Clinical observation of scalp acupuncture plus speech rehabilitation for Broca's aphasia after cerebral stroke 被引量:26

Objective:To explore the clinical efficacy of scalp acupuncture plus speech rehabilitation in treating Broca&#39;s aphasia after cerebral stroke, for providing novel evidences for the treatment. Methods:Ninety-one eligible patients with Broca&#39;s aphasia after cerebral stroke were randomized into an observation group and a control group. Forty-six cases in the observation group were intervened by scalp acupuncture plus speech rehabilitation, while 45 cases in the control group were treated by speech rehabilitation alone. The aphasia battery of Chinese (ABC) and Boston diagnostic aphasia examination (BDAE) were adopted to evaluate the clinical efficacy. Results:After the treatment, the scores of oral expression, reading and writing and global score in the observation group were significantly higher than those in the control group (allP〈0.05). There was a significant difference in comparing the BDAE grading between the two groups after the treatment (P〈0.05). After intervention, the basically-recovered plus markedly-effective rate was 45.7% in the observation group versus 24.4% in the control group, and the between-group difference was statistically significant (P〈0.05). Conclusion:Scalp acupuncture plus speech rehabilitation is effective in treating Broca&#39;s aphasia after cerebral stroke, and worth promoting. 展开更多

Objective:To observe the clinical efficacy of Governor Vessel-unblocking and brain-refreshing scalp acupuncture for cerebral palsy (CP) complicated with intellectual disabilities. Methods:A total of 300 CP cases aged between 1 and 5 years were randomly allocated into a treatment group (n=150) and a control group (n=150). Patients in the treatment group were treated with Governor Vessel-unblocking and brain- refreshing scalp acupuncture combined with rehabilitation training, whereas patients in the control group received rehabilitation training alone. The Beijing Gesell developmental (Gesell) scale and gross motor function measure (GMFM) scale were used to assess the motor and intellectual development before and 3 months after the treatment. In addition, the head CT/MRI examination was applied to assess the brain nerve repair before and after the treatment. Results:After the treatment, the total effective rate in the treatment group was 78.0%, versus 42.7% in the control group, showing a statistical significance (P〈0.05). As for scores of five subscales in the Gesell scale, there were significant intra-group differences in the treatment group (allP〈0.05); and the scores in the treatment group were significantly better than those in the control group (allP〈0.05). As for scores of five dimensions in GMFM scale, there were significant intra-group differences in the treatment group (allP〈0.05); and the scores in the treatment group were significantly better than those in the control group (allP〈0.05). According to the head CT/MRI findings, the total effective rate was 73.3% in the treatment group, versus 62.0% in the control group, showing a statistical significance (P〈0.05). Conclusion:When used in combination with rehabilitative training, Governor Vessel-unblocking and brain-refreshing scalp acupuncture can significantly improve the clinical efficacy for cerebral palsy complicated with intellectual disabilities. It can also improve the patients&#39; motor function, intelligence and language ability. 展开更多

Objective: To observe the effect of Governor Vessel-unblocking and mind-refreshing needling technique on diffusion tensor imaging (DTI) in cerebral palsy (CP) children with periventricular leukomalacia (PVL) and investigate the underlying mechanism. Methods: A total of 18 CP children with PVL received modern rehabilitation training and scalp acupuncture. The before-after study was conducted to compare the fractional anisotropy (FA) of the white matter fiber tracts, gross motor function measure (GMFM) score, Gesell developmental scale and changes in muscle tone. Results: For the comparison before and after the first course of treatment, there were no statistical differences in DTI findings and FA values (both P〉0.05); there were statistical differences in GMFM dimensions A and B (both P〈0.05) and no statistical differences in dimensions C, D and E (all P〉0.05); and there were statistical differences in six items of Gesell development test (P〈0.01 or P〈0.05). For the comparison before and after two courses of treatment, there were statistical differences in DTI findings and FA values in 8 cases between before and after 30 and 60 treatments (both P〈0.05); there were statistical differences in GMFM dimensions A, B and C between before and after 30 and 60 treatments and between 30 and 60 treatments (all P〈0.05); there were no statistical differences in GMFM dimensions D and E (all P〉0.05); and there were statistical differences in six items of Gesell developmental test between before, after 30 and 60 treatments and between 30 and 60 treatments (P〈0.05 or P〈0.01). Conclusion: The Governor Vessel-unblocking and mind-refreshing scalp acupuncture has no significant efficacy on the repair and regeneration of white matter fibers in children with PVL and CP in a short period of time; however, it gradually works along with the prolonged treatment time. In addition, it can improve the motor function and mental development and the efficacy is closely associated with the time of treatment. 展开更多

Objective:To observe the efficacy of long-retaining scalp acupuncture plus interactive training in improving upper-extremity dysfunction in cerebral stroke patients.Methods:Ninety-five patients with upper-extremity dysfunction after cerebral stroke were randomized into two groups,with 48 cases in the treatment group and 47 cases in the control group.Conventional internal medicine treatment was offered to both groups.In both groups,Anterior Oblique Line of Vertex-temporal(MS 6,the middle 2/5)and Posterior Oblique Line of Vertex-temporal(MS 7,the middle 2/5)were selected from the same side of the brain lesion(the side apposing to the hemiplegic limb)for scalp acupuncture treatment.In the treatment group,the scalp acupuncture needles were retained for 7 h,in combination with interactive training,while the needles were also retained for 7 h in the control group but without interactive training.Prior to treatment and at 2-week and 4-week treatment,the two groups were scored using the functional test for the hemiplegic upper extremity-Hong Kong(FTHUE-HK)and simplified Fugl-Meyer assessment-upper extremity(FMA-UE).Results:The total effective rate was 97.9%in the treatment group,higher than 74.5%in the control group(P<0.01).The FTHUE-HK score was higher at 2-week and 4-week treatment than before treatment in both groups,presenting statistically significant intra-group differences(all P<0.001);the FTHUE-HK score was higher at 4-week treatment than at 2-week treatment in both groups,presenting statistically significant intra-group differences(both P<0.001).At 2-week and 4-week treatment,the FTHUE-HK score was higher in the treatment group than in the control group,showing significant between-group differences(both P<0.05).During the whole treatment process,the treatment group had higher FTHUE-HK scores compared with the control group,but there was no statistical significance comparing the change of the score between the two groups at 2-week treatment(P>0.05),while the between-group difference in the change of the score was statistically significant at 4-week treatment(P<0.05).The FMA-UE score was higher at 2-week and 4-weeks treatment than before treatment in both groups,presenting statistically significant intra-group differences(all P<0.001);the FMA-UE score was higher at 4-week treatment than at 2-week treatment in both groups,presenting statistically significant intra-group differences(both P<0.001).At 2-week and 4-week treatment,the FMA-UE was higher in the treatment group than in the control group,and the between-group differences were statistically significant(both P<0.01).The FMA-UE score rose gradually with the increase of treatment session,and there was statistical significance comparing the change of the score between the two groups at 2-week and 4-week treatment,respectively(both P<0.05).Conclusion:Long-retaining scalp acupuncture plus interactive training results in more significant efficacy than long-retaining scalp acupuncture alone in improving the upper-limb dysfunction after cerebral stroke and the advantage becomes more notable after 2-week consecutive treatment. 展开更多

Objective:To compare the effect of scalp acupuncture and scalp acupuncture plus acupuncture exercise therapy(AET)on walking ability in children with spastic cerebral palsy(CP).Methods:A total of 60 spastic CP children with gross motor function classification system(GMFCS)gradesⅠ-Ⅲwere divided into a control group and an observation group by the random number table method,with 30 cases in each group.Both groups were treated with the same conventional rehabilitation and scalp acupuncture therapy for CP.The control group received conventional rehabilitation first and then scalp acupuncture.The observation group received AET,which was to receive the conventional rehabilitation and scalp acupuncture simultaneously.Before and after treatment,the clinical efficacy was evaluated by the modified Ashworth scale(MAS)score,scores of dimensions D and E of the gross motor function measure(GMFM)scale,walking speed,and walking distance.Results:During treatment,there were 2 dropouts in the observation group.After 3 courses of treatment,the MAS scores in both the control group and observation group decreased compared with the same group before treatment(P<0.05),and the scores of dimensions D and E of the GMFM,walking speed,and walking distance were increased(P<0.05);the between-group comparison showed that the MAS score in the observation group was lower than that in the control group(P<0.05),and the scores of dimensions D and E of the GMFM,walking speed,and walking distance in the observation group were higher or longer than those in the control group(P<0.05).Conclusion:W ith the same treatments,scalp acupuncture combined with AET is superior to the conventional scalp acupuncture method in reducing lower-limb muscle tone,improving standing balance ability,and walking stability in children with spastic CP. 展开更多

Objective: To observe the therapeutic effect of scalp acupuncture combined with body acupuncture in treating cancer patients with intractable hiccups after radiochemotherapy. Methods; Thirty-two cancer patients with intractable hiccups after radiochemotherapy who met the inclusion criteria were selected. The Line 2 Lateral to Forehead (MS 3) of scalp stimulation areas as well as body acupoints including Danzhong (CV 17), Zhongwan (CV 12), bilateral Cuanzhu (BL 2), Neiguan (PC 6), Zusanli (ST 36) and Taichong (LR 3) were punctured. The treatment was given once a day, 5 treatments constituted a course, and the efficacy was evaluated after 2 treatment courses. Results: After 10 treatments, 26 cases were cured, 4 cases were improved, and 2 cases were invalid, and the total effective rate was 93.8%. Conclusion: Scalp acupuncture combined with body acupuncture in treating intractable hiccups in cancer patients after radiochemotherapy has reliable therapeutic effect, and it is easy-to-operate and can effectively alleviate the suffering of patients with less adverse reaction. 展开更多

Objective:To observe the therapeutic efficacy of acupuncture plus Tai Ji Quan (Tai Chi) in recovering the neurological function and treating depression state in post-stroke depression patients,together with a 12-month follow-up.Methods:A total of 105 eligible post-stroke depression patients were randomized into an acupuncture plus Tai Ji group (53 cases) and a control group (52 cases) based on their visiting sequence.The patients all received routine treatment and rehabilitation training for stroke.In addition,the control group was given oral administration of citalopram hydrobromide tablets,1 month as a course of treatment,for 3 courses in total.Meanwhile,the acupuncture plus Tai Ji group received acupuncture and practiced Tai Ji Quan,for 1 month and 12 months respectively.Before the intervention,after 1-month intervention and 12 months later,the National Institute of Health stroke scale (NIHSS),Barthel index (BI) and Hamilton depression rating scale (HAMD) were adopted for efficacy evaluation.Results:Prior to the intervention,there were no significant differences in HAMD,NIHSS and BI scores between the two groups (all P〉0.05);after 1-month intervention,there were significant between-group differences in NIHSS,BI and HAMD scores (P〈0.05 or P〈0.01);the 12-month follow-up revealed significant between-group differences in NIHSS,BI and HAMD scores (all P〈0.01).In the treatment of stroke,the total effective rate was 84.4% in the acupuncture plus Tai Ji group,significantly higher than 68.9% in the control group (P〈0.05);in the treatment of depression,the total effective rate was 86.7% in the acupuncture plus Tai Ji group,significantly higher than 77.8% in the control group (P〈0.05).Conclusion:Acupuncture plus Tai Ji Quan can produce a significant efficacy in improving the limb motor function and depression in post-stroke depression patients. 展开更多

Objective:To observe the clinical efficacy of Jin's three-needle therapy on post-stroke cognitive impairment(PSCI)and the effect on neuroelectrophysiology(event-related potentials).Methods:A total of 60 PSCI patients were selected and divided into a treatment group and a control group according to the method of random number table,with 30 cases in each group.The patients in the control group received routine treatment while the patients in the treatment group received additional Jin's three-needle therapy.The treatment for both groups lasted four weeks.Mini-mental state examination(MMSE)and Montreal cognitive assessment(MoCA)scores as well as amplitude and latency of potential 300(P300)were adopted to compare the between-group results before and after treatment.Results:Before treatment,there were no significant differences(all P>0.05)in MMSE and MoCA scores,P300 latency and P300 amplitude between the two groups.After 4 weeks of treatment,the MMSE and MoCA scores and P300 amplitudes were improved in both groups,and the P300 latencies became shorter.The results showed significant intra-group and between-group differences(all P<0.05).Conclusion:Based on the routine treatment,Jin's three-needle therapy is effective for PSCI.The mechanism is probably through its regulation on the patients'neuroelectrophysiology. 展开更多

Fire-needle therapy for deglutition disorders in post-stroke pseudobulbar palsy:a randomized controlled trial 被引量:6

Objective:To observe the clinical efficacy of fire-needle therapy in treating deglutition disorders due to pseudobulbar palsy in the remission stage of stroke.Methods:Sixty-two eligible subjects were divided into a fire-needle group and a rehabilitation group by a simple randomization method at a ratio of 1:1.The two groups received same basic intervention;in addition,the fire-needle group received fire-needle treatment,while the rehabilitation group received rehabilitation training.The two groups of subjects all received a 3-week treatment and were evaluated by the dysphagia severity rating scale (DSRS),modified Mann assessment of swallow ability (MMASA) and Kubota Toshio swallow test (KTST) before and after the intervention.The complications and adverse events occurred during the trial were recorded.The data were statistically analyzed.Results:At the third week,the DSRS,MMASA and KTST scores changed significantly compared with the baseline in both groups (P<0.05),and the changes in the fire-needle group were more significant than those in the rehabilitation group (P<0.05).The between-group comparison at the third week showed that the therapeutic efficacy in the fire-needle group was superior to that in the rehabilitation group (P<0.05).Conclusion:Fire-needle therapy can obviously change the DSRS,MMASA and KTST scores in pseudobulbar palsy in the remission stage of stroke,and significantly enhance the therapeutic efficacy of the treatment of deglutition disorders in this stage. 展开更多

Methods: A total of 92 cases with chronic alc... Objective: To observe the clinical efficacy of brain-benefiting and collateral-unblocking needling technique for chronic alcoholic gastritis complicated with depression. Methods: A total of 92 cases with chronic alcoholic gastritis complicated with depression were included in this trial. They were randomly allocated into an observation group(n=46) and a control group(n=46) by random number(envelope) method. Patients in the observation group received the brain-benefiting and collateral-unblocking needling technique. Scalp points included Shenting(GV 24), Xinhui(GV 22), Qianding(GV 23), Baihui(GV 20), Chengguang(BL 6), Tongtian(BL 7) and Luoque(BL 8). Body points included Neiguan(PC 6), Zusanli(ST 36), Zhongwan(CV 12), Gongsun(SP 4), Shenmen(HT 7), Daling(PC 7), Qimen(LR 14), Xinshu(BL 15) and Taichong(LR 3). The control group only received the same body acupuncture as the observation group. The treatment was conducted once a day, 30 min for each treatment, and 10 times made up a course of treatment. The efficacy was observed after 3 courses of treatment, and there was a 2-d interval between two courses. Results: After 3 courses of treatment, the clinical symptoms and gastroscopic features were significantly improved in the observation group than that in the control group. The clinical efficacy, self-rating anxiety scale(SAS) and self-rating depression scale(SDS) were significantly better than those in the control group(all P〈0.05). Conclusion: The brain-benefiting and collateral-unblocking needling technique can significantly improve clinical symptoms in patients with chronic alcoholic gastritis complicated with depression and substantially alleviate their gastroscopic features, anxiety and depression. 展开更多

Methods: One hundred depression patients were allocated to an observation group and a control group according to the random number table, with 50 cases in each group. The control group was intervened by oral administration of paroxetine tablets, 20 mg each time, once a day for successive 6 weeks; the observation group was additionally given cranial suture acupuncture, once a day for 6 weeks. They were scored by Hamilton depression scale-17 (HAMD-27) before the treatment and respectively after 1-week, 2-week, 4-week and 6-week treatment. The clinical efficacy and safety were also observed. Results: After 6-week treatment, the total effective rate was 94.0% in the observation group versus 78.0% in the control group, and the between-group difference was statistically significant (P〈0.01). The HAMD-17 scores respectively after 1-week, 2-week, 4-week and 6-week treatment were significantly lower than the score before the treatment in the observation group (all P〈0.05); the HAMD-17 scores respectively after 2-week, 4-week and 6-week treatment were significantly different from the score before the treatment in the control group (all P〈0.05). There were significant differences in the HAMD-17 score between the two groups respectively after 4-week and 6-week treatment (both P〈0.05). Conclusion: Cranial suture acupuncture plus paroxetine can ease the symptoms of depression, with faster onset and more significant therapeutic efficacy compared with paroxetine alone. 展开更多

Objective: To observe the clinical efficacy of acupuncture for posterior circulation ischemia vertigo (PCIV). Methods: A total of 71 PCIV cases were randomly allocated into an acupuncture group (37 cases) and a medication (oral Flunarizine Hydrochloride) group (34 cases). After 6 weeks of treatment, the therapeutic efficacies in the two groups were analyzed. Results: After treatment, the mean blood flow velocities (VM) of both the vertebral artery (VA) and the basilar artery (BA) in acupuncture group were significantly increased, showing statistical differences when compared with the pre-treatment result of the same group (P〈0.05); despite some decrease in pulsatility index (PI) after treatment, there was no statistical difference when compared with the pre-treatment result. After treatment, the mean blood flow velocities of both VA and BA in the medication group were increased, showing statistical differences when compared with the pre-treatment result of the same group (P〈0.05); there was no statistical difference in PI before and after the treatment (P〉0.05). Regarding the improvement of VM, the acupuncture group was superior to the medication group, showing a statistical difference (P〈0.05); there was no statistical difference in PI improvement between the two groups. Considering from the clinical efficacy, both acupuncture and medication are effective. However, acupuncture obtained a significantly better effect than medication (P〈0.05). Objective: To observe the effect of individualized needling protocol (on the basis of functional brain mapping and standardized syndrome differentiation in Chinese medicine) on gross motor functions of infants with cerebral palsy (CP) and thus study the innovative means of combining acupuncture with rehabilitation management technique. Methods: A total of 74 CP infants were randomized into a treatment group and a control group, 37 in each group. Combined individualized needling and rehabilitation training were used in the treatment group, versus combined Jin's needling (16 brain-benefiting points) and rehabilitation training in the control group. For both groups, acupuncture treatment was conducted once every other day and rehabilitation training every day. The dynamic changes of gross motor functions before treatment and after every 1-month treatment were measured and assessed using the Gross Motor Function Measure (GMFM)-66. Results: After treatment, the GMFM scores were significantly increased in both groups (P<0.01); there were no between-group statistical differences in increase range of the GMFM scores (P>0.05); the treatment protocol was not interacted with the treatment time; and the GMFM scores for spastic CP infants were better in the treatment group than that in the control group (P<0.05). Conclusion: Combined scalp acupuncture and rehabilitation training can produce positive effects on gross motor functions of CP infants. Individualized needling can obtain better effects than 16 brain-benefiting points in improving the gross motor functions of spastic CP infants. 展开更多

Objective:To observe the event related potential 300 (P300) after scalp acupuncture treatment in patients with cognitive disorder due to cerebral infarction,and evaluate its therapeutic effect.Methods:Sixty-six patients aged below 70 years old with cognitive disorder after initial onset of cerebral infarction were randomized into a treatment group and a control group.Conventional neurological drugs and physical trainings were prescribed to both groups.In addition,the treatment group received scalp acupuncture.The two groups were compared by evaluating the latency and amplitude of P300 before and after the 3-month treatment.Results:After treatment,the latency of P300 was shortened by 39.1 ms in the treatment group,versus 16.7 ms in the control group,and the difference was statistically significant (P0.05).Conclusion:P300 can objectively evaluate the therapeutic effect of scalp acupuncture in treating the patients with cognitive disorder due to cerebral infarction.Scalp acupuncture has positive effect in treating this group of patients. 展开更多

Methods:A total of 94 cases who met the inclusion criteria were randomized into an observation group using combined Jin's three-needle and rehabilitation training and a control group using rehabilitation training alone. The therapeutic efficacies were then assessed respectively before treatment, 14 d and 28 d after treatment using neurological deficient scale (NDS) and short-form Fugl-Meyer assessment (SFFMA) scale. Results:After treatment, the NDS and SFFMA scores in both groups were significantly improved (P<0.05); after 28-day treatment, the inter-group comparison showed a statistical significance (P<0.05). Conclusion:Combined Jin's three-needle and rehabilitation training and rehabilitation training alone can both enhance the nerve function recovery of patients with post-stroke hemiplegia and improve their motor function; however, the former could obtain better effects than the latter. 展开更多

Objective: To observe the effect of Governor Vessel-unblocking and mind-refreshing acupuncture plus functional training on neural development in infants with brain damage and seek an effective method for early intervention of infantile brain damage. Methods: Eighty infants with brain injury were recruited and allocated to a treatment group and a control group by their visiting sequence, with 40 cases in each group. The control group received exercise training, 40 min each session and 6 sessions a week, and tuina treatment, 30 min each time and 6 times a week. Based on the treatment protocol for the control group, the treatment group additionally received Governor Vessel-unblocking and mind-refreshing acupuncture, 3 times a week and 10 sessions as a course at a 2-week interval. Before the treatment and after 14-week treatment, the gross motor function measure (GMFM) and developmental quotient (DQ) of Bejing Gesell developmental scale were used to evaluate the development of the infants. Results: After the treatment, the GMFM score and DOs of Gesell scale all increased by different levels in the two groups, and the intra-group differences were statistically significant (all P〈0.05); the scores of the treatment group were superior to those of the control group, and the between-group differences were statistically significant (all P〈0.05). Conclusion: Governor Vessel-unblocking and mind-refreshing acupuncture plus functional training can significantly promote the development of gross motor and cognitive functions in infants with brain damage, and it is an early and effective intervention for infantile brain damage. 展开更多

Objective:To observe the clinical effect of regulating Conception Vessel and unblocking Governor Vessel by acupuncture combined with tuina for cerebral infarction. Methods:A total of 61 eligible cases with cerebral infarction were treated with regulating Conception Vessel and unblocking Governor Vessel by acupuncture combined with tuina, once a day, 12 d for a course. There was a 3-day interval between two courses. These cases were treated for 3 courses. Before treatment, the infarction severity was graded using neurological deficit score. The score changes and correlation between infarction severity and therapeutic efficacy were evaluated after 1, 2 and 3 courses of treatment. Results:Before treatment, the neurological deficit score was (18.65±3.28), versus (13.63±3.91), (9.53±3.22) and (5.57±3.97) respectively after 1, 2 and 3 courses of treatment. When the third course was completed, 4 cases obtained almost full recovery, 33 cases obtained marked effect, 18 cases obtained improvement and 6 cases had no effect. The total effective rate was 90.2%. Conclusion:Regulating Conception Vessel and unblocking Governor Vessel by acupuncture combined with tuina can substantially improve limb function in patients with hemiplegia due to cerebral infarction. 展开更多
