Method: The present study analyzed all 103 patients with head a...Background: With the recent aging of society, the need for medical treatment of elderly patients with head and neck cancer seems to have been increasing. Method: The present study analyzed all 103 patients with head and neck cancer ≥80 years, and we compared results with those of the previous generation (Group P;range: 75 - 79 years) comprising 104 patients treated in the same period. Results: We provided treatment just as wanted and could not choose it often. The reasons were oncological factors such as unresectable tumor or distant metastasis, refusal of treatment, and physical factors such as poor PS or number of comorbidities. Conclusion: Treatment choices should be based on the wishes and motivations of the patient and the medical assessment of physical function. When a patient ≥80 years old is treated, the high incidence of complications and severity of the disease should be considered.展开更多

METHODS:Three models of colorectal cancer prevention were examined:standard screening,active check-up of suspected cases and summons ...AIM:To explore the problems in the screening of colorecta carcinoma in the elderly. METHODS:Three models of colorectal cancer prevention were examined:standard screening,active check-up of suspected cases and summons to have endoscopic check- up for previously diagnosed colorectal polyps.The study was performed among three groups of elderly individuals: Group 1 (167 cases),hospitalized asymptomatic individuals without symptoms in large intestines.Group 2 (612 cases): old individuals at home for the aged,out of which 32 showed symptoms of colon disorders;Group 3 (44 cases):elderly people with diagnosed polyps.As a result of 1788 rectosigmoidoscopies,we identified 61 individuals with polyps,out of which 44 patients were over 65 years old. However,only 9 of these 44 individuals agreed to have the endoscopy performed again. RESULTS:One cancer and 13 polyps were detected in Group 1,and two polyps in Group 2.However,it should be noted that only eleven individuals from Group 2 agreed to have the endoscopy.In Group 3,there were no relapses of the polyps among the nine individuals who came back for the endoscopy. CONCLUSION:Poor understanding of the screening procedures is one of the greatest problems in early detection of the cancer in the aged.Paradoxically,the cooperation is better with hospitalized patients,than with 'successfully old' persons.展开更多
The aim was to explore the implementation of individual care plans in municipal elderly care in relation to characteristics of staff. Data regarding characteristics of staff were derived through a questionnaire distri...The aim was to explore the implementation of individual care plans in municipal elderly care in relation to characteristics of staff. Data regarding characteristics of staff were derived through a questionnaire distributed to all staff working in the care for older people, (N = 908, n = 245) in four municipalities in Sweden. The number of care plans established during a one-year period was collected through a contact person in each municipality. In total 47 individual care plans were established during the study year. Significantly more staff in the municipality that had the most number of established individual care plans agreed that there had been sufficient education (p = 0.017), sufficient time (p = 0.002) and routines established regarding individual care plans (p = 0.014) and had a significantly better job satisfaction (p = 0.001), compared to staff in the other municipalities. Implementation leaders may need to take the working conditions and the perception of available resources among staff into consideration in the on-going process of implementing individual care plans.展开更多

文摘目的观察地佐辛应用于超高龄髋关节手术患者术后静脉镇痛的安全性和有效性。方法选取拟行髋关节骨折内固定手术的80岁以上超高龄患者120例,美国麻醉医师协会(ASA)分级Ⅰ-Ⅲ级,随机均分为2组。D组手术结束前给予地佐辛5 mg,术后患者自控镇痛(PCA)镇痛液含地佐辛0.5 mg/kg。S组手术结束前给予舒芬太尼5μg,术后PCA镇痛液含舒芬太尼1.5μg/kg。分别记录术后疼痛视觉模拟评分法(VAS)评分、Ramsay镇静评分,术后镇痛期间恶心呕吐、眩晕、呼吸抑制、嗜睡等不良反应发生率。结果两组镇痛效果满意,除术后24 h D组VAS评分低于S组外,其余各时间点两组VAS评分无显著差异;各时间点两组Ramsay镇静评分无显著差异;D组恶心发生率低于S组,两组呕吐、嗜睡、眩晕、皮肤瘙痒的发生率均无显著差异。结论地佐辛、舒芬太尼经静脉患者自控镇痛(PCIA)法用于超高龄髋关节手术术后镇痛均可获得满意镇痛效果,但地佐辛不良反应发生率低于舒芬太尼,是一种较好的针对超高龄老年髋关节手术患者术后镇痛的方法。
