BACKGROUND Preoperative anxiety is a common emotional problem during the perioperative period and may adversely affect postoperative recovery.Emergence agitation(EA)is a common complication of general anesthesia that ...BACKGROUND Preoperative anxiety is a common emotional problem during the perioperative period and may adversely affect postoperative recovery.Emergence agitation(EA)is a common complication of general anesthesia that may increase patient discomfort and hospital stay and may be associated with the development of postoperative complications.Pre-anesthetic anxiety may be associated with the development of EA,but studies in this area are lacking.AIM To determine the relationship between pre-anesthetic anxiety and EA after radical surgery in patients with non-small cell lung cancer(NSCLC).METHODS Eighty patients with NSCLC undergoing surgical treatment between June 2020 and June 2023 were conveniently sampled.We used the Hospital Anxiety and Depression Scale’s(HADS)anxiety subscale(HADS-A)to determine patients’anxiety at four time points(T1-T4):Patients’preoperative visit,waiting period in the surgical waiting room,after entering the operating room,and before anesthesia induction,respectively.The Riker Sedation-Agitation Scale(RSAS)examined EA after surgery.Scatter plots of HADS-A and RSAS scores assessed the correlation between patients’pre-anesthesia anxiety status and EA.We performed a partial correlation analysis of HADS-A scores with RSAS scores.RESULTS NSCLC patients’HADS-A scores gradually increased at the four time points:7.33±2.03 at T1,7.99±2.22 at T2,8.05±2.81 at T3,and 8.36±4.17 at T4.The patients’postoperative RSAS score was 4.49±1.18,and 27 patients scored≥5,indicating that 33.75%patients had EA.HADS-A scores at T3 and T4 were significantly higher in patients with EA(9.67±3.02 vs 7.23±2.31,12.56±4.10 vs 6.23±2.05,P<0.001).Scatter plots showed the highest correlation between HADS-A and RSAS scores at T3 and T4.Partial correlation analysis showed a strong positive correlation between HADS-A and RSAS scores at T3 and T4(r=0.296,0.314,P<0.01).CONCLUSION Agitation during anesthesia recovery in patients undergoing radical resection for NSCLC correlated with anxiety at the time of entering the operating room and before anesthesia induction.展开更多
BACKGROUND:Shared decision-making(SDM)has broad application in emergencies.Most published studies have focused on SDM for a certain disease or expert opinions on future research gaps without revealing the full picture...BACKGROUND:Shared decision-making(SDM)has broad application in emergencies.Most published studies have focused on SDM for a certain disease or expert opinions on future research gaps without revealing the full picture or detailed guidance for clinical practice.This study is to investigate the optimal application of SDM to guide life-sustaining treatment(LST)in emergencies.METHODS:This study was a prospective two-round Delphi consensus-seeking survey among multiple stakeholders at the China Consortium of Elite Teaching Hospitals for Residency Education.Participants were identified based on their expertise in medicine,law,administration,medical education,or patient advocacy.All individual items and questions in the questionnaire were scored using a 5-point Likert scale,with responses ranging from"very unimportant"(a score of 1)to"extremely important"(a score of 5).The percentages of the responses that had scores of 4-5on the 5-point Likert scale were calculated.A Kendall’s W coefficient was calculated to evaluate the consensus of experts.RESULTS:A two-level framework consisting of 4 domains and 22 items as well as a ready-touse checklist for the informed consent process for LST was established.An acceptable Kendall’s W coefficient was achieved.CONCLUSION:A consensus-based framework supporting SDM during LST in an emergency department can inform the implementation of guidelines for clinical interventions,research studies,medical education,and policy initiatives.展开更多

Control of glyphosate-resistant giant ragweed is a challenge, particularly for soybean growers, because of limited effective post-emergence (POST) herbicide options. Many soybean growers in no-till production systems ...Control of glyphosate-resistant giant ragweed is a challenge, particularly for soybean growers, because of limited effective post-emergence (POST) herbicide options. Many soybean growers in no-till production systems use 2,4-D in burndown application for control of broadleaf weeds, including giant ragweed. Field experiments were conducted at David City, NE, in 2012 and 2013 to evaluate 2,4-D followed by PRE or POST herbicide programs for control of glyphosate-resistant giant ragweed in glyphosate-resistant soybean. Results suggested that burndown application of 2,4-D or saflufenacil plus imazethapyr resulted in 89 to 99% control of giant ragweed at 21 days after treatment. Burndown-only treatments of S-metolachlor plus metribuzin or sulfentrazone plus cloransulam resulted in poor control (≤65%) of giant ragweed and reduced soybean yield (≤ 577 kg·ha-1). Burndown application of 2,4-D followed by saflufenacil plus imazethapyr, S-metolachlor plus metribuzin, or sulfentrazone plus cloransulam applied pre-emergence (PRE) or cloransulam, chlorimuron, fomesafen, imazethapyr, or lactofen in tank-mixtures with acetochlor applied POST resulted in 87% to 99% giant ragweed control, reduced density to ≤7 plants m-2, and resulted in soybean yield from 2519 to 3823 kg·ha-1. There was no difference among and between 2,4-D followed by PRE or POST herbicides for giant ragweed control, density, or soybean yield, indicating all the two pass herbicide programs were effective. It is concluded that glyphosate-resistant giant ragweed can be effectively controlled in soybean by including 2,4-D in burndown program followed by PRE or POST herbicides tested in this study.展开更多
BACKGROUND For patients with acute paraplegia caused by spinal giant cell tumor(GCT)who require emergency decompressive surgery,there is still a lack of relevant reports on surgical options.This study is the first to ...BACKGROUND For patients with acute paraplegia caused by spinal giant cell tumor(GCT)who require emergency decompressive surgery,there is still a lack of relevant reports on surgical options.This study is the first to present the case of an acute paraplegic patient with a thoracic spinal GCT who underwent an emergency total en bloc spondylectomy(TES).Despite tumor recurrence,three-level TES was repeated after denosumab therapy.CASE SUMMARY A 27-year-old female patient who underwent single-level TES in an emergency presented with sudden severe back pain and acute paraplegia due to a thoracic spinal tumor.After emergency TES,the patient's spinal cord function recovered,and permanent paralysis was avoided.The postoperative histopathological examination revealed that the excised neoplasm was a rare GCT.Unfortunately,the tumor recurred 9 months after the first surgery.After 12 months of denosumab therapy,the tumor size was reduced,and tumor calcification.To prevent recurrent tumor progression and provide a possible cure,a three-level TES was performed again.The patient returned to an active lifestyle 1 month after the second surgery,and no recurrence of GCT was found at the last follow-up.CONCLUSION This patient with acute paraplegia underwent TES twice,including once in an emergency,and achieved good therapeutic results.TES in emergency surgery is feasible and safe when conditions permit;however,it may increase the risk of tumor recurrence.展开更多
Seeds were subjected to three different pre-sowing seed treatments: immersion in lukewarm water for 2 hours, immersion in concentrated sulfuric acid (H<sub>2</sub>SO<sub>4</sub>) for 5 minutes,...Seeds were subjected to three different pre-sowing seed treatments: immersion in lukewarm water for 2 hours, immersion in concentrated sulfuric acid (H<sub>2</sub>SO<sub>4</sub>) for 5 minutes, hilar removal, and a control in which the seeds were sown without being treated. The experiment was laid out in the Completely Randomized Design (CRD) with four replicates and 60 seeds per treatment. Seeds were sown in an improvised Seedbox in October 2019. Germination was monitored daily for one month. The results showed that Mamalis seeds treated with lukewarm water have the earliest germination of twelve days, with a germination percentage of 66.67%. The germination rate of another treatment ranges from 0 - 44 percent, compared to 45 percent for the control treatment. It seems prudent to conclude that to enhance the vegetative propagation methods is to soaking in warm water at 37.5˚C for 2 hours could provide the best growth.展开更多

The use of pre-emergence herbicides may be an alternative to manual weeding techniques, but so far, information on how they affect the seeds of native tree species is scarce. We established a greenhouse experiment to evaluate the effects of four pre-emergence herbicides (atrazine, diuron, isoxaflutole and oxyfluorfen) on weed suppression and seedling emergence and early growth of seven tropical forest tree species (Annona coriacea Mart., Citharexylum myrianthum Cham., Cordia ecalyculata Vell., Peltophorum dubium (Spreng.) Taub., Psidium guajava L., Pterogyne nitens Tul. and Schinus terebinthifolia Raddi). The experimental design was a randomized complete block design with five treatments and five replicates. The treatments consisted of a single dose of each pre-emergence herbicide and a control. Throughout the 60?days after sowing we evaluated weed cover and seedling emergence and early growth of tree species. Overall, our results suggest that all tested herbicides reduced weed cover; however, they also negatively affected tree species seedling emergence. Of the four herbicides tested, atrazine and diuron showed the greatest effects on tree seedling emergence, oxyfluorfen was least aggressive towards native species and isoxaflutole was most effective in weed control. Native tree species varied in their responses to herbicides, indicating that future experiments should increase the number of species tested as well as investigate how seed traits can affect the species responses to different herbicides.展开更多
BACKGROUND Antiviral treatment of patients with chronic hepatitis B(CHB)in the grey zone of treatment comands risk management in order to optimize the health outcome.In this sense,the identification of HBV mutants rel...BACKGROUND Antiviral treatment of patients with chronic hepatitis B(CHB)in the grey zone of treatment comands risk management in order to optimize the health outcome.In this sense,the identification of HBV mutants related with an increased risk of hepatocellular carcinoma(HCC)could be useful to identify subpopulations with potential indication of antiviral treatment.AIM To analyze the prevalence/persistence of hepatitis B virus(HBV)preS and basal core promoter(BCP)/precore/core variants associated to HCC development in CHB patients in the grey zone.METHODS Work was designed as a longitudinal retrospective study,including 106 plasma samples from 31 patients with CHB in the grey zone of treatment:Hepatitis B e antigen negative,HBV-DNA levels between 12-20000 IU/mL,normal or discordant transaminase levels during follow up and mild/moderate necroinflammatory activity in liver biopsy or Fibroscan(up to 9.5 kPa).Serum HBVDNA was tested using the Abbott Real Time HBV Assay and the BCP/precore/core and the hepatitis B surface antigen(HBsAg)coding regions were analyzed in positive samples by PCR/bulk-sequencing to identify the HCCrelated HBV mutants.RESULTS High-risk HCC related mutants were detected in 24(77%)patients:19(61%)in the BCP/precore/core,and 7(23%)in the HBsAg coding region(2 preS1 and 5 preS2 deletions).The prevalence of preS deletions was genotype-dependent:3/5(60%)patients with preS2 deletions and 1/2 with preS1 deletions were infected with the HBV-E genotype.Since HBV-E was the most prevalent in sub-Saharan patients,a correlation between preS deletions and ethnicity was also found:6/8(75%)sub-Saharan vs 1/19(5%)Caucasian patients had preS deletions(P=0.00016).Remarkably,this correlation was maintained in those patients infected with HBV-A,a minor genotype in sub-Saharan patients:2/2 patients infected with HBV-A from West Africa vs 0/6 of Caucasian origin had preS deletions.The HCC related variants were the major strains and persisted over time(up to 48 mo).Patients with preS deletions had a significant higher prevalence of F2 fibrosis stage than the negatives(57%vs 10%,P=0.0078).CONCLUSION HBV genetic analysis of selected populations,like sub-Saharans infected with HBV-E/A genotypes,will allow identification of subpopulations with risk of HCC development due to accumulation of high-risk HBV variants,thus commanding their increased clinical surveillance.展开更多
BACKGROUND Craniocerebral injuries encompass brain injuries,skull fractures,cranial soft tissue injuries,and similar injuries.Recently,the incidence of craniocerebral injuries has increased dramatically due to the inc...BACKGROUND Craniocerebral injuries encompass brain injuries,skull fractures,cranial soft tissue injuries,and similar injuries.Recently,the incidence of craniocerebral injuries has increased dramatically due to the increased numbers of traffic accidents and aerial work injuries,threatening the physical and mental health of patients.AIM To investigate the impact of failure modes and effects analysis(FMEA)-based emergency management on craniocerebral injury treatment effectiveness.METHODS Eighty-four patients with craniocerebral injuries,treated at our hospital from November 2019 to March 2021,were selected and assigned,using the random number table method,to study(n=42)and control(n=42)groups.Patients in the control group received conventional management while those in the study group received FMEA theory-based emergency management,based on the control group.Pre-and post-interventions,details regarding the emergency situation;levels of inflammatory stress indicators[Interleukin-6(IL-6),C-reactive protein(CRP),and procalcitonin(PCT)];incidence of complications;prognoses;and satisfaction regarding patient care were evaluated for both groups.RESULTS For the study group,the assessed parameters[pre-hospital emergency response time(9.13±2.37 min),time to receive a consultation(2.39±0.44 min),time needed to report imaging findings(1.15±4.44 min),and test reporting time(32.19±6.23 min)]were shorter than those for the control group(12.78±4.06 min,3.58±0.71 min,33.49±5.51 min,50.41±11.45 min,respectively;P<0.05).Pre-intervention serum levels of IL-6(78.71±27.59 pg/mL),CRP(19.80±6.77 mg/L),and PCT(3.66±1.82 ng/mL)in the study group patients were not significantly different from those in the control group patients(81.31±32.11 pg/mL,21.29±8.02 mg/L,and 3.95±2.11 ng/mL respectively;P>0.05);post-intervention serum indicator levels were lower in both groups than pre-intervention levels.Further,serum levels of IL-6(17.35±5.33 pg/mL),CRP(2.27±0.56 mg/L),and PCT(0.22±0.07 ng/mL)were lower in the study group than in the control group(30.15±12.38 pg/mL,3.13±0.77 mg/L,0.38±0.12 ng/mL,respectively;P<0.05).The complication rate observed in the study group(9.52%)was lower than that in the control group(26.19%,P<0.05).The prognoses for the study group patients were better than those for the control patients(P<0.05).Patient care satisfaction was higher in the study group(95.24%)than in the control group(78.57%,P<0.05).CONCLUSION FMEA-based craniocerebral injury management effectively shortens the time spent on emergency care,reduces inflammatory stress and complication risk levels,and helps improve patient prognoses,while achieving high patient care satisfaction levels.展开更多
Chemical eye burns present an avoidable,but frequent,occupational injury with potentially detrimental consequences for the quality of life and occupational rehabilitation of the injured.A periodical review of guidelin...Chemical eye burns present an avoidable,but frequent,occupational injury with potentially detrimental consequences for the quality of life and occupational rehabilitation of the injured.A periodical review of guidelines is required to assure the optimal emergency management.We reviewed the literature with emphasis on current German guidelines,primarily MEDLINE.If the crucial first-line measure,the injury prevention has failed and an eye burn has been sustained,the immediate and copious rinsing of the eye is the pivotal emergency treatment modality.Whereas the immediacy and sufficiency of the emergency rinsing are largely unanimous,there is an ongoing debate about the benefits and risks of specific rinsing solutions,and regular updates on guidelines and recommendations for the emergency treatment are warranted.The easiest and readily available rinsing solution is tap water,which fulfils the crucial criteria conveniently in most industrialized countries:purity,sterility,and neutral p H.Other rinsing solutions are proposing higher osmolality to stabilize the physiological p H,because of their superior buffering capacity.However,there is no compelling evidence for a substantial benefit,and some reports suggest that there could be unwanted side effects.In combination with the substantially increased expenditure and a more complex handling procedure,currently a general recommendation of any other solution than tap water is not warranted.展开更多

However, the conductivity of Ag NWs transparent electrode is still dramatically limited by the contact resistance between nanowires at high transmittance. Polyvinylpyrrolidone(PVP) layer adsorbed on the nanowire surface acts as an electrically insulating barrier at wire–wire junctions, and some devastating post-treatment methods are proposed to reduce or eliminate PVP layer, which usually limit the application of the substrates susceptible to heat or pressure and burden the fabrication with high-cost, time-consuming, or inefficient processes. In this work, a simple and rapid pre-treatment washing method was proposed to reduce the thickness of PVP layer from 13.19 to0.96 nm and improve the contact between wires. Ag NW electrodes with sheet resistances of 15.6 and 204 X sq-1have been achieved at transmittances of 90 and 97.5 %, respectively. This method avoided any post-treatments and popularized the application of high-performance Ag NW transparent electrode on more substrates. The improved Ag NWs were successfully employed in a capacitive pressure sensor with high transparency, sensitivity, and reproducibility.展开更多

Inoculated anaerobic granular biomass was acclimated for 225 d to the coking wastewater, and then the biochemical methane potential (BMP)of the coking wastewater in the acclimated granular biomass was measured. At the same time, some fundamental technological factors, such as the filling time and the reacting time ratio (tf/tr), the mixing intensity and the intermittent mixing mode, that affect anaerobic pretreatment of coking wastewater with ASBR, were evaluated through orthogonal tests. The COD removal efficiency reached 38%~50% in the stable operation period with the organic loading rate of 0.37~0.54 kg COD/(m3.d) at the optimum conditions of tf/tr, the mixing intensity and the intermittent mixing mode. In addition, the biodegradability of coking wastewater distinctly increased after the pretreatment using ASBR. At the end of the experiment, the microorganism forms on the granulated sludge in the ASBR were observed using SEM (scanning electron microscope) and fluoroscope. The results showed that the dominant microorganism on the granular sludge was Methanosaeta instead of Methanosarcina dominated on the inoculated sludge.展开更多
Seed morphology,germination and seedlings growth of Acacia auriculiformis were studied.The experiment was conducted in the nursery of Forestry and Wood Technology Discipline,Khulna University,Bangladesh.Matured seeds ...Seed morphology,germination and seedlings growth of Acacia auriculiformis were studied.The experiment was conducted in the nursery of Forestry and Wood Technology Discipline,Khulna University,Bangladesh.Matured seeds of the species were collected from healthy trees of road side plantation from different areas of Khulna District,Bangladesh and treated with five pre-sowing treatments(control,immersion in cold water,immersion in hot water,scarification with sand paper and immersion in concentrated H2SO4).The average length,breadth and thickness were found to be(0.58±0.017) cm,(0.44±0.007) cm and(0.20±0.089) cm,respectively.Germination was conducted in polybags with a mixture of top soil,coconut husk,coarse sand,and fine sand in a ratio of 3:4:1:1.Results reveal that pre-sowing treatments influences the germination rates of seeds that significantly increase the percentage germination compared with those in control(43%) and cold water treatment(52%).The highest germination success rate was found 83% in hot water treatment followed by 78% in scarification with sand paper,and 75% with immersion in H2SO4.Germination started from 7 to 12 days and completed between 28 and 35 days period in all treatments.ANOVA showed the significant difference(p0.05) among the treatments in seed germination,but no significant difference among treatment with regard to starting day,closing day and total germination period.In case of height and diameter growth,seedlings originated from the seeds with hot water treatment shows significantly higher in wet season(from May to July).Hot water treatment can be recommended on seed germination of the species in rural Bangladesh.展开更多

Selenium posttreatment group( n = 8) was treated with i. p. selenium0.5 mg/kg for 7d after the IR injury with termination at the end of the 7d period. Sham group(n =8) received i.p.saline injections identical to the selenium volume for 7d with termination 24 h after the IR injury. Control group(n =8) received no intervention. Main outcome measures were retina superoxide dismutase(SOD), glutathione(GSH),total antioxidant status(TAS), malondialdehyde(MDA),DNA fragmentation levels, and immunohistological apoptosis evaluation.RESULTS: Compared to the Sham group, selenium pre-treatment had a statistical difference in all parameters except SOD. Post-treatment selenium also resulted in statistical differences in all parameters except the MDA levels. When comparing selenium groups, the pre-treatment selenium group had a statistically higher success in reduction of markers of cell damage such as MDA and DNA fragmentation. In contrast, the post-selenium treatment group had resulted in statisticallyhigher levels of GSH. Histologically both selenium groups succeeded to limit retinal thickening and apoptosis. Pre-treatment use was statistically more successful in decreasing apoptosis in ganglion cell layer compared to post-treatment use.CONCLUSION: Selenium was successful in retinal protection in IR injuries. Pre-treatment efficacy was superior in terms of prevention of tissue damage and apoptosis.展开更多

However, some coastal areas frequently suffer from typho...Due to frequent drinking water pollution accidents in the past decade, it is common that mountain reservoirs were used as the source of drinking water in China. However, some coastal areas frequently suffer from typhoon with extreme precipitation, which results in the water quality deterioration of the reservoirs. The influence of typhoons with extreme precipitation on Jiaokou reservoir and the emergency treatment process of Maojiaping water treatment plant in the past three typical typhoons with extreme precipitation from the year of 2012-2015 were studied. It was found that the degradation of water quality, such as the increase of turbidity and bacteria index, may not merely appear during the events, but last for several days. Changing the dosage of water purification agent, such as coagulant and disinfectant at right time and place may be an efficient emergency water treatment process. Based on the analysis of water quality variation rule during and after the events, it was also found that emergency treatment can be fully prepared before the arrival of a typhoon with extreme precipitation. And in order to better respond to the typhoon with extreme precipitation, several suggestions are also proposed in this paper as follows: establishing vegetated buffers at right place, such as macrophanerophytes,shrub or herbage, increasing investments in infrastructure management, merging or cancelling the small-scale water treatment plants, preparing adequate water purification agent before the typhoon comes, etc.展开更多

The results show that a pre-annealing treatment at 350°C for 15 h before a T6 treatment substantially reduced the sensitivity of the microstructure and mechanical properties of the extruded 7A04 aluminum alloy specimens toward the extrusion temperature. The average grain sizes of the specimens extruded at 390 and 430°C after T6 treatment were 3.4 and 8.1 μm, respectively, and their elongations to failure were 7.0% and 9.2%, respectively. However, after pre-annealing + T6 treatment, the differences in both the grain sizes and the elongations of the specimens became small, i.e., their average grain sizes were 3.2 and 3.8 μm and their elongations were 12.0% and 13.3%, respectively. For the specimens extruded at the same temperature, pre-annealing treatment obviously improved the plasticity of the alloy, which is attributed to an increase in soft texture or to grain refinement in the specimens as a result of the pre-annealing + T6 treatment.展开更多
文摘BACKGROUND Preoperative anxiety is a common emotional problem during the perioperative period and may adversely affect postoperative recovery.Emergence agitation(EA)is a common complication of general anesthesia that may increase patient discomfort and hospital stay and may be associated with the development of postoperative complications.Pre-anesthetic anxiety may be associated with the development of EA,but studies in this area are lacking.AIM To determine the relationship between pre-anesthetic anxiety and EA after radical surgery in patients with non-small cell lung cancer(NSCLC).METHODS Eighty patients with NSCLC undergoing surgical treatment between June 2020 and June 2023 were conveniently sampled.We used the Hospital Anxiety and Depression Scale’s(HADS)anxiety subscale(HADS-A)to determine patients’anxiety at four time points(T1-T4):Patients’preoperative visit,waiting period in the surgical waiting room,after entering the operating room,and before anesthesia induction,respectively.The Riker Sedation-Agitation Scale(RSAS)examined EA after surgery.Scatter plots of HADS-A and RSAS scores assessed the correlation between patients’pre-anesthesia anxiety status and EA.We performed a partial correlation analysis of HADS-A scores with RSAS scores.RESULTS NSCLC patients’HADS-A scores gradually increased at the four time points:7.33±2.03 at T1,7.99±2.22 at T2,8.05±2.81 at T3,and 8.36±4.17 at T4.The patients’postoperative RSAS score was 4.49±1.18,and 27 patients scored≥5,indicating that 33.75%patients had EA.HADS-A scores at T3 and T4 were significantly higher in patients with EA(9.67±3.02 vs 7.23±2.31,12.56±4.10 vs 6.23±2.05,P<0.001).Scatter plots showed the highest correlation between HADS-A and RSAS scores at T3 and T4.Partial correlation analysis showed a strong positive correlation between HADS-A and RSAS scores at T3 and T4(r=0.296,0.314,P<0.01).CONCLUSION Agitation during anesthesia recovery in patients undergoing radical resection for NSCLC correlated with anxiety at the time of entering the operating room and before anesthesia induction.

文摘BACKGROUND:Shared decision-making(SDM)has broad application in emergencies.Most published studies have focused on SDM for a certain disease or expert opinions on future research gaps without revealing the full picture or detailed guidance for clinical practice.This study is to investigate the optimal application of SDM to guide life-sustaining treatment(LST)in emergencies.METHODS:This study was a prospective two-round Delphi consensus-seeking survey among multiple stakeholders at the China Consortium of Elite Teaching Hospitals for Residency Education.Participants were identified based on their expertise in medicine,law,administration,medical education,or patient advocacy.All individual items and questions in the questionnaire were scored using a 5-point Likert scale,with responses ranging from"very unimportant"(a score of 1)to"extremely important"(a score of 5).The percentages of the responses that had scores of 4-5on the 5-point Likert scale were calculated.A Kendall’s W coefficient was calculated to evaluate the consensus of experts.RESULTS:A two-level framework consisting of 4 domains and 22 items as well as a ready-touse checklist for the informed consent process for LST was established.An acceptable Kendall’s W coefficient was achieved.CONCLUSION:A consensus-based framework supporting SDM during LST in an emergency department can inform the implementation of guidelines for clinical interventions,research studies,medical education,and policy initiatives.
文摘Objective:To evaluate the efficacy of emergency ventilator therapy in severe acute left heart failure.Methods:A total of 75 patients with severe acute left ventricular heart failure who were admitted to the hospital from July 2020 to July 2023 were randomly divided into two groups.Group A received additional emergency ventilator treatment,and group B received conventional treatment.The efficacy was compared.Results:The curative effect of patients with severe acute left heart failure in group A was higher than that in group B(P<0.05);all blood gas indicators in group A were better than those in group B(P<0.05);all vital signs indicators in group A were better than those in group B(P<0.05);group A was more satisfied with the treatment of severe acute left ventricular heart failure than group B(P<0.05).Conclusion:Patients with severe acute left heart failure who receive emergency ventilator treatment can stabilize vital signs,improve blood oxygen supply,and enhance curative effect.
文摘Control of glyphosate-resistant giant ragweed is a challenge, particularly for soybean growers, because of limited effective post-emergence (POST) herbicide options. Many soybean growers in no-till production systems use 2,4-D in burndown application for control of broadleaf weeds, including giant ragweed. Field experiments were conducted at David City, NE, in 2012 and 2013 to evaluate 2,4-D followed by PRE or POST herbicide programs for control of glyphosate-resistant giant ragweed in glyphosate-resistant soybean. Results suggested that burndown application of 2,4-D or saflufenacil plus imazethapyr resulted in 89 to 99% control of giant ragweed at 21 days after treatment. Burndown-only treatments of S-metolachlor plus metribuzin or sulfentrazone plus cloransulam resulted in poor control (≤65%) of giant ragweed and reduced soybean yield (≤ 577 kg·ha-1). Burndown application of 2,4-D followed by saflufenacil plus imazethapyr, S-metolachlor plus metribuzin, or sulfentrazone plus cloransulam applied pre-emergence (PRE) or cloransulam, chlorimuron, fomesafen, imazethapyr, or lactofen in tank-mixtures with acetochlor applied POST resulted in 87% to 99% giant ragweed control, reduced density to ≤7 plants m-2, and resulted in soybean yield from 2519 to 3823 kg·ha-1. There was no difference among and between 2,4-D followed by PRE or POST herbicides for giant ragweed control, density, or soybean yield, indicating all the two pass herbicide programs were effective. It is concluded that glyphosate-resistant giant ragweed can be effectively controlled in soybean by including 2,4-D in burndown program followed by PRE or POST herbicides tested in this study.

文摘BACKGROUND For patients with acute paraplegia caused by spinal giant cell tumor(GCT)who require emergency decompressive surgery,there is still a lack of relevant reports on surgical options.This study is the first to present the case of an acute paraplegic patient with a thoracic spinal GCT who underwent an emergency total en bloc spondylectomy(TES).Despite tumor recurrence,three-level TES was repeated after denosumab therapy.CASE SUMMARY A 27-year-old female patient who underwent single-level TES in an emergency presented with sudden severe back pain and acute paraplegia due to a thoracic spinal tumor.After emergency TES,the patient's spinal cord function recovered,and permanent paralysis was avoided.The postoperative histopathological examination revealed that the excised neoplasm was a rare GCT.Unfortunately,the tumor recurred 9 months after the first surgery.After 12 months of denosumab therapy,the tumor size was reduced,and tumor calcification.To prevent recurrent tumor progression and provide a possible cure,a three-level TES was performed again.The patient returned to an active lifestyle 1 month after the second surgery,and no recurrence of GCT was found at the last follow-up.CONCLUSION This patient with acute paraplegia underwent TES twice,including once in an emergency,and achieved good therapeutic results.TES in emergency surgery is feasible and safe when conditions permit;however,it may increase the risk of tumor recurrence.
文摘Seeds were subjected to three different pre-sowing seed treatments: immersion in lukewarm water for 2 hours, immersion in concentrated sulfuric acid (H<sub>2</sub>SO<sub>4</sub>) for 5 minutes, hilar removal, and a control in which the seeds were sown without being treated. The experiment was laid out in the Completely Randomized Design (CRD) with four replicates and 60 seeds per treatment. Seeds were sown in an improvised Seedbox in October 2019. Germination was monitored daily for one month. The results showed that Mamalis seeds treated with lukewarm water have the earliest germination of twelve days, with a germination percentage of 66.67%. The germination rate of another treatment ranges from 0 - 44 percent, compared to 45 percent for the control treatment. It seems prudent to conclude that to enhance the vegetative propagation methods is to soaking in warm water at 37.5˚C for 2 hours could provide the best growth.
文摘Objective:To explore the preventive effects of anti-infective treatment on wound infection in emergency surgical trauma.Methods:180 patients in our hospital from 2019 to June to May 2020 were selected as subjects.The 180 patients were randomly divided into two groups.The control group had 90 cases and adopted the conventional treatment method,and the experimental group received anti-infective treatment.The wound healing,infection status,and patient satisfaction of the two groups of patients were analyzed.Results:The wound healing,infection status,and patient satisfaction of the experimental group were better than those of the control group.Conclusion:The application of anti-infective treatment to the prevention of wound infection in emergency surgical trauma can reduce the infection rate of the patients'incision and promote the recovery of patients.It is suitable for clinical applications.

文摘Testing techniques to reduce weed infestation is a crucial step in developing direct tree seeding systems. The use of pre-emergence herbicides may be an alternative to manual weeding techniques, but so far, information on how they affect the seeds of native tree species is scarce. We established a greenhouse experiment to evaluate the effects of four pre-emergence herbicides (atrazine, diuron, isoxaflutole and oxyfluorfen) on weed suppression and seedling emergence and early growth of seven tropical forest tree species (Annona coriacea Mart., Citharexylum myrianthum Cham., Cordia ecalyculata Vell., Peltophorum dubium (Spreng.) Taub., Psidium guajava L., Pterogyne nitens Tul. and Schinus terebinthifolia Raddi). The experimental design was a randomized complete block design with five treatments and five replicates. The treatments consisted of a single dose of each pre-emergence herbicide and a control. Throughout the 60?days after sowing we evaluated weed cover and seedling emergence and early growth of tree species. Overall, our results suggest that all tested herbicides reduced weed cover; however, they also negatively affected tree species seedling emergence. Of the four herbicides tested, atrazine and diuron showed the greatest effects on tree seedling emergence, oxyfluorfen was least aggressive towards native species and isoxaflutole was most effective in weed control. Native tree species varied in their responses to herbicides, indicating that future experiments should increase the number of species tested as well as investigate how seed traits can affect the species responses to different herbicides.

文摘BACKGROUND Antiviral treatment of patients with chronic hepatitis B(CHB)in the grey zone of treatment comands risk management in order to optimize the health outcome.In this sense,the identification of HBV mutants related with an increased risk of hepatocellular carcinoma(HCC)could be useful to identify subpopulations with potential indication of antiviral treatment.AIM To analyze the prevalence/persistence of hepatitis B virus(HBV)preS and basal core promoter(BCP)/precore/core variants associated to HCC development in CHB patients in the grey zone.METHODS Work was designed as a longitudinal retrospective study,including 106 plasma samples from 31 patients with CHB in the grey zone of treatment:Hepatitis B e antigen negative,HBV-DNA levels between 12-20000 IU/mL,normal or discordant transaminase levels during follow up and mild/moderate necroinflammatory activity in liver biopsy or Fibroscan(up to 9.5 kPa).Serum HBVDNA was tested using the Abbott Real Time HBV Assay and the BCP/precore/core and the hepatitis B surface antigen(HBsAg)coding regions were analyzed in positive samples by PCR/bulk-sequencing to identify the HCCrelated HBV mutants.RESULTS High-risk HCC related mutants were detected in 24(77%)patients:19(61%)in the BCP/precore/core,and 7(23%)in the HBsAg coding region(2 preS1 and 5 preS2 deletions).The prevalence of preS deletions was genotype-dependent:3/5(60%)patients with preS2 deletions and 1/2 with preS1 deletions were infected with the HBV-E genotype.Since HBV-E was the most prevalent in sub-Saharan patients,a correlation between preS deletions and ethnicity was also found:6/8(75%)sub-Saharan vs 1/19(5%)Caucasian patients had preS deletions(P=0.00016).Remarkably,this correlation was maintained in those patients infected with HBV-A,a minor genotype in sub-Saharan patients:2/2 patients infected with HBV-A from West Africa vs 0/6 of Caucasian origin had preS deletions.The HCC related variants were the major strains and persisted over time(up to 48 mo).Patients with preS deletions had a significant higher prevalence of F2 fibrosis stage than the negatives(57%vs 10%,P=0.0078).CONCLUSION HBV genetic analysis of selected populations,like sub-Saharans infected with HBV-E/A genotypes,will allow identification of subpopulations with risk of HCC development due to accumulation of high-risk HBV variants,thus commanding their increased clinical surveillance.

文摘BACKGROUND Craniocerebral injuries encompass brain injuries,skull fractures,cranial soft tissue injuries,and similar injuries.Recently,the incidence of craniocerebral injuries has increased dramatically due to the increased numbers of traffic accidents and aerial work injuries,threatening the physical and mental health of patients.AIM To investigate the impact of failure modes and effects analysis(FMEA)-based emergency management on craniocerebral injury treatment effectiveness.METHODS Eighty-four patients with craniocerebral injuries,treated at our hospital from November 2019 to March 2021,were selected and assigned,using the random number table method,to study(n=42)and control(n=42)groups.Patients in the control group received conventional management while those in the study group received FMEA theory-based emergency management,based on the control group.Pre-and post-interventions,details regarding the emergency situation;levels of inflammatory stress indicators[Interleukin-6(IL-6),C-reactive protein(CRP),and procalcitonin(PCT)];incidence of complications;prognoses;and satisfaction regarding patient care were evaluated for both groups.RESULTS For the study group,the assessed parameters[pre-hospital emergency response time(9.13±2.37 min),time to receive a consultation(2.39±0.44 min),time needed to report imaging findings(1.15±4.44 min),and test reporting time(32.19±6.23 min)]were shorter than those for the control group(12.78±4.06 min,3.58±0.71 min,33.49±5.51 min,50.41±11.45 min,respectively;P<0.05).Pre-intervention serum levels of IL-6(78.71±27.59 pg/mL),CRP(19.80±6.77 mg/L),and PCT(3.66±1.82 ng/mL)in the study group patients were not significantly different from those in the control group patients(81.31±32.11 pg/mL,21.29±8.02 mg/L,and 3.95±2.11 ng/mL respectively;P>0.05);post-intervention serum indicator levels were lower in both groups than pre-intervention levels.Further,serum levels of IL-6(17.35±5.33 pg/mL),CRP(2.27±0.56 mg/L),and PCT(0.22±0.07 ng/mL)were lower in the study group than in the control group(30.15±12.38 pg/mL,3.13±0.77 mg/L,0.38±0.12 ng/mL,respectively;P<0.05).The complication rate observed in the study group(9.52%)was lower than that in the control group(26.19%,P<0.05).The prognoses for the study group patients were better than those for the control patients(P<0.05).Patient care satisfaction was higher in the study group(95.24%)than in the control group(78.57%,P<0.05).CONCLUSION FMEA-based craniocerebral injury management effectively shortens the time spent on emergency care,reduces inflammatory stress and complication risk levels,and helps improve patient prognoses,while achieving high patient care satisfaction levels.
文摘Chemical eye burns present an avoidable,but frequent,occupational injury with potentially detrimental consequences for the quality of life and occupational rehabilitation of the injured.A periodical review of guidelines is required to assure the optimal emergency management.We reviewed the literature with emphasis on current German guidelines,primarily MEDLINE.If the crucial first-line measure,the injury prevention has failed and an eye burn has been sustained,the immediate and copious rinsing of the eye is the pivotal emergency treatment modality.Whereas the immediacy and sufficiency of the emergency rinsing are largely unanimous,there is an ongoing debate about the benefits and risks of specific rinsing solutions,and regular updates on guidelines and recommendations for the emergency treatment are warranted.The easiest and readily available rinsing solution is tap water,which fulfils the crucial criteria conveniently in most industrialized countries:purity,sterility,and neutral p H.Other rinsing solutions are proposing higher osmolality to stabilize the physiological p H,because of their superior buffering capacity.However,there is no compelling evidence for a substantial benefit,and some reports suggest that there could be unwanted side effects.In combination with the substantially increased expenditure and a more complex handling procedure,currently a general recommendation of any other solution than tap water is not warranted.

文摘Transparent electrode based on silver nanowires(Ag NWs) emerges as an outstanding alternative of indium tin oxide film especially for flexible electronics. However, the conductivity of Ag NWs transparent electrode is still dramatically limited by the contact resistance between nanowires at high transmittance. Polyvinylpyrrolidone(PVP) layer adsorbed on the nanowire surface acts as an electrically insulating barrier at wire–wire junctions, and some devastating post-treatment methods are proposed to reduce or eliminate PVP layer, which usually limit the application of the substrates susceptible to heat or pressure and burden the fabrication with high-cost, time-consuming, or inefficient processes. In this work, a simple and rapid pre-treatment washing method was proposed to reduce the thickness of PVP layer from 13.19 to0.96 nm and improve the contact between wires. Ag NW electrodes with sheet resistances of 15.6 and 204 X sq-1have been achieved at transmittances of 90 and 97.5 %, respectively. This method avoided any post-treatments and popularized the application of high-performance Ag NW transparent electrode on more substrates. The improved Ag NWs were successfully employed in a capacitive pressure sensor with high transparency, sensitivity, and reproducibility.

文摘A laboratory-scale anaerobic sequencing batch reactor (ASBR) was used to pretreat coking wastewater. Inoculated anaerobic granular biomass was acclimated for 225 d to the coking wastewater, and then the biochemical methane potential (BMP)of the coking wastewater in the acclimated granular biomass was measured. At the same time, some fundamental technological factors, such as the filling time and the reacting time ratio (tf/tr), the mixing intensity and the intermittent mixing mode, that affect anaerobic pretreatment of coking wastewater with ASBR, were evaluated through orthogonal tests. The COD removal efficiency reached 38%~50% in the stable operation period with the organic loading rate of 0.37~0.54 kg COD/(m3.d) at the optimum conditions of tf/tr, the mixing intensity and the intermittent mixing mode. In addition, the biodegradability of coking wastewater distinctly increased after the pretreatment using ASBR. At the end of the experiment, the microorganism forms on the granulated sludge in the ASBR were observed using SEM (scanning electron microscope) and fluoroscope. The results showed that the dominant microorganism on the granular sludge was Methanosaeta instead of Methanosarcina dominated on the inoculated sludge.
文摘Seed morphology,germination and seedlings growth of Acacia auriculiformis were studied.The experiment was conducted in the nursery of Forestry and Wood Technology Discipline,Khulna University,Bangladesh.Matured seeds of the species were collected from healthy trees of road side plantation from different areas of Khulna District,Bangladesh and treated with five pre-sowing treatments(control,immersion in cold water,immersion in hot water,scarification with sand paper and immersion in concentrated H2SO4).The average length,breadth and thickness were found to be(0.58±0.017) cm,(0.44±0.007) cm and(0.20±0.089) cm,respectively.Germination was conducted in polybags with a mixture of top soil,coconut husk,coarse sand,and fine sand in a ratio of 3:4:1:1.Results reveal that pre-sowing treatments influences the germination rates of seeds that significantly increase the percentage germination compared with those in control(43%) and cold water treatment(52%).The highest germination success rate was found 83% in hot water treatment followed by 78% in scarification with sand paper,and 75% with immersion in H2SO4.Germination started from 7 to 12 days and completed between 28 and 35 days period in all treatments.ANOVA showed the significant difference(p0.05) among the treatments in seed germination,but no significant difference among treatment with regard to starting day,closing day and total germination period.In case of height and diameter growth,seedlings originated from the seeds with hot water treatment shows significantly higher in wet season(from May to July).Hot water treatment can be recommended on seed germination of the species in rural Bangladesh.
文摘AIM: To investigate the effects of selenium in rat retinal ischemia reperfusion(IR) model and compare pretreatment and post-treatment use.METHODS: Selenium pre-treatment group(n =8) was treated with intraperitoneal(i.p.) selenium 0.5 mg/kg for7 d and terminated 24 h after the IR injury. Selenium posttreatment group( n = 8) was treated with i. p. selenium0.5 mg/kg for 7d after the IR injury with termination at the end of the 7d period. Sham group(n =8) received i.p.saline injections identical to the selenium volume for 7d with termination 24 h after the IR injury. Control group(n =8) received no intervention. Main outcome measures were retina superoxide dismutase(SOD), glutathione(GSH),total antioxidant status(TAS), malondialdehyde(MDA),DNA fragmentation levels, and immunohistological apoptosis evaluation.RESULTS: Compared to the Sham group, selenium pre-treatment had a statistical difference in all parameters except SOD. Post-treatment selenium also resulted in statistical differences in all parameters except the MDA levels. When comparing selenium groups, the pre-treatment selenium group had a statistically higher success in reduction of markers of cell damage such as MDA and DNA fragmentation. In contrast, the post-selenium treatment group had resulted in statisticallyhigher levels of GSH. Histologically both selenium groups succeeded to limit retinal thickening and apoptosis. Pre-treatment use was statistically more successful in decreasing apoptosis in ganglion cell layer compared to post-treatment use.CONCLUSION: Selenium was successful in retinal protection in IR injuries. Pre-treatment efficacy was superior in terms of prevention of tissue damage and apoptosis.

文摘Due to frequent drinking water pollution accidents in the past decade, it is common that mountain reservoirs were used as the source of drinking water in China. However, some coastal areas frequently suffer from typhoon with extreme precipitation, which results in the water quality deterioration of the reservoirs. The influence of typhoons with extreme precipitation on Jiaokou reservoir and the emergency treatment process of Maojiaping water treatment plant in the past three typical typhoons with extreme precipitation from the year of 2012-2015 were studied. It was found that the degradation of water quality, such as the increase of turbidity and bacteria index, may not merely appear during the events, but last for several days. Changing the dosage of water purification agent, such as coagulant and disinfectant at right time and place may be an efficient emergency water treatment process. Based on the analysis of water quality variation rule during and after the events, it was also found that emergency treatment can be fully prepared before the arrival of a typhoon with extreme precipitation. And in order to better respond to the typhoon with extreme precipitation, several suggestions are also proposed in this paper as follows: establishing vegetated buffers at right place, such as macrophanerophytes,shrub or herbage, increasing investments in infrastructure management, merging or cancelling the small-scale water treatment plants, preparing adequate water purification agent before the typhoon comes, etc.

The results show that a pre-annealing treatment at 350°C for 15 h before a T6 treatment substantially reduced the sensitivity of the microstructure and mechanical properties of the extruded 7A04 aluminum alloy specimens toward the extrusion temperature. The average grain sizes of the specimens extruded at 390 and 430°C after T6 treatment were 3.4 and 8.1 μm, respectively, and their elongations to failure were 7.0% and 9.2%, respectively. However, after pre-annealing + T6 treatment, the differences in both the grain sizes and the elongations of the specimens became small, i.e., their average grain sizes were 3.2 and 3.8 μm and their elongations were 12.0% and 13.3%, respectively. For the specimens extruded at the same temperature, pre-annealing treatment obviously improved the plasticity of the alloy, which is attributed to an increase in soft texture or to grain refinement in the specimens as a result of the pre-annealing + T6 treatment.
