Polycystic ovarian syndrome(PCOS)as a relatively common clinical reproductive endocrine disease,the incidence is increasing year by year and the treatment is difficult,among which obese PCOS accounts for nearly 50%of the total.In recent years,non-drug therapy of traditional Chinese medicine(TCM)has been effective in the treatment of PCOS.This article summarizes the relevant literature in the past ten years from acupuncture,acupoint embedding,moxibustion,and auricular points,and observes the effects of TCM non-drug therapy on endocrine and glucose and lipid metabolism in obese PCOS patients.The impact of various indicators such as body mass index,body mass index,etc.,and discuss the problems in such treatments,as well as put forward suggestions to achieve the purpose of better guiding the clinical treatment of obese PCOS. 展开更多

The relationship of serum Myostatin content with lipid metabolism, insulin resistance and inflammatory response in patients with polycystic ovary syndrome

Objective:Objective:To investigate the relationship of serum Myostatin content with lipid metabolism,insulin resistance and inflammatory response in patients with polycystic ovary syndrome(PCOS).Methods:118 PCOS patients were taken as the PCOS group,and 100 healthy women of child-bearing age who underwent physical examination in our hospital during the same period were taken as the normal control group.The differences in serum Myostatin content,lipid metabolism index levels,insulin resistance index levels and inflammatory mediator contents were compared between the two groups,and the internal relationship between serum Myostatin content and the above disease-related index levels in PCOS patients was further analyzed.Results:Serum Myostatin content of PCOS group was higher than that of normal control group.Peripheral blood lipid metabolism indexes TC,LDL-C and ApoB levels of PCOS group were higher than those of normal control group whereas HDL-C and ApoA-Ⅰlevels were lower than those of normal control group;insulin resistance indexes FINS and HOMA-IR levels were higher than those of normal control group;inflammatory mediators IL-18,MCP-1 and CRP contents were higher than those of normal control group.Pearson test showed that serum Myostatin content in PCOS patients was directly correlated with the levels of the above disease-related indexes.Conclusion:Serum Myostatin content is abnormally high in PCOS patients,and the specific content is positively correlated with the disease severity. 展开更多

Effect of Bailing capsule combined with ethinylestradiol and cyproterone acetate and metformin on lipid metabolism and insulin resistance in patients with polycystic ovary syndrome

Objective:To investigate the effect of Bailing capsule combined with ethinylestradiol and cyproterone acetate and metformin on lipid metabolism and insulin resistance in patients with polycystic ovary syndrome (PCOS).Methods: A total of 108 patients with PCOS who were treated in our hospital between November 2014 and April 2017 were reviewed and divided into routine group (n=67) who received routine therapy and Bailing capsule group (n=41) who received Bailing capsule combined with routine therapy. Routine group received ethinylestradiol and cyproterone acetate and metformin therapy, and Bailing capsule group were treated with Bailing capsule combined with ethinylestradiol and cyproterone acetate and metformin therapy. The differences in the levels of lipid metabolism, insulin resistance indexes as well as inflammatory factors were compared between the two groups before and after treatment.Results: There was no statistically significant difference in serum levels of lipid metabolism, insulin resistance-related indexes and inflammatory factors between the two groups before treatment. After treatment, serum HDL-C content of Bailing capsule group was higher than that of routine group whereas LDL-C and TC contents were lower than those of routine group;serum insulin resistance-related indexes FINS and HOMA-IR levels were lower than those of routine group;serum inflammatory cytokines IL-6, TNF-α, APN and LEP contents were lower than those of routine group.Conclusion: Bailing capsule combined with ethinylestradiol and cyproterone acetate and metformin therapy can effectively improve the lipid metabolism level and inhibit the insulin resistance in patients with PCOS. 展开更多

Objective: To observe the therapeutic effect of combined traditional Chinese medicine (TCM) therapy with acupoint injection and herbal patch application on patients with ovulatory dysfunction associated with polycystic ovary syndrome (PCOS). Methods: A total of 80 PCOS patients who received treatment at the First Affiliated Hospital of Guangxi University of Chinese Medicine between October 2022 and March 2024 were enrolled in this study. They were randomly divided into a control group (n = 40) and an observation group (n = 40). The control group received conventional Western medical treatments, while the observation group was administered TCM combined with acupoint injection and herbal patch application. Pre- and post-treatment conditions were compared between the two groups. Results: After treatment, the overall effective rate was significantly higher in the observation group compared to the control group (P 0.05). Following treatment, both ovulation and pregnancy rates increased in both groups, but the increase was more evident in the observation group, showing a statistically significant difference (P < 0.01). Conclusion: For patients with oligoovulation or anovulation due to PCOS, the combined use of TCM, acupoint injection, and herbal patch application appears to be effective in improving systemic symptoms and achieving notable clinical outcomes. 展开更多

Objective:To systematically evaluate the effects of acupuncture,auricular point sticking,acupoint catgut embedding,cupping and other non-pharmaceutical Chinese medicine therapies on sex hormone as well as glucose and lipid metabolism in patients with obese polycystic ovary syndrome.Method:The databases such as China National Knowledge Infrastructure were searched by computer to collect the literature on the treatment of obese polycystic by non-pharmaceutical Chinese medicine therapy combined with western medicine or lifestyle change,and the literature quality of the included literature was evaluated,and finally the data were analyzed.A total of 15 randomized controlled trial(RCT)literature were included in this study,with a total of 1263 patients.Results:Non-pharmaceutical Chinese medicine therapy combined with metformin or lifestyle can reduce body mass index,insulin resistance index,fasting plasma insulin,fasting blood glucose,luteinizing hormone,ratio of luteinizing hormone to follicle stimulating hormone,testosterone,but it has no obvious advantage over follicle stimulating hormone(FSH).Conclusion:Compared with simple application of metformin or lifestyle change,traditional Chinese medicine non drug therapy combined with metformin or lifestyle change can better improve sex hormone and blood glucose metabolism in obese PCOS patients. 展开更多

Polycystic ovary syndrome(PCOS)is the most common endocrinopathy in women of reproductive age associated with long-term metabolic and cardiovascular consequences.A plethora of symptoms and their severity differentiate on an individual level,giving the syndrome numerous phenotypes.Due to menstrual cycle abnormalities,women suffer from irregular menstrual bleeding,difficulty in conception,and infertility.Furthermore,the risk of pregnancy complications such as gestational diabetes mellitus,hypertensive disorders of pregnancy,and preterm birth are higher in women with PCOS than in the general population.Often,women with PCOS have comorbidities such as dyslipidemia,obesity,glucose intolerance or diabetes type 2,non-alcoholic fatty liver disease,and metabolic syndrome,which all influence the treatment plan.Historic insulinsensitizing agents,although good for some of the metabolic derangements,do not offer long-term cardiovascular benefits;therefore,new treatment options are of paramount importance.Sodium-glucose co-transporter-2(SGLT-2)inhibitors,a new class of antidiabetic agents with beneficial cardiovascular,bodyweight,and antihyperglycemic effects,although not approved for the treatment of PCOS,might be an attractive therapeutic addition in the PCOS armamentarium.Namely,recent studies with SGLT-2 inhibitors showed promising improvements in anthropometric parameters and body composition in patients with PCOS.It is important to further explore the SGLT-2 inhibitors potential as an early therapeutic option because of the PCOS-related risk of metabolic,reproductive,and psychological consequences. 展开更多

BACKGROUND: Patients with hypertension coupled with metabolic syndrome (MetS) are among the high risk population in cardiovascular and cerebrovascular diseases. To reduce the prevalence of cardiovascular and cerebrovascular diseases, it is essential to appropriately control b^ood pressure together with other cardiovascular risk factors. OBJECTIVE: The current study was designed to investigate the therapeutic effects on blood pressure, blood pressure variability and other cardiovascular risk factors by giving Yiqi Huaju Formula, a compound traditional Chinese herbal medicine, in addition to routine treatment to hypertensive patients coupled with MetS. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS: A total of 43 patients with hypertension coupled with MetS were recruited into this study. The enrolled patients were randomly divided into the Chinese herbal formula group (anti-hypertensive drugs plus Yiqi Huaju Formula, CHF) and the control group (anti-hypertensive drugs plus placebo). The CHF group enrolled 22 patients while the control group received 21 cases. Treatments were given for 12 weeks in both groups. MAIN OUTCOME MEASURES: Parameters examined include 24-hour ambulatory blood pressure monitoring, body mass index, waist circumference, waist-to-hip ratio, homeostatic model assessment for insulin resistance (HOMA-IR), fasting glycosylated hemoglobin (HbAlc), fasting plasma glucose, 2-hour postprandial plasma glucose (PPG), fasting plasma insulin, serum lipid, etc. RESULTS: Compared with the control group, the CHF group had significant improvement (P〈0.01) in anthropometric parameters, FPG, HOMA-IR, blood pressure amplitude, blood pressure variability and blood pressure load. CONCLUSION: This study showed that integrated traditional Chinese and Western medicine treatment can achieve better results in controlling blood pressure as well as other cardiovascular risk factors. The mechanism of controlling of blood pressure may be associated with the improvement of insulin sensitivity due to the Yiqi Huaju intervention. Objective:Polycystic ovarian syndrome(PCOS)is a common endocrine disorder affecting women of reproductive age.This study aimed to use text mining and microarray data analysis to identify drugs that target genes and potential pathways associated with PCOS.Methods:We extracted a common set of genes associated with PCOS using text mining and the microarray dataset GSE48301.Next,we performed Gene Ontology and Kyoto Encyclopedia of Genes and Genomes analyses of these genes,as well as protein–protein interaction(PPI)network analysis.Additionally,we used MCODE and cytoHubba to cluster significant common genes in the PPI network and performed gene–drug interaction analyses to identify potential drugs for further investigation.Finally,we annotated pathways associated with the genes identified.Results:Text mining and microarray analysis yielded 696 text mining genes(TMGs)and 2804 differentially expressed genes(DEGs).Among these,a set of 77 genes was found in both TMGs and DEGs.Interestingly,67 of these genes participated in constructing the PPI network.Seven common hub genes were selected using the MCODE and CytoHubba methods.Finally,five out of seven genes were targeted by 15 existing drugs.Conclusion:Four genes(FASLG,IL13,IL17A,andIL2RA),which are mainly related to the cytokine–cytokine receptor interaction pathway,could be prioritized as targets for PCOS. 展开更多

Over half of the daughters born to women with PCOS may eventually develop their own PCOS-related symptoms. Progress in the treatment of PCOS is currently hindered by the complexity of its clinical manifestations and incomplete knowledge of its etiopathogenesis. Various animal models, including experimentally induced, naturally occurring, and spontaneously arising ones, have been established to emulate a wide range of phenotypical and pathological traits of human PCOS. These studies have led to a paradigm shift in understanding the genetic, developmental, and evolutionary origins of this disorder. Furthermore, emerging evidence suggests that animal models are useful in evaluating state-of-the-art drugs and treatments for PCOS. This review aims to provide a comprehensive summary of recent studies of PCOS in animal models, highlighting the power of these disease models in understanding the biology of PCOS and aiding high-throughput approaches. 展开更多

Background:While combined oral contraceptives (COCs) are commonly used to treat polycystic ovary syndrome (PCOS),comparative data regarding metabolic effects of different progestogens on this patient population are missing.This study aimed to compare the different effects of drospirenone (DRP)-containing COCs with cyproterone acetate (CPA)-containing COCs,combined with metformin and lifestyle modifications in women with PCOS and metabolic disorders.Methods:Ninety-nine women with PCOS and a metabolic disorder between January 2011 and January 2013 were enrolled into this prospective randomized clinical trial.Participants were randomized into two groups such as DRP-containing COCs,and CPA-containing COCs.Participants took COCs cyclically for 6 months,combined with metformin administration (1.5 g/d) and lifestyle modifications (diet and exercise).Clinical measures and biochemical and hormone profiles were compared.Comparisons for continuous variables were evaluated with paired and unpaired Student&#39;s t-tests.The Wilcoxon signed rank test was used when the data were not normally distributed.Analysis of covariance was used to control for age,body mass index (BMI),and baseline data of each analyzed parameter when compared between the two groups.Results:A total of 68 patients have completed the study.The combination regimen of COCs,metformin,and lifestyle modifications in these patients resulted in a significant decrease in BMI,acne,and hirsutism scores when compared to baseline levels in both groups (P 〈 0.05).Blood pressure (BP) was significantly different in the CPA group when compared to baseline (75.14 ± 6.77 mmHg vs.80.70 ± 5.60 mmHg,P 〈 0.01),and after 6 months of treatment,only the change in systolic BP was significantly different between the two groups (4.00 [-6.00,13.00] mmHg vs.-3.50 [-13.00,9.00] mmHg,P =0.009).Fasting glucose,fasting insulin,and homeostasis model assessment-insulin resistance decreased significantly in the DRP group (5.40 ± 0.41 mmol/L vs.5.21 ± 0.32 mmol/L,P =0.041;13.90 [10.50,18.40] μU/ml vs.10.75 [8.60,13.50] μU/ml,P =0.020;3.74 [2.85,4.23] vs.2.55 [1.92,3.40],P =0.008) but did not differ between the two groups.While individual lipid profiles increased in both groups,no statistically significant difference was observed.Conclusions:DRP-containing COCs combined with metformin and lifestyle modifications could better control BP and correct carbohydrate metabolism in women with PCOS and metabolic disorders compared with CPA-containing COCs. 展开更多

OBJECTIVE:To identify active compounds in an Yinyanghuo(Herba Epimedii Brevicornus)-Xianmao(Rhizoma Curculiginis)drug pair(ECD)and investigate its efficacy on polycystic ovary syndrome(PCOS),and its possible mechanism in a rat model of PCOS.METHODS:A network pharmacology approach involving a characteristic drug assessment,active compound and target prediction,PCOS gene collection as well as network analysis was employed.The ovary morphology after treatment was observed using an animal model and western blotting and real-time PCR were used to verify AKT1 as the molecular target.RESULTS:Six networks were constructed,an active compound-target network for the ECD(C-T network),a drug-target network(D-T network),a related genes network,a targets interaction network,a key genes interaction network,and a gene-pathway network.A total of 41 compounds and 261 targets were identified for the ECD,232 PCOS-related genes,31 cogenes,and 14 pathways.These pathways may be involved in the efficacy of ECD on PCOS.The proteins most involved in the signal pathways for all targets were AKT1,IL6,INSR,ESR,and GSK3B.The AKT1 target was selected for experimental verification.Based on the Western blot and real-time PCR results,the expression of AKT1 in the PCOS model varied after treatment with ECD.CONCLUSIONS:Our findings suggest that the ECD can reverse the negative morphological changes in ovarian tissue that occur in model rats of PCOS.AKT1 may be a key mediator of the observed ability of the ECD to protect against PCOS in the model rats. 展开更多

Liquiritincan regulate bile acid metabolism and anti-inflammatory properties;however, limited information is available regarding its therapeutic potential in PCOS.Methods::Female C57BL/6 mice were randomly assigned into four groups ( n = 6 mice/group): the control, letrozole or dehydroepiandrosterone-induced PCOS groups, PCOS + 20 mg/kg liquiritin group, and control + liquiritin groups. After 21 days of treatment, the mice were euthanized, and the associated metabolism indications were investigated. Ovarian histological examinations were performed, and serum hormone concentration was measured. The expression of key genes involved in steroid hormone synthesis, ovarian follicle development, and ovulation was assessed. Results::Liquiritin reduced fasting blood glucose levels and increased insulin sensitivity compared to the PCOS group. Liquiritin also significantly decreased serum levels of total testosterone ( P < 0.001) and dehydroepiandrosterone sulfate ( P < 0.05) in the PCOS group. Histomorphological inspection of ovaries from the liquiritin group revealed fewer cystic dilated follicles than in the PCOS group. Moreover, liquiritinsignificantly ( P < 0.01) decreased Cyp17a1, Cyp19a1, Fshr, Hsd3b2, Runx2, and Ccn2 mRNA expression compared to letrozole-induced PCOS. Conclusion::Liquiritin may be safe and helpful in ameliorating PCOS-associated hyperandrogenemia and hyperglycemia. However, clinical trials investigating different liquiritin dosages are needed to confirm these findings. 展开更多

Objective:To investigate the application of nuclear magnetic resonance(NMR)spectroscopy in the exploration of metabolic differences and potential biomarkers in rats with polycystic ovary syndrome(PCOS).Methods:PCOS model was established by subcutaneous injection of deoxy-epiandrosterone(DHEA)in 30 rats randomly selected from sixty 23-day-old SPF female SD rats and set as PCOS group,and the other 30 rats were subcutaneously injected with oil and set as oil group,PCOS model was successfully established by 20 days of injection in both groups,and blood samples were taken for biochemical detection and NMR detection analysis.At the same time,the body weight,ovarian weight and HE staining results of rats in both groups were observed.Results:Compared with the oil group,the rats in the PCOS group had higher body weight,ovarian weight and serum LH,FPG,T,E_(2)and HOMA levels,and lower serum FSH and INS levels(P<0.05).In the oil group,a large number of corpus luteum,follicles at different stages of development and multi-layered granulosa cells were observed under microscope after HE staining;In PCOS group,corpus luteum formation was only visible in some ovaries after HE staining,and all of them showed a higher number of cystic dilated follicles,thickened follicular membrane layer and less granulosa cell layer.The PCA score map of serum NMR profiles of the rats in the two groups showed that the samples were separated between the PCOS group and the oil group but the samples were aggregated within the group,and there were differences in blood metabolism between the PCOS model constructed by surface DHEA and the rats in the oil group.A total of 70 metabolites were detected in the serum samples of rats in the two groups by NMR technique,and 31 of them were different between the two groups,which can be classified into glucose,lipid,and and amino acid.There were significant differences in three metabolites,including isoleucine,valine,and creatine(P<0.01).Conclusion:There are differences in carbohydrate,lipid and amino acid metabolism between normal rats and PCOS rats,among which the levels of serum isoleucine,valine and creatine can be used as important biomarkers in the diagnosis of PCOS. 展开更多

In recent years, acupuncture has been applied to treat more and more patients suffering from polycystic ovary syndrome(PCOS). On June 27, 2017 a paper titled 'Effect of acupuncture and clomiphene in Chinese women with polycystic ovary syndrome: a randomized clinical trial' was published in Journal of the American Medical Association(JAMA). The results did not support acupuncture as an effective therapy for improving the live births in PCOS. The aim of this article was to analyze the therapeutic regimen and design of the clinical trial. The primary outcome and the conclusion of the paper were discussed as well. There are 5 doubts in the paper.(1) The therapeutic regimen decided by trigger points in the paper was different from that of the traditional Chinese acupuncture.(2) The live birth as the primary outcome was limited during the PCOS recovery.(3) In addition the period of the treatment was too short to reflect the therapeutic effect.(4) Although there was no significant difference between active acupuncture and control acupuncture, the conclusion that acupuncture was not effective for PCOS was not confirmed because the control acupuncture plus clomiphene was also more effective than that of clomiphene alone.(5) The proposal and the statistics were not in accordance with the one planned before. Therefore the result was worth to be discussed so as to provide some suggestions and references for improving the clinical research of acupuncture. 展开更多

The present paper is a brief retrospection on the progress in treatment of PCOS caused infertility with integrative Chinese and Western medicine (ICWM). It can be seen from these materials that using traditional Chinese medicine (TCM) recipes formulated by Shen-replenishing herbs or acupuncture to reinforce Gan-Shen, regulate Chong-Ren Channels in treating PCOS, stable clinical efficacy could be obtained, with less adverse reaction, though the effect initiated somewhat late. Whereas, when Shen-replenishing recipe and acupuncture are combined with hormone or ovulation promoting drugs of Western medicine, the above-mentioned shortcomings would be overcome. Objective:To explore the effects and related mechanism of heat-sensitive moxibustion plus clomifene citrate capsules for infertility due to polycystic ovary syndrome(PCOS).Methods:A total of 70 patients were randomized into a control group and an observation group by the random number table method,with 35 cases in each group.The control group was treated with clomifene citrate capsules,while the observation group was treated with heat-sensitive moxibustion on the basis of the medications in the control group.The treatment course lasted for 6 menstrual cycles.The endometrial thickness and ovarian volume of the patients were measured before and after treatment.The levels of serum sex hormones,tumor necrosis factor-α(TNF-α)and nuclear factor-κB(NF-κB)were detected.Follow-up was performed for 1 year after treatment,and the pregnancy rate was recorded.Results:After treatment,the endometrium of the patients in both groups was significantly thickened,the ovarian volume was significantly reduced,and the intra-group differences were statistically significant(all P<0.05).The endometrium thickness and ovarian volume in the observation group were significantly different from those in the control group(both P<0.05).After treatment,the levels of serum testostrn(T)and luteinizing hormone(LH)in both groups decreased significantly,the level of estradiol(E2)increased obviously,and the intra-group differences were statistically significant(all P<0.05).The levels of serum T and LH in the observation group were lower than those in the control group,and the E2 level in the observation group was higher than that in the control group,and the between-group differences were statistically significant(all P<0.05).After treatment,the levels of serum TNF-αand NF-κB of the patients in both groups decreased,and the intra-group differences were statistically significant(all P<0.05).The levels of serum TNF-αand NF-κB in the observation group were significantly lower than those in the control group(both P<0.05).After 1-year follow-up,the pregnancy rate was 20.0%in the control group,versus 51.5%in the observation group.The pregnancy rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The treatment of heat-sensitive moxibustion plus clomifene citrate capsules can increase the endometrium thickness,reduce the ovarian volume,improve the status of serum sex hormone disorders,and increase the conception rate in patients with infertility due to PCOS,which may be related to the adjustment of the levels of immune inflammatory factors such as serum TNF-αand NF-κB. 展开更多

Objective: To investigate the action mechanism of acupuncture for non-insulin resistant polycystic ovary syndrome (PCOS). Methods: A total of 46 cases with non-insulin resistant PCOS were randomly allocated into an observation group of 23 cases and a control group of 23 cases. Cases in the observation group were treated by needling bilateral Weiwanxiashu (Ex-B 3), whereas those in the control group were treated by needling bilateral Sanyinjiao (SP 6). After 3-month treatments, a comparison was made on the changes of ovulations rates, Follicle-stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T), fasting insulin and body mass index (BMI). Results: There were statistical differences in ovulation rates and LH/FSH before and after the treatment in the observation group (P〈0.05), along with a statistical difference in testosterone changes (P〈0.01). There were also statistical differences in LH and LH/FSH before and after the treatment in the control group (P〉0.05); however, there were no statistical differences in ovulation rates and testosterone changes (P〈0.05). There was significant difference in testosterone changes between the two groups after the treatment (P〈0.01), along with a statistical difference in ovulation rates between the two groups (P〈0.05). However, there were no statistical differences in LH/FSH and BMI between the two groups. Conclusion: Needling Weiwanxiashu (Ex-B 3) and Sanyinjiao (SP 6) can both improve the endocrine disorder of patients with non-insulin resistant PCOS; however, needling Weiwanxiashu (Ex-B 3) can obtain a better result than Sanyinjiao (SP 6) in increasing the insulin sensitivity and preventing insulin resistance. 展开更多
