METHODS: A cohort of 489 women, randomly recrui-ted at age 40-79 years, from a longitu...AIM: To determine the relative risk of selected serious outcomes with variations in use of menopausal hormone treatment (MHT). METHODS: A cohort of 489 women, randomly recrui-ted at age 40-79 years, from a longitudinal study of urbanised population was a study group and was followed for 14 years. Four selected outcomes (coronary artery disease, stroke, peripheral artery disease, breast cancer) were tested. Each woman on entry to the study was interviewed by a dedicated medical practitioner, and data on menstrual and menopausal history and health status were obtained. Outcome information was ascertained by questionnaire and medical reports from attending medical practitioners. In case of death, cause of death was checked with the Registry of Births, Deaths, Marriages and Divorce. This information was available for all women. An ever-user of MHT was defined as use for 6 mo or more at any time during the study. A late start of MHT was defned as 3 years or more from onset of menopause. The generalised linear statistical package was used to examine the data; univariate logistic regression models were used to describe the relationship between patient characteristics and a disease outcome, followed by stepwise multi variate analysis, controlling for age, lifestyle factors and co-morbidities.RESULTS: The risk of ever-use of MHT was signifcantly increased only for peripheral artery disease (RR = 2.16; 0.99, 4.71; P = 0.05), and not for coronary artery disease, stroke and breast cancer. A late start of MHT (three years or more from onset of menopause) was associated with signifcantly increased risks for coronary artery disease (RR = 2.56; 1.15, 5.72; P = 0.02) and peripheral artery disease (RR = 4.42; 1.55, 12.64; P = 0.005), and use after age 60 years with signifcantly increased risks for coronary artery disease (RR = 4.98; 2.19, 11.55; P 〈 0.001), stroke (RR = 2.99; 1.11, 8.08; P = 0.03) and peripheral artery disease (RR = 4.18; 1.24, 14.14; P = 0.02). Use up to 10 years was not associated with signifcant risk for all outcomes. These risks were confrmed by stepwise multi variate analysis, adjusting for age at recruitment, body mass index, smoking, physical activity and alcohol use, and existing diabetes, mellitus, hypertension and hypercholesterolaemia. Regardless of variations in use, risk for breast cancer was not found. CONCLUSION: The study confirms ever-use of MHT affected only risk of peripheral artery disease; but some use variations could have adverse effects.展开更多

The inactivation of total and fecal coliforms using ozone and ultraviolet radiation as separate treatments was evaluated. Different ozone concentrations (3 to 40 mg O3/L) were applied and UV fluencies ranging from 8.5 to 12 mJ/cm2 at different pH values (from 5 to 9) were tested. Best results were obtained for ozone doses near 20 mg/min with removals of 72% and 78% of fecal and total coliforms, respectively. The ozone also was capable of oxidizing organic matter in the effluent measured as COD (the highest removal obtained was 36% for 20 mg O3/min). Maximum bacterial resistance was observed at pH 7 in both cases. The UV light offered a high bacterial inactivation (over 80%) and the lowest bacterial inactivation was observed at pH 7. Finally, we obtained the electric energy per order (EEO, kWh/m3/order), defined as the electric energy (kW-h) required to degrade a contaminant by one order of magnitude in a unit volume of contaminated water, being noteworthy that EEO values for the UV process resulted were lower than those determined for the process with ozone in all the water flow tested.展开更多

The purpose of this study was to evaluate a commercial oz...Development of a technology that can reduce the odor of liquid swine manure during agitation and land application could prove beneficial to the swine industry. The purpose of this study was to evaluate a commercial ozone treatment system for swine slurry under production scale conditions. The facility used for this study was a curtain sided finishing building housing 500 grow–finish market hogs located over a manure pit measuring 12.2 m wide × 25.9 m long × 2.4 m deep with a total pit capacity of 770,142 l, containing 577,607 l. The system evaluated exposes air to ultra-violet light creating O3. The O3 is then injected into slurry at a rate of 851.6 l/min. treating 51,097 l/h. In this study the entire pit contents were treated every 11.3 h. At 0, 24, 48, and 96 h two slurry samples were collected with a 3.05 m probe and six air sample bags were collected via a vacuum pump. No significant differences were detected in slurry samples between time periods. Mean slurry values were 13.6 ± 4.6% solids dry wt., 850 ± 70 mg/l settable solids, 54,200 ± 4384 mg/l total suspended solids, 61,050 ± 12,657 mg/l chemical oxygen demand, 0.86 ± 0.14%N, 0.49 ± 0.27%P, 0.45 ± 0.01%K and dissolved oxygen below detection limits. Ammonia concentrations decreased (P = 0.004) from 0 to 96 h. Odor panelists analyzed air samples for intensity at recognition (IR), offensiveness at recognition (OR), intensity at full strength (IFS) and offensiveness at full strength (OFS). Panelists found OR, IFS and OFS were reduced (P < 0.01) at 48 h and 96 h compared to 0 h and IR was reduced (P < 0.04) at 24 h and 48 h and not at 96 h but trended lower (P = 0.12) at 96 h. The system evaluated significantly improved air quality within the building suggesting that odor emanating from swine buildings and odor generated during land application of slurry should be reduced.展开更多

AIM:To demonstrate the outcomes of translacrimal canalicular drainage using a lacrimal probe and intranasal drainage by D-silicone intubation for acute dacryocystitis(AD).METHODS:This retrospective study included 23 p...AIM:To demonstrate the outcomes of translacrimal canalicular drainage using a lacrimal probe and intranasal drainage by D-silicone intubation for acute dacryocystitis(AD).METHODS:This retrospective study included 23 patients with AD and had undergone abscess decompression with the use of lacrimal probe and intranasal drainage by D-silicone intubation between January 2019 and December 2022.Patients received abscess decompression and systemic antibiotic-corticosteroid from the time of diagnosis.D-silicone tube was inserted within 10d after diagnosis and removed 3-6mo after intubation.The procedure and outcomes of this method were evaluated.RESULTS:All patients showed improvement of signs and symptoms of AD within 72h.No intraoperative and postoperative complications were observed.No recurrence of lacrimal sac abscesses occurred after D-silicone tube removed.CONCLUSION:Lacrimal probe and D-silicone intubation appear to be a feasible,minimally invasive,safe,and effective method,which could be a reasonable choice in the treatment of AD.展开更多

BACKGROUND During emergency endoscopic retrograde cholangiopancreatography(ERCP),the safety and feasibility of performing one-stage endoscopic treatment for patients with acute cholangitis(AC)due to choledocholithiasi...BACKGROUND During emergency endoscopic retrograde cholangiopancreatography(ERCP),the safety and feasibility of performing one-stage endoscopic treatment for patients with acute cholangitis(AC)due to choledocholithiasis are unclear.AIM To investigate the safety and feasibility of one-stage endoscopic treatment for moderate to severe AC.METHODS We enrolled all patients diagnosed with moderate to severe cholangitis due to common bile duct stones from January 2019 to July 2023.The outcomes were compared in this study between patients who underwent ERCP within 24 h and those who underwent ERCP 24 h later,employing a propensity score(PS)frame-work.Our primary outcomes were intensive care unit(ICU)admission rates,ICU length of stay,and duration of antibiotic use.RESULTS In total,we included 254 patients and categorized them into two groups based on the time elapsed between admission and intervention:The urgent group(≤24 h,n=102)and the elective group(>24 h,n=152).Ninety-three pairs of patients with similar characteristics were selected by PS matching.The urgent ERCP group had more ICU admissions(34.4%vs 21.5%,P=0.05),shorter ICU stays(3 d vs 9 d,P<0.001),fewer antibiotic use(6 d vs 9 d,P<0.001),and shorter hospital stays(9 d vs 18.5 d,P<0.001).There were no significant differences observed in adverse events,in-hospital mortality,recurrent cholangitis occurrence,30-d readmission rate or 30-d mortality.CONCLUSION Urgent one-stage ERCP provides the advantages of a shorter ICU stay,a shorter duration of antibiotic use,and a shorter hospital stay.展开更多

We retrospectively analysed the clinical details of 51 diabetic foot patients in our hospital from February 2023 to February 2024 and divided them into two groups according to the different treatment modalities, in which the control group received open debridement and the treatment group received transverse tibial bone transfer and compared the differences between the two groups. There were large differences in VAS score, Toronto Clinical Symptom Score, quality of life score, internal lumen diameter, blood flow velocity in the affected limb, perfusion volume, vascular endothelial growth factor, epidermal growth factor and CRP between the two groups after surgery (P < 0.05). Patients with diabetic foot should be treated with transverse tibial bone grafting, which is safer and more effective and can have a significant impact on improving the status of the affected limb, the inflammation and the patient’s quality of life.展开更多

Several treatments have been proposed with controversial results. The aim of our study was to evaluate treatment by surgical drilling coupled with in situ cancellous grafting. Materials and methods: Our study was a case-control study conducted at Brazzaville University Hospital from 1st January 2018 to 31 December 2023. It compared two groups of patients with ONTF: non-operated (13 patients, 20 hips) and operated (22 patients, 35 hips). We used the visual digital scale (VDS) for pain assessment, the Merle D’Aubigne-Postel (MDP) scoring system for clinical and functional assessment, and the evolution of necrosis. Results: The group of non-operated patients had a mean age of 35.69 ± 3.4 years, no improvement in pain with an EVN above seven at the last recoil and a mean global MDP score falling from 12.7 before offloading to 10.13 at one year. The group of patients operated on had a mean age of 37.86 ± 7.02 years, a significant reduction in pain (p = 0.00004) and a significantly increased MDP score (p = 0.0034). A comparison of the two groups of patients showed significant stabilization of the necrotic lesions in the operated patients (p = 0.00067), with better satisfaction in the same group. Conclusion: Surgical drilling combined with grafting in the treatment of early-stage ONTF has improved progress in our series. The technique is reproducible and less invasive. It has made it possible to delay unfavorable progression and, consequently, hip replacement surgery.展开更多

A treat-to-target strategy in inflammatory bowel disease(IBD)involves treatment intensification in order to achieve a pre-determined endpoint.Such uniform and tight disease control has been demonstrated to improve cli...A treat-to-target strategy in inflammatory bowel disease(IBD)involves treatment intensification in order to achieve a pre-determined endpoint.Such uniform and tight disease control has been demonstrated to improve clinical outcomes compared to treatment driven by a clinician’s subjective assessment of symptoms.However,choice of treatment endpoints remains a challenge in management of IBD via a treat-to-target strategy.The treatment endpoints for ulcerative colitis(UC),recommended by the Selecting Therapeutic Targets in Inflammatory Bowel Disease(STRIDE)consensus have changed somewhat over time.The latest STRIDE-II consensus advises immediate(clinical response),intermediate(clinical remission and biochemical normalisation)and long-term treatment(endoscopic healing,absence of disability and normalisation of health-related quality of life,as well as normal growth in children)endpoints in UC.However,achieving deeper levels of remission,such as histologic normalisation or healing of the gut barrier function,may further improve outcomes among UC patients.Generally,all medical therapy should seek to improve short-and long-term mortality and morbidity.Hence treatment endpoints should be chosen based on their ability to predict for improvement in short-and long-term mortality and morbidity.Potential benefits of treatment intensification need to be weighed against the potential harms within an individual patient.In addition,changing therapy that has achieved partial response may lead to worse outcomes,with failure to recapture response on treatment reversion.Patients may also place different emphasis on certain potential benefits and harms of various treatments than clinicians,or may have strong opinions re certain therapies.Potential benefits and harms of therapies,incremental benefits of achieving deeper levels of remission,as well as uncertainties of the same,need to be discussed with individual patients,and a treatment endpoint agreed upon with the clinician.Future research should focus on quantifying the incremental benefits and risks of achieving deeper levels of remission,such that clinicians and patients can make an informed decision about appropriate treatment end-point on an individual basis.focus on quantifying the incremental benefits and risks of achieving deeper levels of remission,such that clinicians and patients can make an informed decision about appropriate treatment end-point on an individual basis.展开更多

Methods: Forty cases of patients with preeclampsia admitted to our hospital between January 2023 and January 2024 were divided randomly into a control group and an observation group of 20 cases each. The control group applied progesterone to preserve the fetus, and the observation group applied dydrogesterone. The symptom relief time, hormone levels before and after treatment, as well as adverse drug reactions, and the effect of fetal preservation between the two groups were compared. Results: The time to relieve vaginal bleeding, abdominal pain. and lumbago in the observation group was shorter than that in the control group (P < 0.05). After treatment, the progesterone levels and incidence of adverse drug reactions in the observation group were lower than those in the control group (P < 0.05). The success rate of fertility preservation in the observation group was higher than that in the control group (P < 0.05). Conclusion: In the treatment of fetal preservation of preeclampsia, the application of dydrogesterone positively alleviated vaginal bleeding, abdominal pain, and lumbago, with mild adverse reactions and a good effect on fetal preservation.展开更多

The results show that welding heat input has a significant effect on the hardness of the SZ. Under high welding heat input condition, a higher welding speed is beneficial for improving the hardness of the SZ. However, when the welding heat input is low, the hardness of the SZ elevates with increasing the rotation speed. The hardness of the SZ decreases after post-welded heat treatment due to overaging. The joints welded at 500 r/min and 100 mm/min show a high resistance to overaging. The reduction of hardness in the SZ is only 3.8%, while in other joints, the reduction is more than 10%. The morphology of strengthening precipitates plays important roles for the improvement of hardness.展开更多

In order to obtain the optimal combination of strength and ductility, the borided Inconel 718 was subjected to standard heat treatment. This consists of solution treatment and then a two-step aging treatment. The borided layer is composed of the compound layer and the boron diffusion zone. Because of the superior hardness of borides, the borided Inconel 718 exhibits a significant reduction in its wear rate and relatively low coefficient of friction (COF) compared with the unborided Inconel 718. The standard heat treatment efficiently promotes the diffusion of boron into the interior of the material and the generation of new borides (Fe2B, CrB). The borided layer with standard heat treatment shows much better wear resistance due to the thicker borided layer (313.76 μm).展开更多

Back-gated black phosphorus field-effect transistors (FETs) were fabricated using thick black phosphorus flakes obtained by thinning of black phosphorus, as oxygen radicals generated by UV irradiation formed phosphorus oxides on the surface. In order to monitor the thickness effect on the electrical properties, the fabricated FETs were loaded in the UV/ozone chamber, where both the optical (micro-Raman spectroscopy and optical microscopy) and electrical properties (current-voltage characteristics) were monitored in situ. We observed an intensity decrease of the Raman modes of black phosphorus while the field-effect mobility and on/off ratio increased by 48% and 6,800%, respectively. The instability in ambient air limits the investigation and implementation of ultra-thin black phosphorus. However, the method reported in this study allowed us to start with thick black phosphorous flakes, providing a reliable approach for optimizing the electrical performance of black phosphorus-based electronic devices. We believe that these results can motivate further studies using mono- and few-layer black phosphorus.展开更多

The effect of heat trea...Glass-like carbons (GCs) were prepared by carbonization of acetone-furfural resin in nitrogen atmosphere at 850℃, followed by heat treatment over a range of 1 200-2 500℃in inert atmosphere. The effect of heat treatment temperature (HTT) on the oxidation behavior was investigated by dynamic and isothermal thermogravimetric analyses. The structure of GC was examined by X-ray diffractometry (XRD) and the morphologies of GC before and after oxidation were examined by scanning electron microscopy (SEM). It is shown that the GC samples present peculiar oxidation behavior. The anti-oxidation behavior increases with increasing the HTT to 1 600 ℃, whereas decreases gradually thereafter. GC sample heat treated at 1 600℃ obtains relatively optimal anti-oxidation properties under this condition. During the oxidation, this material produces grid network matrix surface and numerous nodular residues on the surface, resulting in excellent resistance to the attack of oxygen atoms.展开更多

Less than 1% of cases of intestinal obstruction are derived from this etiology. The symptoms and signs of gallstone ileus are mostly nonspecific. This entity has been observed with a higher frequency among the elderly, the majority of which have concomitant medical illness. Cardiovascular, pulmonary, and metabolic diseases should be considered as they may affect the prognosis. Surgical relief of gastrointestinal obstruction remains the mainstay of operative treatment. The current surgical procedures are:(1) simple enterolithotomy;(2) enterolithotomy, cholecystectomy and fistula closure(one-stage procedure); and(3) enterolithotomy with cholecystectomy performed later(two-stage procedure). Bowel resection is necessary in certain cases after enterolithotomy is performed. Large prospective laparoscopic and endoscopic trials are expected.展开更多

Outcome information was ascertained by questionnaire and medical reports from attending medical practitioners. In case of death, cause of death was checked with the Registry of Births, Deaths, Marriages and Divorce. This information was available for all women. An ever-user of MHT was defined as use for 6 mo or more at any time during the study. A late start of MHT was defned as 3 years or more from onset of menopause. The generalised linear statistical package was used to examine the data; univariate logistic regression models were used to describe the relationship between patient characteristics and a disease outcome, followed by stepwise multi variate analysis, controlling for age, lifestyle factors and co-morbidities.RESULTS: The risk of ever-use of MHT was signifcantly increased only for peripheral artery disease (RR = 2.16; 0.99, 4.71; P = 0.05), and not for coronary artery disease, stroke and breast cancer. A late start of MHT (three years or more from onset of menopause) was associated with signifcantly increased risks for coronary artery disease (RR = 2.56; 1.15, 5.72; P = 0.02) and peripheral artery disease (RR = 4.42; 1.55, 12.64; P = 0.005), and use after age 60 years with signifcantly increased risks for coronary artery disease (RR = 4.98; 2.19, 11.55; P 〈 0.001), stroke (RR = 2.99; 1.11, 8.08; P = 0.03) and peripheral artery disease (RR = 4.18; 1.24, 14.14; P = 0.02). Use up to 10 years was not associated with signifcant risk for all outcomes. These risks were confrmed by stepwise multi variate analysis, adjusting for age at recruitment, body mass index, smoking, physical activity and alcohol use, and existing diabetes, mellitus, hypertension and hypercholesterolaemia. Regardless of variations in use, risk for breast cancer was not found. CONCLUSION: The study confirms ever-use of MHT affected only risk of peripheral artery disease; but some use variations could have adverse effects.
文摘UV radiation and ozonation were investigated as disinfection alternatives for the wastewater treatment plant. The inactivation of total and fecal coliforms using ozone and ultraviolet radiation as separate treatments was evaluated. Different ozone concentrations (3 to 40 mg O3/L) were applied and UV fluencies ranging from 8.5 to 12 mJ/cm2 at different pH values (from 5 to 9) were tested. Best results were obtained for ozone doses near 20 mg/min with removals of 72% and 78% of fecal and total coliforms, respectively. The ozone also was capable of oxidizing organic matter in the effluent measured as COD (the highest removal obtained was 36% for 20 mg O3/min). Maximum bacterial resistance was observed at pH 7 in both cases. The UV light offered a high bacterial inactivation (over 80%) and the lowest bacterial inactivation was observed at pH 7. Finally, we obtained the electric energy per order (EEO, kWh/m3/order), defined as the electric energy (kW-h) required to degrade a contaminant by one order of magnitude in a unit volume of contaminated water, being noteworthy that EEO values for the UV process resulted were lower than those determined for the process with ozone in all the water flow tested.
文摘Development of a technology that can reduce the odor of liquid swine manure during agitation and land application could prove beneficial to the swine industry. The purpose of this study was to evaluate a commercial ozone treatment system for swine slurry under production scale conditions. The facility used for this study was a curtain sided finishing building housing 500 grow–finish market hogs located over a manure pit measuring 12.2 m wide × 25.9 m long × 2.4 m deep with a total pit capacity of 770,142 l, containing 577,607 l. The system evaluated exposes air to ultra-violet light creating O3. The O3 is then injected into slurry at a rate of 851.6 l/min. treating 51,097 l/h. In this study the entire pit contents were treated every 11.3 h. At 0, 24, 48, and 96 h two slurry samples were collected with a 3.05 m probe and six air sample bags were collected via a vacuum pump. No significant differences were detected in slurry samples between time periods. Mean slurry values were 13.6 ± 4.6% solids dry wt., 850 ± 70 mg/l settable solids, 54,200 ± 4384 mg/l total suspended solids, 61,050 ± 12,657 mg/l chemical oxygen demand, 0.86 ± 0.14%N, 0.49 ± 0.27%P, 0.45 ± 0.01%K and dissolved oxygen below detection limits. Ammonia concentrations decreased (P = 0.004) from 0 to 96 h. Odor panelists analyzed air samples for intensity at recognition (IR), offensiveness at recognition (OR), intensity at full strength (IFS) and offensiveness at full strength (OFS). Panelists found OR, IFS and OFS were reduced (P < 0.01) at 48 h and 96 h compared to 0 h and IR was reduced (P < 0.04) at 24 h and 48 h and not at 96 h but trended lower (P = 0.12) at 96 h. The system evaluated significantly improved air quality within the building suggesting that odor emanating from swine buildings and odor generated during land application of slurry should be reduced.
文摘Osteomyelitis is a devastating disease caused by microbial infection in deep bone tissue.Its high recurrence rate and impaired restoration of bone deficiencies are major challenges in treatment.Microbes have evolved numerous mechanisms to effectively evade host intrinsic and adaptive immune attacks to persistently localize in the host,such as drug-resistant bacteria,biofilms,persister cells,intracellular bacteria,and small colony variants(SCVs).Moreover,microbial-mediated dysregulation of the bone immune microenvironment impedes the bone regeneration process,leading to impaired bone defect repair.Despite advances in surgical strategies and drug applications for the treatment of bone infections within the last decade,challenges remain in clinical management.The development and application of tissue engineering materials have provided new strategies for the treatment of bone infections,but a comprehensive review of their research progress is lacking.This review discusses the critical pathogenic mechanisms of microbes in the skeletal system and their immunomodulatory effects on bone regeneration,and highlights the prospects and challenges for the application of tissue engineering technologies in the treatment of bone infections.It will inform the development and translation of antimicrobial and bone repair tissue engineering materials for the management of bone infections.

文摘AIM:To demonstrate the outcomes of translacrimal canalicular drainage using a lacrimal probe and intranasal drainage by D-silicone intubation for acute dacryocystitis(AD).METHODS:This retrospective study included 23 patients with AD and had undergone abscess decompression with the use of lacrimal probe and intranasal drainage by D-silicone intubation between January 2019 and December 2022.Patients received abscess decompression and systemic antibiotic-corticosteroid from the time of diagnosis.D-silicone tube was inserted within 10d after diagnosis and removed 3-6mo after intubation.The procedure and outcomes of this method were evaluated.RESULTS:All patients showed improvement of signs and symptoms of AD within 72h.No intraoperative and postoperative complications were observed.No recurrence of lacrimal sac abscesses occurred after D-silicone tube removed.CONCLUSION:Lacrimal probe and D-silicone intubation appear to be a feasible,minimally invasive,safe,and effective method,which could be a reasonable choice in the treatment of AD.

文摘The high-temperature oxidation behaviour of the Inconel 625 alloy at 950℃ was investigated after different ageing treatments.The effect of heat treatment on the oxidation behaviour of the alloy was analysed by characterizing the structure and elemental distribution before and after oxidation.The results reveal that the two ageing treatments at 650℃ for 500 h and at 750℃ for 400 h both reduced the oxidation mass gain.After oxidation at 950℃,an outer Cr_(2)O_(3) layer and inner Al_(2)O_(3) are identified as the main oxidation products.Moreover,Nb_(2)O_(5) andδ(Ni_(3)Nb)phases precipitated after oxidation.The ageing treatments cause the rapid generation of a dense Cr_(2)O_(3) layer on the surface,which prevents the diffusion of oxygen into the matrix,reduce the Al_(2)O_(3) inward growth depth,and improve the oxidation resistance of the alloy.

文摘BACKGROUND During emergency endoscopic retrograde cholangiopancreatography(ERCP),the safety and feasibility of performing one-stage endoscopic treatment for patients with acute cholangitis(AC)due to choledocholithiasis are unclear.AIM To investigate the safety and feasibility of one-stage endoscopic treatment for moderate to severe AC.METHODS We enrolled all patients diagnosed with moderate to severe cholangitis due to common bile duct stones from January 2019 to July 2023.The outcomes were compared in this study between patients who underwent ERCP within 24 h and those who underwent ERCP 24 h later,employing a propensity score(PS)frame-work.Our primary outcomes were intensive care unit(ICU)admission rates,ICU length of stay,and duration of antibiotic use.RESULTS In total,we included 254 patients and categorized them into two groups based on the time elapsed between admission and intervention:The urgent group(≤24 h,n=102)and the elective group(>24 h,n=152).Ninety-three pairs of patients with similar characteristics were selected by PS matching.The urgent ERCP group had more ICU admissions(34.4%vs 21.5%,P=0.05),shorter ICU stays(3 d vs 9 d,P<0.001),fewer antibiotic use(6 d vs 9 d,P<0.001),and shorter hospital stays(9 d vs 18.5 d,P<0.001).There were no significant differences observed in adverse events,in-hospital mortality,recurrent cholangitis occurrence,30-d readmission rate or 30-d mortality.CONCLUSION Urgent one-stage ERCP provides the advantages of a shorter ICU stay,a shorter duration of antibiotic use,and a shorter hospital stay.
文摘To analyse the effectiveness of performing a transverse tibial bone graft in the treatment of patients with diabetic foot. We retrospectively analysed the clinical details of 51 diabetic foot patients in our hospital from February 2023 to February 2024 and divided them into two groups according to the different treatment modalities, in which the control group received open debridement and the treatment group received transverse tibial bone transfer and compared the differences between the two groups. There were large differences in VAS score, Toronto Clinical Symptom Score, quality of life score, internal lumen diameter, blood flow velocity in the affected limb, perfusion volume, vascular endothelial growth factor, epidermal growth factor and CRP between the two groups after surgery (P < 0.05). Patients with diabetic foot should be treated with transverse tibial bone grafting, which is safer and more effective and can have a significant impact on improving the status of the affected limb, the inflammation and the patient’s quality of life.
文摘Introduction: Osteonecrosis of the femoral head (ONTF) is a debilitating condition. Several treatments have been proposed with controversial results. The aim of our study was to evaluate treatment by surgical drilling coupled with in situ cancellous grafting. Materials and methods: Our study was a case-control study conducted at Brazzaville University Hospital from 1st January 2018 to 31 December 2023. It compared two groups of patients with ONTF: non-operated (13 patients, 20 hips) and operated (22 patients, 35 hips). We used the visual digital scale (VDS) for pain assessment, the Merle D’Aubigne-Postel (MDP) scoring system for clinical and functional assessment, and the evolution of necrosis. Results: The group of non-operated patients had a mean age of 35.69 ± 3.4 years, no improvement in pain with an EVN above seven at the last recoil and a mean global MDP score falling from 12.7 before offloading to 10.13 at one year. The group of patients operated on had a mean age of 37.86 ± 7.02 years, a significant reduction in pain (p = 0.00004) and a significantly increased MDP score (p = 0.0034). A comparison of the two groups of patients showed significant stabilization of the necrotic lesions in the operated patients (p = 0.00067), with better satisfaction in the same group. Conclusion: Surgical drilling combined with grafting in the treatment of early-stage ONTF has improved progress in our series. The technique is reproducible and less invasive. It has made it possible to delay unfavorable progression and, consequently, hip replacement surgery.

文摘A treat-to-target strategy in inflammatory bowel disease(IBD)involves treatment intensification in order to achieve a pre-determined endpoint.Such uniform and tight disease control has been demonstrated to improve clinical outcomes compared to treatment driven by a clinician’s subjective assessment of symptoms.However,choice of treatment endpoints remains a challenge in management of IBD via a treat-to-target strategy.The treatment endpoints for ulcerative colitis(UC),recommended by the Selecting Therapeutic Targets in Inflammatory Bowel Disease(STRIDE)consensus have changed somewhat over time.The latest STRIDE-II consensus advises immediate(clinical response),intermediate(clinical remission and biochemical normalisation)and long-term treatment(endoscopic healing,absence of disability and normalisation of health-related quality of life,as well as normal growth in children)endpoints in UC.However,achieving deeper levels of remission,such as histologic normalisation or healing of the gut barrier function,may further improve outcomes among UC patients.Generally,all medical therapy should seek to improve short-and long-term mortality and morbidity.Hence treatment endpoints should be chosen based on their ability to predict for improvement in short-and long-term mortality and morbidity.Potential benefits of treatment intensification need to be weighed against the potential harms within an individual patient.In addition,changing therapy that has achieved partial response may lead to worse outcomes,with failure to recapture response on treatment reversion.Patients may also place different emphasis on certain potential benefits and harms of various treatments than clinicians,or may have strong opinions re certain therapies.Potential benefits and harms of therapies,incremental benefits of achieving deeper levels of remission,as well as uncertainties of the same,need to be discussed with individual patients,and a treatment endpoint agreed upon with the clinician.Future research should focus on quantifying the incremental benefits and risks of achieving deeper levels of remission,such that clinicians and patients can make an informed decision about appropriate treatment end-point on an individual basis.focus on quantifying the incremental benefits and risks of achieving deeper levels of remission,such that clinicians and patients can make an informed decision about appropriate treatment end-point on an individual basis.

文摘Objective: To analyze the application effect and value of dydrogesterone in the fertility preservation treatment of preeclampsia. Methods: Forty cases of patients with preeclampsia admitted to our hospital between January 2023 and January 2024 were divided randomly into a control group and an observation group of 20 cases each. The control group applied progesterone to preserve the fetus, and the observation group applied dydrogesterone. The symptom relief time, hormone levels before and after treatment, as well as adverse drug reactions, and the effect of fetal preservation between the two groups were compared. Results: The time to relieve vaginal bleeding, abdominal pain. and lumbago in the observation group was shorter than that in the control group (P < 0.05). After treatment, the progesterone levels and incidence of adverse drug reactions in the observation group were lower than those in the control group (P < 0.05). The success rate of fertility preservation in the observation group was higher than that in the control group (P < 0.05). Conclusion: In the treatment of fetal preservation of preeclampsia, the application of dydrogesterone positively alleviated vaginal bleeding, abdominal pain, and lumbago, with mild adverse reactions and a good effect on fetal preservation.

文摘The microstructure and hardness of the stir zone (SZ) with different welding heat inputs were investigated for friction stir-welded 2024-T3 aluminum by transmission electron microscopy, differential scanning calorimeter and Vickers micro-hardness test. The results show that welding heat input has a significant effect on the hardness of the SZ. Under high welding heat input condition, a higher welding speed is beneficial for improving the hardness of the SZ. However, when the welding heat input is low, the hardness of the SZ elevates with increasing the rotation speed. The hardness of the SZ decreases after post-welded heat treatment due to overaging. The joints welded at 500 r/min and 100 mm/min show a high resistance to overaging. The reduction of hardness in the SZ is only 3.8%, while in other joints, the reduction is more than 10%. The morphology of strengthening precipitates plays important roles for the improvement of hardness.

文摘Boronizing was applied to Inconel 718. In order to obtain the optimal combination of strength and ductility, the borided Inconel 718 was subjected to standard heat treatment. This consists of solution treatment and then a two-step aging treatment. The borided layer is composed of the compound layer and the boron diffusion zone. Because of the superior hardness of borides, the borided Inconel 718 exhibits a significant reduction in its wear rate and relatively low coefficient of friction (COF) compared with the unborided Inconel 718. The standard heat treatment efficiently promotes the diffusion of boron into the interior of the material and the generation of new borides (Fe2B, CrB). The borided layer with standard heat treatment shows much better wear resistance due to the thicker borided layer (313.76 μm).
文摘A simple and reproducible method to control the thickness of black phosphorus flakes in real time using a UV/ozone treatment is demonstrated. Back-gated black phosphorus field-effect transistors (FETs) were fabricated using thick black phosphorus flakes obtained by thinning of black phosphorus, as oxygen radicals generated by UV irradiation formed phosphorus oxides on the surface. In order to monitor the thickness effect on the electrical properties, the fabricated FETs were loaded in the UV/ozone chamber, where both the optical (micro-Raman spectroscopy and optical microscopy) and electrical properties (current-voltage characteristics) were monitored in situ. We observed an intensity decrease of the Raman modes of black phosphorus while the field-effect mobility and on/off ratio increased by 48% and 6,800%, respectively. The instability in ambient air limits the investigation and implementation of ultra-thin black phosphorus. However, the method reported in this study allowed us to start with thick black phosphorous flakes, providing a reliable approach for optimizing the electrical performance of black phosphorus-based electronic devices. We believe that these results can motivate further studies using mono- and few-layer black phosphorus.

文摘Glass-like carbons (GCs) were prepared by carbonization of acetone-furfural resin in nitrogen atmosphere at 850℃, followed by heat treatment over a range of 1 200-2 500℃in inert atmosphere. The effect of heat treatment temperature (HTT) on the oxidation behavior was investigated by dynamic and isothermal thermogravimetric analyses. The structure of GC was examined by X-ray diffractometry (XRD) and the morphologies of GC before and after oxidation were examined by scanning electron microscopy (SEM). It is shown that the GC samples present peculiar oxidation behavior. The anti-oxidation behavior increases with increasing the HTT to 1 600 ℃, whereas decreases gradually thereafter. GC sample heat treated at 1 600℃ obtains relatively optimal anti-oxidation properties under this condition. During the oxidation, this material produces grid network matrix surface and numerous nodular residues on the surface, resulting in excellent resistance to the attack of oxygen atoms.

文摘Gallstone ileus is a mechanical intestinal obstruction due to gallstone impaction within the gastrointestinal tract. Less than 1% of cases of intestinal obstruction are derived from this etiology. The symptoms and signs of gallstone ileus are mostly nonspecific. This entity has been observed with a higher frequency among the elderly, the majority of which have concomitant medical illness. Cardiovascular, pulmonary, and metabolic diseases should be considered as they may affect the prognosis. Surgical relief of gastrointestinal obstruction remains the mainstay of operative treatment. The current surgical procedures are:(1) simple enterolithotomy;(2) enterolithotomy, cholecystectomy and fistula closure(one-stage procedure); and(3) enterolithotomy with cholecystectomy performed later(two-stage procedure). Bowel resection is necessary in certain cases after enterolithotomy is performed. Large prospective laparoscopic and endoscopic trials are expected.
