One definition of futility is any treatment that merely preserves permanent unconsciousness or fails to end total dependence of a patient on intensive medical care However, no agreement has ever been reached on what the exact defmition of futility is, as it is not only based on temporary values but also evolves into different subtypes, making it harder to define. The difficulty in dealing with futility problem is how doctors evaluate the futile situation, which is further complicated by lack of standards for specific diseases and experience-based prognosis, leading to subjectivity in evaluation for futility. 展开更多

Building the clinical therapeutic evaluation system by combing the evaluation given by doctors and patients can form a more comprehensive and objective evaluation system.A literature search on the practice of evidence-based evaluation was conducted in key biomedical databases,i.e.PubMed,Excerpt Medica Database,China Biology Medicine disc and China National Knowledge Infrastructure. However,no relevant study on the subjects of interest was identified. Therefore, drawing on the principles of narrative medicine and expert opinion from systems of Chinese medicine and Western medicine,we propose to develop and pilot-test a novel evidence-based medical record format that captures the perspectives of both patients and doctors in a clinical trial.Further,we seek to evaluate a strategic therapeutic approach that integrates the wisdom of Chinese medicine with the scientific basis of Western medicine in the treatment of digestive system disorders.Evaluation of therapeutic efficacy of remedies under the system of Chinese medicine is an imperative ongoing research. The present study intends to identify a novel approach to assess the synergistic benefits achievable from an integrated therapeutic approach combining Chinese and Western system of medicine to treat digestive system disorders. 展开更多

Objective To study the medication regularity of Chinese herbal medicines based on hyperuricemia patents. Methods Access database was established to summarzie the patents regarding hyperuricemia disclosured by State Intellectual Property Office(SIPO) of the People's Republic of China in 1986-2015. Statistically analysis was performed by Chinese Medicine Inheritance Support System(TCMISS). Results The literatures of Chinese herbal medicines treating hyperuricemia were 1223, and 138 papers were included by excluding and screening, and contain 282 kinds of traditional Chinese medicine. Conclusion By analyzing the frequency, efficacy, flavor, channel tropism, and composition of the herbs used, we recognize that the efficacy of herbs in curing hyperuricemia is mostly clearing heat and promoting diuresis, invigorating the circulation of blood, dispersing stasis, alleviating water retention, and reinforcing deficiency concurrently. Flavors are mostly sweet, bitter, and cold. Channel tropism is mainly to liver, spleen, stomach, lung, and kidney. It provides the medication consideration for clinical treatment of hyperuricemia. 展开更多

Ten acupuncture-related articles were published in The Journal of the American Medical Association(JAMA) between 1998 and 2017. Five studies showed positive results in terms of the effectiveness of acupuncture/Chinese medicine(CM); five studies showed negative results. This article summarizes the acupuncturerelated clinical trials published over the last 20 years in JAMA, and addresses what seems to be a fundamental ambivalence in Western medical journals regarding the scientific validity of acupuncture/CM. As yet there has been no consensus on the role of acupuncture in healthcare in Western countries. This is hardly surprising, considering the conflicting evidence found in published studies. Skepticism regarding acupuncture/CM is largely grounded in the fact that an accurate model for assessing the true clinical effects of acupuncture has yet to be created. This article discusses some of the pitfalls which result from applying Western-based scientific principles to CM, and suggests that in many cases, "negative" studies have been misinterpreted. The clinical experience of acupuncture practitioners is often in direct conflict with many of the negative conclusions published in journals. We are in need of an accurate model for sham and placebo treatments, and must analyze all published studies for design flaws and faulty conclusions. 展开更多
