Improvement of the nutritional support management system for patients in intensive care units

BACKGROUND Nutritional support for patients hospitalized in the intensive care unit(ICU)is an important part of clinical treatment and care,but there are significant implementation difficulties.AIM To introduce a modified nutritional support management system for ICU patients based on closed-loop information management and psychological counseling.METHODS The division of functions,personnel training,system construction,development of an intelligent decision-making software system,quality control,and improvement of the whole process were carried out to systematically manage nutritional support for ICU patients.RESULTS Following the implementation of the whole process management system,the scores of ICU medical staff’s knowledge,attitudes/beliefs,and practices regarding nutritional support were comprehensively enhanced.The proportion of hospital bed-days of total enteral nutrition(EN)in ICU patients increased from 5.58%to 11.46%,and the proportion of EN plus parenteral nutrition increased from 42.71%to 47.07%.The rate of EN initiation within 48 h of ICU admission increased from 37.50%to 48.28%,and the EN compliance rate within 72 h elevated from 20.59%to 31.72%.After the implementation of the project,the Self-rating Anxiety Scale score decreased from 61.07±9.91 points to 52.03±9.02 points,the Self-rating Depression Scale score reduced from 62.47±10.50 points to 56.34±9.83 points,and the ICU stay decreased from 5.76±2.77 d to 5.10±2.12 d.CONCLUSION The nutritional support management system based on closed-loop information management and psychological counseling achieved remarkable results in clinical applications in ICU patients. 展开更多

This is one of the most effective ways to exploit limited oil reserves more economically and efficiently. There are two steps in closed-loop reservoir management: automatic history matching and reservoir production opti- mization. Both of the steps are large-scale complicated optimization problems. This paper gives a general review of the two basic techniques in closed-loop reservoir man- agement; summarizes the applications of gradient-based algorithms, gradient-free algorithms, and artificial intelligence algorithms; analyzes the characteristics and application conditions of these optimization methods; and finally discusses the emphases and directions of future research on both automatic history matching and reservoir production optimization. 展开更多

The saving of non-renewable energies, as well as the reduction of emissions into the environment, are two crucial objectives of industrial production. The recovery of post-consumer products associated with the use of end-of-life products is part of a context of optimization of these objectives. This recovery maximizes the use of resources from end-of-life products in a circular logic while recording the environmental footprint. This study considers a recycling strategy adapted to the need and urgency to reduce greenhouse gas emissions caused by global warming. The proposed model aims to optimize the profits of the circular manufacturing strategies while minimizing operational costs (collection, sorting, recycling), transport, GHG emissions and recycling. In this paper, a compromise between the gains of CM and the costs associated with it was studied. The robustness of the designed model was tested using a case study based on real-world scenarios. A sensitivity analysis was carried out to study the impact of the emission cost on the overall objective, considering the two options currently offered to industries. The obtained results support companies to take the ecological aspect into account and integrate sustainable development into their strategic axes for their logistics supply chains. 展开更多

基于LSTM-SAE网络的隧道内机电设备智能监控系统设计

对隧道机电设备的监控和寿命预测方法进行研究,并与传统方法进行对比。使用LSTM-SAE网络进行设备寿命的预测,并开展智能监控工作,首先确定设备剩余寿命的特征因子,对噪声进行了平滑处理,并进行数据的归一化,然后基于LSTM进行设备的寿命... 对隧道机电设备的监控和寿命预测方法进行研究,并与传统方法进行对比。使用LSTM-SAE网络进行设备寿命的预测,并开展智能监控工作,首先确定设备剩余寿命的特征因子,对噪声进行了平滑处理,并进行数据的归一化,然后基于LSTM进行设备的寿命预测,针对LSTM的不足,引入SAE稀疏编码器进一步提升网络预测的精确性,建立LSTM-SAE网络进行预测。建立B/S结构的监控系统,使用以太网连接服务端和浏览器端,建立拥有状态监测模块、故障处理统计模块、寿命分析预测模块的控制系统。最后对电池寿命进行预测,BP神经网络预测、支持向量机预测、贝叶斯预测和线性回归预测进行对比,证明系统具有较高准确性,MAE、RMSE、SF值分别为8.68、9.23、39.12,具有较高准确性。研究实现了LSTM和SAE的融合,相比传统BP网络更加准确,能够满足隧道机电设备预测的特殊需求。 展开更多

This study simplifies the complex relationship among grapevine plants,aphids,ladybirds,and ants,which is essential for effective pest management and ecological balance.This study investigated the impact of aphid attacks and the presence of ants and ladybirds on the volatile compounds profile released into the chemosphere of the community consisting of the common vine Vitis vinifera,the aphid Aphis illinoisensis,the ladybird Cocci-nella undecimpunctata-and the ant Tapinoma magnum.This study aims to analyze the volatile compounds emitted by the grapevine and surrounding insects in response to these intricate interactions.The extraction of volatile organic compounds(VOCs)was carried out using closed-loop stripping(CLS)and then analyzed via gas chromatography-mass spectrometry(GC-MS)and principles coordinated analysis(PCA)was performed.The grapevine was exposed to different types and order of treatments,including non-infested,aphid-infested,aphid-infested with ant,aphid-infested with ladybird,and various combinations of ant and ladybird.After the aphid attack,the outcomes uncovered massive alterations in the volatile compound profiles.Infested grapevine displayed distinct emissions of germacrene D,an alcohol,and an alkene compared to non-infested plants.The characteristic VOC profile was the share of infested grapes in the presence of ants,with benzene derivatives and sesquiterpenes dominating the components.The coexistence of ladybirds with ants and aphids resulted in a dif-ferent volatile profile characterized by elevated levels of aldehydes,ketones,α-farnesene,and its hydroxy deriva-tive.It was concluded that the emission of VOCs into the chemosphere of the grapevine communities varied qualitatively and quantitatively depending on the level of the relationship complexity within each community in response to the infestation of grapevines by aphids,the presence of ladybirds as natural predators,and the presence of ant as protector.The grapevine’s status-dependent compounds can serve as indicators of infestation status and contribute to non-destructive early-stage diagnosis of the aphid. 展开更多

Closed-loop production management combines the process of history matching and production optimization together to peri-odically updates the reservoir model and determine the optimal control strategy for production development to realize the goal of decreasing the knowledge of model uncertainty as well as maximize the economic benefits for the expected reservoir life. The adjoint-gradient-based methods seem to be the most efficient algorithms for closed-loop management. Due to complicated calculation and limited availability of adjoint-gradient in commercial reservoir simulators, the application of this method is still prohibited for real fields. In this paper, a simultaneous perturbation stochastic approximation (SPSA) algorithm is proposed for reservoir closed-loop production management with the combination of a parameterization way for history matching and a co-variance matrix to smooth well controls for production optimization. By using a set of unconditional realizations, the proposed parameterization method can transform the minimization of the objective function in history matching from a higher dimension to a lower dimension, which is quite useful for large scale history matching problem. Then the SPSA algorithm minimizes the objective function iteratively to get an optimal estimate reservoir model. Based on a prior covariance matrix for production op-timization, the SPSA algorithm generates a smooth stochastic search direction which is always uphill and has a certain time correlation for well controls. The example application shows that the SPSA algorithm for closed-loop production management can decrease the geological uncertainty and provide a reasonable estimate reservoir model without the calculation of the ad-joint-gradient. Meanwhile, the well controls optimized by the alternative SPSA algorithm are fairly smooth and significantly improve the effect of waterflooding with a higher NPV and a better sweep efficiency than the reactive control strategy. 展开更多

Objective:To describe the information technology and artificial intelligence support in management experiences of the pediatric designated hospital in the wave of COVID-19 in Shanghai.Methods:We retrospectively concluded the management experiences at the largest pediatric designated hospital from March 1st to May 11th in 2022 in Shanghai.We summarized the application of Internet hospital,face recognition technology in outpatient department,critical illness warning system and remote consultation system in the ward and the structed electronic medical record in the inpatient system.We illustrated the role of the information system through the number and prognosis of patients treated.Results:The COVID-19 designated hospitals were built particularly for critical patients requiring high-level medical care,responded quickly and scientifically to prevent and control the epidemic situation.From March 1st to May 11th,2022,we received and treated 768 children confirmed by positive RT-PCR and treated at our center.In our management,we use Internet Information on the Internet Hospital,face recognition technology in outpatient department,critical illness warning system and remote consultation system in the ward,structed electronic medical record in the inpatient system.No deaths or nosocomial infections occurred.The number of offline outpatient visits dropped,from March to May 2022,146,106,48,379,57,686 respectively.But the outpatient volume on the internet hospital increased significantly(3,347 in March 2022 vs.372 in March 2021;4,465 in April 2022 vs.409 in April 2021;4,677 in May 2022 vs.538 in May 2021).Conclusions:Information technology and artificial intelligence has provided significant supports in the management.The system might optimize the admission screening process,increases the communication inside and outside the ward,achieves early detection and diagnosis,timely isolates patients,and timely treatment of various types of children. 展开更多

With the increasing scale of information technology(IT)service system,traditional thresholdbased static service level management(SLM)solution appears to be inadequate to meet current increasingly management requirement of SLM.Due to the stochastic service request rate,the random inherent failure and load surge of IT devices during service operating stage of large scaled IT system,service level objective(SLO)maintenance issue has become a realistic and important issue in dynamic SLM.This paper proposes a closed-loop feedback control mechanism to adaptively maintain SLO that service provider(SP)guaranteed at service operation stage.The mechanism can automatically tune the capacity of IT infrastructure according to service performance dispersion and reduce SLO violations.Considering that the tuning operations also affect service performance,fuzzy control is applied to alleviate the negative effect caused by tuning operations.In the dynamic SLM system that is applied with this mechanism compared with the traditional threshold-based solution,it is proved that the amount of SLO violations obviously decreases,the reliability of the service system increases relatively,and the resource utilization of IT infrastructure is optimized. 展开更多

目的:通过分析我院上报的结局为死亡的严重不良事件(SAE)的临床试验受试者信息、项目信息和核查问题,为提高临床试验SAE上报的准确性、及时性、规范性,加强临床试验风险管理、提升临床试验质量提供参考。方法:收集我院2016年1月—2019年... 目的:通过分析我院上报的结局为死亡的严重不良事件(SAE)的临床试验受试者信息、项目信息和核查问题,为提高临床试验SAE上报的准确性、及时性、规范性,加强临床试验风险管理、提升临床试验质量提供参考。方法:收集我院2016年1月—2019年7月上报到本院药物临床试验机构的死亡SAE事件,描述受试者性别、年龄、疾病分期、体能状态、合并疾病情况、上报科室、项目分期、项目设计、受试者死亡原因、SAE与研究药物相关性等,并结合死亡SAE专项核查发现的问题进行讨论。结果:134例死亡SAE受试者中,以男性、50岁以上人群、疾病分期Ⅳ期、入组时ECOG评分1分、接受生物制剂类药物为主,一半以上自身合并有其他疾病,多分布在消化肿瘤内科;死亡原因多为疾病进展相关死亡(73例,54.5%);死亡SAE与研究药物相关性为"有关"(74例,55.2%)。涉及Ⅰ~Ⅲ期临床试验55项,Ⅰ期项目的人均SAE数和人均死亡SAE数均较Ⅱ~Ⅲ期高。SAE报告核查发现问题包括:SAE延迟上报3例(2.2%)、SAE死亡依据不充分3例(2.2%)、SAE填写前后矛盾4例(3.0%),SAE与药物相关性判断依据不充分6例(4.5%)。结论:药物临床试验管理部门需对研究者加强SAE相关培训,并加强SAE相关的质量管理措施。 展开更多

Although online reverse commerce(recommerce)is convenient and efficient,it is not without caveats.It limits recommerce firms’flexibility to offer personalized prices and may cause mismatched grading between the firms and sellers of used products.This study examines a recommerce firm’s decision on grading criteria and prices.We find that the firm’s optimal policy exhibits two distinctly different patterns depending on the trade value of the product.We demonstrate that sellers’overestimate and underestimate errors have qualitatively different effects on firm profitability,and the effects crucially rely on the type of optimal policy.These findings can apprise firms on how to preset sorting criteria and prices as well as reduce grading errors. 展开更多
