Postdural puncture headache(PDPH)is an incapacitating complication that can occur following spinal anesthesia and with inadvertent dural puncture during epidural anesthesia.We present a case of a 32-year-old G2 P1 female who was admitted for induction of labor and received epidural catheter placement for analgesia.After an inadvertent dural puncture and development of a PDPH,the patient was offered conservative measures for the first 48 hours without improvement.An epidural blood patch(EBP)was placed achieving only moderate relief.Two days later,a second EBP was performed and the patient developed severe back pain which radiated bilaterally to her buttocks.Magnetic resonance imaging(MRI)demonstrated the presence of blood in the intrathecal space.This could be the cause of sacral radiculitis,an uncommon complication of an EBP.This suggests that EBPs could potentially cause neurologic symptoms which may be more common than people previously thought.As complicated outcomes have followed both conservative and aggressive management,MRI can be an early diagnostic tool in such cases and a multidisciplinary approach should be taken. 展开更多

Experimental study of Danshen injection on lumbar radiculitis in rats

Objective: To investigate the therapeutic effect and mechanism of Danshen injection on lumbar radiculitis in rats. Methods: A total of 60 Wistar rats were randomly divided into normal group, model group, Danshen group... Objective: To investigate the therapeutic effect and mechanism of Danshen injection on lumbar radiculitis in rats. Methods: A total of 60 Wistar rats were randomly divided into normal group, model group, Danshen group, and control group, with 15 in each group. After the lumbar radiculitis model was established, the model group, the Danshen group and the control group were treated with saline, Danshen injection and Adenocobalamin injection (once a day for 2 weeks), respectively. The normal group was without any intervention. On the 1st, 7th, 14th and 30th day after the operation, the gait and tactile scores of rats were measured, and electrophysiology was measured on the 30th day. The plasma levels of β-endorphin and adrenocorticotropic hormone were measured by enzyme-linked immuno sorbent assay on the 7th,14th and 30th day. Results: Compared with the model group, the gait and tactile scores, the β-endorphin content in plasma and nerve conduction velocity of the Danshen group and the control group were increased (P<0.05), and the adrenocorticotropic hormone content was decreased (P<0.05). The Danshen group was slightly better than the control group (P<0.05). Conclusions: Danshen injection is safe and effective in the treatment of lumbar radiculitis in rats. It may play a therapeutic role by regulating the expression ofβ-endorphin and adrenocorticotropic hormone. 展开更多
