Thus, it is expected that a nurse manager occupies specific roles suited for the above characteristics that are different from those of medical facilities. Thus, we aimed to clarify the role expected of nurse managers via a questionnaire survey administered to nurses and care workers in GHSF. Methods: We used a descriptive, cross-sectional design. The study was conducted in 56 GHSF all over Japan. The instrument used for data collection was a questionnaire. The questionnaires consisted of 35 items for determining role expectation of nurse managers, the background information of respondents. The role of nurse managers was analyzed using principal factor analysis (promax rotation). Findings: A total of 259 nurses and care workers participated in this study. We extracted 34 items by factor analysis, which were classified into three factors (promotion of home nursing, management of medical care, and environment creation for collaboration and education). Among care workers, four factors consisting of 33 items were extracted. Factors one, two, and three were similar to those of nurses;“intervention to ethical problem” was extracted as the additional fourth factor. Conclusion: The roles required for nurse managers in GHSF are the promotion of home nursing, which is the original role of the health services facilities, exhibition of the specialty as a healthcare professional, creation of a work environment that promotes mutual understanding for collaboration with other professionals, and enhancement of the education and training system.展开更多

It is unknown whether abnormal vital signs are acted upon or not and how this affects the outcome of the IHCA. Aim: Compare differences in journal notes regarding abnormal vital signs or worry by nurses up until 24 h between survivors and non-survivors after an in-hospital cardiac arrest (IHCA). Design: Pragmatic retrospective case-control study in a Swedish university hospital. Methods: All IHCA during 2007-2011 was reviewed (n = 720). Out of them, 20 (3%) fulfilled the inclusion criteria; survived 30 d, had their IHCA at a general ward, were aged 〉 18 years and had documented abnormal vital signs or nurse worries. Out of the non-survivors, two controls were after matching for age, sex and number of diseases randomly drawn for each case. Pearson's chi test was used to assess significance on the level of 0.05 in differences between survivors and non-survivors. Results: Of 20 survivors with preceding abnormal vital signs prior to IHCA, 15 patients (75%) had documented worries or action taken by a nurse compared to 23 patients (58%) among non-survivors (p-value: 0.258). Conclusion: The journal documentation 24 h prior to a 1HCA was fairly equal in numbers between patients surviving at least 30 d afterwards compared to those not surviving, but the content of the journal notes had a slightly higher, but not statistical significant, frequency of worry or action taken by attending nurses in survivors.展开更多

What kinds of management skills are required/important currently and also in the future? The view for this might be different according to the present position of midwives. We here attempted to determine this;we divided study population into three (head nurses, assistant head nurses and advanced midwives). A self-administered questionnaire survey was conducted for 1652 advanced midwives in hospitals in Japan during 2018 and 2019. Scores in 8 items showed significant differences among the three groups. The skill with the largest difference among the three groups was the ability to participate in and provide support for social activity. Although higher levels of management skills will be required for nursing managers in the next generation, the levels differ depending on current working positions in advanced midwives.展开更多

文摘Back ground: The characteristics of geriatric health services facilities (GHSF) in Japan include provision of care focused on the life of the residents, and there is a need for cooperation between care workers and physical therapists responsible for the care. Conclusion: The roles required for nurse managers in GHSF are the promotion of home nursing, which is the original role of the health services facilities, exhibition of the specialty as a healthcare professional, creation of a work environment that promotes mutual understanding for collaboration with other professionals, and enhancement of the education and training system.

文摘Objective:Nurse’s dimission and attrition are globally considered as a public health issue. However, few studies have focused on the nurse shortage from the perspective of intent to stay,as previous studies have focused only on why they left. The purpose of this study was to conduct an integrative review of the factors connected with why nurses in China,Japan, and Korea stay in their current workplace. Methods:The review was done using three databases namely CNKI,Wanfang, and Web of science. The relevant studies published by Chinese,Japanese, and Koreans from 2010 were also included in this review.Literature screening and data extraction were performed by the two researchers, and the qualitative research methods were used for analysis. Results:A total of 17 studies were analyzed in this review and of these two were qualitative and 15 were quantitative.Three themes and six sub-themes emerged from the synthesization of the data of 17 studies, which will help us to find the factors for nurses’intent to stay. The three aspects such as professional characteristics, nurses’individual characteristics, and organizational factors are the main contributing factors of nurses’intent to stay. Conclusions:This integrated review has thrown some important factors about nurses’intent to stay.It is increasingly clear that when(1)nurses have a good professional status,(2)nurses could enjoy a good working relationship,(3)the workplace could meet the needs of personal development,and(4)nurses have good organizational support and excellent leaders, they are inclined to stay in their current jobs.Managers need to adopt targeted measures to improve nurses’intent to stay and to provide a practical reference for health care institutions and managers in different countries and areas to increase the retention rate of nurses and to alleviate the current situation of nurse shortage.
文摘Objective Team cohesion is an essential component in a multidisciplinary team and can inspire the team to perform creative and complex works. This descriptive correlational study aimed to determine the styles of conflict management, levels of team cohesion, and the relationship between each style of conflict management and perceived team cohesion among nurses at affiliated hospitals of Dali University. Methods The sample consisted of 336 nurses from two university hospitals in Yunnan province, selected using the proportionate sampling method.Research instruments included demographic data form,Dutch Test for Conflict Handling(DUTCH)and Group Cohesion Scale. The Cronbach’s alpha coefficient for the five subscales of DUTCH were the following:forcing(0.83),yielding(0.81),avoiding(0.88),problem-solving(0.85),and compromising(0.84).The Cronbach’s alpha coefficient of Group Cohesion Scale was 0.83.Data were analyzed using descriptive statistics and Spearman’s rank-order correlations. Results The results of this study revealed that nurses were most likely to use avoiding conflict management style to deal with conflict situations, followed in rank order by problem-solving,compromising,forcing and yielding conflict management style.Nurses perceived team cohesion at a moderate level(x=4.43,SD=2.01).Additionally,conflict management associated with team cohesion. Conclusion The study findings revealed that avoiding was the conflict management style most frequently utilized by nurses, and nurses perceived team cohesion was at a moderate level. It is required to identify various strategies to improve team cohesion in multidisciplinary teams and develop a support system for nurses.
文摘Presence of abnormal vital signs prior to IHCA and consequently higher mortality has been found in numerous studies. Conclusion: The journal documentation 24 h prior to a 1HCA was fairly equal in numbers between patients surviving at least 30 d afterwards compared to those not surviving, but the content of the journal notes had a slightly higher, but not statistical significant, frequency of worry or action taken by attending nurses in survivors.
文摘Similar to any other discipline/field, management skills are important in perinatal nursing/medicine. Although higher levels of management skills will be required for nursing managers in the next generation, the levels differ depending on current working positions in advanced midwives.
文摘Objective:To explore the effectiveness of the competency model for nursing managers in the training of newly appointed head nurses. Methods:Eighteen newly appointed head nurses from Yichang Central People’s Hospital, selected between August 2023 and July 2024,were chosen as the research subjects.Based on the nursing manager competency model, corresponding training programs were developed, and these 18 head nurses were trained accordingly. Results:After the training, all newly appointed head nurses passed the theoretical assessments, with a pass rate of 100.00%.The pass rates for two rounds of assessment were 100.00%and 94.54%,respectively.There was no significant difference in the performance evaluation scores between new and senior head nurses in terms of standardized services, nursing quality, nursing research and teaching, and patient satisfaction(P>0.05).Conclusion:The competency model for nursing managers has a significant effect on the training of newly appointed head nurses. It effectively enhances management awareness, facilitates role transition, and,to some extent, improves the management and leadership capabilities of head nurses. It holds high value for broader implementation.
文摘Aim:The present study assesses the nurse managers’perception of clinical supervision.Method:A cross-sectional,descriptive,non-experimental quantitative research design was utilized in three governmental hospitals in Madinah Munawara city for a purposive sample of different levels of nurse managers(n=84)by using a questionnaire. Results:The total knowledge of nurses managers regarding clinical supervision domains, where 59.5%of participants had a high level of knowledge, with mean±standard deviation(47.262±11.151).The highest mean was in the clinical supervision-facilitating learning domain(18.464±4.155),with 69%of participants having a high level of knowledge regarding this domain.Safety and quality in the clinical supervision domain represent the lowest mean of knowledge(4.607±1.628).The level of total skills of nurses managers regarding clinical supervision domains, where 54.8%of participants had a high level of total skill while the range 19-60,with mean±standard deviation was(44.167±8.958).The highest mean was in the c linical supervision-f acilitating learning domain(17.405±3.671),with 48.8%of participants having an increased knowledge of this domain.Safety and quality in the clinical supervision domain represent the lowest mean of skills(3.952±1.270),with more than half 58.3%of nurse managers having average skills in the same domain. Conclusion:In the Al-Madinah region, there is no single policy or system of clinical supervision;rather,it is based on the needs of each organization.Clinical supervision should be handled appropriately and supportive.Hospital management should offer proper training for newly appointed first-line nurse managers with adequate, regular, and timely feedback on their strengths and weaknesses in their skills.Clinical supervision should be taught as a separate curriculum in nursing administration courses.Hospital managers should create a system, culture and environment that fosters high ethical standards and competent clinical monitoring. The right to supervision, feedback, help, appropriate working circumstances, and respect must be guaranteed to trainees and supervisors in this system.
