OBJECTIVE: To evaluate the effectiveness and safety of the Xingnao Kaiqiao needling method for treating acute ischemic stroke.DATA SOURCES: We retrieved relevant randomized controlled trials involving Xingnao Kaiqia...OBJECTIVE: To evaluate the effectiveness and safety of the Xingnao Kaiqiao needling method for treating acute ischemic stroke.DATA SOURCES: We retrieved relevant randomized controlled trials involving Xingnao Kaiqiao acupuncture for treatment of acute ischemic stroke. The China National Knowledge Infrastructure, Weipu Information Resources System, Wanfang Medical Data System, Chinese Biomedical Literature Database, Cochrane Library, and Pub Med were searched from June 2006 to March 2016.DATA SELECTION: We analyzed randomized and semi-randomized clinical controlled trials that compared Xingnao Kaiqiao acupuncture with various control treatments, such as conventional drugs or other acupuncture therapies, for treatment of acute ischemic stroke. The quality of articles was evaluated according to the Cochrane Handbook for Systematic Reviews of Interventions(Version 5.1), and the study was carried out using Cochrane system assessment methods. Rev Man 5.2 was used for the meta-analysis of the included studies.OUTCOME MEASURES: The mortality rate, disability rate, activities of daily living(Barthel Index), and clinical efficacy were observed.RESULTS: Twelve studies met the inclusion criteria for this review. The meta-analysis showed that between Xingnao Kaiqiao acupuncture and the control treatment, Xingnao Kaiqiao acupuncture reduced the disability rate [risk ratio(RR) = 0.51, 95% confidence interval(CI) = 0.27-0.98, z = 2.03, P 〈 0.05], elevated the activities of daily living(weighted mean difference = 12.23, 95% CI: 3.66-20.08, z = 2.80, P 〈 0.005), and had greater clinical efficacy(RR = 1.61, 95% CI: 1.23-2.09, z = 3.53, P 〈 0.0004). However, there was no significant difference in mortality rate(RR = 0.61, 95% CI: 0.15-2.45, z = 0.70, P 〉 0.05). CONCLUSION: The Xingnao Kaiqiao needling method is effective and safe for acute ischemic stroke. However, there was selective bias in this study, and the likelihood of measurement bias is high. Thus, more high-quality randomized controlled trials are needed to provide reliable evidence of the efficacy and safety of Xingnao Kaiqiao acupuncture in the treatment of acute ischemic stroke.展开更多

Methods: A total of 158 cases of peripheral facial palsy patients are randomly divided into treatment group (n=116 cases, who are treated by penetration needling method) and control group (n=42 cases, who are treated with traditional acupuncture method). In treatment group, main acupoint pairs for penetration needling are Yangbai (GB 14) -Yuyao (EX-HN 4), Tongziliao (GB 1) -Taiyang (EX-HN 5), Jiache (ST 6) -Dicang (ST 4) and Chengjiang (CV 24) -Jiachengjiang (EX-HN) on the affected side, and in control group, the main acupoints used are Yifeng (TE 17), Yangbai (GB 14), Sibai (ST 2), Dicang (ST 4), Jiache (ST 6), Jingming (BL 1) and Chengjiang (CV 24). The treatment is given once daily with 10 sessions being a therapeutic course. Results: After 3 courses of treatment, the cure rates of treatment group and control group are 81.8% and 66.7% separately, showing a significant difference between two groups (P<0.05). The total effective rates of treatment and control groups are 96.5% and 88.1% separately, presenting a significant difference between two groups (P<0.01). Methods Wentong needling method was used in the treatment of 30 cases with long-term cough after common cold. Acupoints including Quchi (曲池 LI 11, left side), Hegu (合谷, LI 4, left side), Lieque (列缺, LU 7, right side), Fenglong (丰隆', ST 40, right side), Chize (尺泽, LU 5, right side), Zusanli (足三里, ST 36, right side), Zhaohai (照海, KI 6, left side), Taichong (太冲h, LR 3, both sides), and Waiqiu (外丘, GB 36, left side) were selected. The treatment was given every other day. After one treatment course, the relation between the long-term cough cases after common cold and Wentong needling method clinical efficacy was observed from aspects of different ages, disease duration and disease degree. Results There were 18 cured cases (60.0%), 8 markedly effective cases (26.7%), 4 effective cases (13.33%), and 0 invalid case (0.0%). The total effective rate was 200%. The treatment efficacy of cough patients after common cold of less than 24 months was better than that of more than 24 months. Along with the increasing of age, the cured and markedly effective rate was of certain downward tendency. Along with the increasing of treatment times, the cured and markedly effective rate increased. Conclusion The clinical efficacy of Wentong needling method is obvious in the treatment of long-term cough cases after common cold.展开更多

Objective To explore the clinical efficacy of muscle regions of meridians needling method for refractory facial paralysis affecting different locations.Methods A total of 110 participants were randomized into an obser...Objective To explore the clinical efficacy of muscle regions of meridians needling method for refractory facial paralysis affecting different locations.Methods A total of 110 participants were randomized into an observation group and a control group,with 55 cases in each group.The observation group was treated with muscle regions of meridians needling method,and the control group was treated with conventional facial three-line needling method.The clinical efficacy was evaluated four weeks after the treatment.And the infrared imaging spectra of the two groups were examined.Results The total effective rate of the observation group was higher than that of the control group(P<0.05),and its curative effect for refractory facial paralysis located above the geniculate ganglion was better than that of the control group(P<0.05).The color scale distribution of different disease locations in the two groups varied significantly(P<0.05),the higher the disease location,the higher the occurrence rate of cool zone and low temperature zone.After treatment,the reductions of the facial and periotic temperature difference between the healthy side and the affected side in the observation group were statistically different from those in the control group(P<0.05).Conclusion Muscle regions of meridians needling method has a better effect than facial three-line needling method for refractory facial paralysis.It can promote the microcirculation of the affected side of the face,improve the blood and oxygen supply to local tissues,and thus promote the repair of the peripheral facial nerve.展开更多
Objective:To evaluate the clinical effect differences between conducting qi and regulating the spirit needling method and regular needling method.Methods:Sixty-nine patients of insomnia were randomly divided into an o...Objective:To evaluate the clinical effect differences between conducting qi and regulating the spirit needling method and regular needling method.Methods:Sixty-nine patients of insomnia were randomly divided into an observation group(35 cases)and a control group(34 cases).Bǎihuì(百会GV20),Sìshéncōng(四神聪EX-HN1),Hégǔ(合谷LI4),Tàichōng(太冲LR3),Zúsānlǐ(足三里ST36)and Sānyīnjiāo(三阴交SP6)were selected in the two groups.The patients in the observation group were treated with needling method for conducting qi and regulating the spirit.The patients in the control group were treated with the regular needling method.The treatment was given once every other day,both groups were treated for 12 times.Pittsburgh Sleep Quality Index(PSQI)and Epworth Sleepiness Scale(ESS)were observed before and after the treatment and the clinical effect was evaluated.Results:After treatment,the total score and the score of each item of PSQI as well as the result of ESS were all improved after the treatment in the two groups(all P<0.05).The improvements in the total score of PSQI and the result of ESS in the observation group were better than the control group(both P<0.05).The total effective rate was 88.57%(31/35)in the observation group,significantly superior to 76.47%(26/34)in the control group(P<0.05).Conclusion:The conducting qi and regulating the spirit needling method achieves a better effect on insomnia as compared with the regular needling method.This method achieves a remarkable improvement in sleep latency,sleep quality and daytime sleepiness.展开更多
Objective To evaluate the clinical efficacy of tube moxibustion plus point-toward-point needling method for refractory facial paralysis.Methods A total of 100 patients with refractory facial paralysis who met the incl...Objective To evaluate the clinical efficacy of tube moxibustion plus point-toward-point needling method for refractory facial paralysis.Methods A total of 100 patients with refractory facial paralysis who met the inclusion criteria were randomized into an observation group and a control group,with 50 cases in each group.Both groups were treated with acupuncture by point-toward-point needling method,mainly in the affected eye,cheek and mouth areas.The observation group was given additional tube moxibustion after receiving the point-toward-point needling method,which inserted a tube moxibustion device into the external acoustic meatus 1 cm on the affected side for 20 min.Both groups were treated once a day,6 times a week,for 4 consecutive weeks.The House-Brackmann scale was scored before and after treatment,and the facial nerve electromyogram data were collected.Results The total effective rate of the observation group was 93.6%,which was higher than 64.6%of the control group,and the difference between the two groups was statistically significant(P<0.05).The proportion of new-onset facial paralysis complications(facial synkinesis,facial spasm,facial paralysis perversion,and crocodile tears)in the observation group was 6.4%,which was lower than 35.4%in the control group.The difference between the two groups was statistically significant(P<0.05).In the observation group,the numbers of cases after treatment with the ratio of action potential amplitude between the affected side and the healthy side increased by 10%–30%and over 30%were more than those in the control group.And the difference between the two groups was statistically significant(P<0.05).Conclusion Tube moxibustion plus point-toward-point needling method has a better effect in improving symptoms of refractory facial paralysis,decreasing the incidence of sequelae,and increasing clinical efficacy than the point-toward-point needling method alone.展开更多

DATA ReTRIevAL: We retrieved relevant random and semi-random controlled trials that used theXingnao Kaiqiao needling method to treat ischemic stroke compared with various control treatments such as conventional drugs or other acupuncture therapies. Searched databases included China National Knowledge Infrastructure, Weipu Information Resources System, Wanfang Medical Data System, Chinese Biomedical Literature Database, Cochrane Library, and PubMed, from May 2006 to July 2014. SeLeCTION CRITeRIA: Two authors independently conducted literature screening, quality evaluation, and data extraction. The quality of articles was evaluated according to the Cochrane Reviewers’ Handbook 5.1, and the study was carried out using Cochrane system assessment methods. RevMan 5.2 was used for meta-analysis of the included studies. MAIN OUTCOMe MeASUReS: Mortality rate, recurrence rate, and disability rate were observed. ReSULTS:Nine randomized and semi-randomized controlled trials treating 931 cases of ischemic stroke were included in this review. Meta-analysis results showed that there were no sig-niifcant differences in mortality reduction (risk ratio (RR) = 0.58, 95% conifdence interval (CI): 0.17–1.93,Z = 0.89,P = 0.37) or recurrence rate (RR = 0.55, 95%CI: 0.18–1.70,Z = 1.04,P = 0.30) of ischemic stroke patients between theXingnao Kaiqiao needling and control treatment groups. However, theXingnao Kaiqiao needling method had a tendency towards higher efifcacy in mor-tality reduction and recurrence rates. TheXingnao Kaiqiao needling method was signiifcantly better than that of the control treatment in reducing disability rate (RR = 0.51, 95%CI: 0.27–0.98, Z = 2.03,P 〈 0.05). CONCLUSION:TheXingnao Kaiqiao needling method has a better effect than control treatment in reducing disability rate. The long-term effect ofXingnao Kaiqiao needling against ischemic stroke is better than that of control treatment. However, the limitations of this study limit the strength of the conclusions. Randomized controlled trials with a strict, reasonable design, and multi-center, large-scale samples and follow-up are necessary to draw conclusions aboutXingnao Kaiqiao needling.展开更多

Generally, if acupuncture therapy is used in combination with other rehabilitation methods, the therapeutic effect of shoulder-pain-relief will be much better. Regarding the acupoint selection, both local acupoints and those of the yang meridians are primarily used in clinical practice. Methods:...Objective: To observe the effect of warm joint needling plus rehabilitation techniques on the balance function and quality of life (QOL) of patients with spastic hemiplegia after ischemic cerebral stroke. Methods: Ninety patients with spastic hemiplegia after ischemic cerebral stroke were randomized into a rehabilitation group, a warm joint needling group and an observation group, with 30 cases in each group. The rehabilitation group was intervened by Bobath therapy, the warm joint needling group was treated with joint needling on the affected side plus warm needling, and the observation group was given the same rehabilitation treatment as the rehabilitation group together with the same warm joint needling as the warm joint needling group. The three groups were treated once another day, 1 month as a treatment course for 6 months. Before the treatment, and respectively after 2-week, 1-month, 3-month, and 6-month treatment, the modified Ashworth scale (MAS) was used to measure the anti-spasm ability of the lower limb, the Berg balance scale (BBS) was adopted to evaluate the balance function, and the stroke-specific quality of life scale (SS-QOL) was employed to estimate the QOL. Results: After 3-month and 6-month treatment, the lower-limb MAS scores in the observation group were significantly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). After 1-month, B-month and 6-month treatment, the BBS scores in the observation group were significantly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). After 2-week, 1-month, 3-month and 6-month treatment, the SS-QOL scores in the observation group were markedly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). Conclusion: Warm joint needling plus rehabilitation can effectively improve the lower-limb spasticity state, balance function and QOL in patients with spastic hemiplegia after ischemic cerebral stroke.展开更多
OBJECTIVE: To investigate the effects on the brain using three needling manipulations(twirling, lifting-thrusting, and twirling plus lifting-thrusting)when the right-side Zusanli(ST 36) acupoint was stimulated with ne...OBJECTIVE: To investigate the effects on the brain using three needling manipulations(twirling, lifting-thrusting, and twirling plus lifting-thrusting)when the right-side Zusanli(ST 36) acupoint was stimulated with needles.METHODS: Seventeen healthy subjects accepted three needling manipulations stimulating the right Zusanli(ST 36) over separate days. Functional magnetic resonance imaging was used to detect changes in the brain during the manipulations, and then the needling sensations were recorded using the MGH acupuncture sensation scale(MASS) after each scan. f MRI data were processed using Statistical Parametric Mapping 8 to analyze the positiveand negative activation in the brain induced by different acupuncture manipulations.RESULTS: The individual needling sensations showed no statistically significant differences among the three manipulations. However, the MASS index showed that lifting-thrusting > twirling plus lifting-thrusting > twirling. Lifting-thrusting activated left premotor cortex, left postcentral,right middle frontal gyrus, left inferior frontal gyrus,right lingual gyrus, left insula, right putamen, bilateral cingulate gyrus and right cerebellum; and deactivated bilateral hippocampus and left caudate.Twirling activated bilateral orbital middle frontal gyrus, left opercular and triangular inferior frontal gyrus, and right middle occipital gyrus; and deactivated bilateral precuneus, right amygdala, left anterior cingulate gyrus, right inferior temporal gyrus,right middle frontal gyrus, right supplementary motor gyrus, and left postcentral. Twirling plus lifting-thrusting activated bilateral postcentral, left inferior occipital gyrus, left insula, left thalamus, left cingulate gyrus, and right putamen; and deactivated right superior frontal gyrus, right superior parietal gyrus, right temporal gyrus, right middle occipital gyrus, right insula, and left lingual gyrus. Pairwise comparisons of the three manipulations showed that signals induced by lifting-thrusting were the strongest, especially in the limbic system,followed by twirling plus lifting-thrusting; twirling alone was the weakest.CONCLUSION: Three methods of needling manipulations similarly activated areas associated with the somatosensory system, vision, cognition, and emotional regulation. This may have significant implications for acupuncture in clinical practice.展开更多

Methods: Sixty-eight eligible subjects with post-stoke upper limb dysfunction were randomized into 2 groups by the random number table, 34 in each group, to respectively receive relaxing needling method or ordinary needling method in addition to rehabilitation training for 2 months. FugI-Meyer assessment of upper limb motor function, neurological defect score, and clinical efficacies were observed and compared. Results: The two groups both showed effects. The total effective rate in the relaxing needling group was 93.8% versus 84.9% in the ordinary needling group, and the difference was statistically significant (P〈0.05). The relaxing needling group was better than the ordinary needling group in improving the motor function of upper limb and neurological defect (P〈0.05). Conclusion: Relaxing needling plus rehabilitation training can significantly improve the post-stroke motor dysfunction of upper limb, better than ordinary needling plus rehabilitation training.展开更多

Objective:To objectively evaluate the clinical effect of resisting method of acupuncture at movementpainful points on treating the refractory cases of frozen shoulder during adhesion period,and to compare with that of...Objective:To objectively evaluate the clinical effect of resisting method of acupuncture at movementpainful points on treating the refractory cases of frozen shoulder during adhesion period,and to compare with that of the conventional acupuncture method.Methods:According to the random grouping principle,90 patients with frozen shoulder during adhesion period were divided into"resisting method acupuncture group,general acupuncture group,and physiotherapy group",30 patients in each.In the resisting method acupuncture group,the resisting method of needling was applied at the"movement-painful points",and during acupuncture the patient was asked to do shoulder movements.In the general acupuncture group,the conventional needling method of acupuncture was applied at the conventional three shoulder-acupoints.In the physiotherapy group,the patients were treated with shock wave at the affected shoulder.All groups were treated once a day,5 times a week,a total of 2 weeks as a course of treatment.After one course of treatment,the clinical effect of the three groups was evaluated.The shoulder pain and shoulder function scale(Constant-Murley),Rating Scale of the American Shoulder and Elbow Surgeons(ASES),and infrared thermal imaging temperature change score were observed.Results:After treatment,the shoulder pain was relieved and movement improved in all three groups,whilst the cured and remarkably effective rate of the resisting method acupuncture group was 96.7%(29/30),better than those of the general acupuncture group 86.2%(25/29)and the physiotherapy group 73.3%(22/30),with a statistically significant difference(P<0.05);the VAS score of the resisting method acupuncture group was significantly lower than those of the other two groups(P<0.05),the ConstantMurley score and the ASES score were significantly lower than those of the other two groups(all P<0.05),and the infrared thermal imaging temperature change score was significantly improved in comparing with those of the other two groups,with a statistically significant difference(both P<0.05).Conclusion:The resisting method of needling has an obvious analgesic effect and is able to restore the joint movement function in the treatment of refractory frozen shoulder during adhesion period.展开更多

文摘OBJECTIVE: To evaluate the effectiveness and safety of the Xingnao Kaiqiao needling method for treating acute ischemic stroke.DATA SOURCES: We retrieved relevant randomized controlled trials involving Xingnao Kaiqiao acupuncture for treatment of acute ischemic stroke. The China National Knowledge Infrastructure, Weipu Information Resources System, Wanfang Medical Data System, Chinese Biomedical Literature Database, Cochrane Library, and Pub Med were searched from June 2006 to March 2016.DATA SELECTION: We analyzed randomized and semi-randomized clinical controlled trials that compared Xingnao Kaiqiao acupuncture with various control treatments, such as conventional drugs or other acupuncture therapies, for treatment of acute ischemic stroke. The quality of articles was evaluated according to the Cochrane Handbook for Systematic Reviews of Interventions(Version 5.1), and the study was carried out using Cochrane system assessment methods. Rev Man 5.2 was used for the meta-analysis of the included studies.OUTCOME MEASURES: The mortality rate, disability rate, activities of daily living(Barthel Index), and clinical efficacy were observed.RESULTS: Twelve studies met the inclusion criteria for this review. The meta-analysis showed that between Xingnao Kaiqiao acupuncture and the control treatment, Xingnao Kaiqiao acupuncture reduced the disability rate [risk ratio(RR) = 0.51, 95% confidence interval(CI) = 0.27-0.98, z = 2.03, P 〈 0.05], elevated the activities of daily living(weighted mean difference = 12.23, 95% CI: 3.66-20.08, z = 2.80, P 〈 0.005), and had greater clinical efficacy(RR = 1.61, 95% CI: 1.23-2.09, z = 3.53, P 〈 0.0004). However, there was no significant difference in mortality rate(RR = 0.61, 95% CI: 0.15-2.45, z = 0.70, P 〉 0.05). CONCLUSION: The Xingnao Kaiqiao needling method is effective and safe for acute ischemic stroke. However, there was selective bias in this study, and the likelihood of measurement bias is high. Thus, more high-quality randomized controlled trials are needed to provide reliable evidence of the efficacy and safety of Xingnao Kaiqiao acupuncture in the treatment of acute ischemic stroke.

文摘Purpose: To observe the clinical therapeutic effect of different needling methods in the treatment of peripheral facial paralysis. Methods: A total of 158 cases of peripheral facial palsy patients are randomly divided into treatment group (n=116 cases, who are treated by penetration needling method) and control group (n=42 cases, who are treated with traditional acupuncture method). In treatment group, main acupoint pairs for penetration needling are Yangbai (GB 14) -Yuyao (EX-HN 4), Tongziliao (GB 1) -Taiyang (EX-HN 5), Jiache (ST 6) -Dicang (ST 4) and Chengjiang (CV 24) -Jiachengjiang (EX-HN) on the affected side, and in control group, the main acupoints used are Yifeng (TE 17), Yangbai (GB 14), Sibai (ST 2), Dicang (ST 4), Jiache (ST 6), Jingming (BL 1) and Chengjiang (CV 24). The treatment is given once daily with 10 sessions being a therapeutic course. Results: After 3 courses of treatment, the cure rates of treatment group and control group are 81.8% and 66.7% separately, showing a significant difference between two groups (P<0.05). The total effective rates of treatment and control groups are 96.5% and 88.1% separately, presenting a significant difference between two groups (P<0.01). Conclusion: The therapeutic effect of penetration needling method is better than that of traditional acupuncture method.
文摘Objective To observe the clinical efficacy of Wentong needling method in the treatment of long-term cough cases after common cold. Methods Wentong needling method was used in the treatment of 30 cases with long-term cough after common cold. Acupoints including Quchi (曲池 LI 11, left side), Hegu (合谷, LI 4, left side), Lieque (列缺, LU 7, right side), Fenglong (丰隆', ST 40, right side), Chize (尺泽, LU 5, right side), Zusanli (足三里, ST 36, right side), Zhaohai (照海, KI 6, left side), Taichong (太冲h, LR 3, both sides), and Waiqiu (外丘, GB 36, left side) were selected. The treatment was given every other day. After one treatment course, the relation between the long-term cough cases after common cold and Wentong needling method clinical efficacy was observed from aspects of different ages, disease duration and disease degree. Results There were 18 cured cases (60.0%), 8 markedly effective cases (26.7%), 4 effective cases (13.33%), and 0 invalid case (0.0%). The total effective rate was 200%. The treatment efficacy of cough patients after common cold of less than 24 months was better than that of more than 24 months. Along with the increasing of age, the cured and markedly effective rate was of certain downward tendency. Along with the increasing of treatment times, the cured and markedly effective rate increased. Conclusion The clinical efficacy of Wentong needling method is obvious in the treatment of long-term cough cases after common cold.

文摘Objective To observe the clinical efficacy of the needling method for regulating wei qi and strengthening brain on insomnia.Method Two hundred patients with insomnia were randomly divided into a group of regulating wei qi and strengthening brain(group A) and a group of normal acupuncture(group B)through random number table method,with 100 cases in each group.In group A,Baihui(百会 GV20),Dazhui(大椎 GV 14),Shenmai(申脉 BL62),Zhaohai(照海 KI 6) and auricular points Yuanzhong(缘中 AT 2,3,4i),and Shenmen(神门TF 4) were selected;and in group B,Sishencong(四神聪 EX-HN 1),Shenmen(神门 HT 7),and Sanyinjiao(三阴交 SP 6) were selected.Acupuncture was given on these points once daily for 15 days.Pittsburgh sleep quality index(PSQI) was used for scoring and curative effect was evaluated before and after treatment.Results The total effective rate was 89.0%(89/100) in group A and 65.0%(65/100) in group B with a very significant difference between the two groups(P〈0.01);the difference of PSQI scores before and after treatment was(-9.15±5.68)in group A and(-5.64±5.73)in group B,with a very significant difference before and after treatment in the two groups(P〈0.01).Conclusion The therapeutic effect of the needling method for regulating wei qi and strengthening brain on insomnia is superior to that of normal needling method.

文摘Objective: To observe the therapeutic effect of warm needling method for knee osteoarthritis. Methods: Three hundred cases were randomly divided into treatment group and control group, 150 cases respectively in each group. The treatment group was treated by warm needling method, and the control group was treated by simple acupuncture. Their therapeutic effects were compared after 20 sessions of treatments. Results: The effective rate was 94.0% in the treatment group and 86.0% in the control group, with a significant difference between the two groups (P〈0.05). Conclusion: The therapeutic effect is better in the warm needling group than in the control group in the treatment of knee osteoarthritis.
文摘Objective To explore the clinical efficacy of muscle regions of meridians needling method for refractory facial paralysis affecting different locations.Methods A total of 110 participants were randomized into an observation group and a control group,with 55 cases in each group.The observation group was treated with muscle regions of meridians needling method,and the control group was treated with conventional facial three-line needling method.The clinical efficacy was evaluated four weeks after the treatment.And the infrared imaging spectra of the two groups were examined.Results The total effective rate of the observation group was higher than that of the control group(P<0.05),and its curative effect for refractory facial paralysis located above the geniculate ganglion was better than that of the control group(P<0.05).The color scale distribution of different disease locations in the two groups varied significantly(P<0.05),the higher the disease location,the higher the occurrence rate of cool zone and low temperature zone.After treatment,the reductions of the facial and periotic temperature difference between the healthy side and the affected side in the observation group were statistically different from those in the control group(P<0.05).Conclusion Muscle regions of meridians needling method has a better effect than facial three-line needling method for refractory facial paralysis.It can promote the microcirculation of the affected side of the face,improve the blood and oxygen supply to local tissues,and thus promote the repair of the peripheral facial nerve.

文摘Objective:To evaluate the clinical effect differences between conducting qi and regulating the spirit needling method and regular needling method.Methods:Sixty-nine patients of insomnia were randomly divided into an observation group(35 cases)and a control group(34 cases).Bǎihuì(百会GV20),Sìshéncōng(四神聪EX-HN1),Hégǔ(合谷LI4),Tàichōng(太冲LR3),Zúsānlǐ(足三里ST36)and Sānyīnjiāo(三阴交SP6)were selected in the two groups.The patients in the observation group were treated with needling method for conducting qi and regulating the spirit.The patients in the control group were treated with the regular needling method.The treatment was given once every other day,both groups were treated for 12 times.Pittsburgh Sleep Quality Index(PSQI)and Epworth Sleepiness Scale(ESS)were observed before and after the treatment and the clinical effect was evaluated.Results:After treatment,the total score and the score of each item of PSQI as well as the result of ESS were all improved after the treatment in the two groups(all P<0.05).The improvements in the total score of PSQI and the result of ESS in the observation group were better than the control group(both P<0.05).The total effective rate was 88.57%(31/35)in the observation group,significantly superior to 76.47%(26/34)in the control group(P<0.05).Conclusion:The conducting qi and regulating the spirit needling method achieves a better effect on insomnia as compared with the regular needling method.This method achieves a remarkable improvement in sleep latency,sleep quality and daytime sleepiness.
文摘Objective To evaluate the clinical efficacy of tube moxibustion plus point-toward-point needling method for refractory facial paralysis.Methods A total of 100 patients with refractory facial paralysis who met the inclusion criteria were randomized into an observation group and a control group,with 50 cases in each group.Both groups were treated with acupuncture by point-toward-point needling method,mainly in the affected eye,cheek and mouth areas.The observation group was given additional tube moxibustion after receiving the point-toward-point needling method,which inserted a tube moxibustion device into the external acoustic meatus 1 cm on the affected side for 20 min.Both groups were treated once a day,6 times a week,for 4 consecutive weeks.The House-Brackmann scale was scored before and after treatment,and the facial nerve electromyogram data were collected.Results The total effective rate of the observation group was 93.6%,which was higher than 64.6%of the control group,and the difference between the two groups was statistically significant(P<0.05).The proportion of new-onset facial paralysis complications(facial synkinesis,facial spasm,facial paralysis perversion,and crocodile tears)in the observation group was 6.4%,which was lower than 35.4%in the control group.The difference between the two groups was statistically significant(P<0.05).In the observation group,the numbers of cases after treatment with the ratio of action potential amplitude between the affected side and the healthy side increased by 10%–30%and over 30%were more than those in the control group.And the difference between the two groups was statistically significant(P<0.05).Conclusion Tube moxibustion plus point-toward-point needling method has a better effect in improving symptoms of refractory facial paralysis,decreasing the incidence of sequelae,and increasing clinical efficacy than the point-toward-point needling method alone.

SeLeCTION CRITeRIA: Two authors independently conducted literature screening, quality evaluation, and data extraction. The quality of articles was evaluated according to the Cochrane Reviewers’ Handbook 5.1, and the study was carried out using Cochrane system assessment methods. RevMan 5.2 was used for meta-analysis of the included studies. MAIN OUTCOMe MeASUReS: Mortality rate, recurrence rate, and disability rate were observed. ReSULTS:Nine randomized and semi-randomized controlled trials treating 931 cases of ischemic stroke were included in this review. Meta-analysis results showed that there were no sig-niifcant differences in mortality reduction (risk ratio (RR) = 0.58, 95% conifdence interval (CI): 0.17–1.93,Z = 0.89,P = 0.37) or recurrence rate (RR = 0.55, 95%CI: 0.18–1.70,Z = 1.04,P = 0.30) of ischemic stroke patients between theXingnao Kaiqiao needling and control treatment groups. However, theXingnao Kaiqiao needling method had a tendency towards higher efifcacy in mor-tality reduction and recurrence rates. TheXingnao Kaiqiao needling method was signiifcantly better than that of the control treatment in reducing disability rate (RR = 0.51, 95%CI: 0.27–0.98, Z = 2.03,P 〈 0.05). CONCLUSION:TheXingnao Kaiqiao needling method has a better effect than control treatment in reducing disability rate. The long-term effect ofXingnao Kaiqiao needling against ischemic stroke is better than that of control treatment. However, the limitations of this study limit the strength of the conclusions. Randomized controlled trials with a strict, reasonable design, and multi-center, large-scale samples and follow-up are necessary to draw conclusions aboutXingnao Kaiqiao needling.
文摘In the present paper, the authors review some new results of acupuncture treatment of shoulder pain in stroke patients from ① application of filiform needle with different needling manipulations, ② selection and combination of acupoints, and ③ some special needling methods including point-injection. Generally, if acupuncture therapy is used in combination with other rehabilitation methods, the therapeutic effect of shoulder-pain-relief will be much better. Regarding the acupoint selection, both local acupoints and those of the yang meridians are primarily used in clinical practice. Further studies on treating shoulder pain of hemiplegia based upon the basic theory of TCM are necessary.

文摘Objective: To observe the effect of warm joint needling plus rehabilitation techniques on the balance function and quality of life (QOL) of patients with spastic hemiplegia after ischemic cerebral stroke. Methods: Ninety patients with spastic hemiplegia after ischemic cerebral stroke were randomized into a rehabilitation group, a warm joint needling group and an observation group, with 30 cases in each group. The rehabilitation group was intervened by Bobath therapy, the warm joint needling group was treated with joint needling on the affected side plus warm needling, and the observation group was given the same rehabilitation treatment as the rehabilitation group together with the same warm joint needling as the warm joint needling group. The three groups were treated once another day, 1 month as a treatment course for 6 months. Before the treatment, and respectively after 2-week, 1-month, 3-month, and 6-month treatment, the modified Ashworth scale (MAS) was used to measure the anti-spasm ability of the lower limb, the Berg balance scale (BBS) was adopted to evaluate the balance function, and the stroke-specific quality of life scale (SS-QOL) was employed to estimate the QOL. Results: After 3-month and 6-month treatment, the lower-limb MAS scores in the observation group were significantly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). After 1-month, B-month and 6-month treatment, the BBS scores in the observation group were significantly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). After 2-week, 1-month, 3-month and 6-month treatment, the SS-QOL scores in the observation group were markedly better than those in the rehabilitation group and the warm joint needling group (all P〈0.05). Conclusion: Warm joint needling plus rehabilitation can effectively improve the lower-limb spasticity state, balance function and QOL in patients with spastic hemiplegia after ischemic cerebral stroke.

文摘OBJECTIVE: To investigate the effects on the brain using three needling manipulations(twirling, lifting-thrusting, and twirling plus lifting-thrusting)when the right-side Zusanli(ST 36) acupoint was stimulated with needles.METHODS: Seventeen healthy subjects accepted three needling manipulations stimulating the right Zusanli(ST 36) over separate days. Functional magnetic resonance imaging was used to detect changes in the brain during the manipulations, and then the needling sensations were recorded using the MGH acupuncture sensation scale(MASS) after each scan. f MRI data were processed using Statistical Parametric Mapping 8 to analyze the positiveand negative activation in the brain induced by different acupuncture manipulations.RESULTS: The individual needling sensations showed no statistically significant differences among the three manipulations. However, the MASS index showed that lifting-thrusting > twirling plus lifting-thrusting > twirling. Lifting-thrusting activated left premotor cortex, left postcentral,right middle frontal gyrus, left inferior frontal gyrus,right lingual gyrus, left insula, right putamen, bilateral cingulate gyrus and right cerebellum; and deactivated bilateral hippocampus and left caudate.Twirling activated bilateral orbital middle frontal gyrus, left opercular and triangular inferior frontal gyrus, and right middle occipital gyrus; and deactivated bilateral precuneus, right amygdala, left anterior cingulate gyrus, right inferior temporal gyrus,right middle frontal gyrus, right supplementary motor gyrus, and left postcentral. Twirling plus lifting-thrusting activated bilateral postcentral, left inferior occipital gyrus, left insula, left thalamus, left cingulate gyrus, and right putamen; and deactivated right superior frontal gyrus, right superior parietal gyrus, right temporal gyrus, right middle occipital gyrus, right insula, and left lingual gyrus. Pairwise comparisons of the three manipulations showed that signals induced by lifting-thrusting were the strongest, especially in the limbic system,followed by twirling plus lifting-thrusting; twirling alone was the weakest.CONCLUSION: Three methods of needling manipulations similarly activated areas associated with the somatosensory system, vision, cognition, and emotional regulation. This may have significant implications for acupuncture in clinical practice.

文摘Objective: To observe the effect of relaxing needling plus rehabilitation training on post-stroke upper limb dysfunction. Methods: Sixty-eight eligible subjects with post-stoke upper limb dysfunction were randomized into 2 groups by the random number table, 34 in each group, to respectively receive relaxing needling method or ordinary needling method in addition to rehabilitation training for 2 months. FugI-Meyer assessment of upper limb motor function, neurological defect score, and clinical efficacies were observed and compared. Results: The two groups both showed effects. The total effective rate in the relaxing needling group was 93.8% versus 84.9% in the ordinary needling group, and the difference was statistically significant (P〈0.05). The relaxing needling group was better than the ordinary needling group in improving the motor function of upper limb and neurological defect (P〈0.05). Conclusion: Relaxing needling plus rehabilitation training can significantly improve the post-stroke motor dysfunction of upper limb, better than ordinary needling plus rehabilitation training.
文摘Objective: To observe the clinical efficacy of nerve-near needling for occipital neuralgia. Methods: Eighty-two subjects were randomly allocated into two groups: treatment group in which 46 cases was treated by nerve-near needling [points Tianzhu (BL 10) and Tianyou (TE 16) were selected], and control group in which 36 cases were treated by conventional acupuncture [points Tianzhu(BL 10), Fengchi(GB 20), Baihui(GV 20), Shimian II (Extra), Wangu (GB 12) and Hegu(LI 4) were selected]. One course of treatment consisted of 3-5 times, and three courses of treatment were given at most before calculating efficacy. Results: The short-term total effective rate was 93.5% in the treatment group and 69.4% in the control group, with a statistically significant difference between the two groups (P〈0.01); the long-term total effective rate was 64.1% in the treatment group and 43.3% in the control group, with a statistically significant difference between the two groups (P〈0.05). Conclusion: Nerve-near needling is better than conventional acupuncture in treating occipital neuralgia.
文摘Objective:To observe the clinical effect of aligned needling method in treating idiopathic blepharospasm.Methods:Seventy-eight patients with idiopathic blepharospasm were randomized into two groups.Forty-six cases in the treatment group were treated with aligned needling method applied to orbicularis oculi muscles;32 cases in the control group were treated with anticonvulsants.Therapeutic effects were compared after 3 treatment courses.Results:The total effective rate was 93.5%in the treatment group versus75.0%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Aligned needling method applied to orbicularis oculi muscle has better therapeutic effect than anticonvulsants in treating blepharospasm.
文摘Depending upon the descriptions of the ancient literature, such as Jin Zhen Fu (Odes to Golden Needles, 《金针赋》), the dragon-tiger fighting needling technique has the effects to dredge the meridians, circulate qi, activate blood, stop and remove pain. "Dragon" implies to twist the needle leftward for reinforcing, and "tiger" means to twist the needle rightward for reducing. To twist the needle leftward and rightward alternately is termed "fighting" After the arrival of the needling sensation by inserting the needle to the proper depth, the needle is first twisted leftward with the thumb forward in a method of nine yang numbers, and then twisted backward and rightward with the thumb backward in a method of six yin numbers, in alternation of reinforcing and reducing repeatedly. Therefore, the general information on clinical application of dragon-tiger fighting needling technique in the recent sixteen years is given in the following.

文摘Objective:To objectively evaluate the clinical effect of resisting method of acupuncture at movementpainful points on treating the refractory cases of frozen shoulder during adhesion period,and to compare with that of the conventional acupuncture method.Methods:According to the random grouping principle,90 patients with frozen shoulder during adhesion period were divided into"resisting method acupuncture group,general acupuncture group,and physiotherapy group",30 patients in each.In the resisting method acupuncture group,the resisting method of needling was applied at the"movement-painful points",and during acupuncture the patient was asked to do shoulder movements.In the general acupuncture group,the conventional needling method of acupuncture was applied at the conventional three shoulder-acupoints.In the physiotherapy group,the patients were treated with shock wave at the affected shoulder.All groups were treated once a day,5 times a week,a total of 2 weeks as a course of treatment.After one course of treatment,the clinical effect of the three groups was evaluated.The shoulder pain and shoulder function scale(Constant-Murley),Rating Scale of the American Shoulder and Elbow Surgeons(ASES),and infrared thermal imaging temperature change score were observed.Results:After treatment,the shoulder pain was relieved and movement improved in all three groups,whilst the cured and remarkably effective rate of the resisting method acupuncture group was 96.7%(29/30),better than those of the general acupuncture group 86.2%(25/29)and the physiotherapy group 73.3%(22/30),with a statistically significant difference(P<0.05);the VAS score of the resisting method acupuncture group was significantly lower than those of the other two groups(P<0.05),the ConstantMurley score and the ASES score were significantly lower than those of the other two groups(all P<0.05),and the infrared thermal imaging temperature change score was significantly improved in comparing with those of the other two groups,with a statistically significant difference(both P<0.05).Conclusion:The resisting method of needling has an obvious analgesic effect and is able to restore the joint movement function in the treatment of refractory frozen shoulder during adhesion period.
