Severe acute pancreatitis complicated with intra-abdominal infection secondary to trauma:A case report

BACKGROUND Pancreatic trauma(PT)is rare among traumatic injuries and has a low incidence,but it can still lead to severe infectious complications,resulting in a high mortality rate.Acute pancreatitis(AP)is a common complication after PT,and when combined with organ dysfunction and sepsis,it will result in a poorer prognosis.CASE SUMMARY We report a 25-year-old patient with multiple organ injuries,including the pancreas,due to abdominal trauma,who developed necrotising pancreatitis secondary to emergency caesarean section,combined with intra-abdominal infection(IAI).The patient underwent performed percutaneous drainage,pancreatic necrotic tissue debridement,and abdominal infection foci debridement on the patient.CONCLUSION We report a case of severe AP and IAI secondary to trauma.This patient was managed by a combination of conservative treatment such as antibiotic therapy and fluid support with surgery,and a better outcome was obtained. 展开更多

BACKGROUND Traumatic pancreatic injury is relatively rare in children,accounting for approximately 3%-12%of blunt abdominal trauma cases.Most traumatic pancreatic injuries in boys are related to bicycle handlebars.Traumatic pancreatic injuries often result in delayed presentation and treatment,leading to high morbidity and mortality.The management of children with traumatic main pancreatic duct injuries is still under debate.CASE SUMMARY We report the case of a 9-year-old boy who was presented at our institution with epigastric pain after being stuck with his bicycle handlebar at the upper abdomen and then treated with endoscopic stenting because of a pancreatic ductal injury.CONCLUSION We believe that endoscopic stenting of pancreatic ductal injuries may be a feasible technique in certain cases of children with traumatic pancreatic duct injuries to avoid unnecessary operations. 展开更多

BACKGROUND Few studies have addressed the question of which drain types are more beneficial for patients with pancreatic trauma(PT).AIM To investigate whether sustained low negative pressure irrigation(NPI)suction drainage is superior to closed passive gravity(PG)drainage in PT patients.METHODS PT patients who underwent pancreatic surgery were enrolled consecutively at a referral trauma center from January 2009 to October 2021.The primary outcome was defined as the occurrence of severe complications(Clavien-Dindo grade≥Ⅲb).Multivariable logistic regression was used to model the primary outcome,and propensity score matching(PSM)was included in the regression-based sensitivity analysis.RESULTS In this study,146 patients underwent initial PG drainage,and 50 underwent initial NPI suction drainage.In the entire cohort,a multivariable logistic regression model showed that the adjusted risk for severe complications was decreased with NPI suction drainage[14/50(28.0%)vs 66/146(45.2%);odds ratio(OR),0.437;95%confidence interval(CI):0.203-0.940].After 1:1 PSM,44 matched pairs were identified.The proportion of each operative procedure performed for pancreatic injury-related and other intra-abdominal organ injury-related cases was comparable in the matched cohort.NPI suction drainage still showed a lower risk for severe complications[11/44(25.0%)vs 21/44(47.7%);OR,0.365;95%CI:0.148-0.901].A forest plot revealed that NPI suction drainage was associated with a lower risk of Clavien-Dindo severity in most subgroups.CONCLUSION This study,based on one of the largest PT populations in a single high-volume center,revealed that initial NPI suction drainage could be recommended as a safe and effective alternative for managing complex PT patients. 展开更多

In modern era of trauma care, pancreatic trauma remains a persistent challenge to radiologists and surgeons alike. Early detection of pancreatic trauma is essential to prevent subsequent complications. However early pancreatic injury is often subtle on computed tomography(CT) and can be missed unless specifically looked for. Signs of pancreatic injury on CT include laceration, transection, bulky pancreas, heterogeneous enhancement, peripancreatic fluid and signs of pancreatitis. Pan-creatic ductal injury is a vital decision-making parameter as ductal injury is an indication for laparotomy. While lacerations involving more than half of pancreatic parenchyma are suggestive of ductal injury on CT, ductal injuries can be directly assessed on magnetic resonance imaging(MRI) or encoscopic retrograde cholangio-pancreatography. Pancreatic trauma also shows temporal evolution with increase in extent of injury with time. Hence early CT scans may underestimate the extent of injures and sequential imaging with CT or MRI is important in pancreatic trauma. Sequential imaging is also needed for successful nonoperative management of pancreatic injury. Accurate early detection on initial CT and adopting a multimodality and sequential imaging strategy can improve outcome in pancreatic trauma. 展开更多

This case report illustrates the difficulty in diagnosing paediatric patients with life threatening pancreatic injuries. A high index of suspicion is essential as late diagnosis significantly affects outcomes. A 9-year-old child presented with epigastric pain following an accident on his pushbike. The patient was examined in paediatric accident and emergency (A/E) and was discharged. He returned twice more to A/E and on the third visit, 5 days after the initial incident, a CT scan was performed. This showed a classical injury to the body of the pancreas with a collection in the lesser sac. The patient was transferred to the regional hepato-pancreato-biliary unit (HPB unit) and underwent surgery. Pancreatic injuries can be difficult to detect clinically and patients may be well on initial presentation with normal observations and routine bloods. Early CT scanning confirms the diagnosis and results in early specialist referral and better outcomes. 展开更多

Traumatic injury to the pancreas is rare and difficult to diagnose.In contrast,traumatic injuries to the liver,spleen and kidney are common and are usually identified with ease by imaging modalities.Pancreatic injuries are usually subtle to identify by different diagnostic imaging modalities,and these injuries are often overlooked in cases with extensive multiorgan trauma.The most evident findings of pancreatic injury are posttraumatic pancreatitis with blood,edema,and soft tissue infiltration of the anterior pararenal space.The alterations of post-traumatic pancreatitis may not be visualized within several hours following trauma as they are time dependent.Delayed diagnoses of traumatic pancreatic injuries are associated with high morbidity and mortality.Imaging plays an important role in diagnosis of pancreatic injuries because early recognition of the disruption of the main pancreatic duct is important.We reviewed our experience with the use of various imaging modalities for diagnosis of blunt pancreatic trauma. 展开更多

Pancreatic trauma is rare compared to other abdominal solid organ injuries,accounting for 0.2%-0.3% of all trauma patients. Moreover, this type of injury may frequently be overlooked or not readily appreciated on initial clinical examinations and investigations. The organ injury scale determines the severity of the trauma. Nonetheless, there are conflicting recommendations for the best strategy in severe cases. Overall, conservative management of induced severe traumatic pancreatitis is adequate. Modern imaging modalities such as ultrasound scanning and computed tomography scanning can detect injuries in fewer than 60% of patients. However, magnetic resonance cholangiopancreatography and endoscopic retrograde cholangiopancreatography(ERCP) have diagnostic accuracies approaching 90%-100%. Thus, management options include ERCP and stent placement or distal pancreatectomy in cases of complete gland transection and wide drainage only for damage control surgery, which can prevent mortality but increases the risk of morbidity. In the majority of cases, surgical intervention is not required and should be reserved for only severe grade Ⅲ to grade Ⅴ injuries. 展开更多

We present a case of an 11 year old boy presenting with history of blunt trauma abdomen following handle bar injury. He had signs of abdominal distension and CECT of the abdomen showed transection of the pancreas in the region of junction between the body and tail. On exploratory laparotomy there was rupture at junction of body and tail of pancreas with intact pancreatic duct and a spleen preserving distal pancreatectomy was performed. In the presence of an intact pancreatic duct conservative management should be offered however this patient underwent spleen preserving distal pancreatectomy because of pancreatitis and necrosis ensuing in the pancreatic remnant margin. Hence, complete transection does not necessarily mean loss of ductal integrity and decision to preserve or remove the distal pancreas should not be based merely on ductal integrity but probably other factors also i.e. pancreatitis and necrosis. 展开更多

BACKGROUND In recent years,we created and employed a new anastomosis method,“bridging”pancreaticogastrostomy,to treat patients with extremely severe pancreatic injury.This surgery has advantages such as short length of surgery,low secondary trauma,rapid construction of shunts for pancreatic fluid,preventing second surgeries,and achieving good treatment outcomes in clinical practice.However,due to the limited number of clinical cases,there is a lack of strong evidence to support the feasibility and safety of this surgical procedure.Therefore,we carried out animal experiments to examine this procedure,which is reported here.AIM To examine the feasibility and safety of a new rapid method of pancreaticogastrostomy,“bridging”pancreaticogastrostomy.METHODS Ten Landrace pigs were randomized into the experimental and control groups,with five pigs in each group.“Bridging”pancreaticogastrostomy was performed in the experimental group,while routine mucosa-to-mucosa pancreaticogastrostomy was performed in the control group.After surgery,the general condition,amylase levels in drainage fluid on Days 1,3,5,and 7,fasting and 2-h postprandial blood glucose 6 mo after surgery,fasting,2-h postprandial peripheral blood insulin,and portal vein blood insulin 6 mo after surgery were assessed.Resurgery was carried out at 1 and 6 mo after the former one to examine the condition of the abdominal cavity and firmness and tightness of the pancreaticogastric anastomosis and pancreas.RESULTS After surgery,the general condition of the animals was good.One in the control group did not gain weight 6 mo after surgery,whereas significant weight gain was present in the others.There were significant differences on Days 1 and 3 after surgery between the two groups but no differences on Days 5 and 7.There were no differences in fasting and 2-h postprandial blood glucose and fasting and 2-h insulin values of postprandial peripheral blood and portal vein blood 6 mo after surgery between the two groups.One month after surgery,the sinus tract orifice/anastomosis was patent in the two groups.Six months after surgery,the sinus tract orifice/anastomosis was sealed,and pancreases in both groups presented with chronic pancreatitis.CONCLUSION“Bridging”pancreaticogastrostomy is a feasible and safe a means of damage control surgery during the early stage of pancreatic injury. 展开更多

Colonoscopy is an integral part of the lower bowel care and is generally considered a potentially safe diagnostic and therapeutic procedure performed as a daycare outpatient procedure.Colonoscopy is associated with different complications that are not limited to adverse events related to the bowel preparation solutions used,the sedatives used,but to the procedure related as well including bleeding and perforation.Injuries to the extra-luminal abdominal organs during colonoscopy are uncommon,however,serious complications related to the procedure have been reported infrequently in the literature.Life threatening injuries to the spleen,liver,pancreas,mesentery,and urinary bladder have been reported as early as in mid-1970s.These injuries should not be overlooked by clinicians and endoscopists.Steadily increasing abdominal pain,abdominal distension,and hemodynamic instability in absence of rectal bleeding should raise the possibility of severe organ injury.Splenic and hepatic injury following colonoscopy are usually serious and may be life threatening.Although conservative management may help,yet they usually need interventional radiology or surgical intervention.Acute pancreatitis following colonoscopy is usually mild and is mostly managed conservatively.The mechanism of abdominal organ injuries during colonoscopy is not fully understood,however many risk factors have been identified,which can be classified as-organ related,procedure related,and local abdominal factors.Difficult colonoscopy and prior intra-abdominal adhesions are probably the most relevant risk factors for these injuries.Left lateral position,avoidance of looping and excessive force during the procedure would probably reduce the risk of such injuries. 展开更多

BACKGROUND Trauma is a common cause of pancreatic duct disruption.Surgical treatment is recommended in current clinical guidelines for adult pancreatic injury because non-surgical treatments have higher risks of serious complications or even death compared with surgical treatment.CASE SUMMARY A 22-year-old woman was admitted to Tiantai People’s Hospital of Zhejiang Province after 1-h duration of abdominal pain and distension following trauma.The diagnosis was“traumatic pancreatic rupture”.The patient’s symptoms were not severe,her vital signs were stable,and signs of peritonitis were not obvious.Therefore,conservative treatment could be considered,with the possibility of emergency surgery if necessary.After 2 mo of conservative treatment with duct drainage,the pancreatic duct healed spontaneously with no significant complications.CONCLUSION We report a case of pancreatic duct disruption in the head and neck caused by trauma that was treated conservatively and healed spontaneously,providing a new choice for clinical practice.For isolated pancreatic injury with rupture of the pancreatic duct in the head and neck,conservative treatment under close observation is feasible. 展开更多

We comment on a study titled“Feasibility and safety of"bridging"pancreaticogastrostomy for pancreatic trauma in Landrace pigs”in which ten pigs were randomized to either experimental“bridging”pancreaticogastrostomy(PG)or a control group with a routine mucosa-to-mucosa PG.At six months anastomoses had strictured and closed in both groups.The authors concluded that“bridging”PG is feasible and safe in damage control surgery during the early stage of pancreatic injury.In this letter we comment on the study design,specifically leaving a 2 cm gap between the pancreatic stump and the stomach and highlight the complexity of performing pancreatic anastomoses following trauma pancreaticoduodenectomy as to our experience in a high volume trauma centre.Our data emphasize that pancreatic anastomoses in trauma are complex procedures with significant postoperative morbidity and are best managed collaboratively by trauma and hepatopancreaticobiliary surgical teams with the required technical skills. 展开更多

BACKGROUND In order to successfully manage traumatic pancreatic duct(PD)leaks,early diagnosis and operative management is paramount in reducing morbidity and mortality.In the acute setting,endoscopic retrograde cholangiopancreatography(ERCP)can be a useful,adjunctive modality during exploratory laparotomy.ERCP with sphincterotomy and stent placement improves preferential drainage in the setting of injury,allowing the pancreatic leak to properly heal.However,data in this acute setting is limited.CASE SUMMARY In this case series,a 27-year-old male and 16-year-old female presented with PD leaks secondary to a gunshot wound and blunt abdominal trauma,respectively.Both underwent intraoperative ERCP within an average of 5.9 h from time of presentation.A sphincterotomy and plastic pancreatic stent placement was performed with a 100%technical and clinical success.There were no associated immediate or long-term complications.Following discharge,both patients underwent repeat ERCP for stent removal with resolution of ductal injury.CONCLUSION These experiences further demonstrated that widespread adaption and optimal timing of ERCP may improve outcomes in trauma centers. 展开更多

Surgery such as digestive tract reconstruction is usually required for pancreatic trauma and severe pancreatitis as well as malignant pancreatic lesions. The most common digestive tract reconstruction techniques (e.g., Child&#x02019;s type reconstruction) for neoplastic diseases of the pancreatic head often encompass pancreaticojejunostomy, choledochojejunostomy and then gastrojejunostomy with pancreaticoduodenectomy, whereas these techniques may not be applicable in benign pancreatic diseases due to an integrated stomach and duodenum in these patients. In benign pancreatic diseases, the aforementioned reconstruction will not only increase the distance between the pancreaticojejunostomy and choledochojejunostomy, but also the risks of traction, twisting and angularity of the jejunal loop. In addition, postoperative complications such as mixed fistula are refractory and life-threatening after common reconstruction procedures. We here introduce a novel pancreaticojejunostomy, hepaticojejunostomy and double Roux-en-Y digestive tract reconstruction in two cases of benign pancreatic disease, thus decreasing not only the distance between the pancreaticojejunostomy and choledochojejunostomy, but also the possibility of postoperative complications compared to common reconstruction methods. Postoperatively, the recovery of these patients was uneventful and complications such as bile leakage, pancreatic leakage and digestive tract obstruction were not observed during the follow-up period. 展开更多

Penetrating abdominal trauma is the main cause of pancreatic lesions and delay in diagnosis or treatment can increase morbimortality. We present a case of acute necrohemorrhagic pancreatitis (ANHP) secondary to airgun injury associated with pulmonary embolism caused by the projectile in a 36 year old man. He underwent urgent surgery, appreciating pancreatic contusion but not visualizing the projectile, located by CT scan 3mm from the inferior vena cava. The patient underwent further surgery 48 hours later for necrosectomy and the insertion of an irrigation tube, due to ANHP after the migration of the projectile into the lung. This case underlines the clinical relevance of pancreatic lesion in patients with a penetrating abdominal trauma, the diagnostic difficulty and the surgical strategy, as well as the repercussions of the migration of a foreign body through the blood stream, crossing the right heart chambers and becoming lodged in the lung. 展开更多

A novel machine learning-assisted clinical diagnosis support model for early identification of pancreatic injuries in patients with blunt abdominal trauma:a cross-national study

Background:The recognition of pancreatic injury in blunt abdominal trauma is often severely delayed in clinical practice.The aim of this study was to develop a machine learning model to support clinical diagnosis for early detection of abdominal trauma.Methods:We retrospectively analyzed of a large intensive care unit database(Medical Information Mart for Intensive Care[MIMIC]-IV)for model development and internal validation of the model,and performed outer validation based on a cross-national data set.Logistic regres-sion was used to develop three models(PI-12,PI-12-2,and PI-24).Univariate and multivariate analyses were used to determine variables in each model.The primary outcome was early detection of a pancreatic injury of any grade in patients with blunt abdominal trauma in the first 24 hours after hospitalization.Results:The incidence of pancreatic injuries was 5.56%(n=18)and 6.06%(n=6)in the development(n=324)and internal validation(n=99)cohorts,respectively.Internal validation cohort showed good discrimination with an area under the receiver operator characteristic curve(AUC)value of 0.84(95%confidence interval[CI]:0.71–0.96)for PI-24.PI-24 had the best AUC,specificity,and positive predictive value(PPV)of all models,and thus it was chosen as the final model to support clinical diagnosis.PI-24 performed well in the outer validation cohort with an AUC value of 0.82(95%CI:0.65–0.98),specificity of 0.97(95%CI:0.91–1.00),and PPV of 0.67(95%CI:0.00–1.00).Conclusion:A novel machine learning-based model was developed to support clinical diagnosis to detect pancreatic injuries in patients with blunt abdominal trauma at an early stage. 展开更多

创伤性胰腺损伤12例外科治疗分析

目的探讨外伤所致闭合性腹部损伤中胰腺损伤的外科治疗方法。方法回顾性收集2018年1月至2023年8月西安交通大学第一附属医院收治并行外科治疗的12例创伤性胰腺损伤病人的临床资料,从受伤原因、诊断方法、外科治疗方法、并发症等方面进... 目的探讨外伤所致闭合性腹部损伤中胰腺损伤的外科治疗方法。方法回顾性收集2018年1月至2023年8月西安交通大学第一附属医院收治并行外科治疗的12例创伤性胰腺损伤病人的临床资料,从受伤原因、诊断方法、外科治疗方法、并发症等方面进行分析。结果12例胰腺损伤病人的受伤方式:交通伤8例,撞击伤4例。胰腺损伤分级:Ⅱ级2例,Ⅲ级6例,Ⅳ级1例,Ⅴ级3例。8例病人术前行血清淀粉酶测定,其中6例血清淀粉酶升高。10例病人术前CT检查诊断胰腺损伤。所有病人均行手术治疗,其中行腹腔积血清除1例,腹腔积血清除术加胰腺修补术1例,胰体尾切除术6例(其中同时行脾脏切除4例),行胰腺坏死组织清除加胰液外引流术并于1年后行二期胰肠吻合术1例,胰十二指肠切除2例,行肠系膜上静脉修补加胰腺坏死组织清除加腹腔置管通畅引流术1例。所有病人中,死于多器官功能衰竭1例,发生术后并发症4例,其中胰瘘2例,腹腔感染2例。结论闭合性腹部损伤病人行CT检查是早期诊断胰腺损伤的重要方式,早期进行分级有助于手术方式的选择决策。经术前检查提示胰腺高级别损伤者应在生命体征维持平稳的前提下及时行手术治疗,选择合理的手术方式及术后通畅引流是防止术后并发症的关键。 展开更多

目的探讨胎盘间充质干细胞(P-MSCs)对胰腺创伤(PT)大鼠的保护作用。方法健康成年雄性SD大鼠60只,随机分为对照组、胰腺创伤组(胰腺损伤区域及创伤周围局部注射1 ml PBS溶液)与P-MSCs组[胰腺损伤区域及创伤周围局部注射1 ml P-MSCs(1... 目的探讨胎盘间充质干细胞(P-MSCs)对胰腺创伤(PT)大鼠的保护作用。方法健康成年雄性SD大鼠60只,随机分为对照组、胰腺创伤组(胰腺损伤区域及创伤周围局部注射1 ml PBS溶液)与P-MSCs组[胰腺损伤区域及创伤周围局部注射1 ml P-MSCs(1×106个/ml)],每组20只。采用400 kPa的致伤压强建立胰腺创伤大鼠模型。于术后1、3、5、7 d每组处死5只大鼠,收集血清及胰腺组织。HE染色观察胰腺组织病理学变化并进行病理损伤评分,ELISA法测定血清中淀粉酶(AMS)、脂肪酶(LPS)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、IL-10、转化生长因子-β1(TGF-β1)浓度,以及胰腺组织中髓过氧化物酶(MPO)、超氧化物歧化酶(SOD)活性,TUNEL法观察胰腺组织细胞凋亡情况。结果与对照组比较,胰腺创伤组与P-MSCs组在胰腺创伤发生后腹腔积液量增加(P<0.05),胰腺/体重比、胰腺组织病理损伤总评分均升高(P<0.05),血清AMS、LPS、TNF-α、IL-6浓度及MPO活性早期升高,随时间延长呈下降趋势(P<0.05),而抗炎因子IL-10浓度及SOD活性随时间延长呈上升趋势(P<0.01),TGF-β1浓度早期下降,随时间延长呈上升趋势(P<0.01),凋亡指数(AI)明显升高(P<0.001)。与胰腺创伤组比较,P-MSCs组大鼠胰腺组织大体形态得以改善,腹腔积液量减少(P<0.001),胰腺/体重比、胰腺组织病理损伤总评分降低(P<0.05),血清AMS、LPS、IL-6、TNF-α浓度及MPO活性更快恢复至正常水平(P<0.05),而抗炎因子IL-10、TGF-β1浓度及SOD活性升高速度增加(P<0.05),AI升高(P<0.001)。结论P-MSCs可促进PT大鼠组织修复,减轻其局部及全身炎症,改善组织氧化应激反应,促进胰腺腺泡细胞凋亡,对PT大鼠具有保护作用。 展开更多
