Evidence-based optimization of integrated traditional Chinese and Western medicine therapies for prevention and treatment of coronary heart disease： design and implementation

As a chronic disease that seriously endangers public health, the number of coronary heart disease （CHD） patients in China has increased in recent years. There is great potentiality to integrate traditional Chinese and Western medicine therapies to prevent and treat CHD in clinical practice. However, most of the current integrated therapies still lack sufficient high-quality evidence, and the key links in how to apply are unclear. It is urgent to optimize them through evidence-based research to further improve the effectiveness. Therefore, we propose strategies to conduct evidence-based optimization of integrated traditional Chinese and Western medicine therapies in prevention and treatment of progressive cardiovascular diseases. These are integrated clinical trial design, attention to the key links of taking effect, combination of clinical and basic research. Based on the strategies, we started the national key research and development project ＂Evidence-based optimization research of TCM therapies in prevention and treatment of CHD （angina pectoris - myocardial infarction - heart failure）＂, which is expected to be a new paradigm in the field.

Effects of Integrated Chinese Herbs and Western Medicine on Swine Toxoplasmosis

[ Objective] To evaluate the efficacy of integrated Chinese herbs and western medicine on the treatment of swine toxoplasmosis, in order to increase cure rate and lower recurrence rate. [ Method] Pigs artificially infected by Toxoplasma gondii were treated with compound suffonamide and a self-designed Chinese herbs formula, and the quantities of toxoplasma in ascites were determined. Animal regression experiment was per- formed after mice were inoculated with pathological samples of the pigs infected with toxoplasma. After treatment with integrated Chinese herbs and western medicine, the clinical manifestations of pigs were observed and the level of circulating antigen （CAg） in sera was detected. Curative effect indexes of integrated Chinese herbs and western medicine-treated group were compared with that of Chinese herbs-treated group, western medi- cine-treated group and positive control group. [ Result] All the toxoplasma trophozoites were killed by integrated and westem medicine. Regression expedment results showed that toxoplasma appeared only in one mouse that was inoculated with brain of pig. The treated pigs had nonsignificant clinical symptoms; the peak of serum CAg was lower than that of the positive control group and CAg turned negative faster than that of Chinese herbs-treated group, westem medicine-treated group, and positive control group. [ Conclusion] The integrated Chinese herbs and western medicine has good effect on the treatment of swine toxoplasmosis.

Progress in diagnosis and treatment of small cell lung cancer with integrated traditional Chinese and Western medicine

Lung cancer is one of the most common major diseases that seriously threaten human health,lung cancer includes small cell lung cancer(SCLC)and non-small cell lung cancer(NSCLC).Although patients with SCLC account for about 20%of the total number of patients with lung cancer,the mortality rate is much higher than that of patients with NSCLC.Integrated traditional Chinese and Western medicine has obvious advantages in the treatment of patients with SCLC.According to the relevant literature reports on the treatment of SCLC in recent years,this article will summarize the research progress of integrated traditional Chinese and western medicine in the treatmentof SCLC from the aspects of traditional Chinese medicine(TCM)combined with surgery,chemotherapy,radiotherapy,and molecular targeted therapy.

Clinical Experience in the Treatment of Vulvovaginal Candida Disease with Integrated Traditional Chinese and Western Medicine

Summarizing two cases of vaginal candidiasis,cured by integrated traditional Chinese and western medicine in clinical work,it is found that the combination of traditional Chinese and western medicine has a significant effect on the treatment of this disease.The curative effect is outstanding,and the recurrence rate is significantly reduced,which can be used as a reference for gynecological medical workers.

Professor YANG Yu-fei’s Experience on the Integrated Chinese and Western Medicine Treatment Strategy for Advanced Colorectal Cancer

Colorectal cancer is one of the common malignant tumors in China.It poses a serious threat to the national health of China.For advanced colorectal cancer, the main goal of treatment is to prolong survival and improve quality of life.It complements other advantages, showing good therapeutic results.However, how to grasp the timing of integrated Chinese and Western Medicine for the treatment of advanced colorectal cancer and use the integrated Chinese and Western Medicine treatment methods flexibly contains profound therapeutic art.Prof.YANG Yu-fei is an authoritative expert in the field of integrated Chinese and Western medicine for colorectal cancer.She is good at accurately grasping the timing of treatment of integrated Chinese and Western Medicine, and flexibly adjusts the treatment strategy according to the specific conditions.In this paper, we shared Professor YANG Yu-fei's strategy for treating advanced colorectal cancer with emphasis on integrated Chinese and Western Medicine and attached a typical case, with a view to provide reference for the treatment of advanced colorectal cancer with integrated Chinese and Western Medicine.

Extreme Obesity Treated with Integrated Traditional Chinese and Western Medicine and Construction of an Innovative Obesity Diagnosis and Treatment Model: A Case Report

Background:The incidence of obesity is increasing each year,and is often accompanied by multi-system metabolic disorders,typically chronic diseases and multi-disease superposition disease states.Metabolic surgery is an important treatment option for patients with extreme obesity;however,surgical difficulties and perioperative complications are more serious in these patients.Therefore,there is an urgent need for a new comprehensive diagnosis and treatment model to reduce the weight of patients prior to surgery,subsequently reducing the risks of surgery.Case summary:This model combines traditional Chinese medicine(TCM),external TCM treatment,and health management with western technique of metabolic surgery.Obesity is recognized and treated by multidisciplinary,multi-channel,and multi-means,constituting an innovative diagnosis and treatment model of obesity in integrated traditional Chinese and Western medicine.The application of this model in the treatment of a patient with extreme obesity having multiple systemic diseases is described herein.Conclusion:The integrated traditional Chinese and Western medicine obesity diagnosis and treatment model can be used for precise syndrome differentiation,individualized treatment,and follow-up management of patients with extreme obesity,with strong scalability and significant clinical efficacy.

Nursing perspective of expert consensus for diagnosis and treatment of colorectal cancer with integrated traditional Chinese and Western medicine

As one of the most common malignant tumors in the world,colorectal cancer(CRC)is accompanied by a variety of symptoms in the process of disease development and treatment,which seriously affect the functional state and quality of life of patients.In China,comprehensive intervention measures of traditional Chinese medicine(TCM)play an indispensable role in the treatment and rehabilitation of CRC.In order to further standardize and improve the treatment and management of CRC by medical staff,the Chinese Association of Integrative Medicine issued the“Expert Consensus for Diagnosis and Treatment of Colorectal Cancer with Integrated Traditional Chinese and Western Medicine”in August 2021.This paper interprets this consensus in detail from the perspective of nursing,focusing on the common understanding of the pathogenesis of CRC,TCM diagnosis and syndrome differentiation,integrated traditional Chinese and Western medicine nursing,TCM characteristic therapies,etc.,with a view to deepening the understanding and practical application of the consensus among clinical doctors and nurses and improving the level of diagnosis,treatment,and nursing of CRC.

Integrated Chinese and Western Medicine for the Treatment of Acne Associated Blepharokeratoconjunctivitis Case 1

This article reported a case of acne associated blepharokeratoconjunctivitis(BKC)in a young woman.During her pregnancy,to reduce the systemic use of drugs due to its side effect,localized eye drop,oral administration of Chinese medicine,and fumigation treatment were adopted.In integrated Chinese and Western medicine treatments,localized treatment simultaneously with the intrinsic conditioning of the patient’s physique could treat both the symptoms and root causes of disease.It can effectively improve clinical efficacy,shorten the course of the disease,and reduce patient suffering and recurrence.

Methods and New Developments in the Treatment of Hematological Diseases with Integrated Traditional Chinese and Western Medicine

Objective:To understand the methods and progress of integrated Chinese and Western medicine in the treatment of hematological diseases,and to provide ideas for integrated Chinese and Western medicine in the treatment of hematological diseases.Methods:Searching the CNKI,Wanfang and Weipu databases with"integrated traditional Chinese and Western medicine","hematological disease",and"traditional Chinese medicine"as the key words,for related literature of integrated traditional Chinese and Western medicine in the treatment of hematological disease,screening and analyzing the literature.Results:After decades of development,the methods of integrated traditional Chinese and Western medicine in the treatment of hematological diseases have been developed from initial exploration to clinical practice,from wide application to recent progress.The combination methods include the combination of Western medicine and traditional Chinese medicine,as well as the combination of Western medicine and non-medicine of traditional Chinese medicine.The combination of traditional Chinese and Western medicine includes the combination of Chinese and Western medicine,sequential and alternate methods,etc.The combination of traditional Chinese and Western medicine in the treatment of hematological diseases has significant effects in enhancing efficiency and reducing toxicity,improving the quality of life and prolonging the survival period.Conclusion:The combination of traditional Chinese and Western medicine has obvious curative effect,high safety and various methods in treating hematologic diseases.

An Expert Consensus of Integrated Chinese and Western Medicine Treatment on Lung Cancer

Lung cancer is a malignant tumor of the lung originating from the bronchial mucosa,glands or alveolar epithelium.With the population aging and environmental pollution getting increasingly severe,lung cancer has become the malignant tumor with the highest incidence and mortality rate in China,seriously endangering people’s life and health.However,there is no standardized guideline for the integrated Chinese and Western medicine treatment of lung cancer in China.In order to standardize and improve integrated Chinese and Western medicine diagnosis and treatment of lung cancer in China,the First Affiliated Hospital of Guangzhou University of Chinese Medicine,together with oncologists from the Sun Yat-sen University Cancer Center,compiled this consensus under the coordination of the Medical Policy Department of the National Administration of Traditional Chinese Medicine,taking into account the main results of the pilot project of Chinese and Western Medicine Clinical Collaboration for Major Difficult Diseases(lung cancer).

Progress on Therapy of Infertility by Integrated Traditional Chinese and Western Medicine

Infertility is one of the difficult complicated diseases. Many couples suffer from it. The pathogenesis is very complicated. The imbalance or lesson of any link of the reproductive system can cause infertility. This paper summarizes the treatment of female infertility by integrated Traditional Chinese Medicine (TCM) and Western Medicine (WM) which can not only improve the ovulation rate and pregnancy rate, but also decrease the complications. The effects are better than that by TCM or WM only. Therefore, the coupling method is worth to be used widely in clinical practice.

Clinical Controlled Study of Integrative Chinese and Western Medicine in Treating 49 Cases of SARS

Objective: To evaluate the efficacy of integrative Chinese and western medicine (ICWM) in treating severe acute respiratory syndrome (SARS) patients. Methods: Through parallel control design, 49 SARS patients were observed. Used as control, there were in the western medicine (WM) group 29 patients, who were treated with Ribavirin, Levofloxacin, Thymopentin, Azithromycin, methylprednisolone, etc. , on the basis of 'Recommended Protocol for Infectious Atypical Pneumonia' (abbreviated as 'Recommended Protocol') issueed by Ministry of Health. As the treated group, there were in the ICWM 20 cases. The protocol for treatment of SARS in 'Special Science and Technological Action to Prevent and Treat SARS' (abbreviated as 'Special S-T Action'), issued by Ministry of Science and Technology, together with the same WM as those for the control group. Results: (1) Time from the disease onset to the symptom improvement were 5.10±2. 83 days and 7. 62±2. 27 days in ICWM and WM group respectively, P<0.05; (2) As to corticosteroid (CS) amount and days before reducing dosage, 2 groups showed no significant difference, P>0. 05;(3) There was no significant difference in the time from disease onset to the body temperature normalization and the total amount of CS and the duration of using CS before reducing it to 80 mg between the ICWM group and the WM group; (4) The days and amounts for use CS after reducing between the ICWM group and the WM group were significantly different (P<0. 05). Conclusion: There were obvious advantages in ICWM to treat SARS, compared with that of WM alone, especially in improving the clinical symptoms, promoting the recovery of immune function, promoting the absorption of pulmonary inflammation and reducing the dosage and duration of CS treatment.

Therapeutic Effect of Integrative Traditional Chinese and Western Medicine on 51 SARS Patients and Its Influence on Their TLymphocyte Subsets

Objective: To observe the clinical effect of integrative Chinese a nd western medicine (ICWM) in treatment of patients with severe acute respirator y syndrome (SARS) and its influence on their T lymphocyte subsets.Methods: Fifty one patients with SARS of severe type were obser ved with synchronous non randomized controlled method. They were divided into the ICWM group (29 patients) and the western medicine (WM) group (22 patients). Western medical treatment was applied to both groups, but to the ICWM group, Ch inese medicine was given additionally. The therapeutic course was 2-3 weeks for both groups. Clinical effect and changes of T lymphocyte subsets (CD4 +) aft er treatment were observed.Results: In the ICWM group, 26 patients (89.66%) were cured and 3 (10.34%) died, while in the WM group, 12 (54.55%) cur ed and 10 (45.45%) died, thus comparison of the cure rate between the two groups showing significant difference ( P <0.01). The score of clinical symptoms in the ICWM group was decreased from 7.14±5.20 scores before treatment to 1.82±3. 75 scores after treatment, while in the WM group, it lowered from 7.36±3.84 sco res before treatment to 5.17±4.17 scores after treatment, significant diffe rence shown in the comparison of the values between the two groups after treatme nt (P<0.01). Immunological function test showed that CD4 + T lymphocyte in the ICWM group rose from 361±278 cells/mm 3 before treatment to 630±454 c ells/mm 3 after treatment, showing significant difference( P <0.01 );bu t in the WM group, it merely rose from 288±186 cells/mm 3 to 376±285 cells/mm 3 in the corresponding period (P>0.05). Conclusion: ICWM could improve the clinical symptoms of SARS pa tients markedly, enhance their T lymphocyte immune function, and reduce their mortality.

Diabetic peripheral vascular disease(PVD)is one of the common chronic complications of diabetes.The main clinical manifestations of PVD are numbness and coldness of the limbs,resting pain,intermittent claudication,and other symptoms.The combined treatment involving Chinese and Western medicine for PVD has various clinical methods and definite curative effects.It is worthy of in-depth clinical research and application.

Progress in the treatment of ovarian cancer with traditional Chinese medicine and western medicine

Ovarian cancer is one of the female genital malignant tumors. Because its early clinical symptoms axe not obvious and there is no specific early diagnosis method, about 70% of patients are in advanced stage （stage III-IV） at the time of diagnosis. Ovarian cancer mortality is the highest in female reproductive system tumor. According to the 2017 edition of the National Comprehensive Cancer Network （NCCN） published in the ＂Ophthalmic Cancer Clinical Practice Guide＂, the main treatment plan is surgery-based, postoperative adjuvant chemotherapy and other comprehensive treatment. Ovarian cancer is similar to ＂Jia Ju＂ in traditional Chinese medicine. In recent years, traditional Chinese medicine has been widely used in clinical practice. Postoperative patients with Chinese medicine, acupuncture, et al. promote rapid recovery, and traditional Chinese medical methods can also reduce the side effects caused by radiotherapy and chemotherapy.

Research Progress of Pulmonary Sarcoidosis in Traditional Chinese Medicine and Western Medicine

Objective:To summarize the current treatments of traditional Chinese medicine and Western medicine and to explore ideas for clinical treatments,rational use of traditional Chinese medicine,avoid medication errors,and make full use of the respective advantages of traditional Chinese medicine and Western medicine to achieve good results.Methods:We selected recent articles on pulmonary sarcoidosis and clinical observation at home and abroad,and summarized relevant traditional Chinese medicine books.Results:At present,hormone therapy is preferred in western medicine,followed by antimetabolic drugs,Anti-TNF,and also potential drug therapeutic strategies,such as"CLEAR"regimen and new target therapy.Traditional Chinese medicine through syndrome differentiation,from the whole,flexible use of dispel evil,phlegm,blood circulation,dredging collaterals,qi and other methods to achieve the effect of dissolution.Conclusion:The treatment research results of pulmonary sarcoidosis are considerable,but there are still many risk factors in Western medicine,and the benefits and treatment indications of individual drugs are still under study.Traditional Chinese medicine has certain safety and efficacy in the prevention and treatment of pulmonary sarcoidosis,but it needs modern medical technology examination.Therefore,the advantages of traditional Chinese and Western medicine should be fully combined to achieve the maximum treatment effects.

Guidelines for diagnosis and treatment of hepatic fibrosis with integrated traditional Chinese and Western medicine(2019 edition)

In 2006, the Hepatology Committee of Chinese Association of Integrative Medicine issued the "Guidelines for the Prevention and Treatment of Liver Fibrosis with Integrated Traditional Chinese and Western Medicine." In recent years, the fields of Chinese medicine, Western medicine, and integrative medicine have made rapid advances in basic and clinical research into chronic liver disease, and accumulated new evidence for the prevention and treatment of hepatic fibrosis. Therefore, in order to meet clinical needs, liver disease experts of integrated traditional Chinese and Western medicine were united to revise the previous guidelines in order to help physicians make correct and reasonable decisions in the diagnosis and treatment of hepatic fibrosis.

Traditional Chinese medicine combined with low-dose glucocorticoid for treating nephrotic syndrome: A case report

Nephrotic syndrome(NS)is a common refractory clinical disease that can develop into end-stage renal disease(ESRD)if cannot be controlled.At present,in Western medicine,glucocorticoid or immunosuppressant drugs with heavy dose and long course are used for the therapy of NS.There are still few reports on the combined treatment of traditional Chinese medicine and low-dose glucocorticoid for NS.Here,we reported a case of a 48-year-old man diagnosed with NS,accompanied by severe pulmonary infection,abundant proteinuria,and pleural effusion.Integrated traditional Chinese and Western medicine was used as the therapeutic regimen of this patient.Traditional Chinese medicine(TCM)mainly focused on invigorating spleen and kidney,promoting blood circulation,removing blood stasis and promoting diuresis,and Chinese patent medicines or TCM immunosuppressants were given to consolidate the treatment.In Western medicine,small dose and short course glucocorticoids combined with symptomatic treatment were adopted.Delightfully,proteinuria subsided after 5 months treatment with integrated TCM and Western medicine and all laboratory reports were negative after 10 months,which has been normal so far(July 2019).This case report indicates that the combination of TCM and low-dose glucocorticoid is effective in curing NS,with low recurrence rate and few side effects.

Multicenter clinical efficacy observation of integrated Traditional Chinese Medicine-Western Medicine treatment in acute onset period of pulmonary heart disease

OBJECTIVE: To evaluate the efficacy of integrated Traditional Chinese Medicine-Western Medicine(TCM-WM) in the treatment of acute onset pulmonary heart disease(PHD).METHODS: A total of 240 patients met the inclusion criteria and were enrolled. These inpatients were divided into group A(treatment group) and B(control group) in order of admission according to the principles of randomization and control. The research was performed simultaneously in three hospitals. Two groups were given basic treatment that included: controlled oxygen therapy, active and effective anti-infection, maintaining airway patency,correcting O_2 deficiency and CO_2 retention, correcting acid-base imbalance and electrolyte disturbance, reducing pulmonary hypertension and treating right heart failure, nutritional support and treatment of complications. Group A was given basic treatment and integrated Traditional Chinese Medi-cine(TCM) differentiating therapy; group B was given basic therapy and a placebo that was similar in appearance and taste to TCM medicinal broth of pharmaceutical preparations, provided by Yibin Pharmaceutical Company(Yibin, China, Wuliangye Group).RESULTS: The mortality in the treatment group decreased by 4.98% compared with the control group. The treatment group reported improved ventilation, corrected hypoxemia, improved nutritional status and promoted digestive functions. It also significantly improved the patient's self-life skills, improved the patient's quality of life and could shorten the length of hospital stay.CONCLUSION: Comprehensive integrated TCMWM treatment showed good clinical efficacy toward the acute onset period of PHD patients.

Thoughts and Approaches of Integrated Chinese and Western Medicine for the Treatment of Oligo-astheno-teratozoospermia

Oligo-astheno-teratozoospermia（OAT） which perplexes doctors and patients due to its complicated etiology, atypical symptoms and poor clinical efficacy, is a general term for the three pathological states of abnormal semen. OAT is the main factor of male infertility. It is also a hot and difficult point in present studies. Empiric drug is the most popular treatment of this disease in the modern medicine. Chinese medicine（CM） is one of the main methods for the treatment of this disease, and it has certain clinical effect. The author believes that the use of modern medical technology to make the diagnosis as clear as possible is the key to treat OAT. The combination of syndrome and disease differentiation is the main mode in the treatment of OAT. Microscopic syndrome differentiation and macroscopic evidence embodies the basic principle of ＂Si Wai Chuai Nei＂ and broadens the perspective of CM syndrome differentiation. Classification and treatment are basic methods for the treatment of OAT. The treatment should not be limited to the Shen（Kidney）, instead it should focus on the whole body condition. At different stages, the treatment should also pay attention to strengthening the Pi（Spleen）, nourishing the Gan（Liver） and promoting blood circulation. Complementing Chinese and Western medicine, and highlighting the characteristics and advantages of CM treatment, have a great guiding value for the treatment of OAT.
