The aim was to find out to whom a patient with kidney disease should be referred in the first instance and for what reason? Hence the evaluation of the prevalence of the first consultation in the nephrology department of the Point G University Hospital in Bamako was made. Patients and Method: This was a prospective descriptive study which took place from July 2017 to June 2018 in the nephrology department of the Point G University Hospital. The study included all patients who came to the nephrology department for the first time during this period. Results: Between July 2017 and June 2018 at the nephrology and haemodialysis service of the G point University Hospital, we retained 643 out of 1031 patients who came for their first consultation, i.e. 62.36%. There were many patients aged between 41 and 50 years. The average age was 42.6 ± 5.03 years with extremes of 30 and 82 years. The sex ratio (M/F) was in favor of males, 1.14. The majority of patients were referred/evacuated from the referral health centres (57.6%). The majority of patients were referred/evacuated by general practitioners (70.5%). Cardiologists were the main specialists to refer/evacuate patients (21.2%). The main reasons for consultation that led to the first diagnostic hypotheses were: arterial hypertension for vascular nephropathy, glomerular syndrome for glomerular nephropathy, acute uraemia syndrome for acute renal failure, hydronephrosis for obstructive uropathy and finally diabetes for diabetic nephropathy. Conclusion: The first consultation in nephrology remains an important step in the management of renal disease, especially when the referral is an emergency. The percentage of referrals is dominated by general practitioners.展开更多

Methods: A total of 52 cases with slow transit constipation in conformity with the inclusion criteria were selected and divided into a control group and an observation group according to their visit order and random digital table, 26 cases in each group. Patients in the control group received routine nursing guide. Besides the same routine nursing guide, patients in the observation group received moxibustion at the abdominal acupoints, once every day. The course of the treatment was 4 weeks in the two groups, and the 3-month follow-up was given after the course was finished, for comparing the clinical symptoms, results of colon transit tests, scores of depression/anxiety scale and nursing satisfaction. Results: The total effective rate was 92.3% in the observation group and 69.2% in the control group, with a significant difference between the two groups. After the treatment and during the follow-up checks, the scores of Chinese medical symptoms in the two groups were remarkably decreased than those before the treatment(all P〈0.01); the scores of the observation group were obviously lower than those in the control group(all P〈0.01). The discharge rates of the markers in the two groups were remarkably increased than those at the same time period before the treatment; moreover, the discharge rates of the markers at various time periods were remarkably better in the observation group than those in the control group(P〈0.01). SDS and SAS scores were remarkably decreased after the treatment in the two groups(P〈0.01). In comparison of SDS score between the two groups after the treatment, the difference was statistically significant(P〈0.01). In comparison of SAS score between the two groups after the treatment, the difference was statistically significant(P〈0.05). The nursing satisfaction was 96.2% in the observation group after the treatment, obviously better than that in the control group(73.1%). The recurrence rate was 8.3% in the observation group, remarkably lower than that in the control group(33.3%). Conclusion: Moxibustion at the abdominal acupoints plus routine nursing can remarkably improve the colon transit functions and anxious and depressive emotion in patients with STC, and the therapeutic effects are remarkable. Not only the clinical satisfaction is higher, but the recurrence rate is obviously lower than that of routine nursing.展开更多

Outcome measures were the NLR and the scores from the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30). The NLR and the EORTC QLQ-C30 were assessed at baseline and at the end of 6 weeks.RESULTS: Five participants dropped out, leaving afinal total of 65 participants who completed the trial. Groups A and B had a similar mean NLR at baseline. After the treatment course, the NLR in group A was significantly lower than that in group B(P <0.001). Compared with group B, the EORTC QLQ-C30 scores in group A were significantly greater in terms of global health status or quality of life(P < 0.001) and function(P < 0.05), and significantly lower in terms of symptoms(P < 0.05).CONCLUSION: The present study suggests that performing scar-producing moxibustion by heat-stimulating the acupoints Zusanli(ST 36) and Feishu(BL13) effectively decreases the NLR and improves the quality of life in patients with NSCLC.展开更多

Methods: It analyzed the occurrence rules of acupoint heat-sensitization and its relationship to the moxibustion location and dose, in view of the original definition of acupoints in Nei Jing (Internal Canon) and the acupoint heat-sensitization in clinical practice. Results: (1) The original definition of acupoint is the reflecting area of a disease on surface of the body, which is individualized, motive and sensitive; (2) The location in which heat-sensitization is generated relating to a heat-sensitive acupoint, which therefore depicts the best choice for point selection and also the most accurate area to give moxibustion; (3) The heat-sensitization time can be taken as a clinical index to quantify the moxibustion dose, in order to apply sufficient moxibustion for each individual. Conclusion: The key points for enhancing the curative effect of moxibustion therapy are to identify heat-sensitive points and a scientific moxibustion dose.展开更多

Methods: A total of 65 cases were randomized into a Zusanli (ST 36) group (n=33) and a Yanglingquan (GB 34) group (n=32) by random serial number, to receive moxibustion therapy to the corresponding point respectively. The 8-lead EGG was used before and after treatment to collect EGG parameter of the gastric body, antrum of stomach, lesser curvature and greater curvature, including the dominant frequency, slow wave percentage, average frequency, response area and motility index. Results: After treatment, the dominant frequency and slow wave percentage were improved in nearly the whole stomach area in Zusanli (ST 36) group, showing statistical significances (both P〈0.05); the slow wave percentage and average frequency were improved in stomach body and antrum of stomach in Yanglingquan (GB 34) group, showing statistical significances (both P〈0.05); moxibustion at Zusanli (ST 36) had a significantly higher influence on EGG than Yanglingquan (GB 34), in which the lifting of slow wave percentage was significantly higher than Yanglingquan (GB 34) group (P〈0.05). Conclusion: Moxibustion at Zusanli (ST 36) and Yanglingquan (GB 34) has the effect of adjusting abnormal EGG pattern in FD patients. Compared with Yanglingquan (GB 34), Zusanli (ST 36) has a superior effect in improving EGG in stomach area.展开更多

文摘Both therapies of traditional crude herb moxibustion and drug acupoint application were used in 209 cases of allergic asthma to compare their long-term and short-term therapeutic effects and to analyze the relationship between clinic therapeutic effects of both therapies and differential types of the disease. The results showed that the short-term total effective rate in the group of drug acupoint application was higher than that in the group of traditional crude herb moxibustion, the therapeutic effects of drug acupoint application being closely related to differential types. Analysis also shows the best short-term therapeutic effect was in the type of heat in the lung while the poorest effect in the type of deficiency of the kidney-yang.
文摘Understanding the first consultation of people with kidney disease seems to us to be essential to understand the attitude of the referring physician and the nephrologist. The percentage of referrals is dominated by general practitioners.

文摘Objective: To observe the clinical effects of moxibustion at abdominal acupoints for slow transit constipation(STC) due to yang deficiency of the spleen and kidney. Not only the clinical satisfaction is higher, but the recurrence rate is obviously lower than that of routine nursing.

文摘Objective: To elucidate the key effects of moxibustion points and quantity of moxibustion used in order to enhance the curative effect of moxibustion therapy. Conclusion: The key points for enhancing the curative effect of moxibustion therapy are to identify heat-sensitive points and a scientific moxibustion dose.
