Efficient care requires a good knowledge of the disease by the parents, and an exploration of psychosocial experience of the parents, who are on the front lines of the support. Objectives: Assess the impact of parents’ level of knowledge, from their psychosocial experience and from the quality of life on the glycemic control of children, adolescents and young adults with type 1 (DT1) diabetes in Brazzaville. Patients and Methods: This was an analytical cross-sectional study with prospective data collection during 7 months at Brazzaville University Hospital CHU, involving 103 parents. We studied socio-demographic variables, the balance of children’s diabetes by glycated hemoglobin, parents’ knowledge of diabetes by questionnaire “Michigan Diabetes Research and Training Center’s Revised Diabetes Knowledge Test”. The assessment of anxiety and depression through the “Hospital Anxiety and Depression” (HAD) score. Parental stress was assessed by the “Pediatric Inventory for Parents” (PIP). Results: We reported certain anxiety symptomatology in 60 parents (58.3%). Certain depression was reported in 63 parents (61.2%), and a lesser degree of parental distress. These psychosocial disorders in the parents did not influence the glycemic balance of the children. Low socio-economic level of the family (p = 0.001), poor knowledge about diabetes (p = 0.046) and poorer quality of life (p Conclusion: Psychosocial disorders were well observed in parents of DT1 children.展开更多

Objective: To identify the prevalence of early life stress in drug users, to compare the intensity of trauma in alcohol and crack users, and to relate the power of injury to the severity of drug dependence. Method: Cross-sectional analytical study with a sample of 105 alcohol and crack users treated by an outpatient service. The instruments for data collection were sociodemographic data questionnaire, the Mini-International Neuropsychiatric Interview, and the Severity of Dependence Scale. The categorical variables association was analyzed using the Chi-squared test, considering p Results: High prevalence of early stress and severity of dependence were identified, with higher rates among crack users. The early stress revealed in the Alcohol group high rates of emotional (88.7%) and physical (94.3%) neglect and in the Crack group significant frequency of physical (61.5%) emotional abuse (51.9%), sexual (46.2%), and emotional (78.8%) and physical (90.4%) neglect. Crack users are 2.6 times more likely to have been emotionally abused, and 2.1 times more likely to have been sexually abused during childhood when compared to the alcohol group. Conclusion: Early stress was prevalent with significant intensity in drug users, and evaluation of this problem is essential for a better understanding of these disorders.展开更多

We studied subjective experiences of donors before and two years after kidney donation using in-depth interviews of eleven subjects. Interviews were audiotaped, transcribed, and analyzed by qualitative content analysis (grounded theory). Resulting categories of this process were grouped in order to construct ideal types of subjective experiences after kidney donation. Of our donors, 64% were female, mean age was 40.4 years. Donors reported that they had had no medical problems after donation. Most displayed psychological problems, e. g. difficulties adjusting to the new situation, hypochondriacal complaints, and fear of rejection of the recipient’s kidney. Psychological complaints were particularly troublesome in case of complications of the recipient. All donors wished extended counselling after kidney donation. Identifying problems of kidney donors may be easier when doctors are trained in typical experiences. This may also guide the tailoring of individual psychological interventions.展开更多

Currently, people's lives cannot be analyzed using a macro-perspective, marked by partitioning phases, but the various stages we undergo from childhood to the third age are being overlapped and this is reflected with particular relevance in the field of learning, where the notion of lifelong learning is present in our personal, social and professional everyday contexts, in Portugal, the New Opportunities Initiative is building new and innovative education and training pathways, which privilege the autobiographical methods, where adults, from their life stories, are challenged to resort to new ways of valuing their experiences, potentiating the acquisition of new skills, in a logic of personal, social and professional empowerments. This formative/training logic has shown to be the promoter of a growing autonomy from the adults. We will present, here, the qualitative multi-case study we are developing in this area, which is centered on adults'/professionals' autobiographies and focuses on the understanding of adult education, lifelong learning and on the analysis of the mediation figures associated with them.展开更多

文摘BACKGROUND Regimens involving direct-acting antiviral agents(DAAs)are recommended for the treatment of infection with hepatitis C virus(HCV)genotypes 1,2 and 3.But real-world data is still not enough, especially in Asia.AIM To investigate the efficacy and safety of DAA-based regimens in a real-life setting in China.METHODS This study included 366 patients infected with HCV genotypes 1,2 and 3,with or without cirrhosis, who were observed between May 2015 and December 2018.They were treated with ledipasvir and sofosbuvir(SOF)(genotype 1)with or without ribavirin(RBV),SOF and RBV(genotype 2),or SOF and daclatasvir(genotype 3),with or without RBV,for 12 or more wk.The participants’sustained virological responses(SVR)at post-treatment week 12(SVR12)was the primary endpoint. The occurrence of adverse events and drug-drug interactions were recorded.RESULTS In the 366 patients, genotype 1(59.0%)was the most common genotype, followed by genotypes 2(34.4%)and 3(6.6%).Liver cirrhosis was diagnosed in 154(42.1%)patients.Fifty(13.7%)patients were treatment-experienced.Intention-to-treat analysis revealed that SVR12 was 86.3%(316/366).For modified intention-totreat analysis,SVR12 was achieved in 96.6%of overall patients(316/327),96.3%in patients with genotype 1,97.5%in those with genotype 2,and 95.0%in those with genotype 3.Most of the treatment failures were due to lack of follow-up(3cases had non-responses,1 had virological breakthrough,11 relapsed and 36 did not participate in the follow-up).There was no significant difference in SVR between different genotypes and liver statuses(P<0.05).Patients with lower alanine aminotransferase levels at baseline who achieved an end of treatment response were more likely to achieve SVR12(P<0.05).High SVR was observed regardless of age, gender, liver status,alpha-fetoprotein,HCV RNA levels or history of antiviral therapy(P>0.05 for all). The cumulative hepatocellular carcinoma occurrence and recurrence rate after using the DAAs was 0.9%.Most of the adverse events were mild.We found two cases of special adverse events. One case involved facial and bilateral lower extremity edema, and the other case showed an interesting change in lipid levels while on medication. No severe adverse events were noted.CONCLUSION The DAA-based regimens tested in this study have excellent effectiveness and safety in all patients infected with HCV genotypes 1,2 and 3,including those with cirrhosis.
文摘Type 1 diabetes is one of the main chronics pathologies in children. Low socio-economic level of the family (p = 0.001), poor knowledge about diabetes (p = 0.046) and poorer quality of life (p Conclusion: Psychosocial disorders were well observed in parents of DT1 children.
文摘Background: Early life stress is a significant public health problem associated with increased rates of psychiatric disorders, especially those related to drug abuse. Conclusion: Early stress was prevalent with significant intensity in drug users, and evaluation of this problem is essential for a better understanding of these disorders.
文摘BACKGROUND Neoadjuvant therapy(NT)has increasingly been utilized for patients with localized pancreatic ductal adenocarcinoma(PDAC).It is the recommended approach for borderline resectable(BR)and locally advanced(LA)cancers and an increasingly utilized option for potentially resectable(PR)disease.Despite its increased use, little research has focused on patient-centered metrics among patients undergoing NT,including patient experiences, preferences, and recommendations.A better understanding of all aspects of the patient experience during NT may identify opportunities to design interventions aimed at improving quality of life;it may also facilitate the completion of NT and receipt of surgery, ultimately optimizing long-term outcomes.AIM To understand the experience of patients initiating and receiving NT to identify opportunities to improve neoadjuvant cancer care delivery.METHODS Semi-structured interviews of patients with localized PDAC during NT were conducted to explore their experience initiating and receiving NT.Interviews took place between August 2020 and October 2021.Due to the descriptive nature of the research, questions were open ended.Interviews were conducted over the phone, audio recorded and then transcribed. All interviews were coded by two independent researchers using NVivo 12,iteratively identifying themes until thematic saturation was achieved. An integrative approach to qualitative analysis was used, utilizing both inductive and deductive methods.RESULTS A total of 12 patients with localized PDAC were interviewed.Patients with BR(n=7),PR(n=2),and LA(n=3)cancers participated in the study. All patients indicated that choosing NT was the doctor’s recommendation, while most reported not being familiar with the concept of NT(n=11)and that NT was presented as the only option(n=8).Five themes describing the patient experience emerged:physical symptoms, emotional symptoms, coping mechanisms, access to care, and life factors. The most commonly cited recommendation for improving the experience of NT was improved education before and during NT(n=7).Patients highlighted the need for more information on the rationale behind choosing NT prior to surgery, the anticipated surgery and its likelihood of surgery occurring after NT,as well as general information prior to starting NT treatment. The need for seeing different members of the healthcare team, including ancillary services was also frequently cited as a recommendation for improving the experience of NT(n=5).CONCLUSION This study provides a framework to allow for a better understanding of the PDAC patient experience during NT and highlights opportunities to improve quality and quantity of life outcomes.
文摘Little is known about long-term quality of life of kidney donors. This may also guide the tailoring of individual psychological interventions.
文摘In our actual framework of constant social and cultural changing, we are witnessing a transformation of the adults' patterns and pathways of life. We will present, here, the qualitative multi-case study we are developing in this area, which is centered on adults'/professionals' autobiographies and focuses on the understanding of adult education, lifelong learning and on the analysis of the mediation figures associated with them.
文摘The reason why religion can exist is that it provides a sense of absolute dependence, which is the ultimate emotion based on the belief of cosmic and human harmony. In a sense, it can be said that religion is absolute piety and humility, which stems from the experience of God' s power.
文摘The rust⁃proof ability relate to the reliability of bolt connection, and the painting is the most simple and effective rust⁃proof method in industry.Because of lacking general standard, there are different coating thicknesses painted on the surface of workpieces, which influences the loose life of connecting bolt. For the longer loose life,Three kinds of applied coatings(primer coating, primer+top⁃coat coating, primer+intermediate⁃coat+top⁃coat coating)in industry are tested in the experiment, and the best coating is found. All the conclusions gotten by analyzing the testing phenomenon of every coating shown in experiment, have profound meaning for practical industry application.
