AIM:To compare short term outcomes of elective laparoscopic and open right hemicolectomy(RH) in an elderly population.METHODS:All patients over the age of 70 undergoing elective RH at Ninewells Hospital and Perth Roya...AIM:To compare short term outcomes of elective laparoscopic and open right hemicolectomy(RH) in an elderly population.METHODS:All patients over the age of 70 undergoing elective RH at Ninewells Hospital and Perth Royal Infirmary between January 2006 and May 2011 were included in our analysis.Operative details,hospital length of stay,morbidity and mortality was collected by way of proforma from a dedicated prospective database.An extracorporeal anastomosis was performed routinely in the laparoscopic group.The primary endpoints for analysis were morbidity and mortality.Our secondary endpoints were operative duration,length of hospital stay and discharge destination.RESULTS:Two hundred and six patients were included in our analysis.One hundred and twenty-five patients underwent an open resection and 81 patients had a laparoscopic resection.The mean operating time was significantly longer in the laparoscopic group(139 ± 36 min vs 197 ± 53 min,P = 0.001).The mean length of hospital stay was similar in both groups(11.2 ± 7.8 d vs 9.6 ± 10.7 d,P = 0.28).The incidence of postoperative morbidities was 27% in the open group and 38% in the laparoscopic group(P = 0.12).Overall inhospital mortality was 0.8% in open procedures vs 1% in laparoscopic.CONCLUSION:Laparoscopic RH was associated with a significantly longer operative time compared to open RH.In our study,laparoscopic RH was not associated with reduced post-operative morbidity or significantly shorter length of hospital stay.展开更多

Due to the concept of peace and development and the principle of dialogue through consultation, non- confrontation and pragmatism with equality, which have been upheld by organizers, such symposiums have made great achievements. The dialogue mechanism for human rights topics between China and Germany has helped construct a common, accessible and interactive platform for pushing forward human rights legal advancement in both countries, which has brought about a positive influence on the develop- ment of the human rights cause in the two countries. Strengthening legal construction and ensuring hu- man rights are not only the shared aspiration of all the people in the world, but also the irreversible his- torical trend of human civilization. The symposium, with the theme of international law and human rights protection, encourages participants to discuss and share theories and practices in human rights studies, which makes a significant contribu- tion to enriching the modern human rights concept and building a new type of international cooperation. The international community always pays great attention to the relation- ship between human rights and legal governance. This "change" is clearly visible and, to a certain extent, reveals the track of the development of human rights theory. However, there is "constancy" in "change". BACKGROUND Typically,right coronary artery(RCA)occlusion causes ST-segment elevation in inferior leads.However,it is rarely observed that RCA occlusion causes STsegment elevation only in precordial leads.In general,an...BACKGROUND Typically,right coronary artery(RCA)occlusion causes ST-segment elevation in inferior leads.However,it is rarely observed that RCA occlusion causes STsegment elevation only in precordial leads.In general,an electrocardiogram is considered to be the most important method for determining the infarct-related artery,and recognizing this is helpful for timely discrimination of the culprit artery for reperfusion therapy.In this case,an elderly woman presented with chest pain showing dynamic changes in precordial ST-segment elevation with RCA occlusion.CASE SUMMARY A 96-year-old woman presented with acute chest pain showing precordial STsegment elevation with dynamic changes.Myocardial injury markers became positive.Coronary angiography indicated acute total occlusion of the proximal nondominant RCA,mild atherosclerosis of left anterior descending artery and 75%stenosis in the left circumflex coronary artery.Percutaneous coronary intervention was conducted for the RCA.Repeated manual thrombus aspiration was performed,and fresh thrombus was aspirated.A 2 mm×15 mm balloon was used to dilate the RCA with an acceptable angiographic result.The patient’s chest pain was relieved immediately.A postprocedural electrocardiogram showed alleviation of precordial ST-segment elevation.The diagnosis of acute isolated right ventricular infarction caused by proximal nondominant RCA occlusion was confirmed.Echocardiography indicated normal motion of the left ventricular anterior wall and interventricular septum(ejection fraction of 54%),and the right ventricle was slightly dilated.The patient was asymptomatic during the 9-mo follow-up period.CONCLUSION Cardiologists should be conscious that precordial ST-segment elevation may be caused by occlusion of the nondominant RCA.展开更多
