Inter-Observer Variability in the Interpretation of 68Ga-PSMA PET-CT Scan according to PROMISE Criteria

Aims & Objectives: Aim of the study was to evaluate inter-observer variability in interpretation of Gallium-68 labeled Prostate Specific Membrane Antigen sub-type 11 (68Ga-PSMA-11) Positron Emission Tomography-Computed Tomography PET_CT scan according to Prostate Cancer Molecular Imaging Standardized Evaluation (PROMISE) criteria. Method and Materials: Forty-four consecutive patients of prostate cancer were prospectively studied between the duration of January 2021 to June 2021 at Institute of Nuclear Medicine and Oncology (INMOL), Lahore. All PET-CT scans were assessed by a researcher and 3 nuclear physicians and divided into two groups, interpreted in two phases. In the first phase, each group independently evaluated the scans while in the second phase, a consensus meeting was held and all the cases with discordance were discussed. Cohen’s Kappa test was used to measure interobserver variability with the cut-off of K’s alpha Results: The study showed 41 out of 44 scans with positive PSMA findings while 03 scans were negative for any PSMA avid disease. In the first phase of image analysis, the level of agreement was slight in T stage (Kappa = 0.068, p = 0.65), moderate in the miN stage (Kappa = 0.46, p = 0.02) and substantial in miM stage (Kappa = 0.77, p ≤ 0.001) was seen. 目的探讨^(18)F-FDG PET-CT联合MRI在前列腺癌诊断及分期中的应用价值。方法选取我院2020年8月—2023年8月期间收治的70例疑似前列腺癌患者为研究对象,纳入患者均接受PET-CT及MRI检查,以病理穿刺活检结果作为诊断金标准,评价二者联合诊... 目的探讨^(18)F-FDG PET-CT联合MRI在前列腺癌诊断及分期中的应用价值。方法选取我院2020年8月—2023年8月期间收治的70例疑似前列腺癌患者为研究对象,纳入患者均接受PET-CT及MRI检查,以病理穿刺活检结果作为诊断金标准,评价二者联合诊断的诊断效能。结果70例疑似前列腺癌患者,经过穿刺活检确诊为前列腺癌50例,前列腺良性病变20例;其中MRI诊断出前列腺癌43例,前列腺良性病变27例;PET-CT诊断出前列腺癌48例,前列腺良性病变23例;PET-CT+MRI诊断出前列腺癌51例,前列腺良性病变19例;PET-CT联合MRI诊断的准确度、敏感度高于单一诊断,漏诊率低于单一诊断(P<0.05),而二者联合诊断前列腺癌的特异度及误诊率无统计学差异(P>0.05)。病理分期诊断上,50例经病理活检确诊为前列腺癌的患者,低分化腺癌13例;T期40例(T1~T2期11例,T3期22例,T4期7例),N期10例;对于前列腺癌T分期评价,PET-CT+MRI检查的准确度高于单一检查(P<0.05)。结论PET-CT联合MRI相比二者任一检查对前列腺癌及其病理分期具有较高的诊断效能,可减少漏诊率及误诊率。 展开更多

目的:探讨规范化流程管理在非小细胞肺癌患者PET-CT检查中的应用效果,为确保临床非小细胞肺癌的诊疗质量提供参考依据。方法:选取2019年3月—2021年10月河南省肿瘤医院收治的112例非小细胞肺癌患者作为研究对象,按照随机数表法分组,患... 目的:探讨规范化流程管理在非小细胞肺癌患者PET-CT检查中的应用效果,为确保临床非小细胞肺癌的诊疗质量提供参考依据。方法:选取2019年3月—2021年10月河南省肿瘤医院收治的112例非小细胞肺癌患者作为研究对象,按照随机数表法分组,患者均接受PET-CT检查,且检查期间,予以对照组(56例)患者原工作流程操作,予以观察组(56例)患者规范化流程管理,比较两组患者图像合格率、检查前准备完好率、检查时准备完好率、检测的等待时间和检查时间,干预前后焦虑、抑郁情绪评分,以及患者对护理工作的满意度。结果:观察组患者检查前准备完好率、检查时准备完好率高于对照组,差异有统计学意义(χ^(2)=5.617、1.734,P<0.05),同时观察组患者检测的等待时间和检查时间均较对照组缩短,差异有统计学意义(t=5.469、4.941,P<0.05)。与干预前比,干预后两组患者情绪焦虑自评量表(SAS)、抑郁自评量表(SDS)评分降低,且试验组患者低于对照组,差异有统计学意义(t=11.298、8.414,P<0.05)。观察组患者对护理工作的满意度高于对照组,差异有统计学意义(χ^(2)=4.350,P<0.05)。结论:非小细胞肺癌患者PET-CT检查中应用规范化流程管理可缩短患者的检查等待和检查时间,提升诊疗效率,并取得良好的图像质量,患者对护理工作的满意度高。 展开更多

Optical reflection anisotropy microscopy mappings of micropipe defects on the surface of a 4H-SiC single crystal are studied by the scanning anisotropy microscopy(SAM)system.The reflection anisotropy(RA)image with a'butterfly pattern'is obtained around the micropipes by SAM.The RA image of the edge dislocations is theoretically simulated based on dislocation theory and the photoelastic principle.By comparing with the Raman spectrum,it is verified that the micropipes consist of edge dislocations.The different patterns of the RA images are due to the different orientations of the Burgers vectors.Besides,the strain distribution of the micropipes is also deduced.One can identify the dislocation type,the direction of the Burgers vector and the optical anisotropy from the RA image by using SAM.Therefore,SAM is an ideal tool to measure the optical anisotropy induced by the strain field around a defect. 展开更多

In recent years,heavy ion accelerator technology has been rapidly developing worldwide and widely applied in the fields of space radiation simulation and particle therapy.Usually,a very high uniformity in the irradiation area is required for the extracted ion beams,which is crucial because it directly affects the experimental precision and therapeutic effect.Specifically,ultra-large-area and high-uniformity scanning are crucial requirements for spacecraft radiation effects assessment and serve as core specification for beamline terminal design.In the 300 MeV proton and heavy ion accelerator complex at the Space Environment Simulation and Research Infrastructure(SESRI),proton and heavy ion beams will be accelerated and ultimately delivered to three irradiation terminals.In order to achieve the required large irradiation area of 320 mm×320 mm,horizontal and vertical scanning magnets are used in the extraction beam line.However,considering the various requirements for beam species and energies,the tracking accuracy of power supplies(PSs),the eddy current effect of scanning magnets,and the fluctuation of ion bunch structure will reduce the irradiation uniformity.To mitigate these effects,a beam uniformity optimization method based on the measured beam distribution was proposed and applied in the accelerator complex at SESRI.In the experiment,the uniformity is successfully optimized from 75%to over 90%after five iterations of adjustment to the PS waveforms.In this paper,the method and experimental results were introduced. 展开更多

To investigate the macroscopic fatigue properties and the mesoscopic pore evolution characteristics of salt rock under cyclic loading,fatigue tests under different upper-limit stresses were carried out on salt rock,and the mesoscopic pore structures of salt rock before and after fatigue tests and under different cycle numbers were measured using CT scanning instrument.Based on the test results,the effects of the cycle number and the upper-limit stress on the evolution of cracks,pore morphology,pore number,pore volume,pore size,plane porosity,and volume porosity of salt rock were analyzed.The failure path of salt rock specimens under cyclic loading was analyzed using the distribution law of plane porosity.The damage variable of salt rock under cyclic loading was defined on basis of the variation of volume porosity with cycle number.In order to describe the fatigue deformation behavior of salt rock under cyclic loading,the nonlinear Burgers damage constitutive model was further established.The results show that the model established can better reflect the whole development process of fatigue deformation of salt rock under cyclic loading. 展开更多

AIM:To propose an algorithm for automatic detection of diabetic retinopathy(DR)lesions based on ultra-widefield scanning laser ophthalmoscopy(SLO).METHODS:The algorithm utilized the FasterRCNN(Faster Regions with CNN features)+ResNet50(Residua Network 50)+FPN(Feature Pyramid Networks)method for detecting hemorrhagic spots,cotton wool spots,exudates,and microaneurysms in DR ultra-widefield SLO.Subimage segmentation combined with a deeper residual network FasterRCNN+ResNet50 was employed for feature extraction to enhance intelligent learning rate.Feature fusion was carried out by the feature pyramid network FPN,which significantly improved lesion detection rates in SLO fundus images.RESULTS:By analyzing 1076 ultra-widefield SLO images provided by our hospital,with a resolution of 2600×2048 dpi,the accuracy rates for hemorrhagic spots,cotton wool spots,exudates,and microaneurysms were found to be 87.23%,83.57%,86.75%,and 54.94%,respectively.CONCLUSION:The proposed algorithm demonstrates intelligent detection of DR lesions in ultra-widefield SLO,providing significant advantages over traditional fundus color imaging intelligent diagnosis algorithms. 展开更多

Optical endoscopy has become an essential diagnostic and therapeutic approach in modern biomedicine for directly observing organs and tissues deep inside the human body,enabling non-invasive,rapid diagnosis and treatment.Optical fiber endoscopy is highly competitive among various endoscopic imaging techniques due to its high flexibility,compact structure,excellent resolution,and resistance to electromagnetic interference.Over the past decade,endoscopes based on a single multimode optical fiber(MMF)have attracted widespread research interest due to their potential to significantly reduce the footprint of optical fiber endoscopes and enhance imaging capabilities.In comparison with other imaging principles of MMF endoscopes,the scanning imaging method based on the wavefront shaping technique is highly developed and provides benefits including excellent imaging contrast,broad applicability to complex imaging scenarios,and good compatibility with various well-established scanning imaging modalities.In this review,various technical routes to achieve light focusing through MMF and procedures to conduct the scanning imaging of MMF endoscopes are introduced.The advancements in imaging performance enhancements,integrations of various imaging modalities with MMF scanning endoscopes,and applications are summarized.Challenges specific to this endoscopic imaging technology are analyzed,and potential remedies and avenues for future developments are discussed. 展开更多

Three-dimensional finite element simulation and reconstruction of jointed rock models using CT scanning and photogrammetry

The geometry of joints has a significant influence on the mechanical properties of rocks.To simplify the curved joint shapes in rocks,the joint shape is usually treated as straight lines or planes in most laboratory experiments and numerical simulations.In this study,the computerized tomography (CT) scanning and photogrammetry were employed to obtain the internal and surface joint structures of a limestone sample,respectively.To describe the joint geometry,the edge detection algorithms and a three-dimensional (3D) matrix mapping method were applied to reconstruct CT-based and photogrammetry-based jointed rock models.For comparison tests,the numerical uniaxial compression tests were conducted on an intact rock sample and a sample with a joint simplified to a plane using the parallel computing method.The results indicate that the mechanical characteristics and failure process of jointed rocks are significantly affected by the geometry of joints.The presence of joints reduces the uniaxial compressive strength (UCS),elastic modulus,and released acoustic emission (AE) energy of rocks by 37%–67%,21%–24%,and 52%–90%,respectively.Compared to the simplified joint sample,the proposed photogrammetry-based numerical model makes the most of the limited geometry information of joints.The UCS,accumulative released AE energy,and elastic modulus of the photogrammetry-based sample were found to be very close to those of the CT-based sample.The UCS value of the simplified joint sample (i.e.38.5 MPa) is much lower than that of the CT-based sample (i.e.72.3 MPa).Additionally,the accumulative released AE energy observed in the simplified joint sample is 3.899 times lower than that observed in the CT-based sample.CT scanning provides a reliable means to visualize the joints in rocks,which can be used to verify the reliability of photogrammetry techniques.The application of the photogrammetry-based sample enables detailed analysis for estimating the mechanical properties of jointed rocks. 展开更多

早期孤立性肺腺癌患者淋巴结转移风险因素及与PET-CT摄取的关系研究 被引量:4

目的 探讨早期孤立性肺腺癌患者淋巴结转移(LNM)风险因素及与正电子发射断层成像-X线计算机断层成像(PET-CT)摄取的关系。方法 纳入2015年1月至2021年9月于我院行手术治疗临床组织分期(cT)1期孤立性肺腺癌患者共251例,根据病理淋巴结(pN... 目的 探讨早期孤立性肺腺癌患者淋巴结转移(LNM)风险因素及与正电子发射断层成像-X线计算机断层成像(PET-CT)摄取的关系。方法 纳入2015年1月至2021年9月于我院行手术治疗临床组织分期(cT)1期孤立性肺腺癌患者共251例,根据病理淋巴结(pN)分期划分为NO组(225例)、N1组(16例)及N2组(10例),分析临床病理资料及术前薄层CT检查资料,采用多因素Logistic回归模型评价患者LNM发生独立危险因素及与最大标准摄取值(SUVmax)的相关性。结果 单因素分析结果 显示,病灶最大径、磨玻璃结节类型、血CEA水平及腺癌病理组织学类型均与早期孤立性肺腺癌患者LNM发生有关(P<0.05);多因素Logistic回归模型分析结果 显示,混合型磨玻璃结节、实性磨玻璃结节、血CEA水平>5ng/mL、腺泡型肺腺癌、乳头状/微乳头状浸润型肺腺癌及实体型肺腺癌均是早期孤立性肺腺癌患者LNM发生独立危险因素(P<0.05)。单因素分析结果 显示,早期孤立性肺腺癌患者LNM发生与SUVmax有关(P<0.05);多因素Logistic回归模型分析结果 显示,SUVmax>5是早期孤立性肺腺癌患者LNM发生独立危险因素(OR=3.42,95%CI:1.40~10.68,P=0.00)。结论 早期孤立性肺腺癌患者LNM发生与磨玻璃结、血CEA水平及病理组织学类型关系密切;同时PET-CT检查SUVmax>5者更易出现淋巴结转移。 展开更多

目的探讨正电子发射计算机断层显像(PET-CT)在非小细胞肺癌术前区域淋巴结转移评估中的应用价值。方法收集2020年1月至12月揭阳市人民医院胸心外科的非小细胞肺癌患者60例为研究对象,其中男性24例,女性36例,年龄(58.98±7.97)岁。... 目的探讨正电子发射计算机断层显像(PET-CT)在非小细胞肺癌术前区域淋巴结转移评估中的应用价值。方法收集2020年1月至12月揭阳市人民医院胸心外科的非小细胞肺癌患者60例为研究对象,其中男性24例,女性36例,年龄(58.98±7.97)岁。术前行18氟脱氧葡萄糖(18F-FDG)PET-CT和增强CT扫描,详细记录肺门纵隔淋巴结转移情况,以术后病理诊断作为金标准,计算并对比两者的准确度、灵敏度、特异度、阳性预测值和阴性预测值。采用配对χ2检验。结果根据术后病理诊断结果,在60例非小细胞肺癌患者中,出现区域淋巴结转移有22例,未出现淋巴结转移有38例。PET-CT正确诊断52例,其中确诊肺门纵隔淋巴结转移20例,未出现淋巴结转移32例,假阳性6例,假阴性2例,其准确度、灵敏度、特异度、阳性预测值和阴性预测值分别为0.87、0.91、0.84、0.77、0.94。增强CT扫描正确诊断38例,其中确诊肺门纵隔淋巴结转移14例,未出现淋巴结转移24例,假阳性14例,假阴性8例,其准确度、灵敏度、特异度、阳性预测值和阴性预测值分别为0.63、0.64、0.63、0.50、0.75。两种检查方法的准确度、灵敏度、特异度差异均有统计学意义(均P<0.05)。结论PET-CT在非小细胞肺癌术前区域淋巴结转移评估中的诊断效能优于增强CT扫描,值得推广应用。 展开更多

The aim of this paper is to discuss the risk of recurrence in patients with differentiated thyroid cancer and emphasize the importance of risk-group stratification,early recurrence identification and application of new imaging modalities,what is the PET-CT.Moreover,follow-up of patients with thyroid carcinoma should be carried out by specialized teams throughout life.Therefore,interdisciplinary case discussions in tumor conferences may improve the use of multimodal therapy especially in patients with poorly differentiated thyroid carcinomas.After baseline follow-up,if there is a suspicion of thyroid carcinoma,early PET-CT should be used for early detection and appropriate planning.Fortunately,due to the good localization possibility,the PET-CT enables a focused surgical procedure with avoidance of an unnecessary tumor search and thereby a reduction of the risk of injury of neighboring structures which is a concern with reoperative neck surgery. 展开更多

近年来,深度学习在各种医学图像语义分割任务中表现出了优异的性能。然而,在癌症的淋巴结转移以及一些细小病灶的分割任务中,基于单模态图像的语义分割仍然存在局限性。PET-CT这一多模态成像模式提供了有关结构和生理病理的解剖和功能信... 近年来,深度学习在各种医学图像语义分割任务中表现出了优异的性能。然而,在癌症的淋巴结转移以及一些细小病灶的分割任务中,基于单模态图像的语义分割仍然存在局限性。PET-CT这一多模态成像模式提供了有关结构和生理病理的解剖和功能信息,被认为是癌症诊断、分期和治疗反应评估的首选成像设备。PET-CT携带的多模态互补信息被引入深度卷积神经网络的语义分割模型中辅助病灶的分割。首先,聚焦于PET-CT多模态图像特征融合在卷积神经网络图像语义分割中的应用,介绍多模态成像的特点以及常用的医学图像分割网络。然后,归纳和总结了现阶段基于深度学习的多模态融合的3种思路,并划分为早期融合、后期融合和混合融合,并从分割的性能、参数量的大小、方法的亮点与不足等维度对多模态融合分割方法进行优缺点分析。最后,对现阶段多模态医学图像分割存在的问题进行讨论。 展开更多
