Objective:To study the effect of mindfulness-based cognitive training on sleep quality and mindfulness cognitive level of college students who score below the critical value of sleep disorders.Methods:The subjects were freshmen of a university in Shanghai who had scored below the critical value of sleep disorders.They were divided into the control group and experimental group by a random number table method,with 35 students in each group.No intervention was provided in the control group,and the mindfulness-based cognitive training(1 hour per day,5 days per week for 8 consecutive weeks)was performed in the intervention group.Eight weeks later,the Pittsburgh Sleep Quality Index(PSQI)and Five Facet Mindfulness Questionnaire(FFMQ)scores were compared between the two groups before and after the intervention for changes in the sleep quality and mindfulness cognitive level.Results:The sleep quality and daytime dysfunction were significantly improved,the sleep latency was shortened,the sleep duration was prolonged(P<0.05),and the mindfulness level of the subjects was significantly improved(P<0.05)in the intervention group compared with the control group.Conclusion:Mindfulness-based cognitive training can significantly improve the sleep quality of college students who scored below the critical value of sleep disorders.Furthermore,its psychological mechanism may be associated with the improvement of the mindfulness level of college students. 展开更多

Objective:To determine the relationship between the blood serum brain-derived neurotrophic factor(BDNF) level and cognitive function deterioration in patients with obstructive sleep apnea/ hypopnea syndrome(OSAHS),and to explore the possible mechanism of cognitive impairment. Methods:Twenty-eight male OSAHS patients and 14 normal males(as controls) were enrolled in the study.Polysomnography and the Montreal cognitive assessment(MoCA) were conducted. The blood serum BDNF levels were measured using ELISA.Results:The OSAHS group had significantly decreased blood serum BDNF levels compared with the control group(t=-10.912, P=0.000).The blood serum BDNF level of the subjects was significantly positively associated with the MoCA score(r=0.544,P=0.000),significantly negatively associated with the apneahypopnea index(AHI) and shallow sleep(S1+S2)(AHI:r=-0.607,P=0.000;S1+S2:r =-0.768,P= 0.000),and significantly positively associated with the lowest SaO<sub>2</sub>(LSO),slow wave sleep(S3+S4), and rapid eye movement sleep(REM)(LSO:r=0.566,P = 0.000;S3+S4:r=0.778,P=0.000;REM: r=0.575,P=0.000).Conclusions:OSAHS patients have significantly decreased blood serum BDNF levels compared with the control.Nocturnal hypoxia as well as the deprivation of slow wave sleep and REM may lead to the decreased serum BDNF level of OSAHS patients.This decreased blood serum BDNF level may contribute to the cognitive impairment in OSAHS. 展开更多

Safety and Quality of Care Evaluation in Japanese Long-Term Facilities Focused on the Activity Condition and Cognitive Function of Residents 被引量:1

Objective: Currently, standards for evaluating long-term care facilities do not exist in Japan. This study aimed to evaluate the quality of healthcare services in Japan’s long-term care facilities and identify the structural and process indicators associated with the facilities’ outcome indicators. Methods: This retrospective study assessed changes in residents’ abilities to participate in physical activities, their cognitive function, and their vulnerability to injuries. From 2012 to 2013, we collected information on the healthcare services at 1067 long-term care facilities registered with Japan’s Welfare, Health and Medical Care Information Network in the Welfare and Medical Service Agency. We examined 12 structural indicators, 26 process indicators, and 7 outcome indicators. We used multivariate linear regression models adjusted to analyze relationships between outcome indicators and structural or process indicators. Results: Residents’ activity and cognitive function indicators either improved by 80% - 90% or were maintained for one year. The Geriatric Functional Independence Measures, the Barthel Index, and holding conferences related to care were all considered activities of daily living. Three adverse events—tumbles and falls, behavioral problems, and aimless wandering or leaving the facility without permission—were factors that restricted residents’ behavior and number of residents per care staff member. Conclusions: Maintaining or improving levels of independence and cognition in daily living requires a care process system that enables ongoing monitoring of residents’ activities of daily living and cognitive functioning. Ensuring the safety of residents and improving the quality of care in long-term care facilities without securing adequate care staff are not possible. 展开更多

Population aging has developed into a significant area of concern in developed countries, especially in relation to thepreservation of functional independence and the quality of life (QoL). However, information on the contribution ofsex differences in quality of life and cognitive function is scarce. Therefore, this study aimed to investigate potentialdifferences in cognitive function and QoL between males and females. For this study, we recruited 382 healthy subjectsaged 19–79 years from communities in Southern Taiwan region,China. Cognitive function and QoL were assessed using the UCSD Performance-based Skills Assessment, Brief Version (UPSA-B) and World Health Organization Quality ofLife Instruments (WHOQOL-BREF), respectively. The effects of sex, age groups, and interaction of sex and ageon the UPSA-B and WHOQOL-BREF scores were examined. We found that the financial and communicationdomains of the UPSA-B showed declining trends with age in both sexes, although these trends were more obviousin females than in males. In the domain of Physical Capacity, QoL showed an age-increased trend in males, whileQoL in the Environment domain showed an age-increased trend in females. Regarding the relationships betweencognitive function and QoL, we found that the financial skill and communication skill of UPSA-B was positivelycorrelated to the psychological well-being domain of WHOQOL-BREF, in males of age <40 years. In conclusion,our findings demonstrate differential relationships between cognitive function and QoL between males and females,which could serve as a basis for further study between cognitive function and quality of life in communities. 展开更多

Objective:Chemotherapy significantly improved the survival of breast cancer,but also brings various symptoms and psychological distresses.Cognitive dysfunction was usually ignored.This study aimed to investigate the changes in cognitive function during chemotherapy and its influence on the quality of life in breast cancer patients.Methods:Female patients with newly diagnosed breast cancer have prospectively enrolled in this study.Participants investigated and completed questionnaires at four-time points:pre-chemotherapy(T1),post-chemotherapy(T2),6 months(T3)and 12 months(T4)after the completion of chemotherapy.The Functional Assessment of Cancer Therapy-Cognitive scale and the Functional Assessment of Cancer Therapy-Breast Cancer scale used was to assess the cognitive function and the quality of life,respectively.Data were analyzed using descriptive and repeated-measures analysis of variance statistics.Results:One hundred and eighty-three patients participated in the study and completed questionnaires at least 2 times.The mean score of cognitive function and the quality of life were significantly different at a different time point(P<0.05 for all).The trends of cognitive scores shown that the most serious damage of cognitive function appeared after beginning chemotherapy and followed by a slowly turn back after the completion of chemotherapy.The trend of the mean score of the quality of life after chemotherapy was similar to that of the cognitive score.The degree of cognitive dysfunction was significantly associated with the score of the quality of life at each time point after chemotherapy(P<0.05).Conclusion:Chemotherapy is closely associated with a cognitive impairment,which contributes to a significant decrease in the quality of life of patients with breast cancer. 展开更多

Sleep benefits the restoration of energy metabolism and thereby suppo rts neuronal plasticity and cognitive behaviors.Sirt6 is a NAD+-dependent protein deacetylase that has been recognized as an essential regulator of energy metabolism because it modulates various transcriptional regulators and metabolic enzymes.The aim of this study was to investigate the influence of Sirt6 on cerebral function after chronic sleep deprivation(CSD).We assigned C57BL/6J mice to control or two CSD groups and subjected them to AAV2/9-CMV-EGFP or AAV2/9-CMV-Sirt6-EGFP infection in the prelimbic cortex(PrL).We then assessed cerebral functional connectivity(FC) using resting-state functional MRI,neuron/astrocyte metabolism using a metabolic kinetics analysis;dendritic spine densities using sparse-labeling;and miniature excitato ry postsynaptic currents(mEPSCs) and action potential(AP) firing rates using whole-cell patchclamp recordings.In addition,we evaluated cognition via a comprehensive set of behavioral tests.Compared with controls,Sirt6 was significantly decreased(P<0.05) in the PrL after CSD,accompanied by cognitive deficits and decreased FC between the PrL and accumbens nucleus,piriform cortex,motor co rtex,somatosensory co rtex,olfactory tubercle,insular cortex,and cerebellum.Sirt6 ove rexpression reve rsed CSD-induced cognitive impairment and reduced FC.Our analysis of metabolic kinetics using [1-13C] glucose and [2-13C] acetate showed that CSD reduced neuronal Glu4and GABA2synthesis,which could be fully restored via forced Sirt6 expression.Furthermore,Sirt6 ove rexpression reversed CSD-induced decreases in AP firing rates as well as the frequency and amplitude of mEPSCs in PrL pyramidal neurons.These data indicate that Sirt6 can improve cognitive impairment after CSD by regulating the PrL-associated FC network,neuronal glucose metabolism,and glutamatergic neurotransmission.Thus,Sirt6 activation may have potential as a novel strategy for treating sleep disorder-related diseases. 展开更多

Detection of cognitive impairment in patients with obstructive sleep apnea hypopnea syndrome using mismatch negativity 被引量:6

In this experiment, 97 patients with obstructive sleep apnea hypopnea syndrome were divided into three groups (mild, moderate, severe) according to minimum oxygen saturation, and 35 healthy subjects were examined as controls. Cognitive function was determined using the mismatch negativity paradigm and the Montreal Cognitive Assessment. The results revealed that as the disease worsened, the mismatch negativity latency was gradually extended, and the amplitude gradually declined in patients with obstructive sleep apnea hypopnea syndrome. Importantly, mismatch negativity latency in severe patients with a persistent time of minimum oxygen saturation 〈 60 seconds was significantly shorter than that with a persistent time of minimum oxygen saturation 〉 60 seconds. Correlation analysis revealed a negative correlation between minimum oxygen saturation latency and Montreal Cognitive Assessment scores. These findings indicate that intermittent night-time hypoxemia affects mismatch negativity waveforms and Montrea Cognitive Assessment scores. As indicators for detecting the cognitive functional status of obstructive sleep apnea hypopnea syndrome patients, the sensitivity of mismatch negativity is 82.93%, the specificity is 73.33%, the accuracy rate is 81.52%, the positive predictive value is 85.00%, the negative predictive value is 70.21%, the positive likelihood ratio is 3, and the negative likelihood ratio is 0.23. These results indicate that mismatch negativity can be used as an effective tool for diagnosis of cognitive dysfunction in obstructive sleep apnea hypopnea syndrome patients. 展开更多

Objective To evaluate the emotional and cognitive status in patients with obstructive sleep apnea syndrome (OSAS), using neuropsychological tests and evoked-related potential (P3). Methods Sixteen patients diagnosed of OSAS were tested by Hamilton rating scale for anxiety (HRSA) and Hamilton rating scale for depression (HRSD). Other three groups, OSAS patient group (n=21), snoring group (n=21), and control group (n=21), were administered polysomnography (PSG), auditory evoked event-related potential (P3), and clinic memory test. The results were analyzed using general linear model (GLM) analysis and Post Hoc test. Results Twelve OSAS patients’ scores of HRSA and HRSD were beyond the normal range, 26.42 ±4.48 and 22.08 ±3.97 respectively. The auditory P3 latency in OSAS group was 363.1 ±22.9 ms (Fz), 368.57 ±28.03 ms (Cz), in snoring group 336.57 ±31.08 ms (Fz), 339.81 ±31.76 ms (Cz), in control group 340.8 ±28.7 ms (Fz), 338.29 ±29.21 ms (Cz). There were significant differences between OSAS group and snoring group, as well as control group (P< 0.05). No significant difference was seen between snoring group and control group. No significant difference was noted in P3 amplitude among three groups. Memory quotient (MQ) reduced in snoring group compared with control group. Conclusions Emotional disturbances are common clinical features in OSAS patients. Abnormal auditory P3 latency indicates the cognitive dysfunction in OSAS patients. Nocturnal hypoxaemia may play an important role on it. Snorers should be monitored because of the tendency to develop cognitive impairment. 展开更多

Neuropsychological Functioning after Adenotonsillectomy in Children with Obstructive Sleep Apnea:A Meta-analysis 被引量:4

The present study aimed to investigate the efficacy of adenotonsillectomy(AT) for children with obstructive sleep apnea syndrome(OSAS) and the improvement of their cognitive function. Studies on cognitive performance of OSAS children treated with or without AT were identified by searching the Pubmed, EMBASE and Cochrane library. A meta-analysis was conducted to analyze the literature. The random-effects model was used to evaluate 11 eligible studies using an inverse-variance method. The neuropsychological test results of 4 cognitive domains(general intelligence, memory, attention-executive function and verbal ability) were obtained and analyzed. By comparison of cognitive function between OSAS children and healthy controls, the effect sizes of each domain were achieved as follows: general intelligence, –0.5(P<0.0001); memory, –0.18(P=0.02); attention-executive function, –0.21(P=0.002); and verbal ability, –0.48(P=0.0006). The effect sizes of general intelligence, memory, attention-executive function, and verbal ability after AT compared to baseline level were –0.37(P=0.008), –0.36(P=0.0005), –0.02(P=0.88), and –0.45(P=0.009), respectively. Comparing the cognitive ability between OSAS children after AT and healthy controls showed that the effect sizes were –0.54(P=0.0009), –0.24(P=0.12), –0.17(P=0.35), and –0.45(P=0.009) in general intelligence, memory, attention-executive function, and verbal ability, respectively. Our results confirmed that OSAS children performed worse than healthy children in terms of the 4 cognitive domains investigated. After 6–12 months of observation, significant improvement in attention-executive function and verbal ability were found in OSAS children treated with AT compared to their baseline level; restoration of attention-executive function and memory were observed in OSAS children after AT in comparison to healthy controls. Further rigorous randomized controlled trials should be conducted to obtain definitive conclusions. 展开更多

BACKGROUND Poor sleep quality is a common clinical feature in patients with type 2 diabetes mellitus(T2DM),and often negatively related with glycemic control.Cognitive behavioral therapy(CBT)may improve sleep quality and reduce blood sugar levels in patients with T2DM.However,it is not entirely clear whether CBT delivered by general practitioners is effective for poor sleep quality in T2DM patients in community settings.AIM To test the effect of CBT delivered by general practitioners in improving sleep quality and reducing glycemic levels in patients with T2DM in community.METHODS A cluster randomized controlled trial was conducted from September 2018 to October 2019 in communities of China.Overall 1033 persons with T2DM and poor sleep quality received CBT plus usual care or usual care.Glycosylated hemoglobin A1c(HbAlc)and sleep quality[Pittsburgh Sleep Quality Index(PSQI)]were assessed.Repeated measures analysis of variance and generalized linear mixed effects models were used to estimate the intervention effects on hemoglobin A1c and sleep quality.RESULTS The CBT group had 0.64,0.50,and 0.9 lower PSQI scores than the control group at 2 mo,6 mo,and 12 mo,respectively.The CBT group showed 0.17 and 0.43 lower HbAlc values than the control group at 6 mo and 12 mo.The intervention on meanΔHbAlc values was significant at 12 mo(t=3.68,P<0.01)and that meanΔPSQI scores were closely related toΔHbAlc values(t=7.02,P<0.01).Intentionto-treat analysis for primary and secondary outcomes showed identical results with completed samples.No adverse events were reported.CONCLUSION CBT delivered by general practitioners,as an effective and practical method,could reduce glycemic levels and improve sleep quality for patients with T2DM in community. 展开更多

OBJECTIVE:To test the influence of acupuncture on cognitive function and a marker of oxidative DNA damage in patients with vascular dementia(VD).METHODS:Sixteen VD patients were evaluated before and after acupuncture,using the Folstein Mini-Mental State Examination-Revised(MMSE-R) to assess cognitive function,and the ADL-R scale to assess independence in activities of daily living(ADL).Life quality was evaluated using the DEMQOL(Dementia quality of life questionnaire) questionnaire,and syndromes and expression of vascular dementia were evaluated with the Scale for the Differentiation of Syndromes of Vascular Dementia(SDSVD).In addition,the urine concentration of 8-hydroxy-2'-deoxyguanosine(8-OHdG)-a marker of oxidative damage-was quantified with enzyme-linked immunosorbent assay.RESULTS:The MMSE-R and DEMQOL scores were higher after acupuncture than before(P<0.05),while there were no obvious differences in the ADL-R or SDSVD scores(P>0.05).The 8-OHdG content in urine significantly decreased after acupuncture(P<0.05).CONCLUSION:Acupuncture reduces the levels of 8-OHdG and improves cognitive function and quality of life in VD patients,suggesting that acupuncture is beneficial at least in part by preventing oxidative damage. 展开更多

In this editorial we comment on the detrimental consequences that post-intensive care syndrome(PICS)has in the quality of life of intensive care unit(ICU)survivors,highlighting the importance of early onset of multidisciplinary rehabilitation from within the ICU.Although,the syndrome was identified and well described early in 2012,more awareness has been raised on the long-term PICS related health problems by the increased number of coronavirus disease 2019 ICU survivors.It is well outlined that the syndrome affects both the patient and the family and is described as the appearance or worsening of impairment in physical,cognitive,or mental health as consequence of critical illness.PICS was described in order:(1)To raise awareness among clinicians,researchers,even the society;(2)to highlight the need for a multilevel screening of these patients that starts from within the ICU and continues after discharge;(3)to present preventive strategies;and(4)to offer guidelines in terms of rehabilitation.An early multidisci-plinary approach is the key element form minimizing the incidence of PICS and its consequences in health related quality of life of both survivors and their families. 展开更多

Maternal sleep deprivation(MSD)is a global public health problem that affects the physical and mental development of pregnant women and their newborns.The latest research suggests that sleep deprivation(SD)disrupts the gut microbiota,leading to neuroinflammation and psychological disturbances.However,it is unclear whether MSD affects the establishment of gut microbiota and neuroinflammation in the newborns.In the present study,MSD was performed on pregnant SpragueDawley rats in the third trimester of pregnancy(gestational days 15-21),after which intestinal contents and brain tissues were collected from offspring at different postnatal days(P1,P7,P14,and P56).Based on microbial profiling,microbial diversity and richness increased in pregnant rats subjected to MSD,as reflected by the significant increase in the phylum Firmicutes.In addition,microbial dysbiosis marked by abundant Firmicutes bacteria was observed in the MSD offspring.Furthermore,quantitative real-time polymerase chain reaction(q RT-PCR)and enzyme-linked immunosorbent assay(ELISA)showed that the expression levels of proinflammatory cytokines interleukin 1β(IL-1β)and tumor necrosis factorα(TNF-α)were significantly higher in the MSD offspring at adulthood(P56)than in the control group.Through Spearman correlation analysis,IL-1βand TNF-αwere also shown to be positively correlated with Ruminococcus_1 and Ruminococcaceae_UCG-005 at P56,which may determine the microbiota-host interactions in MSDrelated neuroinflammation.Collectively,these results indicate that MSD changes maternal gut microbiota and affects the establishment of neonatal gut microbiota,leading to neuroinflammation in MSD offspring.Therefore,understanding the role of gut microbiota during physiological development may provide potential interventions for cognitive dysfunction in MSD-impacted offspring. 展开更多

Previous studies have shown that reduced sleep duration,sleep fragmentation,and decreased sleep quality in patients with Alzheimer's disease are related to dysfunction in orexin signaling.At the same time,blood-brain barrier disruption is considered an early biomarker of Alzheimer's disease.However,currently no report has examined how changes in orexin signaling relate to changes in the blood-brain barrier of patients who have Alzheimer's disease with sleep insufficiency.This cross-sectional study included 50 patients with Alzheimer's disease who received treatment in 2019 at Beijing Tiantan Hospital.Patients were divided into two groups:those with insufficient sleep(sleep duration≤6 hours,n=19,age 61.58±8.54 years,10 men)and those with normal sleep durations(sleep duration>6 hours,n=31,age 63.19±10.09 years,18 men).Demographic variables were collected to evaluate cognitive function,neuropsychiatric symptoms,and activities of daily living.The levels of orexin,its receptor proteins,and several blood-brain barrier factors were measured in cerebrospinal fluid.Sleep insufficiency was associated with impaired overall cognitive function that spanned multiple cognitive domains.Furthermore,levels of orexin and its receptors were upregulated in the cerebrospinal fluid,and the blood–brain barrier was destroyed.Both these events precipitated each other and accelerated the progression of Alzheimer's disease.These findings describe the clinical characteristics and potential mechanism underlying Alzheimer's disease accompanied by sleep deprivation.Inhibiting the upregulation of elements within the orexin system or preventing the breakdown of the blood-brain barrier could thus be targets for treating Alzheimer's disease. 展开更多

BACKGROUND Cancer-related fatigue(CRF)is the most common concomitant symptom in the treatment of colorectal cancer(CRC).Such patients often present with subjective fatigue state accompanied by cognitive dysfunction,which seriously affects the quality of life of patients.AIM To explore the effects of cognitive behavior therapy(CBT)combined with Baduanjin exercise on CRF,cognitive impairment,and quality of life in patients with CRC after chemotherapy,and to provide a theoretical basis and practical reference for rehabilitation of CRC after chemotherapy.METHODS Fifty-five patients with CRC after radical resection and chemotherapy were randomly divided into either an experimental or a control group.The experimental group received the intervention of CBT combined with exercise intervention for 6 mo,and indicators were observed and measured at baseline,3 mo,and 6 mo to evaluate the intervention effect.RESULTS Compared with the baseline values,in the experimental group 3 mo after intervention,cognitive function,quality of life score,and P300 amplitude and latency changes were significantly better(P<0.01).Compared with the control group,at 3 mo,the experimental group had significant differences in CRF,P300 amplitude,and quality of life score(P<0.05),as well as significant differences in P300 latency and cognitive function(P<0.01).Compared with the control group,at 6 mo,CRF,P300 amplitude,P300 latency,cognitive function and quality of life score were further improved in the experimental group,with significant differences(P<0.01).The total score of CRF and the scores of each dimension were negatively correlated with quality of life(P<0.05),while the total score of cognitive impairment and the scores of each dimension were positively correlated with quality of life(P<0.05).CONCLUSION CBT combined with body-building Baduanjin exercise can improve CRF and cognitive impairment in CRC patients after chemotherapy,and improve their quality of life. 展开更多

目的探讨维生素D联合右佐匹克隆治疗老年慢阻肺合并睡眠障碍疗效及对认知功能的保护作用。方法收集140例老年慢阻肺合并睡眠障碍患者作为研究对象,随机分为对照组和研究组,每组70例。对照组给予右佐匹克隆治疗,研究组给予维生素D联合右... 目的探讨维生素D联合右佐匹克隆治疗老年慢阻肺合并睡眠障碍疗效及对认知功能的保护作用。方法收集140例老年慢阻肺合并睡眠障碍患者作为研究对象,随机分为对照组和研究组,每组70例。对照组给予右佐匹克隆治疗,研究组给予维生素D联合右佐匹克隆治疗。采用匹茨堡睡眠质量指数(PSQI)评估睡眠质量。采用简易精神状态量表(MMSE)和蒙特利尔认知评估量表(MoCA)评估认知功能。比较两组睡眠障碍临床疗效、睡眠监测指标、PSQI评分、认知功能和神经营养因子水平。结果研究组睡眠障碍治疗总有效率高于对照组(92.8%vs 80.0%,P<0.05)。睡眠监测指标显示,研究组治疗后总睡眠时间长于对照组[(371.77±27.22)min vs(334.71±28.80 min)],觉醒时间和觉醒次数少于对照组[(32.51±3.89)min vs(46.88±7.32)min,(1.36±0.51)次vs(3.11±0.77)次,P<0.05]。治疗前、治疗后1月和治疗后3月时,不同时间点PSQI评分差异有统计学意义(P<0.05),研究组与对照组PSQI评分变化趋势差异有统计学意义,研究组降低更明显[(15.78±2.02)分vs(15.67±2.05)分,(9.31±2.01)分vs(11.60±2.04)分,(7.60±0.95)分vs(9.40±1.21)分,P<0.05]。研究组治疗后MMSE评分和MoCA评分高于对照组[(28.24±0.92)分vs(26.24±1.53)分,(27.88±1.23)分vs(25.21±1.02)分],脑源性神经生长因子(BDNF)和神经生长因子(NGF)水平高于对照组[(672.29±66.60)pg/mL vs(626.89±55.46)pg/mL,(31.49±5.62)pg/mL vs(23.75±5.18)pg/mL,P<0.05]。结论维生素D联合右佐匹克隆治疗老年慢阻肺合并睡眠障碍疗效良好,而且对认知功能具有一定程度的保护作用。 展开更多
