Clinical Study of Shentong Zhuyu Decoction Combined with Massage Therapy in the Treatment of Exertional Chronic Lumbar Muscle Strain

[Objectives]To explore the effects of Shentong Zhuyu decoction combined with massage therapy in the treatment of exertional chronic lumbar muscle strain.[Methods]Sixty-four cases of exertional chronic lumbar muscle strain were randomly divided into two groups(32 cases each group).The patients in the control group only took celecoxib capsules,and those in the treatment group additionally took Shentong Zhuyu decoction combined with massage therapy.TCM syndrome score,lumbar function,hemorrheology index and clinical effect were compared between the two groups before and after treatment.[Results]After treatment,the TCM syndrome scores of lumbar distension/dull pain,tingling-like lumbago,adverse lateral turn,body weight loss,dark purple tongue,slow or astringent pulse,and Oswestry disability index(ODI)score in the treatment group were lower than those in the control group,and the levels of plasma viscosity,red blood cell aggregation index,platelet aggregation rate(PAG)and fibrinogen(Fib)were lower than those in the control group,showing statistical significance(P<0.05).The overall clinical effect distribution of the treatment group was better than that of the control group,and the difference was statistically significant(P<0.05).[Conclusions]Shentong Zhuyu decoction combined with massage therapy can effectively relieve the symptoms of patients with lumbago and improve the lumbar mobility function and hemorrheology,with obvious therapeutic effects in the treatment of exertional chronic lumbar muscle strain. 展开更多

Objective:To study the therapeutic effect of Shaoyao Jiawei Gancao Decoction(芍药加味甘草汤)with acupoint application on chronic lumbar muscle strain and the recovery of lumbar strength.Methods:From August 2018 to August 2019,100 patients with chronic lumbar muscle strain admitted to our hospital were selected and randomly divided into an acupoint application group and a combined group of 50 cases each.The acupoint application group was treated by acupoint application,and the combined group was treated with Shaoyao Jiawei Gancao Decoction(芍药加味甘草汤)based on acupoint application.The indexes scores,VAS scores,ADL scores,ODI scores,lumbar endurance,TCM syndrome scores and the treatment effects of the two groups before and after treatment were statistically analyzed,and TNF-αand TXB2 levels were detected.Results:After treatment,the scores of indexes,VAS,ODI,TCM syndrome,TNF-αand TXB2 in the combined group were lower than those in the acupoint application group,and the ADL score,lumbar endurance and treatment efficiency were higher than those in the acupoint application group,with a statistical difference(P<0.05).Conclusion:Shaoyao Jiawei Gancao Decoction(芍药加味甘草汤)with acupoint application can effectively reduce pain,improve lumbar function,restore lumbar strength,effectively relieve inflammation,with significant effects. 展开更多

Reduction of epinephrine in the lumbar spinal cord following repetitive blast-induced traumatic brain injury in rats

Traumatic brain inju ry-induced unfavorable outcomes in human patients have independently been associated with dysregulated levels of monoamines,especially epinephrine,although few preclinical studies have examined the epinephrine level in the central nervous system after traumatic brain injury.Epinephrine has been shown to regulate the activities of spinal motoneurons as well as increase the heart rate,blood pressure,and blood flow to the hindlimb muscles.Therefore,the purpose of the present study was to determine the impact of repeated blast-induced traumatic brain injury on the epinephrine levels in seve ral function-s pecific central nervous system regions in rats.Following three repeated blast injuries at 3-day intervals,the hippocampus,motor cortex,locus coeruleus,vestibular nuclei,and lumbar spinal cord were harvested at post-injury day eight and processed for epinephrine assays using a high-sensitive electrochemical detector cou pled with high-performance liquid chromatography.Our results showed that the epinephrine levels were significantly decreased in the lumbar spinal cord tissues of blast-induced traumatic brain injury animals compared to the levels detected in age-and sex-matched sham controls.In other function-specific central nervous system regions,although the epinephrine levels were slightly altered following blast-induced tra u matic brain injury,they were not statistically significant.These results suggest that blast injury-induced significant downregulation of epinephrine in the lumbar spinal cord could negatively impact the motor and cardiovascular function.This is the first repo rt to show altered epinephrine levels in the spinal cord following repetitive mild blast-induced traumatic brain injury. 展开更多

Background Magnetic resonance imaging (MRI) is the most sensitive technique for evaluating the healing process and should be performed before the patients return to their exercise routines. The aim of this research was to diagnose chronic lumbago associated with lumbar muscle strain and to monitor healing process by MRI. Methods Sixty-five symptomatic cases of chronic lumbago caused by lumbar muscle strain were collected from March 2009 to October 2011. MRI was used to examine, diagnose and monitor the healing process. The control group included 65 random cases of asymptomatic volunteers. MRI methods included routine sequences of GRE TlWl, TSE T2WI and special sequences of T2-STIR-FS, combined with DWI. We compared the MRI characteristics of symptomatic cases before and after healing and with asymptomatic controls. Results The important MRI characteristics of chronic lumbago with lumbar muscle strain included: (1) The low back muscle showed edema. (2) The low back intermuscular spaces showed edema and/or fluid. (3) The low back spaces beside the spinous process showed edema and/or fluid. (4) The low back vertebral articular process fossae or transverse process fossae showed fluid. Of these image characteristics, the intermuscular space edema provided the best diagnostic sensitivity, Se=83%, with Y1=0.63, Tr=74%. The low back muscle edema provided the best diagnostic specificity, Sp=100%, with Y1=0.66, Tr=83%. And the spaces edema beside the spinous process provided the best diangnostic accuracy, Tr=86%, with YI = 0.71, Se=80%, Sp=91%. The diagnosis accurate could be improved by combining multiple MRI characteristics. The diagnostic accuracy could achieve n=93%, with Y1=0.86, Se=100% and Sp=86% when two characteristics were combined. After rehabilitation care, the edema disappeared on the repeated MRI. Conclusions MRI may well be a useful diagnostic method for lumbago with lumbar muscle strain. Combining routine sequences with T2-STIR-FS and DWI sequences could demonstrate the pathological changes of lumbar muscle strain and monitor the healing. 展开更多

Objective: To observe the clinical effect of electroacupuncture(EA)combined with tuina on acute lumbar sprain and lumbar muscle tone before and after treatment. Methods: A total of 130 acute lumbar sprain cases were randomly allocated into an observation group and a control group, 65 in each group. Cases in the observation group were treated with EA combined with tuina, whereas cases in the control group were treated with Diclofenac Sodium Dual Release Enteric-coated capsules. The muscle tones in two groups were tested before and after treatment. Results: The recovery rate and overall response rate in the observation group were 66.2% and 93.8% respectively, versus 56.9% and 87.7% in the control group, showing no between-group statistical significances(P>0.05). After treatment, the force-displacement area under curve(AUC) in both groups showed a significance difference(P<0.05). Conclusion: Both EA combined with tuina and aforementioned oral medication have remarkable effects for acute lumbar sprain, and the former can better improve the lumbar muscle tone in patients with acute lumbar sprain. 展开更多

Model-based assessment of muscle forces and strain distribution in the femur of Cabot’s Tragopans(Tragopan caboti)

The hindlimbs play a crucial role in bird locomotion,making the biomechanical properties of the musculoskeletal system in these limbs a focal point for researchers studying avian behaviour.However,a comprehensive analysis of the mechanical performance within the long bones of hindlimbs during locomotion remains lacking.In the present study,the strain and deformation of the femur of Cabot’s Tragopans(Tragopan caboti)were estimated.We employed inverse simulation to calculate the force and moment of femoral muscles during mid-stance terrestrial locomotion and conducted finite element analysis to calculate femoral strain.Results showed that during mid-stance,the femur experiences combined deformation primarily characterized by torsion,bending,and compression.It emphasises the importance of considering the influence of varying loads on bone adaptation when investigating bone form-function relationships.Muscles were found to play a significant role in offsetting joint loads on the femur,subsequently reducing the deformation and overall strain on the bone.This reduction enhances femoral safety during locomotion,allowing birds to meet mechanical demands while maintaining a lightweight bone structure.Notably,the M.iliotrochantericus caudalis significantly reduces torsional deformation of the proximal femur,protecting the vulnerable femoral neck from high fracture risk induced by rotation load.Given that the femur torsion during terrestrial locomotion in birds is associated with changes in hindlimb posture due to their adaptation to flight,the characteristics of M.iliotrochantericus caudalis may provide insight into the locomotor evolution of theropods and the origin of avian flight. 展开更多

目的观察超声引导下外侧弓状韧带上前路腰方肌阻滞(QLB-LSAL)与腹横肌平面阻滞(TAPB)对腹腔镜肝脏部分切除术(LPH)后镇痛与恢复质量的影响。方法选择择期行LPH患者58例,采用随机数字表法将患者分为弓状韧带组和腹横平面组,每组29例。弓... 目的观察超声引导下外侧弓状韧带上前路腰方肌阻滞(QLB-LSAL)与腹横肌平面阻滞(TAPB)对腹腔镜肝脏部分切除术(LPH)后镇痛与恢复质量的影响。方法选择择期行LPH患者58例,采用随机数字表法将患者分为弓状韧带组和腹横平面组,每组29例。弓状韧带组在术前进行双侧QLB-LSAL,腹横平面组在术前进行双侧肋缘下TAPB,2组均每侧给予0.33%的罗哌卡因20 mL,术后均使用自控静脉镇痛(PCIA)。记录2组患者术后2、4、6、12、24和48 h静息和运动数字评价量表(NRS)评分,术前1 d、术后1和3 d进行15项恢复质量量表(QoR-15)评分,围手术期麻醉药物用量,PCIA按压次数,48 h补救镇痛药使用例数,术后恶心呕吐(PONV)发生率及术后首次下床时间。结果与腹横平面组相比,弓状韧带组术后2、4、6、12、24和48 h运动NRS评分,术后2、4、6、12和24 h静息NRS评分降低(P<0.05),术后1 d和3 d QoR-15评分增加(P<0.05),弓状韧带组患者围手术期瑞芬太尼和舒芬太尼用量、术后48 h使用补救镇痛药物比例、镇痛泵按压次数、PONV发生率及术后首次下床时间降低(P<0.05)。结论QLB-LSAL阻滞对LPH术后镇痛效果和恢复质量优于TABP阻滞。 展开更多

Influence of muscle activation on lumbar injury under a specific+Gz load

Purpose:The present study aimed to analyze the influence of muscle activation on lumbar injury under aspecific+Gz load.Methods:A hybrid finite element human body model with detailed lumbar anatomy and lumbar muscleactivation capabilities was developed.Using the specific+Gz loading acceleration as input,the kinematicand biomechanical responses of the occupant's lower back were studied for both activated and deactivated states of the lumbar muscles.Results:The results indicated that activating the major lumbar muscles enhanced the stability of theoccupant's torso,which delayed the contact between the occupant's head and the headrest.Lumbarmuscle activation led to higher strain and stress output in the lumbar spine under+Gz load,such as themaximum Von Mises stress of the vertebrae and intervertebral discs increased by 177.9%and 161.8%,respectively,and the damage response index increased by 84.5%.Conclusion:In both simulations,the occupant's risk of lumbar injury does not exceed 10%probability.Therefore,the activation of muscles could provide good protection for maintaining the lumbar spine andreduce the effect of acceleration in vehicle travel direction. 展开更多

腰肌劳损是一种由腰部肌肉、筋膜、韧带等软组织损伤导致的炎症性疾病,临床常表现为反复发作不同程度的腰骶部酸胀痛,可向臀部放射,活动受限,多在扭转、久坐久立及天气变化时加重,通常在短暂休息或改变体位后,疼痛得到一定程度的缓解。... 腰肌劳损是一种由腰部肌肉、筋膜、韧带等软组织损伤导致的炎症性疾病,临床常表现为反复发作不同程度的腰骶部酸胀痛,可向臀部放射,活动受限,多在扭转、久坐久立及天气变化时加重,通常在短暂休息或改变体位后,疼痛得到一定程度的缓解。本病的病理机制主要与肌组织及其附着点处的过度牵拉,导致局部反应性炎症、局部组织变性有关。近年来,中西医在本病的治疗方面均取得了显著进展,乙酰氨基酚、阿司匹林、塞来昔布等非甾体类抗炎药以及镇痛药效果较好,但存在不良反应,而中药内服、外敷、推拿、艾灸、康复锻炼等中医疗法效果显著,作用时间久,不容易复发,深受患者喜爱。腰肌劳损在军事训练伤中较为常见,管理者与临床医生也开展了系列研究。 展开更多

目的探讨腰椎手术在无肌松药气管插管后即刻监测运动诱发电位(TceMEP)基线的可行性。方法将2023年5-12月在徐州市中心医院拟行经椎间孔入路腰椎椎体间融合术(TLIF)的患者156例随机分为两组,最终对照组72例,男33例,女39例,观察组75例,男3... 目的探讨腰椎手术在无肌松药气管插管后即刻监测运动诱发电位(TceMEP)基线的可行性。方法将2023年5-12月在徐州市中心医院拟行经椎间孔入路腰椎椎体间融合术(TLIF)的患者156例随机分为两组,最终对照组72例,男33例,女39例,观察组75例,男37例,女38例,对照组在术中脊柱暴露完成后监测TceMEP基线,观察组在无肌松药气管插管后即刻监测TceMEP基线。对比两组患者气管插管前后血流动力学变化、插管满意度和操作时间,并比较两组TceMEP基线引出成功率、刺激阈值、敏感性和特异性。结果两组患者气管插管时血流动力学变化与插管满意度差异无统计学意义(P>0.05),插管时间对照组为(6.52±1.22)min,观察组为(9.44±0.84)min,两组差异有统计学意义(P<0.05)。观察组TceMEP基线引出成功率为100%,平均刺激阈值(225.00±22.13)V,对照组TceMEP基线引出成功率为84.72%,平均刺激阈值(342.01±31.07)V,两组差异有统计学意义(P<0.05)。对照组在上钉后监测TceMEP成功率为93.06%,观察组为100%,两组差异有统计学意义(P<0.05)。两组TceMEP的敏感度和特异度差异无统计学意义(P>0.05)。结论无肌松气管插管后即刻监测TceMEP基线成功率高,刺激阈值小,敏感性与特异性与术中脊柱暴露完成后监测TceMEP基线无差别。 展开更多

目的探讨椎旁肌脂肪浸润程度在单侧双通道内镜手术治疗腰椎退行性疾病中的影响。方法选取2020年1月~2022年6月该科室行单侧双通道内镜手术治疗的腰椎椎间盘退变性疾病患者89例。根据腰椎椎旁肌的脂肪浸润程度分为三组:A组22例(正常),0%~... 目的探讨椎旁肌脂肪浸润程度在单侧双通道内镜手术治疗腰椎退行性疾病中的影响。方法选取2020年1月~2022年6月该科室行单侧双通道内镜手术治疗的腰椎椎间盘退变性疾病患者89例。根据腰椎椎旁肌的脂肪浸润程度分为三组:A组22例(正常),0%~10%脂肪浸润;B组36例(轻微),10%~50%脂肪浸润;C组31例(严重),>50%脂肪浸润。所有患者术后随访至少3个月,比较围手术期指标、手术前后VAS评分和ODI指数、术后并发症及临床疗效等。结果所有患者均顺利完成单侧双通道内镜手术并完成随访。在手术时间、住院时间、并发症发生率等指标方面,三组之间无统计学差异(P>0.05)。术后1个月、3个月的腰腿痛VAS评分、腰椎ODI指数方面,三组均较术前显著改善(P<0.05);但C组术后3 d的腰痛VAS评分显著高于A、B组(P<0.05),C组术后1个月的腰痛VAS评分、腰椎ODI指数均显著高于A、B组(P<0.05);术后3个月腰痛VAS评分、腰椎ODI指数方面,三组之间无统计学差异(P>0.05)。术后3个月Fischgrund标准结果显示,优39例,良39例,可11例,优良率为87.6%,三组之间无统计学差异(P>0.05)。结论椎旁肌脂肪浸润程度严重的腰椎退变患者行单侧双通道内镜下减压手术,其腰痛症状及腰椎功能恢复相对较慢,但对总体疗效并无显著影响。 展开更多

【目的】比较腰部肌群稳定性康复训练及其结合肌内效贴扎技术在腰椎间盘突出症(LDH)康复治疗中的应用效果,以期为临床LDH康复治疗提供参考。【方法】纳入2018年1月到2020年1月宁波市鄞州区第二医院收治的99例LDH患者分为对照组(n=49)与... 【目的】比较腰部肌群稳定性康复训练及其结合肌内效贴扎技术在腰椎间盘突出症(LDH)康复治疗中的应用效果,以期为临床LDH康复治疗提供参考。【方法】纳入2018年1月到2020年1月宁波市鄞州区第二医院收治的99例LDH患者分为对照组(n=49)与观察组(n=50)。对照组采用单独肌内效贴扎技术,观察组采用肌内效贴扎技术配合腰部肌群稳定性康复训练,比较两组患者临床治疗效果、治疗前后腰椎功能及疼痛情况、腰背伸肌群表面肌电值、腰椎功能活动情况,以及皮肤过敏的发生情况。【结果】治疗4周后,观察组视觉模拟评分(VAS)、腰痛功能障碍评价表(ODI)评分明显低于对照组(P<0.05),日本矫形外科协会(JOA)评分、背伸肌群平均功率频率、积分肌电值及腰椎功能活动评分均明显高于对照组(P<0.05);治疗期间,两组并发症发生率比较差异无统计学意义(P>0.05);治疗4周后,观察组复发率明显低于对照组(P<0.05)。【结论】腰部肌群稳定性康复训练结合肌内效贴扎技术治疗LDH的临床疗效显著优于单纯肌内效贴扎,可有效改善患者肌肉疲劳程度,有助于减轻其腰椎部位疼痛并提高腰椎活动功能。 展开更多

目的采用磁共振脂肪定量(fat analysis&calculation technique,FACT)序列脂肪定量技术,分析2型糖尿病(type 2 diabetes mellitus,T2DM)患者腰椎椎体及椎旁肌肉的脂肪分数(fat fraction,FF),探讨T2DM患者腰椎椎体、椎旁肌肉内脂肪含... 目的采用磁共振脂肪定量(fat analysis&calculation technique,FACT)序列脂肪定量技术,分析2型糖尿病(type 2 diabetes mellitus,T2DM)患者腰椎椎体及椎旁肌肉的脂肪分数(fat fraction,FF),探讨T2DM患者腰椎椎体、椎旁肌肉内脂肪含量的变化及其相关性。方法选取31例T2DM患者和同期31例健康对照者(与T2DM患者年龄、性别、女性绝经情况、体质量指数进行配对),均行磁共振FACT序列扫描,分别测量腰3椎体及腰3/4椎间盘水平椎旁肌的FF值,利用独立样本t检验对糖尿病组及对照组的腰椎椎体及椎旁肌的脂肪含量进行统计分析,采用Spearman/Pearson相关系数分别对T2DM组、对照组患者腰椎椎体及椎旁肌FF进行相关性分析。结果T2DM组腰椎椎体及椎旁肌肉FF值均高于正常对照组,差异均有统计学意义(P<0.05);T2DM组腰椎椎体与椎旁肌FF值无相关性(P>0.05),对照组腰椎椎体与左侧竖脊肌FF值无相关性(P>0.05);对照组腰椎椎体与左侧多裂肌、右侧竖脊肌、右侧多裂肌FF值呈正相关(P<0.05)。结论磁共振FACT序列脂肪定量技术可以用于评价T2DM患者腰椎椎体及椎旁肌肉脂肪含量;T2DM患者腰椎及椎旁肌的脂肪含量较健康人群产生变化,脂肪沉积更明显;与健康人群不同,T2DM患者腰椎与椎旁肌的脂肪含量之间无相关性。 展开更多

苏红骨熏方治疗慢性腰肌劳损的临床疗效观察

目的探讨苏红骨熏方对慢性腰肌劳损的临床疗效。方法选取2022年7月—2023年9月医院收治的240例慢性腰肌劳损患者,根据随机对照法分为对照组和试验组,每组各120例,对照组口服扶他林,试验组在对照组的基础上采用苏红骨熏方通过中药熏蒸联... 目的探讨苏红骨熏方对慢性腰肌劳损的临床疗效。方法选取2022年7月—2023年9月医院收治的240例慢性腰肌劳损患者,根据随机对照法分为对照组和试验组,每组各120例,对照组口服扶他林,试验组在对照组的基础上采用苏红骨熏方通过中药熏蒸联合电脑中频治疗仪治疗。采用视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)评分和临床体征积分作为慢性腰肌劳损的评价系统,进行统计分析,比较中西医临床疗效,得到苏红骨熏方中药熏蒸联合电脑中频治疗仪治疗慢性腰肌劳损的临床疗效评价。结果治疗后,与对照组进行比较,试验组临床总有效率较高(P<0.05),与治疗前相比较,治疗后两组VAS评分及ODI评分均降低(P<0.05);与对照组相比,试验组VAS评分及ODI评分较低(P<0.05)。经治疗,与治疗前相比较,治疗后两组活动程度、压痛程度、压痛点个数等临床体征积分均降低(P<0.05);与对照组相比较,试验组临床体征积分较低(P<0.05)。与对照组相比较,试验组不良反应发生率较高,差异具有统计学意义(P<0.05)。结论应用中药熏蒸联合电脑中频治疗仪下苏红骨熏方对寒湿型慢性腰肌劳损患者能显著缓解临床症状,增强腰部功能,提高临床疗效,应用安全可靠。 展开更多

目的:探究单节段腰椎管狭窄症患者接受经皮内镜下经椎间孔腰椎椎体间融合术(PE-TLIF)的临床疗效和手术节段术后椎旁肌及相邻节段关节突关节的变化。方法:回顾性分析首都医科大学附属北京朝阳医院2017年3月~2019年3月收治的28例行PE-TLI... 目的:探究单节段腰椎管狭窄症患者接受经皮内镜下经椎间孔腰椎椎体间融合术(PE-TLIF)的临床疗效和手术节段术后椎旁肌及相邻节段关节突关节的变化。方法:回顾性分析首都医科大学附属北京朝阳医院2017年3月~2019年3月收治的28例行PE-TLIF治疗的L4/5单节段腰椎管狭窄症患者,其中男性12例,女性16例,年龄58.0±8.7岁(41~79岁),随访40.7±3.6个月(36~58个月)。收集术前和术后1周、3个月、12个月及末次随访时腰痛视觉模拟评分(VAS-LBP)、腿痛视觉模拟评分(VAS-LP);术前和术后3个月、12个月及末次随访时Oswestry功能障碍指数(ODI),并比较手术前后的差异。分别于术前和术后12个月、24个月及末次随访时进行CT检查,测量多裂肌(MF)横截面积(CSA)和脂肪浸润(FI)评分,并根据关节突关节退变评分标准评价手术相邻节段关节突关节退变程度,比较术后MF CSA、FI评分、相邻节段关节突关节退变评分与术前的差异。结果:患者术前VAS-LBP为7(7,8)分,术后1周为3(2,3)分,3个月为1(1,2)分,12个月为1(0,2)分,末次随访为1(0,1)分,术后较术前显著性改善(P<0.05);术前VAS-LP为6(5,7)分,术后1周为2(1,3)分,3个月为1(1,2)分,12个月为1(0,1)分,末次随访为0(0,1)分,术后较术前显著性改善(P<0.05);术前ODI为61%(55%,67%),术后3个月为23%(19%,31%),12个月为12%(8.5%,17.5%),末次随访为7%(4%,15.6%),术后较术前显著性改善(P<0.05)。术前MF CSA:557.06±46.72mm^(2),术后12个月547.12±53.31mm^(2)、术后24个月558.35±52.37mm^(2)、末次随访时531.21±56.12mm^(2),术后与术前无统计学差异(P>0.05)。术前FI评分:2(2,3),术后12个月3(2,3)、术后24个月3(2,3)、末次随访时3(3,3),术后与术前无统计学差异(P>0.05)。术前手术上节段关节突关节评分:5(4,6)分,术后12个月手术5(4,6)分、术后24个月5(4,5)分、末次随访时5(4,6)分,术后与术前比较无统计学差异(P>0.05);术前手术下节段关节突关节评分:5(4,6)分,与术后12个月5(4,5)分、术后24个月5(4,6)分、末次随访时5(4,7)分,术后与术前比较无统计学差异(P>0.05)。结论:PE-TLIF治疗单节段腰椎管狭窄症的临床疗效满意,且对手术节段多裂肌以及邻近节段关节突关节中长期不产生明显影响。 展开更多

骨骼肌疲劳监测是康复医疗和运动科学等领域热点课题。然而受传感器的尺寸和价格所限,目前的肌肉疲劳监测技术不适用于运动场景。该研究基于织物应变传感器的穿戴式智能带测量人体上臂肱二头肌的厚度变化,以期辅助常规肌电图(electromyo... 骨骼肌疲劳监测是康复医疗和运动科学等领域热点课题。然而受传感器的尺寸和价格所限,目前的肌肉疲劳监测技术不适用于运动场景。该研究基于织物应变传感器的穿戴式智能带测量人体上臂肱二头肌的厚度变化,以期辅助常规肌电图(electromyography,EMG)传感器监测肌肉疲劳。测试方案选用哑铃弯举动作,采用EMG过零率和EMG中位频率检验新提出的2个基于厚度的肌肉疲劳指标,即电阻中位频率和电阻所围面积。结果显示,EMG中位频率和电阻中位频率相关系数达0.7815,P值为0.0220;EMG过零率和电阻所围面积相关系数达0.8747,P值为0.0045。结果表明,基于厚度的肌肉疲劳指标与常用的基于EMG的疲劳指标显著相关。该研究验证了使用面向可穿戴的柔性传感器在运动中监测肌肉疲劳的可行性,显示出该类传感器在肌肉疲劳监测研究领域的广阔前景。 展开更多

运动疗法通过机械-化学偶联治疗慢性非特异性下背痛

背景:目前运动疗法是非药物治疗腰痛的有效方法,运动疗法可通过骨骼和肌肉之间的机械-化学偶联维持腰椎的稳定,但目前尚无关于运动疗法通过机械-化学偶联缓解慢性非特异性下背痛之间研究进展及最佳治疗方案的明确阐述。目的:综述运动疗... 背景:目前运动疗法是非药物治疗腰痛的有效方法,运动疗法可通过骨骼和肌肉之间的机械-化学偶联维持腰椎的稳定,但目前尚无关于运动疗法通过机械-化学偶联缓解慢性非特异性下背痛之间研究进展及最佳治疗方案的明确阐述。目的:综述运动疗法时椎旁肌通过机械-化学偶联影响腰椎稳定性进而缓解慢性非特异性下背痛的相关研究进展,以及目前运动疗法治疗慢性非特异性下背痛的最佳方案。方法:在万方数据库、中国知网、维普、Web of Science和PubMed数据库进行文献检索,以“慢性非特异性下背痛,腰椎稳定,椎旁肌,运动疗法”为中文检索词,以“chronic nonspecific low back pain,lumbar stabilization,paravertebral muscle,exercise therapy”为英文检索词,检索各数据库建库至2024年1月发表的相关文献,最终纳入93篇文献进行归纳总结。结果与结论:运动疗法可以通过适当的机械刺激作用于椎旁肌和骨骼并使其产生相应的变化。运动疗法主要通过机械-化学偶联方式来提高椎旁肌的质量,进而维持腰椎稳定,从而更好地缓解慢性非特异性下背痛,是慢性非特异性下背痛的重要干预措施。但是,对于运动疗法通过腰椎稳定来治疗慢性非特异性下背痛的确切有效方案尚无明确报道。个体化运动方案的制定对于慢性非特异性下背痛的治疗和预后尤为重要。同一个体的肌肉质量与骨骼质量是密切相关的,影像学评估椎旁肌的质量和体积对于疾病的发现和干预具有重要意义。 展开更多
