Improvement of the nutritional support management system for patients in intensive care units

BACKGROUND Nutritional support for patients hospitalized in the intensive care unit(ICU)is an important part of clinical treatment and care,but there are significant implementation difficulties.AIM To introduce a modified nutritional support management system for ICU patients based on closed-loop information management and psychological counseling.METHODS The division of functions,personnel training,system construction,development of an intelligent decision-making software system,quality control,and improvement of the whole process were carried out to systematically manage nutritional support for ICU patients.RESULTS Following the implementation of the whole process management system,the scores of ICU medical staff’s knowledge,attitudes/beliefs,and practices regarding nutritional support were comprehensively enhanced.The proportion of hospital bed-days of total enteral nutrition(EN)in ICU patients increased from 5.58%to 11.46%,and the proportion of EN plus parenteral nutrition increased from 42.71%to 47.07%.The rate of EN initiation within 48 h of ICU admission increased from 37.50%to 48.28%,and the EN compliance rate within 72 h elevated from 20.59%to 31.72%.After the implementation of the project,the Self-rating Anxiety Scale score decreased from 61.07±9.91 points to 52.03±9.02 points,the Self-rating Depression Scale score reduced from 62.47±10.50 points to 56.34±9.83 points,and the ICU stay decreased from 5.76±2.77 d to 5.10±2.12 d.CONCLUSION The nutritional support management system based on closed-loop information management and psychological counseling achieved remarkable results in clinical applications in ICU patients. 展开更多

Counseling people,particularly those in the military engaged in group living,who are in quarantine during the COVID-19 pandemic is a challenge.Therefore,supporting the people in quarantine who are experiencing psy-chological and interpersonal problems has become a new challenge in military mental health.This study’s primary concern was how to overcome the problems caused by physical quarantine.The study subject was a virtual coun-seling network and its operating experience during the quarantine period in Taiwan amidst the global COVID-19 pandemic.For soldiers who mainly live in groups,this study discussed how the virtual counseling network combined with the existing military support group to determine what influence the network had on peo-ple in quarantine.This study found that this group exhibited four types of experiences:togetherness,empathy,confidence,and belonging and cohesion.Such experiences are beneficial for group healing through mutual sup-port and companionship.Collective cohesion and psychological healing can be achieved through a virtual com-munity.This is worthy of attention,particularly in the pandemic or post-pandemic era.Physical isolation has become a fact of life,and such isolation is not just isolation from disease but also between regional boundaries.Counseling and support systems in virtual space or the operation of virtual teams must be considered for the future. 展开更多

Objective To understand the cognitive status and anxiety factors of COVID-19 in 804Hainan residents during the epidemic of corona virus disease 2019(COVID-19).Methods A cross-sectional survey was conducted on 804 Hainan residents.A non-randomized convenient sampling method combining with online and snowball recruitment method was used.Results The COVID-19 cognitive score of 804 Hainan residents was(61.22±22.45),with a total awareness rate of 71.5%;16.3%of the respondents had anxiety symptoms,and there were statistically significant differences among different populations of different genders,ages,educational backgrounds,and cognitive levels(P<0.05).The incidence of anxiety in women(18.1%),13-24years old(20.65%),low education(21.1%),and high cognition of SARS-CoV-2(19.4%)were higher than other groups.Binary Logistic regression analysis showed that the main influencing factors were gender,age and cognition status.Men were 0.563times higher than women(95%CI,0.348~0.911),the age group of 13-24 years old was 1.676times(95%CI,1.124-2.499)higher than 25-65 year old,and people with more COVID-19-related knowledge was 1.532 times higher than that with low cognition(95%CI,1.022~2.296).Conclusion The incidence of anxiety during the COVID-19epidemic was relatively high,and psychological counseling should be actively strengthened to relieve the anxiety symptoms. 展开更多
