At present, unified and effective treatment for hypertension with insomnia is still lacking, traditional Chinese medicine (TCM) nursing technology can improve insomnia symptoms, reduce blood pressure, alleviate fatigue, improve anxiety, depression, improve the quality of life for patients with hypertension and insomnia. There is not only the application of the single TCM nursing technology, such as acupoint massage, acupoint application, ear acupoint pressure beans and so on, but also the application of two or a variety of TCM nursing techniques in hypertension with insomnia. In this article, the results of TCM nursing technology on hypertension with insomnia in recent years are reviewed in order to provide reference for the treatment of hypertensive with insomnia. 展开更多

Objective:To explore the clinical effect of combination of traditional Chinese medicine and western medicine in treatment of vascular aging in patients with type 2 diabetes complicated by hypertension.Methods:Ninety patients with type 2 diabetes complicated by hypertension admitted to our hospital from May 2016 to August 2019 were selected as research objects.They were randomly divided into control group and observational group,with 45 cases each.Control group was given amlodipine besylate combined with metformin hydrochloride.On the basis of control group,observational group was given combination of TCM syndrome differentiation.Blood glucose,blood pressure and blood lipids before and after 14 days of treatment were compared between two groups.Results:Blood glucose,blood pressure and lipid indexes after treatment were lower than before treatment in both groups;observational group was lower than control group and the difference was statistically significant(P<0.05).Conclusion:Combination of traditional Chinese medicine and Western medicine could lower blood glucose and blood pressure indexes,control blood lipids and delay blood vessel aging in patients with type 2 diabetes complicated by hypertension,it is worthy of clinical popularization. 展开更多

At present,hypertension has the phenomenon of increasing incidence and advancing age of onset.It can be complicated by various cardiovascular and cerebrovascular diseases and has a poor prognosis.Its pathogenesis is related to vascular endothelial injury and its mediated inflammation,blood flow,changes in mechanical force,and vascular remodeling.Vascular biomechanical changes are the early and critical pathogenic factors of this disease.Mechanosensitive ion channels and their proteins affect the pathological process of hypertension by improving cell membrane tension,regulating vascular tension and contractility,and participating in vascular biomechanical changes such as vascular remodeling.However,the drugs currently used to treat high blood pressure only slow the progression of the disease.Therefore,it is necessary to find and study effective medicines to treat hypertension.At present,modern medical methods do not have targeted treatment measures.Traditional Chinese medicine plays a significant role in treating this disease based on the principle of“treatment according to the syndrome”.Traditional Chinese medicine regulates the mechanical force of blood flow and maintains blood pressure balance in patients with hypertension.There are many kinds of Chinese patent medicines used to treat hypertension with good curative effects. 展开更多

Effect of a traditional Chinese medicine theory-based mobile app on improving symptoms in patients with type 2 diabetes mellitus:A randomized controlled trial

Objective:The aim of this study was to evaluate the effect of the Traditional Chinese Medicine(TCM)theory-based mobile app on improving symptoms in patients with type 2 diabetes mellitus(T2DM)in China.Materials and Methods:A randomized controlled trial was conducted.Ninety-seven patients with T2DM were enrolled and they were randomized into the control group and the experimental group.The control group was given usual diabetes education and follow-up with telephone call,while the experimental group was followed up by using the TCM theory-based mobile app.After 6-month intervention,the diabetic symptom scores,the blood glucose levels,and self-management ability of the two groups were compared.Results:Compared with intervention before,the levels of fasting blood glucose(FBG),2-h postprandial blood glucose(2hPG),and glycated hemoglobin(HbA1c)decreased in the control group(9.28±3.05 vs.8.26±2.73 mmol/L,12.65±4.18 vs.10.65±3.79 mmol/L,and 8.78%±1.76%vs.7.96%±1.47%,respectively,all P<0.05)and also decreased in the experimental group(8.36±2.64 vs.7.35±2.45 mmol/L,11.50±4.29 vs.9.85±2.71 mmol/L,and 8.64%±2.04%vs.7.29%±1.56%,respectively,all P<0.05)after 6-month intervention,while the summary of diabetes self-care activities(SDSCAs)scale scores after 6-month intervention increased significantly in the control group(35.20±14.68 vs.44.78±10.21,P<0.05)and the experimental group(32.16±13.21 vs.50.37±10.06,P<0.05).After 6-month intervention,the diabetic symptom scores(18.16±2.74 vs.22.18±4.77)and HbA1c(7.29±1.56%vs.7.96%±1.47%)were lower,and SDSCA scale scores(50.37±10.06 vs.44.78±10.21)were higher in the experimental group than the control group(all P<0.05).Conclusion:The mobile app based on TCM theory can effectively improve diabetes-related symptoms in patients with T2DM and help control their blood glucose as well as enhance their self-management ability. 展开更多

Objective: The aim of this study was to compare the clinical emcacy of integration of traditional Chinese medicine (TCM) and we stern medicine, and the western medicine therapy for H type hypertension. Methods: The randomized contmlled trials were collected via searching the internet. The literatures were screened according to inclusion and exclusion criteria. All the assignments were performed by two researchers. If there was disagreement between the two researchers, we would ask evidence based medicine expert for advice, to get an optimal result. The database included the ChianInfo (1978—2015), the Superstar database (1955—2017), the CBM(1985—2017), CNKI(1984—2017), PubMed(1966—2017). Accordingto Meta analysis, the RevMan 5.2 software was used to recover and analyze the literatures, and make the assessment of the integration of TCM and westem medicine in treating H Type Hypertension. Results: In the study, 12 trials were included. The total number of cases is 1139. The Meta analysis results showed that: Compared with western medicine the integration of TCM and western medicine would improve the antihypertensive efficacy [OR=0.39, 95%CI (0.25, 0.59), Z=4.38(P< 0.0001)],lower Hcy levels [WMD=-1.93, 95%CI (-2.27, 0.21), Z=4.38 (P< 0.0001)]. Conclusion: The combination therapy can effectively reduce Hcy levels, control blood pressure, better than Western medicine. 展开更多

Objective:To systematically evaluate the clinical efficacy of traditional Chinese medicine(TCM)in auxiliary treatment of grade 1 hypertension.Methods:The China National Knowledge Infrastructure,Wan Fang Data,VIP,PubMed were searched to screen the literature of randomized controlled trials of TCM or non-drug therapies assisting conventional medicine to treat grade 1 hypertension.The search time was from the establishment of the database to October 1,2020.Revman5.3 was used for meta-analysis.Results:8 Chinese literatures were included.The results of the study showed that 24h diastolic blood pressure(DBP),24h systolic blood pressure(SBP),DBP lowering effect and TCM syndrome curative efficacy of the experimental group using TCM was better than that of the control group.However,TCM combined conventional medicine did not show an advantage on SBP lowering effect,antihypertensive efficacy rate and total clinical efficacy rate.Conclusion:TCM has a significant effect on lowering blood pressure for adjuvant treatment of grade 1 hypertension and can significantly improve clinical symptoms. 展开更多

Objective To compare knowledge about hypertension between elderly Chinese urban patients with preferences for either traditional Chinese medicine (TCM) or Western medicine (WM). Methods Elderly (≥ 65 years old) patients with hypertension who prefer TCM treatment (n=112) or WM (n=126) were questioned about hypertension. Their answers were compared. Results Only 32.6% of participants correctly identified hypertension as a main risk factor of coronary heart disease and stroke, 22.3% of patients answered that the main purpose of hypertension control was preventing cardiovascular disease. Other major reasons for these patients to seek medical treatment for their hypertension included: persuasion by physicians or their family members (21.6%), alleviating symptoms such as headache and dizziness (16.8%), lowering blood pressure without knowing specific reason (12.4%). The predictors for poor knowledge of hypertension were similar irrespective of preference for WM or TCM treatment, and included those with lower levels of education and older age. Television and newspaper (46.8%) were the most frequent sources of hypertension information for both groups. Among those who preferred TCM treatment, "TCM has fewer side effects than WM" and"TCM cures disease while WM only alleviates symptoms" were common beliefs held. Conclusion This study shows that knowledge of hypertension is similar among Chinese urban patients with preferences for either WM or TCM treatment and that misunderstandings about hypertension are common among the elderly patients. In order to control hypertension effectively, public health education is necessary. This should target those with a lower level of education and older age. 展开更多

[Objectives]The research aimed to explore the distribution characteristics of TCM constitution types of patients with hypertension and insomnia,and study the clinical characteristics of patients with different constitutions,in order to provide new ideas for the treatment of patients with hypertension and insomnia.[Methods]Cross sectional observation method was used,and 420 patients with hypertension and insomnia were selected.Required information was collected,and the constitution type of traditional Chinese medicine was determined,and relevant data were recorded.SPSS and Logistic regression analysis method were used to explore the correlation between the distribution of TCM constitution types and gender,age,24 h-SBP,24 h-DBP,24 h-BPV,PSQI score,etc.[Results]Among 420 patients,the proportion of gentleness constitution was the most,and others in turn were Qi deficiency constitution>Yang deficiency constitution>phlegm dampness constitution>Qi stagnation constitution>Yin deficiency constitution>blood stasis constitution>damp heat constitution>special constitution.Among male patients,the proportion of gentleness constitution was the most.Among female patients,the proportion of Qi deficiency constitution was the most.In each constitution,the proportion of men and women was different,and the difference in gentleness constitution,Qi deficiency constitution and Yin deficiency constitution had statistical significance(P<0.05).The proportion of gentleness constitution for young and middle-aged patients was the most,while elderly patients with Qi deficiency constitution was the most.There was difference in the distribution of TCM constitution in different age groups,and the difference had statistical significance(P<0.05).Compared with the patients with gentleness constitution,the patients with Qi deficiency constitution,Yang deficiency constitution,Yin deficiency constitution,damp heat constitution,blood stasis constitution and Qi stagnation constitution had different differences in terms of age,24 h-SBP,24 h-DBP,24 h-BPV and PSQI score,and there was statistical significance(P<0.05).Except damp heat constitution,blood stasis constitution and special constitution,other constitutions had certain correlation with age,24 h-SBP,24 h-DBP,24 h-BPV and PSQI score.[Conclusions]TCM constitution types of patients with hypertension and insomnia were dominant by gentleness constitution,Qi deficiency constitution and Yang deficiency constitution.The distribution of TCM constitution in different gender and age groups was different,and different TCM constitution was related to ABPM and PSQI. 展开更多

METHODS: First 60 patients with hypertension of kidney Qi deficiency type were divided into control group and treatment group randomly; the conventional western medicine was given to patients of the control group for antihypertensive therapy, while the traditional Chinese medicine Bushenjiangya decoction was added for patients in the treatment group based on the therapy for those in control group; after 2 months, the blood pressure, clinical efficacy and urine microalbumin changes of patients in the two groups before and after the treatment were observed. RESULTS: a. The systolic pressure of patients in the two groups was all reduced obviously(P<0.05); b. The total effective rate of experiment group in antihypertensive effect was 76.7%, while that of control group was 83.3%(P>0.05); c. The total effective rate of experiment group in TCM syndrome effect was 86.7%, while that of control group was 63.3%, showing that the effect of experiment group was better than that of control group(P<0.05); d. The 24 h urine microalbumin reduction of experiment group was better than that of control group(P<0.05). CONCLUSIONS: The curative effect of Bushenjiangya decoction in combination with the western medicine is better than that of simple western medicine for hypertension of kidney Qi deficiency type, and also the urine microalbumin can be obviously reduced to protect the kidney function. 展开更多

The novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is the cause of a rapidly spreading illness,coronavirus disease 2019(COVID-19),affecting more than seventeen million people around the world.Diagnosis and treatment guidelines for clinicians caring for patients are needed.In the early stage,we have issued"A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV)infected pneumonia(standard version)";now there are many direct evidences emerged and may change some of previous recommendations and it is ripe for develop an evidence-based guideline.We formed a working group of clinical experts and methodologists.The steering group members proposed 29 questions that are relevant to the management of COVID-19 covering the following areas:chemoprophylaxis,diagnosis,treatments,and discharge management.We searched the literature for direct evidence on the management of COVID-19,and assessed its certainty generated recommendations using the Grading of Recommendations,Assessment,Development and Evaluation(GRADE)approach.Recommendations were either strong or weak,or in the form of ungraded consensus-based statement.Finally,we issued 34 statements.Among them,6 were strong recommendations for,14 were weak recommendations for,3 were weak recommendations against and 11 were ungraded consensus-based statement.They covered topics of chemoprophylaxis(including agents and Traditional Chinese Medicine(TCM)agents),diagnosis(including clinical manifestations,reverse transcription-polymerase chain reaction(RT-PCR),respiratory tract specimens,IgM and IgG antibody tests,chest computed tomography,chest X-ray,and CT features of asymptomatic infections),treatments(including lopinavirritonavir,umifenovir,favipiravir,interferon,remdesivir,combination of antiviral drugs,hydroxychloroquine/chloroquine,interleukin-6 inhibitors,interleukin-1 inhibitors,glucocorticoid,qingfei paidu decoction,lianhua qingwen granules/capsules,convalescent plasma,lung transplantation,invasive or noninvasive ventilation,and extracorporeal membrane oxygenation(ECMO)),and discharge management(including discharge criteria and management plan in patients whose RT-PCR retesting shows SARS-CoV-2 positive after discharge).We also created two figures of these recommendations for the implementation purpose.We hope these recommendations can help support healthcare workers caring for COVID-19 patients. 展开更多

BACKGROUND Polycystic ovary syndrome(PCOS) is characterized by hyperandrogenism, hyperinsulinemia, ovarian polycystic changes, and irregular ovulation, often occurring in women of childbearing age for whom it can be a cause of infertility. Hypothalamus-pituitary-ovarian axis dysregulation is important in the pathogenesis of PCOS and the associated chronic excess of sex hormones can lead to cardiovascular and cerebrovascular diseases, diabetes, and malignancies such as endometrial cancer, and breast cancer. At present, most scholars agree that lifestyle interventions in conjunction with drug treatment can help PCOS patients achieve their goals of successful pregnancy and childbirth.AIM To investigate the clinical effect of an online and offline(O2O) preventive health management model on PCOS with kidney deficiency and phlegm dampness.METHODS A total of 82 patients with PCOS of the kidney deficiency and phlegm dampness type who were admitted to Beijing Luhe Hospital Affiliated to Capital Medical University from April 2019 to June 2020 were randomly divided into two groups. The treatment group was treated with oral Diane-35 for 3 mo and received preventive O2O medical health management for 6 mo(including eating and living, exercise, drug management). The control group was treated with oral Diane-35 for 3 mo and completed outpatient health education. The traditional Chinese medicine(TCM) syndrome score, acne score, hair score, sex hormone level and clinical effects were compared between the two groups before and after the intervention. RESULTS After treatment, the TCM syndrome score, acne score, and serum luteinizing hormone/follicle stimulating hormone level were significantly lower in the treatment group than in the control group(P < 0.05). After 3 mo of treatment, the TCM syndrome curative effect index in the treatment group was 97.30% compared to 54.05% in the control group(P < 0.05), whereas the total treatment effect in the treatment group was 91.89%, compared to 54.05% in the control group(P < 0.05).CONCLUSION An integrated therapeutic approach incorporating medication, TCM methods and social media is more effective than standard treatment for PCOS. 展开更多

Objective:Jue tone is a kind of sound,using 3-mi as the main tone,and is melodious,profound,makes people feel comfortable and pleasant.This study aimed to explore the probable mechanism of Jue tone to reduce blood pressure(BP)in hypertensive rats with a liver-fire hyperactivity pattern by observing changes in BP as well as physiochemical indexes in plasma.Methods:Sixteen male spontaneous hypertensive rats(SHR)with a liver-fire hyperactivity pattern were randomly divided into a control group and a Jue tone group.The rats in the Jue tone group were treated with Jue tone(55e65 dB,played by the five elements of music rhythm instrument,a kind of physiotherapy regimen music played by Shen Wu)once a day for 4 weeks.The BP levels in each group were measured twice a week,on Monday and Friday.The levels of angiotensin II(Ang-II),thromboxane B2(TXB2),endothelin-1(ET-1),calcitonin gene-related peptide(cGRP),norepinephrine(NE),cortisol(CORT),and 5-hydroxytryptamine(5-HT).in plasma were detected by enzyme-linked immunosorbent assay after 4 weeks of intervention.Results:BP and the levels of TXB2 and ET-1 in the plasma of the treatment group were significantly lower than those of the control group,while the level of cGRP was significantly higher.Conclusion:Jue tone can reduce the BP of hypertensive rats with a liver-fire hyperactivity pattern.The mechanism may correlate with a reduction in TXB2 and ET-1 and an increase in cGRP with the reduction of reactive oxygen species(ROS). 展开更多

BACKGROUND: Pulse wave analysis(PWA) quantifies the phenomenon of pulse waveform propagation in patients with cardiovascular diseases, whereas pulse image analysis(PIA) is a subjective examination in traditional Chinese medicine. OBJECTIVE: This study evaluated the association of PIA with PWA and hemodynamics in patients with hypertension. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS: This observational, cross-sectional study enrolled 45 patients(26 men,(55.2± 10.3) years, systolic blood pressure(155± 28) mm Hg, diastolic blood pressure(93± 17) mm Hg) for assessment of clinical and laboratorial data. MAIN OUTCOME MEASURES: Primary outcomes comprised: pattern differentiation based on an automated method; PIA at the radial artery using the ‘simultaneous pressing' method for identification of factors such as strength(strong/weak), depth(superficial/deep), and speed(fast/moderate/slow); and PWA at the same artery using a noninvasive system. RESULTS: Significant multivariate main effects were observed for depth(l=0.648, F_(5,29)=3.149, P=0.022, h^2 =0.352), strength(l=0.608, F_(5,29) =3.736, P=0.010, h^2 =0.392), and speed(l=0.535, F_(5,29) =5.302, P=0.002, h^2 =0.465). General effects comprised high values of PWA and blood pressure for superficial, strong, and fast pulse images. A strong pulse was found for pulse pressure ≥ 62.5 mm Hg and systolic blood pressure ≥ 149.5 mm Hg, whereas a superficial pulse was found for heart rate ≥ 58.25 beats/min; a fast pulse was found for heart rate ≥ 69.6 beats/min and pulse wave velocity ≥ 9.185 m/s. CONCLUSION: Associations were explained by La Place's law, arterial remodeling in hypertension, alongside the traditional criterion for classifying speed in pulse images. PIA is associated with PWA and hemodynamics in patients with hypertension. Systolic and pulse pressures, heart rate, and pulse wave velocity are quantitative variables that have information to describe the qualitative pulse images such as strength, depth and speed. 展开更多
