Objective This study aimed to explore the clinical value of Children Neuropsychological and Behavioral Scale-Revision 2016(CNBS-R2016)for Autism Spectrum Disorder(ASD)screening in the presence of developmental surveillance.Methods All participants were evaluated by the CNBS-R2016 and Gesell Developmental Schedules(GDS).Spearman’s correlation coefficients and Kappa values were obtained.Taking GDS as a reference assessment,the performance of the CNBS-R2016 for detecting the developmental delays of children with ASD was analyzed with receiver operating characteristic(ROC)curves.The efficacy of the CNBS-R2016 to screen for ASD was explored by comparing Communication Warning Behavior with Autism Diagnostic Observation Schedule,Second Edition(ADOS-2).Results In total,150 children aged 12–42 months with ASD were enrolled.The developmental quotients of the CNBS-R2016 were correlated with those of the GDS(r=0.62–0.94).The CNBS-R2016 and GDS had good diagnostic agreement for developmental delays(Kappa=0.73–0.89),except for Fine Motor.There was a significant difference between the proportions of Fine Motor,delays detected by the CNBS-R2016 and GDS(86.0%vs.77.3%).With GDS as a standard,the areas under the ROC curves of the CNBS-R2016 were above 0.95 for all the domains except Fine Motor,which was 0.70.In addition,the positive rate of ASD was 100.0%and 93.5%when the cut-off points of 7 and 12 in the Communication Warning Behavior subscale were used,respectively.Conclusion The CNBS-R2016 performed well in developmental assessment and screening for children with ASD,especially by Communication Warning Behaviors subscale.Therefore,the CNBS-R2016 is worthy of clinical application in children with ASD in China. 展开更多

Children with autism spectrum disorders(ASD)or autism are more prone to gastrointestinal(GI)disorders than the general population.These disorders can significantly affect their health,learning,and development due to various factors such as genetics,environment,and behavior.The causes of GI disorders in children with ASD can include gut dysbiosis,immune dysfunction,food sensitivities,digestive enzyme deficiencies,and sensory processing differences.Many studies suggest that numerous children with ASD experience GI problems,and effective management is crucial.Diagnosing autism is typically done through genetic,neurological,functional,and behavioral assessments and observations,while GI tests are not consistently reliable.Some GI tests may increase the risk of developing ASD or exacerbating symptoms.Addressing GI issues in individuals with ASD can improve their overall well-being,leading to better behavior,cognitive function,and educational abilities.Proper management can improve digestion,nutrient absorption,and appetite by relieving physical discomfort and pain.Alleviating GI symptoms can improve sleep patterns,increase energy levels,and contribute to a general sense of well-being,ultimately leading to a better quality of life for the individual and improved family dynamics.The primary goal of GI interventions is to improve nutritional status,reduce symptom severity,promote a balanced mood,and increase patient independence. 展开更多

Exploring the Lives of Mothers Raising Children with Autism Spectrum Disorder in Japan

Background: Examining the lives that mothers experience and build will allow us to deepen our understanding of children with ASD and their mothers and facilitate developing support methods. The study aimed to examine the lives of mothers raising children with autism spectrum disorder (ASD) and investigate their sources of support. Method: We conducted a qualitative inductive study using semi-structured interviews to identify characteristics of the lives that mothers have created. Results: Semi-structured interviews were conducted with 11 mothers having children with ASD. The analysis comprised three stages of coding and yielded eight categories. The lives of these mothers contained three themes: preoccupation with parenting children with ASD and their siblings;evolving mother;and using social resources. Mothers engaged in “assessing the characteristics, growth, and changes in the child with ASD”, had a “preoccupation with parenting children with ASD”, and were “thinking about the future of the child with ASD”, and “having goals and plans for parenting” while having “consideration toward the child’s siblings”. During this process, mothers experienced “changes in perspective or approach” and created lifestyles while “receiving help from people around them” and engaged in the “use of social resources”. Conclusions: To avoid becoming preoccupied with parenting and being burdened by their lifestyle, mothers require social support to monitor their perceptions. Furthermore, the utilization of social resources requires the supporting individuals to understand the characteristics of children with ASD, provide appropriate information, and assist in decision-making. 展开更多

Dimensional Characteristics of Children and Adolescents with Mood Disorders and Autism Spectrum Disorders

Objective: We sought to identify clinical discriminators between predominantly mood disordered and predominantly autism spectrum disordered research subjects that may reflect phenotypic state and treatment response characteristics. Method: Participants were 26 boys and 4 girls aged 2 to 18 years (Mean Age = 7.70). Subjects with DSM-IV diagnoses of Major depression (N = 2), Bipolar Disorder (N = 4) and Mood Disorder not otherwise specified (NOS) (N = 11) represented the mood disorder group (MD) (N = 17, Mean Age = 8.2) and those with diagnoses of Autistic Disorder (N = 1), Asperger’s Disorder (7) or Pervasive Developmental Disorder (NOS) (N = 3) comprised the autism spectrum disorder (ASD) group (N = 9, Mean Age = 6.8). Primary outcome measurements were continuous actigraphic measurements collected over one to three week periods. Secondary outcomes included personality and observational measurements. Personality characteristics reflected significant cross-group impairments related to self-control and self-discipline and differed relative to intellectual measures. Observational measurements reflected greater general impairments among the ASD group. Results: Predominantly mood disordered children demonstrated greater impairments related to sleep (P = 0.000) and sleep onset latency (P = 0.000) and were more active than ASD children during evening periods (P = 0.000). ASD children had lower verbal functioning and greater deviations from the norm on measures of cognitive development (P = 0.003) and psychosis (P = 0.047). Conclusions: Sleep disturbances, evening activity levels and phase delayed sleep appear to differentiate predominant mood and autism spectrum disordered children suggesting future areas for further exploration of neurological and phenotypic treatment response characteristics. 展开更多

BACKGROUND Autism spectrum disorder(ASD)is a neurodevelopmental condition characterized by deficits in social communication and repetitive behaviors.Metabolomic profiling has emerged as a valuable tool for understanding the underlying metabolic dysregulations associated with ASD.AIM To comprehensively explore metabolomic changes in children with ASD,integrating findings from various research articles,reviews,systematic reviews,meta-analyses,case reports,editorials,and a book chapter.METHODS A systematic search was conducted in electronic databases,including PubMed,PubMed Central,Cochrane Library,Embase,Web of Science,CINAHL,Scopus,LISA,and NLM catalog up until January 2024.Inclusion criteria encompassed research articles(83),review articles(145),meta-analyses(6),systematic reviews(6),case reports(2),editorials(2),and a book chapter(1)related to metabolomic changes in children with ASD.Exclusion criteria were applied to ensure the relevance and quality of included studies.RESULTS The systematic review identified specific metabolites and metabolic pathways showing consistent differences in children with ASD compared to typically developing individuals.These metabolic biomarkers may serve as objective measures to support clinical assessments,improve diagnostic accuracy,and inform personalized treatment approaches.Metabolomic profiling also offers insights into the metabolic alterations associated with comorbid conditions commonly observed in individuals with ASD.CONCLUSION Integration of metabolomic changes in children with ASD holds promise for enhancing diagnostic accuracy,guiding personalized treatment approaches,monitoring treatment response,and improving outcomes.Further research is needed to validate findings,establish standardized protocols,and overcome technical challenges in metabolomic analysis.By advancing our understanding of metabolic dysregulations in ASD,clinicians can improve the lives of affected individuals and their families. 展开更多

Environmental Exposure to Lead, Vanadium, Copper and Selenium: Possible Implications in the Development of Autism Spectrum Disorders

Human exposure to toxic metals is on the increase especially in the developing world;this is compounded by the almost unavoidable application of the metals domestically and industrially and their implication in several genetic defects, aging and some chronic illnesses including Autism Spectrum Disorders (ASD). This study investigated the concentration of toxic metals (Pb and V) and micro-essential elements (Cu and Se) in children with ASD and controls in Nigeria towards establishing their possible associations with the aetiopathogenesis of ASD. Eight children clinically diagnosed by Paediatric Neurologist and Child Psychiatrist for ASD using DMS-IV and fifteen apparently healthy children (age range 2 - 12 years) were recruited as cases and controls respectively. Plasma levels of Pb, V, Cu and Se were analyzed using Induction ICP-MS. Results were analyzed using students t-test. The mean plasma lead and vanadium levels were (7.92 ± 1.30 μg/dl;1.07 ± 0.22 μg/dl) and (6.83 ± 0.72 μg/dl;2.59 ± 0.48 μg/dl) in children with ASD and in controls respectively. The result showed that blood lead level in ASD was slightly increased but not significant when compared with control (p < 0.433). On the other hand, plasma vanadium concentration in ASD was significantly reduced (1.07 ± 0.22 μg/dl) when compared with control (2.59 ± 0.48 μg/dl) (P < 0.038). Mean plasma copper was similar in all participants (1.98 ± 0.13, 2.23 ± 0.12) but selenium concentrations were significantly reduced (0.37 ± 0.05 mg/L;0.57 ± 0.02 mg/L) in ASD relative to controls respectively. Given the physiological functions of vanadium and selenium, the observed reduced levels of the two elements in children with ASD may account for the speech and other neurological dysfunctions of the brain in ASD. 展开更多

Play is a pleasurable physical or mental activity that enhances the child’s skills involving negotiation abilities,problem-solving,manual dexterity,sharing,decision-making,and working in a group.Play affects all the brain's areas,structures,and functions.Children with autism have adaptive behavior,adaptive response,and social interaction limitations.This review explores the different applications of play therapy in helping children with autism disorder.Play is usually significantly impaired in children with autism.Play therapy is mainly intended to help children to honor their unique mental abilities and developmental levels.The main aim of play therapy is to prevent or solve psychosocial difficulties and achieve optimal child-healthy growth and development.Play therapy helps children with autism to engage in play activities of their interest and choice to express themselves in the most comfortable ways.It changes their way of self-expression from unwanted behaviors to more non-injurious expressive behavior using toys or activities of their choice as their words.Play therapy also helps those children to experience feeling out various interaction styles.Every child with autism is unique and responds differently.Therefore,different types of intervention,like play therapy,could fit the differences in children with autism.Proper evaluation of the child is mandatory to evaluate which type fits the child more than the others.This narrative review revised the different types of play therapy that could fit children with autism in an evidence-based way.Despite weak evidence,play therapy still has potential benefits for patients and their families. 展开更多

This work was carried out to investigate the efficacy of static and intermittent gravity inverted therapy techniques in improvement of motor apraxia and cognitive abilities in Autism Spectrum Disorder (ASD). 30 children were randomly joined in this study into two groups;Group A (sensory integration therapy approach plus static and intermittent inversion therapy techniques) and Group B (sensory integration therapy approach only). Block design test was used to locate and follow constructional apraxia, computerized information processing speed test to locate and follow cognitive processing and grooved pegboard test was used to locate and follow fine motor skills. The variance between pre- and post-treatment results was highly significant in the study group in all variables. By comparison of the two groups in constructional apraxia variables test at post-treatment, there was a highly representative elevation in favour to study groups (p < 0.05). By comparison of the two groups in cognitive processing variable at post-treatment, there was a highly representative elevation in favour to study group (p < 0.05). By comparison of the two groups in fine motor skills variable at post-treatment, there was a highly representative elevation in favor to study group (p < 0.05). According to the outcomes of this study, it can be terminated that the sensory integration therapy approach plus static and intermittent inversion therapy techniques can be recommended in improvement constructional apraxia and cognitive processing abilities in autism spectrum disorder (ASD) children. 展开更多

Background Neurodevelopmental disorders are a heterogeneous group of conditions that manifest as delays or deviations in the acquisition of expected developmental milestones and behavioral changes.Autism spectrum disorder(ASD)is a neurodevelopmental disorder characterized by deficits in communication and social interaction and by repetitive and restricted patterns of behavior,interests and activities.The aim of this review is to discuss the clinical features of the differential diagnoses of ASD that are prevalent among preschoolers,focusing on their similarities and disparities.Data sources The international medical literature search was conducted using PubMed and was revised regarding the subject using single and/or combined keywords as follows:differential diagnosis,preschoolers,diagnostic challenge,attention deficit hyperactivity disorder,intellectual disability,high abilities/giftedness,childhood apraxia of speech,social communication disorder,Landau–Kleffner syndrome,stereotyped movement disorder and excessive screen time.Results We describe conditions commonly found in clinical practice,taking ASD as a reference.We addressed converging and divergent aspects of behavior,cognition,communication,language,speech,socialization,and stereotypes for the diagnosis of ASD and other disorders identified as potential differential or comorbid diagnoses.Conclusions The ranking and characterization of symptoms appear to be essential for better understanding the underlying common ground between children with developmental disorders and children with ASD,thus properly diagnosing and directing social,professional,or medication interventions.This detailed discussion adds to the literature since,although ASD differential diagnoses are frequently mentioned and discussed in textbooks and journal articles,they rarely occupy a prominent place as we aimed herein. 展开更多

The coronavirus disease 2019(COVID-19)pandemic affects all countries and populations worldwide,significantly impacting people with autism with a high risk of morbidity and mortality due to COVID-19.Approximately 25%of children with autism have an asymptomatic or symptomatic immune deficiency or dysfunction.In addition,they frequently have various comorbid conditions that increase the severity of COVID-19.In addition,severe COVID-19 during pregnancy may increase the risk of autism in the offspring.Furthermore,severe acute respiratory syndrome coronavirus 2 could target human nervous system tissues due to its neurotrophic effects.The COVID-19 pandemic intensely impacts many patients and families in the autism community,especially the complex management of autism-associated disorders during the complete lockdown.During the complete lockdown,children with autism had difficulties coping with the change in their routine,lack of access to special education services,limited physical space available,and problems related to food and sleep.Additionally,children with autism or intellectual disabilities are more liable to be abused by others during the pandemic when the standard community supports are no longer functioning to protect them.Early detection and vaccination of children with autism against COVID-19 are highly indicated.They should be prioritized for testing,vaccination,and proper management of COVID-19 and other infectious diseases.In this review,we discuss the various effects of COVID-19 on children with autism,the difficulties they face,the increased risk of infection during pregnancy,how to alleviate the impact of COVID-19,and how to correct the inequalities in children with autism. 展开更多

目的:分析家庭坚韧性在学龄前孤独症儿童母亲心理弹性和疾病获益感间的中介效应,为制订学龄前孤独症儿童家庭干预策略提供参考。方法:采用便利抽样法,选取2022年4月至2023年4月在莆田市4所机构进行干预治疗的205名学龄前孤独症儿童母亲... 目的:分析家庭坚韧性在学龄前孤独症儿童母亲心理弹性和疾病获益感间的中介效应,为制订学龄前孤独症儿童家庭干预策略提供参考。方法:采用便利抽样法,选取2022年4月至2023年4月在莆田市4所机构进行干预治疗的205名学龄前孤独症儿童母亲为研究对象。使用疾病获益感量表、心理弹性量表、家庭坚韧性量表对其进行调查。采用AMOS 23.0软件构建结构方程模型,进行中介效应分析。结果:205名孤独症儿童母亲疾病获益感总分为(65.27±15.41)分,心理弹性总分为(54.89±17.04)分,家庭坚韧性总分为(34.98±6.89)分,三者均呈正相关(P<0.001)。家庭坚韧性在心理弹性与疾病获益感间发挥中介作用,中介效应值为0.230,中介效应占总效应的35.44%。结论:家庭坚韧性在孤独症儿童母亲心理弹性与疾病获益感间起部分中介作用。医护人员应采取积极措施提高母亲的心理弹性和家庭坚韧性,以提高其疾病获益感水平。 展开更多

目的:初步研究学龄前孤独症谱系障碍(autism spectrum disorder,ASD)儿童健康体适能水平,探讨ASD儿童健康体适能的影响因素,为其全面康复治疗和家庭运动指导提供参考。方法:选取ASD组儿童50例和正常组儿童50例,健康体适能水平采用身体... 目的:初步研究学龄前孤独症谱系障碍(autism spectrum disorder,ASD)儿童健康体适能水平,探讨ASD儿童健康体适能的影响因素,为其全面康复治疗和家庭运动指导提供参考。方法:选取ASD组儿童50例和正常组儿童50例,健康体适能水平采用身体质量指数、20m往返跑距离、网球掷远距离、立定跳远距离、等长俯卧撑时长、单脚站时长平均值、坐位体前屈距离7个指标,收集整理ASD儿童的影响因素情况。分析学龄前ASD儿童与正常儿童之间的健康体适能水平差异,采用多元线性回归模型分析性别、BMI、平均每天睡眠时间、平均每天久坐时间、平均每天TPA时间、平均每天MVPA时间、母亲文化水平、父亲文化水平、家庭收入、儿童主要看护人年龄对学龄前ASD儿童健康体适能影响。结果:组间对比分析结果显示ASD组网球掷远距离低于正常组(P<0.05),20m往返跑距离、等长俯卧撑时长和单脚站时长平均值显著低于正常组儿童(P<0.001),差异存在显著性意义。两组BMI、立定跳远距离和坐位体前屈距离成绩差异无显著性意义(P>0.05)。多元线性回归模型分析显示平均每天睡眠、总体力活动(total physical activity,TPA)、中高强度体力活动(moderate to vigorous intensity physical activity,MVPA)时间对20m往返跑距离有良好拟合作用(20m往返跑距离=﹣9.561+1.048×平均每天睡眠时间+0.076×均每天TPA时间+0.066×平均每天MVPA时间);平均每天TPA时间对等长俯卧撑时长有良好拟合作用(等长俯卧撑时长=﹣87.625+0.428×平均每天TPA时间);平均每天TPA、MVPA时间对单脚站时长平均值有良好拟合作用(单脚站时长平均值=6.627+0.094×平均每天总身体活动时间+0.071×平均每天中高强度活动时间)。结论:学龄前ASD儿童的心肺适能、运动适能、上肢肌肉适能低于正常儿童,综合康复干预方案应适当加入体适能训练。学龄前ASD儿童睡眠时间越长、TPA时间越长、MVPA时间越长可能心肺适能、运动适能越好,应增强学龄前ASD儿童睡眠时间和中高强度体力活动的有效时长。 展开更多

目的探讨孤独症谱系障碍(ASD)患儿父母心理适应与心理弹性及自我效能的关系。方法通过随机取样法选取2021年10月至2022年4月期间在郑州大学第三附属医院接受治疗的316例ASD患儿及其父亲或母亲316名作为研究对象。采用一般资料调查问卷... 目的探讨孤独症谱系障碍(ASD)患儿父母心理适应与心理弹性及自我效能的关系。方法通过随机取样法选取2021年10月至2022年4月期间在郑州大学第三附属医院接受治疗的316例ASD患儿及其父亲或母亲316名作为研究对象。采用一般资料调查问卷、心理适应量表(PAS)、心理弹性量表(CD-RISC)以及一般自我效能感量表(GSES)对所有ASD患儿父亲或母亲进行调查,应用Spearman相关性分析法评价ASD患儿父亲或母亲心理适应与心理弹性及自我效能的关系,采用多元线性回归分析ASD患儿父亲或母亲心理适应的影响因素。结果ASD患儿父亲或母亲PAS量表总分为(59.71±6.38)分、CD-RISC量表总分为(62.44±5.87)分、GSES量表总分为(20.45±4.16)分;Spearman相关性分析法分析结果显示,ASD患儿父亲或母亲PAS量表各维度得分及总分与CD-RISC量表各维度得分及总分、GSES量表总分均呈正相关(P<0.05);多元线性回归分析结果显示,心理弹性、自我效能、与患儿关系、父亲或母亲年龄均是ASD患儿父亲或母亲心理适应的影响因素(P<0.05)。结论ASD患儿父亲或母亲心理适应、心理弹性、自我效能处于中等水平,心理弹性、自我效能、与患儿关系、父亲或母亲年龄是ASD患儿父亲或母亲心理适应的影响因素,临床应加强对影响因素的关注并采取有效干预,提升ASD患儿父亲或母亲心理适应水平。 展开更多

背景既往已有多项研究证实屏幕暴露现象在学龄前儿童中普遍存在,并且与儿童情绪行为问题相关,现有研究多集中在正常学龄前儿童,有关屏幕暴露与学龄前孤独症谱系障碍(ASD)儿童情绪行为问题相关的研究较为罕见。目的 调查学龄前ASD儿童屏... 背景既往已有多项研究证实屏幕暴露现象在学龄前儿童中普遍存在,并且与儿童情绪行为问题相关,现有研究多集中在正常学龄前儿童,有关屏幕暴露与学龄前孤独症谱系障碍(ASD)儿童情绪行为问题相关的研究较为罕见。目的 调查学龄前ASD儿童屏幕暴露情况及情绪行为问题异常检出情况,探究屏幕暴露与学龄前ASD儿童情绪行为问题有无相关。方法 选择2022年2—8月在郑州大学第三附属医院儿童发育行为科就诊的符合《精神障碍诊断和统计手册》(DSM-V)孤独症诊断标准的ASD学龄前儿童为ASD组,同期选取郑州市某幼儿园正常发育儿童为对照组。由两组儿童主要照护人进行填写屏幕暴露问卷及长处与困难问卷(家长版)。结果 两组儿童屏幕接触时间、家长对屏幕管理、“屏幕剥夺”反应比较,差异无统计学意义(P<0.001);ASD组儿童亲社会行为、困难总分及各个分维度(情绪症状、品行问题、多动/注意缺陷问题、同伴交往问题)异常检出率均高于对照组(P<0.001);Logistic回归分析发现电子设备放置在睡觉或玩耍空间、观看时照护人是否互动讲解、照护人对孩子观看电子屏幕态度、接触电子屏幕是主动要求还是被动参与、强制关掉电子屏幕孩子反应是学龄前ASD儿童情绪行为问题的影响因素(P<0.05)。结论 屏幕暴露可能对ASD儿童情绪行为问题产生影响,家长应合理管理学龄前ASD儿童屏幕使用。 展开更多

目的 探究听力正常孤独症谱系障碍(autism spectrum disorder,ASD)儿童的听性脑干反应(ABR)特征及各波潜伏期和波间期与ASD严重程度分级的关系。方法 (1)将55例(110耳)听力正常ASD儿童和月龄、性别相匹配的55例(110耳)典型发育(typical ... 目的 探究听力正常孤独症谱系障碍(autism spectrum disorder,ASD)儿童的听性脑干反应(ABR)特征及各波潜伏期和波间期与ASD严重程度分级的关系。方法 (1)将55例(110耳)听力正常ASD儿童和月龄、性别相匹配的55例(110耳)典型发育(typical development,TD)儿童,按月龄分为≤24月龄(22耳)、25~36月龄(40耳)、37~48月龄(28耳)及>48月龄(20耳)四个亚组。比较听力正常ASD儿童与TD儿童的ABR潜伏期和波间期是否存在差异。(2)根据第五版精神障碍诊断与统计手册(DSM-V)将ASD儿童按照严重程度分级,探究听力正常ASD儿童ABR各波潜伏期和波间期与其严重程度分级是否相关。结果 (1)≤24月龄时,ASD与TD儿童ABR各波潜伏期及波间期差异均无统计学意义(P>0.05);(2)25~36月龄时,与TD儿童相比,听力正常ASD儿童波Ⅲ潜伏期及Ⅰ-Ⅲ、Ⅰ-Ⅴ波间期显著延长;37~48月龄时波Ⅲ、Ⅴ潜伏期及Ⅰ-Ⅲ、Ⅲ-Ⅴ、Ⅰ-Ⅴ波间期显著延长;>48月龄时波Ⅴ潜伏期及Ⅲ-Ⅴ、Ⅰ-Ⅴ波间期显著延长(P<0.05);(3)ASD严重程度分级越高,ASD儿童波Ⅲ、Ⅴ潜伏期及Ⅰ-Ⅲ、Ⅲ-Ⅴ、Ⅰ-Ⅴ波间期越长(P<0.05)。结论 自25月龄起听力正常ASD儿童与TD儿童的听觉脑干通路发育水平便显现差异,听力正常ASD儿童的听觉脑干发育水平显著落后于同龄TD儿童。听力正常ASD儿童的ABR波潜伏期和波间期与其严重程度分级相关。 展开更多

目的从家长医患沟通效能的视角,探究针对孤独症谱系障碍(ASD)儿童康复效果的潜在影响因素及其复杂作用路径,为提升ASD儿童的康复服务质量提供依据。方法采用面对面匿名问卷调查的方式,收集甘肃省儿童康复定点机构ASD儿童家长的一般信息... 目的从家长医患沟通效能的视角,探究针对孤独症谱系障碍(ASD)儿童康复效果的潜在影响因素及其复杂作用路径,为提升ASD儿童的康复服务质量提供依据。方法采用面对面匿名问卷调查的方式,收集甘肃省儿童康复定点机构ASD儿童家长的一般信息、家长医患沟通效能、家长治疗依从性、儿童康复效果等相关资料,应用结构方程模型探索家长医患沟通效能对ASD儿童康复效果的影响机制。结果共收集甘肃省13个市/州37家儿童康复机构的ASD儿童家长519名,家长医患沟通效能、医患关系、家长治疗依从性和ASD儿童康复效果两两之间均呈正相关(P<0.05);结构方程模型分析结果为:家长医患沟通效能对ASD儿童康复效果的标准化直接效应值为0.151(P=0.023),医患关系、家长治疗依从性的中介效应值分别为0.160(P=0.001)、0.111(P=0.001),医患关系和家长治疗依从性的链式中介效应值为0.035(P=0.001),“家长医患沟通效能→医患关系→ASD儿童康复效果”占总中介效应的比例最高,为52.29%。结论家长医患沟通效能可直接及间接通过医患关系和家长治疗依从性对ASD儿童康复效果发挥积极作用。康复机构应注重家长医患沟通效能的培养,加强高质量及人性化的康复服务。 展开更多

目的探讨海豚屋数字仿生训练系统对孤独症谱系障碍(ASD)儿童的核心症状改善情况及其父亲的影响。方法采用前瞻性临床随机对照研究法,将100例ASD儿童及其父亲按照随机数字表法分为对照组和试验组,每组50例。对照组采取应用行为分析法(ABA... 目的探讨海豚屋数字仿生训练系统对孤独症谱系障碍(ASD)儿童的核心症状改善情况及其父亲的影响。方法采用前瞻性临床随机对照研究法,将100例ASD儿童及其父亲按照随机数字表法分为对照组和试验组,每组50例。对照组采取应用行为分析法(ABA)干预,试验组在ABA基础上联合使用父亲共同参与式海豚屋数字仿生训练系统干预。采用ABC、CARS、PSI-SF、SDS、SAS评估干预前后ASD儿童的核心症状及其父亲的亲职压力和焦虑、抑郁水平。结果干预后试验组ASD儿童CARS总分,ABC感觉、社交、语言维度和总分得分均低于对照组(P<0.05)。干预后试验组父亲的PSI-SF困难儿童维度得分和总分低于对照组(P<0.05)。结论在ABA干预的基础上联合海豚屋数字仿生训练系统可改善ASD儿童的核心症状,减轻父亲的亲职压力。 展开更多
