Practice of Infection Prevention and Control Strategies in Risk Departments during the COVID-19 Epidemic

Objective: To explore the practice and application of infection prevention and control strategies in risk departments during the COVID-19 epidemic, and to formulate the infection prevention and control measures to provide advice and guidance in risk departments. Methods: According to the latest plan of diagnosis and treatment, prevention and control issued by the National Health Commission, expert advice and consensus, combined with the actual situation in our hospital, a series of infection prevention and control measures of COVID-19 in risk department was formulated. Results: During the epidemic period, the prevention and control measures of nine risk departments including emergency operation, anesthesiology, endoscopy center, blood purification center, otolaryngology, stomatology, medical imaging department, medical cosmetology department and pulmonary function room were established from six aspects, including pre-examination and screening, medical technology control, personnel management, personal protection, environmental disinfection, medical waste disposal, etc. Conclusion: During the epidemic period, the infection prevention and control strategy of risk departments is one of the key links to control the spread of the epidemic, and risk departments must pay attention to and strictly implement various infection prevention and control measures. 展开更多

Objective: To evaluate the role of prevention and control strategies for nosocomial infection in a tertiary teaching hospital during the sudden outbreak of Corona Virus Disease 2019 (COVID-19). Methods: The hospital initiated an emergency plan involving multi-departmental defense and control. It adopted a series of nosocomial infection prevention and control measures, including strengthening pre-examination and triage, optimizing the consultation process, improving the hospital’s architectural composition, implementing graded risk management, enhancing personal protection, and implementing staff training and supervision. Descriptive research was used to evaluate the short-term effects of these in-hospital prevention and control strategies. The analysis compared changes in related evaluation indicators between January 24, 2020 and February 12, 2020 (Chinese Lunar New Year’s Eve 2020 to lunar January 19) and the corresponding lunar period of the previous year. Results: Compared to the same period last year, the outpatient fever rate increased by 1.85-fold (P P Conclusion: The nosocomial infection prevention and control strategies implemented during this specific period improved the detection and control abilities for the COVID-19 source of infection and enhanced the compliance with measures. This likely contributed significantly to avoiding the occurrence of nosocomial infection. 展开更多

Severe acute respiratory syndrome coronavirus-2 is a highly contagious positive-sense,single-stranded RNA virus that has rapidly spread worldwide.As of December 17,2023,772838745 confirmed cases including 6988679 deaths have been reported globally.This virus primarily spreads through droplets,airborne transmission,and direct contact.Hospitals harbor a substantial number of confirmed coronavirus disease 2019(COVID-19)patients and asymptomatic carriers,accompanied by high population density and a larger susceptible population.These factors serve as potential triggers for nosocomial infections,posing a threat during the COVID-19 pandemic.Nosocomial infections occur to varying degrees across different countries worldwide,emphasizing the urgent need for a practical approach to prevent and control the intra-hospital spread of COVID-19.This study primarily concentrated on a novel strategy combining preventive measures with treatment for combating COVID-19 nosocomial infections.It suggests preventive methods,such as vaccination,disinfection,and training of heathcare personnel to curb viral infections.Additionally,it explored therapeutic strategies targeting cellular inflammatory factors and certain new medications for COVID-19 patients.These methods hold promise in rapidly and effectively preventing and controlling nosocomial infections during the COVID-19 pandemic and provide a reliable reference for adopting preventive measures in the future pandemic. 展开更多

Vitamin D is a steroid hormone that is naturally produced in the body or obtained through dietary sources,primarily under the influence of UVB radiation.This essential nutrient has a vital role in numerous physiological processes,encom-passing immune function,cell growth,differentiation,insulin regulation,and cardiovascular well-being,along with its pivotal role in sustaining the delicate equilibrium of calcium and phosphate concentrations in the body.Moreover,vitamin D reinforces mucosal defense and bolsters the immune system through immunomodulation,making it a critical component of overall health.Numerous studies have unveiled the profound connection between vitamin D and the predisposition to respiratory tract infections,including well-known viruses such as influenza and the novel severe acute respiratory syndrome coronavirus 2.Vita-min D deficiency has been consistently linked to increased severity of coronavirus disease 2019(COVID-19)and a heightened risk of mortality among afflicted individuals.Retrospective observational studies have further substantiated these findings,indicating that levels of vitamin D are linked with both the occurrence and severity of COVID-19 cases.Vitamin D has its influence on viral infections th-rough a multitude of mechanisms,such as promoting the release of antimicrobial peptides and fine-tuning the responses of the immune system.Additionally,vitamin D is intertwined with the intricate network of the renin–angiotensin system,suggesting a potential impact on the development of complications related to COVID-19.While further clinical trials and extensive research are warranted,the existing body of evidence strongly hints at the possible use of vitamin D as a valuable tool in the prophylaxis and management of COVID-19 and other viral infectious diseases. 展开更多

Therefore, the blood purification center has become a high‑risk environment for the spread of COVID-19 infection. In viewof this, our center quickly responded to the formulation and implementation of infection prevention and control measures suitable for thecharacteristics of the blood purification center and continuous renal replacement therapy (CRRT) emergency plan for fever and suspectedpatients. <strong>Objective:</strong> We assess the application effect of the “3 + 1” mode in the COVID-19 epidemic prevention and control at the infection ward of a designated comprehensive hospital for COVID-19 ... <strong>Objective:</strong> We assess the application effect of the “3 + 1” mode in the COVID-19 epidemic prevention and control at the infection ward of a designated comprehensive hospital for COVID-19 treatment.<strong> Method: </strong>Based on the features of the inpatients of the infection ward and their relatives, a “3 + 1” mode for the COVID-19 prevention and control is developed to conform to the demands for epidemic prevention and control and the overall prevention and control scheme of the whole hospital. Here, “3” stands for the epidemic prevention and control system, personnel management, and prevention and control measures;“1” stands for COVID-19 testing.<strong> Result: </strong>From March 1, 2020 to March 31, 2021, a total of 3056 patients were hospitalized in the three infection wards. Among them, 265 patients had a fever, and 113 patients had respiratory symptoms. None of them were infected with COVID-19. The participation rate of the test about the knowledge related to COVID-19 and the knowledge mastery rate were both 100% among the medical staff. None of the inpatients and their companions or the medical staff was diagnosed with COVID-19.<strong> Conclusion: </strong>Thus the “3 + 1” mode proves successful for avoiding nosocomial infection and the spread of the epidemic. 展开更多

Most patients with COVID-19 disease caused by the SARS-CoV-2 virus recover from this infection, but a significant fraction progress to a fatal outcome. As with some other RNA viruses, co-infection or activation of latent bacterial infections along with pre-existing health conditions in COVID-19 disease may be important in determining a fatal disease course. Mycoplasma spp. (M. pneumonaie, M. fermentans, etc.) have been routinely found as co-infections in a wide number of clinical conditions, and in some cases this has progressed to a fatal disease. Although preliminary, Mycoplasma pneumoniae has been identified in COVID-19 disease, and the severity of some signs and symptoms in progressive COVID-19 patients could be due, in part, to Mycoplasma or other bacterial infections. Moreover, the presence of pathogenic Mycoplasma species or other pathogenic bacteria in COVID-19 disease may confer a perfect storm of cytokine and hemodynamic dysfunction, autoimmune activation, mitochondrial dysfunction and other complications that together cannot be easily corrected in patients with pre-existing health conditions. The positive responses of only some COVID-19 patients to antibiotic and anti-malaria therapy could have been the result of suppression of Mycoplasma species and other bacterial co-infections in subsets of patients. Thus it may be useful to use molecular tests to determine the presence of pathogenic Mycoplasma species and other pathogenic bacteria that are commonly found in atypical pneumonia in all hospitalized COVID-19 patients, and when positive results are obtained, these patients should treated accordingly in order to improve clinical responses and patient outcomes. 展开更多

Background: The world is currently experiencing a healthcare crisis caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Healthcare workers (HCWs) are at the forefront of controlling the spread of the disease;this response has resulted in a huge number of infections amongst HCWs and unfortunately some casualties. Infection prevention and control practices (IPC) are practical, proven methods that prevent avoidable harm to patients and protect health workers from contagious infection. This study was done to evaluate the effectiveness of IPC practices utilized in COVID-19 testing drive through facility in Lagos Nigeria. Method: The facility recruited 42 HCWs who were trained on IPC and use of Personal Protective Equipment (PPEs). Training on donning and doffing of PPEs, preparation of decontaminants and sample handling were done for relevant personnel. A daily log for monitoring COVID-19 symptoms was deployed to all HCWs;to help early detection of COVID-19 infection and prevent transmission amongst staff. Results: Of 42 personnel, 92.8% had a tertiary level of education while 71.4% had at least 5 years work experience. A total of 5 (11.9%) out of 42 HCWs were positive by week six of the study. All infected persons worked at the drive through centre. Most common symptom filled in the daily log tool was headache and fatigue. Conclusion: Despite all the measures introduced, 11.9% of HCWs became positive within 6 weeks, showing the highly infectious nature of the virus and the need to research into more effective measures in preventing the transmission of highly infectious pathogens to HCWs during outbreaks. 展开更多

The outbreak of COVID-19 caused by SARS-CoV-2 is spreading worldwide,with the pathogenesis mostly unclear.Both virus and host-derived microRNA(miRNA)play essential roles in the pathology of virus infection.This study aims to uncover the mechanism for SARS-CoV-2 pathogenicity from the perspective of miRNA.We scanned the SARS-CoV-2 genome for putative miRNA genes and miRNA targets and conducted in vivo experiments to validate the virus-encoded miRNAs and their regulatory role on the putative targets.One of such virus-encoded miRNAs,MR147-3p,was overexpressed that resulted in significantly decreased transcript levels of all of the predicted targets in human,i.e.,EXOC7,RAD9A,and TFE3 in the virus-infected cells.The analysis showed that the immune response and cytoskeleton organization are two of the most notable biological processes regulated by the infection-modulated miRNAs.Additionally,the genomic mutation of SARS-CoV-2 contributed to the changed miRNA repository and targets,suggesting a possible role of miRNAs in the attenuated phenotype of SARS-CoV-2 during its evolution.This study provided a comprehensive view of the miRNA-involved regulatory system during SARS-CoV-2 infection,indicating possible antiviral therapeutics against SARS-CoV-2 through intervening miRNA regulation. 展开更多

Coronavirus disease 2019 (COVID-19) is the official name given by the World Health Organization (WHO). SARS-CoV-2 erupted from Wuhan, China, in 2019. By May 2020, COVID-19 was ubiquitous, infected millions of people on this planet, and became the worst outbreak of this disastrous pandemic. The pandemic’s impact such as a high mortality rate and economic imbalance. Random viruses constantly affect humans’ and animals’ health like severe acute respiratory syndrome. SARS COVID 2 followed. However, initially, coronavirus was identified in 2019. COVID-19 affected several sectors: transportation, health care, education, tourism, food sector, unemployment, trading, agriculture, sports, pharmaceutical industries, and global poverty. 展开更多

Evaluation of COVID-19 outbreak prevention and control in Beijing using the emergency management theory

On April 29,2020,China entered a normalization stage of prevention and control.By December 2021,more than 40 outbreaks had occurred in China,which reflected the shortcomings of the pandemic prevention and control measures in China at that time.As the capital city of China,Beijing faces more pressure in epidemic prevention and control.We used the COVID-19 cluster containment evaluation indicators to determine the effects of prevention and control measures on four COVID-19 outbreaks in Beijing.After considering the specificity and operability of evaluation indicators and the availability of evaluation data,the evaluation system in our study consisted of six dimensions:epidemic prevention and control effect,discovery and detection ability,precision prevention and control capability,public protection effect,medical treatment and nosocomial infection prevention and control ability,and information release and public opinion response ability.The composite scores of the prevention and control effects of the Xinfadi,Shunyi,Daxing,and Ejina Banner–associated COVID-19 outbreaks in Beijing were 62,82,87,and 76,respectively.In the six dimensions,the epidemic prevention and control effect,discovery and detection ability,precision prevention and control capability,and public protection effect scores for the Xinfadi outbreak were lower than those for the Shunyi,Daxing and Ejina Banner–associated outbreaks.The medical treatment and nosocomial infection prevention and control ability scores for the outbreak associated with Ejina Banner were lower than those for the Xinfadi,Shunyi,and Daxing outbreaks.In managing cluster outbreaks,Beijing was able to detect index cases early enough to reduce the scale and duration of the outbreak and consistently release official information to reduce public panic,standardize the management of centralized quarantine sites to prevent cross-infection,adhere to the“dynamic COVID‐zero”strategy to accurately prevent and control outbreaks,reduce the societal influence of the pandemic,and coordinate the epidemic prevention and control and socio-economic development. 展开更多

Coronavirus Disease 2019(COVID-19),caused by a novel coronavirus(SARS-Co V-2),is a highly contagious disease.It firstly appeared in Wuhan,Hubei province of China in December 2019.During the next two months,it moved rapidly throughout China.Most of the infected patients have mild symptoms including fever,fatigue and cough,but in severe cases,patients can progress rapidly and develop into acute respiratory distress syndrome,septic shock,metabolic acidosis and coagulopathy.The new coronavirus was reported to spread via droplets,contact and natural aerosols from human to human.Therefore,aerosol-producing procedures such as endotracheal intubation may put the anesthesiologists at high risk of nosocomial infections.In fact,SARSCo V-2 infection of anesthesiologists after endotracheal intubation for confirmed COVID-19 patients have been reported in hospitals in Wuhan.The expert panel of airway management in Chinese Society of Anaesthesiology has deliberated and drafted this recommendation,by which we hope to guide the performance of endotracheal intubation by frontline anesthesiologists and critical care physicians.During the airway management,enhanced droplet/airborne personal protective equitment(PPE)should be applied to the health care providers.A good airway assessment before airway intervention is of vital importance.For patients with normal airway,awake intubation should be avoided,and modified rapid sequence induction is strongly recommended.Sufficient muscle relaxant should be assured before intubation.For patients with difficult airway,good preparation of airway devices and detailed intubation plans should be made. 展开更多

BACKGROUND During the coronavirus disease 2019(COVID-19)pandemic,traditional teaching methods were disrupted and online teaching became a new topic in education reform and informatization.In this context,it is important to investigate the necessity and effectiveness of online teaching methods for medical students.This study explored stomatology education in China to evaluate the development and challenges facing the field using massive open online courses(MOOCs)for oral medicine education during the pandemic.AIM To investigate the current situation and challenges facing stomatology education in China,and to assess the necessity and effectiveness of online teaching methods among medical students.METHODS Online courses were developed and offered on personal computers and mobile terminals.Behavioral analysis and formative assessments were conducted to evaluate the learning status of students.RESULTS The results showed that most learners had already completed MOOCs and achieved better results.Course behavior analysis and student surveys indicated that students enjoyed the learning experience.However,the development of oral MOOCs during the COVID-19 pandemic faced significant challenges.CONCLUSION This study provides insights into the potential of using MOOCs to support online professional learning and future teaching innovation,but emphasizes the need for careful design and positive feedback to ensure their success. 展开更多

Clustering Countries on COVID-19 Data among Different Waves Using K-Means Clustering

The COVID-19 pandemic has caused an unprecedented spike in confirmed cases in 230 countries globally. In this work, a set of data from the COVID-19 coronavirus outbreak has been subjected to two well-known unsupervised learning techniques: K-means clustering and correlation. The COVID-19 virus has infected several nations, and K-means automatically looks for undiscovered clusters of those infections. To examine the spread of COVID-19 before a vaccine becomes widely available, this work has used unsupervised approaches to identify the crucial county-level confirmed cases, death cases, recover cases, total_cases_per_million, and total_deaths_per_million aspects of county-level variables. We combined countries into significant clusters using this feature subspace to assist more in-depth disease analysis efforts. As a result, we used a clustering technique to examine various trends in COVID-19 incidence and mortality across nations. This technique took the key components of a trajectory and incorporates them into a K-means clustering process. We separated the trend lines into measures that characterize various features of a trend. The measurements were first reduced in dimension, then clustered using a K-means algorithm. This method was used to individually calculate the incidence and death rates and then compare them. 展开更多
