Various accidents have been reported from time to time, resulting in serious social impact and huge economic loss. This paper presents the main progress in the safety risk management of underground engineering in China over the last decade, i.e.(1) establishment of laws and regulations for safety risk management of underground engineering,(2) implementation of the safety risk management plan,(3) establishment of decision support system for risk management and early-warning based on information technology, and(4) strengthening the study on safety risk management, prediction and prevention. Based on the analysis of the typical accidents in China in the last decade, the new challenges in the safety risk management for underground engineering are identified as follows:(1) control of unsafe human behaviors;(2) technological innovation in safety risk management; and(3) design of safety risk management regulations. Finally, the strategies for safety risk management of underground engineering in China are proposed in six aspects, i.e. the safety risk management system and policy, law, administration, economy, education and technology.展开更多

The aim of the study is to measure physician and nurse awareness about four selected patient safety indicators by authors and events reported about these relevant indicators in the hospital. The study uses standardized four patientsafety indicators like "needle sticks, cut wounds, dressing allergy, infections indicators". Cross section study was conducted through three month period in 2011-2012 based on voluntary response to the questionnaire that intend to measure knowledge about four health indicators. Studypopulations consisted of accessible sample of 146 different specialty physicians and 108 nurses present on duty during survey period. The association between the patient safety indicators and events reported about indicators in questionswere analyzed. Meanpatient safety knowledge questionnaire scores of health staff (nurse and physician) for needle sticks, cut wounds, dressing allergy, infections indicators were 47.13(11.8), 39.04(14.5), 38.02(10.5), 39.72(9.7), respectively. Significant statistical differences were also found between the frequency of events reported according to department and patient safety indicators (F = 8.34; p 〈 0.05) Measuring patient safety culture via safety indicators is essential in improving patient safety. How these powers influence health and safety risk management is not well documented. This article therefore assesses the perception and uses of stakeholders' power on health and safety in risk management in construction projects in Tanzania, specifically focuses on sources and types of power, how stakeholders perceive their power, how they use power on health and safety risk management, and what factors hinders their use of power. A case study strategy was adopted and four large on-going construction projects in Dar es Salaam Tanzania were involved. Data was collected through in-depth interviews with clients, consultants, contractors and construction workers. Findings indicate that stakeholders have different sources of power such as technical expertise, legitimate, political position, resources information to influence health and safety risk management. Nonetheless, the use of these powers was generally limited due to low level of knowledge on health and safety risk management among stakeholders, wrong perception on their roles, insufficient health and safety regulations and weak procurement system. The research recommends that, in order to realize health and safety performance through using stakeholder's power, there is a need of clear definition of stakeholders' role and responsibilities on health and safety, wide knowledge and experiences on health and safety risk management, strong regulatory system and procurement system.展开更多

The achievement of this objective depends on balancing social, environ...Green mining is concerned with mining in a sustainable manner, such that the needs of the present are met without compromising future generations. The achievement of this objective depends on balancing social, environmental and economic objectives and has to have regard to both active mining operations and legacy issues associated with mine closure. Ground engineering has a critical role in achieving green mining objectives but its contribution is characterised by pervasive uncertainty. Uncertainty equates to risk. This means that ground engineering should be practiced within a risk management framework that aims to both prevent unwanted outcomes and to mitigate their consequences to an acceptable level. This keynote address presents the fundamentals of risk management and demonstrates its effectiveness by reference to improvements in the safety performance of the NSW coal sector over the past three decades.Nevertheless, ground control remains a mix of art and science, relying heavily on judgements which should be premised on knowledge, skill and experience(that is, competence). Risk management has now been enshrined in mining legislation and operating practice in Australia for over two decades.Notwithstanding this, near-hit and accident and incident investigations, commissions of inquiry and legal proceedings almost invariably identify deficiencies and opportunities for improvements necessary to achieve the objectives of sustainable mining. Three of the more important opportunities which have global application in relation to ground engineering are discussed. Through analyzing the structure characteristics of the management system,the phase type distribution was employed to analyze its subsystem reliability by considering repair process and three states including working,fail-abnormal,and fail-emergency states. The reliability indices of the subsystem were derived respectively,including the probabilities that the subsystem in three states,mean time to the first failure, mean time to first failemergency,mean working time to first fail-emergency,and mean maintenance time to the first fail-emergency, are derived respectively. The probabilities of the membership systems and the management system in three states were also derived. Some numerical examples were used to show the procedures. The correlation analysis method was used to determine correlation parameters indicating gas drilling safety risk. By collecting monitoring data in the safety risk period of more than 20 wells, a sample database of a variety of safety risks in gas drilling was established, and the number of samples was expanded by using the method of few-shot learning. According to the forms of gas drilling monitoring data samples, a two-layer convolution neural network architecture was designed, and multiple convolution cores of different sizes and weights were set to realize the vertical and horizontal convolution computations of samples to extract and learn the variation law and correlation characteristics of multiple monitoring parameters. Finally, based on the training results of neural network, samples of different kinds of safety risks were selected to enhance the recognition accuracy. Compared with the traditional BP(error back propagation) full-connected neural network architecture, this method can more deeply and effectively identify safety risk characteristics in gas drilling, and thus identify and predict risks in advance, which is conducive to avoid and quickly solve safety risks while drilling. Field application has proved that this method has an identification accuracy of various safety risks while drilling in the process of gas drilling of about 90% and is practical.展开更多

Water safety p...Provision of quality drinking water is paramount for sustaining good public health in urban residents. Packaged water produced and consumed across cities in Nigeria lacks integrity in protecting Health. Water safety plan based on hazard identification and risk assessment in each component of the water production system is essential in providing quality water by packaged water producing companies in Nigeria. This study aims at developing water safety plan for selected packaged water manufacturing companies in Abeokuta, Ogun State, Nigeria. Hazard identification and risk assessment were carried out based on site inspection studies, key informant interview, questionnaire survey and water sample analysis, and risk analysis using semi-quantitative risk matrix approach. The results revealed a total of 26 possible hazardous events which may compromise water quality such as on-site septic tanks and effluents discharged at source water and improper maintenance and hygiene practices within the system. Based on these, appropriate mitigation and monitoring plans were drawn for action. The research found that water safety plan is feasible for the packaged water systems, and therefore calls on the relevant stakeholders for urgent implementation towards ensuring clean drinking water and protecting public health as more and more people are opting for packaged waters due to uncertain public water safety.展开更多

文摘Based on the analysis of construction project safety risk and its preventive measures, it is first necessary to clarify the safety risks and existing problems of construction project supervision and to formulate effective construction procedures. It is necessary to check the production safety of the construction unit, strengthen the company's own safety production management, and reasonably control the risk to familiarize with the construction drawings, and make a special construction plan for the work of the bottom line. Finally, an elective solution is drawn to comprehensively expound the importance of construction project construction safety risk and its preventive measures.
文摘Risk precontrol management system of coal mines safety( RPMSCS) provides a set of preventive safety management strategy for high-risk coal industries, which has captured extensive attentions. Fundamentally,there are several membership systems with subsystems in the management system, and the subsystem reliability has an important influence on the management system performance. Through analyzing the structure characteristics of the management system,the phase type distribution was employed to analyze its subsystem reliability by considering repair process and three states including working,fail-abnormal,and fail-emergency states. The reliability indices of the subsystem were derived respectively,including the probabilities that the subsystem in three states,mean time to the first failure, mean time to first failemergency,mean working time to first fail-emergency,and mean maintenance time to the first fail-emergency, are derived respectively. The probabilities of the membership systems and the management system in three states were also derived. Some numerical examples were used to show the procedures. The result is important for better understanding the management system operation and improving its operational performance from the respect of system reliability.
文摘This paper presented detailed information about the timeline of development of drug risk management in FDA. The time process was divided into three stages: the launch of laws and regulations of drug risk management, pre-market approval and post-market safety supervision, adverse drug reaction and risk communication. To address the problems existing in drug risk management in China, suggestions to further and improve the development of drug risk management are proposed.

文摘To introduce the basic concepts of technical specification of nuclear power plant,a risk assessment and management technique based on the probabilistic safety analysis( PSA) method was proposed. The risk-informed method was used,and an example was given to show how to use some specific risk metrics like CDF / LERF /ICDP / ILERP to analyze and manage the risk associated with activities in nuclear power plant operation. The advantage of this technique can be concluded from this paper,and this technique should be used more widely and deeply in nuclear industry.
文摘Within this paper, the process of statistical safety analysis has been presented, which involves the following steps: formulation of basic principles of statistical safety analysis, initial events analysis, accident sceneries progress analysis, risk calculation, and risk calculation results analysis. On this basis, it has been concluded that the bucket wheel excavator SRs 1200×24/4×0(400 kW)+VR safety criteria is the mechanism for the hoist of rotor's arrow failure modes, because in that case whole bucket wheel excavator failure would necessarily happen (excavator falling down on counterweight). Therefore, excavator units statistical safety analysis is accomplished preventively to obtain its effective maintenance management.

文摘In view of the shortcomings of current intelligent drilling technology in drilling condition representation, sample collection, data processing and feature extraction, an intelligent identification method of safety risk while drilling was established. The correlation analysis method was used to determine correlation parameters indicating gas drilling safety risk. By collecting monitoring data in the safety risk period of more than 20 wells, a sample database of a variety of safety risks in gas drilling was established, and the number of samples was expanded by using the method of few-shot learning. According to the forms of gas drilling monitoring data samples, a two-layer convolution neural network architecture was designed, and multiple convolution cores of different sizes and weights were set to realize the vertical and horizontal convolution computations of samples to extract and learn the variation law and correlation characteristics of multiple monitoring parameters. Finally, based on the training results of neural network, samples of different kinds of safety risks were selected to enhance the recognition accuracy. Compared with the traditional BP(error back propagation) full-connected neural network architecture, this method can more deeply and effectively identify safety risk characteristics in gas drilling, and thus identify and predict risks in advance, which is conducive to avoid and quickly solve safety risks while drilling. Field application has proved that this method has an identification accuracy of various safety risks while drilling in the process of gas drilling of about 90% and is practical.
文摘Provision of quality drinking water is paramount for sustaining good public health in urban residents. Packaged water produced and consumed across cities in Nigeria lacks integrity in protecting Health. Water safety plan based on hazard identification and risk assessment in each component of the water production system is essential in providing quality water by packaged water producing companies in Nigeria. This study aims at developing water safety plan for selected packaged water manufacturing companies in Abeokuta, Ogun State, Nigeria. Hazard identification and risk assessment were carried out based on site inspection studies, key informant interview, questionnaire survey and water sample analysis, and risk analysis using semi-quantitative risk matrix approach. The results revealed a total of 26 possible hazardous events which may compromise water quality such as on-site septic tanks and effluents discharged at source water and improper maintenance and hygiene practices within the system. Based on these, appropriate mitigation and monitoring plans were drawn for action. The research found that water safety plan is feasible for the packaged water systems, and therefore calls on the relevant stakeholders for urgent implementation towards ensuring clean drinking water and protecting public health as more and more people are opting for packaged waters due to uncertain public water safety.
