Objectives: Peripheral neuropathy (PN) is a significant contributor to disability in the elderly. It is also one of the most prevalent complications of type 2 diabetes, prediabetes and metabolic syndrome. PN is common...Objectives: Peripheral neuropathy (PN) is a significant contributor to disability in the elderly. It is also one of the most prevalent complications of type 2 diabetes, prediabetes and metabolic syndrome. PN is commonly associated with pain, numbness, tingling, burning, and cramping in the feet and legs. Current treatment options are limited to controlling pain, seizures and use of antidepressant medications. These treatments have undesirable side effects and don’t stop PN progression. Here we utilized a combination of individual-specific modalities to improve local circulation and relieve PN symptoms. Methods: We conducted an open-label, multicenter pilot trial with 34 subjects (19 males and 15 females ranging from 40 - 85 years of age). All of the participants were diagnosed with peripheral neuropathy and had bilateral symptoms in their feet, and many reported the same symptoms (pain, numbness, tingling, burning, and cramping) in their lower legs. The duration of symptoms ranged from four months to over six years. On Day 0, subjects were given a 90-day supply of the oral supplement with dosing instructions and a LED light therapy device. They also received three platelet-rich plasma (PRP) injections in their lower extremities. Subjects also received an extracorporeal shockwave therapy (ESWT) treatment for each foot and subsequently twice per week for the first six weeks, then once weekly for the duration of the study. Subjects filled out the Brief Pain Index (BPI) at weekly intervals. On Day 90, subjects completed the Patient Global Impression of Change (PGIC) survey. Results: There were significant responses to pain, as evidenced by BPI scores at weeks 8, 9, 10 and 11 (p = 0.02, 0.01, 0.02, and 0.003, respectively). Analysis of the final day PGIC survey showed a favorable outcome for 73% of participants (p = 0.003), with the majority reporting Very Much Improved. Conclusions: By utilizing a multi-modality treatment protocol that includes PRP, LED light therapy, ESWT and an oral dietary supplement, we observed significant reductions in BPI scores. Quality of life and their overall impression of change (PGIC) were significantly improved, and there were no significant side effects.展开更多

The method has been evaluated and is used to treat vasculogenic erectile dysfunction (ED). The present study aimed to demonstrate the efficacy of a linear focused piezoelectric shockwave device (Richard Wolf/ELvationPiezowave<sup>2</sup>) to treat patients with vasculogenic ED using a novel linear shockwave tissue coverage LSTC-ED<sup><sup>®</sup> </sup>technique. A total of 75 patients were treated using the Piezowave<sup>2</sup> device and the LSTC-ED<sup><sup>®</sup></sup> technique. Patients’ erectile function was evaluated using the modified IIEF-5 (International Index of Erectile Function) scale at the beginning of treatment and at 1 month post treatment;patients were additionally questioned using our own Treatment Satisfaction Questionnaire (TSQ). The study also included a group of 50 patients treated by placebo;the outcomes of both groups were compared. The average IIEF-5 score of patients in the treatment group increased from 14.4 at baseline to 18.6 at 1 month post treatment. According to the IIEF-5 scale, treatment was successful in 81.33% of patients (61/75). According to the Treatment Satisfaction Questionnaire (answers 1 to 3 of the TSQ), treatment was successful in 77.3% of patients (58/75). In the placebo group of 50 patients only 5 patients showed an improvement based on IIEF score, and 8 reported an improvement based on their answers to the TSQ. No significant adverse effects were observed during treatment or in the follow-up period. The Piezowave<sup>2</sup> device and the LSTC-ED<sup><sup>®</sup> </sup>technique proved to be suitable and effective to treat erectile dysfunction.展开更多
Shockwaves from fuel-air explosive(FAE)cloud explosions may cause significant casualties.The ground overpressure field is usually used to evaluate the damage range of explosion shockwaves.In this paper,a finite elemen...Shockwaves from fuel-air explosive(FAE)cloud explosions may cause significant casualties.The ground overpressure field is usually used to evaluate the damage range of explosion shockwaves.In this paper,a finite element model of multi-sources FAE explosion is established to simulate the process of multiple shockwaves propagation and interaction.The model is verified with the experimental data of a fourfoldsource FAE explosion,with the total fuel mass of 340 kg.Simulation results show that the overpressure fields of multi-sources FAE explosions are different from that of the single-source.In the case of multisources,the overpressure fields are influenced significantly by source scattering distance and source number.Subsequently,damage ranges of overpressure under three different levels are calculated.Within a suitable source scattering distance,the damage range of multi-sources situation is greater than that of the single-source,under the same amount of total fuel mass.This research provides a basis for personnel shockwave protection from multi-sources FAE explosion.展开更多
This paper presents a new technique,termed femtosecond laser shock peening ablation in liquids(fs-LSPAL),which can realize simultaneous crack micro/nanomanufacturing and hierarchical micro/nanolaser ablation,giving ri...This paper presents a new technique,termed femtosecond laser shock peening ablation in liquids(fs-LSPAL),which can realize simultaneous crack micro/nanomanufacturing and hierarchical micro/nanolaser ablation,giving rise to the formation of diverse multiscale hierarchical structures,such as macroporous ratcheted structures and enéchelon microfringes decorated with parabolic nanoripples.Through analysis of surface morphologies,many phenomena have been confirmed to take place during fs-LSPAL,including enéchelon cracks,nanostriation,ripple densification,crack branching,and selective formation of high spatial frequency laser-induced periodic surface structures of 100–200 nm in period.At a high laser power of 700 mW,fs-LSPAL at scanning speeds of 0.2 mm s^-1 and 1 mm s^-1 enables the generation of height-fluctuated and height-homogeneous hierarchical structures,respectively.The height-fluctuated structures can be used to induce‘colony’aggregates of embryonic EB3 stem cells.At 200 mW,fs-LSPAL at 1 mm s^-1 is capable of producing homogeneous tilt macroporous structures with cracked structures interleaved among them,which are the synergistic effects of bubble-induced light refraction/reflection ablation and cracks.As shown in this paper,the conventional laser ablation technique integrated with its self-driven unconventional cracking under extreme conditions expands the horizons of extreme manufacturing and offers more opportunities for complex surface structuring,which can potentially be used for biological applications.展开更多

Jadad rating scale was used for literature quality evaluation, and the extracted data was meta-analyzed by RevMan5.3 software.Results: A total of 15 randomized controlled trials involving 1248 patients were included. The results of meta-analysis showed: The clinical effective rate of shock wave therapy for knee osteoarthritis was reliable[RR=1.12, 95%CI(1.03, 1.20), p=0.005], and the efficacy was reliable in reducing the visual analogue scale [MD=-1.73, 95%CI(-2.31, -1.14), p<0.00001] , Lequesne osteoarthritis severity index[MD=-1.60, 95%CI(-2.55, -0.64), p<0.00001] and WOMAC[MD=-6.03, 95%CI(-9.49,-2.57) p=0.0006] of KOA patients, without serious adverse reactions.Conclusion:At present, the evidence shows that shock wave can effectively improve the pain and other related symptoms in KOA patients, and the curative effect is reliable.展开更多

Th...Background: Stone disease is a wide health problem;certain drugs have been used as supplement with ESWL for the treatment of symptomatic uncomplicated lower ureteric stone like Nefidipine, Alfuzosin and Tamsulosin. The purpose of this study is to evaluate the efficacy of Tamsulosin for the treatment of lower ureteric stone of 5 - 10 mm diameter after ESWL. Methods: A total of 105 patients with lower ureteric stone of less than 10 mm diameters were divided into two groups. Group 1 including 48 patients received 0.4 mg Tamsulosin and diclofenac sodium analgesia as needed immediately after ESWL and continued until stone expulsion was confirmed up to maximum of 30 days. Group 2 including 47 patients was given diclofenac sodium only as needed after ESWL. Result: 46 of 49 patients in group 1 and 44 of 48 patients in group 2 ultimately passed stones. The number of ESWL sessions was 1.4 ± 0.6 in group 1 & 1.42 ± 0.75 in group 2. There were 10 patients who needed analgesia after ESWL in group 1 and 17 patients in group 2. Visual analogue scale pain severity score was 5.32 ± 1.23 and 6.41 ± 1.26 in group 1 and 2, respectively. The time to stone expulsion in group 1 and 2 was 8 ± 5.4 days and 12 ± 10.3 days, respectively. No significant complications or side effects occurred. Conclusion: The use of Tamsulosin in combination with ESWL in patients with lower ureteric stone of 5 - 10 mm was effective for reducing the time to stone expulsion, decreasing need for analgesia, and facilitating stone passage without significant improvement in stone free rate.展开更多
