A key component of both these policies is the development of good governance principles in all health care agencies and organizations. A neglected dimension in these debates has been the role of the public hospital which remains a vital and expensive dimension of service provision. In a case study of a public hospital in South Africa, this paper charts the readiness of its management principles and activities to provide quality health care linked to population health and to contribute to the non-medical determinant of health. Using Taylor’s principles of good governance, we conclude that there is a mixture of good and not so good governance activity. The paper concludes by examining the external constraints on the public hospital and by providing suggestions for future activities.展开更多

As an aspect of human capital,a positive association exists amongst health,productivity,and growth in output per capita.On the other hand,social infrastructure defined by the institution of governance has a direct eff...As an aspect of human capital,a positive association exists amongst health,productivity,and growth in output per capita.On the other hand,social infrastructure defined by the institution of governance has a direct effect on the environment upon which productive activities take place to determine outcomes.Nigeria like most African countries is bedevilled by the high prevalence of inadequate health financing and poor governance.Health financing for Nigeria consistently has fallen short of the AU health funding commitment of 15%of annual budgetary allocation to the health sector.Secondly,poor governance conditions available resources and shape the state of infrastructure,particularly health infrastructure and socioeconomic conditions.In turn,this determines individuals’level of exposure to health risks and their capacity to actively contribute to productive activity for growth stimulation and sustainability.Against this backdrop,this study added to the existing literature in the context of Nigeria,by theoretically applying the Solow augmented Mankiw-Romer-Weil structural model in the examination of the impact of government size and governance quality in the health sector,on economic growth.Autoregressive Distributed Lag(ARDL)model was adopted in the estimation.Findings show that governance quality adversely affects growth and this reduces the capacity of health spending to stimulate growth by an almost equal margin.As a result,this study recommends legislative backing to the AU health funding commitment in Nigeria.展开更多

Preliminary findings are presented from semi- structured qualitative interviews that were conducted with eleven directors and managers of local branches in the Knox County Public Library (KCPL) System that is located in the East Tennessee region in the United States. Select feedback reported by research participants is summarized in the article about strategies in LIS education that can help local public librarians and others in their efforts to become more responsive information providers to PLWHA. Public-private partnerships(PPPs) have reshaped the institutional structure of global governance.They have advanced collaboration on core issues such as health, clean energy, the rights of women and children, and acce...Public-private partnerships(PPPs) have reshaped the institutional structure of global governance.They have advanced collaboration on core issues such as health, clean energy, the rights of women and children, and access to infrastructure, among others. But which actors create and finance public-private partnerships in global governance to advance such objectives? What are the implications of the agency behind such collaboration for influencing the global agenda on sustainable development? While some scholars and advocacy groups see the growing role of private actors as a powershift away from public institutions, others argue that such arrangements tend to be complementary to public mandates and indeed may provide a new means for international institutions to pursue such mandates. The article probes this debate by analyzing the type of actors that finance global health partnerships, an area in which the influence of hybrid initiatives is particularly prominent. It reveals that public financing remains a core and necessary condition for the emergence and functioning of PPPs. The growing share of private financing,nonetheless, has important implications for shaping partnerships agendas and steering global health and sustainability governance. Analysis is based on data from a primary survey conducted among 2000 households, covering 10,929 individuals from four cities of India. Summary statistics and regressions (using STATA) are used for data analysis. Results show lack of government facilities and services, a very high preference for private health facilities, high expenses especially in private but also in public facilities, and a perception that private facilities are offering high quality services as important concerns. An econometric analysis of the determinants of acute illness indicates the insufficiency of basic amenities like sanitation, garbage disposal and potable water. Together with the lack of availability of government health facilities in the vicinity, these results indicate continued vulnerability of the urban poor, and the need for urgent government action.展开更多

The aim of this study was to investigate antibiotics prescribing practices to febrile under-five children outpatients in urban public health services in a low income country. Methods: From March to April 2013, a cross-sectional epidemiological study of care facilities visit by under-five age, for febrile illness, was carried out in urban health services in Bobo-Dioulasso, Burkina Faso. Patient demographics, diagnoses and medications were recorded. We calculated for each diagnoses several indicators for antibiotics use. Results: Our study showed an over-prescription of antibiotics at the university teaching hospital (78.08%) and at the first level facilities (57.71%) for under-five outpatients for febrile illness. There was evidence of high antibiotic prescription in children with diarrhea (more than 9 on 10 at university teaching hospital of diarrhea cases and 60% at the first level facilities), in children with Upper respiratory tract infections (respectively 60% and 85.2% of cases at university teaching hospital and at the first level facilities) and in children with malaria (respectively 47.5% and 17.6% of cases at university teaching hospital and at the first level facilities). Overuse, misuse and inappropriately prescribed antibiotic coexisted in our results: at university teaching hospital 90.9% of diarrhea cases, 60% of URTI cases,?47.5% of malaria cases received antibiotic prescription;at first level heath care facilities 85.2% of URTI, 17.6% of malaria cases received an prescribed antibiotic and 11.8% of LRTI did not received a prescribed antibiotic. Developing countries have poor access to newer antibiotics and irrational antibiotics use remains a global problem. Overuse and misuse of antibiotics combat, rigorous infectious diseases diagnosis, antimicrobial resistance consequences education of users and health professional’s prescribers, and improved surveillance of antimicrobial resistance, must be strengthened.展开更多

Access to basic public health services and satisfaction of the system, analysis of the factors affecting the satisfaction of the basic public health services in Shanghai revealed the problems and causes of development. Finally, putting forward the concrete countermeasures and suggestions for perfecting and improving the basic public health service in Shanghai.

文摘Noncommunicable chronic diseases have become the most important public health problem in the world today,so the focus of public health services should be shifted from the traditional area of communicable diseases to the prevention and control of chronic diseases.Since bad living habits are the most direct cause of chronic diseases,the most effective measure to prevent and control chronic diseases is to promote healthy lifestyles for the individual citizen.The theories of equal health opportunity,the right to health from the perspective of human rights,and determinants of a healthy society provide justified reasons for the intervention of public power in individual health choices.In the current legal system,the intervention of public power is limited to flexible measures such as health education,which shows respect for individual autonomy.However,it is inconsistent with the needs of current public health practice.We should expand diversified intervention means to encourage individuals to make healthy choices under the guidance of the management model.The“ladder of intervention”outlines a panoramic view of the intervention measures available.However,for the selection of specific measures,it is necessary to consider the public health objectives and the invasion of individual freedom,introduce the“legal reservation principle”and“proportionality principle”as policy analysis tools,and reasonably choose intervention measures at different levels on the ladder to properly handle the tension between public power and private rights.

文摘Hong Kong has a long history with its high efficiency and clean and self-disciplined government.Within the past over 20 years,different social development trend has occurred in Hong Kong.The article observed the relationship between political trust from residence and public service efficiency of government in Hong Kong from 1992 to 2015 and found that the value of public service efficiency has a significant effect on political trust in Hong Kong government,the higher the efficiency of public services,the higher the political trust.The author tried to find the path for the Hong Kong government to improve its public service quality and efficiency after testifying the positive correlation between public service efficiency and residential political trust with empirical analysis.

文摘In the recent years,with rapid development of mobile internet,new media platform has been developing at full speed,such as twitter,WeC hat and APP. It has wide information dissemination channels,fast release frequency and efficient interactive mode. According to the characteristics of new media,we set forth the current situation of public weather service. Through the questionnaire survey,we have analyzed the actual demand of public from three aspects,including the public reading habits,characteristics of sharing and transmission and defect of information distribution. Finally,we have discussed the future trend of meteorological new media,which will promote public service effectiveness of meteorological new media by considering requirement,applying new technology and reasonably using platform.
文摘Civil science and technology associations have been widely springing up everywhere in Britain, especially the new industrial towns in the eighteenth and nineteenth centuries. Associations held various academic activities, organized and funded scientific research, which played an important role in promoting the progress of science and technology and the development of industrial society. Based on case study on British Association for the Advancement of Science and Manchester Literary and Philosophical Society by academic circles at home and abroad, development situation of British science and technology associations in the middle and late 19^(th) century is summarized, and a series of activities of associations participating in industrial and agricultural development and environmental health reform are systematically sorted out, while the promoting role of science and technology associations in the social development of Britain in the 19^(th) century is explored.
