Although advancements in technique have reduced the frequency and severity of common complications, reports of rare and serious complications such as pneumocephalus, remain scarce. Case Report: This article presents a case of pneumocephalus following CSEA in a middle-aged female patient undergoing surgery for an intrauterine space-occupying lesion. The patient experienced severe headache postoperatively, and imaging confirmed the presence of intracranial air. After receiving active symptomatic treatment, the patient recovered and was discharged. Conclusion: This case underscores the importance of adhering to standard anesthesia protocols and increasing awareness of rare CSEA complications, particularly the risk of pneumocephalus. Early recognition and timely management are crucial. There is a need to further enhance training and research in anesthetic procedures to improve clinical anesthesia quality and ensure patient safety.展开更多

文摘This study focused on the mechanical behavior of a deep-buried tunnel constructed in horizontally layered limestone, and investigated the effect of a new combined rockboltecable support system on the tunnel response. The Yujingshan Tunnel, excavated through a giant karst cave, was used as a case study.Firstly,a multi-objective optimization model for the rockboltecable support was proposed by using fuzzy mathematics and multi-objective comprehensive decision-making principles.Subsequently, the parameters of the surrounding rock were calibrated by comparing the simulation results obtained by the discrete element method(DEM)with the field monitoring data to obtain an optimized support scheme based on the optimization model. Finally, the optimization scheme was applied to the karst cave section, which was divided into the B-and C-shaped sections. The distribution range of the rockboltecable support in the C-shaped section was larger than that in the B-shaped section. The field monitoring results, including tunnel crown settlement, horizontal convergence, and axial force of the rockboltecable system, were analyzed to assess the effectiveness of the optimization scheme. The maximum crown settlement and horizontal convergence were measured to be 25.9 mm and 35 mm,accounting for 0.1%and 0.2%of the tunnel height and span, respectively.Although the C-shaped section had poorer rock properties than the B-shaped section, the crown settlement and horizontal convergence in the C-shaped section ranged from 46%to 97%of those observed in the B-shaped section. The cable axial force in the Bshaped section was approximately 60%of that in the C-shaped section. The axial force in the crown rockbolt was much smaller than that in the sidewall rockbolt.Field monitoring results demonstrated that the optimized scheme effectively controlled the deformation of the layered surrounding rock, ensuring that it remained within a safe range. These results provide valuable references for the design of support systems in deep-buried tunnels situated in layered rock masses.

文摘The deformation control of surrounding rock in gobside roadway with thick and hard roof poses a significant challenge to the safety and efficiency of coal mining. To address this issue,a novel approach combining directional and non-directional blasting techniques, known as combined blasting, was proposed. This study focuses on the experimental investigation of the proposed method in the 122108 working face in Caojiatan Coal Mine as the engineering background. The initial phase of the study involves physical model experiments to reveal the underlying mechanisms of combined blasting for protecting gob-side roadway with thick and hard roof. The results demonstrate that this approach effectively accelerates the collapse of thick and hard roofs, enhances the fragmentation and expansion coefficient of gangue, facilitates the filling of the goaf with gangue, and provides support to the overlying strata, thus reducing the subsidence of the overlying strata above the goaf.Additionally, the method involves cutting the main roof into shorter beams to decrease the stress and disrupt stress transmission pathways.Subsequent numerical simulations were conducted to corroborate the findings of the physical model experiments, thus validating the accuracy of the experimental results.Furthermore, field engineering experiments were performed, affirming the efficacy of the combined blasting method in mitigating the deformation of surrounding rock and achieving the desired protection of the gob-side roadway.

文摘The determination of source-side extracted heating parameters is of great significance to the economic operation of cogeneration systems. This paper investigated the coupling performance of a cogeneration heating and power system multidimensionally based on the operating characteristics of the cogeneration units, the hydraulic and thermodynamic characteristics of the heating network, and the energy loads.Taking a steam network supported by a gas-steam combined cycle cogeneration system as the research case, the interaction effect among the source-side prime movers, the heating networks, and the terminal demand thermal parameters were investigated based on the designed values, the plant testing data, and the validated simulation. The operating maps of the gas-steam combined cycle cogeneration units were obtained using THERMOFLEX,and the minimum source-side steam parameters of the steam network were solved using an inverse solution procedure based on the hydro-thermodynamic coupling model. The cogeneration operating maps indicate that the available operating domain considerably narrows with the rise of the extraction steam pressure and flow rate. The heating network inverse solution demonstrates that the source-side steam pressure and temperature can be optimized from the originally designed 1.11 MPa and 238.8°C to 1.074 MPa and 191.15°C,respectively.Under the operating strategy with the minimum source-side heating parameters, the power peak regulation depth remarkably increases to 18.30%whereas the comprehensive thermal efficiency decreases. The operation under the minimum source-side heating steam parameters can be superior to the originally designed one in the economy at a higher price of the heating steam. At a fuel price of$0.38/kg and the power to fuel price of 0.18 kg/(kW·h),the critical price ratio of heating steam to fuel is 119.1 kg/t.The influence of the power-fuel price ratio on the economic deviation appears relatively weak.

文摘BACKGROUND The management of polyps involving the appendiceal orifice(AO)presents notable challenges.Endoscopic resection is frequently hindered by operational complexities,a heightened risk of incomplete removal, and an elevated risk of procedural complications, including appendicitis.Conversely, surgical resection may entail unnecessary excision of intestinal segments, leading to potential morbidity.CASE SUMMARY Here,we reported two patients who presented with polyps deeply situated within the AO,with indistinct boundaries making it challenging to ensure completeness using traditional endoscopic resection. To overcome these challenges,we em-ployed combined endo-laparoscopic surgery(CELS),achieving curative resection without postoperative complications.CONCLUSION The application of CELS in managing polyps involving the AO is emerging as a safe and effective treatment modality.

文摘To effectively quantify the impact of distributed photovoltaic(PV)access on the distribution network, this paper proposes a comprehensive evaluation method of distributed PV grid connection combining subjective and objective combination of assignment and technique for order preference by similarity to an ideal solution(TOPSIS)—rank sum ratio(RSR)(TOPSIS-RSR)method.Based on the traditional distribution network evaluation system,a comprehensive evaluation system has been constructed. It fully considers the new development requirements of distributed PV access on the environmental friendliness and absorptive capacity of the distribution grid and comprehensively reflects the impact of distributed PV grid connection. The analytic hierarchy process(AHP)was used to determine the subjective weights of the primary indicators, and the Spearman consistency test was combined to determine the weights of the secondary indicators based on three objective assignment methods. The subjective and objective combination weights of each assessment indicator were calculated through the principle of minimum entropy.Calculate the distance between the indicators to be evaluated and the positive and negative ideal solutions, the relative closeness ranking, and qualitative binning by TOPSIS-RSR method to obtain the comprehensive evaluation results of different scenarios.By setting up different PV grid-connected scenarios and utilizing the IEEE33 node simulation algorithm, the correctness and effectiveness of the proposed subject-object combination assignment and integrated assessment method are verified.

文摘BACKGROUND There are very few cases of cardiac occluder detachment, and it is rare to completely remove the occluder using interventional methods without undergoing thoracotomy surgery after detachment. This case innovatively used ultrasound guidance combined with digital subtraction angiography(DSA)to completely remove the occluder, accumulating some experience.CASE SUMMARY The patient underwent left atrial appendage occlusion surgery in our hospital due to atrial fibrillation. After the surgery, the occluder fell off and became free in the left ventricle, which is very dangerous. We innovatively used ultrasound guidance, combined with DSA,and interventional surgery to successfully capture the free occluder using a catcher, completely remove it,and then re implant a new left atrial appendage occluder. After the surgery, the patient recovered very well.CONCLUSION The size selection of the occluder is slightly conservative, and the shape of the left atrial appendage opening is irregular.
文摘Plant derived natural fibers have been widely investigated as alternatives to synthetic fibers in reinforcing polymers.Researchers over the years have explored many plant fibers using different extraction processes to study their physical, chemical, and mechanical properties. In this context, the present study relates to the extraction, characterization, and optimization of Typha angustata L.stem fibers. For this purpose, desirability functions and response surface methodology were applied to simultaneously optimize the diameter(D),linear density(LD);yield(Y),lignin fraction(L),and tenacity(T)of Typha stem fibers.Typha stems have been subjected to both alkali(NaOH)and enzymatic(pectinex ultra-SPL)treatments.Three levels of process variables including enzyme concentration(10,15,and 20 ml/L)and treatment duration(10,15,and 20 days)were used to design the experiments according to the factorial design.Experimental results were examined by analysis of variance and fitted to second order polynomial model using multiple regression analysis. The Derringer’s desirability function released that the values of process variables generating optimized diameter, linear density, yield, lignin ratio and tenacity are 20 ml/L and 20 days for concentration of pectinex ultra-SPL enzyme and treatment duration, respectively.Confirmation was performed and high degree of correlation was found between the experimental and statistical values. Moreover, the morphological structure has been investigated by the scanning electron microscope, showing a crenelated structure of ultimate fiber bundles of cellulose composing the Typha fiber.Compared to Typha stem non-treated fibers(TSNTF),Typha stem combined treated fibers(TSCTF),brings to improve mechanical properties. This change in mechanical properties is affected by modifying the fiber structure showing alpha cellulose of(66.86%)and lignin ratio of(10.83%)with a crystallinity index of(58.47%).
文摘Background: Combined spinal-epidural anesthesia (CSEA) is widely used in clinical anesthesia due to its rapid onset, reliable anesthetic effect, and strong controllability. There is a need to further enhance training and research in anesthetic procedures to improve clinical anesthesia quality and ensure patient safety.

文摘Precise and timely prediction of crop yields is crucial for food security and the development of agricultural policies. However, crop yield is influenced by multiple factors within complex growth environments.Previous research has paid relatively little attention to the interference of environmental factors and drought on the growth of winter wheat. Therefore, there is an urgent need for more effective methods to explore the inherent relationship between these factors and crop yield, making precise yield prediction increasingly important. This study was based on four type of indicators including meteorological, crop growth status, environmental, and drought index, from October 2003 to June 2019 in Henan Province as the basic data for predicting winter wheat yield.Using the sparrow search al-gorithm combined with random forest(SSA-RF)under different input indicators, accuracy of winter wheat yield estimation was calcu-lated.The estimation accuracy of SSA-RF was compared with partial least squares regression(PLSR),extreme gradient boosting(XG-Boost),and random forest(RF)models. Finally, the determined optimal yield estimation method was used to predict winter wheat yield in three typical years.Following are the findings:1)the SSA-RF demonstrates superior performance in estimating winter wheat yield compared to other algorithms. The best yield estimation method is achieved by four types indicators’composition with SSA-RF)(R^(2)=0.805,RRMSE=9.9%.2)Crops growth status and environmental indicators play significant roles in wheat yield estimation, accounting for 46%and 22%of the yield importance among all indicators,respectively.3)Selecting indicators from October to April of the follow-ing year yielded the highest accuracy in winter wheat yield estimation, with an R^(2)of 0.826 and an RMSE of 9.0%.Yield estimates can be completed two months before the winter wheat harvest in June.4)The predicted performance will be slightly affected by severe drought.Compared with severe drought year(2011)(R^(2)=0.680)and normal year(2017)(R^(2)=0.790),the SSA-RF model has higher prediction accuracy for wet year(2018)(R^(2)=0.820).This study could provide an innovative approach for remote sensing estimation of winter wheat yield.yield.
