The etiology and pathogenesis of traditional Chinese medicine is liver-qi discomfort, and soothing liver and relieving depression should be taken as the basic treatment method and treatment principle. By sorting out the relevant literature on PI3K/Akt signaling pathway, the relationship between PI3K/Akt signaling pathway and depression and insomnia was sorted out, and the possible mechanism of Liver-soothing and Depression-Relieving therapy for insomnia of liver-depression syndrome was found.展开更多

Insomnias can be broadly divided into primary and secondary types. Primary insomnias can further be divided into parasomnias and dyssomnias. Diagnostic approaches to insomnias can be challenging, thereby complicating the treatment process. Obstructive sleep apnea (OSA) falls under the category of primary insomnias and is a good example of frequently missed diagnoses. The goal of this paper is to document the development of a self-reporting sleep apnea scale based on graded percentage scores to correlate with primary, secondary and tertiary risks. Scores falling under fifty percent represent primary prevention risks of OSA and suggest possible interventions. A score of fifty percent highlights three core symptoms on the scale as described in detail below to represent early detection of OSA or secondary prevention. Finally, scores exceeding fifty percent will include associated risks of comorbidities as well as other factors contributing to OSA, or tertiary prevention, with separate scores assigned to individual risk factors. Analysis of final scores on the proposed scale would therefore, in our opinion help achieve the goal of grading the risk of OSA from a multidisciplinary preventive care aspect.展开更多

This pilot study aims to test the efficacy of an extended version of CBT-I called Integrative Therapy for Insomnia (IT-I). The traditional CBT-I is complemented with exercises that are expected to enhance relearning to sleep by providing different stimuli, and access options for the retrieval and application of the acquired information, cognition, and skills. Method: The efficacy of the IT-I is measured by comparing the participants’ scores on the Insomnia Severity Index at baseline and post-treatment. In total 29 people, with a wide range of ages, participated in the 8 sessions’ treatment of one hour 45 minutes per session, in mixed groups of age, gender, and primary or comorbid insomnia. Results: Comparing the ISI baseline and post-treatment scores, results show a mean improvement of 7.2 points and a p r = −0.43. Conclusions: The integration of exercises that generate sensory-motor stimuli, and address the mirror neuron system fitting with the Mental Contrasting as a metacognitive strategy seems to facilitate re-learning to sleep in cases of primary insomnia as well as for comorbid insomnia. Further studies will be needed to arrive at more firm conclusions.展开更多
