Background:Saccades are often observed on video head impulse tests(vHIT)in patients with Meniere's Disease(MD)and Vestibular Migraine(VM).However,their saccadic features are not fully described.Objective:This stud...Background:Saccades are often observed on video head impulse tests(vHIT)in patients with Meniere's Disease(MD)and Vestibular Migraine(VM).However,their saccadic features are not fully described.Objective:This study aims to identify the saccades characteristics of MD and VM.Methods:75 VM patients and 103 definite unilateral MD patients were enrolled in this study.First raw saccades were exported and analyzed.The VM patients were divided into left and right based on their ears,while the MD patients were separated into affected and unaffected subgroups based on their audiograms and symptoms.Results:The MD patients have more saccades on the affected side(85%vs.69%),and saccade velocity is more consistent than the contralateral side(shown by the coefficient of variation).The saccades occurrence rates on both sides are similar in VM(77%vs.76%),as are other saccadic parameters.The MD patients have more significant inter-aural differences than the VM patients,manifested in higher velocity(p-value 0.000),earlier arriving(p-value 0.010),and more time-domain gathered(p-value 0.003)on the affected side.Conclusions:Bilateral saccades are commonly observed in MD and VM.In contrast to MD,saccades on VM are subtle,scattered,and late-arrived.Furthermore,the MD patients showed inconsistent saccadic distribution with more velocity-uniform saccades on the affected side.展开更多
Background:Exploring the efficacy,potential components,and mechanism of the combination of ginger essential oil and gingerols in the treatment of head wind disease based on network pharmacology technology with content...Background:Exploring the efficacy,potential components,and mechanism of the combination of ginger essential oil and gingerols in the treatment of head wind disease based on network pharmacology technology with content weight.Methods:The experimental groups were divided into:0:10,1:4,1:2,1:1,2:1,4:1,10:0.The relative content(Ri)of the chemical constituents of ginger's volatile oil was determined using gas chromatography-mass spectrometry(GC-MS).Additionally,the physicochemical and biological property parameters(LogP,MDCK,PPB,MW)of the components were considered.To assess the quantitative effect of the components,a grading score was performed,and the quantitative effect index(Ki)was calculated.Subsequently,the target effect index(Ti)was calculated by combining the component-target matching score(Fit score).Using these calculations,the target effect score A was determined under the influence of multiple components targeting different targets.Key targets with A≥1000 were identified.To predict the targets related to head wind disease,the Comparative Toxicogenomics Database(https://ctdbase.org/),Gene Cards(https://www.genecards.org/),and Disgenet database(https://www.disgenet.org/)were utilized.The key targets,obtained from different proportions of ginger's volatile oil and gingerol,were intersected with the predicted targets.This facilitated network pharmacological analysis and verification of the efficacy.Results:The content of volatile oil in ginger demonstrated an impact on key targets associated with the volatile oil group.Each specific combination of volatile oil consistently activated distinct pathways,with variations stemming from changes in content.Experimental testing revealed that different combinations of ginger's volatile oil and gingerol effectively alleviated migraine symptoms in rats.Conclusion:Through the application of content-weighted network pharmacology technology and pharmacodynamic verification,it was determined that altering the ratio between ginger's volatile oil and gingerol leads to variations in potential targets and pathways,consequently impacting its efficacy.展开更多
Objective:Saccades accompanied by normal gain in video head impulse tests(vHIT)are often observed in patients with vestibular migraine(VM).However,they are not considered as an independent indicator,reducing their uti...Objective:Saccades accompanied by normal gain in video head impulse tests(vHIT)are often observed in patients with vestibular migraine(VM).However,they are not considered as an independent indicator,reducing their utility in diagnosing VM.To better understand clinical features of VM,it is necessary to understand raw saccades data.Methods:Fourteen patients with confirmed VM,45 patients with probable VM(p-VM)and 14 agematched healthy volunteers were included in this study.Clinical findings related to spontaneous nystagmus(SN),positional nystagmus(PN),head-shaking nystagmus(HSN),caloric test and vHIT were recorded.Raw saccades data were exported and numbered by their sequences,and their features analyzed.Results:VM patients showed no SN,PN or HSN,and less than half of them showed unilateral weakness(UW)on caloric test.The first saccades from lateral semicircular canal stimulation were the most predominant for both left and right sides.Neither velocity nor time parameters were significantly different when compared between the two sides.Most VM patients(86%)exhibited small saccades,around 35%of the head peak velocity,with a latency of 200e400 ms.Characteristics of saccades were similar in patients with p-VM.Only four normal subjects showed saccades,all unilateral and seemingly random.Conclusions:Small saccades involving bilateral semicircular canals with a scattered distribution pattern are common in patients with VM and p-VM.展开更多

CGRP was injected into the dura mater to induce a migraine. The frequency of head scratching, cage climbing, and facial grooming was observed. The pain threshold, the levels of CGRP,pituitary adenylate cyclase activating polypeptide(PACAP), substance P(SP), and the plasma protein extravasation(PPE) ratio were measured. The phosphorylation levels of p38, extracellular signalregulated kinase 1/2(ERK1/2), and expression of iNOS were detected by western blot.Results: Compared with the model group, the three modified XZDLD groups showed reduced frequency of head scratching and cage climbing in the first 30 min(all P <.05). Facial grooming frequency was reduced in XZDLD and XZDLD(external wind) groups(P =.0003 and P =.0131, respectively). External wind medicine played a more important role in increasing mechanical pain threshold than internal wind medicine. Moreover, compared with the model group, the three modified XZDLD groups demonstrated reduced plasma levels of CGRP and PACAP(all P <.05). No difference in the SP level was observed among the six groups. XZDLD reduced PPE ratio. XZDLD and XZDLD(external wind) groups suppressed the CGRP/iNOS pathway by inhibiting the p-p38/p38 ratio and the expression of iNOS. Objective:To explore the formula principles for“head wind”disease by data-mining and provide reference for the treatment of“head wind”disease.Methods:Prescriptions for headache from“Dictionary of Traditional Chin...Objective:To explore the formula principles for“head wind”disease by data-mining and provide reference for the treatment of“head wind”disease.Methods:Prescriptions for headache from“Dictionary of Traditional Chinese Medicine Prescriptions”were collected,sorted and entered into the“Traditional Chinese Medicine Inheritance System”(V2.5)to analyze the composition principles by the methods of revised mutual information,complex system entropy cluster and unsupervised hierarchical cluster.Results:The herbs with high usage frequency in these prescriptions for“head wind”disease included Chuan-xiong,Bai-zhi,Xi-xin,Fang-feng,Gan-cao,Shi-Gao and Qiang-Huo.Three new prescriptions were obtained based on the composition principles and rules.The first new prescription is composed of Quan-xie,Xiong-huang,Jiang-can,Zhi-chuan-wu and Di-long.The second new prescription is composed of Du-huo,Rou-gui,Ku-shen;Fang-feng,Gao-ben,Qiang-huo.The third new prescription is composed of Fu-zi,Sheng-chuan-wu,Tian-nan-xing,Chuan-xiong,Bai-zhi,Shi-gao.Conclusion:We found that the most of herbs used to treat the“head wind”disease are warm and spicy,mainly belong to diaphoretic herbs,wind-expelling herbs,and dampness-dispelling herbs.In addition,the new prescriptions for infertility mainly focus on eliminating evil,with the characteristics of“eliminating phlegm and turbid”,“dredging the channels and collaterals”,“expelling wind-evil”and“promoting blood circulation”.展开更多

The simulation is using...A nonlinear finite element model is developed to examine the lateral behaviors of monopiles, which support offshore wind turbines(OWTs) chosen from five different offshore wind farms in Europe. The simulation is using this model to accurately estimate the natural frequency of these slender structures, as a function of the interaction of the foundations with the subsoil. After a brief introduction to the wind power energy as a reliable alternative in comparison to fossil fuel, the paper focuses on concept of natural frequency as a primary indicator in designing the foundations of OWTs. Then the range of natural frequencies is provided for a safe design purpose. Next, an analytical expression of an OWT natural frequency is presented as a function of soil-monopile interaction through monopile head springs characterized by lateral stiffness KL, rotational stiffness KRand cross-coupling stiffness KLRof which the differences are discussed. The nonlinear pseudo three-dimensional finite element vertical slices model has been used to analyze the lateral behaviors of monopiles supporting the OWTs of different wind farm sites considered. Through the monopiles head movements(displacements and rotations), the values of KL, KRand KLRwere obtained and substituted in the analytical expression of natural frequency for comparison. The comparison results between computed and measured natural frequencies showed an excellent agreement for most cases. This confirms the convenience of the finite element model used for the accurate estimation of the monopile head stiffness.展开更多
The optimal path planning for fixed-wing unmanned aerial vehicles(UAVs) in multi-target surveillance tasks(MTST) in the presence of wind is concerned.To take into account the minimal turning radius of UAVs,the Dubins ...The optimal path planning for fixed-wing unmanned aerial vehicles(UAVs) in multi-target surveillance tasks(MTST) in the presence of wind is concerned.To take into account the minimal turning radius of UAVs,the Dubins model is used to approximate the dynamics of UAVs.Based on the assumption,the path planning problem of UAVs in MTST can be formulated as a Dubins traveling salesman problem(DTSP).By considering its prohibitively high computational cost,the Dubins paths under terminal heading relaxation are introduced,which leads to significant reduction of the optimization scale and difficulty of the whole problem.Meanwhile,in view of the impact of wind on UAVs' paths,the notion of virtual target is proposed.The application of the idea successfully converts the Dubins path planning problem from an initial configuration to a target in wind into a problem of finding the minimal root of a transcendental equation.Then,the Dubins tour is derived by using differential evolution(DE) algorithm which employs random-key encoding technique to optimize the visiting sequence of waypoints.Finally,the effectiveness and efficiency of the proposed algorithm are demonstrated through computational experiments.Numerical results exhibit that the proposed algorithm can produce high quality solutions to the problem.展开更多

文摘目的 观察头穴丛刺长留针法治疗偏头痛的临床疗效。方法 将88例偏头痛患者随机分为观察组(44例,脱落2例)和对照组(44例,脱落3例)。观察组采用常规针刺联合头穴丛刺长留针法治疗,对照组采用常规针刺方法治疗。比较两组临床疗效,观察两组治疗前后的视觉模拟量表(visual analog scale, VAS)评分、偏头痛特异性生活质量问卷(migraine-specific quality of life questionnaire, MSQ)、血清5-羟色胺(5-hydroxytryptamine, 5-HT)浓度。结果 观察组总有效率为92.9%,高于对照组的78.0%,差异有统计学意义(P>0.05)。两组治疗后VAS评分较治疗前降低(P<0.05),两组随访时VAS评分较治疗前和治疗后降低(P<0.05);观察组治疗后及随访时,VAS评分均低于对照组(P<0.05)。两组治疗后MSQ评分较治疗前升高(P<0.05),两组随访时MSQ评分较治疗前和治疗后升高(P<0.05);观察组治疗后及随访时,MSQ评分均高于对照组(P<0.05)。两组治疗后血清5-HT浓度均升高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论 在常规针刺基础上,头穴丛刺长留针法治疗偏头痛临床疗效优于常规针刺方法,且在减轻偏头痛患者疼痛程度,改善其生活质量及提高5-HT浓度方面优于常规针刺方法。
