A biologically active antibacterial reagent, 2-amino-6-hydroxy-4-(4-N, N-dimethylaminophenyl)-pyr- imidine-5-carbonitrile (AHDMAPPC), was synthesized. It was employed to investigate the binding in- teraction with the bovine serum albumin (BSA) in detail using different spectroscopic methods. It ex- hibited antibacterial activity against Escherichia cali and Staphylococcus aureus which are common food poisoning bacteria. The experimental results showed that the fluorescence quenching of model carrier protein BSA by AHDMAPPC was due to static quenching. The site binding constants and number of binding sites (n ≈ 1) were determined at three different temperatures based on fluorescence quenching results. The thermodynamic parameters, enthalpy change (AH), free energy (AG) and entropy change (AS) for the reaction were calculated to be 15.15 kJ/mol, -36.11 kJ/mol and 51.26J/mol K according to van't Hoff equation, respectively. The results indicated that the reaction was an endothermic and spontaneous process, and hydrophobic interactions played a major role in the binding between drug and BSA. The distance between donor and acceptor is 2.79 nm according to Forster's theory. The alterations of the BSA secondary structure in the presence of AHDMAPPC were confirmed by UV-visible, synchronous fluorescence, circular dichroism (CD) and three-dimensional fluorescence spectra. All these results in- dicated that AHDMAPPC can bind to BSA and be effectively transported and eliminated in the body. It can be a useful guideline for further drug design. 展开更多

Albumin nanoparticles are considered to be an effective way to load water-insoluble anticancer drugs and target tumors via the gp60-mediated pathway. Herein, we fabricated an albumin nanoparticle formulation for co-loading paclitaxel(PTX) and curcumin(CCM), both of which have prominent anticancer efficacy, via nanoparticle albumin-bound(NabTM) technology using high-pressure homogenization. The PTX/CCM co-bound albumin nanoparticles(PTX/CCM Alb-NPs) had a slightly greater particle size of ~250 nm than that of plain PTX AlbNPs and CCM Alb-NPs(~234 and ~134 nm, respectively), with spherical surface morphology and stable size maintenance. However, the zeta potential of PTX/CCM Alb-NPs(ca.-30 mV)was not significantly different from that of PTX or CCM Alb-NPs. The loaded PTX and CCM were released gradually from the PTX/CCM Alb-NPs over ~24 h(97.7 ± 1.7% and 76.2 ± 0.5%,respectively). Furthermore, PTX/CCM Alb-NPs appeared to be efficiently internalized into Mia Paca-2 cells and exhibited a 71% increased IC50 versus PTX Alb-NPs in terms of cytotoxicity to Mia Paca-2 cells. These results suggest that PTX/CCM Alb-NPs are a new potential anticancer agent for combination therapy. 展开更多

Hydroxycinnamic acids (HCAs) possess numer- ous biological effects including antioxidant, antiallergic, antimicrobial, and immunomodulatory activities and due to these properties are widely used in folk medicine. Nevertheless, they can interact with protein molecules and cause some structural and functional changes. The possib- ility of HCAs binding to bovine serum albumin (BSA) under physiological conditions was inve- stigated by the UV-VIS absorption spectroscopy and fluorescence quenching method. Apart from rosmarinic acid, all tested HCAs quenched tryptophan fluorescence of BSA in the studied range of concentrations (0-20 μM) mainly by static quenching mechanism (formation of non- fluorescent HCA-BSA complexes). The binding constants, number of binding sites and free energy changes were determined. The binding affinities of HCAs were ranked in the order: chlorogenic acid > sinapic acid ≥ caffeic acid > ferulic acid > o-coumaric acid > p-coumaric acid ≥ m-coumaric acid, which was confirmed by spectral overlaps of BSA emission spectrum with absorption spectrum of HCA. All free energy changes possessed negative sign indicating the spontaneity of HCA-BSA interaction. 展开更多

Weekly albumin-bound paclitaxel/cisplatin versus gemcitabine/cisplatin as first-line therapy for patients with advanced non-small-cell lung cancer:A phase II open-label clinical study 被引量:9

Objective: The aim of this trial was to compare both the efficacy and the safety of a weekly nanoparticle albumin-bound paclitaxel(nab-paclitaxel) plus cisplatin vs. gemcitabine plus cisplatin in patients with advanced non-small-cell lung cancer(NSCLC).Methods: A total of 84 participants received either 100 mg/m^2 nab-paclitaxel each week on d 1, 8 and 15 of a 28 day cycle, as well as cisplatin 75 mg/m^2 on d 1 every three weeks(nab-TP arm); or gemcitabine 1,000 mg/m^2 on d 1 and 8, plus cisplatin 75 mg/m^2 on d 1 every three weeks(GP arm). The primary end point was progression-free survival(PFS). The secondary end points were overall response rate(ORR) and overall survival(OS).Results: According to our analysis, the median PFS was 4.8 months for the nab-TP arm vs. 5.2 months for the GP arm(P=0.55). Analysis showed the median OS was 14.6 months for participants who were in the nab-TP arm vs. 15.1 months for those in the GP arm(P=0.94). Besides, nab-TP showed OS advantages over GP in patients harboring epidermal growth factor receptor(EGFR) mutation(26.7 vs. 15.3 months, P=0.046) and patients with a performance status of 0(23.5 vs. 14.7 months, P=0.020). It was found that incidences of drug-related grade 3 or 4 toxicities were comparable between the two treatment arms.Conclusions: Therefore, it can be seen that weekly nab-TP treatment has a similar efficacy and tolerability to GP treatment for patients who are undergoing their first-line treatment for NSCLC. It could be that survival differences among platinum doublets in the context of both EGFR mutation and performance status have the potential to be the basis for our further clinical trials. 展开更多

The interaction of baicalein with bovine serum albumin(BSA) was investigated with the help of spectroscopic and molecular docking studies.The binding affinity of baicalein towards BSA was estimated to be in order of 10~5 M^(-1) from fluorescence quenching studies.Negative ΔH°(-5.66 + 0.14 kJ/mol) and positive(ΔS°)(+ 79.96 + 0.65J/mol K) indicate the presence of electrostatic interactions along with the hydrophobic forces that result in a positive ΔS°.The hydrophobic association of baicalein with BSA diminishes in the presence of sodium dodecyl sulfate(SDS) due to probable hydrophobic association of baicalein with SDS,resulting in a negative ΔS°(-40.65 + 0.87 J/mol K).Matrix-assisted laser desorption ionization/time of flight(MALDI-TOF) experiments indicate a 1:1 complexation between baicalein and BSA.The unfolding and refolding phenomena of BSA were investigated in the absence and presence of baicalein using steady-state and fluorescence lifetime measurements.It was observed that the presence of urea ruptured the non-covalent interaction between baicalein and BSA.The presence of metal ions(Ag~+,Mg^(2+),Ni^(2+),Mn^(2+),Co^(2+) and Zn^(2+)) increased the binding affinity of ligand towards BSA.The changes in conformational aspects of BSA after ligand binding were also investigated using circular dichroism(CD) and Fourier transform infrared(FT-IR) spectroscopic techniques.Site selectivity studies following molecular docking analyses indicated the binding of baicalein to site 1(subdomain MA) of BSA. 展开更多

The purpose of this study was to evaluate the efficacy and safety of nanoparticle albumin-bound paclitaxel as a rescue regimen in the treatment of patients with advanced non-small-cell lung cancer. We retrospectively reviewed the medical records of 20 patients with stage IV non-small-cell lung cancer. The patients had progressive disease after standard antitumor therapy and subsequently received intravenous albumin-bound paclitaxel at the dose of 100 mg/m2 in weekly schedule. Cumulative findings showed that the overall response rate was 30.0%, the disease control rate amounted to 40%, and the 1 year survival rate was 30%. In addition, the median time to progression and the median survival time reached 5 and 10 months, respectively. Meanwhile, no severe hypersensitivity reactions and grade 4 adverse effects were reported. In summary, weekly-administered albumin-bound paclitaxel seems to be an effective and safe regimen for elderly patients with stage IV non-small-cell lung cancer who were refractory to conventional therapy. 展开更多

Objective: To observe the efficacy and safety of albumin-bound paclitaxel (ABP) monotherapy in treating recurrent advanced non-small-cell lung cancer (NSCLC). Methods: We retrospectively analyzed the short-term efficacy and toxicities of ABP monotherapy in treating 21 patients who had previously undergone multiple cycles of therapy for their advanced NSCLC in our hospital since 2010. The treatment-related survival was also analyzed. Results: Of these 21 patients, the best overall response was partial response (PR) in 6 patients (28.6%), stable disease (SD) in I0 patients (47.6%), and progressive disease (PD) in 5 patients (23.8%). The overall response rate (ORR) was 28.6% and the disease control rate (DCR) (PR + SD) was 76.2%. The median progression-flee survival (PFS) was 4.0 months (95% CI, 5.0-7.0 months). The main grade 3/4 toxicities included neutropenia (11.1%), peripheral nerve toxicity (5.6%), muscle and joint aches (5.6%), and fatigue (5.6%). Conclusions: The ABP monotherapy can achieve good objective response in advanced NSCLC patients who have previously received multiple cycles of treatment and be well tolerated. 展开更多

The binding mechanism of the interactions of halide ions (F–, Br– and I–) with bovine serum albumin (BSA) and hemoglobin (Hb) were studied at different temperatures, by using ion-selective electrodes. The experimental data were treated according to Klotz equation, and the number of binding sites and the binding constants were determined. The results show that the binding sites of F– on protein molecules are more than those of Br– and I–. Additionally, the number of the binding sites for halide ions on protein molecules increases with increasing temperature. This study also indicates that the binding constants for the interactions of halide ions with proteins gradually decrease as the size of halide ions and temperature increases. These behaviors were reasonably interpreted with the structural and thermodynamic factors. The thermodynamic functions at different temperatures were calculated with thermodynamic equations, and the enthalpy change for the interactions were also determined by isothermal titration calorimetry (ITC) at 298.15 K, which indicate that the interactions of halide ions with proteins are mainly electrostatic interaction. 展开更多

The binding of (-)-epigallocatechin-3-O-gallate (EGCG) to bovine serum albumin (BSA) were investigated for the first time with spectral methods, including fluorescence and absorption spectrometry under simulative physiological conditions. A strong fluorescence quenching reaction of EGCG to BSA was observed. It was proved that the fluorescence quenching of BSA by addition of EGCG was a result of the formation of EGCG-BSA complex. The binding constant K and the number of binding sites n were determined at physiological conditions and three different temperatures with fluorescence quenching method. The binding distance R and transfer efficiency E between BSA and EGCG were also obtained according to Forster theory of non-radiation energy transfer. The effects of Al^3+, Cu^2+, Mg^2+ and Fe^2+ on the binding constant between EGCG and BSA were studied at 298 K. 白蛋白结合型紫杉醇联合奈达铂治疗晚期食管癌的临床效果分析

目的分析白蛋白结合型紫杉醇联合奈达铂治疗晚期食管癌的效果。方法66例组织学证实不可切除或者手术后复发或转移性晚期食管癌患者,采用随机数字表法分为对照组和观察组,各33例。对照组行紫杉醇+顺铂治疗,观察组行白蛋白结合型紫杉醇联... 目的分析白蛋白结合型紫杉醇联合奈达铂治疗晚期食管癌的效果。方法66例组织学证实不可切除或者手术后复发或转移性晚期食管癌患者,采用随机数字表法分为对照组和观察组,各33例。对照组行紫杉醇+顺铂治疗,观察组行白蛋白结合型紫杉醇联合奈达铂治疗。对比两组患者临床疗效、生活质量评分、血清肿瘤标记物[糖类抗原125(CA125)、糖类抗原19-9(CA19-9)、鳞状细胞癌抗原(SCC)、癌胚抗原(CEA)]水平及不良反应发生情况。结果观察组客观缓解率(ORR)60.61%高于对照组的36.36%(P<0.05);两组疾病控制率(DCR)比较无差异(P>0.05)。观察组躯体功能、心理功能、社会功能以及物质生活评分分别为(88.80±4.50)、(89.05±4.05)、(88.20±3.50)、(87.80±5.02)分,高于对照组的(73.20±3.80)、(73.60±3.50)、(72.60±3.30)、(71.50±3.50)分(P<0.05)。两组治疗后CA125、CA19-9、SCC、CEA水平均较治疗前降低,且观察组CA125(8.08±1.60)U/ml、CA19-9(8.99±1.22)U/ml、SCC(1.08±0.20)μg/L、CEA(1.47±0.14)ng/ml低于对照组的(9.33±1.45)U/ml、(10.30±1.33)U/ml、(1.67±0.25)μg/L、(1.72±0.18)ng/ml(P<0.05)。观察组血小板减少、中性粒减少、恶心呕吐、肌痛、关节痛、周围神经病变发生率与对照组接近(P>0.05)。结论晚期食管癌患者予以白蛋白结合型紫杉醇联合奈达铂治疗可以提升治疗效果,促进患者血清肿瘤标志物水平及生活质量改善,且未增加不良反应。 展开更多

目的 探究信迪利单抗联合紫杉醇(白蛋白结合型)、铂类化疗对ⅢB~Ⅳ期肺鳞状细胞癌(LSCC)的临床疗效及其对肿瘤标志物的影响。方法 回顾性分析2021年6月至2023年3月乐山市人民医院收治的87例ⅢB~Ⅳ期LSCC患者的临床资料,根据用药方案不... 目的 探究信迪利单抗联合紫杉醇(白蛋白结合型)、铂类化疗对ⅢB~Ⅳ期肺鳞状细胞癌(LSCC)的临床疗效及其对肿瘤标志物的影响。方法 回顾性分析2021年6月至2023年3月乐山市人民医院收治的87例ⅢB~Ⅳ期LSCC患者的临床资料,根据用药方案不同分为对照组(39例)和观察组(48例),对照组采用紫杉醇(白蛋白结合型)、铂类化疗,观察组在对照组的基础上联合信迪利单抗治疗。对比两组患者治疗12周后的临床疗效、肿瘤标志物、不良反应及生存情况。结果 治疗12周后,观察组的客观缓解率和疾病控制率分别为41.67%和81.25%,高于对照组(20.51%和61.54%),差异具有统计学意义(χ^(2)=4.412,4.185,均P<0.05);观察组癌胚抗原、血清细胞角蛋白19片段21-1、鳞状上皮细胞癌抗原水平[分别为(5.78±1.51) ng·m L^(-1)、(4.34±1.21) ng·m L^(-1)、(16.78±3.45) ng·m L^(-1)]低于对照组[分别为(8.45±1.87) ng·m L^(-1)、(6.02±2.02) ng·m L^(-1)、(25.48±4.31) ng·m L^(-1)],差异具有统计学意义(均P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);观察组的中位无进展生存期(PFS)、中位总生存期(OS)分别为14.09个月和17.59个月,长于对照组(10.82个月和12.88个月),差异具有统计学意义(均P<0.05)。结论 信迪利单抗联合紫杉醇(白蛋白结合型)、铂类化疗对ⅢB~Ⅳ期LSCC的疗效较好,可以改善肿瘤标志物的表达,安全性可控。 展开更多

目的:基于肿瘤因子、炎症状态探究紫杉醇白蛋白辅助铂类化疗治疗复发转移性宫颈癌的疗效及安全性。方法:选取2020年1月至2023年1月本院100例复发转移性宫颈癌患者,采用电脑随机数字表法随机分为两组,其中对照组50例,采用紫杉醇辅助奈达... 目的:基于肿瘤因子、炎症状态探究紫杉醇白蛋白辅助铂类化疗治疗复发转移性宫颈癌的疗效及安全性。方法:选取2020年1月至2023年1月本院100例复发转移性宫颈癌患者,采用电脑随机数字表法随机分为两组,其中对照组50例,采用紫杉醇辅助奈达铂方案治疗,研究组50例,采用紫杉醇白蛋白辅助奈达铂方案治疗。比较两组近期疗效、肿瘤因子、炎症因子、复发转移相关指标、生存质量、功能状态、不良反应。结果:研究组总缓解率(72.00%)高于对照组(48.00%)(P<0.05);研究组治疗1个周期、3个周期后血清细胞角蛋白19片段抗原21-1(CYFRA21-1)、糖链抗原724(CA724)、鳞状细胞癌抗原(SCC-Ag)、白介素-4(IL-4)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于对照组,血清微小RNA-367(mi R-367)、微小RNA-383(miR-383)水平、肿瘤患者生活质量评分(QOL)、卡氏功能状态评分(KPS)评分高于对照组(P<0.05);研究组治疗期间恶心/呕吐、腹泻、疲倦、白细胞减少、过敏反应发生率与对照组比较,差异无统计学意义(P>0.05),但总不良反应发生率(12.00%)低于对照组(32.00%)(P<0.05)。结论:紫杉醇白蛋白辅助奈达铂在复发转移性宫颈癌治疗中疗效可靠,能进一步下调肿瘤因子水平,缓解炎症状态,且安全性较高。 展开更多

目的 分析晚期宫颈癌患者行白蛋白结合型紫杉醇联合顺铂治疗前后的变化。方法 100例晚期宫颈癌患者,随机分为研究组和对照组,每组50例。两组患者均接受规定治疗方案同步放化疗,研究组使用注射用紫杉醇(白蛋白结合型)联合顺铂化疗治疗,... 目的 分析晚期宫颈癌患者行白蛋白结合型紫杉醇联合顺铂治疗前后的变化。方法 100例晚期宫颈癌患者,随机分为研究组和对照组,每组50例。两组患者均接受规定治疗方案同步放化疗,研究组使用注射用紫杉醇(白蛋白结合型)联合顺铂化疗治疗,对照组采用溶剂型紫杉醇联合顺铂化疗治疗。比较两组患者病情缓解程度、毒副反应发生情况(血红蛋白降低、血小板减少、白细胞减少、中性粒细胞减少、胃肠道反应、肝功能异常)、治疗前后肿瘤标志物指标[癌胚抗原(CEA)、鳞状细胞癌抗原(SCC-Ag)、细胞角蛋白19片段抗原(CYFRA21-1)、糖类抗原125(CA125)]水平、2年内病情恶化、生存情况。结果 研究组治疗后的病情总缓解率66.00%高于对照组的46.00%,差异有统计学意义(P<0.05)。研究组胃肠道反应发生率16.00%低于对照组的34.00%,差异有统计学意义(P<0.05)。治疗后,研究组患者CEA(8.05±1.19)U/ml、SCC-Ag(1.86±0.41)ng/ml、CYFRA21-1(8.11±1.52)ng/ml、CA125(26.32±2.86)U/ml均低于对照组的(8.86±1.38)U/ml、(2.13±0.49)ng/ml、(9.03±1.85)ng/ml、(27.64±3.13)U/ml,差异有统计学意义(P<0.05)。研究组治疗后2年内的病情恶化率46.00%低于对照组的66.00%,生存率82.00%高于对照组的64.00%,恶化时间(1.15±0.54)年、死亡时间(1.48±0.46)年均晚于对照组的(0.91±0.52)、(1.23±0.45)年,差异有统计学意义(P<0.05)。结论 给予晚期宫颈癌患者白蛋白结合型紫杉醇联合顺铂治疗可增加病灶缓解程度,减少胃肠道反应,降低治疗后短期内的病情恶化发生率,同时提高患者生存能力。 展开更多

目的探讨白蛋白结合型紫杉醇联合卡铂治疗晚期卵巢癌的临床疗效及安全性,分析人附睾蛋白4(HE4)、糖类抗原(CA)125、淋巴细胞亚群检测的临床意义。方法回顾性分析2018年12月至2022年12月该院收治的晚期卵巢癌患者临床资料,对照组(29例)... 目的探讨白蛋白结合型紫杉醇联合卡铂治疗晚期卵巢癌的临床疗效及安全性,分析人附睾蛋白4(HE4)、糖类抗原(CA)125、淋巴细胞亚群检测的临床意义。方法回顾性分析2018年12月至2022年12月该院收治的晚期卵巢癌患者临床资料,对照组(29例)采用紫杉醇脂质体联合卡铂治疗,观察组(29例)采用白蛋白结合型紫杉醇联合卡铂治疗,两组均治疗6个疗程。比较两组客观缓解率(ORR)、疾病控制率(DCR)、不良反应及治疗前后血清HE4、CA125、T淋巴细胞亚群的变化。结果观察组ORR为79.31%,高于对照组的51.72%(χ^(2)=4.884,P=0.027)。观察组DCR为93.10%,高于对照组的79.31%(χ^(2)=4.062,P=0.044)。对照组各项不良反应发生率均高于观察组(P<0.05)。重复测量方差分析结果显示,两组HE4及CA125水平存在组间效应、时间效应和交互效应(P<0.001)。单因素重复测量方差分析结果显示,与治疗前比较,两组患者治疗2、4、6个周期后HE4、CA125水平均下降(P<0.008),多变量方差分析结果显示,观察组治疗2、4、6个周期后HE4、CA125水平均低于对照组(P<0.001)。两组治疗前CD3+、CD4^(+)、CD8^(+)T淋巴细胞比例及CD4^(+)T淋巴细胞/CD8^(+)T淋巴细胞比值比较,差异无统计学意义(P>0.05),治疗后两组CD3+、CD4^(+)淋巴细胞比例及CD4^(+)淋巴细胞/CD8^(+)淋巴细胞比值升高(P<0.05),观察组高于对照组(P<0.05),治疗后两组CD8^(+)T淋巴细胞比例下降,观察组低于对照组(P<0.05)。结论白蛋白结合型紫杉醇联合卡铂治疗晚期卵巢癌患者临床疗效更佳,安全性更好,且可明显降低血清HE4、CA125水平,促进淋巴细胞发挥免疫调节作用,改善机体免疫状态。 展开更多

目的研究血清三叶因子1(TFF1)、胸苷激酶1(TK1)与晚期乳腺浸润性小叶癌(ILC)患者白蛋白结合型紫杉醇化疗疗效和预后的关系。方法选取2018年4月至2020年7月邯郸市中心医院诊治的97例晚期ILC患者纳入ILC组,以同期我院诊治的70例女性乳腺... 目的研究血清三叶因子1(TFF1)、胸苷激酶1(TK1)与晚期乳腺浸润性小叶癌(ILC)患者白蛋白结合型紫杉醇化疗疗效和预后的关系。方法选取2018年4月至2020年7月邯郸市中心医院诊治的97例晚期ILC患者纳入ILC组,以同期我院诊治的70例女性乳腺良性疾病患者纳入良性对照组,70例健康体检的女性志愿者纳入健康对照组。检测并比较3组研究对象血清TFF1、TK1水平。比较不同临床病理特征ILC患者血清TFF1、TK1水平差异;多因素logistic回归分析影响化疗疗效的因素;Kaplan-Meier曲线分析不同血清TFF1、TK1表达对晚期ILC患者预后的影响。结果ILC组患者血清TFF1、TK1水平高于良性对照组及健康对照组,差异有统计学意义(P<0.05)。组织学分级Ⅲ级、肿瘤最大径>4 cm的ILC患者血清TFF1、TK1水平高于组织学分级Ⅰ~Ⅱ级、肿瘤最大径≤4 cm患者,差异具有统计学意义(P<0.05)。根据化疗后疗效分为有效组51例,无效组46例。无效组患者组织学分级Ⅲ级比例、血清TFF1、TK1水平高于有效组,差异有统计学意义(P<0.05),组织学分级Ⅲ、血清TFF1、TK1水平是影响ILC患者化疗疗效的独立危险因素(OR=1.885、1.716、1.205;P<0.001、0.017、0.011)。TFF1高表达组和低表达组患者的3年生存率分别为44.19%(19/43),72.22%(39/54);TK1高表达组和低表达组患者的3年生存率分别为43.48%(20/46),74.51%(38/51)。TFF1、TK1高表达组患者3年累积生存率分别低于TFF1、TK1低表达组,差异具有统计学意义(P<0.05)。结论晚期ILC患者血清TFF1、TK1水平升高,两者水平升高是影响白蛋白结合型紫杉醇化疗疗效的危险因素,且与晚期ILC患者预后密切相关。 展开更多

目的:探讨注射用紫杉醇(白蛋白结合型)在苏州市中医医院用于恶性肿瘤的临床使用合理性,为临床合理用药提供参考。方法:采用回顾性调查分析,通过医院信息系统汇总2022年1月1日至2024年1月1日苏州市中医医院使用注射用紫杉醇(白蛋白结合型... 目的:探讨注射用紫杉醇(白蛋白结合型)在苏州市中医医院用于恶性肿瘤的临床使用合理性,为临床合理用药提供参考。方法:采用回顾性调查分析,通过医院信息系统汇总2022年1月1日至2024年1月1日苏州市中医医院使用注射用紫杉醇(白蛋白结合型)患者的用药特征信息,评价用药合理性。结果:共106例患者使用注射用紫杉醇(白蛋白结合型)840例次,396例次存在不合理,不合理率为47.14%,主要表现为无循证医学证据的超适应证用药(98例次,发生率为11.67%)、用法与用量不合理(207例次,发生率为24.64%)、联合用药不合理(2例次,发生率为0.24%)及止吐预处理不合理(346例次,发生率为41.19%);常见的不良反应为疲劳乏力、血液毒性、周围神经毒性等,发生率分别为66.67%(560例次)、45.60%(383例次)、12.14%(102例次)。结论:注射用紫杉醇(白蛋白结合型)被广泛用于多种恶性肿瘤的治疗,临床应用不合理现象较普遍,需要引起临床重视,药师需做好药学监护,保障用药安全。 展开更多
