This improved method for determination of weight of the evaluating indicators was applied in water quality assessment of the Three Gorges reservoir area. The results showed that this method was favorable for fuzzy synthetic evaluation when there were more than one evaluating objects. One calculation was enough for calculating every monitoring point. Compared with the original evaluation method, the method predigested the fuzzy synthetic evaluation process greatly and the evaluation results are more reasonable.展开更多

Colonoscopy with removal of detected polyps has been shown to reduce the incidence and mortality of subsequent CRC. In many countries, population screening programs for CRC have been initiated, either by selection of patients for colonoscopy with fecal occult blood testing or by offering colonoscopy directly to average-risk individuals. Several endoscopy societies have formulated quality indicators for colonoscopy. These quality indicators are almost always incorporated as process indicators, rather than outcome measures. This review focuses on the quality indicators bowel preparation, cecal intubation rate, withdrawal time, adenoma detection rate, patient comfort, sedation and complication rate, and discusses the scientific evidence supporting them,as well as their potential shortcomings and issues that need to be addressed. For instance, there is still no clear and generally accepted definition of adequatebowel preparation, no robust scientific evidence is available supporting a cecal intubation rate ≥ 90% and the association between withdrawal time and occurrence of interval cancers has not been clarified. Adenoma detection rate is currently the only quality indicator that has been shown to be associated with interval colorectal cancer, but as an indicator it does not differentiate between subjects with one or more adenoma detected.展开更多

Post-ERCP pancreatitis is an important concern, and prophylaxis, cannulation and other related technical procedures have been well documented by endoscopists. In addition, medical radiation exposure is of great concern in the general population because of its rapidly increasing frequency and its potential carcinogenic effects. International organizations and radiological societies have established diagnostic reference levels, which guide proper radiation use and serve as global standards for all procedures that use ionizing radiation. However, data on gastrointestinal fluoroscopic procedures are still lacking because the demand for these procedures has recently increased. In this review, we present the current status of quality indicators for ERCP and the methods for measuring radiation exposure in the clinical setting as the next quality indicator for ERCP. To reduce radiation exposure, knowledge of its adverse effects and the procedures for proper measurement and protection are essential. Additionally, further studies on the factors that affect radiation exposure, exposure management and diagnostic reference levels are necessary. Then, we can discuss how to manage medical radiation use inthese complex fluoroscopic procedures. This knowledge will help us to protect not only patients but also endoscopists and medical staff in the fluoroscopy unit.展开更多

The European Society of Gastrointestinal Endoscopy guideline for pediatric endoscopy and North American Society for Pediatric Gastroenterology, Hepatology and Nutrition training guidelines were used as benchmarks.RESULTS Median(± SD) age of 121 children [males = 74(61.2%)] who had 177 colonoscopies was 7.0(± 4.6) years. On average, 30 colonoscopies were performed each year(range: 19-58). Except for investigations of abdominal pain(21/177, 17%), indications for colonoscopies were appropriate in the remaining 83%. Bowel preparation was good in 87%. One patient(0.6%) with severe Crohn's disease had bowel perforation. Cecum examination and ileal intubation rate was 95% and 68.1%. Ileal intubation rate was significantly higher in diagnosing or assessing inflammatory bowel disease(IBD) than non-IBD(72.9% vs 50.0%; P = 0.016). Performance of four trainees was consistent throughout the study period. Average cecum and ileal examination rate among trainees were 97% and 77%.CONCLUSION Benchmarking against established guidelines helps units with a low-volume of colonoscopies to identify area for further improvement.展开更多

However, the symptom of laying hens usually is not typical and hard to diagnosis. In the present study, it is studied that the influences of SE infection on layers' performance, egg quality and blood biochemical indicators. It will help us to improve the strategy to control SE infection in commercial layers. One hundred layers at 20 wk of age were divided into 2 groups, 60 hens for experiment and others for control. The experiment group was fed with the dosage of 108 CFU SE per hen. The specific PCR was used to detect the deposition of SE. On the 8 d after SE infection, 10 hens from the control group and 30 hens from the experimenta group were slaughtered to detect the SE colonization. The production performance, egg quality and blood biochemical indices were also analyzed. Results: The results showed that the colonization rate of SE was highest in caecum contents (55.17%) and lowest in vagina (17.24%). For the eggs the detection rate of SE was highest on the eggshell (80.00%) and lowest in yolk (18.81%). SE infection had no significant influence on production performance and egg qualities (P 〉 0.05). The difference of laying rate between the experimental and control groups was less than 0.30%, and both were approximately equal to 82.00%. The blood analysis showed that the aspartic aminotransferase (AST) and alanine aminotransferase (ALT) of experimental group was significantly higher than those of control group (P 〈 0.05). For experimental and control groups AST values were 236.22 U/I and 211.84 U/I respectively, and ALT values were 32.19 U/I and 24.55 U/I. All of coefficients were less than 20%. The colonization of SE in organs increases the enzyme activities of AST and ALT in blood. Conclusions: SE in feed could invade the oviduct and infect the forming eggs. It significantly increased the concentration of ALT and AST in blood. However,SE infection was hard to be observed from the appearances of layer and egg. It might be a dangerous risk to human health.展开更多

METHODS: A national quality control study of EUS was performed with expanded international comparison. Ten different healthcare institutions in Israel participated in coordination with University of Chicago Medical Center. Each Israeli center provided ten patient reports, compared with twenty reports from University of Chicago Medical Center. Quality indicator forms were prepared with sections to be completed before, during, and after EUS. Physician compliance to all listed indicators wasevaluated. Quality indicators were evaluated prior to, during, and after performing EUS. RESULTS: One hundred different EUS procedural reports were analyzed. The mean patient age was 59 years old. Indications for referral were mostly for pancreatic or biliary reasons. QC showed several strongly reported areas, including indications for EUS(97%), anesthesia given(94%), periprocedural pancreatic evaluation(87%), and an overall summary of the EUS examination(82%). Intermediately reported areas included patients' pertinent past medical history(71.7%), evaluation of the biliary tree(63%), and providing medical guidance about potential procedural adverse events, including pancreatitis and bleeding(52%). Half of the reports(50%) did not include a systemic organ evaluation. Other areas, including systematic reporting of screened organs(36%), description of fine needle aspiration(15%), tumor description via tumor-nodemetastasis(5%), and listing of adverse events(0%) were largely lacking from procedural documentation. CONCLUSION: Documenting specific EUS quality indicators including listing post-procedural recommendations may improve the quality and efficiency of future EUS examinations and subsequent patient follow-up.展开更多

Quality improvement projects were instituted in 2011 in line with ISO 15189 requirements for accreditation of medical laboratory. In this study we evaluated the performance of the laboratory through some set of quality indicators (QI). Methodology: This was a retrospective study to evaluate laboratory QIs monitored from January 2011 to December 2013. The QIs were specimen rejection rate (SSR), turnaround time (TAT), proficiency testing performance (PTP) and client satisfaction survey (CSS). Data was collected into an excel file for analysis and percentage performance compared among years. SSR & TAT were evaluated with the Sigma scale. Results: A total of 7920 (2194 in 2011, 2715 in 2012, 3011 in 2013) specimens were received for testing. 22 (0.28%) specimens were rejected and 81 (1.02%) specimens’ results were reported after the acceptable TAT, giving a Sigma level of 4.27 and 3.82 for SSR and TAT respectively. There was steady improvement in PTP: CD4+ from 67% in 2011 to 90% in 2013, hematology from 81% in 2012 to 83% in 2013, blood film reading 79% in 2011 to 83% in 2013 and chemistry from 90% in 2011 to 93% in 2013. HIV serology recorded 100% throughout. CSS increased from 59% in 2012 to 78% in 2013. However, there was no statistically significance difference reported for PTP and CSS over the years (P > 0.05). Conclusion: The study highlights the need to continuously evaluate QIs and calls for more effort to improve on PTP and focuses on understanding and improving on clients concerns.展开更多

To maintain quality in health services provided, organizations and other stakeholders in maternal and child health recommend regular quality measurement. Quality indicators are the key components in the quality measurement process. However, the literature shows neither an indicator selection process nor a set of quality indicators for quality measurement that is universally accepted. The lack of a universally accepted quality indicator selection process and set of quality indicators results in the establishment of a variety of quality indicator selection processes and several sets of quality indicators whenever the need for quality measurement arises. This adds extra processes that render quality measurement process. This study, therefore, aims to establish a set of quality indicators from a broad set of quality indicators recommended by the World Health Organization (WHO). The study deployed a machine learning technique, specifically a random forest classifier to select important indicators for quality measurement. Twenty-nine indicators were identified as important features and among those, eight indicators namely maternal mortality ratio, still-birth rate, delivery at a health facility, deliveries assisted by skilled attendants, proportional breach delivery, normal delivery rate, born before arrival rate and antenatal care visit coverage were identified to be the most important indicators for quality measurement.展开更多

文摘Considering the difficulty of fuzzy synthetic evaluation method in calculation of the multiple factors and ignorance of the relationship among evaluating objects, a new weight evaluation process using entropy method was introduced. Compared with the original evaluation method, the method predigested the fuzzy synthetic evaluation process greatly and the evaluation results are more reasonable.
文摘Colonoscopy is the diagnostic modality of choice for investigation of symptoms suspected to be related to the colon and for the detection of polyps and colorectal cancer(CRC). Adenoma detection rate is currently the only quality indicator that has been shown to be associated with interval colorectal cancer, but as an indicator it does not differentiate between subjects with one or more adenoma detected.
文摘Endoscopic retrograde cholangiopancreatography(ERCP) is one of the most frequently used image-guided procedures in gastrointestinal endoscopy. This knowledge will help us to protect not only patients but also endoscopists and medical staff in the fluoroscopy unit.

文摘AIM To study implications of measuring quality indicators on training and trainees' performance in pediatric colonoscopy in a low-volume training center.METHODS We reviewed retrospectively the performance of pediatric colonoscopies in a training center in Malaysia over 5 years(January 2010-December 2015), benchmarked against five quality indicators: appropriateness of indications, bowel preparations, cecum and ileal examination rates, and complications. Average cecum and ileal examination rate among trainees were 97% and 77%.CONCLUSION Benchmarking against established guidelines helps units with a low-volume of colonoscopies to identify area for further improvement.

文摘Background: Salmonella enterica serovar Enteritidis (SE), as a major cause of foodborn illness, infects humans mainly through the egg. It might be a dangerous risk to human health.

文摘AIM: To perform a quality control(QC) review of endoscopic ultrasound(EUS) with emphasis on current consensus established quality indicators. CONCLUSION: Documenting specific EUS quality indicators including listing post-procedural recommendations may improve the quality and efficiency of future EUS examinations and subsequent patient follow-up.

文摘A total of 60 crossbred(Large White×Landrace) pigs of halothane genotype NN(castrated males and females) were allotted to three treatments:3 h lairage with toys,3 h lairage and 0 h lairage in a randomized complete block design and used to evaluate the influence of lairage conditions on behavior, biochemical indicators and meat quality for finishing pigs at slaughter.Behavior of the pigs was scored subjectively during lairage.Blood samples were taken at exsanguination s to measure blood temperature, plasma Cortisol,ACTH,glucose, lactate, plasma enzymes and hematological indices.Post-mortem meat quality measurements included muscle colour value(MCV),electrical conductivity(EC),pH at 45 min and 24 h from Longissimus thoracis(LM) and Semimembranosus(SM) mucles and drip loss from LM. The results showed that 3 h lairage group with toys demonstrated significantly improved behavior than the group without toys at 3 sampling times. All the pigs showed increasing calmness as the time of lairage progressed. The omission of lairage increased plasma Cortisol,ACTH,glucose and lactate(P【0.05),and decreased plasma lactate dehydrogenase(LDH),and creatine kinase(CK)(P【0.05).No biochemical index was influenced by the presence or absence of toys during lairage(P【0.05).Muscle colour value, electrical conductivity,pH at 45 min and 24 h from LM and SM and drip loss were not affected by any treatment(P【0.05).Pigs provided 3 h lairage, with or without toys, exhibited lower red blood cell(RBC), hemoglobin(HGB),and haematocrit(HCT) when compared to 0 h lairage.3 h lairage with or without toys resulted in higher white blood cell(WBC) and lymphocyte(W-SCC) levels than 0 h lairage.None of the hemocytic indices in pigs given lairage was affected by the presence or absence of toys.We conclude from this pilot study that in local commercial conditions, from the point of view of animal welfare and meat quality, lairage time of 3 h after short travel was beneficial.Pigs resting showed increased relief from stress and a recovery in immune competence.Holding pigs in lairage with toys for a few hours after arrival at the abattoir may be beneficial for the animal’s well-being.
文摘Background: In January 2010, the implementation of quality management systems toward WHO-AFRO laboratory accreditation commenced in the Antiretroviral Treatment Laboratory of the Infectious Disease Hospital, Kano, Nigeria. Conclusion: The study highlights the need to continuously evaluate QIs and calls for more effort to improve on PTP and focuses on understanding and improving on clients concerns.

文摘In order to find out the correlation between tobacco quality evaluation indicators in China's traditional " moderate aroma" tobaccoproducing areas and simplify the tobacco quality evaluation indicators,we evaluate the appearance quality and smoking quality of 143 flue-cured tobacco leaf samples in China's " moderate aroma" tobacco-producing areas, test the physical traits and chemical component, and analyze the canonical correlation between four quality evaluation indicators. The results show that there is significant or extremely significant canonical correlation between four evaluation indicators( tobacco smoking quality, chemical component, appearance quality and physical trait quality); the cumulative variance contribution rate of evaluation indicators is in the order of chemical component( 69. 17%) > appearance quality( 68. 76%) > physical traits( 64. 13%); appearance quality is most closely related to physical traits( 93. 84%). The individual indicators for tobacco quality evaluation make different contribution to the correlation between quality evaluation indicators. The chemical component evaluation indicators mainly include total sugar and ratio of total sugar to betaine; sensory taste indicators mainly include aroma volume, smoke concentration, irritation and softness degree; physical trait evaluation indicators mainly include leaf weight, leaf length and leaf density; appearance quality indicators mainly include leaf organizational structure, color, maturity and identity. Studies have shown that in the large-scale ecoregion, using canonical correlation analysis to simplify tobacco quality evaluation indicators is feasible.

文摘The aim of this study was to establish a scientific evaluation system for fruit radish quality. 8 quality indicators were measured for23 varieties, such as Meinong,Qiaotouqing and Chunbulao. The indicators included root weight, root shape index, water content, crispness, soluble solids, crude fiber, isothiocyanate and vitamin C. These data were analyzed by variation correlation analysis, principal component analysis and system cluster analysis. 23 varieties of fruit radish were analyzed by variation correlation; 8 indicators were compressed into 6 integrated variables using principal component analysis. Finally, the main evaluation indicators of fruit radish were determined by using cluster analysis and correlation analysis. The results indicated that the range of variation coefficient was from 1. 64%-89. 99%. Water content, crispness, soluble solids, crude fiber, isothiocyanate were the important components of fruit radish quality, playing a major role in the quality. These 8indicators were closely related and relatively independent. Water content was significantly and positively correlated with crispness; soluble solids and crude fiber were significantly and negatively related to vitamin C; crispness was significantly and positively correlated with crude fiber and vitamin C. Based on the above results,4 representative indicators were used for evaluating fruit radish quality, that is water content, crispness, soluble solids, crude fiber, which could reflect the most information of fruit radish quality.
文摘Quality of Maternal, Neonatal and Child (MNCH) care is an important aspect in ensuring healthy outcomes and survival of mothers and children. Twenty-nine indicators were identified as important features and among those, eight indicators namely maternal mortality ratio, still-birth rate, delivery at a health facility, deliveries assisted by skilled attendants, proportional breach delivery, normal delivery rate, born before arrival rate and antenatal care visit coverage were identified to be the most important indicators for quality measurement.
