Traditional transgenic detection methods require high test conditions and struggle to be both sensitive and efficient.In this study,a one-tube dual recombinase polymerase amplification(RPA)reaction system for CP4-EPSPS and Cry1Ab/Ac was proposed and combined with a lateral flow immunochromatographic assay,named“Dual-RPA-LFD”,to visualize the dual detection of genetically modified(GM)crops.In which,the herbicide tolerance gene CP4-EPSPS and the insect resistance gene Cry1Ab/Ac were selected as targets taking into account the current status of the most widespread application of insect resistance and herbicide tolerance traits and their stacked traits.Gradient diluted plasmids,transgenic standards,and actual samples were used as templates to conduct sensitivity,specificity,and practicality assays,respectively.The constructed method achieved the visual detection of plasmid at levels as low as 100 copies,demonstrating its high sensitivity.In addition,good applicability to transgenic samples was observed,with no cross-interference between two test lines and no influence from other genes.In conclusion,this strategy achieved the expected purpose of simultaneous detection of the two popular targets in GM crops within 20 min at 37°C in a rapid,equipmentfree field manner,providing a new alternative for rapid screening for transgenic assays in the field. 展开更多

The preparation of large crystals is highly important for the characterization and application of a newly found structure but remains a challenge for one-dimensional(1D)C_(60)polymers.In this work,we successfully fabricated a 1D C_(60)polymer crystal via on-site annealing of a millimeter-sized C_(60)molecular crystal withα-Li3N at 500°C and ambient pres�sure.Characterizations show that the C_(60)cages in the crystal have been efficiently connected,forming 1D chains along the<110>direction in an orthorhombic 3D structure.At the same time,the crystal maintains a morphology similar to that of the pristine C_(60)crystal,providing opportunities for characterization of all the facets of the crystal via Raman spectroscopy and thus suggesting the formation mechanism of such crystals. 展开更多

BACKGROUND The concept of macroscopic on-site evaluation(MOSE)was introduced in 2015 when the endoscopist observed better diagnostic yield when the macroscopically visible core on MOSE was superior to 4 mm.Recent studies suggest that MOSE by the endoscopist may be an excellent alternative to rapid on-site evaluation,and some classi-fications have been published.Few studies have assessed the adequacy of histologic cores in MOSE during endoscopic ultrasound-guided fine-needle aspiration/biopsy(EUS-FNA/FNB).AIM To evaluate the performance of MOSE during EUS-FNA/FNB.METHODS This multicentric prospective study was conducted in 16 centers in 3 countries(Egypt,Iraq,and Morocco)and included 1108 patients with pancreatic,biliary,or gastrointestinal pathology who were referred for EUS examination.We prospectively analyzed the MOSE in 1008 patients with available histopathological reports according to 2 classifications to determine the adequacy of the histological core samples.Data management and analysis were performed using a Statistical Package for Social Sciences(SPSS)version 27.RESULTS A total of 1074 solid lesions were biopsied in 1008 patients with available cytopathological reports.Mean age was 59 years,and 509 patients(50.5%)were male.The mean lesion size was 38 mm.The most frequently utilized needles were FNB-Franseen(74.5%)and 22 G(93.4%),with a median of 2 passes.According to 2 classifications,618 non-bloody cores(61.3%)and 964 good samples(95.6%)were adequate for histological evaluation.The overall diagnostic yield of cytopathology was 95.5%.The cytological examination confirmed the diagnosis of malignancy in 861 patients(85.4%),while 45 samples(4.5%)were inconclusive.Post-procedural adverse events occurred in 33 patients(3.3%).Statistical analysis showed a difference between needle types(P=0.035)with a high sensitivity of FNB(97%).The analysis of the relationship between the MOSE-score and the final diagnosis showed a significant difference between the different scores of the MOSE(P<0.001).CONCLUSION MOSE is a simple method that allows endoscopists to increase needle passes to improve sample quality.There is significantly higher FNB sensitivity and cytopathology diagnostic yield with good MOSE cores. 展开更多

In this setting, dif... Endoscopic ultrasound (EUS) has become an essential tool for the study of pancreatic diseases. Specifically, EUS plays a pivotal role evaluating patients with a known or suspected pancreatic mass. In this setting, differential diagnosis remains a clinical challenge. EUS-guided fine-needle aspiration (FNA) and fine-needle biopsy (FNB) have been proven to be safe and useful tools in this setting. EUS-guided FNA and FNB, by obtaining cytological and/or histological samples, are able to diagnose pancreatic lesions with high sensitivity and specificity. In this context, several methodological features, trying to increase the diagnostic yield of EUS-guided FNA and FNB, have been evaluated. In this review, we focus on the role of rapid on-site evaluation (ROSE). From data reported in the literature, ROSE may increase diagnostic yield of EUS-FNA specimens by 10%-30%, and thus, diagnostic accuracy. However, we should point out that many recent studies have reported adequacy rates of &#x0003e; 90% without ROSE, indicating that, perhaps, at high-volume centers, ROSE may not be indispensable to achieve excellent results. Daily observations for ambulance transports (from July 1 to September 30, 2009) were obtained from Fire Department Service in Osaka. Data of meteorological parameters in Osaka area, Japan (from July 1 to September 30, 2009) were also obtained from Japan Meteorological Agency. Effect of meteorological parameters on ambulance transports was analyzed. A total of 49,526 ambulance transports were observed from July 1 to September 30, 2009 in Osaka, Japan. Ambulance transports were significantly correlated with mean air temperature (r = 0.298), the highest air temperature (r = 0.347), the lowest air temperature (r = 0.209) and daylight hours (r = 0.301). The higher air temperature was associated with higher ambulance transports of summer in Osaka area, Japan. 展开更多

Daily observations for ambulance transports due to heat stroke from July to September in 2010 in Okayama prefecture, Japan were obtained from Fire and Disaster Management Agency in Japan. Data of meteorological parameters in Okayama prefecture, Japan were also obtained from Japan Meteorological Agency. Effect of meteorological parameters on ambulance transports due to heat stroke was analyzed. A total of 1133 ambulance transports due to heat stroke were observed in from July to September of 2010 in Okayama prefecture, Japan. Ambulance transports due to heat stroke was significantly correlated with air temperature. In addition, number of subjects with ambulance transports due to heat stroke over 34°C in the highest air temperature was 21.2 ± 9.8 per day. Higher air temperature was closely associated with higher ambulance transports due to heat stroke by using daily data in Okayama, prefecture, Japan. 展开更多

Helicopter emergency medical services can reduce out-of-hospital transport times because of their high speed and their ability to travel in straight paths?unlike ground ambulance which are restricted to road network paths, as well as the ability toaccess rural or remote area injuries that are difficult to reach by ground ambulance. GIS technology aids air ambulance movement planning to reduce out-of-hospital response time based on mathematical and geographic models to support decision making which is necessary from out-of-hospital care providers. The goal of this study is to use GIS to develop an efficient DSS to outline where air ambulance can reduce response times, by using spatial analysis tools to create Euclidean distance and direction zones around receiving hospitals. The study concludes that GIS technology can be used to develop an efficient DSS to outline where air ambulance can reduce response times, by creating surfaces of Euclidean allocation, direction, and distance that can be used to improve initial response times for the civil defense air rescue and air ambulance services. 展开更多

To investigate the link between higher air temperature and ambulance transports due to heat stroke, especially in elderly subjects, using daily data in Sai-tama prefecture in August, 2010, Japan. We obtained daily data of air temperature in Saitama prefecture in August, 2010, Japan from Japan Meteorological Agency. Number of ambulance transports due to heat stroke was also used by Saitama prefecture official website. The effect of high air temperature on am-bulance transports due to heat stroke was evaluated. A total of 1688 ambulance transports due to heat stroke was observed in Saitama prefecture in August, 2010, Japan. Seven hundred and twenty one subjects (42.7%) were over the age of 65. Mean air temperature, the highest air temperature and day light hours were significantly associated with ambulance transports due to heat stroke in total subjects (mean air temperature: r = 0.749, p < 0.0001) and the over the age of 7. In addition, by separate analysis as classified by age groups, the correlation coefficient between ambulance transports due to heat stroke and mean air temperature was the highest in subjects with over 65. However, the lowest air temperature was not cor-related with ambulance transports due to heat stroke. Higher air temperature was closely associated with higher ambulance transports due to heat stroke using daily data in Saitama prefecture in August, 2010, Ja-pan, especially in elderly subjects. 展开更多

A“cloud-edge-end”collaborative system architecture is adopted for real-time security management of power system on-site work,and mobile edge computing equipment utilizes lightweight intelligent recognition algorithms for on-site risk assessment and alert.Owing to its lightweight and fast speed,YOLOv4-Tiny is often deployed on edge computing equipment for real-time video stream detection;however,its accuracy is relatively low.This study proposes an improved YOLOv4-Tiny algorithm based on attention mechanism and optimized training methods,achieving higher accuracy without compromising the speed.Specifically,a convolution block attention module branch is added to the backbone network to enhance the feature extraction capability and an efficient channel attention mechanism is added in the neck network to improve feature utilization.Moreover,three optimized training methods:transfer learning,mosaic data augmentation,and label smoothing are used to improve the training effect of this improved algorithm.Finally,an edge computing equipment experimental platform equipped with an NVIDIA Jetson Xavier NX chip is established and the newly developed algorithm is tested on it.According to the results,the speed of the improved YOLOv4-Tiny algorithm in detecting on-site dress code compliance datasets is 17.25 FPS,and the mean average precision(mAP)is increased from 70.89%to 85.03%. 展开更多

Endoscopic ultrasound-guided fine-needle biopsy(EUS-FNB)is an excellent investigation to diagnose pancreatic lesions and has shown high accuracy for its use in pathologic diagnosis.Recently,macroscopic on-site evaluation(MOSE)performed by an endoscopist was introduced as an alternative to rapid on-site cytologic evaluation to increase the diagnostic yield of EUS-FNB.The MOSE of the biopsy can estimate the adequacy of the sample directly by the macroscopic evaluation of the core tissue obtained from EUS-FNB.Isolated pancreatic tuberculosis is extremely rare and difficult to diagnose because of its non-specific signs and symptoms.Therefore,this challenging diagnosis is based on endoscopy,imaging,and the bacteriological and histological examination of tissue biopsies.This uncommon presentation of tuberculosis can be revealed as pancreatic mass mimicking cancer.EUS-FNB can be very useful in providing a valuable histopathological diagnosis.A calcified lesion with a cheesy core in MOSE must be suggestive of tuberculosis,leading to the request of the GeneXpert,which can detect Mycobacterium tuberculosis deoxyribonucleic acid and resistance to rifampicin.A decent diagnostic strategy is crucial to prevent unnecessary surgical resection and to supply conservative management with antitubercular therapy. 展开更多

On-site programming big data refers to the massive data generated in the process of software development with the characteristics of real-time,complexity and high-difficulty for processing.Therefore,data cleaning is essential for on-site programming big data.Duplicate data detection is an important step in data cleaning,which can save storage resources and enhance data consistency.Due to the insufficiency in traditional Sorted Neighborhood Method(SNM)and the difficulty of high-dimensional data detection,an optimized algorithm based on random forests with the dynamic and adaptive window size is proposed.The efficiency of the algorithm can be elevated by improving the method of the key-selection,reducing dimension of data set and using an adaptive variable size sliding window.Experimental results show that the improved SNM algorithm exhibits better performance and achieve higher accuracy. 展开更多

To investigate the link between low air temperature and ambulance transports in Japan. Data on climate parameters i.e. air temperature, humidity and daylight hours in Sakata area, Yamagata prefecture, Japan, which is northern part (Tohoku) in Japan, was obtained from Japan Meteorological Agency. Monthly data on ambulance transports due to acute disease from 2005 to 2011 was also directly obtained from Sakata area official website, Japan. The relations between air temperature and ambulance transports were evaluated by ecological study. Ambulance transports per day were 10.1 ± 1.0 and mean air temperature was 13.0 ± 8.4 (°C). Ambulance transports per day were weakly and negatively correlated with mean air temperature, mean of the highest air temperature, mean of the lowest air temperature, the highest air temperature and daylight hours. It was also weakly and positively correlated with mean humidity. However, the relation between ambulance transports and mean air temperature was well estimated by quadratic curve (r = 0.633, 展开更多

This is aggravated and difficult when the care is carried out on public roads or the patient’s home, that is, it is a pre-hospital context. Our Psychiatric Emergency Hospital, located in the City of Buenos Aires, provides pre-hospital care with a psychiatric ambulance, which is then referred to general hospitals or other health system providers. Not all effectors in the network have the same resources and, at the time of the emergency, the reasons for consultation such as the patient’s clinic are heterogeneous and require the greatest effectiveness in the intervention that is taken. The triage system is a resource that allows the systematization of approaches against the multiplicity of reasons for consultation and presentation of mental pathologies in the emergency room, in addition to providing greater effectiveness in interventions, homogeneity for the health system and consensus among the health effectors. Framed in the Mental Health Law, and especially in the concept of risk, a triage proposal was designed for pre-hospital interventions by the psychiatric ambulance dependent on Hospital Alvear and under current legal regulations with the aim of homogenizing care and referrals with the highest rate of effectiveness to provide quality interdisciplinary medicine. 展开更多

Monthly observations for ambulance transports (2004-2008) were obtained from Fire Department Service in Takamatsu. Data of temperatures in Takamatsu area, Japan (2004-2008) were used by Japan Meteorological Agency. Effect of high temperatures on ambulance transports was analyzed. By using data from July to September, there were not clear differences of mean temperatures and ambulance transports among years. Ambulance transports were significantly correlated with parameters of temperatures. Correlation coefficient rate between ambulance transports and the mean temperature of maximum temperatures in a month was highest among parameters (r = 0.738, p = 0.0017). In addition, ambulance transports were also significantly correlated with the number of days over the level of 32?C in a month (r = 0.782, p = 0.0006). Higher temperatures were closely associated with higher ambulance transports in Takamatsu area, Japan. 展开更多

This includes seismometer sensitivity, linearity and clip levels based on the shake table test, as well as the seismometer natural period, damping constant based on electrical calibration(Wang Guangfu,1986; Ple?inger A.,1993) and instrumental self-noise collocation estimation(Holcomb L.G., 1989; Sleeman R. et al., 2006). We study the dynamical energy equipartition properties in the integrable Toda model with additional uniform or disordered on-site energies by extensive numerical simulations. The total energy is initially equidistributed among some of the lowest frequency linear modes. For the Toda model with uniform on-site potentials, the energy spectrum keeps its profile nearly unchanged in a relatively short time scale. On a much longer time scale, the energies of tail modes increase slowly with time. Energy equipartition is far away from being attached in our studied time scale. For the Toda model with disordered on-site potentials, the energy transfers continuously to the high frequency modes and eventually towards energy equipartition. We further perform a systematic study of the equipartition time teq depending on the energy density ε and the nonlinear parameter α in the thermodynamic limit for the Toda model with disordered on-site potentials. We find teq∝ (1/ε)^a(1/α)^b, where b≈ 2a. The values of a and b are increased when increasing the strengths of disordered on-site potentials or decreasing the number of initially excited modes. 展开更多

BACKGROUND In developed countries,the economic feasibility of using ultrasound in ambulances before arriving at the hospital has been achieved through comprehensive studies,and this of course does not apply to Arab countries,as there are no current studies to support this.Therefore,this study is a preliminary measure of the economic feasibility of using ultrasound in ambulances in Saudi Arabia.AIM To measure the demand for ultrasound equipment in ambulances in Saudi Arabia.METHODS A cross-sectional study of five different groups of participants including radiation technologists,emergency physicians,paramedics,Red Crescent managers and the public.Email and social media were used to deliver a questionnaire to these groups.The questionnaire included specific questions to measure the purpose of ultrasound use in each group of participants.RESULTS Each group had some knowledge on ultrasound and its benefits.More than 50%in each study group supported the availability of ultrasound in ambulances.Additionally,60%of emergency physicians reported that they had difficulties in venous access,checking the presence of internal bleeding,recognizing pregnancy in trauma cases,and inserting endotracheal tubes,and the majority of them confirmed the effective role of ultrasound in achieving such tasks.Almost all paramedics(93.33%),physicians(98.89%),and Saudi Red Crescent managers(96.3%)emphasized the importance of communication between ambulance staff and emergency departments.Moreover,most physicians(77.78%),and technologists(82.73%)supported the presence of paramedics in ambulances to operate ultrasound in order to improve patient outcomes.CONCLUSION Most of the study groups evaluated had knowledge on ultrasound and supported the presence of ultrasound devices in ambulances. 展开更多
