Real-time Three-dimensional Echocardiographic Assessment of Left Ventricular Remodeling Index in Patients with Hypertensive Heart Disease and Coronary Artery Disease 被引量:15

Left ventricular remodeling index (LVRI) was assessed in patients with hypertensive heart disease (HHD) and coronary artery disease (CAD) by real-time three-dimensional echocardiography (RT3DE). RT3DE data of 18 patients with HHD, 20 patients with CAD and 22 normal controis (NC) were acquired. Left ventricular end-diastolic volume (EDV) and left ventricular end-diastolic epicardial volume (EDVepi) were detected by RT3DE and two-dimensional echocardiography Simpson biplane method (2DE). LVRI (left ventricular mass/EDV) was calculated and compared. The results showed that LVRI measurements detected by RT3DE and 2DE showed significant differences inter-groups (P〈0.01). There was no significant difference in NC group (P〉0.05), but significant difference in HHD and CAD intra-group (P〈0.05). There was good positive correlations between LVRI detected by RT3DE and 2DE in NC and HHD groups (t=0.69, P〈0.01; r=0.68, P〈0.01), but no significant correlation in CAD group (r=0.30, P〉0.05). It was concluded that LVRI derived from RT3DE as a new index for evaluating left ventricular remodeling can provide more superiority to LVRI derived from 2DE. 展开更多

The optimal plane for measurement of the right ventricular (RV) volumes by real-time three-dimensional echocardiography (RT3DE) was determined and the feasibility and accuracy of RT3DE in studying RV systolic function was assessed. RV “Full volume” images were acquired by RT3DE in 22 healthy subjects. RV end-diastolic volumes (RVEDV) and end-systolic volumes (RVESV) were outlined using apical biplane, 4-plane, 8-plane, 16-plane offline separately. RVSV and RVEF were calculated. Meanwhile tricuspid annual systolic excursion (TASE) was measured by M-mode echo. LVSV was outlined by 2-D echo according to the biplane Simpson's rule. The results showed: (1) There was a good correlation between RVSV measured from series planes and LVSV from 2-D echo (r=0.73; r=0.69; r=0.63; r=0.66, P<0.25—0.0025); (2) There were significant differences between RVEDV in biplane and those in 4-, 8-, 16-plane (P<0.001). There was also difference between RV volume in 4-plane and that in 8-plane (P<0.05), but there was no significant difference between RV volume in 8-plane and that in 16-plane (P>0.05); (3) Inter-observers and intro-observers variability analysis showed that there were close agreements and relations for RV volumes (r=0.986, P<0.001; r=0.93, P<0.001); (4) There was a significantly positive correlation of TASE to RVSV and RVEF from RT3DE (r=0.83; r=0.90). So RV volume measures with RT3DE are rapid, accurate and reproducible. In view of RV's complex shape, apical 8-plane method is better in clinical use. The recently introduced real-time three-dimensional color Doppler flow imaging (RT-3D CDFI) technique provides a quick and accurate calculation of regurgitant jet volume (RJV) and fraction. In order to evaluate RT-3D CDFI in the noninvasive assessment of aortic RJV and regurgitant jet fraction (RJF) in patients with isolated aortic regurgitation, real-time three-dimensional echocardiographic studies were performed on 23 patients with isolated aortic regurgitation to obtain LV end-diastolic volumes (LVEDV), end-systolic volumes (LVESV) and RJV, and then RJF could be calculated. The regurgitant volume (RV) and regurgitant fraction (RF) calculated by two-dimensional pulsed Doppler (2D-PD) method served as reference values. The results showed that aortic RJV measured by the RT-3D CDFI method showed a good correlation with the 2D-PD measurements (r= 0.93, Y=0.89X+ 3.9, SEE= 8.6 mL, P〈0.001 ); the mean (SD) difference between the two methods was - 1.5 (9.8) mL. % RJF estimated by the RT-3D CDFI method was also correlated well with the values obtained by the 2D-PD method (r=0.88, Y=0.71X+ 14.8, SEE= 6.4 %, P〈0. 001); the mean (SD) difference between the two methods was -1.2 (7.9) %. It was suggested that the newly developed RT-3D CDFI technique was feasible in the majority of patients. In patients with eccentric aortic regurgitation, this new modality provides additional information to that obtained from the two-dimensional examination, which overcomes the inherent limitations of two-dimensional echocardiography by depicting the full extent of the jet trajectory. In addition, the RT-3D CDFI method is quick and accurate in calculating RJV and RJF. 展开更多

The present study evaluated the application of three dimensional echocardigraphy (3DE) in the diagnosis of atrial septal defect (ASD) and the measurement of its size by 3DE and compared the size with surgical findings. Two-dimensional and real-time three dimensional echocardiography (RT3DE) was performed in 26 patients with atrial septal defect, and the echocardiographic data were compared with the surgical findings. Significant correlation was found between defect diameter by RT3DE and that measured during surgery (r=0.77, P〈0.001). The defect area changed significantly during cardiac cycle. Percentage change in defect size during cardiac cycle ranged from 6%-70%. Our study showed that the size and morphology of atrial septal defect obtained with RT3DE correlate well with surgical findings. Therefore, RT3DE is a feasible and accurate non-invasive imaging tool for assessment of atrial septal size and dynamic changes. 展开更多

The application of real-time three-dimensional echocardiography (RT 3DE) in the diagnosis of double orifice mitral valve (DOMV) was explored. Five cases of DOMV were examined by using 2-dimensional echocardiography (2DE) and RT 3DE. The spatial morphology of malformed mitral valve and its change in hemodynamics were observed. DOMV associated with partial atrioventricular septal defect was found in 3 cases (in which 2 cases had cleft mitral valve) and isolated DOMV in 2 cases; and moderate to severe mitral regurgitation was detected in 3 cases, and mild mitral regurgitation in 1, and no regurgitation in 1 case; 1 case had complicated rhumatic heart disease. Three cases were preoperatively discovered by 2DE, while 2 missed (1 case was discovered postoperatively). Four cases were diagnosed by RT 3DE preoperatively, and 1 case was diagnosed postoperatively (not examined by RT 3DE preoperatively). It was suggested that RT 3DE is a reliable technique in the diagnosis of DOMV; it permitted comprehensive and noninvasive assessment of mitral valve and may supplement 2D TTE in the assessment of DOMV. 展开更多

The volume-time curve change in patients with normal left ventricular (LV) diastolic function and diastolic dysfunction was evaluated by real-time three-dimensional echocardiography (RT3DE). LV diastolic dysfunction was defined by E'〈A' in pulse-wave tissue Doppler for inter-ventricular septal (IVS) of mitral annulus. In 24 patients with LV diastolic dysfunction, including 12 patients with delayed relaxation (delayed relaxation group) and 12 patients with pseudo-normal function (pseudo-normal group) and 24 normal volunteers (control group), data of full-volume image were acquired by real-time three-dimensional echocardiography and subjected to volume-time curve analysis. EDV (end-diastolic volume), ESV (end-systolic volume), LVEF (left ventricular ejection fraction), PER (peak ejection rate), PFR (peak filling rate) from RT3DE were examined in the three groups. Compared to the control group, PFR (diastolic filling index of RT3DE) was significantly reduced in the delayed relaxation group and pseudo-normal group (P〈0.05). There were no significant differences in EDV, ESV, LVEE PER (P〉0.05). It is concluded that PFR, as a diastolic filling index of RT3DE, can reflect the early diastolic function and serve as a new non-invasive, quick and accurate tool for clinical assessment of LV diastolic function. 展开更多

Real-time crash prediction on freeways using data mining and emerging techniques 被引量:4

Recent advances in intelligent transportation system allow traffic safety studies to extend from historic data-based analyses to real-time applications. The study presents a new method to predict crash likelihood with traffic data collected by discrete loop detectors as well as the web-crawl weather data. Matched case-control method and support vector machines (SVMs) technique were employed to identify the risk status. The adaptive synthetic over-sampling technique was applied to solve the imbalanced dataset issues. Random forest technique was applied to select the contributing factors and avoid the over-fitting issues. The results indicate that the SVMs classifier could successfully classify 76.32% of the crashes on the test dataset and 87.52% of the crashes on the overall dataset, which were relatively satisfactory compared with the results of the previous studies. Compared with the SVMs classifier without the data, the SVMs classifier with the web-crawl weather data increased the crash prediction accuracy by 1.32% and decreased the false alarm rate by 1.72%, showing the potential value of the massive web weather data. Mean impact value method was employed to evaluate the variable effects, and the results are identical with the results of most of previous studies. The emerging technique based on the discrete traffic data and web weather data proves to be more applicable on real- time safety management on freeways. 展开更多

目的:分析不孕症女性经阴道实时三维超声子宫输卵管造影(real-time three-dimensional hysterosalpingo-contrast sonography,RT-3D-HyCoSy)时发生造影剂逆流的影响因素,为造影剂逆流防治提供参考。方法:收集2022年1月至2023年9月于我... 目的:分析不孕症女性经阴道实时三维超声子宫输卵管造影(real-time three-dimensional hysterosalpingo-contrast sonography,RT-3D-HyCoSy)时发生造影剂逆流的影响因素,为造影剂逆流防治提供参考。方法:收集2022年1月至2023年9月于我院接受RT-3D-HyCoSy检查的125例不孕症患者的病例资料,根据是否发生造影剂逆流将其分为造影剂逆流组(n=21)及造影剂未逆流组(n=104),单因素及多因素Logistic回归分析RT-3D-HyCoSy时发生造影剂逆流的危险因素。结果:造影剂逆流组患者不孕类型为继发、有宫内手术史、有子宫腺肌症、有异位妊娠史、有附件包块、有子宫畸形、中重度造影剂推注压力、子宫内膜厚度≤3.5mm、输卵管阻塞、造影时间为经期后≤5d的占比高于造影剂未逆流组,差异均有统计学意义(P均<0.05);继发型不孕(OR=2.088,95%CI=1.119~3.893,P=0.021)、有异位妊娠史(OR=2.061,95%CI=1.136~3.739,P=0.018)、子宫内膜厚度≤3.5mm(OR=2.319,95%CI=1.186~4.533,P=0.014)、输卵管阻塞(OR=2.109,95%CI=1.259~3.531,P=0.005)为不孕症女性RT-3D-HyCoSy时出现造影剂逆流的危险因素(P<0.05)。结论:继发型不孕、异位妊娠史、子宫内膜厚度≤3.5mm及输卵管阻塞是不孕症女性RT-3D-HyCoSy检查时发生造影剂逆流的危险因素;临床需重点关注合并上述危险因素不孕症RT-3D-HyCoSy检查时造影剂逆流的预防。 展开更多

目的探讨实时三维超声心动图(RT-3DE)参数与射血分数保留心衰(HFpEF)患者血清可溶性致癌抑制因子2(sST2)水平及心功能相关性。方法选取80例HFpEF患者作为HFpEF组,依据美国纽约心脏病协会(NYHA)分级分为:Ⅱ级(14例)、Ⅲ级(48例)、Ⅳ级(18... 目的探讨实时三维超声心动图(RT-3DE)参数与射血分数保留心衰(HFpEF)患者血清可溶性致癌抑制因子2(sST2)水平及心功能相关性。方法选取80例HFpEF患者作为HFpEF组,依据美国纽约心脏病协会(NYHA)分级分为:Ⅱ级(14例)、Ⅲ级(48例)、Ⅳ级(18例),另选择100例同期于我院体检的健康志愿者作为对照组,采用酶联免疫吸附测定法检测所有患者的血清sST2水平,所有患者均行常规超声心动图和RT-3DE检查,记录左心室6、12、16节段到达收缩期最小容积时间最大差异值标准化值(TmsvDif%)、标准差标准化值(TmsvSD%)和左心室射血分数(LVEF)、左心室收缩末容积(LVESV)、左心室舒张末容积(LVEDV);分析各RT-3DE参数与血清sST2水平及心功能之间的相关性。结果不同心功能分级HFpEF患者LVEF均明显低于对照组,且心功能等级越高LVEF越低,差异有统计学意义(P<0.05);不同心功能分级HFpEF患者LVEDV、LVESV明显高于对照组,且心功能等级越高LVEDV、LVESV越高,差异有统计学意义(P<0.05);不同心功能分级HFpEF患者Tmsv 6-SD%、Tmsv12-SD%、Tmsv16-SD%及Tmsv 6-Dif%、Tmsv 12-Dif%、Tmsv 16-Dif%较对照组均明显延长,且心功能等级越高,患者左心室6、12、16节段TmsvSD%和TmsvDif%时间越长,差异有统计学意义(P<0.05);不同NYHA分级HFpEF血清sST2表达水平高于对照组,且心功能分级较高的HFpEF患者sST2水平明显增高,差异有统计学意义(P<0.05)。相关性分析结果显示,RT-3DE相关参数LVEDV、LVESV、Tmsv6-SD%、Tmsv12-SD%、Tmsv16-SD%、Tmsv 6-Dif%、Tmsv 12-Dif%、Tmsv 16-Dif%均与NYHA心功能分级存在显著正相关性(r=0.687,0.752,0.501,0.565,0.581,0.522,0.596,0.603;P<0.05),LVEF与NYHA心功能分级存在负相关性(r=-0.731,P<0.05)。LVEDV、LVESV与血清sST2表达存在显著正相关性(r=0.335,0.356;P均<0.05),LVEF与血清sST2、心功能分级存在负相关性(r=-0.313,P<0.05)。结论RT-3DE能够有效监测HFpEF患者左心房容积功能和左心室舒张功能变化情况,RT-3DE相关参数与心功能分级密切相关,对于心衰患者程度评估具有重要价值。 展开更多

实时三维超声心动图与二维斑点追踪技术对PD-1免疫抑制剂诱发早期心肌损伤的评估

目的 应用实时三维超声心动图(RT-3DE)与二维斑点追踪技术(2D-STI)评估PD-1/PD-L1免疫检查点抑制剂(ICIs)治疗期间患者左室收缩功能变化,探讨其在评估ICIs诱发早期心肌损伤中的应用价值。方法 选择2021年3月—2022年9月在本院接受ICIs... 目的 应用实时三维超声心动图(RT-3DE)与二维斑点追踪技术(2D-STI)评估PD-1/PD-L1免疫检查点抑制剂(ICIs)治疗期间患者左室收缩功能变化,探讨其在评估ICIs诱发早期心肌损伤中的应用价值。方法 选择2021年3月—2022年9月在本院接受ICIs治疗的32例肿瘤患者作为研究对象。患者在治疗前(T0)、治疗后1个周期(T1)、2个周期(T2)、3个周期(T3)、4个周期(T4)内进行实验室及经胸超声心动图检查。记录患者治疗前后心脏标记物、左心室常规超声心动图参数、RT-3DE参数变化、2D-STI参数变化。结果 在整个随访期间未观察到心脏标记物、血压、心率、心室腔大小的显著变化。从表中可以看到与T0相比,CK、CK-MB、LDH、BNP在T1~T2时有升高趋势,在T3、T4趋于稳定,差异无统计学意义(P>0.05)。所有研究对象的LVEF均>50%。与T0相比,接受PD-1抑制剂治疗后LVGLS降低,并且在T1~T3期上述变化已明显,T4时显著;T0~T4期间LVGCS差异无统计学意义(P>0.05)。T16-SD/R-R(%)、T16-Dif/R-R(%);T12-SD/R-R(%)、T12-Dif/R-R(%);T6-SD/R-R(%)、T6-Dif/R-R(%)在T1~T3期升高,T4期显著升高,差异有统计学意义(P<0.001)。STI联合RT-3DE是预测免疫抑制剂相关早期亚临床心脏毒性的最佳方法,其诊断敏感性为91.67%,特异性为85%。结论 应用二维斑点追踪与实时三维超声心动图对无明显LVEF降低的ICIs诱发早期心肌损伤具有较高的诊断价值。 展开更多

RT-3DE评估透析类型对尿毒症患者左心房结构与功能的影响

目的探讨应用实时三维超声心动图(real time three-dimensional echocardiography,RT-3DE)从容积变化的角度观察比较不同透析方式对尿毒症患者左心房结构和功能的影响。方法选取在我院就诊的尿毒症患者81例。其中慢性肾脏病5期未透析组(... 目的探讨应用实时三维超声心动图(real time three-dimensional echocardiography,RT-3DE)从容积变化的角度观察比较不同透析方式对尿毒症患者左心房结构和功能的影响。方法选取在我院就诊的尿毒症患者81例。其中慢性肾脏病5期未透析组(CKD5组)20例,腹膜透析组(PD组)30例,血液透析组(HD组)31例。应用常规超声心动图及RT-3DE技术对3组进行检查,比较各组间左心房结构及功能的差异。结果常规超声心动图检查显示各组左心室舒张末期内径、室间隔厚度、左心室质量指数、左心室射血分数等参数三组间比较差异均无统计学意义(均P>0.05);左心室舒张功能障碍、左心室肥厚的发生率在各组差异无统计学意义(均P>0.05)。CKD5组和PD组、CKD5组和HD组间左心房容积参数LAVmax、LAVmin、LAVp以及LAVI差异均无统计学意义(均P>0.05)。PD组左心房容积参数LAVmax、LAVmin均明显小于HD组,差异有统计学意义(P=0.030、P<0.001)。HD组和PD组的容积参数LAVp差异无统计学意义(P>0.05)。PD组LAVI较HD组更低,差异有统计学意义(P=0.024)。PD组功能参数LAEF明显高于HD组(P<0.001),亦高于CKD5组但差异无统计学意义(P>0.05)。PD组和CKD5组功能参数LAAEF均显著高于HD组,测量结果间的差异有统计学意义(P<0.05)。各组间的LAPEF差异无统计学意义(P=0.780)。结论实时三维超声心动图可简便、准确地评价不同透析方式的尿毒症患者左心房结构改变和功能差异。PD组和HD组无法阻止左心房重构的发生,但PD组相对于HD组可能在保留和维持左心房结构和功能方面略好。 展开更多

目的:探讨经食管实时三维超声心动图(RT-3D-TEE)、CT血管造影(CTA)以及脑卒中危险(CHA_(2)DS_(2)-VASc)评分三者联合对非瓣膜性房颤患者左心耳血栓形成的预测价值。方法:选取2022年1月至2023年6月沧州市人民医院心血管内科收治的88例非... 目的:探讨经食管实时三维超声心动图(RT-3D-TEE)、CT血管造影(CTA)以及脑卒中危险(CHA_(2)DS_(2)-VASc)评分三者联合对非瓣膜性房颤患者左心耳血栓形成的预测价值。方法:选取2022年1月至2023年6月沧州市人民医院心血管内科收治的88例非瓣膜性房颤患者,根据是否发生左心耳血栓将其分为血栓组(36例)和无血栓组(52例)。所有患者均接受RT-3D-TEE和CTA检查,并进行CHA_(2)DS_(2)-VASc评分。logistic回归分析非瓣膜性房颤患者左心耳形成相关因素。绘制受试者工作特征(ROC)曲线分析RT-3D-TEE、CTA及CHA_(2)DS_(2)-VASc对非瓣膜性房颤患者左心耳血栓形成的预测价值,计算ROC曲线下面积(AUC)、灵敏度及特异度。结果:血栓组左心耳最大排空流速(LAAeV)为(51.48±5.87)、左心耳最大充盈速度(LAAfV)为(48.36±5.49)及三维容积法射血分数(3D-EF)(39.26±3.24)均小于无血栓组,差异具有统计学意义(t=5.933、6.767、4.605,P<0.05)。血栓组左心耳开口长径、短径及周长,以及容积、面积均大于无血栓组,射血分数小于无血栓组,差异具有统计学意义(t=6.581、3.410、9.220、3.178、4.453、4.162,P<0.05)。血栓组菜花型和仙人掌型占比大于无血栓组,风向标型占比和鸡翅型占比小于无血栓组,差异具有统计学意义(χ^(2)=3.771、7.132、1.003、5.455,P<0.05)。血栓组CHA_(2)DS_(2)-VASc评分显著高于无血栓组,差异具有统计学意义(t=12.269,P<0.05)。LAAeV、LAAfV、3D-EF、开口长径、开口短径、开口周长、容积、射血分数、空间形态以及CHA_(2)DS_(2)-VASc评分均与非瓣膜性房颤患者左心耳血栓形成显著相关(OR=4.323~19.562,P<0.05)。RT-3D-TEE、CTA及CHA_(2)DS_(2)-VASc评分三者联合预测非瓣膜性房颤患者左心耳血栓形成的灵敏度、特异度及AUC分别为93.48%、90.83%和0.895(0.654~0.963)。结论:RT-3D-TEE、CTA及CHA_(2)DS_(2)-VASc评分对非瓣膜性房颤患者左心耳血栓形成的预测价值均较高,其中两项联合和三项联合均可有效提高单项检测的预测效能,临床可根据实际情况进行选择。 展开更多

目的 探讨实时三维超声心动图联合血浆NT-proBNP评价不同QRS波时限心力衰竭患者左室收缩同步性的临床价值和可行性.方法 入选74例慢性心衰患者设为病例组,分为A组(QRS≥120 ms,46例)、B组(QRS< 120 ms,28例),再根据实时三维超声心... 目的 探讨实时三维超声心动图联合血浆NT-proBNP评价不同QRS波时限心力衰竭患者左室收缩同步性的临床价值和可行性.方法 入选74例慢性心衰患者设为病例组,分为A组(QRS≥120 ms,46例)、B组(QRS< 120 ms,28例),再根据实时三维超声心动图结果分为A1组(运动同步组)、A2组(运动不同步组),B1组(运动同步组)、B2组(运动不同步组);入选45名同期在我院体检的健康人设为对照组(C组).研究对象均行心电图、实时三维超声心动图及NT-proBNP等检查,记录各组的左室射血分数(LVEF)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV).应用Qlab(5.0)定量分析软件,描绘左室容量—时间曲线,并计算出左室16节段、12节段收缩期容积达峰时间的标准差(Tmsv-SD)、最大差异(Tmsv-Dif)及其标准化值(Tmsv-SD% 、Tmsv-Dif%).采用酶联免疫法检测各组NT-proBNP,常规体表心电图记录QRS时限.结果 ①三组一般资料,以及A、B两组用药情况、伴随疾病等比较未见统计学差异(P>0.05);三组QRS比较,A组(137.7±9.9)ms、B组(111.8±7.6)ms与C组(90.5±9.6)ms比较明显增宽,差异有统计学意义(P<0.05),A、B两组比较,差异也有统计学意义(P<0.05);A、B两组LVEF、LVEDV、LVESV与C组比较差异均有统计学意义.②心衰组存在心脏收缩不同步,左室各节段运动曲线排列紊乱,达峰时间不一;C组心脏运动均同步,左室各节段运动曲线形态协调一致,达峰时间一致.心衰组收缩同步性参数与C组比较,差异均有统计学意义(P<0.05).③A组中33例患者(71.7%)收缩不同步,B组中11例患者(39.3%)收缩不同步,C组心脏运动均呈同步性.④心衰组NT-proBNP水平明显高于C组,差异有统计学意义(P<0.05),A2及B2两组患者的NT-proBNP水平分别高于A1 、B1组,差异有统计学意义(P<0.05),A1、B1组间及A2、B2组间比较差异无统计学意义,与实时三维超声心动图反映的心脏运动情况有较好的相关性.结论 ①实时三维超声心动图通过在同一个心动周期内同步显示左心室壁的各个节段,敏感地反映各个节段容量的时间变化来评估心肌运动的同步性.②无论心衰患者QRS波时限增宽与否,均有一定比例患者存在心脏运动不同步现象.③左室运动不同步心衰患者NT-proBNP血浆浓度较高,NT-proBNP与实时三维超声心动图反映的心脏运动情况有较好的相关性.④RT-3DE联合NT-proBNP可以更好地评估心力衰竭患者左室运动同步性,帮助临床医生在CRT术前筛选合适患者,从而提高CRT疗效. 展开更多

目的通过经胸实时三维超声心动图(RT3D-TTE)评价急性心肌梗死(AMI)伴中度二尖瓣反流患者经皮冠状动脉介入治疗(PCI)术前后二尖瓣构型变化,预测PCI术后患者二尖瓣返流改善情况。方法选取保定市第一中心医院AMI伴中度二尖瓣反流患者行PCI... 目的通过经胸实时三维超声心动图(RT3D-TTE)评价急性心肌梗死(AMI)伴中度二尖瓣反流患者经皮冠状动脉介入治疗(PCI)术前后二尖瓣构型变化,预测PCI术后患者二尖瓣返流改善情况。方法选取保定市第一中心医院AMI伴中度二尖瓣反流患者行PCI术治疗患者90例,均接受RT3D-TTE检查。根据术后3个月患者的二尖瓣反流改善情况将其分为未改善组(n=37)和改善组(n=53),采集两组的二尖瓣参数变化,比较治疗前后二尖瓣反流的改善效果。采用logistic回归法筛查影响PCI术后反流未改善情况的相关参数,预测改善效能。结果未改善组术后的瓣环高度(AH)、幕状区容积(TV)、瓣环三维面积(AA3D)、瓣环联合处直径(CD)、瓣环周长(AC)和瓣环前外侧至后内侧直径(AL-PM)与本组术前比较差异无统计学意义,瓣环前后直径(AP)低于本组术前(P<0.05);改善组术后的AH高于本组术前,TV、AA3D、CD、AC、AL-PM和AP低于本组术前(P<0.05)。术前改善组的AA3D、CD、AC、AL-PM和AP低于未改善组(P<0.05),术后改善组的TV、AA3D、CD、AC、AL-PM和AP低于未改善组,AH高于未改善组(P<0.05)。多因素分析结果为:CD、AC、AL-PM、AP与PCI术后反流未改善情况关联密切(P <0.05);二尖瓣参数评价PCI术后反流未改善的灵敏度、特异度和准确性的结果分别如下,CD:76%、64%、69%;AC:78%、64%、70%;AL-PM:78%、74%、76%;AP:84%、83%、83%。结论应用RT3DTTE检查能够掌握AMI伴中度二尖瓣反流患者的二尖瓣构型特征,预测PCI术后患者二尖瓣反流改善情况。 展开更多
