Intercellular Adhension Molecule-1 in the Pathogenesis of Heroin-induced Acute Lung Injury in Rats 被引量:3

The expression of intercellular adhesion molecule-1 (ICAM-1) in the pathogenesis of heroin-induced acute lung injury (ALI) in rats was investigated. The model of ALI was established by intravenous injection of heroin into tail vein in rats. Thirty-six rats were randomly divided into heroin-treated groups (1 h, 2 h, 4 h, 6 h and 24 h) and normal control group. Changes in histopathologic morphology and biological markers of ALI were measured. The expression of ICAM-1 in lung tissue was detected by using immunohistochemistry and RT-PCR. The results showed that the W/D ratio and protein contents in BALF of the heroin-treated groups were significantly higher than that of the control group (P<0.01). The histopathological changes in the lung tissue were more obvious in heroin-treated groups. The ICAM-1 protein and mRNA expression in the lung tissue of heroin-treated groups were significantly increased as compared with that of the control group (P<0.01), and correlated with the ALI parameters in a time-dependent manner. Increasing of ICAM-1 expression was involved in the formation of heroin-induced lung injury. Furthermore, the level of expression was positively correlated with the severity of lung injury. 展开更多

BACKGROUND:Platelet endothelial cell adhesion molecule-1(PECAM-1),also known as CD31,is mainly distributed in vascular endothelial cells.Studies have shown that PECAM-1 is a very significant indicator of angiogenesis,and has been used as an indicator for vascular endothelial cells.The present study aimed to explore the relationship between the expression of PECAM-1 and the degree of acute lung injury(ALI) and fibrosis in paraquat(PQ) induced lung injury in rabbits.METHODS:Thirty-six adult New Zealand rabbits were randomly divided into three groups(12rabbits in each group) according to PQ dosage:8 mg/kg(group A),16 mg/kg(group B),and 32 mg/kg(group C).After PQ infusion,the rabbits were monitored for 7 days and then euthanized.The lungs were removed for histological evaluation.Masson staining was used to determine the degree of lung fibrosis(LF),and semi-quantitative immune-histochemistry analysis to determine the expression of PECAM-1.Pearson's product-moment correlation analysis was performed to evaluate the relationship between the expression of PECAM-1 and the extent of lung injuries expressed by ALI score and degree of LF.RESULTS:Rabbits in the three groups showed apparent poisoning.The rabbits survived longer in group A than in groups B and C(6.47±0.99 days vs.6.09±1.04 days vs.4.77±2.04 days)(P<0.05).ALI score was lower in group A than in groups B and C(8.33±1.03 vs.9.83±1.17 vs.11.50±1.38)(P<0.05),and there was statistically significant difference between group B and group C(P=0.03).LF was slighter in group A than in groups B and C(31.09%±2.05%vs.34.37%±1.62%vs.36.54%±0.44%)(P<0.05),and there was statistically significant difference between group B and group C(P=0.026).The PEACAM-1 expression was higher in group A than in groups B and C(20.31%±0.70%vs.19.34%±0.68%vs.18.37%±0.46%)(P<0.05),and there was statistically significant difference between group B and group C(P=0.017).Pearson's correlation analysis showed that the expression of PECAM-1 was negatively correlated to both ALI score(Coe=-0.732,P=0.001)and degree of LF(Coe=-0.779,P<0.001).CONCLUSIONS:The PECAM-1 expression significantly decreases in New Zealand rabbits after PQ poisoning,and the decrease is dose-dependent.The PECAM-1 expression is negatively correlated with ALI score and LF,showing a significant role in the development of lung injuries induced by PQ. 展开更多

AIM:To assess the effect of inhibition of caspase-1 on acute renal injury in rats with severe acute pancreatitis(SAP).METHODS:Forty-two Sprague-Dawley rats were randomly divided into three groups:healthy controls(HC,n=6),SAP rats treated with saline(SAP-S,n=18),or SAP rats treated with a caspase-1/interleukin(IL)-1β-converting-enzyme(ICE)inhibitor(SAP-I-ICE,n=18).SAP was induced by retrograde infusion of 5%sodium taurocholate into the bile-pancreatic duct.HC rats were subjected to identical treatment and surgical procedures without sodium taurocholate.Rats received an intraperitoneal injection of isotonic saline(SAP-S)or the inhibitor(SAP-ICE-I)at 2 and 12 h after induction of acute pancreatitis.Surviving rats were sacrificed at different time points after SAP induction;all samples were obtained and stored for subsequent analyses.The levels of blood urea nitrogen(BUN)and creatinine(Cr)were measured using automatic methods,and serum IL-1βconcentrations were measured by an enzymelinked immunosorbent assay.Intrarenal expression of IL-1β,IL-18 and caspase-1 mRNAs was detected by RT-PCR.IL-1βprotein expression and the pathologic changes in kidney tissues were observed by microscopy after immunohistochemical or hematoxylin and eosin staining,respectively.RESULTS:The serum levels of BUN and Cr in the SAP-S group were 12.48±2.30 mmol/L and 82.83±13.89μmol/L at 6 h,23.53±2.58 mmol/L and 123.67±17.67μmol/L at 12 h,and 23.60±3.33 mmol/L and125.33±21.09μmol/L at 18 h,respectively.All were significantly increased compared to HC rats(P<0.01for all).Levels in SAP-ICE-I rats were significantly decreased compared to SAP-S rats both at 12 and 18 h(P<0.01 for all).Serum IL-1βlevels in the SAP-S group were 276.77±44.92 pg/mL at 6 h,308.99±34.95pg/mL at 12 h,and 311.60±46.51 pg/mL at 18 h;all significantly higher than those in the HC and SAP-ICE-I groups(P<0.01 for all).Intrarenal expression of IL-1βmRNA was weak in HC rats,but increased significantly in SAP-S rats(P<0.01).ICE inhibition significantly decreased the expression of IL-1βand IL-18 mRNAs(P<0.05 for all vs SAP-S),whereas caspase-1 mRNA expression was not significantly different.Weak IL-1βimmunostaining was observed in HC animals,and marked staining was found in the SAP-S group mainly in renal tubular epithelial cells.IL-1βimmunostaining was significantly descended in SAP-ICE-I rats compared to SAP-S rats(P<0.05).Caspase-1 inhibition had no effect on the severity of kidney tissue destruction.CONCLUSION:The expression of caspase-1-activated cytokines IL-1βand IL-18 plays a pivotal role in acute renal injury in rats with experimental SAP.Caspase-1inhibition improves renal function effectively. 展开更多

CD38 is known to play roles in various inflammatory pathways.However,whether it has a protective or detrimental effect during bacterial septicemia remains disputed.Herein,this study aimed to determine the potential effect of CD38 on renal injury in septicemia.Escherichia coli(E.coli)was used to induce sepsis-associated renal injury in mice.WT and CD38-/-mice were stimulated with E.coli.After three hours,the serum was collected to detect renal function.Function mRNA expressions inflammatory cytokines in kidneys were quantified by real-time PCR.Hematoxylin and eosin staining were used to observe the histomorphology of kidney.The expression of TLR4,NF-κB,MAPK and cytokines were detected by Western Blot.Our results demonstrated that 3×10^(8) cfu/mL E.coli is the appropriate dose to induce sepsis mice model.Compared to WT sepsis mice,CD38-/-mice showed aggravated kidney injuries with impaired renal function,increased inflammation and apoptosis after E.coli stimulation.Interestingly,CD38 deficiency also led to elevated expression of TLR4 and increased phosphorylation of NF-κB p65/p105 and ERK1/2.To sum up,our results suggested that CD38 deficiency could aggravate E.coli-induced renal injury through activating ERK1/2-NF-κB signaling pathway. 展开更多

目的 观察恩格列净(EM)对2型糖尿病(T2DM)大鼠肾损伤的治疗效果,并探讨其可能存在的机制。方法 将SD雄性大鼠随机分为正常对照(NC)组、 T2DM组和EM组,每组6只。T2DM组和EM组,给予腹腔注射链脲佐菌素(STZ)建立T2DM模型,记录各组大鼠空腹... 目的 观察恩格列净(EM)对2型糖尿病(T2DM)大鼠肾损伤的治疗效果,并探讨其可能存在的机制。方法 将SD雄性大鼠随机分为正常对照(NC)组、 T2DM组和EM组,每组6只。T2DM组和EM组,给予腹腔注射链脲佐菌素(STZ)建立T2DM模型,记录各组大鼠空腹血糖(FBG)和体质量。EM组给予EM溶液灌胃,其余两组予以等量的羧甲基纤维素钠溶液灌胃,给药12周。记录大鼠体质量和FBG后处死大鼠,留存腹主动脉血液和肾脏组织。全自动生化分析仪检测血清肌酐(Scr)、血尿素氮(BUN)、尿酸(UA)、甘油三酯(TG)、总胆固醇(TC);行Masson染色、过碘酸希夫(PAS)染色、 HE染色观察肾脏组织学变化,透射电镜观察肾脏超微结构变化;免疫组织化学染色法检测大鼠肾脏组织中环磷酸腺苷直接激活的交换蛋白1(Epac1)、肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、白细胞介素18(IL-18)的表达和分布。结果 与NC组相比,T2DM组大鼠体质量下降,FBG、 Scr、 BUN、 UA、 TC、 TG水平明显升高;肾小球基底膜增厚,足细胞足突融合,排列紊乱,内皮细胞窗孔消失;Epac1蛋白表达水平下降,TNF-α、 IL-1β、 IL-18的蛋白表达水平明显增高。与T2DM组相比,EM组大鼠体质量上升,FBG、 Scr、 BUN、 UA、 TC、 TG水平降低;肾损伤减轻,Epac1蛋白表达水平升高,TNF-α、 IL-1β、 IL-18的表达显著降低。结论 EM能够改善T2DM肾损伤。这种治疗效果是通过上调Epac1蛋白表达,抑制炎症反应介导的。 展开更多

目的探讨术前血清V-set和免疫球蛋白结构域4(VSIG4)以及长链非编码核糖核酸(LncRNA)SBF2反义RNA1(SBF2-AS1)与肾结石患者经皮肾镜取石术后急性肾损伤(AKI)的关系。方法选择2020年1月—2022年12月本院收治的109例肾结石患者为研究对象。... 目的探讨术前血清V-set和免疫球蛋白结构域4(VSIG4)以及长链非编码核糖核酸(LncRNA)SBF2反义RNA1(SBF2-AS1)与肾结石患者经皮肾镜取石术后急性肾损伤(AKI)的关系。方法选择2020年1月—2022年12月本院收治的109例肾结石患者为研究对象。术前检测患者血清VSIG4水平以及LncRNA SBF2-AS1表达,术后记录AKI发生情况。采用多因素Logistic回归模型分析肾结石患者经皮肾镜取石术后发生AKI的影响因素;采用受试者工作特征(ROC)曲线分析VSIG4、LncRNA SBF2-AS1预测肾结石患者经皮肾镜取石术后发生AKI的价值。结果本研究中,术后发生AKI者16例。AKI组血清VSIG4水平低于非AKI组,LncRNA SBF2-AS1表达高于非AKI组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析显示,术前高尿酸水平、术前高LncRNA SBF2-AS1表达、较长的手术时间、术中低血压是肾结石患者经皮肾镜取石术后发生AKI的危险因素(P<0.05),术前高VSIG4水平是保护因素(P<0.05)。术前血清VSIG4、LncRNA SBF2-AS1水平预测肾结石患者经皮肾镜术后发生AKI的曲线下面积分别为0.854、0.705,二者联合预测的曲线下面积为0.948,大于各指标单独预测(Z=1.995、2.958,P<0.05)。结论肾结石患者术前血清VSIG4水平降低、LncRNA SBF2-AS1表达增高与经皮肾镜取石术后AKI的发生有关,联合检测术前VSIG4、LncRNA SBF2-AS1可预测术后AKI的发生风险。 展开更多

血清Apelin-13和网膜素-1表达与肾移植术后患者肾功能的相关性研究

目的探讨血清Apelin-13、网膜素-1(Omentin-1)在早期预测肾移植术后并发急性肾功能损伤中的价值。方法纳入89例肾移植患者为肾移植组,根据肾移植术后1个月内患者是否发生急性肾功能损伤分为肾功能损伤组21例和非肾功能损伤组68例。同时... 目的探讨血清Apelin-13、网膜素-1(Omentin-1)在早期预测肾移植术后并发急性肾功能损伤中的价值。方法纳入89例肾移植患者为肾移植组,根据肾移植术后1个月内患者是否发生急性肾功能损伤分为肾功能损伤组21例和非肾功能损伤组68例。同时随机选取健康体检者87例作为对照组。采用酶联免疫吸附(ELISA)法检测血清Apelin-13、Omentin-1水平;采用粒子增强透射免疫比浊法测定胱抑素C水平;采用酶法测定血肌酐水平;采用紫外-谷氨酸脱氨酶法测定血尿素氮水平;采用比浊法测定β2-微球蛋白、24 h尿蛋白水平。校正eMDRD方程,计算估算肾小球滤过率(eGFR)。采用Pearson法进行术后肾功能损伤的肾移植患者血清Apelin-13、Omentin-1水平与肾功能指标的相关性分析;采用受试者工作特征(ROC)曲线分析血清Apelin-13、Omentin-1水平对肾移植患者术后肾功能损伤的预测价值;采用Logistic回归分析法分析肾移植患者术后肾功能损伤的影响因素。结果肾移植组的血肌酐、血尿素氮、胱抑素C、β2-微球蛋白、24 h尿蛋白水平高于对照组,Apelin-13、Omentin-1、eGFR水平低于对照组,差异有统计学意义(P<0.05)。肾功能损伤组的血肌酐、血尿素氮、胱抑素C、β2-微球蛋白、24 h尿蛋白水平高于非肾功能损伤组,Apelin-13、Omentin-1、eGFR水平低于非肾功能损伤组,差异有统计学意义(P<0.05)。术后肾功能损伤的肾移植患者血清Apelin-13、Omentin-1水平与肌酐、血尿素氮、胱抑素C、β2-微球蛋白、24 h尿蛋白呈负相关,与eGFR呈正相关(P<0.05)。Apelin-13、Omentin-1单独及联合预测肾移植患者术后肾功能损伤的曲线下面积(AUC)分别为0.784、0.773、0.841。Apelin-13、Omentin-1是肾移植患者术后肾功能损伤的保护因素(P<0.05)。结论肾移植术后发生肾功能损伤患者血清中Apelin-13、Omentin-1均呈低表达,其或可成为早期预测指标。 展开更多

血清MCP-1、TRAF-6水平在脓毒症严重程度和急性肾损伤评估的作用

目的探讨血清单核细胞趋化蛋白-1(MCP-1)、肿瘤坏死因子受体相关因子-6(TRAF-6)水平在脓毒症严重程度和急性肾损伤评估中的作用。方法回顾性分析2021年6月至2022年6月在新疆维吾尔自治区人民医院重症医学科接受治疗的110例脓毒症患者的... 目的探讨血清单核细胞趋化蛋白-1(MCP-1)、肿瘤坏死因子受体相关因子-6(TRAF-6)水平在脓毒症严重程度和急性肾损伤评估中的作用。方法回顾性分析2021年6月至2022年6月在新疆维吾尔自治区人民医院重症医学科接受治疗的110例脓毒症患者的病例资料,依据脓毒症相关急性肾损伤发生情况分为发生组(50例)、未发生组(60例)。统计分析两组患者基线资料、血清MCP-1、TRAF-6水平,并分析脓毒症患者血清MCP-1、TRAF-6水平与序贯多器官功能障碍(SOFA)评分、急性生理和慢性健康Ⅱ(APACHEⅡ)评分的相关性,多因素Logistic回归分析脓毒症相关急性肾损伤影响因素。结果110例患者中,脓毒症相关急性肾损伤发生50例,未发生60例,发生率为45.45%。发生组患者的高血压、肺部感染比例均高于未发生组(P<0.05),SOFA评分、APACHEⅡ评分均高于未发生组(P<0.05),氧合指数低于未发生组(P<0.05),肾小球滤过率(eGFR)、血肌酐(SCr)、尿素氮(BUN)水平均高于未发生组(P<0.05),动脉血乳酸水平高于未发生组(P<0.05),但两组患者的性别、年龄、糖尿病比例的差异无统计学意义(P>0.05)。发生组患者的血清MCP-1、TRAF-6水平均高于未发生组(P<0.05)。脓毒症患者血清MCP-1、TRAF-6水平与SOFA评分、APACHEⅡ评分均呈显著的正相关关系(P<0.05)。多因素Logistic回归分析显示,脓毒症相关急性肾损伤影响因素包括高血压、肺部感染、SOFA评分、APACHEⅡ评分、肾小球滤过率(eGFR)、动脉血乳酸、MCP-1、TRAF-6(P<0.05),不包括氧合指数、尿素氮(BUN)、SCr(P>0.05)。结论血清MCP-1、TRAF-6水平和脓毒症严重程度和关系密切,可作为急性肾损伤的诊断参考指标。 展开更多

目的探究积雪草苷通过核因子E2相关因子2(nuclear factor-erythroid 2-related factor 2,Nrf2)/血红素加氧酶1(hemeoxygenase-1,HO-1)信号通路对创伤引起的脓毒血症大鼠肾损伤的影响以及对氧化应激的调控作用。方法将60只SD雄性大鼠随... 目的探究积雪草苷通过核因子E2相关因子2(nuclear factor-erythroid 2-related factor 2,Nrf2)/血红素加氧酶1(hemeoxygenase-1,HO-1)信号通路对创伤引起的脓毒血症大鼠肾损伤的影响以及对氧化应激的调控作用。方法将60只SD雄性大鼠随机分为对照组(n=10)和脓毒血症组(n=50)。脓毒血症组通过盲肠结扎建立动物模型,造模成功后随机均分为模型组、低剂量组、中剂量组、高剂量组和阳性组。低剂量组、中剂量组、高剂量组灌胃积雪草苷(1.0、2.5、5.0 mg/mL);阳性组注射氨苄西林钠(200 mg/kg);模型组与对照组灌胃生理盐水。记录大鼠的生存时间;取样观察肾组织病理形态;检测血清中血肌酐(serum creatinine,Scr)、尿素氮(blood urea nitrogen,BUN)、丙二醛(malonic dialdehyde,MDA)、超氧化物歧化酶(superoxide dismutase,SOD)水平以及肾组织中Nrf2和HO-1蛋白含量。结果与对照组比较,模型组生存时间更短(P<0.05),肾组织炎症反应明显,Scr、BUN、MDA水平升高(P<0.05),SOD水平降低(P<0.05),Nrf2、HO-1蛋白表达水平降低(P<0.05);与模型组相比,积雪草苷治疗各组及阳性组生存时间延长(P<0.05),肾组织炎症减轻,Scr、BUN、MDA水平降低(P<0.05),SOD水平升高(P<0.05),Nrf2、HO-1蛋白表达水平升高(P<0.05);积雪草苷治疗组上述指标呈现剂量依赖性,其中高剂量组与模型组对比最为明显(P<0.05);高剂量组BUN和MDA水平较阳性组更低(P<0.05),HO-1蛋白表达水平较阳性组更高(P<0.05)。结论积雪草苷可能通过Nrf2/HO-1通路,抑制氧化应激反应,改善脓毒血症大鼠的肾损伤状况,其作用呈现剂量依赖性,且较抗生素治疗有一定的优势。 展开更多

基于KIM-1、MCP-1采用甲花片治疗造影剂诱导肾损伤疗效观察

目的:通过肾损伤分子-1 (kidney injury molecule-1,KIM-1)、单核细胞趋化蛋白-1 (monocyte chemo-attractant protein-1,MCP-1)等肾小管相关损伤标志物,评估甲花片对造影剂诱导的肾损伤(contrast-induced nephropathy,CIN)的防治作用... 目的:通过肾损伤分子-1 (kidney injury molecule-1,KIM-1)、单核细胞趋化蛋白-1 (monocyte chemo-attractant protein-1,MCP-1)等肾小管相关损伤标志物,评估甲花片对造影剂诱导的肾损伤(contrast-induced nephropathy,CIN)的防治作用。方法:采用前瞻性、随机对照研究方法,纳入拟行PCI手术的患者共60例,根据随机数字表法将纳入患者随机分为两组各30例;对照组予常规基础治疗,治疗组在常规基础治疗上加服甲花片;所有患者均于术前24 h、术后48 h观察KIM-1、MCP-1、血尿素氮(blood urea nitrogen,BUN)、血肌酐(serum creatinine,SCr)、肾小球滤过率(estimatedglomerular filtration rate,e GFR)等指标水平。结果:治疗后,治疗组KIM-1、MCP-1、BUN、SCr、e GFR水平与术前比较基本持平或有所降低(P﹤0.05);对照组KIM-1、MCP-1、BUN、SCr水平较术前升高(P﹤0.05),eGFR较术前降低(P﹤0.05)。结论:甲花片对造影剂诱导的肾损伤有一定的防治作用,可在冠脉造影围手术期应用甲花片。 展开更多

目的研究检测血清胱抑素C(CysC)、尿液肾损伤分子-1(KIM-1)水平在评估妊娠期高血压疾病(HDP)患者肾小管间质损伤程度的意义。方法选取2021年1月至2021年12月在常州市妇幼保健院就诊的74例有肾小管间质损伤的HDP患者为肾损伤组,86例无肾... 目的研究检测血清胱抑素C(CysC)、尿液肾损伤分子-1(KIM-1)水平在评估妊娠期高血压疾病(HDP)患者肾小管间质损伤程度的意义。方法选取2021年1月至2021年12月在常州市妇幼保健院就诊的74例有肾小管间质损伤的HDP患者为肾损伤组,86例无肾小管间质损伤的HDP患者为非肾损伤组,另选取常规体检孕妇80例作为健康对照组。于孕22~26周比较三组的CysC水平、KIM-1水平。采用Cox回归分析肾小管间质损伤程度的影响因素,采用受试者工作特征曲线(ROC)评估CysC、KIM-1指标的预测价值。结果肾损伤组CysC水平、KIM-1水平高于非肾损伤组和健康对照组,非肾损伤组高于健康对照组(F值分别为84.712、516.732,P<0.05);肾损伤组内重度29例,轻度45例;重度患者CysC水平、KIM-1水平高于轻度患者(t值分别为5.301、5.706,P<0.05);经Cox回归分析显示CysC水平、KIM-1水平是肾小管间质损伤程度的独立影响因素,其OR值及95%CI分别为7.412(3.547~11.236)、7.215(2.842~13.985),P<0.05;经ROC分析显示,CysC、KIM-1评估HDP患者肾小管间质损伤程度的截断值分别为1.57mg/L、6.08ng/mL,ROC曲线下面积(AUC)分别为0.844、0.783,且两指标均具有较好的灵敏度和特异度。结论血清CysC、尿液KIM-1在诊断HDP患者的肾小管间质损伤程度中有着重要价值,能为临床评估患者病情和制定后续治疗方案提供参考依据。 展开更多

BACKGROUND Patients with chronic hepatitis B(CHB)with long-term nucleos(t)ide therapy may experience renal insufficiency.Traditional renal function indicators,such as urine protein,serum urea nitrogen(BUN),and serum creatinine,are normal when early mild lesions occur.Therefore,more sensitive renal function indicators are needed.AIM To investigate the significance of early renal injury indicators in evaluating renal injury in patients with CHB with long-term nucleos(t)ide therapy.METHODS We collected the clinical data of 69 outpatients with CHB at Peking University First Hospital from March 2018 to January 2020 who had been treated with longterm nucleos(t)ide therapy and analyzed the results of early renal injury indicators.Continuous normal distribution data were analyzed by the t-test to determine the difference between two groups.Continuous non-normally distributed data were analyzed by the Mann-Whitney U-test between two groups.The Kruskal-Wallis H test was used to determine the differences among multiple groups.Enumeration data were analyzed by the chi-square test.The related factors of early renal injury indicators were analyzed by logistic regression analysis.RESULTS The average treatment duration with nucleos(t)ide analogs of the 69 patients with CHB was 99.7±28.7 mo.The cases of patients with elevated BUN and hypophosphatemia were 6(8.7%)and 13(18.8%),respectively;31(44.9%)patients had abnormal early renal injury indicators,including 9 patients with abnormal urine microalbumin,7 patients with abnormal urine immunoglobulin,6 patients with abnormal urine transferrin,and 19 patients with abnormalα1 microglobulin.There were no significant differences in the mean values of age,sex,BUN,estimated glomerular filtration rate(eGFR),serum uric acid,serum calcium,or serum phosphorus between the two groups of patients with and without early renal injury indicators.However,the mean levels of serum creatinine and urine creatinine,N-acetyl-β-D-glucosidase enzyme,α1 microglobulin,and urine immunoglobulin in the former group of patients were significantly higher than those in the latter group of patients(P<0.05).The incidence of early renal injury in patients with eGFR≥90,60-89,and 30-59 mL/(min·1.73 m2)was 36.4%(8/22),47.6%(20/42),and 60%(3/5),respectively.Logistic regression analysis results showed that gamma-glutamyl transpeptidase[odds ratio(OR)=1.05(1.008-1.093),P=0.020],direct bilirubin[OR=1.548(1.111-2.159),P=0.010],serum creatinine[OR=1.079(1.022-1.139),P=0.006],and age[OR=0.981(0.942-1.022),P=0.357]were independent predictors of early renal injury.CONCLUSION Patients with CHB treated with long-term nucleos(t)ide analog therapy had a high probability of early renal injury,and early renal injury indicators were highly sensitive and could be used to monitor early renal impairment. 展开更多

目的:探究近端肾小管上皮细胞中肾损伤分子1(KIM-1)介导免疫球蛋白轻链(FLCs)内吞对肾小管损伤的影响及机制。方法:(1)选取4例多发性骨髓瘤肾损伤患者尿标本,采用离子交换层析法纯化FLCs;(2)体外实验构建纯化的FLCs诱导人肾脏近端肾小... 目的:探究近端肾小管上皮细胞中肾损伤分子1(KIM-1)介导免疫球蛋白轻链(FLCs)内吞对肾小管损伤的影响及机制。方法:(1)选取4例多发性骨髓瘤肾损伤患者尿标本,采用离子交换层析法纯化FLCs;(2)体外实验构建纯化的FLCs诱导人肾脏近端肾小管上皮细胞(HK-2细胞)损伤模型,继续给予FLCs刺激,并予KIM-1抑制剂TW-37干预;体内实验向小鼠腹腔注射FLCs,14 d后建立FLCs致小鼠肾小管损伤的动物模型,并予TW-37干预。Western Blot检测HK-2细胞和小鼠肾组织KIM-1、megalin、核因子E2相关因子2(Nrf2)、葡萄糖调节蛋白78(GRP78)、α平滑肌肌动蛋白(α-SMA)及转化生长因子β(TGF-β)等水平,ELISA法检测小鼠尿白蛋白/肌酐比值、尿液KIM-1的含量,天狼星红染观察小鼠肾间质纤维化程度等。结果:(1)体外实验中FLCs刺激HK-2细胞诱导KIM-1表达增加,并与FLCs存在共定位,TW-37处理后可减少HK-2细胞中FLCs内吞,减轻FLCs刺激所致的肾小管细胞溶酶体的核周聚集,降低氧化应激及纤维化指标表达。(2)动物模型结果显示,TW-37干预可缓解FLCs所致的肾小管损伤,包括尿KIM-1水平及肾组织KIM-1表达下降,肾组织氧化应激和纤维化指标表达下降,肾组织胶原沉积减轻。结论:KIM-1可介导肾近端小管细胞内吞FLCs,抑制KIM-1有助于缓解FLCs造成的肾损伤。 展开更多

Correlation of PECAM-1 gene C373G locus polymorphism with endothelial injury and placental pathological damage in patients with preeclampsia

Peripheral blood was collected to determine PECAM-1 gene C373G polymorphism as well as the contents of endothelial injury molecules sEng, PAR-1, sFlt-1 and ET-1;placenta tissue was collected to determine the contents of pathological damage molecules Gadd45 , TIMP2, Fas, Apaf-1 and caspase-3.Results:PECAM-1 gene C373G locus GC and GG genotype constituent ratio and allele G constituent ratio in peripheral blood of PE group were significantly higher than those of control group while CC genotype constituent ratio and allele C constituent ratio were significantly lower than those of control group. sFlt-1, sEng, PAR-1 and ET-1 contents in serum as well as Gadd45 , TIMP2, Fas, Apaf-1 and caspase-3 contents in placenta of PE group were significantly higher than those of control group;sFlt-1, sEng, PAR-1 and ET-1 contents in serum as well as Gadd45 , TIMP2, Fas, Apaf-1 and caspase-3 contents in placenta of PE patients with PECAM-1 gene C373G locus GC and GG genotypes were higher than those of PE patients with CC genotype.Conclusion: Increased allele G in PECAM-1 gene C373G loci is closely correlated with endothelial injury and placental pathological damage in patients with preeclampsia. 展开更多

HIF-2α通过调控NEK1表达减轻肾小管上皮细胞缺氧损伤的研究

目的探讨缺氧诱导因子-2α(HIF-2α)在肾小管上皮细胞缺氧损伤中的作用及机制。方法体外培养人近曲小管上皮细胞(HK-2细胞),以2.5μmol/L抗霉素A作用24 h构建缺氧HK-2细胞模型,采用qRT-PCR法和Western blot法检测HIF-2α在正常HK-2细胞... 目的探讨缺氧诱导因子-2α(HIF-2α)在肾小管上皮细胞缺氧损伤中的作用及机制。方法体外培养人近曲小管上皮细胞(HK-2细胞),以2.5μmol/L抗霉素A作用24 h构建缺氧HK-2细胞模型,采用qRT-PCR法和Western blot法检测HIF-2α在正常HK-2细胞和缺氧HK-2细胞中的表达;将HIF-2α干扰序列(siRNA-HIF-2α)及其阴性对照(siRNA-NC)、NIMA相关激酶-1(NEK1)过表达质粒(pcDNA3.0-NEK1)及空载体质粒(pcDNA3.0-NC)转染至缺氧HK-2细胞后,采用CCK-8法检测缺氧HK-2细胞增殖活性,Annexin V-FITC/PI双染法检测缺氧HK-2细胞凋亡率,qRT-PCR法和Western blot法检测缺氧HK-2细胞中NEK1表达。结果与正常HK-2细胞比较,缺氧HK-2细胞中HIF-2αmRNA和蛋白表达量均升高,差异均有统计学意义(P<0.05);与Control组比较,Hypoxia组细胞增殖活性降低、细胞凋亡率升高、细胞中NEK1 mRNA和蛋白表达量均升高,差异均有统计学意义(P<0.05);与Hypoxia组比较,si-NC组细胞增殖活性、细胞凋亡率、细胞中NEK1 mRNA和蛋白表达量均无明显变化,差异无统计学意义(P>0.05);与si-NC组比较,si-HIF-2α组细胞增殖活性降低、细胞凋亡率升高、细胞中NEK1 mRNA和蛋白表达量降低,差异均有统计学意义(P<0.05);与si-HIF-2α组比较,si-HIF-2α+pcDNA3.0-NC组细胞增殖活性、细胞凋亡率均无明显变化,差异无统计学意义(P>0.05);与si-HIF-2α+pcDNA3.0-NC组细胞比较,si-HIF-2α+pcDNA3.0-NEK1组细胞增殖活性升高、细胞凋亡率降低,差异均有统计学意义(P<0.05)。结论HIF-2α可能通过调控NEK1表达促进缺氧HK-2细胞增殖、抑制细胞凋亡,从而减轻HK-2细胞缺氧损伤。 展开更多

目的:研究Maresin 1对脓毒症相关急性肾损伤(S-AKI)的保护作用及相关机制。方法:SPF级雄性SD大鼠,采用盲肠结扎穿孔法(CLP)制备脓毒症大鼠模型。模型大鼠按随机数字表法分成CLP组、CLP+Maresin 1组、CLP+Maresin 1+Nigericin(NLRP3激活... 目的:研究Maresin 1对脓毒症相关急性肾损伤(S-AKI)的保护作用及相关机制。方法:SPF级雄性SD大鼠,采用盲肠结扎穿孔法(CLP)制备脓毒症大鼠模型。模型大鼠按随机数字表法分成CLP组、CLP+Maresin 1组、CLP+Maresin 1+Nigericin(NLRP3激活剂)组。另取正常大鼠作为Sham组。CLP+Maresin 1组、CLP+Maresin 1+Nigericin组大鼠于术后1 h单次腹腔注射Maresin 1(4 ng/g,溶于200μl 0.9%氯化钠溶液)。CLP+Maresin 1+Nigericin组在给予Maresin 1基础上联合单次Nigericin(1 mg/kg)给药。Sham组和CLP组给予200μl 0.9%氯化钠溶液。12 h后,采血用于血肌酐(Scr)和血尿素氮(BUN)生化指标和血清中性粒细胞明胶酶相关载脂蛋白(NGAL)、肾损伤分子1(KIM1)及炎症因子ELISA检测。HE染色检测肾脏病理改变。免疫组化和Western blot检测NOD样受体热蛋白结构域3(NLRP3)、半胱氨酸蛋白酶-1(Caspase-1)、白介素-1β(IL-1β)、白介素-18(IL-18)表达变化。另取大鼠按照上述方法分组及治疗,统计大鼠7 d存活率。结果:与Sham组相比,CLP组Scr、BUN、尿中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、肾损伤分子-1(KIM-1)及炎症因子水平明显增加,NLRP3、Caspase-1、IL-1β、IL-18表达明显上调,肾脏出现炎性损伤改变,大鼠7 d存活率明显降低(均P<0.05)。与CLP组相比,CLP+Maresin 1组Scr、BUN、NGAL、KIM-1及炎症因子水平明显降低,NLRP3、Caspase-1、IL-1β、IL-18表达明显下调,肾脏炎性损伤减轻,大鼠7 d存活率明显增加(均P<0.05)。然而,CLP+Maresin 1组给予Nigericin后,Maresin 1的保护作用被部分逆转。结论:Maresin 1对S-AKI具有保护作用,该机制与抑制NLRP3/Caspase-1轴有关。 展开更多

血清高迁移率蛋白B1与原发性高血压患者早期肾损伤的临床关系研究

目的 观察原发性高血压早期肾损伤患者血清高迁移率蛋白B1(HMGB1)水平改变以及对早期肾损伤的影响和预测价值。方法 选择2020年10月至2022年10月在衡水市人民医院就诊的原发性高血压患者210例,根据尿微量白蛋白/尿肌酐比值(UACR)分为早... 目的 观察原发性高血压早期肾损伤患者血清高迁移率蛋白B1(HMGB1)水平改变以及对早期肾损伤的影响和预测价值。方法 选择2020年10月至2022年10月在衡水市人民医院就诊的原发性高血压患者210例,根据尿微量白蛋白/尿肌酐比值(UACR)分为早期肾损伤组(UACR30~300 mg/g,92例)和单纯高血压组(UACR<30 mg/g,118例)。另选取同期成人健康体检者100例纳入对照组。比较三组血清HMGB1、白介素-6(IL-6)水平变化。观察早期肾损伤组血清HMGB1与IL-6水平之间的相关性。多因素logistic回归分析影响原发性高血压早期肾损伤发生的危险因素。应用受试者工作特征(ROC)曲线分析HMGB1、IL-6单独及联合检测对原发性高血压早期肾损伤的预测价值。结果 早期肾损伤组血清HMGB1、IL-6水平明显高于单纯高血压组和对照组[(10.44±3.62)ng/ml,(6.87±1.89)ng/ml比(3.32±1.24)ng/ml;(47.08±15.19)pg/ml,(31.25±6.09)pg/ml比(4.36±0.92)pg/ml],三组间两两比较均有统计学差异(均P<0.01)。早期肾损伤组血清HMGB1与IL-6水平之间呈正相关(r=0.650,P<0.01)。多元logistic回归分析显示,血清HMGB1、IL-6水平升高是原发性高血压早期肾损伤发生的危险因素(P<0.01)。HMGB1诊断原发性高血压早期肾损伤最佳界值为6.13 ng/ml,其Roc曲线下面积(AUC)为0.901,敏感性97.4%,特异性59.8%。IL-6预测早期肾损伤的AUC为0.937,其敏感性为97.4%,特异性52.3%。两者联合预测早期肾损伤的AUC为0.983,其敏感性为98.7%,特异性65.9%。结论 原发性高血压患者血清HMGB1水平超过6.13 ng/ml对早期肾损伤有一定的预测价值,其可能通过炎症反应影响早期肾损伤的发生。HMGB1联合IL-6检测可提高对早期肾损伤的预测价值。 展开更多

目的探究血清补体C1q肿瘤坏死因子相关蛋白9(CTRP9)、血管生成抑制蛋白-1(VASH-1)对妊娠期高血压疾病(HDCP)患者早期肾损伤的评估价值。方法选取2022年1~12月于河北中石油中心医院产科就诊治疗的HDCP患者112例为研究对象,根据是否发生... 目的探究血清补体C1q肿瘤坏死因子相关蛋白9(CTRP9)、血管生成抑制蛋白-1(VASH-1)对妊娠期高血压疾病(HDCP)患者早期肾损伤的评估价值。方法选取2022年1~12月于河北中石油中心医院产科就诊治疗的HDCP患者112例为研究对象,根据是否发生肾损伤,将其分为非肾损伤组(n=52)和肾损伤组(n=60);另选取同期孕检正常孕妇70例作为对照组。采用酶联免疫吸附法(ELISA)检测研究对象血清CTRP9、VASH-1水平;采用Pearson分析血清CTRP9、VASH-1水平与收缩压、舒张压、肿瘤坏死因子α(TNF-α)、血尿酸、尿蛋白水平的相关性;采用多因素logistic回归分析HDCP患者发生早期肾损伤的影响因素;采用受试者工作特征(ROC)曲线分析血清CTRP9、VASH-1在早期预测HDCP患者发生肾损伤中的价值。结果肾损伤组收缩压、舒张压、TNF-α、血尿酸、尿蛋白、VASH-1水平均高于非肾损伤组和对照组,血清CTRP9水平为(125.39±26.43)pg/mL,低于非肾损伤组和对照组,差异有统计学意义(P<0.05);患者血清CTRP9水平与收缩压、舒张压、TNF-α、血尿酸、尿蛋白水平均呈负相关(r=-0.474、-0.506、-0.487、-0.528、-0.534,P<0.05),患者血清VASH-1水平与收缩压、舒张压、TNF-α、血尿酸、尿蛋白水平均呈正相关(r=0.495、0.524、0.479、0.506、0.533,P<0.05);多因素logistic回归分析显示,高水平CTRP9是HDCP患者发生肾损伤的独立保护因素,高水平VASH-1、TNF-α、血尿酸、尿蛋白均是HDCP患者发生肾损伤的独立危险因素(P<0.05);ROC结果显示,血清CTRP9、VASH-1预测HDCP患者发生早期肾损伤的曲线下面积(AUC)分别为0.873、0.896,二者联合预测的AUC为0.955,优于各自单独预测(Z=2.319,P=0.020;Z=1.755,P=0.079),特异度为92.31%,灵敏度为86.67%。结论发生早期肾损伤HDCP患者血清CTRP9水平降低,血清VASH-1水平升高,二者在早期预测评估HDCP患者发生肾损伤中具有一定潜在价值。 展开更多
