Objective:To explore the impact of psychological counseling for underage medical students in clinical teaching.Methods:24 of the 48 underage medical students who were interned in our hospital from March 2023 to February 2024 were randomly selected to adopt the teaching management method of one-on-one psychological guidance as the observation group;the remaining 24 adopted the traditional teaching management method as the control group.The observation of the effects of psychological detachment on various aspects of underage medical students allows the determination of whether psychological detachment impacts clinical teaching.Results:The observation group’s theoretical examination scores,skill assessment scores,and departmental performance evaluation scale score excellence rate were higher than the control group’s,with P<0.05 being statistically significant.At the same time,the clinical teaching and teaching management ability of the hospital was also improved.Conclusion:Psychological guidance for underage medical students can not only improve the comprehensive quality of medical students but also improve the teaching level,which is conducive to improving the teaching effect,and it is a clinical teaching mode worth promoting. 展开更多

Objective:To analyze the effects of providing extended rehabilitation nursing services at home to patients with coronary heart disease(CHD)combined with heart failure(CHF)on psychological improvement and adherence to medical compliance behavior.Methods:79 patients with CHD with CHF admitted to Sijia Town Central Health Hospital,Haimen District,Nantong City,Jiangsu Province,between June 2021 and June 2023 were selected and grouped according to the randomized numerical table method.The control group(39 cases)was provided with conventional nursing care and extended rehabilitation nursing care at home was provided to the observation group(40 cases).The psychological status,adherence to medical behaviors,cardiac function,and complications between both groups were compared.Results:The scores of anxieties and depression self-assessment scales(SAS,SDS)of patients in the observation group were lower than those of the control group(t=2.954,3.212;P<0.05);the compliance of patients in the observation group was higher than that of the control group(P<0.05).The levels of left ventricular ejection fraction,end-systolic and end-diastolic internal diameters(LVEF,LVESD,LVEDD)of patients in the observation group at 58.02±5.34%,44.49±5.16 mm,and 49.16±5.76 mm respectively were better than those of the control group after nursing care(t=3.205,3.288,2.633;P<0.05);the complication rate of the observation group was lower than that of the control group(P<0.05).Conclusion:Extended rehabilitation nursing at home exhibited a psychological regulation effect on CHD with CHF patients,improved their medical compliance,improved cardiac function,reduced the incidence of complications,and had significant application value. 展开更多

Background: The world of cancer care is an emotional place, given the severity of the disease being treated, the heavy workload, the suffering of patients, and the high number of deaths. The nursing staff who work there may be exposed to constant stress from these factors. This study aimed to explore the psychological experience of the nursing staff of the Medical Oncology Department of the Yaoundé General Hospital. Methods: We carried out a cross-sectional study, in June 2017, involving the nursing staff of the Medical Oncology Department of the Yaoundé General Hospital. Results: Sixteen out of 18 nurses were interviewed. All had worked in at least one other department prior to Oncology. The number of years of experience in Oncology ranged from less than one year to 13 years. The choice of the Oncology department was involuntary for 100% of the staff;of these, 62% had intentions of changing departments. Twenty-five percent had applied for a change of department, which was unsuccessful. Stress was expressed by 94% of the staff;93% thought that there was a solution to the stress experienced on a daily basis in the Oncology department. The main solutions proposed to manage this stress were: support for the staff, provision of work materials, and staff training. Conclusion: The nursing staff of the Medical Oncology Department of the Yaoundé General Hospital experiences a high level of stress and are in need of solutions. 展开更多

Objective:To explore the clinical efficacy of the integrated medical care model combined with psychological intervention in pulmonary tuberculosis patients with lung cancer.Methods:From January to December 2022,60 pulmonary tuberculosis patients with lung cancer admitted to our hospital were selected as the research subjects.Using the random number table method,the patients were divided into two groups,a control group and a study group,with 30 cases in each group.The chest computed tomography(CT)examination results,mental state assessment(including depression scale and anxiety scale)scores,incidence of adverse reactions,treatment effect,and length of hospital stay were compared between the two groups.Results:The treatment effect of the patients in the study group was better than that of the patients in the control group(P<0.05);the duration of hospitalization,chest CT examination results,mental state assessment scores,and incidence of adverse reactions of the study group and the control group,were significantly different(P<0.05).Conclusion:The integrated medical care model combined with psychological intervention can effectively improve the treatment effect of pulmonary tuberculosis patients with lung cancer and prevent the occurrence of adverse reactions;thus,it should be promoted in clinical practice. 展开更多

The present study aimed to clarify the current status and awareness of psychological preparation for children undergoing medical procedures in pediatric nursing in Japan as compared with that in Germany. An original questionnaire about the current status and awareness of psychological preparation for children in hospitals was distributed by mail to nurses’ working on Japanese pediatric wards in 2010. The same questionnaire, translated into German, was distributed to nurses working on German pediatric wards via the internet in 2010. A large majority of respondents strongly agreed that children have a right to informed consent. German nurses expressed a longer-term viewpoint on the effects of preparation than Japanese nurses. Japanese nurses recognized a greater need for improvement in their duties than German nurses. The results suggest that we should consider our own country’s nursing practices and need for improvement, but also learn from studies of other countries to address each culture and medical situation appropriately. 展开更多

Like soldiers,frontline medical staff provide a first line of defense and have played a critical role in responses to the outbreak of coronavirus disease-2019 in December 2019.It is important to acknowledge the considerable pressure placed on frontline medical staff in the face of a new type of coronavirus that is highly infectious and for which no specific treatment is available.Here,we review the various kinds of psychological problems afflicting frontline medical staff who are combatting the severe acute respiratory syndrome epidemic.These include anxiety,insomnia,depression,interpersonal difficulties,and post-traumatic stress disorder syndrome.We further present a summary of countermeasures for alleviating these problems based on our findings.These countermeasures include ensuring the provision of adequate protective gear for frontline medical staff,developing timely and clear guidelines,strengthening social support,and providing clear criteria and additional training,focusing on the choice of frontline medical staff.An understanding of the psychological impacts of an epidemic situation and of relevant countermeasures will contribute to reducing the psychological pressures on frontline medical staff.Consequently,they will be able to cope better with outbreaks of infectious diseases in the future,to reduce the psychological pressure of the front-line medical staff,and to improve the treatment level. 展开更多

Objective: We evaluated the psychological distress of medical doctor using a 6-item instrument (the K6) in Kagawa prefecture, Japan. Methods: A total of 284 medical doctors (236 men and 48 women) were analyzed in a cross-sectional investigation study. The association between psychological distress and clinical factors were evaluated by the K6 instrument, with psychological distress defined as 13 or more points out of a total of 24 points. Results: A total of 17 doctors (6.0%) as defined as psychological distress. The significant relationships between the K6 score and age, experience duration as clinician were not noted. The K6 score in subjects with consciousness of suicide was significantly higher than that without. In addition, the K6 score in subjects without cooperation with specialists was higher than those without, but not at a significant level. Conclusions: Some factors i.e. consciousness of suicide and cooperation with specialists might be associated with psychological distress, as assessed by the K6 instrument, in medical doctor in Kagawa prefecture, Japan. 展开更多

Hence, understanding medical word-of-mouth (WOM) process among patients in medical services of sport therapists could be interesting and useful. This study would find out the role of WOM on patients psychology when choosing the sport therapists, understand how patients look for sport therapists’ information and which information source is important to them. As a pioneer, this study would like to improve medical WOM marketing of sport therapists. The authors designed a questionnaire based on how patients choosing sport therapists, including questions search importance of different information sources of sport therapists with medical service decision, and respondents’ psychological demographics. We released 253 questionnaires of judo therapy clinics inTaiwan. More than half of the respondents see sport therapists due to others’ recommendations (referral of family, friends and professionals). Women are more likely to seek medical word-of-mouth information than men;and respondents who think choosing sport therapists are important tend to seek medical word-of-mouth information. Preparation is a means to support the psychological readiness and autonomy of children undergoing medical treatment. In this study, to understand the current practice and application of psychological preparation, using “explaining medical information to a child” as an indicator of respect for children’s rights, we investigated the effect of psychological preparation since 2002. The present study aimed to clarify the current status and awareness of psychological preparation for children undergoing medical procedures in pediatric nursing in Japan. An original questionnaire regarding psychological preparation for children in hospitals was sent by mail to nurses who were working in Japanese pediatric wards in 2010. Based on results from a previous study, nurses’ awareness of psychological preparation for children undergoing medical procedures seems to have improved. However, it was evident that nursing staff working in mixed wards require more specialized knowledge regarding child care, especially in terms of preparation. Furthermore, Japanese nurses recognized a need for improvement concerning their duties. It was also clear that the meaning of preparation remains ambiguous for many nurses. These results suggest that preparation has both psychological and ethical meanings. 展开更多

Comparison of posttraumatic growth and posttraumatic stress of nurses with physicians and medical students after caring of COVID‑19 patients 被引量:1

Objectives:The present study compared the prevalence and psychosocial factors affecting posttraumatic growth(PTG),posttraumatic stress disorder(PTSD),and psychological distress in nurses,physicians,and medical students.Materials and Methods:In a cross‑sectional study,three groups of medical staff including nurses(n=57),physicians(n=40),and medical students(n=34)who were responsible for the care/treatment of COVID‑19 patients admitted to a general hospital for 4 months responded to Posttraumatic Growth Inventory,Brief Symptom Inventory‑18,Resilience Questionnaire,PTSD Screen,and Social Capital‑Integrated Questionnaire.Results:After the exposure of medical staff to at least 4 months of treatment/care of COVID‑19 patients,PTG prevalence was higher than PTSD(38.2%vs.14.6%),but they experienced some degrees of psychological distress(65.5%).The nurses had 8.33(confidence interval[CI]:2.5–26.7)times higher PTG rate than medical students(P<0.001).Physicists also experienced 5.00(CI:1.4–26.7)times higher PTG than medical students(P<0.001).PTG was aided by age,married status,strong resilience,and high social capital,but gender had no influence.Resilience played an important protective role to prevent the incidence of psychological distress in nurses,medical students,and physicians.Conclusion:Despite the fact that the PTSD and psychological distress were same in the three groups of medical staff,the nurses had a greater rate of PTG than physicians and medical students. 展开更多

Stroke research and rehabilitation have traditionally focused on the physical and functional impact of a stroke. Less attention has been given to the psychosocial factors associated with this chronic condition. By the few studies that have specifically focused on psychosocial factors in the context of stroke, poststroke depression is demonstrated to significantly influence stroke outcomes. Associations of stroke with psychological symptoms other than depression have rarely been evaluated. This study was aimed to investigate the changes of psychological stress, social support and medication adherence in patients with ischemic stroke in the mainland of China. In this study, 90 patients with hemiplegia one year after first-ever middle cerebral artery infarction(stroke group) in the Zhongnan Hospital of Wuhan University from June 2008 to June 2011 were recruited for interview. Ninety age- and sex-matched normal volunteers(control group) were also examined at the same period. The psychological distress was assessed by the Symptom Checklist 90(SCL-90), the social support by the Social Support Rating Scale(SSRS), and medication adherence by Morisky's self-reported inventory, respectively. Group differences were analyzed using unpaired-t test and chi-squared test. The results showed that total mean scores of the SCL-90 in the stroke group were higher than those in the control group(P〈0.01). Except two dimensions, paranoid ideation and psychoticism, mean scores of the rest dimensions(including somatization, obsession-compulsion, interpersonal sensitivity, depression, anxiety, hostility, and phobic anxiety) of SCL-90 were significantly higher in the stroke group than those in the control group(P〈0.05, or P〈0.01). The objective support, subjective support, support availability and total social support scores in the stroke group were significantly higher than those in the control group(P〈0.05, or P〈0.01). Those in the "SCL-90 total scores 〉150 group" were significantly higher than in the "SCL-90 total scores 〈100 group" and the "SCL-90 total scores between 100 to 150 group"(P〈0.05, or P〈0.01). Those in the "SCL-90 total scores between 100 to 150 group" were significantly higher than in the "SCL-90 total scores 〈100 group"(P〈0.05). In 90 patients with ischemic stroke, 26(28.89%) patients obtained high medication adherence, 47(52.22%) patients medium medication adherence, and 17(18.89%) patients low medication adherence, respectively. Among these stroke patients, there were 17(50.00%) patients with high medication adherence in the "SCL-90 total scores 〉150 group", 28(75.67%) patients with medium medication adherence in the "SCL-90 total scores between 100 to 150 group", and 12(61.16%) patients with low medication adherence in the "SCL-90 total scores 〈100 group", respectively. There was significant difference in the medication adherence rate among the different SCL-90 scores groups in these stroke patients(P〈0.05 or P〈0.01). It was led to conclude that ischemic stroke patients one year after hemiplegia have psychological distress, low level of social support and poor medication adherence in the mainland of China. Therefore, it is necessary to mobilize the government, medical institutions and various social support groups to offer psychological interventions to relieve the stress of patients with ischemic stroke, and improve their medication adherence. 展开更多

Hepatitis B is an infectious disease worthy of attention.Considering the incubation period,psychological inhibition factor,vaccine,limited medical resources and horizontal transmission,an SIRS model is proposed to describe hepatitis B transmission dynamics.In order to describe the behavior changes caused by people's psychological changes,the non-monotonic incidence rate is adopted in the model.We use the saturated treatment rate to describe the limited medical resources.Mathematical analysis shows the existence conditions of the equilibria,forward or backward bifurcation,Hopf bifurcation and the Bogdanov-Takens bifurcation.During the observation of the case data of hepatitis B in China,it is found that there are mainly three features,periodic outbreaks,aperiodic outbreaks,and periodic outbreaks turns to aperiodic outbreaks.According to the above features,we select three different representative regions,Jiangxi,Zhejiang province and Beijing,and then use our model to fit the actual monthly hepatitis B case data.The basic reproduction numbers that we estimated are 1.7712,1.4805 and 1.4132,respectively.The results of data fitting are consistent with those of theoretical analysis.According to the sensitivity analysis of Ro,we conclude that reducing contact,increasing treatment rate,strengthening vaccination and revaccinating can effectively prevent and control the prevalence of hepatitis B. 展开更多

Method: All students were asked to evaluate the courses anonymously online. Effect sizes [1] were calculated to compare acceptance of the different course types and also changes across time. Results: Both the Basic Course in Medical Psychology and the Systemblock Psyche were rated as “good” to “satisfactory” and were in their overall acceptance comparable to other courses and system blocks. Continuous improvement in acceptance was found for the Basic Course (d = 0.30 - 0.57). The Systemblock Psyche received varying evaluations but achieved higher scores on comprehensibility (d = 0.20) and communication among teachers (d = 0.34) than other system blocks. On the other hand, students rated the education in medical psychology as less relevant than other courses (d = 0.28 and 0.77, respectively). Conclusion: Overall, the acceptance rating was satisfactory and comparable to other evaluation studies conducted in earlier curricula. However, ratings of the relevance of the courses in medical psychology were disappointing and indicate the difficulty of teaching a biopsychosocial model to medical students. 展开更多

The aim of this cross-sectional study was to compare attitudes towards psychiatric medication and concordance in medicine taking between medical and psychology students, as they represent future members of mental health teams. Two hundred and sixteen medical students and 222 psychology students completed the Leeds Attitudes toward Concordance scale (LATCon) and the Beliefs about Medication Questionnaire (BMQ) (both adapted for psychiatric medication). Psychology students saw psychiatric medicines as more harmful and were also most likely to believe that psychiatrists overprescribed these medicines. They also scored higher than medical students on attitudes towards concordance, although this difference remained at the limit of statistical significance. 展开更多

They were randomly allocated to either treatment. There was a significant improvement in pain ratings with both treatment types. Specifically a greater effect was seen in migraine patients treated by massage with relaxation when compared to acupuncture. No psychological factors were found to predict response to either treatment. At the end of the study, 13% of patients were significantly more worried that there may be a more serious cause underlying their headache despite reassurance and an improvement in their headache scores. 展开更多

Exploring whether medical staff perceive stress on the assigned medical tasks,what are the specific sources of stress,what are the tangible sources of support they expected to be helpful,and individual coping with stress to provide more accurate,personal support for psychological crisis.This study uses a cross-sectional descriptive survey adopting convenience sampling among the medical staff who worked for over seven days in the infected areas of one Grade 2A and three Grade 3A hospitals during the COVID-19 pandemic.The assessment includes attitude when receiving tasks,major stressors,factors relieving stress,and personal management of stress.A total of 104(76.8%)valid questionnaires are received.A majority(94.3%)of the medical staff held a positive attitude taking duty for granted but feeling nervous and afraid(81.1%).Contacting the infected(90.6%)and compensation(92.5%)are the main concerns.The major pressure sources are regarding safety for oneself(98.1%),colleagues(92.5%),and family(94.3%),as well as lack of effective treatment(92.5%),inadequate protective equipment(94.3%)and incomplete disinfection(94.3%).The main positive pressure adjustments include strict infection controlling measures(98.4%),maintaining a positive attitude(95.2%),actively attain more information 95.2%,and engage in recreational activities(93.6%),but more than 40%takes negative adjustments to vent their emotions and relieve anxiety,suggesting the importance of early psychological intervention.During the COVID-19 pandemic,the main stressors among medical staff are safety,uncertainty and shortage of resources.Positive coping includes taking effective protective measures and achieving thorough understanding of the virus. 展开更多

Objective:The ongoing coronavirus disease 2019(COVID-19)epidemic has caused extensive damage worldwide.We explored whether the medical care conduct of patients with psoriasis has changed and whether the COVID-19 epidemic has placed more psychological pressure on these patients.Methods:A questionnaire survey was administered to patients with psoriasis in the Dermatology Department of Peking University Third Hospital in July 2022.Information about the patients’general demographics,psoriasis condition,mental state of anxiety(Generalized Anxiety Disorder-7[GAD-7]score),changes in medical care conduct,and Concerns about COVID-19-Related Risk Score for Psoriasis(CCRSP,a self-designed questionnaire)was collected.Univariate analysis with the Mann-Whitney U test for continuous variables was used in the estimation of statistical differences.Results:A total of 112 patients with psoriasis completed the survey.Purchasing medicine was the factor that added difficulty for most participants(32.1%).Fewer participants(26.8%)encountered medical registration difficulties than economic and transportation difficulties.Nearly three-quarters of participants(73.2%)reported at least one medical care conduct change resulting from the COVID-19 epidemic.A small proportion of participants(12.5%)thought that COVID-19 prevented them from seeking medical services for psoriasis.The top three items that caused the most concern were psoriasis aggravation resulting from drug reduction and withdrawal because of COVID-19(44.7%),psoriasis aggravation after infection with COVID-19(38.4%),and psoriasis-related side effects of the COVID-19 vaccine(35.7%).The GAD-7 score of patients with a high CCRSP(score of>10)was significantly higher than that of patients with a low CCRSP(score of≤10)(nonparametric test,P=0.047).Conclusion:COVID-19 poses challenges and places a mental burden on patients with psoriasis.Doctors should regulate the medical behavior of patients with psoriasis according to their specific COVID-19 situation and help patients ease their anxiety to maintain the stability of their psoriasis condition. 展开更多

Background: There have been concerns regarding the mental wellbeing of emergency medical service staff, particularly when faced with multiple trauma exposures. This study aimed to describe a cross-sectional view of the mental wellbeing status of emergency medical service workers, as well as to comment on the usefulness of an electronic survey tool to reach staff that is distributed across large geographical areas. Methods: The data presented here are drawn from an anonymous, voluntary, electronic survey, made available via the internet, to emergency medical service workers in the Western Cape province of South Africa. This paper reports on four domain expressions of mental wellbeing, namely disordered mood, problematic substance use, post-traumatic stress disorder, and domestic discord. Results: Elevated rates were reported by this sample, across all four distress domains, i.e. mood disorder = 12.3%;problematic substance use = 14.9%;post-traumatic stress disorder = 11.9%;and domestic discord = 11.9%. These rates were generally similar to data from comparable emergency worker groups, and much higher than local population estimates. Conclusions: The data support earlier findings that emergency medical work is associated with increased risk for symptoms of psychological distress, which present a strong case for regular organizational monitoring of employees’ mental wellbeing. In this regard, a brief, internet delivered, survey-type tool showed promise for screening employees, in order to guide further streaming of distressed individuals towards appropriate support services. Further work is required to develop clear mechanisms for referral and intervention in order to optimize a confidential and supportive monitoring program. 展开更多
