finding more traditional Chinese herbal medicines and their mechanisms of action should be promoted.This study aimed to provide a scientific reference for further research and the rational clinical application of TCM for primary liver cancer treatment. 展开更多

Notably, current treatment strateg... Primary liver cancer(PLC) is one of the most common malignant tumors in China. PLC is characterized by insidious onset, rapid progress, poor quality of life, and short survival time. Notably, current treatment strategies remain unsatisfactory. Traditional Chinese medicines(TCM) have been used to treat a variety of diseases, including liver diseases, for more than 2000 years. In this study, we performed a review of the use frequency and clinical efficacy of TCM in treating PLC. Relevant literature from January 1, 2009, to January 1, 2021 was retrieved from network databases of China National Knowledge Infrastructure(CNKI), Chongqing VIP, Wanfang, PubMed, and SinoMed. The most frequently used TCM and their efficacy in PLC treatment were summarized. Based on the inclusion and exclusion criteria, 33 articles were selected. Overall, the efficacy of the combination of TCM and Western medicines in the treatment of PLC was higher than that in the control groups(i.e. treatment with Western medicines alone)(65.11% vs.44.31%, P <.05). Among the 33 selected articles, 11 were investigated for TCM preparation(marketed drugs) and 22 for TCM formulas. In total, 102 types of TCM(single herbs) were used to treat PLC. The top five most frequently used TCM were Poria(14.71%), Astragali radix(13.73%), Atractylodis Macrocephalae Rhizoma(12.75%), Bupleuri radix(12.75%), and Glycyrrhizae radix et Rhizoma(11.76%). Of the 102 types of TCM, tonics were the most frequently used categories, followed by heat-clearing medicines, blood-invigorating medicines, and stasis-resolving medicines. Of 207 papers, 174(84.06%) could not be subjected to statistical analysis due to research quality. Further high-quality research on herb sources, formula components and dosage, toxicology, and ethics of TCM is necessary. In conclusion, TCM play a promising role in the treatment and management of PLC, although further investigations are warranted. 展开更多

Method: Retrieval randomizedcontrolled trials (RCT) of TCM combined with chemotherapy in the treatment of the liver cancer from the networkdatabase (such as MEDLINE, EMBASE, PUBMED, Chinese CNKI database, Wanfang database, VIP databaseetc.). The retrieval period was from January 2011 to July 2017. Results: A total of 16 RCT studies were includedand involving 1873 patients, with 949 patients in the treatment (TCM with chemotherapy) group and 924 patientsin the control group (chemotherapy alone). Meta-analysis results suggest that TCM combined with chemotherapyin the treatment of primary hepatocellular carcinoma can improve curative effect, improve the degree of ascites ofpatients, improve the quality of life of patients and alleviate chemotherapy side effects. Conclusion: TCMcombined with chemotherapy in the treatment of primary liver cancer is helpful to improve the short term curativeeffect, improve the quality of life of patients, and alleviate the toxic and side effects. 展开更多

Objective:To explore the clinical drug use of Professor Liu Shangyi,a master of Chinese medicine,in the treatment of thyroid cancer based on the ancient and modern medical record cloud platform v1.5.7 software.Methods:From January 01,2018 to April 01,2019,the information input system of Professor Liu Shangyi,traditional Chinese Medicine Hospital of Guizhou Province,was screened.The data mining and analysis module was used to explore the clinical drug use of Professor Liu Shangyi in the treatment of thyroid cancer.Results:A total of 207 medical cases were recorded in this study,with 207 prescriptions,involving 129 kinds of herbs.The five most frequently used herbs are Ezhu(Curcumae Rhizoma),Biejia(Trionycis Carapax),Maozhuacao(Ranunculi Ternati Radix),Donglingcao(Rabdosia rubescens)and Chaojiezi(Sinapis Semen).Most of the drugs are warm and cold;most of the five flavors are sweet and bitter;most of the Meridian tropism are lung and liver;and the main efficacy are clearing away heat and toxic materials,eliminating phlegm and resolving hard mass.The most frequent correlation analysis of traditional Chinese medicines(TCM)are Bie Jia(Trionycis Carapax)-Ezhu(Curcumae Rhizoma),Donglingcao(Rabdosia rubescens)-Ezhu(Curcumae Rhizoma).Conclusion:Professor Liu Shangyi often use clearing away heat and toxic materials,resolving phlegm and disperse knot combined with tonifying liver and kidney in order to help fostering vital Qi and dispeling evil in clinical treatment of thyroid cancer. 展开更多

Aiming at starting the ball rolling and contributing humble effort to promote CTM (Chinese traditional medicine), we performed the present study to assess the therapy response of Chinese herbal decoction compared to conventional therapy on critical ill patients of advanced liver cancer. A total of 6 patients (1 female and 5 males) with histologically confirmed liver cancer were included in this retrospective observational clinical trial. We administered Chinese medicine (Gan Decoction, mixed with a variety of effective herbal components) to help them to recover from poor condition. In the meantime, conventional treatment of surgical resection and artery catheterization chemotherapy was applied in cases compared. In 3 cases of CTM combined treatment, the tumor marker level decreased. Residual intrahepatic metastatic sites reduced according to ultrasonography/CT imaging, and the patients felt free from the complaint of abdominal discomfort. The quality of life has been improved, we managed to have prolonged the PFS (Progression-Free-Survival) and TTP (Time-to-Progression) from the onset to date. While in 3 cases with conventional treatment only of surgical resection and artery catheterization chemotherapy, we were not able to decrease the level of tumor marker, metastatic lesions increased according to ultrasonography/CT imaging, and the patient's condition worsen more. We failed in having prolonged the PFS and TTP in the compared cases of conventional treatment only. The retrospective clinical study showed no OS (overall survival) benefit for liver cancer patients treated with Gan Decoction, while the QOL (quality of life) evaluation seemed to predict survival better. Chinese herbs might be an additional choice with its better benefits and tolerability in the treatment of primary liver cancer. 展开更多

Objective:To systematically evaluate the effectiveness and safety of traditional Chinese medicine(TCM)in the treatment of posttransarterial chemoembolization(TACE)syndrome of primary liver cancer.Methods:CNKI,VIP database,Wanfang database,CBM,PubMed,MEDLINE,and Cochrane library were retrieved,and the RCTs of Chinese medicine in the treatment of primary liver cancer TACE postoperative syndrome were included from the establishment of the database to March 2022.The Cochrane bias risk tool was used to evaluate the quality of included articles.RevMan 5.4 software was used for metaanalysis of outcome indicators and GRADEprofiler 3.6 software was used to evaluate the quality of evidence.Results:A total of 3992 patients were included in 44 studies.Metaanalysis results showed that compared with conventional western medicine treatment,combined Chinese medicine treatment could reduce the incidence of fever[RR=1.45,95%CI(1.09,1.92)],nausea and vomiting[RR=1.47,95%CI(1.10,1.95)],liver pain[RR=1.21,95%CI(1.02,1.43)];shorten the duration of fever[SMD=2.11.95%CI(1.43,2.79)],nausea and vomiting[SMD=1.67,95%CI(1.22,2.11)],hepatic pain[SMD=1.90,95%CI(1.30,2.51)],abdominal distension and pain[SMD=1.27,95%CI(0.75,1.79)],loose stool[SMD=2.30,95%CI(1.24,3.36)],constipation[SMD=2.18,95%CI(1.52,2.84)]after TACE,also it can improve patient quality of life[RR=0.73,95%CI(0.65,0.82)],clinical TCM syndrome[RR=0.73,95%CI(0.65,0.82)],shortterm efficacy[RR=0.75,95%CI(0.66,0.85)],oneyear survival rate[RR=0.87,95%CI(0.78,0.96)]and liver function and immune function.There was no significant difference in the incidence of abdominal distension and abdominal pain.Conclusion:Compared with only symptomatic western medicine after surgery,the combination of Chinese medicine has a significant effect and good safety in the treatment of postTACE syndrome. 展开更多

Objective:To assess the methodological quality of clinical research on Chinese medicine(CM)applied by infra-arterial infusion in treating primary liver cancer(PLC).Methods:Cochrane Central Register of Controlled Trials(CENTRAL),PubMed,and three Chinese databases,including Chinese BioMedical Database(CBM),China National Knowledge Infrastructure(CNKI)and China Academic Journal(VIP)were searched.Chinese articles were also searched manually in 16 journals.Two reviewers independently selected studies,the quality of literatures were assessed according to the Cochrane Collaboration method of quality assessment.Results:A total of 14 articles met the inclusion criteria for this review.Only three of these articles described the randomization method used.None of the studies was blinded.All of the articles didn't report the calculation of the sample size.Only six studies mentioned adverse reactions.All of the studies were of grade C according to the Cochrane Collaboration method.Six studies reported results of survival,and only two of these reported better efficacy in the treatment groups.Conclusions:The quality of studies concerned intra-arterial infusion of CM in treating with PLC was poor and the exact effect of these medicines still need evaluation.Well designed RCTs with large sample sizes,adequate follow-up data and reliable methods of assessment are needed to better appraise the real effect of CMs in the treatment of PLC patients. 展开更多

Objective: To identify specific Chinese medicines(CM) that may benefit patients with primary liver cancer(PLC), and to explore the mechanism of action of these medicines. Methods: In this retrospective, singlecenter study, prescription information from PLC patients was used in combination with Traditional Chinese Medicine Inheritance Supports System to identify the specific core drugs. A system pharmacology approach was employed to explore the mechanism of action of these medicines. Results: Taking CM more than 6 months was significantly associated with improved survival outcomes. In total, 77 putative targets and 116 bioactive ingredients of the core drugs were identified and included in the analysis(P<0.05). A total of 1,036 gene ontology terms were found to be enriched in PLC. A total of 75 pathways identified from Kyoto Encyclopedia of Genes and Genomes were also enriched in this disease, including fluid shear stress, interleukin-17 signaling, signaling between advanced glycan end products and their receptors, cellular senescence, tumor necrosis factor signaling, p53 signaling, cell cycle signaling,steroid hormone biosynthesis, T-helper 17 cell differentiation, and metabolism of xenobiotics by cytochrome. Docking studies suggested that the ingredients in the core drugs exert therapeutic effects in PLC by modulating c-Jun and interleukin-6. Conclusions: Receiving CM for 6 months or more improves survival for the patients with PLC. The core drugs that really benefit for PLC patients likely regulates the tumor microenvironment and tumor itself. 展开更多

The classification of Qi stagnation and blood stasis (QSBS) is the most common Zheng in liver cancer. This Zheng is frequently encountered in liver cancers falling into the pathological category of massive type. QSBS is the principal mechanism for a tumor to develop. Liver cancer with QSBS Zheng has characteristic clinical manifestations. Evidence from ex vivo, in vivo and clinical studies has been reported for the identification and management of liver cancer with QSBS Zheng. This article reviews evidence-based study advances of QSBS Zheng, the most common Zheng encountered in liver cancer. 展开更多

Objective: To investigate the new approach of two-step resection of primary massive liver cancer (PLC). Methods: 10 cases of primary massive liver cancer were treated with two-step resection after marked shrinkage of the tumor by integrating radiotherapy from June 1987 to December 1997. Step one: modified radiotherapy, including radiation of the whole liver by moving split fields and diminution field radiation, plus Traditional Chinese Medicine (TCM) of strengthening the spleen and regulating qi. The total dosage of the radiation was 50–60 Gy. Step two: 3–4 weeks after radiation, resecting the tumor. Results: the diameters of the masses shrinked from 11–15 cm to 6–8 cm after preoperative radiation and TCM. CT showed focus scan one hepatic portal lymph node (2 cm) and a satellite lesions around the tumor (1–3 cm) disappeared after treatment. In one AFP negative case survival cancerous cells was found. The operation did not cause death or serious postoperative complications. During followed-up, 10 cases survived more than half a year (100%), 8 more than one year (80%), 5 more than 3 years (50%), 4 more than 5 years (40%), 3 more than 7 years (30%) and 1 more than 13 years. Conclusion: The integration of moving split fields radiation of the whole liver, diminution field radiation technique, split course and TCM, followed by operation is a new approach to treat massive liver cancer. 展开更多

Objective:To investigate the clinical effect of Aidi injection combined with TACE on patients with primary liver cancer and its influence on serum AFP,bFGF,VEGF,inflammatory cytokines,immune function and quality of life.Methods From June 2017 to June 2019,116 patients with primary liver cancer from Shanghai Yangsi hospital and Zhongshan Hospital Affiliated to Fudan University were selected as the study objects.They were randomly divided into TACE group and TACE+Aidi group with 58 cases in each group.The TACE group was treated with TACE.The clinical efficacy,serum tumor markers,bFGF,VEGF,TGF-beta 1,oxidative stress,inflammatory cytokines,cellular immune function and quality of life were observed.Results The total effective rate was 89.66%in the case group,which was significantly higher than 74.14%in the TACE group,The difference was statistically significant(P<0.05).After treatment,the levels of alpha-fetoprotein(AFP),carbohydrate chain antigen(CA)242,CA724,basic fibroblast growth factor(bFGF),vascular endothelial growth factor(VEGF),transforming growth factor-beta 1(TGF-beta),malondialdehyde(MDA),interleukin(IL)-6,IL-10,tumor necrosis factor(TNF)-a,circulating immune complex(CIC),CD8+in the two groups were lower than those before treatment;total antioxidant capacity(TAOC),measurement The levels of superoxide dismutase(SOD),IL-12,CD3+,CD4+,CD4+/CD8+and QOL scores of quality of life were higher than those before treatment.The serum tumor markers,bFGF,VEGF,TGF-beta 1,oxidative stress,inflammatory cytokines,cellular immune function and quality of life in theTACE+Aidi group were significantly better than those in the TACE group.The difference was statistically significant(P<0.05).The incidence of adverse reactions in theTACE+Aidi group was 6.89%,which was lower than 13.78%in the TACE group,There was no significant difference(P>0.05).Conclusions Addie injection combined with transcatheter arterial chemoembolization is effective in treating primary liver cancer,reducing serum tumor markers,reducing toxic and side effects,regulating oxidative stress,reducing inflammatory reaction,reducing serum bFGF,VEGF and TGF-beta 1 levels,improving the quality of life of patients,and has high efficiency and safety,which is worthy of further promotion in clinic. 展开更多

Objective:To study the effect of sal ammoniac extract on the treatment of liver cancer and analyze its possibility of replacing absolute ethanol.Methods:Sixty Kunming mice(5-6 weeks old,18-22g in weight,male and female in half)were selected and inoculated with 0.1 ml of 1:4 ascitic diluent from mouse liver cancer H22 under the axilla of the right limb.After tumor formation,they were randomly divided into 3 groups with 20 mice in each group.Normal saline(NS),sal ammoniac extract(N),and absolute ethanol(E)were injected into the tumor once a day for four times.The death,tumor weight,tumor inhibition rate,lactate dehydrogenase(LDH),and succinate dehydrogenase(SDH)expression of the mice in each group were analyzed.Results:In the course of treatment,3 mice died in the normal saline group,2 mice died in the sal ammoniac treatment group,and 7 mice died in the absolute ethanol group.The tumor weight of the normal saline group,sal ammoniac treatment group,and absolute ethanol group were 1.880.26,1.41±0.49,and 1.51±0.46,respectively.The tumor inhibition rates were 0%,31.0%,and 21.6%,respectively.Comparing the sal ammoniac treatment group and the normal saline group,t=3.5133,p=0.0013;comparing the absolute ethanol group and the normal saline group,t=2.7926,p=0.0093.The expression of LDH was 81±10,51±11,and 32±9 in the normal saline group,sal ammoniac treatment group,and absolute ethanol group,respectively,while the expression of SDH was 80±10,5110,and 51±12 in the normal saline group,sal ammoniac treatment group,and absolute ethanol group,respectively.Comparing the LDH of the sal ammoniac treatment group with that of the saline group,t=8.4264,ρ=0.0000;comparing the absolute ethanol group and normal saline group,t=13.8763,p=0.0000.Comparing the SDH of the sal ammoniac treatment group with that of the normal saline group,t=8.1455,ρ=0.0000;comparing the absolute ethanol group with the normal saline group,t=7.2197,ρ=0.0000.Conclusion:The traditional Chinese antitumor medicine,sal ammoniac and its main effective components have good antitumor effect,which can be further popularized and applied in clinical practice. 展开更多

Prediction and validation of molecular biological mechanism of Fuzheng Huayu capsule in the treatment of liver cancer

Background:Fuzheng Huayu capsule(FZHY)combined with antiviral treatment has been shown to significantly reduce the risk of liver cancer in patients with hepatitis B cirrhosis.However,the potential of FZHY to directly treat liver cancer remains largely unknown.This study aims to investigate the molecular mechanism underlying the potential of FZHY in treating liver cancer.Methods:A network pharmacological analysis was performed using the Traditional Chinese Medicine Systems Pharmacology database to identify FZHY compounds and targets.Disease targets were searched using the Genecards database,and transcriptome data was downloaded from the NCBI database.Gene Ontology analysis was conducted using the DAVID database,and Kyoto Encyclopedia of Genes and Genomes analysis was based on KOBAS and bioinformatics methods.The Swissdock database was used for molecular docking.In cell experiments,the half inhibitory concentration(IC50)of FZHY was determined using the CCK8 method.The effects of FZHY on cell viability,apoptosis,and mitochondrial membrane potential were evaluated using a fluorescence microscope and flow cytometry.The molecular mechanism of FZHY in treating liver cancer was verified using quantitative polymerase chain reaction.Results:A total of 127 compounds and 184 proteins were identified as potential active ingredients and putative liver cancer-related targets.Additionally,1,899 liver cancer targets,279 transcriptome targets,and 3 pathways(p53 signaling pathway,apoptosis and PI3K-Akt pathway)were collected.The FZHY-targets-liver cancer interaction network was constructed.IC50 of FZHY lyophilized powder solution to liver cancer was 5.13 mg/mL(IC50=5.13 mg/mL).FZHY treatment led to an increase in the ratio of cell apoptosis and induced mitochondrial membrane potential damage,resulting in an increase in the number of dead cells.The expression levels of CCNB1 and BIRC5 were induced with FZHY treatment,while the expression levels of AKR1C3 and IGF2 were reduced.Conclusion:FZHY promotes apoptosis of liver cancer cells by acting on the p53 signaling pathway,apoptosis,and PI3K-Akt pathway.CCNB1,BIRC5,AKR1C3,and IGF2 are potential target proteins for FZHY in treating liver cancer. 展开更多

目的观察加味逍遥散对乳腺癌术后情绪改变肝郁脾虚证患者的临床疗效。方法将上犹县人民医院64例乳腺癌术后患者作为研究对象,各32例。对照组予心理干预,观察组加用加味逍遥散,两组均连续治疗8周。观察疗效、中医症状积分、免疫功能指标... 目的观察加味逍遥散对乳腺癌术后情绪改变肝郁脾虚证患者的临床疗效。方法将上犹县人民医院64例乳腺癌术后患者作为研究对象,各32例。对照组予心理干预,观察组加用加味逍遥散,两组均连续治疗8周。观察疗效、中医症状积分、免疫功能指标、炎症指标以及负性情绪、生活质量评分情况。结果观察组总有效率为78.12%(25/32),高于对照组的50.00%(16/32)(P<0.05)。与对照组比较,观察组炎症指标水平、负性情绪评分、中医症状积分更低,免疫功能指标、生存质量评分改善更显著(P<0.05)。两组均未出现明显不良反应(P>0.05)。结论加味逍遥散可以帮助乳腺癌术后患者提高免疫功能、缓解炎症、改善负性情绪、提高生活质量,且安全性好。 展开更多
