BACKGROUND Congenital anomalous retinal artery is rare and does not typically affect visual acuity.However,an abnormal artery that passes through and supplies blood to the macular area complicated with branch retinal ...BACKGROUND Congenital anomalous retinal artery is rare and does not typically affect visual acuity.However,an abnormal artery that passes through and supplies blood to the macular area complicated with branch retinal artery occlusion may negatively impact visual acuity.This study reports an unusual case of anomalous retinal artery combined with retinal artery occlusion.CASE SUMMARY A 52-year-old male presented with severely reduced vision in the right eye.The fundus examination revealed an anomalous artery,extending from the superior temporal arcade and crossing the macula into the inferior temporal quadrant.The anomalous artery was partially occluded,with a narrowed lumen.A cherry-red spot was observed with whitening of the macular area,suggesting macular edema.Fundus fluorescein angiography revealed disc leakage and a delayed filling time.Optical coherence tomography revealed increased thickness of the neuroretina and underlying layers.The patient was treated with vessel dilation,hyperbaric oxygen,ocular massage,and thrombolytics.Visual acuity of the right eye subsequently improved to 20/200 from hand motion at 4 cm.This improvement in visual acuity persisted when the patient was examined at the 1-mo follow-up visit.The patient was subsequently followed via telephone interview.The information provided via interview indicated that visual acuity in the affected eye was stable up to 6 years from the time of the initial presentation.However,after 3 additional years,the patient was diagnosed with neovascular glaucoma in the right eye,which was subsequently enucleated.CONCLUSION Although congenital retinal vascular anomaly,including anomalous retinal artery,rarely affects vision,when complicated with branch retinal artery occlusion,the abnormal artery that supplies the macula may severely reduce visual acuity.展开更多

Objectives:We aim to describe the efficacy,safety,and characteristics of the Amplatzer Vascular Plug(AVP)II and IV“off-label”use for multiple cardiovascular occlusions in children under 10 years.Methods:Observationa...Objectives:We aim to describe the efficacy,safety,and characteristics of the Amplatzer Vascular Plug(AVP)II and IV“off-label”use for multiple cardiovascular occlusions in children under 10 years.Methods:Observational retrospective multicenter(2007–2020,6 centers)review of paediatric procedures using AVP II or IV.Results:A total of 125 children(49.6%aged≤1 year,147 lesions)underwent 136 successive procedures(success rate:98.5%)using 169 devices(109 AVP IV,60 AVP II).The mean device diameter was 7.7±3.2 mm(4–20 mm).The median AVP size to vessel diameter ratio was 1.3(0–2).The median age and weight at implantation were 1.0 year(0.01–9.98)and 8.4 kg(1–69).Procedures were heterogeneous(55 patent ductus arteriosus(PDA),28 collaterals,18 sequestrations,22 arteriovenous/veinovenous/coronary fistulas,6 vertical veins,6 conduits,5 ventricular septal defects,7 miscellaneous).Day 1 and 6-month occlusion rates were respectively 94.8%and 98.5%.Major adverse events(MAE)occurred in 5.2%of cases(no procedure-related deaths),and more frequently in weight≤5 kg(p=0.01),younger patients(p=0.03)during PDA closure(p=0.02)of tubular types(p=0.02)using larger devices(p=0.03)and AVP II(p=0.003).Independent predictor of MAE risk was a higher AVP diameter to patient weight ratio(Odds-ratio:2.33,95%confidence interval 1.31–4.13,p=0.004,optimal cut off:1.45).Conclusions:Both AVPs are safe and effective for percutaneous occlusions in children under 10.Such devices represent an alternative“off label”use for well selected paediatric patients.展开更多

Four cases without sufficient details were found. A protocol was adopted from a scientific reference to deal with similar cases properly. Results: The abnormal rates of Ops and b wave were higher in retinal vascular occlusion. The abnormal ERG appeared mostly in CRVO among four types of retinal vascular occlusion. The comparisons of some amplitudes and latencies between the fellow eyes and the affected eyes showed statistically significant difference.
Conclusion: The oscillatory potentials and b waves have important roles in evaluating the retinal function of patients with retinal vascular occlusions. There are some clinical significance for comparing ERG between the affected eye and the fellow eye. Eye Science 2001; 17: 50 ~ 53.展开更多

METHODS: Randomized controlled trials (RCTs) co- mparing hemihepatic vascular occlusion and total he- patic inflow occlusion were included by a systematic literature search. Two authors independently assessed the trials for inclusion and extracted the data. A meta- analysis was conducted to estimate blood loss, transfu- sion requirement, and liver injury based on the levels of aspartate aminotransferase (AST) and alanine arni- notransferase (ALT). Either the fixed effects model or random effects model was used. RESULTS- Four RCTs including 338 patients met the predefined inclusion criteria. A total of 167 patients were treated with THO and 171 with HHO. Metaanalysis of AST levels on postoperative day 1 indicated higher levels in the THO group with weighted mean dif- ference (WMD) 342.27; 95% confidence intervals (CI) 217.28-467.26; P = 0.00001; I2 = 16%. Meta-analysis showed no significant difference between THO group and HHO group on blood loss, transfusion requirement, mortality, morbidity, operating time, ischemic duration, hospital stay, ALT levels on postoperative day 1, 3 and 7 and AST levels on postoperative day 3 and 7. CONCLUSION: Hemihepatic vascular occlusion does not offer satisfying benefit to the patients undergoing hepatic resection. However, they have less liver injury after liver resections.展开更多

All of the original articles were evaluated according to date of publications, countries, clinics and topics. Studies about occlusive vascular diseases were evaluated according to their qualifications, review methods and results. SPSS for Windows 16.0 was used in data analysis and data were expressed as mean, standard deviation and percentage.RESULTS: A total of 735 original research articles were investigated. The number of publications have shown a regular logarithmic increase over the years. Thirty-two percent of all publications were performed by clinics in Turkey and 56.4% were performed by general-oncological surgery and cardiology clinics. A total of 107 publications were identified to be about occlusive vascular diseases, 80.3% of these publications were found to be prognostic and 19.6% to be diagnostic, 82.2% of them were found to be planned as retrospective and 17.7% as prospective. In 95.3% of prognostic publications, there was a positive correlation between high NLR values at admission and poor prognosis. In 95.3% of diagnostic publications high NLR values at admission were identifi ed to be signifi cant diagnostically.CONCLUSION: Elevated neutrophil-to-lymphocyte ratio at admission, could be used as a diagnostic and/or prognostic parameter in occlusive vascular diseases.展开更多
BACKGROUND Conventional recanalization techniques may fail in patients with completely occluded superior vena cava(SVC).AIM To analyze the effectiveness and complications of sharp recanalization for completely occlude...BACKGROUND Conventional recanalization techniques may fail in patients with completely occluded superior vena cava(SVC).AIM To analyze the effectiveness and complications of sharp recanalization for completely occluded SVC.METHODS This was a retrospective study of patients that underwent puncture and recanalization of the SVC between January 2016 and December 2017 at our hospital.Sharp recanalization was performed using the RUPS-100 system.The patients were followed for 12 mo.The main outcomes were the patency rate of SVC and arteriovenous fistula flow during dialysis.RESULTS The procedure was successful in all 14 patients(100%).Blood pressure in the distal SVC decreased in all 14 cases(100%)from 26.4±2.7 cmH2O to 14.7±1.3 cmH2O(P<0.05).The first patency rates of the SVC at 24 h and at 3,6,9 and 12 mo after sharp recanalization were 100%,92.9%,85.7%,78.6%and 71.4%,respectively.There were two(14.3%)severe,one(7.1%)moderate and one(7.1%)minor complication.The severe complications included one case of pericardial tamponade and one case of hemothorax.CONCLUSION The results suggest that sharp recanalization can be an additional tool to extend or renew the use of an occluded upper extremity access for hemodialysis.This could be of use in patients with long-term maintenance hemodialysis in whom the maintenance of central venous access is often a challenge.展开更多

Laser photocoagulation has been an established method for treating macular edema for many years but nowadays intravitreal injection of Anti-Vascular endothelial growth factors (Ranibizumab, Aflibercept Bevacizumab and Pegaptanib sodium) is the treatment of choice for macular edema from retinal vein occlusion. Intra-vitreal corticosteroids Triamcinolone Acetonide and Dexamethasone implant, are also being used to treat in some macular edema cases but with higher rates of side effects. Numerous surgical methods have been attempted for treating RVO and preventing macular edema;they include pars plana vitrectomy, radial optic neurotomy, laser induced chorioretinal anastomosis, and arteriovenous sheathotomy. Surgical methods supposedly relieve compression of the central retinal vein, altering the pathophysiology of vein occlusion at the level of the lamina cribrosa thus improving blood flow and oxygenation. METHODS: This study included 16 eyes of 16 patients with RVO-associated ME. Patients were initially treated with oral prednisone and an intravitreal anti-VEGF agent. Two weeks later, patients underwent standard laser photocoagulation. Best-corrected visual acuity (BCVA), central retinal thickness (CRT), and retinal vessel oxygenation were examined over 12mo. RESULTS: Patients received 1.43+0.81 anti-VEGF injections. Mean baseline and 12-month IogMAR BCVA were 0.96±0.51 (20/178) and 0.31±0.88 (20/40), respectively, in eyes with central retinal vein occlusion (CRVO) (P〈0.00), and 1.02±0.45 (201209) and 0.60±0.49 (20/80), respectively, in eyes with branch retinal vein occlusion (BRVO) (P〈0.00). At 12mo, CRT had significantly decreased in eyes with CRVO (P〈0.00) and BRVO (P〈0.00). Venous oxygen saturation had significantly increased in eyes with CRVO (P〈0.00) and BRVO (P〈0.00). No examined parameters were significantly different between the 2 RVO groups. No serious adverse effects occurred. CONCLUSION: Anti-VEGF, glucocorticoid, and photocoagulation combination therapy improves visual outcome, prolongs therapeutic effect, and reduces the number of intravitreal injections in eyes with RVO- associated ME.展开更多
AIM: To observe the best-corrected visual acuity(BCVA) and central foveal thickness(CFT) repeatedly after the intravitreal injection of conbercept(IVC) for treating cystoid macular edema(CME) in branch retinal vein oc...AIM: To observe the best-corrected visual acuity(BCVA) and central foveal thickness(CFT) repeatedly after the intravitreal injection of conbercept(IVC) for treating cystoid macular edema(CME) in branch retinal vein occlusion(BRVO) and explore the relationship between the duration of CME and visual outcome.METHODS: Subgroup analysis was performed to compare short-term(within 90 d of CME onset) and longterm(over 90 d of CME onset) macular edema in BRVO.After an initial IVC, a pro re nata(PRN) strategy was performed according to the recurrence of CFT or decrease of BCVA.Analysis of variance using repeated measurements, statistical analysis following indicators including BCVA and CFT collected at baseline and 1, 3, and 6 mo after IVC.RESULTS: Among the 60 cases included in this retrospective study, 36 were short-term CME, and 24 were long-term CME.There were statistical significances between and within groups of the BCVAs at different time points(P<0.001).The interaction was found between group and time(P=0.006), indicating the difference in the speed of BCVA improvement between groups.In particular, the improvement speed of BCVA in the short-term CME group was faster than that in the long-term CME group.There were significant differences between and with groups of the CFT at different time points(P<0.001).However, the interaction between group and time in relation to CFT had no significant differences(P=0.59).CONCLUSION: IVC treatment for CME following BRVO is effective and safe.The duration of CME before treatment is a significant predictor of the visual outcomes of patients with BRVO.The improvement of vision might be faster with early IVC treatment than with delayed treatment.展开更多

Complications after injections were recorded. The changes in CFT and BCVA were compared with Wilcoxon sign-rank tests.Results:.The CFT was significantly reduced and the BCVA was significantly improved at 1, 2, and 3 months after injection(all P < 0.05). Tomography findings revealed no recurrence within 3 months. No systemic thromboembolic events,elevated intraocular pressure, retinal detachment, or infectious endophthalmitis occurred following injection.Conclusion:.Single intravitreal aflibercept may be useful in treating macular edema associated with BRVO within 3months. AIM:To evaluate whether a novel tyrosine kinase inhibitor nintedanib could inhibit basic fibroblast growth factor(bFGF)and vascular endothelial growth factor(VEGF)simultaneously for retinal vascular disease in vivo.ME...AIM:To evaluate whether a novel tyrosine kinase inhibitor nintedanib could inhibit basic fibroblast growth factor(bFGF)and vascular endothelial growth factor(VEGF)simultaneously for retinal vascular disease in vivo.METHODS:After a laser induced rabbit retinal vein occlusion(RVO)model was made,0.5 mg of nintedanib was injected intravitreally in the left eye on the third day while the right eye was as a control.Intracameral samples were taken on the day before laser treatment and days 1,3,7,14,21,and 28 after treatment.Enzyme-linked immunosorbent assay(ELISA)was used to test the bFGF and VEGF-A concentrations in the aqueous humor.RESULTS:Both bFGF and VEGF-A rose significantly on the third day after laser treatment in both eyes.In the control eye the bFGF concentration peaked on the 14th day while the VEGF-A concentration dropped rapidly soon after the third day.After nintadanib injection in the study eye,both bFGF and VEGF-A showed a significant reduction on the 4th day(7th day after laser treatment)when compared to the control eye,and kept on low level in the following several weeks.CONCLUSION:Intravitreal injection of nintedanib can inhibit the expression of bFGF and VEGF in the process of RVO model to a certain extent,which is expected to become a new method for the treatment of retinal vascular diseases or fibrotic diseases.展开更多
Object:The aim is to evaluate how effective Traditional Chinese Medicine(TCM)is in treating patients who have central retinal vein occlusion with macular edema(CRVO-ME)in a real-world study.Furthermore,the objective o...Object:The aim is to evaluate how effective Traditional Chinese Medicine(TCM)is in treating patients who have central retinal vein occlusion with macular edema(CRVO-ME)in a real-world study.Furthermore,the objective of the research was to examine the TCM prescription trends in the management of CRVO-ME.Method:A single-center real-world study(RWS)was carried out over a span of 19 years,following the established design.The study encompassed 113 patients diagnosed with CRVO-ME.Out of these,74 patients received TCM treatment,while the remaining individuals underwent a combined therapy involving TCM and anti-VEGF drugs through intravitreal injection.The patients were matched using propensity score matching(PSM).The result measured in the RWS was BCVA.The oral prescriptions for CRVO-ME that led to observable and effective outcomes were collected.Excel and the TCM Inheritance Auxiliary Platform V2.5 were utilized to optimize mutual information,hierarchical clustering based on entropy,and other techniques to extract medication regulations and features.Result:After applying PSM,each group comprised 29 cases.Both groups exhibited improved BCVA following treatment;however,there was no statistically significant distinction in BCVA or effectiveness between the two groups(all P>0.05).Apart from the analysis of oral prescriptions for CRVO-ME,the investigation pinpointed the most frequently used TCMs,namely Flos Carthami,Semen Persicae,Radix Angelica sinensis,Radix Rehmanniae,and Radix et Rhizoma Notoginseng.Frequently utilized medications tended to possess cold,warm,or mild attributes and exhibited a taste profile that was either bitter or sweet.The primary meridians associated with the medicines employed in treating CRVO-ME were liver,spleen,stomach,heart,and lung.Through the application of association rule analysis,it was discerned that there were 195 commonly employed combinations of medicines.Additionally,a complex system entropy cluster analysis unveiled 13 key combinations of medicines.By employing an unsupervised entropy hierarchical clustering analysis,a novel prescription was formulated.Conclusion:Within a real-world population of CRVO-ME patients,TCM exhibited its effectiveness.The treatment approach for CRVO-ME predominantly involved the regulation of qi(Qi is an exceedingly subtle substance within the human body,brimming with vitality and ceaseless motion.It constitutes the fundamental element that shapes and sustains the various processes of human life.)and blood as well as the resolution of dampness.The oral prescriptions frequently targeted the meridians of liver,spleen,stomach,heart,and lungs.展开更多
Background:To evaluate a fully automated vascular density(VD),skeletal density(SD)and fractal dimension(FD)method for the longitudinal analysis of retinal vein occlusion(RVO)eyes using projection-resolved optical cohe...Background:To evaluate a fully automated vascular density(VD),skeletal density(SD)and fractal dimension(FD)method for the longitudinal analysis of retinal vein occlusion(RVO)eyes using projection-resolved optical coherence tomography angiography(OCTA)images and to evaluate the association between these quantitative variables and the visual prognosis in RVO eyes.Methods:Retrospective longitudinal observational case series.Patients presenting with RVO to Creteil University Eye Clinic between October 2014 and December 2018 and healthy controls were retrospectively evaluated.Group 1 consisted of central RVO(CRVO)eyes,group 2 consisted of eyes with branch RVO(BRVO)and group 3 of healthy control eyes.OCTA acquisitions(AngioVue RTVue XR Avanti,Optovue,Inc.,Freemont,CA)were performed at baseline and last follow up visit.VD,SD,and FD analysis were computed on OCTA superficial and deep vascular complex(SVC,DVC)images at baseline and final follow up using an automated algorithm.Logistic regression was performed to find if and which variable(VD,SD,FD)was predictive for the visual outcome.Results:Forty-one eyes,of which 21 consecutive eyes of 20 RVO patients(13 CRVO in group 1,8 BRVO in group 2),and 20 eyes of 20 healthy controls were included.At the level of SVC,VD and FD were significantly lower in RVO eyes compared to controls(P<0.0001 and P=0.0008 respectively).Best-corrected visual acuity(BCVA)at last follow-up visit was associated with baseline VD(P=0.013),FD(P=0.016),and SD(P=0.01)at the level of the SVC,as well as with baseline FD at the DVC level(P=0.046).Conclusions:Baseline VD,SD,and FD are associated with the visual outcome in RVO eyes.These parameters seem valuable biomarkers and may help improve the evaluation and management of RVO patients.展开更多

Recently, resveratrol has been shown to exhibit neuroprotective effects in models of Parkinson's disease, cerebral ischemia and Alzheimer's disease. However, its effects on vascular dementia remain unclear. The present study established a rat model of vascular dementia using permanent bilateral common carotid artery occlusion. At 8-12 weeks after model induction, rats were intragastrically administered 25 mg/kg resveratrol daily. Our results found that resveratrol shortened the escape latency and escape distances in the Morris water maze, and pro- longed the time spent percentage and swimming distance percentage in the target quadrant during the probe test, indicating that resveratrol improved learning and memory ability in vascular dementia rats. Further experiments found that resveratrol decreased malonyldialdehyde levels, and increased superoxide dismutase activity and glutathione levels in the hippocampus and cerebral cortex of vascular dementia rats. These results confirmed that the neuroprotective effects of resveratrol on vascular dementia were associated with its anti-oxidant properties.展开更多

文摘BACKGROUND Congenital anomalous retinal artery is rare and does not typically affect visual acuity.However,an abnormal artery that passes through and supplies blood to the macular area complicated with branch retinal artery occlusion may negatively impact visual acuity.This study reports an unusual case of anomalous retinal artery combined with retinal artery occlusion.CASE SUMMARY A 52-year-old male presented with severely reduced vision in the right eye.The fundus examination revealed an anomalous artery,extending from the superior temporal arcade and crossing the macula into the inferior temporal quadrant.The anomalous artery was partially occluded,with a narrowed lumen.A cherry-red spot was observed with whitening of the macular area,suggesting macular edema.Fundus fluorescein angiography revealed disc leakage and a delayed filling time.Optical coherence tomography revealed increased thickness of the neuroretina and underlying layers.The patient was treated with vessel dilation,hyperbaric oxygen,ocular massage,and thrombolytics.Visual acuity of the right eye subsequently improved to 20/200 from hand motion at 4 cm.This improvement in visual acuity persisted when the patient was examined at the 1-mo follow-up visit.The patient was subsequently followed via telephone interview.The information provided via interview indicated that visual acuity in the affected eye was stable up to 6 years from the time of the initial presentation.However,after 3 additional years,the patient was diagnosed with neovascular glaucoma in the right eye,which was subsequently enucleated.CONCLUSION Although congenital retinal vascular anomaly,including anomalous retinal artery,rarely affects vision,when complicated with branch retinal artery occlusion,the abnormal artery that supplies the macula may severely reduce visual acuity.

Vascular occlusion due to microembolism is more common in choroidal than in retinal vasculature. Due to its unique hemodynamic characteristics, which involve one of the highest rates of blood flow in the body and the fact that it is functionally an end artery system, the choroidal circulation is prone to platelet emboli associated with cardiovascular disease and other pathologies with increased risk of microembolismt.

文摘The vascular occlusion test (VOT) is an experiment that simulates the process of distal ischemia-reperfusion of tissue based on the monitoring of tissue oxygen saturation (StO2) to obtain VOT parameters related to StO2 changes to evaluate microcirculation function of the measured muscle tissue. VOT has the advantages of real-time, noninvasive and continuous, and is widely used to evaluate the functional status of peripheral microcirculation and the relationship between microcirculation changes and prognosis of patients. In clinical work, doctors mainly focus on hemodynamics of patients, while microcirculation function is often neglected due to the lack of real-time monitoring methods, which may lead to severe poor prognosis of patients, especially patients with critical illness. This paper focuses on the clinical application of vascular occlusion test based on tissue oxygen saturation monitoring.
文摘Objectives:We aim to describe the efficacy,safety,and characteristics of the Amplatzer Vascular Plug(AVP)II and IV“off-label”use for multiple cardiovascular occlusions in children under 10 years.Methods:Observational retrospective multicenter(2007–2020,6 centers)review of paediatric procedures using AVP II or IV.Results:A total of 125 children(49.6%aged≤1 year,147 lesions)underwent 136 successive procedures(success rate:98.5%)using 169 devices(109 AVP IV,60 AVP II).The mean device diameter was 7.7±3.2 mm(4–20 mm).The median AVP size to vessel diameter ratio was 1.3(0–2).The median age and weight at implantation were 1.0 year(0.01–9.98)and 8.4 kg(1–69).Procedures were heterogeneous(55 patent ductus arteriosus(PDA),28 collaterals,18 sequestrations,22 arteriovenous/veinovenous/coronary fistulas,6 vertical veins,6 conduits,5 ventricular septal defects,7 miscellaneous).Day 1 and 6-month occlusion rates were respectively 94.8%and 98.5%.Major adverse events(MAE)occurred in 5.2%of cases(no procedure-related deaths),and more frequently in weight≤5 kg(p=0.01),younger patients(p=0.03)during PDA closure(p=0.02)of tubular types(p=0.02)using larger devices(p=0.03)and AVP II(p=0.003).Independent predictor of MAE risk was a higher AVP diameter to patient weight ratio(Odds-ratio:2.33,95%confidence interval 1.31–4.13,p=0.004,optimal cut off:1.45).Conclusions:Both AVPs are safe and effective for percutaneous occlusions in children under 10.Such devices represent an alternative“off label”use for well selected paediatric patients.
文摘With the increased demand for dermal filler injections, more complications and adverse events are reported. Vascular occlusion is one of the most serious complications. The need urges to adopt a powerful protocol to manage such cases. The private practice I am working for has six branches in the middle east. I tried to investigate if there were any vascular occlusion incidences through the last three years. Four cases without sufficient details were found. A protocol was adopted from a scientific reference to deal with similar cases properly. A re-audit was done after six months to test the efficiency of the suggested protocol.
文摘Purpose: to measure and analyze the electroretinogram (ERG) in patients with retinal vascular occlusion.
Method: fifty-eight cases (59 eyes) of retinal vascular occlusion and the fellow eyes of 47 cases were tested with Ganzfeld ERG. The scotopic and photopic ERG, and oscillatory potentials were tested according to the ERG standard of ISCEV.
Results: The abnormal rates of Ops and b wave were higher in retinal vascular occlusion. The abnormal ERG appeared mostly in CRVO among four types of retinal vascular occlusion. The comparisons of some amplitudes and latencies between the fellow eyes and the affected eyes showed statistically significant difference.
Conclusion: The oscillatory potentials and b waves have important roles in evaluating the retinal function of patients with retinal vascular occlusions. There are some clinical significance for comparing ERG between the affected eye and the fellow eye. Eye Science 2001; 17: 50 ~ 53.

文摘It is very important to study the factors affecting the incidence,progress and prognosis of patients with vascular dementia.50 cases of severe middle cerebral artery stenosis or occlusion underwent endovascular treatment(25 cases of mild cognitive dysfunction,25 cases of moderate cognitive dysfunction)were divided into two groups,where a medical drug treatment group and a control group established with 25 cases in each group.The cognitive function of each group of patients was evaluated before operation,7 days after operation,30 days after operation,and 180 days after operation.CTP was used to compare the hemodynamic changes in patients before and after operation.The severe stenosis or occlusion of the middle cerebral artery in patients can be improved,and the intracranial blood supply of patients with poorly compensated medial cranial circulation and hypoperfusion can be restored to a certain extent.Meanwhile,improvement of cognitive function was definitive in some patients with cognitive dysfunction.To guide the formulation of treatment plans for patients with severe middle cerebral artery stenosis or occlusion.
文摘AIM: To evaluate the clinical outcomes of patients undergoing hepatectomy with hemihepatic vascular occlusion (HHO) compared with total hepatic inflow occlusion (THO). METHODS: Randomized controlled trials (RCTs) co- mparing hemihepatic vascular occlusion and total he- patic inflow occlusion were included by a systematic literature search. Two authors independently assessed the trials for inclusion and extracted the data. A meta- analysis was conducted to estimate blood loss, transfu- sion requirement, and liver injury based on the levels of aspartate aminotransferase (AST) and alanine arni- notransferase (ALT). Either the fixed effects model or random effects model was used. RESULTS- Four RCTs including 338 patients met the predefined inclusion criteria. A total of 167 patients were treated with THO and 171 with HHO. Metaanalysis of AST levels on postoperative day 1 indicated higher levels in the THO group with weighted mean dif- ference (WMD) 342.27; 95% confidence intervals (CI) 217.28-467.26; P = 0.00001; I2 = 16%. Meta-analysis showed no significant difference between THO group and HHO group on blood loss, transfusion requirement, mortality, morbidity, operating time, ischemic duration, hospital stay, ALT levels on postoperative day 1, 3 and 7 and AST levels on postoperative day 3 and 7. CONCLUSION: Hemihepatic vascular occlusion does not offer satisfying benefit to the patients undergoing hepatic resection. However, they have less liver injury after liver resections.
文摘Intracranial arterial occlusion is rarely encountered in association with head injury.Only six cases of traumatic occlusion of the anterior cerebral artery(ACA) have previously been reported.In this paper,the authors describe a case of a posttraumatic occlusion of ACA.A 35-year-old male presented to the emergency room with severe head injury.Computed tomography(CT) scan displayed diffuse brain swelling with multiple skull fractures.Follow up CT scan showed extensive cerebral infarction in the territory of ACA.The patient underwent CT angiography that demonstrated occlusion of the ACA by a fracture of the anterior fossa.He died after 3 d.ACA traumatic occlusion is a rare condition,with poor prognosis.In this case,fracture was responsible for dissection and direct obstruction of the artery.
文摘BACKGROUND: This study aims to evaluate the results of studies investigating neutrophilto-lymphocyte ratio(NLR) and to identify the prognostic and diagnostic value of NLR in occlusive vascular diseases.METHODS: With the aim of identifying the studies related to NLR, a search was performed on http://www.ncbi.nlm.nih.gov/pubmed by using the key words "neutrophil lymphocyte ratio" between January 2005 and December 2014. All of the original articles were evaluated according to date of publications, countries, clinics and topics. Studies about occlusive vascular diseases were evaluated according to their qualifications, review methods and results. SPSS for Windows 16.0 was used in data analysis and data were expressed as mean, standard deviation and percentage.RESULTS: A total of 735 original research articles were investigated. The number of publications have shown a regular logarithmic increase over the years. Thirty-two percent of all publications were performed by clinics in Turkey and 56.4% were performed by general-oncological surgery and cardiology clinics. A total of 107 publications were identified to be about occlusive vascular diseases, 80.3% of these publications were found to be prognostic and 19.6% to be diagnostic, 82.2% of them were found to be planned as retrospective and 17.7% as prospective. In 95.3% of prognostic publications, there was a positive correlation between high NLR values at admission and poor prognosis. In 95.3% of diagnostic publications high NLR values at admission were identifi ed to be signifi cant diagnostically.CONCLUSION: Elevated neutrophil-to-lymphocyte ratio at admission, could be used as a diagnostic and/or prognostic parameter in occlusive vascular diseases.
文摘BACKGROUND Conventional recanalization techniques may fail in patients with completely occluded superior vena cava(SVC).AIM To analyze the effectiveness and complications of sharp recanalization for completely occluded SVC.METHODS This was a retrospective study of patients that underwent puncture and recanalization of the SVC between January 2016 and December 2017 at our hospital.Sharp recanalization was performed using the RUPS-100 system.The patients were followed for 12 mo.The main outcomes were the patency rate of SVC and arteriovenous fistula flow during dialysis.RESULTS The procedure was successful in all 14 patients(100%).Blood pressure in the distal SVC decreased in all 14 cases(100%)from 26.4±2.7 cmH2O to 14.7±1.3 cmH2O(P<0.05).The first patency rates of the SVC at 24 h and at 3,6,9 and 12 mo after sharp recanalization were 100%,92.9%,85.7%,78.6%and 71.4%,respectively.There were two(14.3%)severe,one(7.1%)moderate and one(7.1%)minor complication.The severe complications included one case of pericardial tamponade and one case of hemothorax.CONCLUSION The results suggest that sharp recanalization can be an additional tool to extend or renew the use of an occluded upper extremity access for hemodialysis.This could be of use in patients with long-term maintenance hemodialysis in whom the maintenance of central venous access is often a challenge.
文摘Retinal Vein Occlusion (RVO) is a common retinal vascular disease secondary to diabetic retinopathy resulting in permanent loss of vision despite available treatment options. Main vision impending complication of retinal vein occlusion is macular edema. Laser photocoagulation has been an established method for treating macular edema for many years but nowadays intravitreal injection of Anti-Vascular endothelial growth factors (Ranibizumab, Aflibercept Bevacizumab and Pegaptanib sodium) is the treatment of choice for macular edema from retinal vein occlusion. Intra-vitreal corticosteroids Triamcinolone Acetonide and Dexamethasone implant, are also being used to treat in some macular edema cases but with higher rates of side effects. Numerous surgical methods have been attempted for treating RVO and preventing macular edema;they include pars plana vitrectomy, radial optic neurotomy, laser induced chorioretinal anastomosis, and arteriovenous sheathotomy. Surgical methods supposedly relieve compression of the central retinal vein, altering the pathophysiology of vein occlusion at the level of the lamina cribrosa thus improving blood flow and oxygenation. But limitations result from its complications.

文摘AIM: To evaluate the efficacy and safety of combined anti-vascular endothelial growth factor (VEGF) agents, oral glucocorticoid, and laser photocoagulation therapy for macular edema (ME) secondary to retinal vein occlusion (RVO). METHODS: This study included 16 eyes of 16 patients with RVO-associated ME. Patients were initially treated with oral prednisone and an intravitreal anti-VEGF agent. Two weeks later, patients underwent standard laser photocoagulation. Best-corrected visual acuity (BCVA), central retinal thickness (CRT), and retinal vessel oxygenation were examined over 12mo. RESULTS: Patients received 1.43+0.81 anti-VEGF injections. Mean baseline and 12-month IogMAR BCVA were 0.96±0.51 (20/178) and 0.31±0.88 (20/40), respectively, in eyes with central retinal vein occlusion (CRVO) (P〈0.00), and 1.02±0.45 (201209) and 0.60±0.49 (20/80), respectively, in eyes with branch retinal vein occlusion (BRVO) (P〈0.00). At 12mo, CRT had significantly decreased in eyes with CRVO (P〈0.00) and BRVO (P〈0.00). Venous oxygen saturation had significantly increased in eyes with CRVO (P〈0.00) and BRVO (P〈0.00). No examined parameters were significantly different between the 2 RVO groups. No serious adverse effects occurred. CONCLUSION: Anti-VEGF, glucocorticoid, and photocoagulation combination therapy improves visual outcome, prolongs therapeutic effect, and reduces the number of intravitreal injections in eyes with RVO- associated ME.
文摘AIM: To observe the best-corrected visual acuity(BCVA) and central foveal thickness(CFT) repeatedly after the intravitreal injection of conbercept(IVC) for treating cystoid macular edema(CME) in branch retinal vein occlusion(BRVO) and explore the relationship between the duration of CME and visual outcome.METHODS: Subgroup analysis was performed to compare short-term(within 90 d of CME onset) and longterm(over 90 d of CME onset) macular edema in BRVO.After an initial IVC, a pro re nata(PRN) strategy was performed according to the recurrence of CFT or decrease of BCVA.Analysis of variance using repeated measurements, statistical analysis following indicators including BCVA and CFT collected at baseline and 1, 3, and 6 mo after IVC.RESULTS: Among the 60 cases included in this retrospective study, 36 were short-term CME, and 24 were long-term CME.There were statistical significances between and within groups of the BCVAs at different time points(P<0.001).The interaction was found between group and time(P=0.006), indicating the difference in the speed of BCVA improvement between groups.In particular, the improvement speed of BCVA in the short-term CME group was faster than that in the long-term CME group.There were significant differences between and with groups of the CFT at different time points(P<0.001).However, the interaction between group and time in relation to CFT had no significant differences(P=0.59).CONCLUSION: IVC treatment for CME following BRVO is effective and safe.The duration of CME before treatment is a significant predictor of the visual outcomes of patients with BRVO.The improvement of vision might be faster with early IVC treatment than with delayed treatment.
文摘Purpose: To investigate the short-term efficacy and safety of intravitreal aflibercept in a case series of patients from Taiwan, China, with macular edema secondary to branch retinal vein occlusion(BRVO).Methods: A total of 32 patients with macular edema associated with BRVO,.without prior macular laser or other intervention, were enrolled consecutively from September 2013 to February 2015..The cases received single 2 mg injections of intravitreal aflibercept..Primary outcome measures included changes in central foveal thickness.(CFT;.1 mm increments by spectral-domain optic coherence tomography).and best corrected visual acuity.(BCVA),.determined at 1,.2,.and 3months after the injection. Complications after injections were recorded. The changes in CFT and BCVA were compared with Wilcoxon sign-rank tests.Results:.The CFT was significantly reduced and the BCVA was significantly improved at 1, 2, and 3 months after injection(all P < 0.05). Tomography findings revealed no recurrence within 3 months. No systemic thromboembolic events,elevated intraocular pressure, retinal detachment, or infectious endophthalmitis occurred following injection.Conclusion:.Single intravitreal aflibercept may be useful in treating macular edema associated with BRVO within 3months. No adverse systemic or ocular effects were found in this case series.

文摘AIM:To evaluate whether a novel tyrosine kinase inhibitor nintedanib could inhibit basic fibroblast growth factor(bFGF)and vascular endothelial growth factor(VEGF)simultaneously for retinal vascular disease in vivo.METHODS:After a laser induced rabbit retinal vein occlusion(RVO)model was made,0.5 mg of nintedanib was injected intravitreally in the left eye on the third day while the right eye was as a control.Intracameral samples were taken on the day before laser treatment and days 1,3,7,14,21,and 28 after treatment.Enzyme-linked immunosorbent assay(ELISA)was used to test the bFGF and VEGF-A concentrations in the aqueous humor.RESULTS:Both bFGF and VEGF-A rose significantly on the third day after laser treatment in both eyes.In the control eye the bFGF concentration peaked on the 14th day while the VEGF-A concentration dropped rapidly soon after the third day.After nintadanib injection in the study eye,both bFGF and VEGF-A showed a significant reduction on the 4th day(7th day after laser treatment)when compared to the control eye,and kept on low level in the following several weeks.CONCLUSION:Intravitreal injection of nintedanib can inhibit the expression of bFGF and VEGF in the process of RVO model to a certain extent,which is expected to become a new method for the treatment of retinal vascular diseases or fibrotic diseases.

文摘Object:The aim is to evaluate how effective Traditional Chinese Medicine(TCM)is in treating patients who have central retinal vein occlusion with macular edema(CRVO-ME)in a real-world study.Furthermore,the objective of the research was to examine the TCM prescription trends in the management of CRVO-ME.Method:A single-center real-world study(RWS)was carried out over a span of 19 years,following the established design.The study encompassed 113 patients diagnosed with CRVO-ME.Out of these,74 patients received TCM treatment,while the remaining individuals underwent a combined therapy involving TCM and anti-VEGF drugs through intravitreal injection.The patients were matched using propensity score matching(PSM).The result measured in the RWS was BCVA.The oral prescriptions for CRVO-ME that led to observable and effective outcomes were collected.Excel and the TCM Inheritance Auxiliary Platform V2.5 were utilized to optimize mutual information,hierarchical clustering based on entropy,and other techniques to extract medication regulations and features.Result:After applying PSM,each group comprised 29 cases.Both groups exhibited improved BCVA following treatment;however,there was no statistically significant distinction in BCVA or effectiveness between the two groups(all P>0.05).Apart from the analysis of oral prescriptions for CRVO-ME,the investigation pinpointed the most frequently used TCMs,namely Flos Carthami,Semen Persicae,Radix Angelica sinensis,Radix Rehmanniae,and Radix et Rhizoma Notoginseng.Frequently utilized medications tended to possess cold,warm,or mild attributes and exhibited a taste profile that was either bitter or sweet.The primary meridians associated with the medicines employed in treating CRVO-ME were liver,spleen,stomach,heart,and lung.Through the application of association rule analysis,it was discerned that there were 195 commonly employed combinations of medicines.Additionally,a complex system entropy cluster analysis unveiled 13 key combinations of medicines.By employing an unsupervised entropy hierarchical clustering analysis,a novel prescription was formulated.Conclusion:Within a real-world population of CRVO-ME patients,TCM exhibited its effectiveness.The treatment approach for CRVO-ME predominantly involved the regulation of qi(Qi is an exceedingly subtle substance within the human body,brimming with vitality and ceaseless motion.It constitutes the fundamental element that shapes and sustains the various processes of human life.)and blood as well as the resolution of dampness.The oral prescriptions frequently targeted the meridians of liver,spleen,stomach,heart,and lungs.
文摘Background:To evaluate a fully automated vascular density(VD),skeletal density(SD)and fractal dimension(FD)method for the longitudinal analysis of retinal vein occlusion(RVO)eyes using projection-resolved optical coherence tomography angiography(OCTA)images and to evaluate the association between these quantitative variables and the visual prognosis in RVO eyes.Methods:Retrospective longitudinal observational case series.Patients presenting with RVO to Creteil University Eye Clinic between October 2014 and December 2018 and healthy controls were retrospectively evaluated.Group 1 consisted of central RVO(CRVO)eyes,group 2 consisted of eyes with branch RVO(BRVO)and group 3 of healthy control eyes.OCTA acquisitions(AngioVue RTVue XR Avanti,Optovue,Inc.,Freemont,CA)were performed at baseline and last follow up visit.VD,SD,and FD analysis were computed on OCTA superficial and deep vascular complex(SVC,DVC)images at baseline and final follow up using an automated algorithm.Logistic regression was performed to find if and which variable(VD,SD,FD)was predictive for the visual outcome.Results:Forty-one eyes,of which 21 consecutive eyes of 20 RVO patients(13 CRVO in group 1,8 BRVO in group 2),and 20 eyes of 20 healthy controls were included.At the level of SVC,VD and FD were significantly lower in RVO eyes compared to controls(P<0.0001 and P=0.0008 respectively).Best-corrected visual acuity(BCVA)at last follow-up visit was associated with baseline VD(P=0.013),FD(P=0.016),and SD(P=0.01)at the level of the SVC,as well as with baseline FD at the DVC level(P=0.046).Conclusions:Baseline VD,SD,and FD are associated with the visual outcome in RVO eyes.These parameters seem valuable biomarkers and may help improve the evaluation and management of RVO patients.
文摘Resveratrol possesses beneficial biological effects, which include anti-oxidant, anti-inflammatory and anti-carcinogenic properties. Recently, resveratrol has been shown to exhibit neuroprotective effects in models of Parkinson's disease, cerebral ischemia and Alzheimer's disease. However, its effects on vascular dementia remain unclear. The present study established a rat model of vascular dementia using permanent bilateral common carotid artery occlusion. At 8-12 weeks after model induction, rats were intragastrically administered 25 mg/kg resveratrol daily. Our results found that resveratrol shortened the escape latency and escape distances in the Morris water maze, and pro- longed the time spent percentage and swimming distance percentage in the target quadrant during the probe test, indicating that resveratrol improved learning and memory ability in vascular dementia rats. Further experiments found that resveratrol decreased malonyldialdehyde levels, and increased superoxide dismutase activity and glutathione levels in the hippocampus and cerebral cortex of vascular dementia rats. These results confirmed that the neuroprotective effects of resveratrol on vascular dementia were associated with its anti-oxidant properties.
