Increased serum levels of soluble CD146 and vascular endothelial growth factor receptor 2 in patients with exudative age-related macular degeneration 被引量:5

AIM: To investigate serum levels of soluble CD146(s CD146) and vascular endothelial growth factor receptor 2(VEGFR2) in patients with age-related macular degeneration(AMD). METHODS: Eighty-eight patients with exudative AMD and 45 sex-and age-matched healthy controls were enrolled in this study conducted in China. Serum samples was obtained from the patients with exudative AMD and from the controls. Serum sCD146 and VEGFR2 protein levels were measured using an enzyme-linked immunosorbent assay.RESULTS: We found that serum sCD146 and VEGFR2 protein levels were significantly higher in the patients with exudative AMD group than in the controls(t=3.859, P<0.001 and t=3.829, P<0.001, respectively). Serum sCD146 levels were significantly higher in patients with classic choroidal neovascularization(CNV) than in those with occult CNV(t=9.899, P<0.001). There was a significant difference in the trend for exudative AMD in the highest versus lowest quartile of circulating sCD146 levels(χ2=10.29, P=0.001). The receiver operating characteristic curve analysis showed that the area under the curve was 0.696 for s CD146(95%CI: 0.601-0.791) with an optimum diagnostic cut-off value of 157.16 ng/mL, a sensitivity of 55.7%, and a specificity of 82.2%.CONCLUSION: The serum sCD146 level increases and may be a biomarker for exudative AMD. 展开更多

AIM: To investigate the serum levels of vascular endothelial growth factor receptor-2(VEGFR-2) and adropin in age-related macular degeneration(AMD)patients.·METHODS: Ninety-eight AMD patients were included in the study. Seventy-eight age- and sex-matched healthy volunteers were recruited as the control group.Fundus florescein angiography and optical coherence tomography were performed to assess the posterior segment details. Serum VEGFR-2 and adropin levels were measured using enzyme-linked immunosorbent assays and compared between the study groups.· RESULTS: AMD group had significantly increased foveal retinal thickness, serum LDL and HDL levels and significantly decreased subfoveal choroidal thickness(P =0.01, 0.047, 0.025 and 〈0.001, respectively). Serum VEGFR-2level revealed a significant decrease in AMD patients compared to controls(26.48 ±6.44 vs 30.42 ±7.92 ng/m L,P 〈0.001). There was an insignificant increase in serum adropin level in AMD patients(6.17±3.19 vs 5.79±2.71 ng/m L,P =0.4). Serum level of VEGFR-2 in AMD patients had a significant negative correlation with foveal retinal thickness(r =-0.226, P =0.025) and a significant positive correlation with subfoveal choroidal thickness(r=0.2, P=0.048).·CONCLUSION: The current study demonstrated that the decreased serum VEGFR-2 level may be considered in the development of AMD. Adropin does not seem to play a role in the pathogenesis of AMD. 展开更多

目的探究恩格列净联合重组人脑利钠肽(rhBNP)治疗射血分数降低型心力衰竭(HFrEF)的疗效及对患者心功能和血清成纤维细胞生长因子-21(FGF-21)、可溶性血管内皮生长因子受体-2(sVEGFR-2)水平的影响。方法前瞻性选取2022年1月至2023年3月... 目的探究恩格列净联合重组人脑利钠肽(rhBNP)治疗射血分数降低型心力衰竭(HFrEF)的疗效及对患者心功能和血清成纤维细胞生长因子-21(FGF-21)、可溶性血管内皮生长因子受体-2(sVEGFR-2)水平的影响。方法前瞻性选取2022年1月至2023年3月山东第一医科大学附属人民医院心内科收治的100例HFrEF患者为研究对象。按照随机数字表法将其分为对照组和观察组,每组各50例。对照组予以常规强心、利尿治疗及静脉泵入rhBNP,观察组在对照组基础上加用恩格列净。比较两组治疗前、治疗3个月后糖化血红蛋白(HbA1c)、总胆固醇、低密度脂蛋白胆固醇(LDL-C)、6 min步行距离测试(6MWD)、N末端脑钠肽前体(NT-proBNP)、左室射血分数(LVEF)及血清FGF-21、sVEGFR-2水平,并记录两组主要心血管不良事件(MACE)发生情况。结果观察组临床总有效率为94.00%,高于对照组(80.00%),差异有统计学意义(P<0.05)。治疗3个月后,观察组HbA1c水平为(5.34±1.51)%,低于对照组[(6.03±1.65)%],差异有统计学意义(P<0.05);两组患者的总胆固醇、LDL-C水平比较,差异均无统计学意义(P>0.05)。治疗3个月后,观察组LVEF、6MWD水平分别为(38.52±7.24)%、(493.66±47.79)m,均高于对照组[(34.02±8.06)%、(455.92±50.25)m],NT-proBNP水平为(4386.75±875.89)pg/mL,低于对照组[(4326.04±843.66)pg/mL],差异均有统计学意义(P<0.05)。治疗3个月后,观察组血清FGF-21水平为(54.93±12.51)pg/mL,低于对照组[(61.50±15.66)pg/mL],sVEGFR-2水平为(8.96±1.95)ng/mL,高于对照组[(8.14±1.67)ng/mL],差异均有统计学意义(P<0.05)。观察组与对照组MACE总发生率分别为6.00%、16.00%,差异无统计学意义(P>0.05)。结论在rhBNP治疗HFrEF的基础上加用恩格列净可明显提高临床疗效,改善患者心功能,降低HbA1c、FGF-21水平,并提高sVEGFR-2水平可能是其增效的作用机制。 展开更多

OBJECTIVE:To investigate the effects of emodin on alkali burn-induced corneal inflammation and neovascularization.METHODS:The ability of emodin to target vascular endothelial growth factor receptor 2(VEGFR2)was predicted by molecular docking.The effects of emodin on the invasion,migration,and proliferation of human umbilical vein endothelial cells(HUVEC)were determined by cell counting kit-8,Transwell,and tube formation assays.Analysis of apoptosis was performed by flow cytometry.CD31 levels were examined by immunofluorescence.The abundance and phosphorylation state of VEGFR2,protein kinase B(Akt),signal transducer and activator of transcription 3(STAT3),and P38 were examined by immunoblot analysis.Corneal alkali burn was performed on 40 mice.Animals were divided randomly into two groups,and the alkali-burned eyes were then treated with drops of either 10μM emodin or phosphate buffered saline(PBS)four times a day.Slitlamp microscopy was used to evaluate inflammation and corneal neovascularization(CNV)in all eyes on Days 0,7,10,and 14.The mice were killed humanely 14 d after the alkali burn,and their corneas were removed and preserved at-80℃ until histological study or protein extraction.RESULTS:Molecular docking confirmed that emodin was able to target VEGFR2.The findings revealed that emodin decreased the invasion,migration,angiogenesis,and proliferation of HUVEC in a dose-dependent manner.In mice,emodin suppressed corneal inflammatory cell infiltration and inhibited the development of corneal neovascularization induced by alkali burn.Compared to those of the PBS-treated group,lower VEGFR2 expression and CD31 levels were found in the emodintreated group.Emodin dramatically decreased the expression of VEGFR2,p-VEGFR2,p-Akt,p-STAT3,and p-P38 in VEGF-treated HUVEC.CONCLUSION:This study provides a new avenue for evaluating the molecular mechanisms underlying corneal inflammation and neovascularization.Emodin might be a promising new therapeutic option for corneal alkali burns. 展开更多

老年慢性心力衰竭患者血清SCD-1和sVEGFR-2表达水平及其与预后的评估价值研究 被引量:1

目的探讨血清多配体蛋白聚糖-1(serum syndecan-1,SCD-1)和可溶性血管内皮生长因子受体-2(soluble vascular endothelial growth factor receptor-2,sVEGFR-2)对老年慢性心力衰竭(chronic heart failure,CHF)患者预后的预测价值。方法选... 目的探讨血清多配体蛋白聚糖-1(serum syndecan-1,SCD-1)和可溶性血管内皮生长因子受体-2(soluble vascular endothelial growth factor receptor-2,sVEGFR-2)对老年慢性心力衰竭(chronic heart failure,CHF)患者预后的预测价值。方法选择2018年3月~2021年10月北京市丰台中西医结合医院心内科住院部收治的175例老年CHF患者,检测血清SCD-1和sVEGFR-2水平,出院后随访和统计因心力衰竭再次住院和心源性死亡发生情况。多因素Logistic回归分析老年CHF患者预后不良的危险因素。受试者工作特征曲线(ROC)分析SDC-1和sVEGFR-2预测老年CHF患者预后不良的价值。结果177例老年CHF患者随访期间失访2例,最终175例纳入结果分析。预后不良组血清SCD-1(6.95±1.87 ng/ml)和sVEGFR-2(2.75±0.46μg/L)水平高于预后良好组(4.21±0.63 ng/ml,1.02±0.35μg/L),差异均有统计学意义(t=14.454,24.465,均P<0.05)。多因素Logistic回归分析结果显示NYHAⅣ级、高水平B型脑钠肽、高水平SCD-1,高水平sVEGFR-2是老年CHF患者预后不良的危险因素(Waldχ^(2)=8.827,10.856,7.594,8.627,均P<0.01)。SCD-1和sVEGFR-2预测老年CHF患者预后的曲线下面积为0.700(95%CI:0.626~0.766)和0.761(95%CI:0.691~0.822),与BNP[0.804(95%CI:0.737~0.860)]比较,差异均无统计学意义(z=1.769,1.123,均P>0.05);联合SCD-1,sVEGFR-2和BNP预测老年CHF患者预后的曲线下面积为0.943(95%CI:0.898~0.973),高于单独SCD-1,sVEGFR-2和BNP(z=4.586,3.851,3.094,均P<0.05)。结论老年CHF预后不良患者血清SCD-1和sVEGFR2水平均显著增高,且与老年CHF患者预后不良有关,可作为老年CHF患者预后分析的潜在标志物。 展开更多

目的 探讨血清成纤维细胞生长因子-21(FGF-21)、白细胞介素-13(IL-13)、可溶性血管内皮生长因子受体-2(sVEGFR-2)水平与急性心力衰竭患者短期预后的相关性。方法 选取2019年1月至2020年12月在我院接受治疗的急性心力衰竭患者136例作为... 目的 探讨血清成纤维细胞生长因子-21(FGF-21)、白细胞介素-13(IL-13)、可溶性血管内皮生长因子受体-2(sVEGFR-2)水平与急性心力衰竭患者短期预后的相关性。方法 选取2019年1月至2020年12月在我院接受治疗的急性心力衰竭患者136例作为心力衰竭组。选取同期在我院健康体检者80例作为健康对照组。采用ELISA法检测血清FGF-21、IL-13、sVEGFR-2水平。随访1年,根据急性心力衰竭患者有无不良心血管事件发生分为预后良好组和预后不良组,并分析影响患者1年内临床结局的危险因素。采用ROC曲线,分析FGF-21、IL-13、sVEGFR-2对急性心力衰竭患者预后不良的诊断效能。结果 心力衰竭组血清FGF-21、IL-13高于健康对照组[FGF-21(pg/mL):74.81±16.29 vs. 19.37±5.11, IL-13(pg/mL):5.14±1.23 vs.1.62±0.54,P<0.001]、sVEGFR-2低于健康对照组(ng/mL:6.30±1.57 vs.15.18±4.29,P<0.05)。预后不良组血清FGF-21、IL-13、NT-proBNP分别高于预后良好组[FGF(pg/mL):92.31±20.84 vs.67.52±13.70, IL-13(pg/mL):6.94±1.75 vs.4.39±1.18, NT-proBNP(ng/L):4689.16±132.50 vs.2120.59±51.48],LVEF值、血清sVEGFR-2低于预后良好组[LVEF(%):45.27±6.13 vs.53.95±7.80, sVEGFR(ng/mL):4.85±1.24 vs.6.98±2.31,P<0.05]。多因素Logistic回归分析结果显示,LVEF(95%CI 0.265~0.840,P<0.001)及血清NT-proBNP(95%CI 1.002~2.317,P=0.015)、FGF-21(95%CI 1.438~5.019,P<0.001)、IL-13(95%CI 1.027~3.165,P=0.006)、sVEGFR-2(95%CI 1.190~4.283,P=0.002)是影响急性心力衰竭患者不良预后的独立危险因素。ROC曲线结果显示,血清FGF-21、IL-13、sVEGFR-2及三项联合检测预测急性心力衰竭患者不良预后的曲线下面积分别为0.810、0.775、0.821、0.918。结论 血清FGF-21、IL-13、sVEGFR-2水平与急性心力衰竭患者NYHA分级及临床预后密切相关,联合检测血清FGF-21、IL-13、sVEGFR-2对评估患者不良预后有较高参考价值。 展开更多

血清LCN2、sFlt-1对脓毒症患者并发肾损伤的预测价值

目的 分析血清脂质运载蛋白2(LCN2)、可溶性血管内皮生长因子受体-1(sFlt-1)对脓毒症患者并发肾损伤(AKI)的预测价值。方法 将南京医科大学附属苏州医院2021年6月1日至2023年6月30日收治的脓毒症患者120例作为研究对象,根据是否并发AKI... 目的 分析血清脂质运载蛋白2(LCN2)、可溶性血管内皮生长因子受体-1(sFlt-1)对脓毒症患者并发肾损伤(AKI)的预测价值。方法 将南京医科大学附属苏州医院2021年6月1日至2023年6月30日收治的脓毒症患者120例作为研究对象,根据是否并发AKI分为AKI组(27例)和无AKI组(93例),另选同期慢性肾脏病(CKD)患者120例,同期健康体检者120例作为对照,采用酶联免疫吸附实验测定法检测血清LCN2、sFlt-1水平。采用Pearson法分析脓毒症患者血清LCN2与sFlt-1水平的相关性;采用Box-Tidwell法验证自变量与因变量间线性关系;采用多因素Logistic回归分析探讨脓毒症患者并发AKI的影响因素;采用ROC曲线分析血清LCN2、sFlt-1水平预测脓毒症患者并发AKI的诊断价值。结果 脓毒症患者、CKD患者、健康体检者血清LCN2、sFlt-1水平依次降低(P<0.05)。AKI组患者血清LCN2、sFlt-1水平明显高于无AKI组(P<0.05)。AKI组患者C反应蛋白、APACHEⅡ评分高于无AKI组(P<0.05)。Pearson相关分析结果显示,血清LCN2与sFlt-1水平呈正相关(r=0.412,P<0.001)。多因素Logistic回归分析结果显示,LCN2、sFlt-1是脓毒症患者合并AKI的危险因素(P<0.05)。ROC曲线显示,血清LCN2、sFlt-1水平联合预测脓毒症患者发生AKI的AUC为0.928,明显高于二者单独检测(ZLCN2-联合=2.177,P=0.030;ZsFlt-1-联合=2.038,P=0.042),联合预测的敏感性为88.89%,特异性为88.17%。结论 脓毒症患者血清LCN2、sFlt-1水平较高,其高表达与并发AKI有关,且对AKI的发生有一定预测价值。 展开更多

Objective: To observe the effect of Kang'ai Injection (康艾注射液, KAI) on serum level of soluble interleukin-2 receptor (sIL-2R) and vascular endothelial growth factor (VEGF) in patients with esophageal carcinoma (EC) during radiotherapy (RT), and to investigate its synergistic effect with RT and its influence on immunological function of the body. Methods: One hundred and seventy patients with EC, who had missed the chance of surgical operational therapy, were assigned to the treated group (90 cases) and the RT group (80 cases), and at the same time a control group consisting of 80 inpatients without tumors was set up. Patients in the RT group were treated with RT alone but KAI was given additionally to those in the treated group, with 50 ml given once per day via intravenous dripping, 15 days as one course, and 2 courses administered in total. The immediate therapeutic efficacy and changes of serum slL-2R and VEGF levels were observed, and the effect of KAI on patients' quality of life (QOF) was evaluated by Karnofsky scoring. Results: In 16 patients of the treated group it was completely remission (OR), in 54 partially remission (PR), in 18 it was stabilized disease (SD) and in 2 progressive disease (PD), with the total effective rate (CR + PR) as 77.8%, while in those of the control group it was 12, 46, 18, 4 and 72.5%, respectively, the immediate therapeutic efficacy in the treated group was somewhat better than that in the RT group, but showed no statistical significance (P〉0.05). Serum levels of slL-2R and VEGF in all the patients before treatment were higher than those in the control group, which were decreased after treatment in both groups (P〈0.05), but the improvement in the treated group was better than that in the RT group, showing significant difference (P〈0.05), and patients' QOF improved more significantly in the former as well (62.2 % vs 40.0%, P〈 0.05). Conclusion: KAI in combination with RT in treating patients with EC could enhance the immunological function of patients, improve their QOF and enhance their sensitivity to RT. 展开更多

Objective::The role of Vitamin D-binding protein(DBP)in preeclampsia(PE)pathogenesis is unknown.In this study,we compared the expression of DBP in the placentas of PE patients with the placentas of normotensive pregnant women with placenta previa controls,and aimed to explore the effect of DBP on endothelial cells(ECs)and the underlying mechanism.Methods::DBP expression in placental tissues collected from PE patients and controls was evaluated by immunohistochemistry.The downregulation and upregulation of DBP expression in HTR-8/SVneo cells were examined using DBP-targeting small interfering RNA(siRNA)and DBP-expression vector,respectively.The conditioned media of these DBP-overexpressing and DBP-siRNA HTR-8/SVneo cells were collected and added to human umbilical vein EC(HUVEC)cultures.Angiogenic effects on HUVECs were assessed by tube formation assays,and the proliferation and migration of HUVECs were examined using the Real-Time Cell Analyzer.The expression of vascular endothelial growth factor(VEGF)and VEGF receptor(VEGFR)-2,as well as the phosphorylation of different residues of VEGFR-2 in HUVECs,were determined by western blotting.Results::DBP expression was significantly increased in the placental tissues collected from PE patients.The conditioned medium of DBP-overexpressing HTR-8/SVneo cells potently inhibited tube formation by HUVECs,in addition to their proliferation and migration.Furthermore,treatment of HUVECs with the conditioned medium of DBP-overexpressing HTR-8/SVneo cells decreased the phosphorylation of VEGFR-2 at tyrosine 996,whereas the treatment of these cells with the conditioned medium of DBP-siRNA HTR-8/SVneo cells increased the phosphorylation of VEGFR-2 at tyrosine 951,996,and 1,175.Conclusions::The expression of DBP is increased in the placentas of PE patients.DBP plays potential roles in endothelial dysfunction,which contributes to PE development,by inhibiting tyrosine phosphorylation of VEGFR-2 in ECs. 展开更多

The remaining rats were injected intradermally with emulsified type Ⅱ bovine collagen at the tail base and back, followed by a booster 7 d post first immunization. The rats were treated orally for 30 d as follows: group 1, saline; group 2, model(saline); group 3, tripterygium polyglycoside(TP; 7.81 mg/kg, positive control);group 4, Huatan Tongluo decoction(HTTL; 7.5 g/kg). Body weight, ankle swelling and arthritis index were measured over the course of the study. The rats were sacrificed 30 d after treatment. Morphological changes in the synovium were observed by hematoxylin and eosin staining. Pannus formation and synovial thickness in the left ankle were observed by color Doppler ultrasoundVascular endothelial growth factor(VEGF) and VEGFR2 protein levels were measured by immunohistochemistry.VEGF/VEGFR2 mRNA levels were measured by real-time quantitative polymerase chain reaction.RESULTS: Compared with the model group, a significantly lower arthritis index was observed in the positive control group(P < 0.05) and HTTL group(P < 0.01), after treatment. Both positive control and HTTL reduced intra-articular pannus formation and synovial thickening. Furthermore, VEGF mRNA,and VEGFR2 protein and mRNA levels were significantly downregulated(P < 0.05) in the treatment groups.CONCLUSION: Inhibition of the expression of VEGF and VEGFR2 in synovial tissues and the formation of pannus and synovial hyperplasia may be part of the mechanism of HTTL for relieving the symptoms of rheumatoid arthritis in CIA rats. 展开更多

目的:探讨促甲状腺激素(thyroid stimulating hormone,TSH)抑制疗法对分化型甲状腺癌(differentiated thyroid carcinoma,DTC)患者术后血清甲状腺球蛋白(thyroglobulin,Tg)、血管内皮生长因子(vascular endothelial growth factor,VEGF... 目的:探讨促甲状腺激素(thyroid stimulating hormone,TSH)抑制疗法对分化型甲状腺癌(differentiated thyroid carcinoma,DTC)患者术后血清甲状腺球蛋白(thyroglobulin,Tg)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、肿瘤特异性生长因子(tumors pecific growth factor,TSGF)、白细胞分化抗原44变异型6(CD44V6)、可溶性白细胞介素-2受体(soluble interleukin-2receptor,sIL-2R)及T淋巴细胞亚群水平的影响。方法:选择2014年1月~2017年1月我院收治的接受甲状腺全切手术治疗的100例DTC患者,随机分为对照组和实验组,各50例。对照组患者常规给予甲状腺素替代治疗,实验组患者给予TSH抑制疗法(口服左甲状腺素钠片,控制血清TSH水平低于0.1mU/L),两组患者均给予治疗1个月。比较两组患者治疗前后血清Tg、VEGF、TSGF、CD44V6、sIL-2R水平及外周血CD3^+、CD4^+、CD8^+水平。结果:两组治疗前的血清Tg、VEGF、TSGF、CD44V6、sIL-2R水平比较,均无显著性差异(P>0.05);两组治疗后的血清Tg、VEGF、TSGF、CD44V6、sIL-2R水平相比治疗前均较低,且实验组治疗后血清Tg、VEGF、TSGF、CD44V6、sIL-2R水平变化均显著优于对照组(P<0.05)。两组治疗前的外周血CD3^+、CD4^+、CD8^+水平比较,均无显著性差异(P>0.05);两组治疗后的外周血CD3^+、CD4^+水平相比治疗前均较高、CD8^+水平相比治疗前均较低,且实验组治疗后血外周血CD3^+、CD4^+、CD8^+水平变化均优于对照组,具有显著性差异(P<0.05)。结论:DTC行甲状腺全切手术治疗后接受TSH抑制疗法能够有效降低血清Tg、VEGF、TSGF、CD44V6、sIL-2R水平,改善细胞免疫功能,值得在临床上推广应用。 展开更多

目的检测妊娠期糖尿病并发子痫前期患者血清Lipocalin-2、可溶性血管内皮生长因子受体1(s Flt-1)、胎盘生长因子(PIGF)水平,并探讨其临床意义。方法选取2013年3月至2015年6月定期孕前检查并住院分娩的单胎妊娠的孕妇212例作为研究对象... 目的检测妊娠期糖尿病并发子痫前期患者血清Lipocalin-2、可溶性血管内皮生长因子受体1(s Flt-1)、胎盘生长因子(PIGF)水平,并探讨其临床意义。方法选取2013年3月至2015年6月定期孕前检查并住院分娩的单胎妊娠的孕妇212例作为研究对象。并将其分为妊娠期糖尿病并发子痫前期组(GDM-PE组)45例,妊娠期糖尿病组(GDM组)55例,子痫前期组(PE组)52例,正常孕妇60例(对照组)。然后用酶联免疫吸附法(ELISA)检测各组Lipocalin-2及s Flt-1、PIGF水平。结果四组研究对象的年龄、孕周及孕前BMI比较,差异均无统计学意义(P>0.05)。GDM-PE组孕期BMI、收缩压、舒张压等各项指标均大于GDM组和对照组(P<0.05)。PE组和GDM-PE组孕期BMI、收缩压、舒张压等各项指标均大于GDM组和对照组(P<0.05)。ELISA检测结果显示:和对照组相比,Lipocalin-2在GDM、PE及GDM-PE组表达水平均升高(P<0.05),GDM-PE组Lipocalin-2表达高于GDM组和PE组(P<0.05);s Flt-1表达在GDM、PE及GDM-PE组均高于对照组(P<0.05),在PE及GDM-PE组均高于GDM组(P<0.05);PIGF表达在GDM、PE及GDM-PE组均低于对照组(P<0.05)。结论血清Lipocalin-2和s Flt-1水平在GDM-PE患者中会明显升高,而PIGF水平则会明显降低,其原因可能与胰岛素抵抗和内皮细胞损伤有关。三个指标联合应用于早期预测GDM-PE疾病具有一定的临床意义。 展开更多

sFlt-1、sST2和MSP-α联合检测预测子痫前期的价值研究

目的巨噬细胞刺激蛋白-α(MSP-α)、可溶性基质素2(sST2)和可溶性血管内皮生长因子受体-1(sFlt-1)联合检测对子痫前期的预测价值。方法采用前瞻性研究,选择2017年10月至2019年10月在上海市嘉定区妇幼保健院定期产检和分娩的孕产妇,根据... 目的巨噬细胞刺激蛋白-α(MSP-α)、可溶性基质素2(sST2)和可溶性血管内皮生长因子受体-1(sFlt-1)联合检测对子痫前期的预测价值。方法采用前瞻性研究,选择2017年10月至2019年10月在上海市嘉定区妇幼保健院定期产检和分娩的孕产妇,根据是否并发子痫前期分为子痫前期组(70例)和对照组(87例),采用ELISA法检测孕产妇血清中MSP-α、sST2和sFlt-1的浓度,分析其与子痫前期的相关性,采用ROC曲线分析3项指标单独和联合检测对该病的预测价值。结果子痫前期组在早孕期和中孕期的sFlt-1、MSP-α水平均高于对照组(t值分别为5.877、5.674、2.427、3.390),中孕期的sST2水平亦高于对照组(t=2.852),差异均有统计学意义(P<0.05)。Logistic回归分析结果提示早、中孕期的sFlt-1及中孕期sST2是子痫前期发病的危险因素,sFlt-1在早孕期和中孕期增高,子痫前期发病的危险增大,OR及其95%CI分别为:1.434(1.186~1.733)、1.724(1.403~2.117),P<0.05;中孕期sST2增高,子痫前期发病的危险性增大,OR及其95%CI为1.163(1.001~1.303),P<0.05。sFlt-1、sST2和MSP-α三者单独检测的敏感性和特异性均低于联合检测,联合检测的敏感性为81.4%,特异性为94.3%,曲线下面积达91.1%(P<0.05)。结论早、中孕期sFlt-1、sST2和MSP-α血清学分子联合检测对子痫前期的发病具有一定的预测价值。 展开更多

目的从血管内皮生长因子(VEGF)和可溶性血管内皮生长因子受体2(sVEGFR2)探究分子靶向药物舒尼替尼(Sunitinib)治疗转移性肾癌的机制。方法选取100例转移性肾癌患者,其中50例患者服用Sunitinib进行治疗记为Sunitinib组;另50例患者不服用... 目的从血管内皮生长因子(VEGF)和可溶性血管内皮生长因子受体2(sVEGFR2)探究分子靶向药物舒尼替尼(Sunitinib)治疗转移性肾癌的机制。方法选取100例转移性肾癌患者,其中50例患者服用Sunitinib进行治疗记为Sunitinib组;另50例患者不服用相关药物,记为不服药组。治疗组50例患者中按照获益与非获益人群分组为获益组(n=25)和非获益组(n=25)。分析所有患者的疗效、并发症、在治疗初始及结束后的肾门淋巴结CT总径值、VEGF和sVEGFR2水平。结果 Sunitinib组患者的疗效提升(P<0.05),Sunitinib治疗的2个亚组与不服药组在电解质紊乱、胃肠道反应(恶心、腹泻、呕吐)、左室射血分数降低、肾功能减退、严重电解质紊乱等并发症发生率有显著性差异(P<0.05)。治疗初始肾门淋巴结个数与血管内皮生长因子水平呈负相关(P<0.05),与可溶性血管内皮生长因子受体2呈正相关(P<0.05)。治疗末期的肾门淋巴结数与血管内皮生长因子水平呈负相关(P<0.05),而与可溶性血管内皮生长因子受体2无相关性(P>0.05)。结论 Sunitinib治疗过程中,病灶的转移与VEGF水平呈显著负相关,与治疗前sVEGFR2水平呈显著正相关。 展开更多

目的探讨真武汤加减对阳虚水泛型冠心病慢性心力衰竭的治疗作用及对脑钠肽(BNP)、可溶性生长刺激表达基因2蛋白(sST2)、血管内皮生长因子(VEGF)水平的影响。方法选择2017年2月—2019年2月医院心内科门诊及住院患者收治的慢性心力衰竭患... 目的探讨真武汤加减对阳虚水泛型冠心病慢性心力衰竭的治疗作用及对脑钠肽(BNP)、可溶性生长刺激表达基因2蛋白(sST2)、血管内皮生长因子(VEGF)水平的影响。方法选择2017年2月—2019年2月医院心内科门诊及住院患者收治的慢性心力衰竭患者62例,依据治疗方法不同分为实验组(30例)和参照组(32例)。参照组采用西药常规治疗,实验组在对照组治疗的基础上配合真武汤加减治疗。两周均以8周为1个治疗周期。比较两组治疗前后的中医证候积分和治疗疗效,比较两组治疗前后心功能指标:N末端脑钠肽前体(NT-proBNP)水平、左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD),比较两组治疗前后BNP、sST2、VEGF水平。结果实验组治疗总有效率90.00%(27/30)高于对照组治疗总有效率的(68.75%,22/32)(P<0.05),实验组治疗后心悸、气短喘息、胸闷、浮肿和肢寒体冷等中医证候积分低于参照组(P<0.05);实验组治疗后心功能指标NT-proBNP、LVESD和LVEDD水平低于参照组,LVEF水平高于参照组(P<0.05);实验组治疗后BNP、sST2水平低于参照组,VEGF高于参照组(P<0.05)。结论真武汤加减可有效的改善冠心病慢性心力衰竭的临床症状,减轻心力衰竭程度,提升心脏功能,增强心肌功能,控制病情进展,提高治疗疗效,值得临床进一步研究探讨。 展开更多
